    From the database of the Danish Breast Cancer Cooperative Group, we identified 531 women diagnosed with early-stage breast cancer from 1977 to 2005, who developed subsequent ischemic heart disease (cases) and matched them to 1069 controls without heart disease after radiotherapy. Data were available for precise dose estimation for 196 cases and 413 controls receiving tangential photon techniques.

    The median of the mean heart doses for the women receiving tangential radiotherapy was 2.41Gy for left- and 0.68Gy for right-sided radiotherapy. The mean heart dose was higher for cases than controls (0.84Gy and 0.71Gy, respectively; p<0.001). In this group, the linear increase in the excess odds ratio of major coronary events per gray of mean heart dose (K) was 19 percent (95% Confidence Interval(CI) 1% to 63%, p=0.02).     Bioelectronic retinal prostheses that stimulate the remaining inner retinal neurons, bypassing degenerated photoreceptors, have been demonstrated to restore some vision in patients blinded by retinitis pigmentosa (RP). These implants encode luminance of the visual scene into electrical stimulation, however, leaving out chromatic information. Yet color plays an important role in visual processing when it comes to recognizing objects and orienting to the environment, especially at low spatial resolution as generated by current retinal prostheses. In this study, we tested the feasibility of partially restoring color perception in blind RP patients, with the aim to provide chromatic information as an extra visual cue.

  • Miang, a traditional fermented tea leaf (Camellia sinensis var. assamica) consumed in northern Thailand, was simulated in laboratory conditions using non-filamentous fungi process (NFP) and microbial community was periodically investigated for over 6 months of fermentation by both culture-dependent and -independent techniques. The viable cell numbers of lactic acid bacteria (LAB), yeast, and Bacillus enumerated by the culture-dependent technique markedly surged over 3 days of initial fermentation and then smoothly declined by the end of fermentation. LAB were found as the main microbial population throughout the fermentation period followed by yeast and Bacillus. High-throughput sequencing of microbial community during fermentation revealed that Firmicutes (86.9-96.0%) and Proteobacteria (4.0-12.4%) were the dominant bacterial phyla, whereas Ascomycota was found to be the main fungal phylum with an abundance of over 99% in the fungal community. The dominant bacterial family was Lactobacillaceae (39.7-79.5%) fivity. Lactic acid and butyric acid reached maximum of 50.9 and 48.9 mg/g dry weight (dw) at 9 and 63 days of fermentation, respectively. This study provided essential information for deeper understanding of the Miang fermentation process based on the chemical and biological changes during production.Human cytomegalovirus (HCMV), a ubiquitous beta-herpesvirus, is able to establish lifelong latency after initial infection. Virion-carried mediators are considered to play pivotal roles in viral adaptation into a new cellular environment upon entry. Additionally, one clear difference between primary infection and reactivation is the idea that virion-packaged factors are already formed such that those molecules can be used swiftly by the virus. In contrast, virion-carried mediators have to be transcribed and translated; thus, they are not readily available during reactivation. Hence, understanding virion-carried molecules helps to elucidate HCMV reactivation. In this article, the impact of virion-packaged molecules on viral structure, biological behavior, and viral life cycle will be reviewed.The attachment of foodborne pathogens to leaf surfaces is a complex process that involves multiple physical, chemical, and biological factors. Here, we report the results from a study designed to specifically determine the contribution of spinach leaf surface topography as it relates to leaf axis (abaxial and adaxial) and leaf age (15, 45, and 75 days old) to the ability of Escherichia coli to resist removal by surface wash, to avoid inactivation by chlorine, and to disperse through splash impact. We used fresh spinach leaves, as well as so-called "replicasts" of spinach leaf surfaces in the elastomer polydimethylsiloxane to show that leaf vein density correlated positively with the failure to recover E. coli from surfaces, not only using a simple water wash and rinse, but also a more stringent wash protocol involving a detergent. Such failure was more pronounced when E. coli was surface-incubated at 24°C compared to 4°C, and in the presence, rather than absence, of nutrients. Leaf venation also contributed to the ability of E. coli to survive a 50 ppm available chlorine wash and to laterally disperse by splash impact. Our findings suggest that the topographical properties of the leafy green surface, which vary by leaf age and axis, may need to be taken into consideration when developing prevention or intervention strategies to enhance the microbial safety of leafy greens.Bacterial biofilms do serious harm to the diabetic foot ulcer (DFU) because they play a crucial role in infection invasion and spread. Molecular analysis demonstrated that the IPDC pathway involved in the conversion of tryptophan (Trp) to indole-3-acetic acid (IAA) is highly conserved among P. agglomerans strains at both gene and protein levels. Results also indicated that the promoter region controlling the inducible expression of ipdC gene differs from the model system Enterobacter cloacae, which is in accordance with the observation that P. agglomerans accumulates higher levels of IAA when cells are collected in the exponential phase of growth. To assess the potential applications of these microorganisms for IAA production, P. agglomerans C1, an efficient auxin-producer strain, was cultivated in 5 L fermenter so as to evaluate the effect of the medium formulation, the physiological state of the cells, and the induction timing on the volumetric productivity. Results demonstrated that higher IAA levels were obtained by using a saline medium amended with yeast extract and saccharose and by providing Trp, which acts both as a precursor and an inducer, to a culture in the exponential phase of growth.

  • ObjectiveTo explore the feasibility and effectiveness of medialization thyroplasty for the treatment of unilateral vocal fold immobility (UVFI). MethodEight UVFI patients were performed medialization thyroplasty under local anesthesia. We made a window in the thyroid cartilage under local anesthesia, then insert the preformed silicone implant. The paralyzed vocal fold was medialized to make the glottis closed. Fibrolaryngoscope video recording, subjective voice analysis and CT thin slice scan of larynx were done before and after surgery to evaluate closure of vocal cords, improvement of voice and position of implantation. ResultThe preoperative and postoperative voice handicap index 30(VHI-30) of the 8 patients were 91.5(64.5, 97.5) and 22.5(5.0, 47.5) respectively, which showed statistical difference(P less then 0.05). GRBAS results The postoperative G, R, B, A were all smaller than preoperative ones, which showed statistical difference(P less then 0.05); the pre and postoperative S was both 0. The fibrolaryhgoscope recording showed the preoperative and postoperative score of incomplete glottis closure was 1.0(1.0, 1.0) and 4.0(2.5, 5.0) respectively, which showed statistical difference(P less then 0.05). Postoperative laryngeal CT showed significant vocal cord medialization on the affected side. Aspiration was significantly improved in 4 patients who were suffered from this symptom before the surgery. No complication occurred with the 8 patients during 5 to 48 months follow up. Eustachian tube pressure measurement(TMM) was performed preoperatively and postoperatively, and the Eustachian tube score(ETS-5) was recorded before and 6 and 12 months after surgery and statistical analysis was performed. ResultThere was a statistically significant difference between the preoperative ETS-5 scores at 6, 12, and 18 months after surgery. There was no significant difference between the 12 months and 18 months after surgery. There was no significant difference in the recurrence rate of OME in patients with tympanic membrane indwelling time of 12 months and 18 months, but the rate of perforation was 18 months. Statistical software was used to evaluate the recovery of Eustachian tube function and the prognosis of OME. ResultThe average pure tone audiometry(PTA) of 29 patients was 17.5, 22.5, 18.5, 22.5, 18.5, 22.5, 22.5 dB at 1 month, 6 months, 9 months, 12 months, 18 months, 24 months and 36 months postoperatively. In oVEMP, there was no significant difference in N1-P1 amplitude between control group and OME preoperative group, N1 latency, P1 latercy and N1-P1 latency showed no significant difference as well.
    ObjectiveTo explore the feasibility and effectiveness of medialization thyroplasty for the treatment of unilateral vocal fold immobility (UVFI). MethodEight UVFI patients were performed medialization thyroplasty under local anesthesia. We made a window in the thyroid cartilage under local anesthesia, then insert the preformed silicone implant. The paralyzed vocal fold was medialized to make the glottis closed. Fibrolaryngoscope video recording, subjective voice analysis and CT thin slice scan of larynx were done before and after surgery to evaluate closure of vocal cords, improvement of voice and position of implantation. ResultThe preoperative and postoperative voice handicap index 30(VHI-30) of the 8 patients were 91.5(64.5, 97.5) and 22.5(5.0, 47.5) respectively, which showed statistical difference(P less then 0.05). GRBAS results The postoperative G, R, B, A were all smaller than preoperative ones, which showed statistical difference(P less then 0.05); the pre and postoperative S was both 0. The fibrolaryhgoscope recording showed the preoperative and postoperative score of incomplete glottis closure was 1.0(1.0, 1.0) and 4.0(2.5, 5.0) respectively, which showed statistical difference(P less then 0.05). Postoperative laryngeal CT showed significant vocal cord medialization on the affected side. Aspiration was significantly improved in 4 patients who were suffered from this symptom before the surgery. No complication occurred with the 8 patients during 5 to 48 months follow up. All of the electrodes were fabricated by growing the Ni1-xCuxCo2O4 electrode spinel-structural active materials onto the graphite felt (GF). Five types of electrodes, NiCo2O4/GF, Ni0.875Cu0.125Co2O4/GF, Ni0.75Cu0.25Co2O4/GF, Ni0.625Cu0.375Co2O4/GF, and Ni0.5Cu0.5Co2O4/GF, were prepared for application to the oxygen evolution reaction (OER). As Cu2+ ions were substituted, the electrochemical performances of the NiCo2O4-based structures were improved, and eventually the OER activities were also greatly increased. The experimental results indicate that the developed "Expand-Extract-Combine" strategy can not only overcome problems of background noise and spectral overlapping but also mine the deeper spectrum information by integrating important features. Moreover, the SPSE-XGBoost strategy utilizes the selected feature subset instead of the full-spectrum for calculation, which effectively improves the computing speed. The comparisons of different data processing methods are conducted. It outcomes that the proposed strategy outperforms other routine methods and can profoundly determine the concentrations of zinc, copper, cobalt, and nickel with the lowest RMSEP. Specifically, we concentrate on the two most studied families of the Ras superfamily the Ras and Rho families. We summarize the latest findings of small GTPases of the Ras and Rho families in neurodegeneration in order to highlight these small proteins as potential therapeutic targets capable of slowing down different neurodegenerative diseases.

The results indicated that improvements in environmental efficiency significantly ameliorated PM2.5 pollution. The effect of technological progress on PM2.5 reduction is limited, but the pure efficiency and scale efficiency promoted by enhanced management level and optimized production scale strongly contribute to PM2.5 mitigation. The significant spatial spillover of environmental efficiency and PM2.5 pollution introduces challenges and opportunities for lowering PM2.5 emissions. The impact of environmental efficiency on PM2.5 pollution exhibits significant spatiotemporal heterogeneity, and the strength of influence tends to increase with PM2.5 concentration and become more pronounced over time. Its main idea is to train the model under the supervision of feature maps from the ground truth. Four key technical contributions are made in the paper. First, pairs of hazy and clean images are used as inputs to supervise the encoding process and obtain high-quality feature maps. Second, the basic GAN formulation is modified by introducing perception loss, style loss, and feature regularization loss to generate better results. Third, multi-scale images are applied as the input to enhance the performance of discriminator. Finally, a hazy remote sensing dataset is created for testing our dehazing method and environmental detection. Here, we employed 18S rDNA amplicon sequencing to study the assembly processes governing the microeukaryotic communities and their habitat generalists and specialists across three hydrological seasons in a subtropical river in China. The results showed that deterministic and stochastic processes jointly shaped the microeukaryotic communities, where the relative importance of stochastic processes decreased in the following order wet > normal > dry seasons. First, pairs of hazy and clean images are used as inputs to supervise the encoding process and obtain high-quality feature maps. Second, the basic GAN formulation is modified by introducing perception loss, style loss, and feature regularization loss to generate better results. Third, multi-scale images are applied as the input to enhance the performance of discriminator. Finally, a hazy remote sensing dataset is created for testing our dehazing method and environmental detection. NASH worsens chronic diabetes complications, and T2DM aggravate the NASH progression to fibrosis, cirrhosis, and even hepatocellular carcinoma (HCC). The objectives in NAFLD and NASH therapy are to reduce disease activity, to slow down progression of fibrosis, and to lower the risk factors. Unfortunately, there are no specific validated pharmacological therapies. Of 65 publications identified, nine trials on subjects with overweight/obesity, type 2 diabetes mellitus, or cardiovascular disease were included. Five studies reported that the plant-based intervention significantly improved markers of glycemic control from baseline to end point, of which four revealed a significant improvement in the intervention group compared to the control intervention. The remaining four studies did not observe a significant effect of a plant-based intervention on outcomes related to glucose metabolism. Our findings suggest that a shift to a plant-based diet may lead to favorable effects on glycemic control in individuals diagnosed with type 2 diabetes mellitus and/or obesity. The data were however somewhat conflicting, and the included trials reported results based on different intervention diets and study populations. The predominant patterns of lung lesions were pure ground-glass opacity (GGO), GGO with consolidation or reticulation, and pure consolidation in both groups. Pure consolidation on chest CT was more common at presentation in pregnant cases. Pregnant women progressed with a higher consolidation frequency compared with non-pregnant group in stage 2 (95% vs 82%). Improvement was identified in stages 3 and 4 for both groups, but consolidation was still more frequent for pregnant women in stage 4. Most patients (38/40, 95%) were grouped as mild or common type. The length of hospitalization between the two groups was similar. The purpose of this study was to analyze the activity of cryptanshinone, a traditional Chinese medicine monomer, in combination with fosfomycin against fosfomycin-sensitive (FSSA) and fosfomycin-resistant (FRSA). The MICs of fosfomycin and/or cryptanshinone were determined by agar dilution assay and checkerboard microdilution assay. Furthermore, synergistic effects from fosfomycin and/or cryptanshinone were analyzed by the time-kill assay in vitro. The combination of fosfomycin and cryptotanshinone had a synergistic effect on most (71.43%) of the FRSA and had a partial (28.57%) synergistic effect on a small part. In addition, time sterilization curve verified synergistic activity between cryptanshinone and fosfomycin against FSSA and FRSA, especially when fosfomycin was added for a second time.

  • The second epicenter of the global COVID-19 epidemic following Wuhan, and the first in the Western world, occurred unexpectedly in the Lombardy region of Italy, whose capital city is Milan. The aggressive nature of the outbreak in the region was dramatic, leading to a 2-month period of lockdown. Within the Policlinico, the historic hospital in the center of Milan, many units were rapidly converted into intensive care units or semi-intensive units for adult patients. During lockdown, the pediatric inpatient units had to face daily reorganization caused by the necessary logistic and structural transformations, thus restricting routine care pathways for chronic patients, while the Pediatric Emergency Unit had to develop a system able to effectively separate the children and caregivers infected with COVID-19 from those who were not affected. These 2 months enhanced resilience among both doctors and nurses, and facilitated the transversal transmission of data aimed at helping colleagues and patients in any way possible, in spite of the restrictive measures limiting the rate of activity in pediatric care.     Two pharmaceutical agents have been approved for treatment of idiopathic pulmonary fibrosis (IPF) pirfenidone and nintedanib.

    We investigated the need of dose reduction in consecutive patients treated with nintedanib in relation to gender and body max index, comparing the population over and under 80 years of age.

    We retrospectively reviewed the data of all consecutive IPF patients treated with nintedanib for at least 3 months. Data on age, gender, body max index, side effects, and duration of therapy after enrolment were recorded.

    A total of 82 patients has been evaluated. All dose reductions were related to side effects and/or toxicities. The need for a dose reduction was significantly more frequent in patients aged 80 years or older (50 vs. 26.8%, p = 0.039), independently from their body mass index. A total of 52% of males >80 years and only 16% of males <80 years reduced the dose (p = 0.002).

A total of 5,263 BP patients and 21,052 age- and gender-, hospital visit number-matched controls were identified in the National Health Insurance Research Database of Taiwan (1997-2013). Demographic characteristics and comorbidities including IBD were compared. Logistic regression was conducted to examine the predicting factors for BP. The mean age at diagnosis was 74.88 years and 54.3% of subjects were male. BP patients tended to have more cardiovascular risk factors, autoimmune and neurologic comorbidities, and hematologic cancers than matched controls. There were 20 cases of IBD (0.38%), mostly ulcerative colitis (N = 17, 0.32%) among BP patients, compared to 33 IBD cases (0.16%) among controls (p  less then  0.001). Ulcerative colitis was found to be significantly associated with BP [adjusted odds ratio (OR) 3.60, 95% confidence interval (CI) 1.91-6.77, p  less then  0.001] on multivariate analysis. Treatment for IBD was not associated with BP development. Information about diet, lifestyle, alcohol consumption, and smoking habit was not available.     This prospective interventional study included 110 CVC insertions, both umbilical venous catheterization (UVC) and peripherally inserted central catheterization (PICC). Catheter position was monitored by real-time TNE.

    Overall visibility of catheter tip (combined UVC and PICC) was significantly better in AS (n = 55) compared with NS group (n = 55) [48/55 (87.2%) vs. 28/55 (50.9%); p < 0.0001]. Time to detect catheter tip by AS push was significantly less than that of NS push. There was no difference in the amount of saline flush required with either method. No major adverse effect was observed.

WMH severity in CD patients positively correlated with age (r = 0.532, P = 0.000), disease course (r = 0.257, P = 0.009), postprandial glucose (r = 0.278, P = 0.005), frequency of left ventricular hypertrophy (r = 0.398, P = 0.001) and hypothyroidism (r = 0.246, P = 0.014).   • Aim of the study is to evaluate the incidence of DVT in COVID-19 patients and its correlation with the severity of the disease and with clinical and laboratory findings.

    234 symptomatic patients with COVID-19, diagnosed according to the World Health Organization guidelines, were included in the study. The severity of the disease was classified as moderate, severe and critical. Doppler ultrasound (DUS) was performed in all patients. DUS findings, clinical, laboratory's and therapeutic variables were investigated by contingency tables, Pearson chi square test and by Student t test and Fisher's exact test. ROC curve analysis was applied to study significant continuous variables.

    Overall incidence of DVT was 10.7% (25/234) 1.6% (1/60) among moderate cases, 13.8% (24/174) in severely and critically ill patients. Prolonged bedrest and intensive care unit admission were significantly associated with the presence of DVT (19.7%). Fraction of inspired oxygen, P/F ratio, respiratory rate, heparin administration, D-dimer, IL-6, ferritin and CRP showed correlation with DVT.

    DUS may be considered a useful and valid tool for early identification of DVT. In less severely affected patients, DUS as screening of DVT might be unnecessary. High rate of DVT found in severe patients and its correlation with respiratory parameters and some significant laboratory findings suggests that these can be used as a screening tool for patients who should be getting DUS.

COMPASS employs inducible artificial transcription factors derived from plant (Arabidopsis thaliana) regulators to control the expression of pathway genes in yeast (Saccharomyces cerevisiae). It utilizes homologous recombination for parts assembly and employs a positive selection scheme to identify correctly assembled pathway variants after both in vivo and in vitro recombination. Finally, COMPASS is equipped with a CRISPR/Cas9 genome modification system allowing for the one-step multilocus integration of genes. Although COMPASS was initially developed for pathway engineering, it can equally be employed for balancing gene expression in other synthetic biology projects.Modular cloning standards based on Golden Gate DNA assembly allow for construction of complex DNA constructs over several rounds of assembly.
