Lidocaine Patches: A Non-Invasive Treatment for Neuropathic Pain

Introduction
Neuropathic pain is a chronic pain condition that affects millions of people worldwide. It occurs when the nervous system is damaged or dysfunctional, often as a result of conditions like shingles, diabetes, spinal cord injuries, or other health issues. Neuropathic pain can be described as burning, shooting, stabbing or electric-shock sensations. It is often very difficult to treat as conventional pain medications are not always effective. In recent years, lidocaine patches have emerged as a safe and effective non-invasive treatment option for neuropathic pain relief.

What are Lidocaine Patches?
Lidocaine  patches are medicated adhesive patches which contain the anesthetic lidocaine. When applied to the skin, lidocaine is absorbed transdermally through the skin layers into the affected area. Lidocaine works by blocking sodium channels in nerve membranes, preventing the generation and conduction of pain signals to the brain. Commercially available lidocaine patches include Lidoderm, Versatis, and Ztlido. These patches continuously deliver lidocaine through the skin for up to 12 hours, providing localized pain relief.

How do Lidocaine Patches Work?
Lidocaine patches work differently than oral pain medications. When applied, lidocaine enters the skin and is carried by blood vessels to nerves beneath the skin surface. It selectively blocks sodium channels in pain sending C-fibers and A-delta pain fibers, inhibiting the conduction of pain signals. This interrupting of pain transmission provides relief within 30-60 minutes of application. The patches deliver a low concentration of lidocaine continuously over several hours, giving prolonged neuropathic pain relief without peaks and troughs seen with oral medications.

Efficacy of Lidocaine Patches
Numerous clinical trials have demonstrated the efficacy of lidocaine patches in reducing various types of neuropathic pain, including postherpetic neuralgia (PHN) pain after shingles and painful diabetic neuropathy. In one study, over 60% of patients reported pain relief of at least 30% with lidocaine patch treatment compared to only 30% receiving placebo patches. Other trials reported significant reductions in pain interference with sleep, general activity, walking ability and mood compared to placebo. Lidocaine patches have also shown effectiveness as an adjunctive treatment added on to first line medications like gabapentin or pregabalin for improved pain relief.

Applications of Lidocaine Patches
While originally developed and approved for postherpetic neuralgia pain, lidocaine patches have found use in managing other forms of neuropathic pain as well. Some common applications of lidocaine patch therapy include:

Postherpetic Neuralgia: Chronic nerve pain caused by shingles/herpes zoster infections. Lidocaine patches have shown good relief of PHN pain.

Diabetic Neuropathy: Painful nerve damage due to diabetes. Patch application over feet provides foot pain relief.

Back/Nerve Pain: Neuropathic pain after surgery, injuries or chronic back/nerve conditions. Patches placed on affected areas ease pain.

Cancer Pain: Neuropathic components of cancer-related pains like post-mastectomy pain have responded to lidocaine patches.

HIV/AIDS Neuropathy: HIV infection or drug toxicities can cause peripheral neuropathy helped with lidocaine patches.

When used appropriately under medical guidance, lidocaine patches safely improve quality of life for millions affected by nerve pain worldwide.
