Although IVF can seem overwhelming at first, understanding the step-by-step process can help you feel more informed and prepared for what’s ahead.

Step 1: Ovulation Stimulation

The first step of IVF is ovulation stimulation. In this phase, the woman is given hormone injections to stimulate her ovaries to produce multiple eggs, rather than the single egg that typically matures during a regular menstrual cycle. This is done because IVF requires several eggs to increase the chances of successful fertilization.

The medication typically used during this stage includes injectable hormones like FSH (follicle-stimulating hormone) and LH (luteinizing hormone), which help mature several eggs at once. Throughout this process, the patient will have frequent visits to the fertility clinic for blood tests and ultrasound scans. These tests monitor hormone levels and track the development of the eggs to ensure they are growing properly.

Step 2: Monitoring and Triggering Ovulation

After about 10-14 days of stimulation, once the eggs are mature, the fertility doctor will trigger ovulation by administering a shot of hCG (human chorionic gonadotropin), which mimics the body’s natural ovulation process. This shot causes the eggs to be released from the ovaries in preparation for egg retrieval.

Timing is critical at this stage. The egg retrieval must happen at the right moment—typically 34-36 hours after the hCG shot—to ensure the eggs are mature and ready for fertilization.

Step 3: Egg Retrieval (Aspiration)

Egg retrieval, also known as aspiration, is a minor surgical procedure performed under sedation. During this step, a needle is inserted through the vaginal wall and into the ovaries to collect the eggs. This is done with the help of an ultrasound to guide the needle and ensure the eggs are properly retrieved.

The procedure typically lasts about 20-30 minutes, and patients are usually able to go home the same day after a short recovery period. Some women may experience mild cramping, bloating, or spotting afterward, but these side effects generally resolve quickly.

Step 4: Sperm Collection

While the eggs are being retrieved, the male partner or sperm donor will provide a sperm sample. The sperm is then processed in the lab to separate the healthiest and most active sperm from the semen. In cases of male infertility, where sperm quality is low, the ICSI procedure (Intracytoplasmic Sperm Injection) may be used, where a single sperm is injected directly into an egg to increase the chances of successful fertilization.

Once the sperm is prepared, it is ready to be combined with the eggs in the next step.

Step 5: Fertilization

In the fertilization step, the sperm is introduced to the eggs in the laboratory. If ICSI is not being used, sperm and eggs are placed together in a dish, and fertilization occurs naturally. If ICSI is part of the plan, a sperm is injected into each mature egg to ensure fertilization.

After fertilization, the eggs are monitored for a few days to check for successful development. The embryos are observed for quality, and the embryologist will assess how many have developed into healthy embryos. This step typically takes about 2-5 days, depending on the fertility clinic’s protocol.

Step 6: Embryo Culture

Once fertilization has occurred, the embryos are cultured in an incubator. Over the next several days, the embryos continue to develop. They are typically grown for 3 to 5 days before being considered for transfer into the woman’s uterus.

During this phase, the embryologist will monitor the embryos closely, selecting the best-quality embryos for transfer. This is when the decision is made whether to freeze any remaining healthy embryos for potential future use in case the current IVF cycle doesn’t result in a pregnancy.

Step 7: Embryo Transfer

The next step is the embryo transfer. This is a simple, non-surgical procedure where one or more embryos are transferred into the woman’s uterus. The process is similar to a Pap smear and typically doesn’t require anesthesia.

Using a thin, flexible tube (catheter), the embryos are gently placed into the uterus. The fertility doctor will guide the catheter using ultrasound to ensure that the embryos are positioned in the right location. After the transfer, the woman may be advised to rest for a short period before going home.

The number of embryos transferred is typically based on the woman’s age, the quality of the embryos, and her overall health. In some cases, more than one embryo is transferred to increase the chances of success.

Step 8: Pregnancy Test

About 10-14 days after the embryo transfer, a blood test will be performed to check for pregnancy. This test measures the level of the pregnancy hormone, hCG, in the blood. If the test is positive, it indicates that the embryo has implanted and is developing into a pregnancy.

In some cases, if the first test is positive, an ultrasound will be done around 6 weeks to confirm the pregnancy and check for a heartbeat. If the test is negative, the fertility clinic will discuss the next steps, which may include another round of IVF or other fertility treatments.
