However, large numbers of DNA strands are expensive and would hinder applications, such as pharmaceutical investigations, because of the complicated components. Therefore, one challenge is how to reduce the number of staple strands for construction of a DNA origami. For a DNA origami structure, the scale-free folding pattern of the scaffold strand is determined by staple strands at the branching vertexes. Simple duplex regions help defining the size-related features of the origami geometry. In this study, we hypothesized that a scaffold strand can be correctly folded into a designed topology by using only staple strands involved in branching vertexes. After assembly, any remaining, flexible, single-stranded regions of the scaffold can be converted into rigid duplexes by DNA polymerase to achieve the designed geometric structures. To demonstrate the concept, we have used only18 staple strands (covering 15% of the scaffold strand) to assemble a porous DNA nanostrucure, which was visualized by atomic force microscopy (AFM). In addition to direct effects of climate on species, there can also be indirect effects through an intermediary species, such as in host-plant interactions. Indirect effects are expected to be more pronounced in alpine environments because these ecosystems are sensitive to temperature changes and there are limited areas for migration of both species (i.e. closed systems), and because of simpler trophic interactions. We tested the hypothesis that climate change will reduce the range of an alpine butterfly (Parnassius smintheus) because of indirect effects through its host plant (Sedum sp.). To test for direct and indirect effects, we used the simulations of climate change to assess the distribution of P. smintheus with and without Sedum sp. We also compared the projected ranges of P. smintheus to four other butterfly species that are found in the alpine, but that are generalists feeding on many plant genera. We found that P. smintheus gained distributional area in climate-only models, but these gains were significantly reduced with the inclusion of Sedum sp. and in dry-climate scenarios which resulted in a reduction in net area. When compared to the more generalist butterfly species, P. smintheus exhibited the largest loss in suitable habitat. Our findings support the importance of including indirect effects in modelling species distributions in response to climate change. We highlight the potentially large and still neglected impacts climate change can have on the trophic structure of communities, which can lead to significant losses of biodiversity. In the future, communities will continue to favour species that are generalists as climate change induces asynchronies in the migration of species. © 2020 John Wiley & Sons Ltd.Single-atom catalysts (SACs) have great potential in electrocatalysis. Their performance can be rationally optimized by tailoring the center metal atoms, adjacent coordinative dopants, and metal loading. However, it is still of great challenge due to the limited synthesis approach and insufficient understanding of the structure-property relation. Herein, we reported a new kind of Mo SAC with a unique O, S-dual coordination and a high metal loading over 10 wt%. The isolation feature and local environment was identified by high-angle annular dark-field scanning transmission electron microscopy and extended X-ray absorption fine structure. The obtained SACs can catalyze oxygen reduction reaction via 2e- pathway with a high H2O2 selectivity above 95% in 0.10 M KOH. The critical role of Mo single atoms and the coordination structure was revealed by both electrochemical tests and theoretical calculations. This work enriches the family of SACs and highlights the importance of local coordination, thus rendering new opportunities to tune the activity and selectivity in multi-electron electrocatalysis.

  • Karanja (Pongamia pinnata) is a medicinal tree used in the Indian traditional ayurvedic system for treating several ailments. The seeds contain a unique furano-flavonoid karanjin, which has shown to possess many medicinal properties. Its usage at the clinical level is affected due to poor solubility and absorption. In the present investigation, molecular modifications of karanjin were attempted and evaluated their effect on anti-inflammatory activity. Firstly, Karanja ketone was obtained from karanjin by hydrolysis, and it was converted into karanja ketone oxime. The oxime undergoes Beckmann rearrangement and cyclized to yield furano benzoxazole (karanja oxazole). https://www.selleckchem.com/products/baricitinib-ly3009104.html The new derivatives were purified with >95% purity (HPLC) and spectrally characterized (HR-MS, FTIR, and NMR). Among the test compounds, karanja ketone oxime exhibited higher antioxidant activity with an IC50 value of 360 µg/ml (DPPH). Soy lipoxygenase-1 (LOX-1) inhibitory activity of oxime was higher (IC50 = 65.4 µM) than other compounds. Fluorescence studies showed that oxime had higher quenching capacity with a Qmax of 76.3% and a binding constant of 0.9 × 105 M-1 for soy LOX-1. In-silico interaction studies showed that karanja ketone oxime had the least binding energy of -5.76 kcal/mol with LOX-1 by forming two hydrogen bonds with hydrophobic amino acids Leu 390 and Gly 392. The compounds were evaluated for their acute anti-inflammatory activity by the paw and ear edema in the rat model. Karanjin inhibits paw edema and ear edema by 34.13% and 51.13%, respectively, whereas the derivatives inhibited by 45-57 % and 70-76.8%. This study reports a rational approach to synthesize karanjin derivatives with considerable anti-inflammatory properties, both in-vitro and in-vivo.Knowledge of preoperative right heart function of adult patients with cystic fibrosis (CF) awaiting lung transplant (LUTX) is limited. The echocardiography of adult patients with CF enlisted for LUTX was retrospectively analyzed and compared with standards and invasive analyses (right heart catheterization, multigated radionuclide ventriculography). We included 49 patients (reported as mean ± standard deviation; 29 ± 9 years of age; forced expiratory volume in first second of expiration, 31% ± 11% predicted; lung allocation score, 36 ± 5; invasive mean pulmonary artery pressure, 17 ± 5 mm Hg; multigated radionuclide ventriculography right ventricle [RV] ejection fraction, 50% ± 9%). Patients had increased RV end-diastolic area, RV wall thickness, and increased pulmonary artery acceleration time with subnormal tricuspid annular plane systolic excursion, tissue Doppler positive peak systolic velocity, and fraction area change. Subnormal tricuspid annular plane systolic excursion ( less then 23 mm), tissue Doppler positive peak systolic velocity ( less then 14 cm/s), and fraction area change ( less then 49%) had high sensitivity and negative predictive value in predicting impaired RV. EJECTION FRACTION A good correlation between echocardiographic estimated and invasively measured systolic pulmonary artery pressure was observed (R2 = 0.554, P less then .001). Adults with CF awaiting LUTX have morphologic alterations of the right heart, with subclinical impairment of RV systolic function. Echocardiography may be used as a bedside, repeatable, and reliable noninvasive test to screen further deterioration in RV function while on the waiting list for LUTX. More prospective follow-up echocardiographic studies are necessary to confirm such a hypothesis.
    Graft survival in pediatric kidney transplant recipients has increased in the last decades. Determining prognostic factors for graft function over time allows the identification of patients at risk for graft loss and could lead to improvement of current guidelines.

    Data were collected among pediatric kidney transplant recipients in a single center during the first 5 years after transplantation. Mixed model analysis was used to indicate possible prognostic factors for the loss of graft function.

    A total of 100 pediatric kidney transplant recipients were analyzed. Negative prognostics of graft function are higher donor age and higher recipient age, presence of obstructive uropathology, re-transplant, and occurrence of BK viremia. The negative influence on graft function of both donor age and presence of obstructive uropathology increased over time. In this study, the factors that did not influence graft function over time were the number of HLA mismatches, pre-transplant dialysis, intra-abdominal graft placement, ischemia time, occurrence of acute rejection, presence of lower urinary tract dysfunction, occurrence of urinary tract infections, and infections with cytomegalovirus and Epstein-Barr virus.

    This study showed that a higher donor age and higher recipient age, presence of obstructive uropathology, a re-transplant, and the occurrence of BK viremia were negative prognostic factors of graft function over time, in the first 5 years after transplant. Two groups-intervention and control-were formed in the study. For the intervention group, individual psychoeducation based on the Neuman systems model and involving cognitive behavioral therapy techniques (coping with depression program) was administered at six sessions.   • PURPOSE The relationship between postoperative complications and long-term survival after surgery for esophageal squamous cell carcinoma (ESCC) is controversial. METHOD A total of 210 patients with ESCC who underwent subtotal esophagectomy with a reconstructed gastric tube were investigated according to the development of postoperative complications. The associations of age, gender, T and N factors, and pStage with grade 0-2 complications (NSC) and grade 3 and higher complications (SC) were compared by propensity score-matching analysis. Fifty-one pairs of NSC and SC groups were selected for the final analysis. We divided 102 patients between the NSC and SC groups or between the no pulmonary complication (NPC) and the pulmonary complication (PC) groups. The overall survival (OS) and disease-free survival (DFS) were determined by the Kaplan-Meier method and were compared by log-rank tests. Possible predictors of OS and DFS were subjected to univariate analysis and multivariate Cox proportional hazard regression analysis. RESULTS The propensity score matching revealed that the 5-year OS and DFS of the NSC group were not different from those of the SC group. However, the 5-year OS of the PC group was significantly worse than that of the NPC group, while no significant differences were observed in the DFS between the PC and NPC groups. In the multivariate analysis, UICC pStage, pulmonary complication, and American Heart Association (AHA) classification for OS and UICC pStage for DFS were significant prognostic factors. CONCLUSION The OS and DFS did not differ in patients with or without severe postoperative complications. To solve the problem, we propose the evolutionary optimized multiple timescale recurrent neural network (EO-MTRNN) that is inspired by the neural plasticity of the human cortex. Our proposed network uses a method of evolutionary optimization to adjust its timescales and to rewire itself in terms of number of neurons and synapses. Moreover, it does not require additional neural networks for pre- and postprocessing input-output data. We validate our EO-MTRNN by applying it to a proposed benchmark training dataset with single and multiple sequence training cases, as well as by applying it to sensory-motor data from a robot. We compare different configuration modes of the network, and we compare the learning performance between a network configuration with manually set hyperparameters and a configuration with automatically estimated hyperparameters. The results show that automatically estimated hyperparameters yield approximately 43% better performance than manually estimated ones, without overfitting the given teaching data. Time-dependent bias was reduced using Cox regression analysis, which revealed that female (OR, 2.29; P = 0.041), overweight (OR, 2.67; P = 0.022), and initial biliary stent insertion (OR, 2.59; P = 0.016) were statistically significant predictors.

We calculated pooled odds ratios (ORs) of cognitive impairment for people living with HIV (PLWH) and 95 % confidence intervals (CIs) using random-effect models and calculated pooled mean difference (MD) for major cognitive domains between PLWH and HIV-uninfected adults. We assessed risk of bias using the Newcastle-Ottawa scale.

    Spinal cord injury (SCI) patients may have intestinal dysmotility and digestive symptoms that are associated with small intestinal bacterial overgrowth (SIBO). The aim of this study is to describe the prevalence of SIBO in SCI patients and the risk factors of its development.

    Twenty-nine consecutive SCI patients were studied (10 women/19 men; mean age 47 years), 16 with subacute injuries (<9 months) and 13 with chronic injuries (>1 year). Nine patients were affected by tetraplegia and 15 by paraplegia. Each patient underwent a glucose breath test according to the North American Consensus and the presence of abdominal symptoms was evaluated during the test. The results were compared with 15 non-neurological patients with SIBO.

    Six patients tested positive for SIBO (21%), all of them affected by SCI in the subacute phase, 6/16 vs. 0/13 in the chronic phase (P<.05) and the majority with tetraplegia, 5/9 vs. 1/19 with paraplegia (P<.05). No statistically significant relationship was found with other clinical characteristics. All the tests were positive for methane or mixed (methane and hydrogen), while only 67% of the controls had methane-predominant production (P>.05).

The aim of this article is to review the risk of infections in patients with IBD and RA, and the potential preventive measures. Spinal cord injury (SCI) patients may have intestinal dysmotility and digestive symptoms that are associated with small intestinal bacterial overgrowth (SIBO). The aim of this study is to describe the prevalence of SIBO in SCI patients and the risk factors of its development. Twenty-nine consecutive SCI patients were studied (10 women/19 men; mean age 47 years), 16 with subacute injuries (<9 months) and 13 with chronic injuries (>1 year). Nine patients were affected by tetraplegia and 15 by paraplegia. Each patient underwent a glucose breath test according to the North American Consensus and the presence of abdominal symptoms was evaluated during the test. The results were compared with 15 non-neurological patients with SIBO. Six patients tested positive for SIBO (21%), all of them affected by SCI in the subacute phase, 6/16 vs. 0/13 in the chronic phase (P<.05) and the majority with tetraplegia, 5/9 vs. 1/19 with paraplegia (P<.05). No statistically significant relationship was found with other clinical characteristics. All the tests were positive for methane or mixed (methane and hydrogen), while only 67% of the controls had methane-predominant production (P>.05). The primary objective of this study is to evaluate the effectiveness of an evidence-based educational program on healthcare providers' (HCPs) overall knowledge of stroke.

    A randomized block design with post-test only was used. A total of 189 HCPs (physicians, registered nurses, and paramedics) involved with treating stroke patients in the emergency were recruited. Participants were randomly assigned to either the intervention or waiting list control group. A one-session, stroke educational program was offered to the HCPs followed by a post-test designed to assess knowledge about stroke.

    A significant main effect on the profession type was found, with physicians having higher mean scores of stroke knowledge compared with nurses and paramedics (

    We retrospectively analyzed a total of 2,000 patients presenting to the ED of a tertiary teaching hospital in two time periods group 1 (G1) comprised 1,000 consecutive patients enrolled from December 21, 2012 to January 5, 2013 (all patients were examined by an internal medicine specialist); group 2 (G2) comprised 1,000 consecutive patients enrolled from December 21, 2018 to January 3, 2019 (all patients were examined by an emergency physician).

    The chest X-ray (CXR) was performed in 40.6% of all patients. There was no difference in the frequency of CXR (38.9% in G1 vs. 42.3% in G2,
    =0.152). More CXRs were performed in G2 patients older than 65 years, in female patients older than 65 years, in patients presenting during the evening and night shifts or off-hours, in patients with a history of malignancy, in patients with gastrointestinal bleeding, and in patients with bradycardia, but fewer in patients presenting with arrhythmia. No difference in the rates of pathological CXR was found (47.3% in G1 vs. 52.2% in G2,

  • To evaluate the efficacy of combined medication of risedronate sodium and raloxifene, a selective estrogen receptor modulator (SERM) on the postmenopausal osteoporosis (PMOP). PMOP patients underwent the combined medication of risedronate sodium and raloxifene (SERM, Treatment group), or only medication of risedronate sodium (Control group). After medication, more significant increases were observed in the bone densities of the lumber vertebra (L) and the neck of left femur of patients in the treatment group. https://www.selleckchem.com/products/abbv-cls-484.html Simultaneously, the levels of estrogen and progesterone in serum decreased sharply in the treatment group. The 190 refractory PM child patients were selected for the culture of pathogenic bacteria and analysis of drug resistance. In total, 190 pathogenic bacteria were detected, consisting of gram-positive bacteria (77.37%). Of the gram-positive bacteria, the resistance rate of patients with staphylococcus epidermidis, streptococcus pneumoniae or Staphylococcus haemolyticus to levofloxacin was 100%, while in gram-negative bacteria, the resistance rate of patients with klebsiella pneumoniae to gentamycin was 100%. In the observation group, patients had a higher effectiveness rate. Besides, patients in the observation group recovered rapidly from the fever and anomalies in cerebral spine fluid and peripheral white blood cells, and the inflammation was greatly improved. However, difference in the incidence rates of adverse reactions of patients between two groups showed no statistical significance. The rats were fed with the high-fat diet for 30d, which could induce a model of hyperlipidemia rat successfully. After the model rats were fed with nano yam polysaccharide of 50mg/ml and 100mg/ml per day for 12d and 30d, respectively. For each nano yam polysaccharide group, the blood glucose level was significantly reduced, the glucose tolerance, glycogen and the content of C-peptide were improved in alloxan rats. The method is coupled to double salting-out assisted liquid-liquid extraction (SALLE) which facilitates the detection and quantification of Metronidazole and Furazolidone in different dosage forms by subsequent HPLC analysis. Extraction parameters such as solvent, pH and salt concentration are optimized and liquid liquid extraction showed efficiency up to 102% and 100% for Metronidazole and Furazolidone respectively in chloroform, at pH 3 and low salt concentration (0.5g KCl and 0.5 gNaNO0.994 and 0.9936 respectively. LOD was 2.1μg/ml, 1.5μg/ml and LOQ was 21.1μg/ml, 15.2μg/ml for metronidazole benzoate and furazolidone. Precision of the method is 1.84% (RSD) and 1.72% (RSD) for Metronidazole and Furazolidine respectively. Moreover, accuracy is measured in terms of percent recovery which is more than 100% for both analytes under optimized conditions. Finally, robustness is evaluated by changing the flow rate and detection wavelength which is also obtained within permissible limit. This indicates that the proposed method is simple, efficient and shows excellent recoveries for simultaneous determination of Metronidazole and Furazolidone in different dosage forms and Industrial effluents.

LOD was 2.1μg/ml, 1.5μg/ml and LOQ was 21.1μg/ml, 15.2μg/ml for metronidazole benzoate and furazolidone. Precision of the method is 1.84% (RSD) and 1.72% (RSD) for Metronidazole and Furazolidine respectively. Moreover, accuracy is measured in terms of percent recovery which is more than 100% for both analytes under optimized conditions. Finally, robustness is evaluated by changing the flow rate and detection wavelength which is also obtained within permissible limit. This indicates that the proposed method is simple, efficient and shows excellent recoveries for simultaneous determination of Metronidazole and Furazolidone in different dosage forms and Industrial effluents.

  • RATIONALE The capsular warning syndrome (CWS) is a rare and special type of transient ischemic attacks (TIAs) syndrome. The pathophysiology of CWS is very complicate, and intracranial atherosclerotic stenosis (ICAS) is rare cause. Moreover, the effective and standard therapy has not yet been established. PATIENT CONCERNS A 47-year-old man experienced repeated and exacerbated TIAs of right hemiparesis and dysarthria. Fourteen hours after the first episode of TIAs, he developed more severe right hemiparesis and dysarthria, the National Institute of Health Stroke Scale (NIHSS) score was 12 points, and did not recover in a long time. DIAGNOSIS The computed tomography (CT) angiography displayed high stenosis in the M1 segment of the left middle cerebral artery. The patient was diagnosed as CWS with ICAS. INTERVENTIONS Loading dose of clopidogrel and aspirin were started but were ineffective, then we used recombinant tissue plasminogen (r-tPA) for thrombolysis therapy after repeat CT scan that showed small acute infarcts in the right putamen and no bleeding. OUTCOMES The patient was successfully treated by r-tPA intravenous thrombolysis after loading dose of dual-anti-platelet. He recovered rapidly, and the NIHSS score was 0 point, modified Rankin Scale score was 0 point, and Barthel Index score was 100 points at 3-month follow-up. LESSONS r-tPA combined with loading dose of dual antiplatelet appears safe and effective in carefully selected CWS patients with ICAS. In our study, we tried to check the correlation of tFMI with balance and physical performance and to determine the factors associated with tFMI.The tFMI was positively correlated with mean values of 4 m gait speed, repeat chair stand time in SPPB, TUG, and SARC-F and negatively correlated with hand grip, IADL, total balance test score in SPPB, total SPPB score, and age. Then, we randomly extracted 80% of all CT images from each patient for training, 10% for validation, and 10% for testing. Our results showed that machine learning (83.4%) outperformed CRP (53.2%), ESR (68.8%), and D-dimer (68.1%) separately in accuracy. Meanwhile, machine learning (88.0%) demonstrated highest sensitivity when compared with CRP (50.6%), ESR (73.0%), and D-dimer (51.7%). Considering the specificity, machine learning (77.0%) is better than CRP (59.4%) and ESR (62.2%), but not D-dimer (83.8%). The levels of STIM1 protein in plasma of these subjects were also assessed by enzyme-linked immunosorbent assay (ELISA).We found that the SNPs of STIM1 gene rs7934581, rs3794050, rs1561876, and rs3750996 were associated with increased PD risk, while rs3750994 SNP was not. An increased risk of PD was observed in subjects with the TAAG and TGAG haplotypes of rs7934581, rs3794050, rs1561876, rs3750996. Moreover, PD risk was significantly elevated only in subjects with age ≥60 years or females who carry the STIM1 rs3794050 minor allele.

  • Introduction Several measures to assess family planning service quality (FPQ) exist, yet there is limited evidence on their association with contraceptive discontinuation. Using data from the Measurement, Learning & Evaluation (MLE) Project, this study investigates the association between FPQ and discontinuation-while-in-need without switching in five cities in Kenya. Two measures of FPQ are examined - the Method Information Index (MII) and a comprehensive service delivery point (SDP) assessment rooted in the Bruce Framework for FPQ. Methods Three models were constructed two to assess MII reported in household interviews (as an ordinal and binary variable) among 1,033 FP users, and one for facility-level quality domains among 938 FP users who could be linked to a facility type included in the SDP assessment. Cox proportional hazards ratios were estimated where the event of interest was discontinuation-while-in-need without switching. Facility-level FPQ domains were identified using exploratory factor analysis (EFA) using SDP assessment data from 124 facilities. Results A woman's likelihood of discontinuation-while-in-need was approximately halved whether she was informed of one aspect of MII (HR 0.45, p less then 0.05), or all three (HR 0.51, p less then 0.01) versus receiving no information, when MII was assessed as an ordinal variable. Six facility-level quality domains were identified in EFA. Higher scores in information exchange, privacy, autonomy & dignity and technical competence were associated with a reduced risk of discontinuation-while-in-need (p less then 0.05). Conclusions The MII has potential as an actionable metric for FPQ monitoring at the health facility level. Furthermore, family planning facilities and programs should emphasize information provision and client-centered approaches to care alongside technical competence in the provision of FP care. Failure to incorporate cultural considerations such as food preferences, gender congruency with nursing care, awareness of faith and religious practices, and the use of alternative medicinal and herbal approaches to healthcare treatment, can result in poor treatment engagement and outcomes for patients. The provision of culturally competent health care for psychiatric patients must be patient-centered; with nurses and all healthcare team members collaborating with patients and their families (significant others) in the delivery of safe, high quality, and effective care. This study used a qualitative approach with interviews from 7 participants recruited from barbershops in Northern California. As a result, 3 major themes emerged from the data (a) minimal understanding of how to become a nurse, (b) the stigma of a feminized profession, and (c) a need for African-American men in nursing. Furthermore, this study found that barbershops are a viable setting for obtaining the perspectives of African-American men. This data provides important information about where schools of nursing can focus resources when trying to recruit African-American men. Providing detailed information about what nurses do and by depicting how nursing has changed over the decades to include men might be a mechanism for African-American men to consider nursing as a career option. The responsibilities associated with multiple roles can be overwhelming, especially when experiencing injustices (e.g., microaggression and role strain) while performing roles both at work and at home. African-American women remain the most disrespected and vulnerable population currently in healthcare as well as having the highest cardiovascular adverse event rate in the 20 to 49 years of age range. Chronic exposure to inequities as a Superwoman can activate inflammatory markers resulting in a weakened immune system, increased cardiovascular risk factors, and a shortened life expectancy. This article explores the Superwomen experience and its impact on health outcomes, with a focus on minimizing cardiovascular morbidity and mortality. In addition, PubMed and the journals the American Journal of Pharmaceutical Education, Currents in Pharmacy Teaching and Learning, and INNOVATIONS in pharmacy were searched. Data were retrieved on January 31, 2019 and systematic content analysis method was used. Number of publications, citations, H-index, and scope of IPE research including IPE settings, IPE curriculum, pedagogy, and assessment methods was recorded. https://www.selleckchem.com/products/cx-5461.html A total of 1693 references were retrieved that met the inclusion criteria. From the review of the literature, it was plausible to assume that the number of publications increased exponentially from 2010 to 2017. The results indicated that the top 5 healthcare disciplines contributing to the IPE literature included medicine (41.2%), nursing (36.2%), pharmacy (9.0%), dentistry (8.2%), occupational therapy (5.6%), and physical therapy (4.6%). A total of 295 journals published IPE-related articles. The Journal of Interprofessional Care published a total of 566 articles, accounting for 33% of all published articles. In addition, a total of 66 countries contributed to IPE publications. The top 4 countries publishing in the area of IPE were the United States (27%), Canada (17.5%), the United Kingdom (16.1%), and Australia (10.2%).
