Formal medical student engagement in curricular evaluation provides significant value through identification of opportunities for curricular change. Students provide diverse perspectives and have a unique vantage point, which allows them to see aspects of the curriculum that educators and administrators might not recognize. Current descriptions of student engagement are focused largely on collection, analysis, and presentation of summative feedback in the pre-clerkship curriculum. However, medical students could potentially contribute to curricular improvement in ways extending beyond
curricular evaluation. Student teams focused on identification of specific needs and project-based implementation of solutions represent one means of doing so but require a structured, organizing method in order to succeed.
We describe a novel, project-based, student-driven medical education initiative, the Special Projects Team, which is focused on identifying opportunities for forward-looking curricular enhancements beyrricular enhancements. This approach is broadly applicable to other medical schools with implementation requiring only a motivated student team, faculty advisor, and basic knowledge of the lean startup method.Phytochemical investigation of the fruit of Acronychia pedunculata (L.) Miq. led to the isolation of a new prenylated flavone, acroflavone A (1), together with eight known compounds (2-9). Their structures were elucidated by thorough analysis of mass spectrometric and NMR spectroscopic data. The isolated compounds were evaluated against several bacterial strains. Three known compounds (3-5) demonstrated antibacterial activities. To summarise the range and nature of decision-making aids that guide upper limb (UL) neurorehabilitation.
Our scoping review followed Arksey and O'Malley's 6-step framework. Electronic databases were systematically searched; grey literature was hand-searched. Included papers were (1) published in English; (2) related to UL rehabilitation for people with upper motor neuron conditions; and (3) provided a description of, or research on, a decision-making aid that guides therapists when choosing between two or more intervention approaches, techniques, or applications of a technique. Levels of evidence were rated. An expert panel of occupational therapists working in UL neurorehabilitation was consulted.
The 24 included articles described 15 decision-making aids with varying breadth and depth of the assessment process and suggested interventions. Six aids had published research, but lacked high quality evidence. The expert panel identified four key decision-making aids as being clinically useful. Preferred aids either included client-centred goal-setting within a holistic assessment or matched specific intervention options to distinct UL assessment results.

Periprosthetic hip and knee infection remains one of the most severe complications following arthroplasty, with an incidence between 0.5% to 1%. This study compares the outcomes of revision surgery for periprosthetic joint infection (PJI) following hip and knee arthroplasty prior to and after implementation of a specialist PJI multidisciplinary team (MDT).
Data was retrospectively analyzed from a single centre. In all, 29 consecutive joints prior to the implementation of an infection MDT in November 2016 were compared with 29 consecutive joints subsequent to the MDT conception. All individuals who underwent a debridement antibiotics and implant retention (DAIR) procedure, a one-stage revision, or a two-stage revision for an acute or chronic PJI in this time period were included. The definition of successfully treated PJI was based on the Delphi international multidisciplinary consensus.
There were no statistically significant differences in patient demographics or comorbidities between the groups. Thereetic knee and hip joint infection shows a significant reduction in failure rates following revision surgery. Following implementation of our MDT, our success rate in treating PJI is 96.55%, higher than what current literature suggests. We advocate the role of a specialist infection MDT in the management of patients with a PJI to allow an individualized patient-centred approach and care plan, thereby reducing postoperative complications and failure rates. Four (23.5%) patients had in-field failures at 3.7, 11.2, 24.5, and 27.5 months after start of radiation, all treated with definitive dosing of radiation therapy. Here, we present a rare case of recurrent gynandroblastoma in a premenopausal woman with a DICER1 germline mutation. The patient was referred to our clinic for new symptoms of hormonal imbalance with a history of ovarian juvenile granulosa cell tumor (JGCT). Evaluation revealed a 5x5cm complex right adnexal mass and rising inhibin B. Patient underwent total abdominal hysterectomy with right salpingo-oophorectomy, omentectomy and right pelvic and para-aortic lymphadenectomy. Pathology showed a right ovarian gynandroblastoma. Somatic biallelic mutations in the RNase IIIb domain of DICER1 were identified; a 23-gene germline panel confirmed a germline DICER1 pathogenic variant. Cascade testing of her children documented that both daughters inherited the pathogenic variant. Here, we present a rare case of recurrent gynandroblastoma in a premenopausal woman with a DICER1 germline mutation. The patient was referred to our clinic for new symptoms of hormonal imbalance with a history of ovarian juvenile granulosa cell tumor (JGCT). Evaluation revealed a 5x5cm complex right adnexal mass and rising inhibin B. Patient underwent total abdominal hysterectomy with right salpingo-oophorectomy, omentectomy and right pelvic and para-aortic lymphadenectomy. Pathology showed a right ovarian gynandroblastoma. Somatic biallelic mutations in the RNase IIIb domain of DICER1 were identified; a 23-gene germline panel confirmed a germline DICER1 pathogenic variant. Obesity and diabetes mellitus are prevalent among the African-American/Black population. Within this group of SCD patients, obesity was associated with significantly higher prevalence of diabetes mellitus. The underlying reasons for our observed results of low prevalence rate of obesity in SCD remain speculative but may be related to reduced calorie intake, increased calorie use due to hypermetabolism, reduced intestinal absorption, or intestinal dysbiosis.A novel geminivirus was identified in France and Spain in asymptomatic plants of white clover (Trifolium repens) and shrub medick (Medicago arborea). After reviewing sinus anatomy and sinogenic pain, we provide information to assist clinicians in correctly diagnosing patients with the additional goal of avoiding unnecessary investigations and treatments. Migraine medications can be used as both a treatment and a diagnostic tool. Other differential diagnoses of facial pain are discussed. Sinus headache is not a diagnosis. All patients with facial pain or pressure with sinus symptoms should be evaluated for migraine because most sinus headache presentations are migraine and require migraine-directed treatment.

Herein, we explore these advances and reflect on the future directions to take for the development of sensing devices based on AIECL.

Expression of ATMIN messenger RNA (mRNA) was detected in CRC specimens with microsatellite instability (MSI) characteristics. The effects of ectopic ATMIN expression and ATMIN knockdown on invasion abilities were evaluated in MSI-high cell lines, and liver metastasis ability was investigated in vivo. Sports participation has the potential to contribute to young people's health. A prerequisite for young people to benefit from sports is that they stay in sports. Studies that consider both personal and contextual factors are needed to unpack the broader health-promoting potential of youth sports. The purpose of the study is to contribute to knowledge about the health-promoting potential of young people's participation in organized sports by exploring youth perspectives on what makes a sports club health-promoting with a focus on health resources that young people consider important for sports club participation. For this cross-sectional study a brief survey was conducted with 15-16 year old students (n = 123) at two schools in Sweden, asking three open-ended questions about their participation in sports. The study used a salutogenic theory-driven analysis in combination with statistical analysis. Five health resources that young people consider important for sports club participation are revealed. On an individual, more 'swimmer'-related level, these are personal well-being and social relations, including relationally meaningful activities, and on an organizational level, relating to the 'river', that sports clubs offer a supportive and well-functioning environment. For sports clubs to be health-promoting settings for young people and thus hopefully to reduce drop-out, we need a more sustainable approach emphasizing drop-in, drop-through, and drop-over as a continuous iterative process. We also need to consider the complexity of sports participation for young people, involving individual, organizational and environmental issues.We show a description of pregnancy cohorts in the European region. Our investigation identified 66 pregnancy cohorts, mostly hosted in Western Central Europe. Among these 66 cohorts, 24 began recruitment before the year 2000, while six cohorts are still enrolling. The most common topics were lifestyle, environment and nutrition with allergies and neurodevelopment being a minority. We observed a pattern of positive correlations between data collected using medical records, structured interviews, and the collection of biological samples. Objectively assessed data were negatively correlated with self-administered questionnaires. Eight cohorts addressed intrauterine exposure, focusing on environmental pollutants such as endocrine-disrupting chemicals. The effects of these compounds on the developing foetus have been studied greatly, but more research on their effects is still needed. Many cohorts investigated genetics through the collection of biological samples from the mothers and children, to improve knowledge on the mother-to-child transmission of genetic information, antibodies, microbiota, etc. Paediatric epidemiology represents an important field of research since preserving healthy lives from conception onwards is the most efficient way to improve population health. According to our report, it seems that this field of research is well developed in Europe, where numerous high profile studies are currently ongoing.To address climate change, health, and food-related challenges at the international and regional level, policy makers and researchers are starting to acknowledge the importance of building and developing sustainable food systems (SFSs). This study aims to discuss the drivers of, barriers to, and policy recommendations for developing sustainable food systems in four European countries (Germany, Italy, Norway, and Romania). We used critical frame analysis to investigate national policy documents on sustainable food systems and conducted in-depth interviews with various national stakeholders representing policy makers, agrifood businesses, and civil society. The novelty of this research lies in comparing national policy approaches and stakeholders' opinions on SFS development in a multi-country analysis. These European countries have different conditions in terms of geography, socioeconomic situation, environmental performance, and sustainability orientation. Several cross-cultural differences and gaps in the existing national policies for sustainable food systems were identified, and solutions that help overcome these issues have been suggested. The first step in developing SFS should focus on interdisciplinary and trans-sectorial policy integration combined with increasing stakeholder collaboration across all sectors of the economy. Sports participation has the potential to contribute to young people's health. A prerequisite for young people to benefit from sports is that they stay in sports. Studies that consider both personal and contextual factors are needed to unpack the broader health-promoting potential of youth sports. The purpose of the study is to contribute to knowledge about the health-promoting potential of young people's participation in organized sports by exploring youth perspectives on what makes a sports club health-promoting with a focus on health resources that young people consider important for sports club participation. For this cross-sectional study a brief survey was conducted with 15-16 year old students (n = 123) at two schools in Sweden, asking three open-ended questions about their participation in sports. The study used a salutogenic theory-driven analysis in combination with statistical analysis. Five health resources that young people consider important for sports club participation are revealed. On an individual, more 'swimmer'-related level, these are personal well-being and social relations, including relationally meaningful activities, and on an organizational level, relating to the 'river', that sports clubs offer a supportive and well-functioning environment. For sports clubs to be health-promoting settings for young people and thus hopefully to reduce drop-out, we need a more sustainable approach emphasizing drop-in, drop-through, and drop-over as a continuous iterative process. We also need to consider the complexity of sports participation for young people, involving individual, organizational and environmental issues.We show a description of pregnancy cohorts in the European region. Our investigation identified 66 pregnancy cohorts, mostly hosted in Western Central Europe. Among these 66 cohorts, 24 began recruitment before the year 2000, while six cohorts are still enrolling. The most common topics were lifestyle, environment and nutrition with allergies and neurodevelopment being a minority. We observed a pattern of positive correlations between data collected using medical records, structured interviews, and the collection of biological samples. According to our report, it seems that this field of research is well developed in Europe, where numerous high profile studies are currently ongoing.To address climate change, health, and food-related challenges at the international and regional level, policy makers and researchers are starting to acknowledge the importance of building and developing sustainable food systems (SFSs). This study aims to discuss the drivers of, barriers to, and policy recommendations for developing sustainable food systems in four European countries (Germany, Italy, Norway, and Romania). We used critical frame analysis to investigate national policy documents on sustainable food systems and conducted in-depth interviews with various national stakeholders representing policy makers, agrifood businesses, and civil society. The novelty of this research lies in comparing national policy approaches and stakeholders' opinions on SFS development in a multi-country analysis. These European countries have different conditions in terms of geography, socioeconomic situation, environmental performance, and sustainability orientation. Several cross-cultural differences and gaps in the existing national policies for sustainable food systems were identified, and solutions that help overcome these issues have been suggested. The first step in developing SFS should focus on interdisciplinary and trans-sectorial policy integration combined with increasing stakeholder collaboration across all sectors of the economy. With the focus of leukaemia management shifting to the implications of low-level disease burden, increasing attention is being paid on the development of highly sensitive methodologies required for detection. There are various techniques capable of identification of measurable residual disease (MRD) either evidencing as relevant mutation detection [e.g. nucleophosmin 1 (NPM1) mutation] or trace levels of leukaemic clonal populations. The vast majority of these methods only permit detection of a single clone or mutation. However, mass spectrometry and next-generation sequencing enable the interrogation of multiple genes simultaneously, facilitating a more complete genomic profile. In the present review, we explore the methodologies of both techniques in conjunction with the important advantages and limitations associated with each assay. We also highlight the evidence and the various instances where either technique has been used and propose future strategies for MRD detection.Experimental autoimmune vasculitis (EAV) is a model of antineutrophil cytoplasm antibody (ANCA)-associated vasculitis (AAV) induced by immunisation of susceptible rat strains with myeloperoxidase (MPO). This resulted in a significant increase in disease severity at day 28 compared to MPO immunisation alone - with more urinary abnormalities, infiltrating glomerular leucocytes, and crescent formation that progressed to glomerular and tubulointerstitial scarring by day 56, recapitulating important features of human disease. Importantly, the glomerulonephritis remained pauci-immune, and was strictly dependent on the presence of autoimmunity to MPO, as there was no evidence of renal disease following administration of sub-nephritogenic NTS alone or after immunisation with a control protein in place of MPO. Detailed phenotyping of glomerular leucocytes identified an early infiltrate of non-classical monocytes following NTS administration that, in the presence of autoimmunity to MPO, may initiate the subsequent influx of classical monocytes which augment glomerular injury. We also showed that this model can be used to test novel therapeutics by using a small molecule kinase inhibitor (fostamatinib) that rapidly attenuated both glomerular and pulmonary injury over a 4-day treatment period. Many believe women's oral health deteriorates as a result of having children. If so, such associations should exist among women but not among men. The aims of this study were to investigate whether number of children is associated with experience of dental disease and tooth loss among both men and women and to examine whether this association is affected by other variables of interest.
This study used data from the Dunedin Multidisciplinary Health and Development study, a longitudinal study of 1037 individuals (48.4% female) born from April 1972 to March 1973 in Dunedin, New Zealand, who have been examined repeatedly from birth to age 45 years.
Data were available for 437 women and 431 men. Those with low educational attainment were more likely to have more children and began having children earlier in life. While the association with HIV infection has already been characterized, limited information is available on the association between chronic viral hepatitis and NHL in sub-Saharan Africa. A case-control study was conducted in referral hospitals of Abidjan (Cote d'Ivoire) and Dakar (Senegal). Cases of NHL were matched with controls on age, gender and participating site. The diagnosis of NHL relied on local pathological examination completed with immunohistochemistry. HIV, HBV and HCV serology tests were systematically performed. A conditional logistic regression model estimated the associations by the Odds Ratio (OR) with their 95% confidence interval (CI). A total of 117 NHL cases (Abidjan n = 97, Dakar n = 20) and their 234 matched controls were enrolled. Cases were predominantly men (68.4%) and had a median age of 50 years (IQR 37-57). While Diffuse Large B-cell lymphoma were the most reported morphological type (n = 35) among mature B-cell NHL, the proportion mature T-cell NHL (30%) was high. The prevalence figures of HBV, HCV and HIV infection were 12.8%, 7.7% and 14.5%, respectively among cases of NHL. In multivariate analysis, HBV, HCV and HIV were independently associated with NHL with OR of 2.23 (CI 1.05-4.75), 4.82 (CI 1.52-15.29) and 3.32 (CI 1.54-7.16), respectively. Chronic viral hepatitis B and C were significantly associated with NHL in West Africa. Timely preventive measures against HBV infection and access to curative anti-HCV treatment might prevent a significant number of NHL.

Social factors associated with both the timing of reproductive patterns and health behaviors influence the risk of dental disease and its management.Non-Hodgkin lymphomas (NHL) are underestimated causes of cancer in West Africa where chronic viral hepatitis and HIV are endemic. While the association with HIV infection has already been characterized, limited information is available on the association between chronic viral hepatitis and NHL in sub-Saharan Africa. A case-control study was conducted in referral hospitals of Abidjan (Cote d'Ivoire) and Dakar (Senegal). Cases of NHL were matched with controls on age, gender and participating site. The diagnosis of NHL relied on local pathological examination completed with immunohistochemistry. HIV, HBV and HCV serology tests were systematically performed. A conditional logistic regression model estimated the associations by the Odds Ratio (OR) with their 95% confidence interval (CI). A total of 117 NHL cases (Abidjan n = 97, Dakar n = 20) and their 234 matched controls were enrolled. Cases were predominantly men (68.4%) and had a median age of 50 years (IQR 37-57). While Diffuse Large B-cell lymphoma were the most reported morphological type (n = 35) among mature B-cell NHL, the proportion mature T-cell NHL (30%) was high. The prevalence figures of HBV, HCV and HIV infection were 12.8%, 7.7% and 14.5%, respectively among cases of NHL. In multivariate analysis, HBV, HCV and HIV were independently associated with NHL with OR of 2.23 (CI 1.05-4.75), 4.82 (CI 1.52-15.29) and 3.32 (CI 1.54-7.16), respectively. Chronic viral hepatitis B and C were significantly associated with NHL in West Africa. It has been reported that the viability and migration of vascular smooth muscle cells contributes to arteriovenous fistula stenosis. Hydroxysafflor Yellow A (HSYA) has been demonstrated to inhibit the viability and migration of VSMCs by regulating Akt signaling. The present study aimed to investigate the role of HSYA on the viability and migration of human umbilical vein smooth muscle cells (HUVSMCs) following stimulation using serum from rats with chronic renal failure (CRF), and to determine the effects of HSYA on PI3K/Akt signaling. Wistar rats were randomly divided into two groups, control and CRF groups. Serum from each group was collected to stimulate the HUVSMCs. A cohort study was conducted between March 2013 and March 2020, in Montreal and Winnipeg. All children referred with reported history of benign reactions (i.e., limited to the skin with no mucosal lesions and no vesicles) to amoxicillin were recruited and a two-step graded oral challenge (GOC) was conducted. Data were collected on demographics, clinical characteristics, and comorbidities. Eligible children were followed to assess reactions to subsequent use of amoxicillin and to assess the safety of cephalexin use in children with a positive GOC.
Among 1914 children recruited, 1811 (94.6%) tolerated the GOC, 42 (2.2%) developed mild immediate reactions, and 61 (3.2%) developed mild nonimmediate reactions. Among 265 participants who had a negative GOC and reused amoxicillin; 226 (85.3%) reported tolerance, and 39 (14.7%) had mild cutaneous reactions. Chronic urticaria (adjusted Odds Ratio [aOR = 1.16; 95%CI, 1.09-1.23]) and an index reaction occurring within 5 minutes of exposure [aOR = 1.09; 95%CI,1.04-1.14] were associated with immediate reactions during the GOC. Symptoms lasting longer than 7 days [aOR = 1.05; 95%CI, 1.02-1.09] and parental drug hypersensitivity [aOR = 1.04; 95%CI, 1.03-1.06] were associated with nonimmediate reactions. Among those reacting to the GOC 12.5% reacted with mild cutaneous reactions to cephalexin challenge.

Teeth are characterized by a specific chemical composition and microstructure, which are related to their nature, permanent and deciduous, and to the sides, lingual and vestibular. Deeper knowledge in this topic could be useful in clinical practice to develop new strategies in restorative dentistry and in the choice of materials with the best performances. In this study, Raman MicroSpectroscopy (RMS), Scanning Electron Microscopy (SEM), Energy Dispersive X-ray Spectrometry (EDS), and Vickers MicroHardness (VMH) were exploited to (1) identify the microstructure and the chemical/elemental composition of permanent and deciduous human teeth, also characterizing their lingual and vestibular sides, and (2) validate a new multidisciplinary analytical approach, for obtaining multiple information on calcified tissues. A good linear correlation was found between RMS spectral profiles and EDS and VMH measurements, suggesting that RMS may be considered a useful and non-destructive diagnostic tool for obtaining multiple information on calcified tissues.We hereby review molecular dynamics simulations for anharmonic gas phase spectroscopy and provide some of our opinions of where the field is heading. With these new directions, the theoretical IR/Raman spectroscopy of large (bio)-molecular systems will be more easily achievable over longer time-scale MD trajectories for an increase in accuracy of the MD-IR and MD-Raman calculated spectra. With the new directions presented here, the high throughput 'decoding' of experimental IR/Raman spectra into 3D-structures should thus be possible, hence advancing e.g. the field of MS-IR for structural characterization by spectroscopy. We also review the assignment of vibrational spectra in terms of anharmonic molecular modes from the MD trajectories, and especially introduce our recent developments based on Graph Theory algorithms. Graph Theory algorithmic is also introduced in this review for the identification of the molecular 3D-structures sampled over MD trajectories.β-resorcylic acid (BR) is a phytochemical which is widely used in the food industry as a flavouring agent and preservative. It has also been found to exhibit antibacterial action against several types of food-borne bacteria. DNA is the main molecular target for many small molecules of therapeutic importance. Hence, the interest is rapidly growing among the researchers to elucidate the interaction between small molecules and DNA. Thus, paving the way to design novel DNA-specific drugs. In this study, an attempt was made to examine the mechanism of binding of BR with calf thymus DNA (ctDNA) with the help of various experiments based on spectroscopy and in silico studies. The spectroscopic studies like UV absorption and fluorescence affirmed the complex formation between BR and ctDNA. The observed binding constant was in the order of 103 M-1 which is indicative of the groove binding mechanism. These findings were further verified by dye-displacement assay, potassium iodide quenching, urea denaturation assay, the study of the effect of ssDNA, circular dichroism and DNA thermal denaturing studies. Different temperature-based fluorescence and isothermal titration calorimetry (ITC) experiments were employed to evaluate thermodynamic parameters. The analysis of thermodynamic parameters supports the enthalpically driven, exothermic and spontaneous nature of the reaction between BR and ctDNA. The forces involved in the binding process were mainly found to be hydrogen bonding, van der Waals and hydrophobic interactions. The results obtained from the molecular docking and molecular dynamics (MD) simulation were consistent with the in vitro experiments, which support the groove binding mode of BR with ctDNA.Solid-state NMR spectroscopy has played a significant role in elucidating the structure and dynamics of materials and biological solids at a molecular level for decades. In particular, the 1H double-quantum/single-quantum (DQ/SQ) chemical shift correlation experiment is widely used for probing the proximity of protons, rendering it a powerful tool for elucidating the hydrogen-bonding interactions and molecular packing of various complex molecular systems. Two factors, namely, the DQ filtering efficiency and t1-noise, dictate the quality of the 2D 1H DQ/SQ spectra. Experimentally different recoupling sequences show varied DQ filtering efficiencies and t1-noise. Herein, after a systematic search of symmetry-based DQ recoupling sequences, we report that the symmetry-based γ-encoded RNnν sequences show superior performance to other DQ recoupling sequences, which not only have a higher DQ recoupling efficiency but can also significantly reduce t1-noise. The origin of t1-noise is further discussed in detail via extensive numerical simulations. We envisage that such γ-encoded RNnν sequences are superior candidates for DQ recoupling in proton-based solid-state NMR spectroscopy due to its capability of efficiently exciting DQ coherences and suppressing t1-noise.Uptake of most metal nanoparticles (NPs) in organisms is assumed to be mainly driven by the bioavailability of the released ions, as has been verified in controlled and short-term exposure tests. However, the changeability of NPs and the dynamic processes which NPs undergo in the soil environment, bring uncertainty regarding their interactions with soil organisms over a long period of time. To assess the potential impacts of long-term exposure scenarios on the toxicokinetic of metal NPs, earthworms Eisenia fetida were exposed to soils spiked with pristine Ag-NP, aged Ag-NP (Ag2S-NP) and ionic Ag for nine months, and results were compared to those from a similar short-term (28 days) experiment, conducted under similar conditions. Overall, there were no statistical differences between long-term accumulation patterns in earthworms exposed to pristine Ag-NP and AgNO3, while for Ag2S-NP, the amount of Ag internalized after 9 months was five times lower than for the other treatments. Average Ag concentrations in soil pore water in all treatments did not change over time, however the soil pH decreased and electrical conductivity increased in all treatments.
Teeth are characterized by a specific chemical composition and microstructure, which are related to their nature, permanent and deciduous, and to the sides, lingual and vestibular. Deeper knowledge in this topic could be useful in clinical practice to develop new strategies in restorative dentistry and in the choice of materials with the best performances. In this study, Raman MicroSpectroscopy (RMS), Scanning Electron Microscopy (SEM), Energy Dispersive X-ray Spectrometry (EDS), and Vickers MicroHardness (VMH) were exploited to (1) identify the microstructure and the chemical/elemental composition of permanent and deciduous human teeth, also characterizing their lingual and vestibular sides, and (2) validate a new multidisciplinary analytical approach, for obtaining multiple information on calcified tissues.
