  • In this paper, we contribute to the theorizing of patient involvement in organizational improvement by exploring concepts of "learning from patients" as mechanisms of organizational change. Using the concept of metaphor as a theoretical bridge, we analyse interview data (

    Patient involvement activities are used as part of quality improvement efforts in healthcare organizations worldwide. One fundamental assumption underpinning this activity is the notion that organizations must "learn from patients" in order to enact positive organizational change. Despite this emphasis on learning, there is a paucity of research that theorizes learning or connects concepts of learning to organizational change within the domain of patient involvement.

    Intracranial pressure (ICP), variability in perfusion, and resulting ischemia are leading causes of secondary brain injury in patients treated in the neurointensive care unit. Continuous, accurate monitoring of cerebral blood flow (CBF) and ICP guide intervention and ultimately reduce morbidity and mortality. Currently, only invasive tools are used to monitor patients at high risk for intracranial hypertension.

    Diffuse correlation spectroscopy (DCS), a noninvasive near-infrared optical technique, is emerging as a possible method for continuous monitoring of CBF and critical closing pressure (CrCP or zero-flow pressure), a parameter directly related to ICP.

    We optimized DCS hardware and algorithms for the quantification of CrCP. Toward its clinical translation, we validated the DCS estimates of cerebral blood flow index (CBFi) and CrCP in ischemic stroke patients with respect to simultaneously acquired transcranial Doppler ultrasound (TCD) cerebral blood flow velocity (CBFV) and CrCP.

    We found CrCP derived from DCS and TCD were highly linearly correlated (ipsilateral R2  =  0.77, p  =  9  ×  10  -  7; contralateral R2  =  0.83, p  =  7  ×  10  -  8). We found weaker correlations between CBFi and CBFV (ipsilateral R2  =  0.25, p  =  0.03; contralateral R2  =  0.48, p  =  1  ×  10  -  3) probably due to the different vasculature measured.

We discuss a case of a 23-year-old woman, who presented during the 8th month of her first pregnancy with debilitating pain in her lower ****. Several differential diagnoses were explored, including autoimmune aetiologies and pregnancy associated osteoporosis, before OCI was diagnosed via an MRI. The patient was managed conservatively with intravenous analgesics and physiotherapy, after which her pain abated gradually. On her 3rd day of admission, the patient went into premature labour and gave birth to a healthy child via vaginal delivery. Her pain resolved completely within days of delivering her baby.

Histologically, it shows sheets of polyhedral epithelial cells with prominent intercellular bridges, hyperchromatic nuclei and globules of amyloid-like material among tumour cells which undergoes calcification. We present a case of CEOT with classical histopathological picture in a 37-year-old HIV positive patient with long evolution history. Its association with HIV positivity, long clinical history and delay in surgical treatment make it a challenging case to report.Emphysematous pyelonephritis (EPN) is a type of critical renal infection having dire consequences at times. Although she had an initial improvement clinically but later on due to recurrent urinary tract infections and non-resolving right kidney abscess and fever right sided nephrectomy was done. This is a very rare presentation and has not previously reported **** in literature.

The patient received leflunomide and low-dose corticosteroids, then etanercept was introduced because of persistent disease activity. He had no history of tuberculosis infection or contact, chest CT-scan was normal. Infectious Disease (ID) specialists manage severe infections that require hospitalization and are a logical resource to engage patients in medication treatment for OUD (MOUD). An injectable long-acting monthly formulation of buprenorphine (LAB) has a potential advantage for initiating MOUD within hospital settings and bridging to treatment after discharge.

    A randomized multi-site trial tests a new model of care (ID/LAB) in which OUD and infections are managed by ID specialists and hospitalists using LAB coupled with referrals to community resources for long-term MOUD. A sample of 200 adults admitted to three U.S. hospitals for OUD and infections are randomly assigned 11 to ID/LAB or treatment as usual (TAU). The primary outcome measure is the proportion of patients enrolled in effective MOUD at 12weeks after randomization. Secondary outcomes include relapse to opioid use, adherence to infectious disease treatment, infection morbidity and mortality, and drug overdose.

However, MTDL combining with the inhibitory activity against PDE2A and other anti-AD factors such as antioxidants has not been developed yet. Herein, a novel series of PDE2 inhibitors with antioxidant capacities were designed, synthesized, and evaluated. Most compounds showed remarkable inhibitory activities against PDE2A as well as antioxidant activities.   • The Shoulder Pain and Disability Index (SPADI) is a measuring instrument for pain and disability in patients with shoulder dysfunction. However, its structure is controversial (ie, 1 or 2 domains).

    To investigate the structural validity of the Brazilian version of the SPADI in patients with chronic shoulder pain.

    Cross-sectional study.

    Brazilian Portuguese speakers of both genders aged 18 years or older with shoulder pain for at least 90 days (chronic) were included in this study. The structure of the SPADI was assessed by means of exploratory factor analysis with the implementation of parallel analysis and by confirmatory factor analysis. For the latter, the following goodness-of-fit indices were considered root mean square error of approximation with 90% confidence interval, comparative fit index, Tucker-Lewis index, standardized root mean square residual, and chi-square/degrees of freedom.

    A total of 103 participants were included in the study. The original version of the SPADI with 2 domains and 13 items showed high covariance between domains (0.98), suggesting one-dimensionality. After exploratory factor analysis with the implementation of parallel analysis, one-dimensionality was found in the SPADI. Comparing models with 1 and 2 domains by means of confirmatory factor analysis, both presented adequate values (chi-square/degrees of freedom<3; comparative fit index and Tucker-Lewis index>0.90; root mean square error of approximation and standardized root mean square residual<0.08). However, the model with one domain is more suitable for presenting lower values of Akaike information criterion (3308.713) and Bayesian information criterion (3377.216).

    Hand therapists and health care providers across the spectrum have been profoundly impacted by COVID-19. Greater insight and information regarding how practitioners have been affected by this unparalleled pandemic is important.

    Survey research was performed to examine the impact of the COVID-19 pandemic on hand therapy practice.

    Four constructs guided the development of the survey psychosocial and financial impact; safety practice patterns; changes in current practice patterns; use of telehealth. The survey was distributed to members of the American Society of Hand Therapists from April 14, 2020 through May 4, 2020. Descriptive demographic data were obtained. Frequencies were examined using ChiSquare, correlations were examined using Spearman Correlation Coefficient, and means were compared via independent t-test.

    A total of 719 members responded to the survey. Eighty-six percent of therapists reported feeling more stress than they did prior to the COVID-19 pandemic. This levn caseloads. In-person caseloads shifted primarily to postoperative cases.

Several new technologies have been developed with an aim to reduce this from mechanical devices to advanced imaging techniques. Fluorescence angiography (FA) with indocyanine green (ICG) in colorectal surgery is now a well-established technique and may have a role in reducing AL. By using FA, we are able to have a visual representation of perfusion which aids intraoperative decision making. The main impact is change in the level of bowel transection at the proximal side of an anastomosis and provide a more objective and confident assessment of bowel perfusion. Previous studies have shown that routine FA use is safe and reproducible. Recent results from randomized control trials and meta-analyses show that FA use reduces the rate of anastomotic leak. The main limitation of FA is its lack of ability to quantify perfusion. Novel technologies are being developed that will quantify tissue perfusion and oxygenation. CWI decreased tumor necrosis factor α (TNF-α) immediately and 1 h after the CWI. Although TNF-α had recovered to the pre-immersion level at 2 h after CWI, its natural circadian cycle kinetics was disrupted until 12 h after the CWI.
    The Shoulder Pain and Disability Index (SPADI) is a measuring instrument for pain and disability in patients with shoulder dysfunction. However, its structure is controversial (ie, 1 or 2 domains). To investigate the structural validity of the Brazilian version of the SPADI in patients with chronic shoulder pain. Cross-sectional study. Brazilian Portuguese speakers of both genders aged 18 years or older with shoulder pain for at least 90 days (chronic) were included in this study. The structure of the SPADI was assessed by means of exploratory factor analysis with the implementation of parallel analysis and by confirmatory factor analysis. For the latter, the following goodness-of-fit indices were considered root mean square error of approximation with 90% confidence interval, comparative fit index, Tucker-Lewis index, standardized root mean square residual, and chi-square/degrees of freedom. A total of 103 participants were included in the study. The original version of the SPADI with 2 domains and 13 items showed high covariance between domains (0.98), suggesting one-dimensionality. After exploratory factor analysis with the implementation of parallel analysis, one-dimensionality was found in the SPADI. Comparing models with 1 and 2 domains by means of confirmatory factor analysis, both presented adequate values (chi-square/degrees of freedom<3; comparative fit index and Tucker-Lewis index>0.90; root mean square error of approximation and standardized root mean square residual<0.08). However, the model with one domain is more suitable for presenting lower values of Akaike information criterion (3308.713) and Bayesian information criterion (3377.216). By using FA, we are able to have a visual representation of perfusion which aids intraoperative decision making. The main impact is change in the level of bowel transection at the proximal side of an anastomosis and provide a more objective and confident assessment of bowel perfusion. Previous studies have shown that routine FA use is safe and reproducible. Recent results from randomized control trials and meta-analyses show that FA use reduces the rate of anastomotic leak. The latter illustrates resilience of glassylike dynamics in the presence of active photonic degrees of freedom, suggesting that disordered quantum many-body systems with resonant photons or phonons can display a rich diagram of nonequilibrium responses, with near future applications for quantum information science.When a high power laser beam irradiates a small aperture on a solid foil target, the strong laser field drives surface plasma oscillation at the periphery of this aperture, which acts as a "relativistic oscillating window." Here, we demonstrate a supercompact integrated quantum CNOT gate on silicon by using the concept of symmetry breaking of a six-channel waveguide superlattice. The present path-encoded quantum CNOT gate is implemented with a footprint of 4.8×4.45  μm^2 (∼3λ×3λ) as well as a high-process fidelity of ∼0.925 and a low excess loss of less then 0.2  dB. The footprint is shrunk significantly by ∼10 000 times compared to those previous results based on dielectric waveguides. This offers the possibility of realizing practical large-scale quantum information processes and paving the way to the applications across fundamental science and quantum technologies.Microresonators on a photonic chip could enhance nonlinear optics effects and thus are promising for realizing scalable high-efficiency frequency conversion devices. However, fulfilling phase matching conditions among multiple wavelengths remains a significant challenge. This study also underscores the potentiality of next-generation sequencing in the genetic diagnosis of patients with neurologic diseases with an unknown cause.Biogenics are compounds produced by living organisms such as animals, plants, bacteria, etc. Probiotics and their biogenics are known for their antimicrobial potential. Therefore, the present study was designed to evaluate the antibiofilm potential of probiotic-derived biogenics in conjunction with zingerone against the Pseudomonas aeruginosa PAO1 biofilm. Cell-free supernatant (CFS) of potential probiotics Pediococcus acidilactici BNS5B and Lactobacillus fermentum PUM was found to inhibit the growth of Ps. aeruginosa PAO1 maximally among the nineteen isolated lactic acid bacteria. L. fermentum PUM produced precipitated protein fraction (PP), organic acids (OAs), exopolysaccharides (EPSs), biosurfactants (BSs) and various volatile antimicrobial compounds, while Ped. acidilactici BNS5B was found to produce PP, OA, EPS, BS and fewer volatile antimicrobial compounds only. The effect of flubendiamide and thiocyclam hydrogen oxalate insecticides was assessed on cellular energy allocation (CEA) of the third instar larvae of T. absoluta ingesting six different tomato cultivars. Plant leaves were treated with LC50 concentration of both insecticides and their energy available (Ea), energy consumption (Ec), and CEA were calculated. The results showed that total energy reserves (protein, carbohydrate, and lipid budgets) were significantly reduced after exposure to insecticides and tomato secondary metabolites. The larvae fed on Riogrande and Super Chief cultivars showed the least amount of energy available after treatment with both insecticides. The highest rate of oxygen consumption was observed in larvae fed on Calj, Super Luna, and Super strain B after treatment with flubendiamide. The amount of CEA decreased in treated larvae compared to untreated larvae. This reduction was statistically higher in treated larvae that fed on Riogrande and Super strain B cultivars. Reduction in CEA is probably due to the expenses of dealing with detoxification of insecticides and plant metabolites. Given this, attention has turned to the use of targeted therapies addressing dysregulation of B-cell signaling pathways. Drugs such as immunomodulatory agents, proteasome inhibitors, and Bruton's tyrosine kinase inhibitors have shown success in the relapsed/refractory population, and there is ongoing investigation into the utilization of novel Bruton's tyrosine kinase, B-cell leukemia/lymphoma-2, and spleen tyrosine kinase inhibitors alone or in combination in both the front-line and relapsed settings. Other areas of research in novel immunotherapies include investigations of bispecific T-cell engagers and antibody-drug conjugates. I provide a critique of Progressive Education from the Critical Dialogue Education and Democratic Education perspectives.In this report, we describe the fluorescent labeling of bacterial polysaccharides (Escherichia coli O86B7, Escherichia coli O19ab, Pseudomonas aeruginosa O10a10b, and Shigella flexneri 2b) at the "natural" amino group of their phosphoethanolamine moiety. Two protocols for labeling are compared 1) on a scale of a few mg of the polysaccharide, with a dialysis procedure for purification from excessive reagents; and 2) on a scale of 0.1 mg of the polysaccharide, with a simple precipitation procedure instead of dialysis. The microscale version is sufficient for comfortable cytofluorometric analysis. The resulting probes were found to specifically bind to human dendritic cells in a dose-dependent manner. The used limited set of polysaccharides did not allow us even to get close to understanding which dendritic cell-associated lectins and which cognate polysaccharide epitopes are involved in recognition, but the proposed microscale protocol allows to generate a library of fluorescent probes for further mapping of the polysaccharide specificity of the dendritic cells.

The effect of flubendiamide and thiocyclam hydrogen oxalate insecticides was assessed on cellular energy allocation (CEA) of the third instar larvae of T. absoluta ingesting six different tomato cultivars. Plant leaves were treated with LC50 concentration of both insecticides and their energy available (Ea), energy consumption (Ec), and CEA were calculated. The results showed that total energy reserves (protein, carbohydrate, and lipid budgets) were significantly reduced after exposure to insecticides and tomato secondary metabolites. The larvae fed on Riogrande and Super Chief cultivars showed the least amount of energy available after treatment with both insecticides. The highest rate of oxygen consumption was observed in larvae fed on Calj, Super Luna, and Super strain B after treatment with flubendiamide. The amount of CEA decreased in treated larvae compared to untreated larvae. This reduction was statistically higher in treated larvae that fed on Riogrande and Super strain B cultivars. Reduction in CEA is probably due to the expenses of dealing with detoxification of insecticides and plant metabolites. I provide a critique of Progressive Education from the Critical Dialogue Education and Democratic Education perspectives.In this report, we describe the fluorescent labeling of bacterial polysaccharides (Escherichia coli O86B7, Escherichia coli O19ab, Pseudomonas aeruginosa O10a10b, and Shigella flexneri 2b) at the "natural" amino group of their phosphoethanolamine moiety. Two protocols for labeling are compared 1) on a scale of a few mg of the polysaccharide, with a dialysis procedure for purification from excessive reagents; and 2) on a scale of 0.1 mg of the polysaccharide, with a simple precipitation procedure instead of dialysis. The microscale version is sufficient for comfortable cytofluorometric analysis. The resulting probes were found to specifically bind to human dendritic cells in a dose-dependent manner. The used limited set of polysaccharides did not allow us even to get close to understanding which dendritic cell-associated lectins and which cognate polysaccharide epitopes are involved in recognition, but the proposed microscale protocol allows to generate a library of fluorescent probes for further mapping of the polysaccharide specificity of the dendritic cells.

    This study aimed to investigate the attitudes and awareness about the development of retinopathy among the patients suffering from diabetes in Amman, Jordan.

    A questionnaire was distributed among patients with diabetes visiting internal medicine and endocrinology clinics in a tertiary hospital, in addition to a primary health care center in Amman. The questionnaire inquired about retinopathy awareness, history of screening for retinopathy, and awareness about retinopathy treatment. The patients were also asked about risk factors and preventive measures for diabetic retinopathy management.

    A total of 303 patients were recruited in the study. Majority of the patients (77%) had their HbA1c levels tested at least once since diagnosis, and most of them (60%) do it regularly. However, 86% of patients reported an A1c >6.5%. Two thirds of patients are not aware about retinopathy being a recognized complication of diabetes. Lower level of education, female sex, younger age, and shorter duration of diabetes mellitus were associated with less awareness.

    The study concluded that majority of patients with diabetes were aware of diabetes management. However, the sample population indicated less awareness of diabetic retinopathy. Hence, more educational efforts are needed to further boost patients' awareness about diabetic retinopathy.

    To investigate the clinicopathological characteristics of stage I-III colorectal cancer (CRC) patients with deficient mismatch repair (dMMR) protein.

    A retrospective analysis of 61 patients with stage I-III CRC confirmed by immunohistochemistry as dMMR after radical resection at Shenjing Hospital of China Medical University from May 2017 to June 2019 was performed. A total of 183 stage I-III CRC patients with proficient mismatch repair (pMMR) protein from the same period were randomly selected as a control group.     Two radiologists identified the most suspicious ALN or the largest ALN in negative axilla by T2W imaging features, including short axis (Size-S), long axis (Size-L)/S ratio, fatty hilum, margin, and signal intensity on T2W imaging. The IVIM parameters of these selected ALNs were also obtained. The Mann-Whitney U test or t-test was used to compare the metastatic and non-metastatic ALN groups. Finally, logistic regression analysis with T2W imaging and IVIM features for predicting ALNM was conducted.

    Energy spectrum computed tomography (CT) has become a promising approach for the differential diagnosis of tumor subtypes.

    To explore the value of energy spectrum CT parameters in the differential diagnosis of high-grade clear cell renal cell carcinoma (ccRCC) and type II papillary renal cell carcinoma (pRCC).

    Forty-two cases of high-grade ccRCC and 28 cases of type II pRCC were retrospectively reviewed. All region of interest (ROI) measurements were maintained consistently between the two-phase contrast-enhanced examinations. The ROIs encompassed as **** of the enhancing areas of the lesions as possible. Energy spectrum CT parameters of all cases, including the 70 keV (HU) value, normalized iodine concentration (NIC), and energy spectrum curve slope were recorded by two radiologists with over 10 years of experience in abdominal CT diagnosis.

    In the cortical phase (CP) and parenchymal phase (PP), the 70 keV (HU) value, NIC, and slope value of the energy spectrum curve of high-grade ccRCC were significantly higher than those of type II pRCC. In the CP, NIC showed the highest differential diagnosis efficiency for the two group tumors, with a sensitivity of 78.9% and a specificity of 77.0%. There was no statistical difference in tumor hemorrhage, tumor envelope, tumor morphology, tumor border, lymph node metastasis, embolism, renal pelvis invasion, or tumor calcification between the two tumor types.     To determine the feasibility, effectiveness, and safety of ultrasound-guided transperineal laser ablation (US-TPLA) for the treatment of BPH.

    Twenty patients with BPH (mean age = 73.9 ± 9.2 years) who underwent US-TPLA from June 2018 to January 2020 with a subsequent six-month follow-up were retrospectively reviewed. After local anesthesia, a 21-G trocar was inserted into the prostate tissue under ultrasound monitoring, followed by 1064 nm diode laser irradiation. Changes in international prostate symptom score (IPSS), quality of life (QoL), maximum urinary flow rate (Qmax), postvoid residual (PVR), prostate volume, and complications were evaluated six months after surgery.

Energy spectrum CT parameters of all cases, including the 70 keV (HU) value, normalized iodine concentration (NIC), and energy spectrum curve slope were recorded by two radiologists with over 10 years of experience in abdominal CT diagnosis. In the cortical phase (CP) and parenchymal phase (PP), the 70 keV (HU) value, NIC, and slope value of the energy spectrum curve of high-grade ccRCC were significantly higher than those of type II pRCC. In the CP, NIC showed the highest differential diagnosis efficiency for the two group tumors, with a sensitivity of 78.9% and a specificity of 77.0%. There was no statistical difference in tumor hemorrhage, tumor envelope, tumor morphology, tumor border, lymph node metastasis, embolism, renal pelvis invasion, or tumor calcification between the two tumor types. We are currently exploring a new minimally invasive interventional therapy for benign prostatic hyperplasia (BPH). To determine the feasibility, effectiveness, and safety of ultrasound-guided transperineal laser ablation (US-TPLA) for the treatment of BPH. Twenty patients with BPH (mean age = 73.9 ± 9.2 years) who underwent US-TPLA from June 2018 to January 2020 with a subsequent six-month follow-up were retrospectively reviewed. After local anesthesia, a 21-G trocar was inserted into the prostate tissue under ultrasound monitoring, followed by 1064 nm diode laser irradiation. Changes in international prostate symptom score (IPSS), quality of life (QoL), maximum urinary flow rate (Qmax), postvoid residual (PVR), prostate volume, and complications were evaluated six months after surgery. All patients underwent the operation successfully without serious complications. After six months, the average IPSS improved from 22.

    We developed a 2-week "Transitions of Care Curriculum" to train first-year internal medicine residents to improve their knowledge and skills to deliver optimal transitional care. Our objective was to use reflective writing essays to evaluate the impact of the curriculum on the residents.

    The rotation included Transition of Care Teaching modules, Transition Audit, Transitional Care Site Visits, and Transition of Care Conference. Residents performed the above elements of care transitions during the curriculum and wrote reflective essays about their experiences. These essays were analyzed to assess for the overall impact of the curriculum on the residents.Qualitative analysis of reflective essays was used to evaluate the impact of the curriculum. Of the 20 residents who completed the rotation, 18 reflective essays were available for qualitative analysis.

    Previous reports show conflicting results regarding hepatitis B virus (HBV) vaccine efficacy in Hepatitis C virus (HCV) infected individuals and in those with isolated hepatitis B core antibodies (HBcAb). We aimed to evaluate the effectiveness of HBV vaccine and identify possible factors that may contribute to hyporesponsivness in HCV-treated patients, including those with isolated HBcAb.

    We conducted a prospective study with 118 enrolled chronic HCV patients who followed a 12-week regimen of direct acting antivirals (DAAs) and were evaluated for HBV serological markers. Eventually, 98 received appropriate HBV vaccination and were assessed for response.

    A total of 57.1% were vaccine responders although only 5.1% achieved a seroprotective level of HBsAb titre. The response rate was significantly lower among treated HCV patients with isolated HBcAb [2 (5.6%) vs. 40 (64.5%) respectively]. On multivariate analysis, advanced age [OR (95% CI)=1.09 (1.02-1.17)] and presence of isolated HbcAb [OR (95% CI)=39.59 (7.98-196.63)] were predictors of vaccine non-response. In our cost-effectiveness models, the cost of HBV serological screening was less than the nationally adopted non-screening approach. A model ratifying reinforced vaccination in non-responder HBcAb seropositive HCV patients would incur extra cost.

The whole picture of the multi-step reactions has been revealed experimentally with theoretical supports.   • Anaerobic digestion (AD) systems with high substrate concentrations are characterized by high viscosity, which affects material and energy transfer efficiencies, thereby influencing methane production efficiency. In this study, adding granular activated carbon (GAC) and increasing the temperature decreased the viscosity by 4.56-10.19% and 27.13-28.85%, respectively, and improved AD efficiency. Adding GAC and increasing the temperature enhanced the methane yields by 34.37-38.15% and 25.60-28.31%, respectively. Distance-based redundancy analysis showed that the viscosity, temperature, and GAC had the greatest effects on the composition of the microbial community. The dominant bacteria in the medium-temperature AD system at the phylum level belonged to Firmicutes, Bacteroidetes, and Euryarchaeota. In addition to the dominant bacteria in the medium-temperature AD system, the thermophilic phylum Thermotogae was abundant in the high-temperature AD system. Moreover, the relative abundance of Euryarchaeota, which contained most of the methanogens, was higher in the high-temperature AD system than in the medium-temperature AD system.In this study, the seed endosphere of a bacterial wilt tolerant chilli cv. Firingi Jolokia was explored in order to find effective agents for bacterial wilt disease biocontrol. A total of 32 endophytic bacteria were isolated from freshly collected seeds and six isolates were selected based on R. solanacearum inhibition assay. In the in planta R. solanacearum inhibition assay carried out by seedling root bacterization method, Bacillus subtilis (KJ-2) exhibited highest biocontrol efficacy of 86.6 % on 7th day post R. solanacearum inoculation and a minimum biocontrol efficacy of 52.9 % was noted for Leuconostoc mesenteroides (KP-1). GC-HRMS analysis detected several known antimicrobial compounds in the extract of the culture supernatant of Bacillus subtilis (KJ-2); which may contribute to inhibition of R. solanacearum. In the growth promotion assay conducted using these isolates, only two of them namely Bacillus subtilis (KJ-2) and Bacillus amyloliquefaciens (WK-2) showed growth promotion in true leafed tomato plants. All the selected seed endophytic isolates were able to control bacterial wilt of tomato at the seedling stage and Bacillus subtilis (KJ-2) was found to be most effective in controlling the disease. The application of biocontrol agents can be a possible solution, but requires deep understanding of the molecular mechanisms in order to be effective. In this study, we investigated the multitrophic interactions between the biocontrol agent Bacillus subtilis MBI 600, a new commercialized biopesticide, the pathogen B. cinerea and their plant host. Our analysis showed that this biocontrol agent reduced B. cinerea mycelial growth in vitro, and was able to suppress the disease incidence on cucumber plants. Moreover, treatment with B. subtilis led to induction of genes involved in plant immunity. RNA-seq analysis of B. cinerea transcriptome upon exposure to bacterial secretome, showed that genes coding for MFS and ABC transporters were highly induced. Deletion of the Bcmfs1 MFS transporter gene, using a CRISP/Cas9 editing method, affected its virulence and the tolerance of B. cinerea to bacterial secondary metabolites. These findings suggest that specific detoxification transporters are involved in these interactions, with crucial role in different aspects of B. cinerea physiology.Obstructive sleep apnea (OSA) is an independent risk factor for hypertension and cardiovascular disease. Studies comparing patients with and without OSA, or examining the association of OSA severity with changes in autonomic function were included. Seventy-one studies met the inclusion criteria and autonomic function has been assessed using a range of techniques. The profile of autonomic function found in OSA include increased sympathetic activity, reduced parasympathetic activity and less consistently found low heart rate variability. Altered autonomic function in OSA may explain the pathophysiology of increased cardiovascular risk. Evidence from intervention studies is required to determine if treatment improves autonomic function associated with OSA.Trichloropropyl phosphate (TCPP) as a widely used typical chlorinated organophosphate flame retardant has received significant attention because of its widespread presence in water and negative effects on human health.
