    Green tea, traditionally used as antidiabetic medicine, positively affects the diabetic nephropathy. It was assumed that these beneficial effects were due to the hypoglycemiant capacity of the tea, wich reduces the glycemic overload and, consequently, the advanced glycation end products rate and oxidative damage. However, these results are still controversial, since tea is not always able to exert a hypoglycemic action, as demonstrated by previous studies.

    We treated streptozotocin type 1 diabetic young rats with 100mg/kg of green tea, daily, for 42 days, and evaluated the serum and tissue markers for stress and function. We also analyzed the ion dynamics in the organ and the morphological alterations promoted by diabetes and green tea treatment. Besides, we analyzed, by an in silico approach, the interactions of the green tea main catechins with the proteins expressed in the kidney.

    Our findings reveal that the components of green tea can interact with the proteins participating in cell signaling pathways that regulate energy metabolism, including glucose and glycogen synthesis, glucose reabsorption, hypoxia management, and cell death by apoptosis. Such interaction reduces glycogen accumulation in the organ, and protects the DNA. These results also reflect in a preserved glomerulus morphology, with improvement in pathological features, and suggesting a prevention of kidney function impairment.

  • Accumulating, converging evidence indicates that the anterior temporal lobe (ATL) appears to be the transmodal hub for semantic representation. A series of repetitive transcranial magnetic stimulation (rTMS) investigations utilizing the 'virtual lesion' approach have established the brain-behavioural relationship between the ATL and semantic processing by demonstrating that inhibitory rTMS over the ATL induced impairments in semantic performance in healthy individuals. However, a growing body of rTMS studies suggest that rTMS might also be a tool for cognitive enhancement and rehabilitation, though there has been no previous exploration in semantic cognition. Finally, the ON ASSR proved sensitive to musicality, demonstrating positive correlations between musical sophistication and ASSR power, as well as with the degree of attentional ASSR modulation at the left and right parietal cortices. We evaluate MB factors of 2, 3, 4, 6, 8, 9, and 12 with 2 mm isotropic voxels, and additionally 2 mm and 3.3 mm single-band acquisitions, on a 32-channel head coil. Noise amplification was greater in deeper brain regions, including subcortical regions. Correlations were attenuated by noise amplification, which resulted in spatially varying biases that were more severe at higher MB factors. Temporal filtering decreased spatial biases in correlations due to noise amplification, but also tended to decrease effect sizes. In seed-based correlation maps, left-right putamen connectivity and thalamo-motor connectivity were highest in the single-band 3.3 mm protocol. In correlation matrices, MB 4, 6, and 8 had a greater number of significant correlations than the other acquisitions (both with and without temporal filtering). We recommend single-band 3.3 mm for seed-based subcortical analyses, and MB 4 provides a reasonable balance for studies analyzing both seed-based correlation maps and connectivity matrices. In multiband studies including secondary analyses of large-scale datasets, we recommend reporting effect sizes or test statistics instead of correlations. If correlations are reported, temporal filtering (or another method for thermal noise removal) should be used. Measuring CTS with OPMs is a priori challenging given the complications associated with OPM measurements at frequencies below 4 Hz, due to increased intrinsic interference and head movement artifacts. Still, this represents an important development as OPM-MEG provides lifespan compliance and substantially improved spatial resolution compared with classical MEG. In this study, four healthy right-handed adults listened to continuous speech for 9 min. The radial component of the magnetic field was recorded simultaneously with 45-46 OPMs evenly covering the scalp surface and fixed to an additively manufactured helmet which fitted all 4 participants. We estimated CTS with reconsteural network involved in CTS across the lifespan and potential alterations in, e.g., language developmental disorders. Data also suggest that OPMs are generally suitable for recording neural activity at frequencies below 4 Hz provided PCA is used as a preprocessing step; 0.2-1.5-Hz being the lowest frequency range successfully investigated here.
    Accumulating, converging evidence indicates that the anterior temporal lobe (ATL) appears to be the transmodal hub for semantic representation. A series of repetitive transcranial magnetic stimulation (rTMS) investigations utilizing the 'virtual lesion' approach have established the brain-behavioural relationship between the ATL and semantic processing by demonstrating that inhibitory rTMS over the ATL induced impairments in semantic performance in healthy individuals. However, a growing body of rTMS studies suggest that rTMS might also be a tool for cognitive enhancement and rehabilitation, though there has been no previous exploration in semantic cognition. Here, we explored a potential role of rTMS in enhancing and inhibiting semantic performance with contrastive rTMS protocols (1 Hz vs. https://www.selleckchem.com/products/pf-04620110.html 20 Hz) by controlling practice effects. Twenty-one healthy participants were recruited and performed an object category judgement task and a pattern matching task serving as a control task before and after the stimulation over the ATL (1 Hz, 20 Hz, and sham).   • A circular economy system aims to eliminate waste and pollution, keep products and materials in use, and regenerate natural systems. Although these principles are not yet met, some approaches are being made in this direction; the transformation of crude glycerol by actinobacteria is a process with great potential to be scaled on an industrial level. This review discusses the reports on glycerol as a promising source of carbon and energy for obtaining biomass and high-added value products by actinobacteria. Also, the factors influencing the biomass and secondary metabolites production in bioreactors are analyzed, and the tools available to overcome those that generate the main problems are discussed.Recently, there has been an urgent need to develop new materials and technologies for extracting lithium ions. Herein, the membrane-type adsorbent of manganese dioxide (MnO2) is prepared by a vacuum filtration method using graphene oxide (GO) as a binder and amino-β-cyclodextrin (amino-β-CD) as an adjuvant. The results of thermogravimetric analysis show that MnO2 is successfully immobilized on GO layers with a content of about 24 wt%, which enabled rapid adsorb lithium ions from the ionic solution. In addition, the permeation experiment shows the membrane has specific selectivity for lithium ion transport and adsorption, which is manifested in the selectivity ratios of K+/Li+, Na+/Li+ and K+/Na+ to 2.5, 3.2 and 0.8, respectively. Adsorption experiments show that GO-β-CD/MnO2 membrane has a high adsorption capacity for lithium ions (37.5 mg g-1). The adsorption kinetic curve indicates that the lithium adsorption process is controlled by the chemical adsorption mechanism. In the enrichment experiment, the concentration of lithium ions from seawater can be enriched to 1.2 mg L-1 after 100 cycles. The results suggest that the developed GO-β-CD/MnO2 membrane could effectively extract lithium ions from seawater.Oxygen vacancy engineering has emerged as an effective approach to improve the performance of catalysts for peroxymonosulfate (PMS) activation. Herein, we report a facile precipitation method followed by calcination to synthesize cost-effective and environmentally friendly magnesium-doped hematite (Mg/Fe2O3) composites. Multiple characterization results reveal that the incorporation of Mg can significantly increase the oxygen vacancies and specific surface area of 5%Mg/Fe2O3, leading to a significantly enhanced performance in degrading Rhodamine B (RhB) through PMS activation. In a typical reaction, almost complete RhB (10 mg/L) removal can be achieved by the activation of PMS (0.2 g/L) using 5%Mg/Fe2O3 (0.5 g/L). Moreover, the as-synthesized catalyst exhibits a broad pH working range (3.96-10.69), high stability, and recyclability. In the sedimentation step, waste oyster shell(WOS) and a PE tube diffuser(PE250) containing Na2S (PECa/S), PECa/S were prepared, which were used to precipitate heavy metals with a removal efficiency of 97% or more. In the adsorption step, fluorine (F), arsenic (As), and thorium (Th) were precipitated and removed when heavy metals were removed using coal mine drainage sludge (CMDS) and an adsorbent (PUCMDS) made of polyurethane (PU). Running a semi and pilot scale continuous process using PECa/S, PUCMDS and O3/HC systems resulted in a semi and pilot scale operating period of 120 and 62 days, and 60.26, 797.84, 46.94, 78.62 g, and 7.120 kg and 266.35, 42556.8, 191.95, 3108.43 g and 629.84 kg for As, F, Th, Pb and CODcr has been removed respectively. In addition, the removal efficiencies of As, F, Th, Pb and CODcr were 99.75, 99.98, 93.60, 99.99, and 88.82%, respectively, when treating real RE wastewater using the pilot scale system. Without the use of agitated reactors and regulators, the new concept of continuous process can effectively treat RE real wastewater, and the quality of the process outlet has met the pollutant limits recommended by EPA and China for irrigation.Host markers to monitor the response to tuberculosis (TB) therapy hold some promise. We evaluated the changes in concentration of Mycobacterium tuberculosis (M.tb)-induced soluble biomarkers during early treatment for predicting short- and long-term treatment outcomes. Whole blood samples from 30 cured and 12 relapsed TB patients from diagnosis, week 1, 2, and 4 of treatment were cultured in the presence of live M.tb for seven days and patients followed up for 24 weeks after the end of treatment. 57 markers were measured in unstimulated and antigen-stimulated culture supernatants using Luminex assays. Top performing multi-variable models at diagnosis using unstimulated values predicted outcome at 24 months after treatment completion with a sensitivity of 75.0% (95% CI, 42.8-94.5%) and specificity of 72.4% (95% CI, 52.8-87.3%) in leave-one-out cross validation. Month two treatment responder classification was correctly predicted with a sensitivity of 79.2% (95% CI, 57.8-92.9%) and specificity of 92.3% (95% CI, 64.0-99.8%). This study provides evidence of the early M.tb-specific treatment response in TB patients but shows that the observed unstimulated marker models are not outperformed by stimulated marker models. Performance of unstimulated predictive host marker signatures is promising and requires validation in larger studies.

The urothelium has a very slow turn-over under normal conditions but is capable of extremely fast response to injury. During regeneration urothelium either restores normal function or undergoes altered differentiation pathways, the latter being the cause of several bladder diseases. In this review, we describe the structure of the apical plasma membrane that enables barrier function, the role of urothelium specific proteins uroplakins and the machinery for polarized membrane transports in terminally differentiated superficial umbrella cells. We address key markers, such as keratins, cancer stem cell markers, retinoic acid signalling pathway proteins and transient receptor potential channels and purinergic receptors that drive normal and altered differentiation in bladder cancer and bladder pain syndrome. Finally, we discuss uncertainties regarding research, diagnosis and treatment of bladder pain syndrome. In this study, the distribution and density of key molecular factors involved in RNA pathways namely, nuclear actin (a motor protein and regulator of RNA transcription), 5-methyl cytosine (an epigenetic regulator of gene transcription), and ribonuclease A (an RNA degrading enzyme) were compared in different nuclear compartments of late adult and old **** myonuclei by means of ultrastructural immunocytochemistry.     Összefoglaló. Bevezetés A családorvosok testi, lelki egészségi állapota hatással van a munkavégzésükre, a betegellátás minÅ‘ségére, ezáltal a társadalom egészségmutatóira is. Az életmód pedig az egyik legjelentÅ‘sebb, egészségi állapotot befolyásoló tényezÅ‘. Célkitűzés A vizsgálat célja a magyar háziorvosok egészségi állapotának és az azt befolyásoló életmódtényezÅ‘knek a felmérése. Módszer Keresztmetszeti vizsgálat. Kvantitatív, papíralapú felmérés családorvosok körében (n = 569, életkor 54 ± 10 év, nÅ‘k 42%). Eredmények A háziorvosok 61%-a túlsúlyos vagy elhízott, 88%-ának a vércukorszintje ≤5,5 mmol/l. The urothelium has a very slow turn-over under normal conditions but is capable of extremely fast response to injury. During regeneration urothelium either restores normal function or undergoes altered differentiation pathways, the latter being the cause of several bladder diseases. In this review, we describe the structure of the apical plasma membrane that enables barrier function, the role of urothelium specific proteins uroplakins and the machinery for polarized membrane transports in terminally differentiated superficial umbrella cells. We address key markers, such as keratins, cancer stem cell markers, retinoic acid signalling pathway proteins and transient receptor potential channels and purinergic receptors that drive normal and altered differentiation in bladder cancer and bladder pain syndrome. Finally, we discuss uncertainties regarding research, diagnosis and treatment of bladder pain syndrome. In this study, the distribution and density of key molecular factors involved in RNA pathways namely, nuclear actin (a motor protein and regulator of RNA transcription), 5-methyl cytosine (an epigenetic regulator of gene transcription), and ribonuclease A (an RNA degrading enzyme) were compared in different nuclear compartments of late adult and old mice myonuclei by means of ultrastructural immunocytochemistry.   • Transcranial direct current stimulation (DCS) has lasting effects that may be explained by a boost in synaptic long-term potentiation (LTP). We hypothesized that this boost is the result of a modulation of somatic spiking in the postsynaptic neuron, as opposed to indirect network effects. To test this directly we record somatic spiking in a postsynaptic neuron during LTP induction with concurrent DCS.

    We performed rodent in-vitro patch-clamp recordings at the soma of individual CA1 pyramidal neurons. LTP was induced with theta-burst stimulation (TBS) applied concurrently with DCS. To test the causal role of somatic polarization, we manipulated polarization via current injections. We also used a computational multi-compartment neuron model that captures the effect of electric fields on membrane polarization and activity-dependent synaptic plasticity.

    TBS-induced LTP was enhanced when paired with anodal DCS as well as depolarizing current injections. In both cases, somatic spiking during the TBS was increased, suggesting that evoked somatic activity is the primary factor affecting LTP modulation. However, the boost of LTP with DCS was less than expected given the increase in spiking activity alone. In some cells, we also observed DCS-induced spiking, suggesting DCS also modulates LTP via induced network activity. The computational model reproduces these results and suggests that they are driven by both direct changes in postsynaptic spiking and indirect changes due to network activity.

Over the last 40 years there has been a progressive increase in both the complexity and the translatability (to humans) of the cognitive tasks investigated. This review outlines technical and conceptual advances in space radiation rodent testing approaches, along with the advances in analytical approaches, that will make data from ground based studies more amenable to probabilistic risk analysis. While great progress has been made in determining the impact of space radiation on many advanced cognitive processes, challenges remain that need to be addressed prior to commencing deep space missions. A summary of on-going attempts to address existing knowledge gaps and the critical role that rodent studies will have in establishing the impact of space radiation on even more complex (human) cognitive tasks are presented and discussed.
    Persistent tic disorders (PTDs) and attention-deficit hyperactivity disorder (ADHD) are common neurodevelopmental conditions which tend to co-occur. Both diagnoses are associated with sleep problems. This systematic review and meta-analysis investigates overlaps and distinctions in objective sleep parameters based on diagnosis (PTD-only, PTD + ADHD, and ADHD-only).

    Databases were searched to identify studies with objective sleep measures in each population. Meta-analyses were conducted using a random effects model.

    Polysomnography was the only measure included in all three groups. Twenty studies met final inclusion criteria, combining PTD-only (N = 108), PTD + ADHD (N = 79), and ADHD-only (N = 316). Compared to controls (N = 336), PTD-only and PTD + ADHD groups had significantly lower sleep efficiency and higher sleep onset latency. PTD + ADHD also had significantly increased time in bed and total sleep time. No significant differences were observed between ADHD-only groups and controls.

The ANT has not only been very widely adopted by scholars around the world, it has inspired a wide range of variants, the ANTs, - each with its own purpose. This review will describe the origin of the ANT in the taxonomic contributions of Posner and the evolution of the ANTs with some discussion of the nature and rationale for each major variant described here. We briefly allude to minor modifications of the ANT and present some suggestions related to data reporting and data analysis. Sensory organs and the central nervous system (CNS) are forced to adapt to new environmental conditions of weightlessness. In comparison with peripheral vestibular organs and behavioural disturbances in weightlessness conditions, the CNS vestibular centres of vertebrates, including the cerebellum, have been poorly examined in orbital experiments, as well as in experimental micro- and hypergravity. However, the cerebellum serves as a critical control centre for learning and sensory system integration during space-flight. Thus, it is referred to as a principal brain structure for adaptation to gravity and the entire sensorimotor adaptation and learning during weightlessness. This paper is focused on the prolonged spaceflight effects on the vestibular cerebellum evidenced from animal models used in the Bion-M1 project. The changes in the peripheral vestibular apparatus and brainstem primary vestibular centres with appropriate behavioural disorders after altered gravity exposure are briefly reviewed.

The swelling behaviors of highly charged polyelectrolyte gels can be well explained using the Flory-Rehner model combined with the Gibbs-Donnan effect and Manning's counterion condensation effect (the FRGDM model). This study investigated the swelling properties of a series of model polyelectrolyte gels, namely tetra-polyacrylic acid-polyethylene glycol gels (Tetra-PAA-PEG gels), and determined the applicability of the FRGDM model. The swelling ratio (Vs/V0) was well reproduced by the FRGDM model in the moderate swelling regime (Vs/V0 10), the FRGDM model is approx. 1.6 times larger than the experimental results. When we introduced the finite extensibility to the elastic free energy in the FRGDM model, the swelling behavior was successfully reproduced even in the high swelling regime. Our results reveal that finite extensibility is one of the factors determining the swelling equilibrium of highly charged polyelectrolyte gels. The physico-chemical textural and surface properties of the monoliths were investigated. The homogeneity of the textural properties along the entire length of the monoliths was assessed, as well as the reproducibility of the synthesis method. The adsorption properties of the monoliths for gaseous toluene, as a model of Volatile Organic Compounds (VOCs), were evaluated and compared to those of a reference meso-structured silica powder (MCM-41) of commercial origin. Silica monoliths adsorbed comparable amounts of toluene with respect to MCM-41, with better performances at low pressure. Finally, considering their potential application in water phase, the adsorption properties of monoliths toward Rhodamine B, selected as a model molecule of water soluble pollutants, were studied together with their stability in water. After 24 h of contact, the silica monoliths were able to adsorb up to the 70% of 1.5 × 10-2 mM Rhodamine B in water solution.Blueberries (Vaccinium spp.) are highly vulnerable to changing climatic conditions, especially increasing temperatures. RNAseq analysis yielded ~135 million reads and identified 8305 differentially expressed genes (DEGs) during heat stress against the control samples. In V. corymbosum, 2861 and 4565 genes were differentially expressed at 6 and 9 h of heat stress, whereas in V. darrowii, 2516 and 3072 DEGs were differentially expressed at 6 and 9 h, respectively. Among the pathways, the protein processing in the endoplasmic reticulum (ER) was the highly enriched pathway in both the species however, certain metabolic, fatty acid, photosynthesis-related, peroxisomal, and circadian rhythm pathways were enriched differently among the species. KEGG enrichment analysis of the DEGs revealed important biosynthesis and metabolic pathways crucial in response to heat stress. The GO terms enriched in both the species under heat stress were similar, but more DEGs were enriched for GO terms in V. darrowii than the V. corymbosum. This study investigated the swelling properties of a series of model polyelectrolyte gels, namely tetra-polyacrylic acid-polyethylene glycol gels (Tetra-PAA-PEG gels), and determined the applicability of the FRGDM model. The swelling ratio (Vs/V0) was well reproduced by the FRGDM model in the moderate swelling regime (Vs/V0 10), the FRGDM model is approx. 1.6 times larger than the experimental results. When we introduced the finite extensibility to the elastic free energy in the FRGDM model, the swelling behavior was successfully reproduced even in the high swelling regime. Our results reveal that finite extensibility is one of the factors determining the swelling equilibrium of highly charged polyelectrolyte gels. The modified FRGDM model reproduces well the swelling behavior of a wide range of polyelectrolyte gels.The impressive estimated number of chronic kidney disease (CKD) patients in the world justifies any possible effort at implementing preventive measures of disease progression. Renal insufficiency is associated with significant changes in the electrolyte handling and body balance of sodium, potassium, phosphate, magnesium, and calcium, all of which are biologically vital molecules. Dietary habits could contribute significantly to the optimal management of possible derangements. In this review, we examined the available evidence recommending dietary prescriptions for these five elements aiming at reducing CKD progression. Clear evidence that specific dietary prescriptions may halt or reduce CKD progression is lacking. However, some practical recommendations are possible to prescribe the best possible therapy to the individual CKD patient.This study focused on the application of mesoporous silica monoliths for the removal of organic pollutants. The physico-chemical textural and surface properties of the monoliths were investigated. The homogeneity of the textural properties along the entire length of the monoliths was assessed, as well as the reproducibility of the synthesis method. The adsorption properties of the monoliths for gaseous toluene, as a model of Volatile Organic Compounds (VOCs), were evaluated and compared to those of a reference meso-structured silica powder (MCM-41) of commercial origin. Silica monoliths adsorbed comparable amounts of toluene with respect to MCM-41, with better performances at low pressure. Finally, considering their potential application in water phase, the adsorption properties of monoliths toward Rhodamine B, selected as a model molecule of water soluble pollutants, were studied together with their stability in water. After 24 h of contact, the silica monoliths were able to adsorb up to the 70% of 1.5 × 10-2 mM Rhodamine B in water solution.Blueberries (Vaccinium spp.) are highly vulnerable to changing climatic conditions, especially increasing temperatures. To gain insight into mechanisms underpinning the response to heat stress, two blueberry species were subjected to heat stress for 6 and 9 h at 45 °C, and leaf samples were used to study the morpho-physiological and transcriptomic changes. As compared with Vaccinium corymbosum, Vaccinium darrowii exhibited thermal stress adaptation features such as small leaf size, parallel leaf orientation, waxy leaf coating, increased stomatal surface area, and stomatal closure. RNAseq analysis yielded ~135 million reads and identified 8305 differentially expressed genes (DEGs) during heat stress against the control samples. In V. corymbosum, 2861 and 4565 genes were differentially expressed at 6 and 9 h of heat stress, whereas in V. darrowii, 2516 and 3072 DEGs were differentially expressed at 6 and 9 h, respectively. Among the pathways, the protein processing in the endoplasmic reticulum (ER) was the highly enriched pathway in both the species however, certain metabolic, fatty acid, photosynthesis-related, peroxisomal, and circadian rhythm pathways were enriched differently among the species. KEGG enrichment analysis of the DEGs revealed important biosynthesis and metabolic pathways crucial in response to heat stress. The GO terms enriched in both the species under heat stress were similar, but more DEGs were enriched for GO terms in V. darrowii than the V. corymbosum. Together, these results elucidate the differential response of morpho-physiological and molecular mechanisms used by both the blueberry species under heat stress, and help in understanding the complex mechanisms involved in heat stress tolerance.

Methods We randomly assigned 65 students reporting trauma histories to time-matched narrating (identifying and sharing details of elicited positive memories), writing (identifying and writing details of elicited positive memories), or control conditions. Participants completed self-report measures (T0) and repeated their assigned task condition and self-report measures 6-8 days later (T1). Results Half-longitudinal models demonstrated direct associations of (1) being in the narrating versus other conditions with decreases in posttrauma maladaptive cognitions and negative affect, and increases in positive affect; and (2) increases in posttrauma maladaptive cognitions and negative affect with greater PTSD symptom severity. Although, when controlling for posttrauma maladaptive cognitions and negative/positive affect, being in the narrating versus other conditions was associated with decreases in PTSD symptom severity, these constructs did not explain examined relations. Conclusions Results suggest beneficial impacts of narrating positive memories on PTSD symptom severity (accounting for cognitions/affect) and improved cognitions/affect, and a need to examine moderating variables (e.g., emotion regulation) in the Positive Memory-PTSD model.     Childhood trauma is common and has implications for mental health. Research conducted retrospectively with clinical samples of adults and prospectively with high-risk samples of children has identified factors that moderate negative mental health sequelae (e.g., age, gender, nature and amount of trauma). Presently, however, there is a paucity of research examining mental health outcomes, and potential moderators of these outcomes, that may be associated with the experience of trauma among children in the general community.

    To address this knowledge gap, the present study analyzed data from youth aged 8-17 years in the Nathan Kline Institute-Rockland Sample, a publicly available repository of information collected from a nationally representative sample of participants across the life span. We report the frequency and nature of trauma in our sample; bivariate correlations between trauma, demographic variables, and mental health outcomes; and hierarchical regressions in which these outcomes were modeled as a function of multivariate predictors.

  • Patients with circumcaval ureter and right renal calculi are not often seen in the clinical practice and require special approach. Hereby we report a case of a 34-year-old male that was diagnosed with circumcaval ureter complicated by right kidney stones. Laparoscopic ureteroplasty and flexible ureteroscopic lithotripsy were implemented in the treatment of both of these entities. A combination of these approaches allowed us to perform the surgery effectively, efficiently and safely without having to do the second one.Comparatively to scrotal gangrene, isolated penile gangrene is very rare due to the rich blood supply of the organ. It is thought to be initiated by a traumatic or vascular insult to the penis. This condition requires parenteral antibiotic therapy and serial debridement of necrotic tissue. Split thickness skin graft is thought to be the best approach to cover penile skin loss. We share our experience on the presentation of an isolated penile gangrene in a 35-year-old male. In the light of this case, we review the predisposing factors and the management of this entity.We are reporting a very rare case in the current era of modern diagnostics of an incidentally detected Transitional Cell Carcinoma(TCC) renal pelvis, detected pathologically when Right laparoscopic radical nephrectomy specimen of lower pole mass(RCC) was processed, with no prior suspicion of TCC clinically or pre-operative imaging. A 75-year-old female with 4 × 4cm strongly enhancing lower pole renal mass on CTscan underwent a Laparoscopic Radical Nephrectomy. Surprisingly histopathological examination revealed a focus of low-grade TCC, arising from renal pelvis, completely separate from lower pole mass which was a clear cell RCC.Inguinal hernia is a common clinical presentation, with 1-3% containing some of the urinary bladder. Most cases are asymptomatic and diagnosed intraoperatively at hernia repair. We describe a case of an 84-year-old man presenting with acute urinary retention in a bladder had herniated entirely into his scrotum. He was managed conservatively with urinary catheterisation and discharged home. He re-presented with urosepsis secondary to bilateral ureteric obstruction. This was treated with antibiotics and nephrostomy and his hernia was ultimately repaired. Clinicians should consider both upper and lower urinary tract obstruction in patients with significant bladder herniation.An avulsion is very rare and the most serious complication that may occur during ureteroscopy in 0.06-0.45% of patients during the ureteral stone procedure. A sixty years-old female with a small stone in the upper third of the right ureter with high-grade hydronephrosis and underwent a ureteroscopy procedure. The patient underwent a robotic partial nephrectomy and partial adrenalectomy without complication.Self-insertion of needles into the urethra is rare among children, especially in adolescents with no evident history of mental illness. It is usually an urological emergency condition and needs surgery as soon as possible. The case we report here is a 14 year-old boy with urethral self-insertion of a sewing needle which was lodged in urethra for 9 years. The needle was successfully removed by urethrocystoscope with a surgical grasper. Our objective of this case report is to remind urologists and pediatrician of the possibility of urethral foreign bodies and recommend scrotal ultrasonography as the first examination modality.Salvage radiotherapy (SRT) is a widely used treatment option for patients demonstrating biochemical recurrence post radical prostatectomy. Urologists are familiar with common adverse effects of SRT, such as voiding and erectile dysfunction. In rare instances, more devastating complications can occur. We report a case of bladder neck necrosis following SRT resulting in urethrorectal fistula, fistulae extending into bilateral thighs, and infectious osteomyelitis (OM) of the pubis symphysis. Although rare, this case highlights the severity of complications that can occur following SRT, as well as the surgical management of such complications.
    Creatine transporter deficiency (CTD) is an X-linked form of intellectual disability (ID) caused by
    mutations. Limited information exists on the adult course of CTD, and there are no treatment studies in adults.

    We report two half-brothers with CTD, 36 and 31years at intervention start. Their clinical phenotypes were consistent with CTD, and intervention was indicated because of progressive disease course, with increased difficulties speaking, walking and eating, resulting in fatigue, and malnutrition. We therefore performed treatment trials with arginine, glycine and a proprietary product containing creatine and betaine, and then a trial supplementing with betaine alone.

    In the older patient, glycine and arginine were accompanied by adverse effects, while betaine containing proprietary product gave improved balance, speech and feeding. When supplementation stopped, his condition deteriorated, and improved again after starting betaine supplement. Betaine supplementation was also beneficial in the younger patient, reducing his exhaustion, feeding difficulties and weight loss, making him able to resume his protected work.

    The concern about massive haemorrhage associated with placenta accreta spectrum (PAS) prompts the routine use of general anaesthesia (GA) at many centres. We aimed to describe the effects of establishing a fixed multidisciplinary team (PAS team) on anaesthetic practices and clinical results.

    In this before-and-after study, we included patients with prenatal PAS suspicion treated between December 2011 and December 2019. We evaluated the anaesthetic techniques used before (Group 1) and after (Group 2) a PAS team was established.

    Eighty-one patients were included. Neuraxial anaesthesia (NA) was used in 23.3% of group 1 patients and 76.4% of group 2 patients. Likewise, the frequency of conversion to GA after initial management with NA decreased from 14.3% in group 1 to 7.7% in group 2.

    Respiratory depression is a rare but serious complication during opioid administration. Therefore, early detection of signs of deterioration is paramount. The current standard of care of using manual intermittent respiratory rate (RR) measurement is labour intensive and inefficient. We evaluated a wireless sensor monitor, Aingeal (Renew Health Ltd, Ireland), to continuously monitor RR, heart rate (HR) and temperature compared to standard clinical measurements.

    Patients who underwent major gynaecological operations and received postoperative opioid patient-controlled analgesia were recruited. Patients were connected to the sensor monitor via a central station software platform. The primary outcome was comparison of RR between sensor and nursing monitoring, with secondary outcomes being HR and temperature between two methods. Feedback from patients and healthcare providers was also collected. Bland-Altman analyses were used to compare the vital signs recorded in sensor against those in patient's electronic record.

    A total of 1121 hours of vital signs data were analysed. Bias for RR was -0.90 (95% confidence interval (CI) -9.39, 7.60) breaths/min between nursing and averaged sensor readings. Bias for heart rate was -1.12 (95% CI -26.27, 24.03) and bias for temperature was 1.45 (95% CI -5.67, 2.76) between the two methods.

    Myomectomy is associated with perioperative bleeding. The aim of the study is to evaluate the effect of intramyometrial vasopressin on blood loss and the associated cardiovascular complications during myomectomy.

    The study included 194 patients classified into two groups- 1) Vasopressin group the vasopressin was diluted as 0.1 unit/ml and 15 ml was injected by the surgeon in the plane between the myometrium and the myoma. 2) Control group The patients received an equal amount of normal saline. The monitored parameters included the amount of blood loss, required blood transfusion, heart rate, mean arterial blood pressure, the incidence of hypertension, hypotension, bradycardia, tachycardia, electrocardiogram (ECG) changes and the blood troponin I level.

In our study, we compared the ease of oral intubation with C-****video laryngoscope and direct laryngoscopy, when the aerosol box was used. The secondary objectives were to compare the incidence of airway loss, haemodynamic changes, number of attempts, and time required for intubation between these two techniques.

By comparing spectra of membrane proteins prepared using the different membrane mimetics, we found that SMA may favor selective solubilization of correctly folded proteins and better preserve native lipid interactions than other membrane mimetics.
