Methods In this population-based, panel data analysis, we used data from three waves of the nationally representative China Health and Retirement Longitudinal Study (CHARLS) for 2011, 2013, and 2015. We included participants aged 50 years and older in 2015, who had complete follow-up for the three waves. We used 11 physical non-communicable diseases to measure physical multimorbidity and annual per-capita household consumption spending as a proxy for socioeconomic status. Findings Of 17 708 particferent social health insurance programmes. Overall, physical multimorbidity was associated with a significantly increased likelihood of catastrophic health expenditure (for the overall population odds ratio 1·29, 95% CI 1·26-1·32, adjusted for sociodemographic variables). The effect of physical multimorbidity on catastrophic health expenditures persisted even among the higher socioeconomic groups and across all health insurance programmes. Interpretation Concerted efforts are needed to reduce health inequalities that are due to physical multimorbidity, and its adverse economic effect in population groups in China. Social health insurance reforms must place emphasis on reducing out-of-pocket spending for patients with multimorbidity to provide greater financial risk protection. Methods In our study done in October, 2018, we used a counterfactual analysis to assess the reduction in deaths and disability-adjusted life years (DALYs) lost if eight proven vehicle safety technologies were made more widely available in LAC countries. We estimated (1) country-level incidence of traffic injuries, (2) the effectiveness of technologies through a systematic literature review, (3) the prevalence of car safety technologies, and (4) the lives saved and DALYs averted if all cars had these technologies. We characterised uncertainty in to be prioritised in the SDG agenda for 2030. INTRODUCTION The relationships among PIK3CA mutations, medication use and tumor progression remains poorly understood. Aspirin use post-diagnosis may modify components of the PI3K pathway, including AKT and mTOR, and has been associated with lower risk of breast cancer recurrence and mortality. We assessed time to metastasis (TTM) and survival with respect to aspirin use and tumor PIK3CA mutations among women with metastatic breast cancer. METHODS Patients with hormone receptor positive, HER2 negative (HR+/HER2-) metastatic breast cancer treated in 2009-2016 who received tumor genotyping were included. https://www.selleckchem.com/Proteasome.html Aspirin use between primary and metastatic diagnosis was extracted from electronic medical records. TTM and survival were estimated using Cox proportional hazards regression. RESULTS Among 267 women with metastatic breast cancer, women with PIK3CA mutated tumors had longer TTM than women with PIK3CA wildtype tumors (7.1 vs. 4.7 years, p = 0.008). There was a significant interaction between PIK3CA mutations and aspirin use on TTM (p = 0.006) and survival (p = 0.026). PIK3CA mutations were associated with longer TTM among aspirin non-users (HR = 0.60 95% CI0.44-0.82 p = 0.001) but not among aspirin users (HR = 1.57 0.86-2.84 p = 0.139). Similarly, PIK3CA mutations were associated with reduced mortality among aspirin non-users (HR = 0.70 95% CI0.48-1.02 p = 0.066) but not among aspirin users (HR = 1.75 95% CI0.88-3.49 p = 0.110). CONCLUSIONS Among women who develop metastatic breast cancer, tumor PIK3CA mutations are associated with slower time to progression and mortality only among aspirin non-users. Larger studies are needed to confirm this finding and examine the relationship among aspirin use, tumor mutation profile, and the overall risk of breast cancer progression.BACKGROUND In Tanzania, there is no national trauma registry. The World Health Organization (WHO) has developed a data set for injury that specifies the variables necessary for documenting the burden of injury and patient-related clinical processes. As a first step in developing and implementing a national Trauma Registry, we determined how well hospitals currently capture the variables that are specified in the WHO injury set. METHODS This was a prospective, observational cross-sectional study of all trauma patients conducted in the Emergency Units of five regional referral hospitals in Tanzania from February 2018 to July 2018. Research assistants observed the provision of clinical care in the EU for all patients, and documented performed assessment, clinical interventions and final disposition. Research assistants used a purposefully designed case report form to audit the injury variable capture rate, and to review Ministry of Health (MoH) issued facility Register book recording the documentation of variabl.9%) were documented in the clinical charts. CONCLUSIONS In the regional hospitals of Tanzania, there is inadequate documentation of the minimum trauma variables specified in the WHO injury data set. The objective of this study is to introduce such a method based on an application of visualization frameworks to enable a systematic approach to answer the question, "What was visualized?" in studies of audit and feedback. METHODS The proposed method uses 3 steps to systematically identify and describe the content of visual displays in feedback interventions 1) identify displays, 2) classify content, and 3) identify elements. The use of a visualization framework led us to identify information content types as representations of measures (metrics or indicators), ascribees (feedback recipients and comparators), performance levels, and time intervals. We illustrate the proposed method in a series of 3 content analyses, one for each step, to identify visual displays and their information content in published example performance summaries. RESULTS We analyzed a convenience sample of 44 published studies of audit and feedback. Through each step, two coders had good agreement. We identified 42 visual displays of performance, containing 6 unique combinations of content types. What was visualized most commonly in the sample was performance levels across a recipient and comparators (i.e. ascribees) for a single measure and single time interval (n = 16). Content types varied in their inclusion of measures, ascribees, and time intervals. CONCLUSIONS The proposed method appears to be feasible to use as a systematic approach to describing visual displays of performance. The key implication of the method is that it offers more granular and consistent description for empirical, theoretical, and design studies about the information content of feedback interventions.BACKGROUND Heart rate variability (HRV) decreases in Parkinson's disease (PD) and it can be considered a marker for cardiovascular dysautonomia. The purpose of this pilot study is to evaluate long-term time-domain analysis of HRV of PD patients and compare the results with those of matched healthy individuals. METHODS Idiopathic PD patients without comorbidity impairing HRV, and age-matched healthy individuals were recruited in a pilot study. A long-term time domain analysis of HRV using 24-h ambulatory ECG was performed. RESULTS Overall, 18 PD patients fulfilling inclusion criteria completed the evaluation (mean age was 55.6 ± 8.8, disease duration 5.0 ± 4.7). Mean SCOPA-AUT score was 10.1 ± 7.3. Patients were on Hoehn & Yahr stage 1-2 and mean Levodopa Equivalent Dose (LED) was 311 ± 239.9. Mean of the 5-min standard deviation (SD) of R-R intervals distribution (SDNN) for all 5 min segments of the entire recording (ISDNN) was significantly lower in patients compared to controls. ISDNN was significantly different between Parkinson's disease patients and healthy controls.

MicroRNAs are small noncoding RNAs that are important for proper cardiac development. In our previous study of fetuses with ventricular septal defects, we discovered that microRNA-375 (miR-375) is obviously upregulated compared with that in healthy controls. Our study also confirmed that miR-375 is crucial for cardiomyocyte differentiation. This research mainly focused on the biological significance and mechanism of miR-375 using a zebrafish model. We injected zebrafish embryos with 1-2 nl of a miR-375 mimic at various concentrations (0/2/4/8 μM) or with negative control. The deformation and mortality rates were separately assessed. The different expression levels of miR-375 and related genes were examined by qRT-PCR, and luciferase assays and in situ hybridization were used to clarify the mechanism of miR-375 during embryonic development. Overexpression of miR-375 disrupted the cardiac development of zebrafish embryos. Disruption of miR-375 led to a decreased heart rate, pericardial edema, and abnormal cardiac looping. Various genes involved in cardiac development were downregulated due to the overexpression of miR-375. Moreover, the NOTCH signaling pathway was affected, and the luciferase reporter gene assays confirmed notch2, which was predicted by bioinformatics analysis, as the target gene of miR-375. Our findings demonstrated that the overexpression of miR-375 is detrimental to embryonic development, including cardiac development, and can partially simulate a multisystemic disorder. MiR-375 has an important role during cardiac morphogenesis of zebrafish embryos by targeting notch2, indicating its potential as a diagnostic marker.Hemoglobinopathies are examples of autosomal recessive disorders of human hemoglobin. Hemoglobin E (HbE) and Hemoglobin D Punjab (HbD Punjab) are two of the most common hemoglobin variants geographically spread across Asian continent. These two variants differ from normal human hemoglobin (HbA) at a single amino acid residue caused by the point mutation of β globin gene. The presence of the mutated amino acid residue causes perturbation in the function of both variants. However, the structure-function correlation of these variants has not been established yet. In the present study, we analyzed the conformational changes associated with oxygenation of hemoglobin variants using hydrogen/deuterium exchange-based mass spectrometry of backbone amide hydrogens of α and β globin chains in the tetrameric hemoglobin molecule. We also performed the functional assay of these variants using oxygen dissociation equilibrium curve. Compared to HbA, both variants showed reduced oxygen affinity, as reported earlier. The functional perturbations exhibited by these variants were correlated well with their structural alterations with respect to the reported changes in the residue level interactions upon oxygenation of normal hemoglobin, monitored through the hydrogen/deuterium exchange kinetics of several peptic peptides originated from the isotopically exchanged oxy and deoxy forms of HbE and HbD Punjab.This work aimed to assess the ability of plant growth-promoting Bacilli isolated from wheat rhizosphere and rock phosphate mine soils to convert inorganic phosphate (Pi) from Moroccan natural phosphate (NP) to soluble forms. The effect of these bacteria on wheat plants in order to increase their phosphorus (P) uptake in vitro was also investigated. Bacteria were isolated from wheat rhizosphere and natural rock phosphate soils and screened for their ability to solubilize Tri-Calcium Phosphate (TCP) and Natural Rock Phosphate (NP), to produce indole-3-acetic acid (IAA), siderophores and 1-aminocyclopropane-1-carboxylate (ACC) deaminase. Isolates were identified by 16S rRNA sequencing and tested for their capacity to increase wheat plants growth and their phosphorus uptake.Twenty-four strains belonging to Bacillus genus isolated from both biotopes were screened for their ability to solubilize Pi. The highest NP solubilization was showed by strains isolated from wheat rhizosphere. Solubilization of Pi was accompanied by organic acid production. Strains produce IAA, siderophore and ACC deaminase. Inoculation assays using efficient NP-solubilizing bacilli strains from both sources showed the ability of these isolates to increase wheat growth and the phosphorus uptake under in vitro conditions. Bacilli strains isolated from rhizosphere soil and natural rock phosphorus soil showed effective solubilization of Pi from rock phosphate. Phosphate solubilizing Bacilli were evaluated for their plant growth promotion under in vitro conditions. MicroRNAs are small noncoding RNAs that are important for proper cardiac development. In our previous study of fetuses with ventricular septal defects, we discovered that microRNA-375 (miR-375) is obviously upregulated compared with that in healthy controls. Our study also confirmed that miR-375 is crucial for cardiomyocyte differentiation. This research mainly focused on the biological significance and mechanism of miR-375 using a zebrafish model. We injected zebrafish embryos with 1-2 nl of a miR-375 mimic at various concentrations (0/2/4/8 μM) or with negative control. The deformation and mortality rates were separately assessed. The different expression levels of miR-375 and related genes were examined by qRT-PCR, and luciferase assays and in situ hybridization were used to clarify the mechanism of miR-375 during embryonic development. Overexpression of miR-375 disrupted the cardiac development of zebrafish embryos. Disruption of miR-375 led to a decreased heart rate, pericardial edema, and abnormal cardiac looping. Various genes involved in cardiac development were downregulated due to the overexpression of miR-375. Moreover, the NOTCH signaling pathway was affected, and the luciferase reporter gene assays confirmed notch2, which was predicted by bioinformatics analysis, as the target gene of miR-375. Our findings demonstrated that the overexpression of miR-375 is detrimental to embryonic development, including cardiac development, and can partially simulate a multisystemic disorder. Hemoglobin E (HbE) and Hemoglobin D Punjab (HbD Punjab) are two of the most common hemoglobin variants geographically spread across Asian continent. These two variants differ from normal human hemoglobin (HbA) at a single amino acid residue caused by the point mutation of β globin gene. The presence of the mutated amino acid residue causes perturbation in the function of both variants. However, the structure-function correlation of these variants has not been established yet. In the present study, we analyzed the conformational changes associated with oxygenation of hemoglobin variants using hydrogen/deuterium exchange-based mass spectrometry of backbone amide hydrogens of α and β globin chains in the tetrameric hemoglobin molecule. We also performed the functional assay of these variants using oxygen dissociation equilibrium curve. Compared to HbA, both variants showed reduced oxygen affinity, as reported earlier. The functional perturbations exhibited by these variants were correlated well with their structural alterations with respect to the reported changes in the residue level interactions upon oxygenation of normal hemoglobin, monitored through the hydrogen/deuterium exchange kinetics of several peptic peptides originated from the isotopically exchanged oxy and deoxy forms of HbE and HbD Punjab.This work aimed to assess the ability of plant growth-promoting Bacilli isolated from wheat rhizosphere and rock phosphate mine soils to convert inorganic phosphate (Pi) from Moroccan natural phosphate (NP) to soluble forms. The effect of these bacteria on wheat plants in order to increase their phosphorus (P) uptake in vitro was also investigated. Bacteria were isolated from wheat rhizosphere and natural rock phosphate soils and screened for their ability to solubilize Tri-Calcium Phosphate (TCP) and Natural Rock Phosphate (NP), to produce indole-3-acetic acid (IAA), siderophores and 1-aminocyclopropane-1-carboxylate (ACC) deaminase. Isolates were identified by 16S rRNA sequencing and tested for their capacity to increase wheat plants growth and their phosphorus uptake.Twenty-four strains belonging to Bacillus genus isolated from both biotopes were screened for their ability to solubilize Pi. The highest NP solubilization was showed by strains isolated from wheat rhizosphere. Solubilization of Pi was accompanied by organic acid production. Strains produce IAA, siderophore and ACC deaminase. Inoculation assays using efficient NP-solubilizing bacilli strains from both sources showed the ability of these isolates to increase wheat growth and the phosphorus uptake under in vitro conditions. Bacilli strains isolated from rhizosphere soil and natural rock phosphorus soil showed effective solubilization of Pi from rock phosphate. Phosphate solubilizing Bacilli were evaluated for their plant growth promotion under in vitro conditions. Background Cetuximab has been shown to be clinically active when given in combination with irinotecan in patients with irinotecan-refractory metastatic colorectal cancer (mCRC). However, it has remained unclear whether panitumumab is effective when combined with irinotecan. We compared efficacies of both regimens in this randomised phase II study. Patients and methods Patients with wild-type KRAS exon 2 mCRC previously treated with fluorouracil-, oxaliplatin- and irinotecan-based chemotherapies were randomised (11) to either panitumumab plus irinotecan (panitumumab arm) or cetuximab plus irinotecan (cetuximab arm). The primary end-point was progression-free survival (PFS). The planned sample size was 120, expecting a hazard ratio (HR) of 1.0 with non-inferiority margin of 1.3 (one-sided alpha error 0.2 and power 0.7). Major secondary end-points were overall survival (OS), response rate and safety. Results From December 2011 to September 2014, 121 patients were enrolled, and 61 and 59 patients were randomised to the panitumumab and cetuximab arms, respectively (1 patient excluded). Most patients (97%) had received prior chemotherapies containing bevacizumab. The median PFS was 5.42 months in the panitumumab arm and 4.27 months in the cetuximab arm (HR = 0.64, 95% confidence interval [CI] = 0.44-0.94, P less then 0.001 for non-inferiority, P = 0.058 for superiority), and median OS was 14.85 and 11.53 months (HR = 0.66, 95% CI = 0.44-1.00, P = 0.050 for superiority), respectively. The incidence of grade 3 or 4 hypomagnesaemia was higher in the panitumumab arm than that in the cetuximab arm (17% vs. 7%). AKI was defined by the Kidney Disease Improving Global Outcome (KDIGO) criteria. The primary outcome was persistent AKI, defined as non-resolved AKI on day three. We tested specificity, sensitivity and diagnostic accuracy of both RRI and VII for persistent AKI. Results In total, 371 patients were included of whom 123 patients (33%) had persistent AKI. RRI and VII did not differ between patients with and those without persistent AKI (p = .08 and p = .59). RRI had a moderate specificity (72%, 95%CI 66-78%) and low sensitivity (32%, 95%CI 24-41%) and VII had high sensitivity (93%, 95%CI 85-98%) and low specificity (11%, 95%CI 6-16%) for persistent AKI. Overall diagnostic accuracy of RRI and VII was moderate. Conclusions In acutely admitted critically ill patients, measures of renal perfusion by renal ultrasound were not different between patients with and without AKI, and show limited diagnostic accuracy for AKI. Cryolite was introduced to the reaction system to dissolve the SiO2 layer existed on the surface of the Si particles in SCW. Alloys with 12.02 wt% of Si were prepared and the mechanism of the alloying process was investigated in detail. The Si-Al-cryolite system and SiO2-Al-cryolite system were studied individually to analyze the reaction process and transferring behavior of Si and SiO2 in SCW. The SiO2 shell was firstly transformed into Si-O-F ions. Then the Si-O-F ions diffused to the reaction interface by the effect of the concentration gradient and were reduced to Si by the aluminothermic reduction reaction 4Al (l) + 3SiO2 (dissolved in the melt) = 3Si (Al)+ 2Al2O3 (dissolved in the melt). Then the internal Si particles were released into cryolite after the dissolution of SiO2 and transferred to the reaction interface by the effect of gravity. The influences of the mass ratio of Al/SCW and agitation modes on the Si content of the alloys and the Si recovery ratio in SCW were investigated. Methods Soft x-ray spectromicroscopy was applied in the form of Scanning Transmission X-ray Microscopy (STXM) to analyse resin-embedded tissue, without requirement for chemically disruptive processing or staining. Measurements were performed at the oxygen and iron K-edges in order to characterise both organic and inorganic components of anatomical tissue using a single label-free method. Results STXM revealed evidence for mixed oxidation states of neuronal iron deposits associated with neuromelanin clusters in Parkinson's disease substantia nigra. The excellent sensitivity, specificity and spatial resolution of these STXM measurements showed that the iron oxidation state varies across sub-micron length scales. Conclusions The label-free STXM approach is highly suited to characterising the distributions of both inorganic and organic components of anatomical tissue, and provides a proof-of-concept for investigating trace metal speciation within Parkinson's disease neuromelanin-containing neurons.

STATEMENT OF PROBLEM Tooth preparations for ceramic crowns require precision and accuracy, which may be influenced by the choice of dental handpiece. However, comparisons of the accuracy of tooth preparations made with traditional air-turbine handpieces and electric handpieces are lacking. PURPOSE The purpose of this in vitro study was to evaluate operator preferences and tooth preparation performance by using electric and air-turbine handpieces with self-reported preferences, sound levels, surface roughness, and the fit of the crown produced. MATERIAL AND METHODS Twenty dentists were asked to use the air-turbine or the electric handpiece. Feedback on the noise, weight, feel of grip, flexibility, and tooth preparation in general was scored according to a visual analog scale (VAS). STATEMENT OF PROBLEM The release of titanium (Ti) particles from the surface of endosseous dental implants is not well understood. PURPOSE The purpose of this in vitro study was to evaluate the effect of thermocycling on the surface texture and release of Ti particles from the surface of dental implants. MATERIAL AND METHODS Three MSI dental implants and 3 Ti alloy (Ti6Al4V) plates were divided into 6 subgroups (n=3). Specimens in each group were subjected to 0 (control group), 100, 200, 500, 1000, and 2000 thermocycles. After each cycling process, artificial saliva was collected, and the concentrations of released Ti particles were quantified by inductively coupled plasma-mass spectrophotometry (ICP-MS). The surfaces of the dental implants and Ti plates were evaluated before and after thermocycling by scanning electron microscopy (SEM), and SEM images were analyzed by using the ImageJ software. Data were analyzed by mixed-model ANOVA and post hoc Tukey tests (α=.05). RESULTS The greatest Ti release was seen after 2000 thermocycles. After increasing the number of cycles, additional Ti particles were released. SEM images of the surfaces of the dental implants and Ti plates displayed significant changes in surface texture. CONCLUSIONS Thermocycling continuously removed the protective TiO2 layer on the surface of dental implants, resulting in the release of Ti particles. The surface treatment and texture did not affect the release of Ti particles. STATEMENT OF PROBLEM Although numerous studies have been performed on the accuracy of intraoral scanners, determining the clinical significance of the results is problematic. PURPOSE The purpose of this in vitro study was to evaluate the trueness and precision of 4 intraoral optical scanners (IOSs) on a 6-implant model and provide a method to help determine clinical significance. MATERIAL AND METHODS A polymer mandibular edentulous model with 6 hexagonal scan bodies (Ritter) was fabricated, and a control scan was made by using an industrial laser line probe (FARO Edge HD Arm). Four IOSs (True Definition, TRIOS, CEREC Omnicam, Emerald Scanner) were used to scan the same model 5 times the 20 standard tessellation language (STL) files were individually imported to a 3D inspection software program (Geomagic Control X) and superimposed over the computer-aided design (***) control scan. The tolerance was set at a limit of ±0.01 mm. RESULTS None of the tested scanners were true even 10% of the time at the ±0.01-mm tolerance, and the Emerald scanner was true less than 5% of the time. Within scanners, results were precise, showing variations of no more than 2% over repeated scans. When a ±0.05-mm tolerance was selected, the percentage within tolerance increased dramatically. This made the performance of the scanners to appear better but obscured valuable information. The 3D color map was the best method for understanding the data. The color maps showed how **** was within tolerance and, equally important, the amount and direction of out of tolerance, providing an easily understandable qualitative and quantitative image. CONCLUSIONS No statistical or clinical differences were found among the scanners tested. The 3D map was the best method for observing the data. Grief is the natural response to the death of a loved one and is encountered frequently in clinical practice with the elderly; it can also precede the death. Knowledge about four distinct forms of grief can aid clinicians with the conceptualization of grief, and the assessment and care of grievers. First, predeath grief is experienced by many caregivers of terminally ill patients. Second, acute grief arises immediately after the death of a loved one; and third, this normally evolves to a permanent state of integrated grief after a process of adaptation. Finally, failure of adaptation results in Prolonged Grief Disorder (PGD) which has been recently included in ICD-11. The hallmark feature of PGD is intense longing for the deceased or persistent preoccupation with the deceased that lasts longer than 6 months after the death. Validated instruments are available to assist practitioners with assessment of predeath grief and screening for PGD, thereby enabling identification of patients in need of additional support. Increased risks of morbidity and mortality following bereavement are important health issues for clinicians to be aware of. All grievers can benefit from support focused on understanding their grief, managing emotional pain, thinking about the future, strengthening their relationships, telling the story of the death, learning to live with reminders of the deceased, and connecting with memories. A short-term evidence-based intervention for PGD is based upon these seven themes and is efficacious in the elderly. Caregivers of the terminally ill benefit from psychological support that validates and normalizes their grief experiences and helps them recognize and accept their losses. OBJECTIVE To analyze the risk of megestrol, a glucocorticoid and progesterone receptor agonist used to enhance appetite, on the development of a new psychiatric diagnosis. DESIGN AND PARTICIPANTS Deidentified data of megestrol (n = 706) and propensity score-matched comparison (age, gender, and body mass index) patients (n = 2,118) from January 1, 2001 to June 30, 2018 were obtained from the UT Southwestern patient database. Data were analyzed using a series of conditional binary logistic regressions controlling for comorbidities, pre-existing psychiatric disorders, and number of patient encounters. SETTING A large academic medical center database of megestrol-treated patients and matched comparison patients was used. MEASUREMENTS AND RESULTS The regression model showed that megestrol was significantly associated with developing a new psychiatric diagnosis (B = 1.28, Wald χ21 = 83.12, odds ratio [OR] = 3.60, p less then 0.001). STATEMENT OF PROBLEM Tooth preparations for ceramic crowns require precision and accuracy, which may be influenced by the choice of dental handpiece. However, comparisons of the accuracy of tooth preparations made with traditional air-turbine handpieces and electric handpieces are lacking. PURPOSE The purpose of this in vitro study was to evaluate operator preferences and tooth preparation performance by using electric and air-turbine handpieces with self-reported preferences, sound levels, surface roughness, and the fit of the crown produced. MATERIAL AND METHODS Twenty dentists were asked to use the air-turbine or the electric handpiece. Feedback on the noise, weight, feel of grip, flexibility, and tooth preparation in general was scored according to a visual analog scale (VAS). STATEMENT OF PROBLEM The release of titanium (Ti) particles from the surface of endosseous dental implants is not well understood. PURPOSE The purpose of this in vitro study was to evaluate the effect of thermocycling on the surface texture and release of Ti particles from the surface of dental implants. MATERIAL AND METHODS Three MSI dental implants and 3 Ti alloy (Ti6Al4V) plates were divided into 6 subgroups (n=3). Specimens in each group were subjected to 0 (control group), 100, 200, 500, 1000, and 2000 thermocycles. After each cycling process, artificial saliva was collected, and the concentrations of released Ti particles were quantified by inductively coupled plasma-mass spectrophotometry (ICP-MS). The surfaces of the dental implants and Ti plates were evaluated before and after thermocycling by scanning electron microscopy (SEM), and SEM images were analyzed by using the ImageJ software. Data were analyzed by mixed-model ANOVA and post hoc Tukey tests (α=.05). RESULTS The greatest Ti release was seen after 2000 thermocycles. After increasing the number of cycles, additional Ti particles were released. SEM images of the surfaces of the dental implants and Ti plates displayed significant changes in surface texture. CONCLUSIONS Thermocycling continuously removed the protective TiO2 layer on the surface of dental implants, resulting in the release of Ti particles. The surface treatment and texture did not affect the release of Ti particles. STATEMENT OF PROBLEM Although numerous studies have been performed on the accuracy of intraoral scanners, determining the clinical significance of the results is problematic. PURPOSE The purpose of this in vitro study was to evaluate the trueness and precision of 4 intraoral optical scanners (IOSs) on a 6-implant model and provide a method to help determine clinical significance. MATERIAL AND METHODS A polymer mandibular edentulous model with 6 hexagonal scan bodies (Ritter) was fabricated, and a control scan was made by using an industrial laser line probe (FARO Edge HD Arm). Four IOSs (True Definition, TRIOS, CEREC Omnicam, Emerald Scanner) were used to scan the same model 5 times the 20 standard tessellation language (STL) files were individually imported to a 3D inspection software program (Geomagic Control X) and superimposed over the computer-aided design (CAD) control scan. The tolerance was set at a limit of ±0.01 mm. RESULTS None of the tested scanners were true even 10% of the time at the ±0.01-mm tolerance, and the Emerald scanner was true less than 5% of the time. Within scanners, results were precise, showing variations of no more than 2% over repeated scans. When a ±0.05-mm tolerance was selected, the percentage within tolerance increased dramatically. This made the performance of the scanners to appear better but obscured valuable information. The 3D color map was the best method for understanding the data. The color maps showed how much was within tolerance and, equally important, the amount and direction of out of tolerance, providing an easily understandable qualitative and quantitative image. CONCLUSIONS No statistical or clinical differences were found among the scanners tested. The 3D map was the best method for observing the data. Grief is the natural response to the death of a loved one and is encountered frequently in clinical practice with the elderly; it can also precede the death. Knowledge about four distinct forms of grief can aid clinicians with the conceptualization of grief, and the assessment and care of grievers. First, predeath grief is experienced by many caregivers of terminally ill patients. Second, acute grief arises immediately after the death of a loved one; and third, this normally evolves to a permanent state of integrated grief after a process of adaptation. Finally, failure of adaptation results in Prolonged Grief Disorder (PGD) which has been recently included in ICD-11. The hallmark feature of PGD is intense longing for the deceased or persistent preoccupation with the deceased that lasts longer than 6 months after the death. Validated instruments are available to assist practitioners with assessment of predeath grief and screening for PGD, thereby enabling identification of patients in need of additional support. Increased risks of morbidity and mortality following bereavement are important health issues for clinicians to be aware of. All grievers can benefit from support focused on understanding their grief, managing emotional pain, thinking about the future, strengthening their relationships, telling the story of the death, learning to live with reminders of the deceased, and connecting with memories. A short-term evidence-based intervention for PGD is based upon these seven themes and is efficacious in the elderly. Caregivers of the terminally ill benefit from psychological support that validates and normalizes their grief experiences and helps them recognize and accept their losses. OBJECTIVE To analyze the risk of megestrol, a glucocorticoid and progesterone receptor agonist used to enhance appetite, on the development of a new psychiatric diagnosis. DESIGN AND PARTICIPANTS Deidentified data of megestrol (n = 706) and propensity score-matched comparison (age, gender, and body mass index) patients (n = 2,118) from January 1, 2001 to June 30, 2018 were obtained from the UT Southwestern patient database. Data were analyzed using a series of conditional binary logistic regressions controlling for comorbidities, pre-existing psychiatric disorders, and number of patient encounters. SETTING A large academic medical center database of megestrol-treated patients and matched comparison patients was used. MEASUREMENTS AND RESULTS The regression model showed that megestrol was significantly associated with developing a new psychiatric diagnosis (B = 1.28, Wald χ21 = 83.12, odds ratio [OR] = 3.60, p less then 0.001). However, the screening and analysis of TDCs in environmental samples remain a tough work. In this study, we reported a structure-directed strategy for analyzing TDCs targeting transthyretin (TTR) based on molecular imprinting and chromatographic separation. The imprinted composites were prepared using l-thyroxine (T4) as a template and a tryptophan-like monomer screened from the amino acid library. The imprinted composites exhibited an adsorption capacity of 22.2 μmol g-1 for T4 and an imprinting factor of 2.1. Chromatographic testing was then conducted among 72 chemicals using the imprinted composites-packed column. High retention factors were observed for chemicals that were structurally similar to T4. The chromatographic results were compared with a data set of 45 chemicals with known activities toward TTR. The results suggested that chemicals can be distinguished as TTR binders and nonbinders by retention factors, with a predictive accuracy of more than 90%. Moreover, the retention factors of chemicals were highly correlated with the reported relative potencies obtained from TTR assays. Thus, screening of TTR-binding chemicals can be realized through this simple chromatographic method. The imprinted composites were applied for target analysis and nontarget analysis of TTR-binding chemicals in dust samples. Three new TTR binders were successfully identified and verified by this method. The gradients of DP concentrations correlated well with microbial biomass in the rhizosphere (R2 = 0.898). The rhizosphere facilitated the bioavailability of DP (excitation) and modified the bound-residue formation of DP (aging). DP concentrations in roots were positively correlated with the labile fraction of DP in soil (R2 = 0.852-0.961). There were spatiotemporal variations in the DP fractions. Dissolved and soil organic carbon were important influences on fraction transformation. Contributions to total DP dissipation were in the following ranges microbial degradation (8.33-54.14%), bound-residue formation (3.64-16.43%), and plant uptake (0.54-3.85%). The highest power conversion efficiency (PCE) of CJ-04-based cell is 13.75%, which is increased to 20.06% when CJ-03 is used as HTM, superior to the PCE of the cell based on 2,2',7,7'-tetrakis(N,N-di-p-methoxyphenylamine)-9,9'-spirobifluorene (spiro-OMeTAD) (18.90%). The preparation cost of CJ-03 accounts for merely 23.1% of the price of commercial spiro-OMeTAD, while the concentration of CJ-03 solution used in the device fabrication (60.0 mg mL-1) is lower compared with that of the spiro-OMeTAD solution (72.3 mg mL-1). These results corroborate that the screw-like HTMs with a highly distorted configuration are facilely available and promising candidates for PSCs. The bottom layer consists of spin-coated PEDOT-PSS (poly(styrenesulfonate)). Together, these two layers constitute a current divider. We demonstrate here that reducing the thickness of the bottom PEDOT-PSS layer increases its resistance and concentrates the resistance drop of the channel in the top virus-PEDOT layer, thereby increasing the sensitivity of the VBR and enabling the detection of DJ-1.

Proper treatment and disposal of sludge is a substantial task around the biosphere. To address this issue, sludge deflocculation using photocatalyst was opted to enhance bacterial disintegration which in turn accelerate sludge digestion anaerobically. During this investigation, Direct current (DC) sputtering together with annealing process was used to immobilize Zinc oxide (ZnO). This immobilized ZnO removes the extracellular components at 15 min. The deflocculation mediated bacterial pretreatment induced 22.9% of soluble organics solubilization which auguments the biodegradability to 0.195 g COD/g COD during anaerobic digestion. The quantity of methane generated by deflocculated sludge was 39.2% higher than sludge with bacterial disintegration only with maximum methane yield of 437.14 mL/g COD. Hence, the outcome of the proposed work confirmed that the method is scalable with a net profit of 27 USD with the maximum methane generation of 413.1 kWh. Additionally, this method reduced 57% of dry sludge (solid). Novel processes for the production of acetone-butanol-ethanol (ABE) from municipal solid waste (MSW) were developed and simulated using Aspen Plus®. In scenario 1, a conventional distillation system was used, while a gas stripping system was coupled with a fermenter in scenario 2. In scenario 3, pervaporation (PV) and gas stripping systems right after the fermentation reactor were applied. Gas stripping increased the total ABE produced while the addition of the PV module decreased the number of distillation columns from 6 to 2 as well as created 6.4% increments in the amount of butanol in comparison with scenario 1. Economical evaluation resulted in having payout periods of 15.9, 4.4, and 2.9 years for scenarios 1 to 3, respectively. These results show that using MSW as an inexpensive sugar-rich feedstock together with gas stripping PV system is a promising solution to overcome the major obstacles in the way of the ABE production. In most cases, it is necessary to add nutrients to obtain a more balanced AD process and to ensure favorable conditions for the growth and metabolism of microorganisms. The aim of this study was to verify the influence of the components of nutrient solution added to AD of tannery wastes with regard to the energy and waste treatment efficiency and to the cost-saving analysis. The findings provides better understanding of the nutritional requirement of co-AD of tannery wastes, with the highest value of biogas production of 30.14 mL/g of added VSS, besides representing a step in the search for a more balanced, efficient and viable process. The results obtained proving the competitiveness of co-AD of tannery wastes instead of disposing it in landfills (saving off about 71% in terms of electric consumption). The performance of nitrogen and organic carbon removal in a single reactor (R1) operating with A. faecalis strain NR aerobically was assessed. Under 150 mg/L influent NH4+-N, 91.3%, 71.4% and 90.9% of NH4+-N, TN and TOC were removed, presenting **** higher efficiency than a control bioreactor inoculating activated sludge (R0). The amoA gene expression from strain NR in R1 was 7.8 times higher than that from activated sludge in R0, demonstrating the role of strain NR in removing NH4+. The analysis of microbial community composition revealed that strain NR was the dominant species and outcompeted ammonium oxidizing bacterium (AOB) under high organic carbon as well as ammonium. Simultaneous ammonium and organic carbon removal still maintained for a long-term operation with NH4+-N loadings of 300 and 450 mg/L in R1. This study thus investigated facultative QQ bacterial strains that exhibit potential for use in aerobic and anaerobic MBRs. Two novel QQ strains from the genus Pseudomonas (KS2 and KS10) were isolated from anaerobic digester sludge using signal molecules as the sole carbon source. The two QQ strains exhibited significant signal molecule degradation depending on the oxygen levels and demonstrated endogenous QQ activity, with KS2 producing lactonase and KS10 producing acylase. The QQ strains significantly reduced the formation of the biofilm generated by both Pseudomonas aeruginosa (PAO1) and real sludge. Facultative QQ strains have the potential to offer a more flexible option for effective biofouling control in both aerobic and anaerobic MBRs. Sludge retention time (SRT) is vital for advanced anaerobic digestion (AD) to realize energy self-sufficient. However, the criteria on reasonable SRT has not been fully understood. This study investigated the performance and kinetics response of AD under different SRT in semi-continuous AD with microwave (MW) pretreatment, according to the long-term operation and methane production during one feeding interval. Results showed that modified Gompertz model better described the kinetics than first-order model. At short SRT (15 d), pretreatment coupled with two-stage AD preserved methane production with the high attainable methane potential (B0) of 257.98 mL/g VS and hydrolysis rate constant (khyd) of 0.075 h-1. But the acceptable decrease of methane production rate seems to be unavoidable, which was possibly derived from the evolution of methanogenesis pathway. This study emphasized the importance of improved methane production rate in semi-continuous AD under short SRT rather than methane production potential obtained from batch experiment.

The quantity of methane generated by deflocculated sludge was 39.2% higher than sludge with bacterial disintegration only with maximum methane yield of 437.14 mL/g COD. Hence, the outcome of the proposed work confirmed that the method is scalable with a net profit of 27 USD with the maximum methane generation of 413.1 kWh. Additionally, this method reduced 57% of dry sludge (solid). Novel processes for the production of acetone-butanol-ethanol (ABE) from municipal solid waste (MSW) were developed and simulated using Aspen Plus®. In scenario 1, a conventional distillation system was used, while a gas stripping system was coupled with a fermenter in scenario 2. In scenario 3, pervaporation (PV) and gas stripping systems right after the fermentation reactor were applied. Gas stripping increased the total ABE produced while the addition of the PV module decreased the number of distillation columns from 6 to 2 as well as created 6.4% increments in the amount of butanol in comparison with scenario 1. Economical evaluation resulted in having payout periods of 15.9, 4.4, and 2.9 years for scenarios 1 to 3, respectively. These results show that using MSW as an inexpensive sugar-rich feedstock together with gas stripping PV system is a promising solution to overcome the major obstacles in the way of the ABE production. The aim of this study was to verify the influence of the components of nutrient solution added to AD of tannery wastes with regard to the energy and waste treatment efficiency and to the cost-saving analysis. The findings provides better understanding of the nutritional requirement of co-AD of tannery wastes, with the highest value of biogas production of 30.14 mL/g of added VSS, besides representing a step in the search for a more balanced, efficient and viable process. The performance of nitrogen and organic carbon removal in a single reactor (R1) operating with A. faecalis strain NR aerobically was assessed. Under 150 mg/L influent NH4+-N, 91.3%, 71.4% and 90.9% of NH4+-N, TN and TOC were removed, presenting much higher efficiency than a control bioreactor inoculating activated sludge (R0). This study investigated the performance and kinetics response of AD under different SRT in semi-continuous AD with microwave (MW) pretreatment, according to the long-term operation and methane production during one feeding interval. Results showed that modified Gompertz model better described the kinetics than first-order model. At short SRT (15 d), pretreatment coupled with two-stage AD preserved methane production with the high attainable methane potential (B0) of 257.98 mL/g VS and hydrolysis rate constant (khyd) of 0.075 h-1. But the acceptable decrease of methane production rate seems to be unavoidable, which was possibly derived from the evolution of methanogenesis pathway. Results RT-qPCR results showed that the relative expression levels of c-Met mRNA in siRNA-Met group, blank control group and siRNA negative control (siRNA-NC) group were 0.32±0.26, 1.01±0.03 and 1.05±0.23, respectively, and the difference was statistically significant (P less then 0.05). Western blot analysis showed that the expression level of c-Met protein in the siRNA-Met group was 0.24±0.03, sigsignificantly decreased while Bcl-2 associated X protein (BAX) expression level was significantly increased. Glycemic control had remarkable improvement without major adverse events, although he required a small dose of octreotide to maintain euglycemia. Sirolimus therapy was discontinued when the patient was 15 months old. The patient had been having a good glycemic control at the time of this report when he was three years and 8 months old. This report suggests that sirolimus may be an effective treatment option in patients with CHI resistant to traditional medical therapy or failure of subtotal pancreatectomy. Conclusions If women who would seek a safe and legal abortion in RI are denied one, clear and undue burden will exist for those who carry to term and raise the child, as well as affecting existing children.

Mothers, particularly those who were on maternity leave when their child was diagnosed with cancer, reported ongoing employment impacts including unemployment. CONCLUSIONS Clinical staff including social workers could more consistently assess families' financial distress and refer to professional services who can offer guidance for financial decision-making as standard care. Flexible workplace agreements appear important for parents of children with cancer. RESULTS VTP-50469 showed limited antitumor activity, statistically significantly slowing tumor progression in four tumor models but with no evidence of tumor regression. METHODS A total of 13 LPL polymorphisms were genotyped in 1.057 European adolescents (12-18 years old) from the Healthy Lifestyle in Europe by Nutrition in Adolescence Cross-Sectional Study. Serum lipids, glucose, insulin and leptin levels were measured and a CVD risk score was computed. We also measured body weight and height, waist and hip circumferences, and triceps and subscapular skinfold thickness. Physical activity was objectively measured by accelerometry for 7 days. RESULTS The rs1534649, rs258, rs320 and rs328 polymorphisms were associated with several CVD risk factors (i.e., body mass index [BMI], Triglycerides [TG], Leptin, and cholesterol/High density lipoprotein [HDL], Low density lipoprotein [LDL]/HDL, TG/HDL ratios). TG and TG/HDL were associated with haplotype blocks 3 (rs282, rs285 polymorphisms) and 4 (rs3126, rs320, rs328, rs10099160 polymorphisms), being the latter also associated with the CVD risk score. Physical activity modulated the association of adiposity with rs1534649 and rs258 polymorphisms. CONCLUSIONS Polymorphisms rs1534649, rs258, rs320 and rs328, and two haplotypes of LPL were significantly associated with CVD risk factors in European adolescents. The aim was to test if the Nordic DISABKIDS® Health-related Quality-of-Life (HrQoL) modules were construct valid and measurement comparable within the three Nordic countries. METHODS Data from three DISABKIDS® validation studies in Sweden, Denmark and Norway were compared using Rasch and the graphical log linear Rasch modeling. Monte Carlo methods were used to estimate reliability coefficient and target was defined as the point with the lowest standard error of the mean. Self-report data were available from 99 Danish (8-18 years), 103 Norwegian (7-19 years) and 131 Swedish (8-18 years) young people. RESULTS For the DISABKIDS® higher scores on most subscales were noted in the Norwegian population. The Swedish sample had a significantly higher score on the "Diabetes treatment" subscale and scores closer to optimal target than the other countries. For each country construct validity and sensitivity were acceptable when accounting for differential item function (DIF) and local dependency (LD). Less LD and DIF were found if only Denmark and Norway were included. The combined model was reliable however, some differences were noted in the scale translations relating to the stem and response alternatives, which could explain the discrepancies. CONCLUSION The Nordic versions of the DISABKIDS® questionnaires measures valid and reliable HrQoL both within and between countries when adjusted for DIF and LD. PROCEDURE VTP-50469 was evaluated against seven Ewing sarcoma (EwS) xenograft models and in vitro against EwS cell lines. RESULTS VTP-50469 showed limited antitumor activity, statistically significantly slowing tumor progression in four tumor models but with no evidence of tumor regression. In vitro, the IC50 concentration was 10 nM for the mixed lineage leukemia (MLL)-rearranged leukemia cell line MV4;11, but > 3 μM for EwS cell lines. CONCLUSIONS In contrast to its high level of activity against MLL1-rearranged leukemia xenografts, VTP-50469 shows little activity against EwS models. © 2020 Wiley Periodicals, Inc.OBJECTIVES To examine the association of lipoprotein lipase (LPL) polymorphisms with cardiovascular disease (CVD) risk factors in European adolescents, along with the influence of physical activity on these associations. METHODS A total of 13 LPL polymorphisms were genotyped in 1.057 European adolescents (12-18 years old) from the Healthy Lifestyle in Europe by Nutrition in Adolescence Cross-Sectional Study. Serum lipids, glucose, insulin and leptin levels were measured and a CVD risk score was computed. We also measured body weight and height, waist and hip circumferences, and triceps and subscapular skinfold thickness. Physical activity was objectively measured by accelerometry for 7 days. RESULTS The rs1534649, rs258, rs320 and rs328 polymorphisms were associated with several CVD risk factors (i.e., body mass index [BMI], Triglycerides [TG], Leptin, and cholesterol/High density lipoprotein [HDL], Low density lipoprotein [LDL]/HDL, TG/HDL ratios). TG and TG/HDL were associated with haplotype blocks 3 (rs282, rs285 polymorphisms) and 4 (rs3126, rs320, rs328, rs10099160 polymorphisms), being the latter also associated with the CVD risk score. Physical activity modulated the association of adiposity with rs1534649 and rs258 polymorphisms. CONCLUSIONS Polymorphisms rs1534649, rs258, rs320 and rs328, and two haplotypes of LPL were significantly associated with CVD risk factors in European adolescents. All rights reserved.OBJECTIVES Government guidance promote benchmarking comparing quality of care including both clinical values and patient reported outcome measures in young persons with Type 1 diabetes (T1D). The aim was to test if the Nordic DISABKIDS® Health-related Quality-of-Life (HrQoL) modules were construct valid and measurement comparable within the three Nordic countries. METHODS Data from three DISABKIDS® validation studies in Sweden, Denmark and Norway were compared using Rasch and the graphical log linear Rasch modeling. Monte Carlo methods were used to estimate reliability coefficient and target was defined as the point with the lowest standard error of the mean. Self-report data were available from 99 Danish (8-18 years), 103 Norwegian (7-19 years) and 131 Swedish (8-18 years) young people. RESULTS For the DISABKIDS® higher scores on most subscales were noted in the Norwegian population. The Swedish sample had a significantly higher score on the "Diabetes treatment" subscale and scores closer to optimal target than the other countries. For each country construct validity and sensitivity were acceptable when accounting for differential item function (DIF) and local dependency (LD). Less LD and DIF were found if only Denmark and Norway were included. The combined model was reliable however, some differences were noted in the scale translations relating to the stem and response alternatives, which could explain the discrepancies. CONCLUSION The Nordic versions of the DISABKIDS® questionnaires measures valid and reliable HrQoL both within and between countries when adjusted for DIF and LD.
