  • In the last two decades, new insights have been gained regarding sex/gender-related differences in cardiovascular disease (CVD). CVD represents the leading cause of death worldwide in both men and women, accounting for at least one-third of all deaths in women and half of deaths in women over 50 years in developing countries. Important sex-related differences in prevalence, presentation, management, and outcomes of different CVDs have been recently discovered, demonstrating sex/gender-specific pathophysiologic features in the presentation and prognosis of CVD in men and women. A large amount of evidence has highlighted the role of sex hormones in protecting women from CVDs, providing an advantage over men that is lost when women reach the menopause stage. This hormonal-dependent shift of sex-related CVD risk consequently affects the overall CVD epidemiology, particularly in light of the increasing trend of population aging. The benefits of physical activity have been recognized for a long time as a powerful preventive approach for both CVD prevention and aging-related morbidity control. Exercise training is indeed a potent physiological stimulus, which reduces primary and secondary cardiovascular events. However, the underlying mechanisms of these positive effects, including from a sex/gender perspective, still need to be fully elucidated. The aim of this work is to provide a review of the evidence linking sex/gender-related differences in CVD, including sex/gender-specific molecular mediators, to explore whether sex- and gender-tailored physical activity may be used as an effective tool to prevent CVD and improve clinical outcomes in women.In modern society, the oil industry has become the foundation of the world economy, and how to efficiently extract oil is a pressing problem. Among them, the accurate measurement of oil-gas two-phase parameters is one of the bottlenecks in oil extraction technology. It is found that through the experiment the flow patterns of the oil-gas two-phase flow will change after passing through the venturi tube with the same flow rates. Under the different oil-gas flow rate, the change will be diverse. Being motivated by the above experiments, we use the dual ECT sensors to collect the capacitance values before and after the venturi tube, respectively. Additionally, we use the linear projection algorithm (LBP) algorithm to reconstruct the image of flow patterns. This paper discusses the relationship between the change of flow patterns and the flow rates. Furthermore, a convolutional neural network (CNN) algorithm is proposed to predict the oil flow rate, gas flow rate, and GVF (gas void fraction, especially referring to sectional gas fraction) of the two-phase flow. We use ElasticNet regression as the loss function to effectively avoid possible overfitting problems. In actual experiments, we compare the Typical-ECT-imaging-based-GVF algorithm and SVM (Support Vector Machine) algorithm with CNN algorithm based on three different ECT datasets. Three different sets of ECT data are used to predict the gas flow rate, oil flow rate, and GVF, and they are respectively using the venturi front-based ECT data only, while using the venturi behind-based ECT data and using both these data.Research on emotion recognition from facial expressions has found evidence of different muscle movements between genuine and posed smiles. To further confirm discrete movement intensities of each facial segment, we explored differences in facial expressions between spontaneous and posed smiles with three-dimensional facial landmarks. Advanced machine analysis was adopted to measure changes in the dynamics of 68 segmented facial regions. A total of 57 normal adults (19 men, 38 women) who displayed adequate posed and spontaneous facial expressions for happiness were included in the analyses. The results indicate that spontaneous smiles have higher intensities for upper face than lower face. On the other hand, posed smiles showed higher intensities in the lower part of the face. Furthermore, the 3D facial landmark technique revealed that the left eyebrow displayed stronger intensity during spontaneous smiles than the right eyebrow. These findings suggest a potential application of landmark based emotion recognition that spontaneous smiles can be distinguished from posed smiles via measuring relative intensities between the upper and lower face with a focus on left-sided asymmetry in the upper region.RNA virus genomes are multifunctional entities endowed with conserved structural elements that control translation, replication and encapsidation, among other processes. Their function is ensured by the association of protein factors, but also by the establishment of complex, active, long-range RNA-RNA interaction networks-the so-called HCV RNA interactome. This review describes the RNA genome functions mediated via RNA-RNA contacts, and revisits some canonical ideas regarding the role of functional high-order structures during the HCV infective cycle. By outlining the roles of long-range RNA-RNA interactions from translation to virion budding, and the functional domains involved, this work provides an overview of the HCV genome as a dynamic device that manages the course of viral infection.Forkhead Box L2 (FOXL2) is a member of the FOXL class of transcription factors, which are essential for ovarian differentiation and function. Once cancers occupy bone, the prognosis is poor as disease recurrence and visceral spread is imminent. Understanding the pathways and cellular interactions, which regulate tumour cell seeding, dormancy and growth in bone, is pertinent to improving outcomes for patients with advanced cancers. Advances in imaging techniques have facilitated the development of the concept that the behavior of bone marrow resident cells dictates the fate of tumour cells upon arrival in bone. This review summarises recent findings achieved through intravital imaging. It highlights the importance of developing both longitudinal static and acute dynamic data to develop our understanding of tumour cell engraftment, dormancy, activation and the subsequent establishment of metastases. We also describe how imaging techniques have developed our knowledge of the elements that make up the complex bone microenvironment which tumour cells interact with to survive and grow. We also discuss how through combining these imaging insights with single cell RNA sequencing data, we are entering a new era of research which has the power to define the cell-cell interactions which control tumour cell growth in bone.

    SGA and AGA children did not differ in any auxological, body composition and bone parameters appropriately adjusted for height and FFM. BMI, FM and, remarkably, FFM were significantly correlated with bone mass parameters in both groups while no correlation was found between FM and FFM with the BW SDS in the univariate analysis.

A novel bacterial consortium CTB-4 which stands for the molecularly identified species Burkholderia sp., Xanthomonas sp., Shewanella sp., and Pseudomonas mosselii was successfully developed. The CTB-4 consortium showed high efficiency in the removal of naphthalene and phenol. It also revealed reduction in lignin, hemicellulose, and cellulose by 19.4, 52.5, and 76.8%, respectively. The main metabolites after the CRO degradation were acetic acid, succinate, pyruvate, and acetaldehyde. Pretreatment of treated BSD mixed with CRO enhanced the total methane yield (162 L/kg VS) by 82.7% and biomass reduction by 54.7% compared to the untreated substrate. CRO showed a toxicity decrease of >90%, suggesting the efficiency of constructed bacterial consortia in bioremediation and biofuel production.The aromatic nature of lignin makes it a good source for the production of numerous platform chemicals. The valorization of lignin into valuable compounds depends upon the type of bonds and functionality present in lignin. Here, we have studied the depolymerization of rice straw alkali lignin in N2 and O2 with acidic (ZSM-5), basic (MgO) catalyst and with their mixtures (11, 37 and 73). The effect of hydrogen peroxide on lignin depolymerization was also examined. Maximum yield of bio-oil (50 wt%) was obtained with pure ZSM-5 and 1 ml hydrogen peroxide in nitrogen atmosphere, while maximum conversion (60%) was observed in oxygen environment during the non-catalytic depolymerization of lignin. Bio-oil characterization through GC-MS showed maximum selectivity towards 2-methoxy-4-vinylphenol with 38.5 area% in the bio-oil of ZSM-5-N2.   • The success of osteosarcoma treatment strategies improved survival rates. The need of diagnosing and managing adverse effects is increasing. We aimed to investigate the outcomes and late results of pediatric osteosarcoma treatment in the survivors.

    Out of osteosarcoma patients (n=54), we assessed the long-term outcomes of survivors (n=39) diagnosed from 2002-2018. We compared the survivors' (n=39) health status (cardiac, renal, neurologic, psychiatric, physical limitations), pain, and psychosocial outcomes (education level, smoking history, and alcohol consumption, marital status, parenthood, health care services usage) with their siblings (n=77). The quality of life and overall survival of amputee and nonamputee survivors are also compared. We provided the retrospective data from the files and phone calls and used Kaplan Meier survival analysis, Ki-Kare, and t-test. Results The overall survival (OS) of children with osteosarcoma (n=54) who survived at 2 years and 5 years from the diagnosis was 90.7 and 7h conditions, physical limitations, and pain up to 16 years follow up. Follow-up clinics and clinical guidelines are required for the survivors of children with osteosarcoma.

    The aim of this study was to investigate the clinical efficacy and safety of laparoscopic radical resection through retroperitoneal and transperitoneal approaches in treating large-volume renal carcinoma.

    A total of 116 patients with large-volume (>7 cm) renal carcinoma underwent laparoscopic radical resection for renal carcinoma. Among them, 58 were treated through retroperitoneal approach (Retroperitoneal group), and 58 were treated through transperitoneal approach (Abdominal group). The levels of interleukin-6 (IL-6), IL-12 and IL-1β in the patients were compared before and after operation. Furthermore, the levels of tumor markers were explored, and the tumor recurrence and survival of the patients were followed up and recorded.

  • Tight junction and gap junction are major cell junctions that play important roles in cellular communication and structural integrity. Alterations of these junctions are known to be involved in cancer pathogenesis. Claudins and connexins are major tight and gap junction proteins, but genetic alterations of these genes have not been reported in gastric (GC) and colorectal cancers (CRC) with microsatellite instability (MSI). Claudin genes CLDN5 and CKDN6, and connexin genes GJB6 and GJB7 have mononucleotide repeats in the coding sequences that might be mutation targets in the cancers with MSI. We analyzed 79 GCs and 145 CRCs, and found CLDN5 frameshift mutations in 3 (3%) CRCs and 1 (2.9%) GC, CLDN6 frameshift mutations in 6 (6%) CRCs, GJB6 frameshift mutations in 5 (5%) CRCs and GJB7 frameshift mutation in one CRC (1%) with high MSI (MSI-H). We also analyzed intratumoral heterogeneity (ITH) of the frameshift mutations in 16 CRCs and found that CLDN6 and GJB6 frameshift mutations showed regional ITH in 2 (12.5%) and 2 (12.5%) cases, respectively. Renal biopsy could not be performed, because he needed anticoagulation therapy for venous thrombosis. Pembrolizumab was discontinued, and prednisolone was started. Hemodialysis was also started, because oliguria was not resolved, and dyspnea due to pulmonary congestion appeared even with the high dose of diuretics. Hemodialysis was successfully withdrawn within 5-week duration because of renal function recovery and increase of urine volume. Complete remission was achieved 4 months after initiating prednisolone. He has never experienced hemodialysis again and remains remission of nephrotic syndrome even the dose of prednisolone was tapered for 8 months. Renal pathology in the current case was uncertain. However, minimal change disease seemed to be a plausible cause of nephrotic syndrome with AKI because of a good response to steroid therapy and acute onset of nephrotic syndrome. In addition, renal pathology in all of the reported cases of pembrolizumab-associated nephrotic syndrome with AKI was minimal change disease.     Tight junction and gap junction are major cell junctions that play important roles in cellular communication and structural integrity. Alterations of these junctions are known to be involved in cancer pathogenesis. Claudins and connexins are major tight and gap junction proteins, but genetic alterations of these genes have not been reported in gastric (GC) and colorectal cancers (CRC) with microsatellite instability (MSI). Claudin genes CLDN5 and CKDN6, and connexin genes GJB6 and GJB7 have mononucleotide repeats in the coding sequences that might be mutation targets in the cancers with MSI. We analyzed 79 GCs and 145 CRCs, and found CLDN5 frameshift mutations in 3 (3%) CRCs and 1 (2.9%) GC, CLDN6 frameshift mutations in 6 (6%) CRCs, GJB6 frameshift mutations in 5 (5%) CRCs and GJB7 frameshift mutation in one CRC (1%) with high MSI (MSI-H). We also analyzed intratumoral heterogeneity (ITH) of the frameshift mutations in 16 CRCs and found that CLDN6 and GJB6 frameshift mutations showed regional ITH in 2 (12.5%) and 2 (12.5%) cases, respectively. Our results show that CLDN5, CLDN6, GJB6 and GJB7 genes harbor not only frameshift mutations but also mutational ITH, which together may be features of GC and CRC with MSI-H. Renal biopsy could not be performed, because he needed anticoagulation therapy for venous thrombosis. Pembrolizumab was discontinued, and prednisolone was started. Hemodialysis was also started, because oliguria was not resolved, and dyspnea due to pulmonary congestion appeared even with the high dose of diuretics. Hemodialysis was successfully withdrawn within 5-week duration because of renal function recovery and increase of urine volume. Complete remission was achieved 4 months after initiating prednisolone. He has never experienced hemodialysis again and remains remission of nephrotic syndrome even the dose of prednisolone was tapered for 8 months. Renal pathology in the current case was uncertain. However, minimal change disease seemed to be a plausible cause of nephrotic syndrome with AKI because of a good response to steroid therapy and acute onset of nephrotic syndrome. The two techniques conducted were open procedures using Twin Tail Tightrope system (Group A, n = 30) and Endobutton system (Group B, n = 30). 60 patients were followed up at least two years. Surgical parameters including incision length, operation time and operative blood loss were analyzed. Functional outcomes were evaluated using the Constant-Murley Score. Radiological results were assessed based on coracoclavicular distance preoperatively, one day postoperatively, and at the final follow-up. 60 patients were followed up for at least 24 months (range 24 to 32). The incision length and operation time were shorter in Group A than that in Group B. The blood loss of surgery was significantly less in the Group A. There were no significant differences between the two groups regarding the Constant-Murley Score at the final follow-up. No significant differences were found in the coracoclavicular distance preoperatively, immediately postoperatively, and at the final follow-up.   • BACKGROUND Parents' prenatal mental representations (i.e., thoughts and expectations) of their future child and relationship to that child have been associated with parenting and parent-child relationships after birth. OBJECTIVE To explore how prenatal care providers contribute to parents' mental representations of the baby they are expecting. METHODS Routine prenatal ultrasounds of 22 pregnant women recruited through prenatal care were observed. Detailed notes were taken using an adaptation of the "Observation of Routine Screen Form" (Boukydis, 2006). Data collection included interaction among parents and providers relevant to the relational, rather than medical, aspect of the exam (e.g., comments on the "personality" of the fetus, speculation about how the future baby will be like and unlike parents). Principles of grounded theory informed thematic analysis of the data. FINDINGS Providers varied widely in their recognition of the relational aspect of prenatal ultrasound and their interactive style. Through informal interactions during ultrasounds, providers alternately inhibited, amplified, and shaped parents' mental representations of their baby. KEY CONCLUSIONS AND IMPLICATIONS FOR PRACTICE The manner in which providers narrate and interpret images has implications for parents' prenatal mental representations of the baby. Given the importance of prenatal representations for future parenting and parent-child relationships, providers should attend to and facilitate parents' efforts to develop their own mental representations and establish feelings of connection to the baby. Some clinical and pharmacogenetic data suggest that obese patients have altered pain sensitivity and analgesic requirements. OBJECTIVE To test the primary hypothesis that increased BMI in pediatric patients is associated with increased pain during the initial 48 postoperative hours. Secondarily, we tested whether BMI is associated with increased opioid consumption during the same period. DESIGN Retrospective single-center cohort study. SETTING Pediatric surgical wards in a tertiary medical center. PATIENTS A total of 808 opioid naïve patients aged 8 to 18 years having elective non-cardiac surgery with hospital stay of at least 48 h in the Cleveland Clinic between 2010 and 2015. INTERVENTIONS None. MEASUREMENTS Using U.S. Centers for Disease Control definitions for childhood weight classifications, we retrospectively evaluated the association between body mass index (BMI) percentile and time-weighted average pain scores and opioid consumption. We used multivariable linear regression to test for an association with postoperative pain scores, and multivariable gamma regression to test for an association with postoperative opioid consumption (in mg morphine equivalents Kg-1). RESULTS BMI was not associated with postoperative pain after general, orthopedic, or neuro-spinal surgeries. Pain increased by 0.07 [98.75% CI (0.01, 0.13), Padj  less then  0.05] points per 5 percentile increase in BMI after neuro-cranial surgery. Higher BMI was associated with a decrease in postoperative opioid consumption (mean change [95% CI] -2.12% [-3.12%, -1.10%] in morphine equivalents Kg-1 per 5 percentile increase in BMI, P  less then  0.001). CONCLUSION We found no clinically important increase in pain scores or opioid consumption in association with higher BMI in patients 8 to 18 years of age recovering from elective non-cardiac surgery. Cervix cancer is traditionally diagnosed in younger women but it has a 2nd incidence peak between 60 and 70 years old. Cervix cancer in elder patients is a subject to many questions in terms of screening and is a therapeutic challenge. This article reviews literature data on these different aspects, from screening to surgery, from radiotherapy to brachytherapy, from chemotherapy to supportive care, from immunotherapy to geriatric assessment. We tried to show how modern therapeutic innovations may benefit elder patients. Expected benefits in terms of efficacy and toxicity may overcome the long-lasting tendency to undertreatment in elder patients and improve their quality of life after cancer treatment.

This article reviews literature data on these different aspects, from screening to surgery, from radiotherapy to brachytherapy, from chemotherapy to supportive care, from immunotherapy to geriatric assessment. We tried to show how modern therapeutic innovations may benefit elder patients. Expected benefits in terms of efficacy and toxicity may overcome the long-lasting tendency to undertreatment in elder patients and improve their quality of life after cancer treatment.

The tumor cells were interspersed with slit-like thin-walled vessels and scattered short nerve bundles. Immunohistochemically, the tumor cells were positive for smooth muscle actin, h-caldesmon, and CD34 (focal), but completely negative for HMB45, S100, EMA, desmin, DOG-1, and CD117. The histologic features and immunohistochemical profile supported a diagnosis of primary colonic glomus tumor.

  • Anti-tubercular treatment was routinely administered (6-9 months) with drug combinations of Isoniaside, Rifampicin, Pyrazinamide and Ethambutol. The evolution was favorable in all cases without complications or recurrences. Conclusion Drainage of chest wall abscess and complete debridement provide adequate treatment. Methods We extended our work by analyzing early functional magnetic resonance imaging (fMRI) biomarkers of the default mode network in older African Americans and Caucasians. We calculated connectivity between nodes of the regions belonging to the various default mode network subsystems and correlated these imaging biomarkers with non-imaging biomarkers implicated in AD (CSF amyloid, total tau, and cognitive performance). Results We found that race modifies the relationship between functional connectivity of default mode network subsystems and cognitive performance, tau, and amyloid levels. Conclusion These findings provide further support that race modifies the AD phenotypes downstream from cerebral amyloid deposition, and identifies key inter-subsystem connections for deep imaging and neuropathologic characterization. © The Author(s). 2020.Background Eczema phenotypes based on eczema onset and persistence might better identify groups prone to allergic and respiratory conditions than a binary definition of eczema. We examined the associations of childhood eczema phenotypes with allergic sensitization, allergy, asthma and lung function at school age. Methods This study among 4277 children was embedded in a multi-ethnic population-based prospective cohort study. Five eczema phenotypes (never, early transient, mid-transient, late transient, persistent) based on parental-reported physician-diagnosed eczema from age 6 months until 10 years were identified. At age 10 years, allergic sensitization was measured by skin ***** tests, physician-diagnosed allergy and asthma by parent-reported questionnaires, and lung function by spirometry. Adjusted linear, logistic and multinomial regression models were applied. Results Compared with never eczema, all eczema phenotypes were associated with increased risks of asthma (odds ratios (OR) range (95% confidence interval) 2.68 (1.58, 4.57) to 11.53 (6.65, 20.01)), food and inhalant allergic sensitization (1.72 (1.25, 2.36) to 12.64 (7.20, 22.18)), and physician-diagnosed inhalant allergy (1.92 (1.34, 2.74) to 11.91 (7.52, 18.86)). Strongest effect estimates were observed of early and persistent eczema with the risk of physician-diagnosed food allergy (OR 6.95 (3.76, 12.84) and 35.05 (18.33, 70.00), respectively) and combined asthma and physician-diagnosed allergy (7.11 (4.33, 11.67) and 29.03 (15.27, 55.22), respectively). Eczema phenotypes were not associated with lung function measures. Conclusion Eczema phenotypes were differentially associated with risks of respiratory and allergic conditions in school-aged children. It is characterized by the accumulation of air inside the bladder wall and/or lumen and is common among elderly diabetic females. This case of a 90-year-old female, who underwent bilateral iliac angioplasty and stenting, is the first published correlation between iliac angioplasty and the development of EC. The endovascular procedure performed was a success, but later on, she was confused and developed frank haematuria with a fall in haemoglobin levels. CT revealed severe EC with abundant gas tracking outside of urinary bladder into the extraperitoneal space. She was catheterized and empirical antibiotics were started and converted based on the culture sensitivities. Frequent follow-up ensured the resolution of a conservatively managed case. It is essential to increase the awareness of EC associated with angioplasties among the healthcare staff, particularly those undertaking vascular procedures.
