    To report the first case of transoral excision of a paraglottic laryngeal schwannoma.

    Case report.

    Laryngeal schwannomas are rare tumors, with approximately 75 cases reported to date worldwide. Most cases involve superficial supraglottic tumors excised transorally, and some have been found in deeper compartments and treated using transcervical approaches. This report describes the first excision of a schwannoma of the paraglottic space via endoscopic laser laryngotomy. The procedure allowed return to normal function, without recurrence within 8 months, after which the patient was lost to follow-up.

Past work on migration-infection interactions assumes migration is the only strategy available to organisms for dealing with the parasite infection, that is they migrate to a different environment to recover or escape from infection. Thus, migration is similar to the non-spatial strategy of resistance, where hosts prevent infection or kill parasites once infected. However, an alternative defence strategy is to tolerate the infection and experience a lower cost to the infection. To our knowledge, no studies have examined how migration can change based on combining two host strategies (migration and tolerance) for dealing with parasites. In this paper, we aim to understand how both parasite transmission and infection tolerance can influence the host's migratory behaviour. We constructed a model that incorporates two host strategies (migration and tolerance) to understand whether allowing for tolerance affects the proportion of the population that migrates at equilibrium in response to infection. We show that the benefits of tolerance can either decrease or increase the host's migration. Also, if the benefit of migration is great, then individuals are more likely to migrate regardless of the presence of tolerance. Finally, we find that the transmission rate of parasite infection can either decrease or increase the tolerant host's migration, depending on the cost of migration. These findings highlight that adopting two defence strategies is not always beneficial to the hosts. Instead, a single strategy is often better, depending on the costs and benefits of the strategies and infection pressures. Our work further suggests that multiple host-defence strategies as a potential explanation for the evolution of migration to minimize the parasite infection. Moreover, migration can also affect the ecological and evolutionary dynamics of parasite-host interactions.
    Advance care planning (ACP), in which patients or their surrogates discuss goals and preferences for care with physicians, attorneys, friends, and family, is an important approach to help align goals with actual treatment. ACP may be particularly valuable in patients with advanced serious illnesses such as Alzheimer's disease and related dementias (ADRDs) for whom surgery carries significant risks.

    To determine the frequency, timing, and factors associated with ACP billing in Medicare beneficiaries with ADRD undergoing nontrauma inpatient surgery.

We use Pakistan Demographic and Health Survey to evaluate FALAH's impact on continuous and binary measures of birth intervals. To estimate the causal effects of the project, we compare the outcomes for multiple children born to the same mother before and after the project. We find that FALAH increased interbirth intervals by 2.4 months on average and reduced the proportion of short birth intervals by approximately 7.1 percentage points. This finding suggests that birth spacing as a goal of contraceptive use may resonate better with Pakistani couples than limiting family size. The project's effects were more pronounced for women with high education, in rural areas, and in the middle of the wealth distribution.

Notably, it is very rare that grade 4 CRS related to ICI therapy overlaps with the drug-induced hypersensitivity syndrome (DiHS). A 46-year old woman with metastatic kidney cancer had grade 3 interstitial pneumonitis induced by four cycles of combination therapy of anti-programmed death-1 and anti-cytotoxic T lymphocyte-4 antibodies after right cytoreductive nephrectomy. Prophylactic administration of trimethoprim/sulfamethoxazole (TMP/SMX) was started concomitantly with prednisolone therapy to treat the interstitial pneumonitis. She developed hypotensive shock when reducing the dosage of prednisolone, and required intubation and ventilation using vasopressors at the intensive care unit. She subsequently exhibited prominent leukocytosis and an increased level of C-reactive protein, suggesting markedly increased cytokine levels. Interestingly, facial edema and erythema increased in association with pyrexia, leukocytosis, liver dysfunction, and renal failure, suggesting that she developed DiHS. She received hemodialysis three times, a plasma exchange, and anti-interleukin-6 therapy to treat severe renal dysfunction, a thrombotic thrombocytopenic purpura-suspected condition, and possible grade 4 CRS, respectively. Although these therapies did not elicit sufficient effects, high-dose administration of intravenous immunoglobulin was successful. With steroid mini-pulse therapy and the subsequent administration of prednisolone, she recovered successfully. To the best of our knowledge, this is the first report that ICIs and TMP/SMX can induce hypotensive shock accompanied with CRS and DiHS during immunosuppressive therapy for an irAE. Importantly, the prophylactic administration of TMP/SMX should be performed cautiously to avoid severe drug reactions such as CRS or DiHS.
    We investigated the prognostic effects and their patterns of immune infiltration of hippo pathway core genes in lung squamous cell carcinoma, in order to find some clues for underlying mechanisms of LUSC tumorigenesis and help developing new therapeutic methods.

    The mutational data, transcriptome data and corresponding clinical medical information of LUSC patients were extracted from The Cancer Genome Atlas (TCGA) database. Differential expression genes (DEGs) and Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were explored. Survival analysis for the hippo core genes and the prognostic model were performed. Immune infiltration was estimated by CIBERSORT algorithm and some immune checkpoints-related genes were further investigated.

This search was conducted for studies published between 1 January, 2000 and 31 December, 2020.A random-effects meta-analysis was performed to analyze the pooled incidence and odds ratios of VTE in patients with different driver genes.

However, histological analysis revealed no viable cancer despite the evidence of radiological progression. Instead, extensive necrosis and lymphohistiocytic infiltration were noted, as described previously in patients with ICI-induced pseudoprogression. Based on these observations, we postulate that exuberant antitumor inflammatory responses may contribute to adverse clinical outcomes in some patients with ICI-induced radiographic progression. Prospective studies incorporating tumor biopsies may shed more light on this rare phenomenon.

  • The US Food and Drug Administration (FDA) uses 510(k) clearance and premarket approval (PMA) pathways to ensure device safety before marketing. Premarket approval evaluates high-risk medical devices and requires clinical trials, whereas 510(k) clearance evaluates moderate-risk devices and relies on benchtop (nonclinical and biomechanical) and descriptive data. Existing literature suggests that the clinical trials required by PMA are associated with reduced risk of recall compared with devices granted 510(k) clearance. Several investigators have found weaknesses in pivotal PMA trials, raising safety concerns. Furthermore, methodological factors may have led to a previous underestimation of recall risk for devices with PMA.

    To compare risk of recall and high-risk recall between devices that received 510(k) clearance and those that received PMA and to compare the risk of recall between devices for medical specialties.

    To examine the extent, variability, associated factors, and costs of low-value tests (LVTs) prior to cataract operations in the VHA.

    This cohort study examined records of all patients receiving cataract operations within the VHA in fiscal year 2017 (October 1, 2016, to September 31, 2017). Records from 135 facilities nationwide supporting both ambulatory and inpatient surgery were included.

Premarket approval evaluates high-risk medical devices and requires clinical trials, whereas 510(k) clearance evaluates moderate-risk devices and relies on benchtop (nonclinical and biomechanical) and descriptive data. Existing literature suggests that the clinical trials required by PMA are associated with reduced risk of recall compared with devices granted 510(k) clearance. Several investigators have found weaknesses in pivotal PMA trials, raising safety concerns. Furthermore, methodological factors may have led to a previous underestimation of recall risk for devices with PMA.     Resection of the hepatocellular carcinoma (HCC) in the caudate lobe (CL) is challenging even for accomplished surgeons. This retrospective study evaluated the safety and efficacy of transarterial chemoembolization (TACE) and iodine 125 seeds implantation (ISI) for unresectable or "ablation unsuitable" HCC-CL detected at the initial presentation in clinical practice.

    A total of 20 HCC-CL patients undergoing sequential TACE and ISI from January 2014 to October 2018 were enrolled in this study. The overall survival (OS), progression-free survival (PFS), tumor response rate, and complication rates were analyzed and compared to non-caudate lobe (NCL) HCC patients. Multivariate analyses for potential clinical and radiological factors were performed using the Cox proportional hazard model.

    The technical success rate was 100%, as all the patients received 28 ISI treatments. The median OS was 35 months. The 1-, 3-, and 5-year OS rates were 100%, 63.2%, and 11.1%, respectively. The median PFS was 16 months. The objective response rate was 60.0%. The puncture tract bleeding (2/20) and pneumothorax (1/20) were the most common complications in operation, but no operation-related deaths occurred. One year after the surgery, biliary tract injury occurred in 1 patient, necessitating percutaneous biliary intervention. No statistical difference was observed between the CL and NCL groups. Cases and controls were then randomly divided into two groups, with two thirds for building the risk prediction model and the other one third for model validation. Known risk factors were included for risk prediction models using logistic regressions.     Based on the leakage of the coalbed methane (CBM) drilling engineering practice of Luan mining area in China, the author determines the safe drilling fluid density range for the stable borehole wall based on borehole wall collapse and fracture pressure. Feeding intensity as represented by a fullness index (FI), changing with various length groups and season. Out of 185 Brown trout 24 fish stomaches were found empty. The most important and common food items of brown trout were Brachycentridae, Blepharocera, Hydropsychidae, ephemerella spp. Kruskal Wallis H-test were applied on feeding intensity groups consisting of three month group. The test statistic for K.W-H-test were (H=8.13 with df =3) had a p-value of 0.043 less then (alpha=0.05 indicates favor of the alternative hypothesis of at least one difference among the feeding intensity groups.   • The EXIT-to-airway procedure is aimed to provide the time required to secure airways when an extrinsic or intrinsic fetal mass raise concerns about airways control at delivery. Due to the rarity of the procedure, we aim to provide a summary of the appropriate prenatal planning by a multidisciplinary team.

    Report of a case of EXIT-to-airway procedure.

    A 30 years-old woman, G2P1 with previous cesarean section, was referred to our Unit at 34 gestational weeks due to a fetal cervical mass of 7cm. An EXIT-to-airways procedure was performed by a multidisciplinary team after accurate preoperative planning and the practice simulations. The partial fetal extraction and the amnioinfusion of pre-heated saline were used to prevent fetal complications. The use of supplemental intravenous anesthesia with remifentanil and better control of uterine tone with nitroglycerin allowed to reduce the exposure to volatile halogen for both the mother and the fetus. The accurate preoperative planning and the practice simulations allowed us to perform the treatment safely in urgency due to the onset of spontaneous labor at 37 weeks and 6 days.

    Diagnosis of walnut allergy includes the evaluation of IgE sensitization by skin ***** tests (SPT) with standardized commercial extracts. When assuming the loss of relevant allergens due to extract preparation and storage, it is possible to perform SPT with fresh foods, i.e., ***** by ***** (PbP). To our knowledge, there is no published comparison between SPT with commercial extracts and PbP with fresh food about their sensitivity to the diagnosis of walnut allergy. Therefore, we describe our experience.

    We observed seven children (mean age + SD 6.8 years + 5.2, range 2 - 15 years; male 85%) with an history of immediate adverse reaction following walnut ingestion. All but one the patients underwent SPT with at least two out of three walnut commercial extracts (Lofarma, Milan, Italy; ALK-Abellò, Milan, Italy; Allergopharma, Rome, Italy). It has also been performed PbP with raw walnut. IgE-mediated walnut allergy was diagnosed based on suggestive history, positivity of PbP and failed open food challenge with walnut.

    The SPT with Lofarma extract was never positive (sensitivity = 0%), that performed with ALK extract was positive in 2/5 cases (sensitivity 40%) and that of Allergopharma extract was positive in 1/5 cases (sensitivity 20%). PbP was positive in 7/7 cases (sensitivity 100%).

    In the specific case of walnut allergy in pediatric age, the execution of SPT alone with commercial extract may not be sufficient and clarifying in the diagnostic iter. We suggest to always associate the execution of PbP test.

Detailed questioning revealed significant exposure to incense smoke, highlighting a rare presentation of non-cigarette smoke-related PLCH. Avoidance of incense smoke combined with oral prednisolone (0.5mg/kg) tapered over 6 months led to complete resolution of symptoms, disappearance of nodules in high resolution computed tomography (HRCT) of the thorax and improvement in lung function.Laparoscopic splenectomy is nowadays widely performed for the treatment of benign and malignant diseases of the spleen.     Finding new therapeutic uses for existing medicines could lead to safe, affordable and timely new treatment options for patients with high medical needs. However, due to a lack of economic incentives, pharmaceutical developers are rarely interested to invest in research with approved medicines, especially when they are out of basic patent or regulatory protection.     Uveal melanoma (UM) represents the most common primary intra-ocular malignancy in adults. Up to 50% of the patients develop distant metastases within 10 years from diagnosis, with the liver as the most common site. Upon metastatization, life expectancy strongly reduces and immune checkpoint inhibitors that prove effective in cutaneous melanoma do not modify clinical outcome. To date, few studies have focused on deciphering the immunomodulatory features of metastatic UM microenvironment, and there are no prognostic models for clinical use. This highlights the urgent need to understand the delicate interplay between tumor and immune cells acting at the site of metastasis.

    We collected a patient cohort comprising 21 metastatic UM patients. Hepatic and extra-hepatic UM metastasis samples were studied by multiplex immunofluorescence to assess the tumor immune cell composition. Consequently, potential new uses for these medicines are mainly studied in independent clinical trials initiated and led by researchers from academia, research institutes, or collaborative groups. Uveal melanoma (UM) represents the most common primary intra-ocular malignancy in adults. Up to 50% of the patients develop distant metastases within 10 years from diagnosis, with the liver as the most common site. Upon metastatization, life expectancy strongly reduces and immune checkpoint inhibitors that prove effective in cutaneous melanoma do not modify clinical outcome. To date, few studies have focused on deciphering the immunomodulatory features of metastatic UM microenvironment, and there are no prognostic models for clinical use. This highlights the urgent need to understand the delicate interplay between tumor and immune cells acting at the site of metastasis. We collected a patient cohort comprising 21 metastatic UM patients.   • Allergy is a disorder owing to hyperimmune responses to a particular kind of substance like food and the disease remains a serious healthcare burden worldwide. This unpleasant and sometimes fatal allergic disease has been tackled vigorously by allergen-specific immunotherapy over a century, but the progress made so far is far from satisfactory for some allergies. Herein, we introduce innovative, allergen powder-based epicutaneous immunotherapies (EPIT), which could potentially serve to generate a new stream of technological possibilities that embrace the features of super safety and efficacious immunotherapy by manipulating the plasticity of the skin immune system via sufficient delivery of not only allergens but also tolerogenic adjuvants. We attempt to lay a framework to help understand immune physiology of the skin, epicutaneous delivery of powdered allergy, and potentials for tolerogenic adjuvants. Preclinical and clinical data are reviewed showing that deposition of allergen powder into an array of micropores in the epidermis can confer significant advantages over intradermal or subcutaneous injection of aqueous allergens or other epicutaneous delivery systems to induce immunological responses toward tolerance at little risk of anaphylaxis. Finally, the safety, cost-effectiveness, and acceptability of these novel EPITs are discussed, which offers the perspective of future immunotherapies with all desirable features.Despite the available antibiotics, tuberculosis (TB) has made its return since the 90's of the last century as a global threat mostly due to co-infection with HIV, to the emergence of drug resistant strains and the lack of an effective vaccine. Among the available drugs to control HIV infection, protease inhibitors (PIs), acting at post-integrational stages of virus replication cycle, are the only drugs able to interfere with virus production and release from macrophages during chronic infection. For Mtb we recently found that the pathogen induces a general down-regulation of lysosomal proteases, helping bacteria to establish an intracellular niche in macrophages. Here we found that the PI saquinavir, contrary to ritonavir, is able to induce an increase of endolysosomal proteases activity especially of cathepsin S in Mtb infected macrophages and during co-infection with HIV. Our results indicate that saquinavir treatment of infected macrophages led not only to a significant intracellular killing of Mtb but also (i) to an improved expression of the HLA class II antigen presentation machinery at the cell surface; (ii) to increased T-lymphocyte priming and proliferation; and (iii) to increased secretion of IFN-γ. Plasma SO levels and drowsiness duration were assessed.

    No significant changes were detected in [C-11]raclopride binding in striatum overall 1 or 7 h after SO, but a small non-significant increase in [C-11]raclopride binding, implying decreased dopamine occupancy, was noted in limbic striatal subdivision at one hour (+6.5%; p uncorrected=0.045; +13.2%, narcolepsy participant), returning to baseline at 7 h.     SARS-CoV-2 RNA has been recovered from different sites in the human body, including the mouth. The present study aimed to investigate the presence of SARS-CoV-2 RNA in the dental biofilm of symptomatic patients who tested positive in nasopharyngeal and oropharyngeal (NASO/ORO) samples.

    An observational clinical study of individuals with flu-like symptoms was conducted between July and September 2020. Dental biofilm (BIO) samples were collected and analysed using real-time quantitative polymerase chain reaction (RT-qPCR) to determine the virus's presence.

    Seventy participants (40±9.8years of age, 71.4% female) tested positive for SARS-CoV-2 RNA in NASO/ORO samples and were included in the study. Among them, 13 tested positive in BIO samples (18.6%; 95% CI [9.5, 27.7]). The median and interquartile range of cycle quantification (Cq) for NASO/ORO and BIO samples were 15.9 [6.9] and 35.9 [4.0] (p=.001), respectively. BIO-positive participants showed a higher virus load in NASO/ORO samples (p=.012) than those testing negative (Cq=20.4 [6.1]).

The present study aimed to quantify the effects of different dimensions of smartphone behaviours (frequency of daytime use, problematic use, use before sleep and use during the sleep period) on disturbed sleep (sleep quality and sleep quantity) and to disentangle their inter-relationship in a large population-based sample of 24,856 Danish adults aged ≥16 years. Data come from the SmartSleep Experiment, which is a web-based survey carried out using a citizen science approach. Tested items were used to evaluate smartphone use and disturbed sleep was evaluated with the Karolinska Sleep Questionnaire (KSQ). Linear and multinomial logistic regression was employed to evaluate the relationship between smartphone use and disturbed sleep. While several of the smartphone measures were associated with disturbed sleep when assessed individually, smartphone use during the sleep period was the only dimension consistently associated with disturbed sleep when assessed independently of other smartphone behaviours. Weekly smartphone use during the sleep period versus no use was associated on average with a 0.96 point higher score (95% confidence interval [CI] 0.90-1.02) on the 5-point KSQ scale, and a higher risk of both short (odds ratio [OR] 1.32, 95% CI 1.08-1.62) and long (OR 1.94, 95% CI 1.63-2.32) sleep duration. Smartphone use during the sleep period is the factor strongest associated to disturbed sleep relative to other dimensions of smartphone use. Recommendations around smartphone use during the sleep period are warranted in order to protect the fundamentally important biological and mental processes of sleep.This study aimed to evaluate the effect of polyphenol (PE) and avenanthramide (AE) extracts from oat grains (OG) and sprouts (OS) on genes related to glucose and lipid metabolisms in 3T3 L1 adipocytes. The AE-OS exerted the greatest effect on genes involved in glucose metabolism, increasing Glut4, Irs1, and Pi3k expression by 3.0- to 3.9-fold. Conversely, the PE-OS exerted the greatest effect on genes involved in lipid metabolism, decreasing Fasn and Acaca expression by 0.2- to 0.3-fold, and increasing Cpt1a and Acadm expression by 2.7- to 3.0-fold. These effects were mainly related to their high content of avenanthramides A (2p), B (2f), and C (2c), quercetin 3-O-rutinoside, kaempferol, sinapoylquinic acid, and apigenin and luteolin derivatives according to the chemometric analysis. In conclusion, this study demonstrated that oat sprouts extract exerts a greater effect than oat grains on the regulation of genes involved in glucose and lipid metabolisms in adipocytes. PRACTICAL APPLICATIONS This study demonstrates that polyphenols and avenanthramides extracted from oat (Avena sativa L.) grains and sprouts modulate key genes involved in glucose and lipid metabolisms in adipocytes and that oat sprouts exert a greatest health beneficial effect than oat grains due to their higher content of bioactive compounds. In addition, the chemometric analysis identified the bioactive compounds that can be associated with the beneficial effects of oat grains and sprouts, which can be further used for the identification of oat varieties and oat-derived products with high content of these bioactive compounds and, thus, with high nutraceutical potential.

SARS-CoV-2 RNA has been recovered from different sites in the human body, including the mouth. The present study aimed to investigate the presence of SARS-CoV-2 RNA in the dental biofilm of symptomatic patients who tested positive in nasopharyngeal and oropharyngeal (NASO/ORO) samples. An observational clinical study of individuals with flu-like symptoms was conducted between July and September 2020. Dental biofilm (BIO) samples were collected and analysed using real-time quantitative polymerase chain reaction (RT-qPCR) to determine the virus's presence. Seventy participants (40±9.8years of age, 71.4% female) tested positive for SARS-CoV-2 RNA in NASO/ORO samples and were included in the study. Among them, 13 tested positive in BIO samples (18.6%; 95% CI [9.5, 27.7]). The median and interquartile range of cycle quantification (Cq) for NASO/ORO and BIO samples were 15.9 [6.9] and 35.9 [4.0] (p=.001), respectively. BIO-positive participants showed a higher virus load in NASO/ORO samples (p=.012) than those testing negative (Cq=20.4 [6.1]). The present study aimed to quantify the effects of different dimensions of smartphone behaviours (frequency of daytime use, problematic use, use before sleep and use during the sleep period) on disturbed sleep (sleep quality and sleep quantity) and to disentangle their inter-relationship in a large population-based sample of 24,856 Danish adults aged ≥16 years. Data come from the SmartSleep Experiment, which is a web-based survey carried out using a citizen science approach. Tested items were used to evaluate smartphone use and disturbed sleep was evaluated with the Karolinska Sleep Questionnaire (KSQ). Linear and multinomial logistic regression was employed to evaluate the relationship between smartphone use and disturbed sleep. While several of the smartphone measures were associated with disturbed sleep when assessed individually, smartphone use during the sleep period was the only dimension consistently associated with disturbed sleep when assessed independently of other smartphone behaviours. Weekly smartphone use during the sleep period versus no use was associated on average with a 0.96 point higher score (95% confidence interval [CI] 0.90-1.02) on the 5-point KSQ scale, and a higher risk of both short (odds ratio [OR] 1.32, 95% CI 1.08-1.62) and long (OR 1.94, 95% CI 1.63-2.32) sleep duration. Smartphone use during the sleep period is the factor strongest associated to disturbed sleep relative to other dimensions of smartphone use. Recommendations around smartphone use during the sleep period are warranted in order to protect the fundamentally important biological and mental processes of sleep.This study aimed to evaluate the effect of polyphenol (PE) and avenanthramide (AE) extracts from oat grains (OG) and sprouts (OS) on genes related to glucose and lipid metabolisms in 3T3 L1 adipocytes. The AE-OS exerted the greatest effect on genes involved in glucose metabolism, increasing Glut4, Irs1, and Pi3k expression by 3.0- to 3.9-fold. Conversely, the PE-OS exerted the greatest effect on genes involved in lipid metabolism, decreasing Fasn and Acaca expression by 0.2- to 0.3-fold, and increasing Cpt1a and Acadm expression by 2.7- to 3.0-fold. These effects were mainly related to their high content of avenanthramides A (2p), B (2f), and C (2c), quercetin 3-O-rutinoside, kaempferol, sinapoylquinic acid, and apigenin and luteolin derivatives according to the chemometric analysis. In conclusion, this study demonstrated that oat sprouts extract exerts a greater effect than oat grains on the regulation of genes involved in glucose and lipid metabolisms in adipocytes. PRACTICAL APPLICATIONS This study demonstrates that polyphenols and avenanthramides extracted from oat (Avena sativa L.) grains and sprouts modulate key genes involved in glucose and lipid metabolisms in adipocytes and that oat sprouts exert a greatest health beneficial effect than oat grains due to their higher content of bioactive compounds. In addition, the chemometric analysis identified the bioactive compounds that can be associated with the beneficial effects of oat grains and sprouts, which can be further used for the identification of oat varieties and oat-derived products with high content of these bioactive compounds and, thus, with high nutraceutical potential.
