We present the development of a PET insert system for potential simultaneous PET/MR imaging using a 9.4 T small animal MRI scanner to test our system. The detectors of the system adopt a strip-line based multiplexing readout method for SiPM signals. In this readout, multiple SiPM outputs in a row share a common strip-line. The position information about a hit SiPM is encoded in the propagation time difference of the signals arriving at the two ends of the strip-line. The axial field of view of the prototype is 25.4 mm. The strip-lines were extended by using 5-meter cables to a sampling data acquisition (DAQ) board placed outside the magnet. The detectors were not shielded in the interest of investigating how they may affect and be affected by the MRI. Experimental tests were conducted to evaluate detection performance, and phantom and animal imaging were carried out to assess the spatial resolution and the MR compatibility of the PET insert. Initial results are encouraging and demonstrate that the prototype insert PET can potentially be used for PET/MR imaging if appropriate shielding will be implemented for minimizing the mutual interference between the PET and MRI systems.One Welfare describes the interconnection between animal welfare, human wellbeing and their physical and social environment. The SARS-CoV-2 virus is the cause of COVID-19 and emerged as a human pathogen in 2019 although is thought to have a zoonotic source. The original wildlife reservoir and any potential intermediate hosts have not yet been identified. The combination of the virus zoonotic condition together with the impacts of disease control measures has exposed clear interconnections between animals, people and their environment from both a health and a welfare perspective. The One Welfare Framework comprises five sections that can help understand the different One Welfare levels on which the COVID-19 pandemic has impacted the world. This paper uses the One Welfare Framework to provide an overview of examples, within each of the five sections, where evidence is and/or can be made available to document COVID-19 impacts on One Welfare. The paper identifies a number of areas where further research and evidence gathering is required to better understand the different One Welfare impacts. Based on evidence summarised in this paper the author recommends that those responsible for managing the COVID-19 impacts and for planning the future recovery phase of the pandemic should consider adopting a holistic approach, including both health and welfare, by adopting & One Health, One Welfare & policies.The emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) as a novel coronavirus and the etiological agent of global pandemic coronavirus disease (COVID-19) requires quick development of potential therapeutic strategies. Computer aided drug design approaches are highly efficient in identifying promising drug candidates among an available pool of biological active antivirals with safe pharmacokinetics. The main protease (MPro) enzyme of SARS-CoV-2 is considered key in virus production and its crystal structures are available at excellent resolution. This marks the enzyme as a good starting receptor to conduct an extensive structure-based virtual screening (SBVS) of ASINEX antiviral library for the purpose of uncovering valuable hits against SARS-CoV-2 MPro. A compound hit (BBB_26580140) was stand out in the screening process, as opposed to the control, as a potential inhibitor of SARS-CoV-2 MPro based on a combined approach of SBVS, drug likeness and lead likeness annotations, pharmacokinetics, molecular dynamics (MD) simulations, and end point MM-PBSA binding free energy methods. The lead was further used in ligand-based similarity search (LBSS) that found 33 similar compounds from the ChEMBL database. A set of three compounds (SCHEMBL12616233, SCHEMBL18616095, and SCHEMBL20148701), based on their binding affinity for MPro, was selected and analyzed using extensive MD simulation, hydrogen bond profiling, MM-PBSA, and WaterSwap binding free energy techniques. The compounds conformation with MPro show good stability after initial within active cavity moves, a rich intermolecular network of chemical interactions, and reliable relative and absolute binding free energies. Findings of the study suggested the use of BBB_26580140 lead and its similar analogs to be explored in vivo which might pave the path for rational drug discovery against SARS-CoV-2 MPro.Amid growing concern regarding the potential added burden of care due to population aging, we have very little understanding of what is the burden of care in aging populations. To answer this question, we introduce a novel metric that encompasses demographic complexity and social context to summarize unpaid family care work provided to children, elderly, and other family members across the life cycle at a population level. The measure (Care Life Expectancy), an application of the Sullivan method, estimates the number of years and proportion of adult life that people spend in an unpaid caregiving role. We demonstrate the value of the metric by using it to describe gender differences in unpaid care work in 23 European aging countries. We find that at age 15, women and men are expected to be in an unpaid caregiving role for over half of their remaining life. For women in most of the countries, over half of those years will involve high-level caregiving for a family member. We also find that men lag in caregiving across most countries, even when using the lowest threshold of caregiving.

These results suggest that chronic TMX exposure induces dyslipidemia and NAFLD in ****. Moreover, inhibition of NNMT in hepatocytes significantly reversed the effects of TMX. The molecular mechanism of TMX-induced NAFLD is mostly through NNMT-mediated methionine metabolism and methyl donor balance, which ultimately regulates PPARα signaling pathway. Inhibition of NNMT could be a potentially novel strategy for blocking the progression of NAFLD induced by TMX.The effects of pretreatment methods on the structure of functional groups and denitrification promotion capacity of solid carbon sources derived from reeds and cattails were elucidated. Alkaline treatment improved the relative content of carbon in the plant tissues, as well as prolonged the high denitrification rate of 0.40 mg/(L·h) from 6 days up to circa 28 days. Moreover, alkaline-heated cattails (ALH-C) showed high denitrification promotion capacity, and increased the removal rate of TN, NO3--N and NH4+-N in the CW by 24.41%, 31.80% and 8.80%, respectively. Furthermore, the quantity, quality and migration of dissolved organic matter (DOM) released from ALH-C in CW analyzed via fluorescence excitation-emission matrix spectrophotometry showed mainly humic acid-like, tyrosine-like, and tryptophan-like components. These DOM components were highly bioavailable and had minimal effects on COD removal. These results provide insights into the preparation and environmental applications of plant carbon sources.Over the past decades, inspired by the outstanding properties of clay minerals such as abundance, low-cost, environmental benignity, high stability, and regularly arranged silica-alumina framework, researchers put **** efforts on the interface assembly and surface modification of natural minerals with bare photocatalysts, i.e. TiO2, g-C3N4, ZnO, MoS2, etc. The clay-based hybrid photocatalysts have resulted in a rich database for their tailor-designed microstructures, characterizations, and environmental-related applications. Therefore, in this study, we took a brief introduction of three representative minerals, i.e. kaolinite, montmorillonite and rectorite, and discussed their basic merits in photocatalysis applications. After that, we summarized the recent advances in construction of stable visible-light driven photocatalysts based on these minerals. The structure-activity relationships between the properties of clay types, pore structure, distribution/dispersion and light absorption, carrier separation efficiency as well as redox performance were illustrated in detail. Such representative information would provide theoretical basis and scientific support for the application of clay based photocatalysts. Finally, we pointed out the major challenges and future directions at the end of this review. Undoubtedly, control and preparation of novel photocatalysts based on clays will continue to witness many breakthroughs in the arena of solar-driven technologies.Greenhouse gases (GHGs; particularly, CO2, CH4, and N2O) emission from wastewater treatment systems (WWTS) is one of the inevitable concerns for sustainable development. This indicator is directly linked with the carbon footprint and potential impacts of WWTS on climate change. In this view, various modeling, design, and operational tools have been introduced to mitigate the WWTS associated GHGs, at regional and global scales. In this study, authors have critically reviewed the selected potential operational control strategies for GHGs emission, particularly emitted from the operational stages of biological WWTS. The investigated operational control strategies and/or treatment configurations included intermittent aeration, varying dissolved oxygen, enhanced sludge retention time, coupled aerobic-anoxic nitrous decomposition operation, and microalgae integrated treatment process. In this work we investigated for the first time the addition of chitosan nanoparticles to the washing solution as an alternative and greener method to ensure the cleanup of contaminated sediments. Results showed 65.1 ± 6.4% of average removal from sand and seafloor sediments artificially contaminated at 4 wt % with crude oils displaying different specific gravity. These results were comparable to those obtained with commercial surfactant mixtures containing non-ionic and anionic surfactant (67.8 ± 5.2% removal). Moreover it was possible to recover the crude oil from the washing solution thanks to the reversible demulsification induced by the addition of CO2.Anthropogenically impacted surface waters are an important reservoir for multidrug-resistant bacteria and antibiotic-resistant genes. The present study aimed at MDR, ESBL, AmpC, efflux genes, and heavy metals resistance genes (HMRGs) in bacterial isolates from four Indian rivers belonging to different geo-climatic zones, by estimating the mode of resistance transmission exhibited by the resistant isolates. A total 71.27% isolates exhibited MDR trait, showing maximum resistance towards β-lactams (P = 66.49%; AMX = 59.04%), lincosamides (CD = 65.96%), glycopeptides (VAN = 25.19%; TEI = 56.91%), cephalosporins (CF = 53.72%; CXM = 30.32%) sulphonamide (COT = 43.62%; TRIM = 12.77%), followed by macrolide and tetracycline. The dfrA1 and dfrB genes were detected in total 37.5% isolates whereas; dfrA1 genes were detected in 33.34%. The sul1 gene was detected in 9.76% and sul2 gene was detected in 2.44% isolates. A total of 69.40% MDR integron positive isolates were detected with intI1and intI2 detected at 89.25% and 1.

Uterine carcinosarcomas (UCS) are rare tumors that carry a poor prognosis and high recurrence rate. Standard treatment consists of surgical resection and chemotherapy, though the benefit of adjuvant radiotherapy (RT) has yet to be determined. This study assessed survival rates between patients with UCS who underwent surgical resection alone and patients who underwent combinations of surgery, chemotherapy, and RT.
We conducted a retrospective review of all patients who underwent surgical resection for UCS between 1993 and 2011 at a single institution. We assessed 3-year disease-free survival, locoregional recurrence-free survival, distant metastases-free survival (DMFS), and overall survival rates and utilized Kaplan-Meier modeling to analyze differences between UCS treatment modalities.
Twenty-four patients underwent UCS surgical resection between 1993 and 2011. The mean age was 61 (range 39 to 75 y). Of these patients, 100% (n=24) underwent surgical resection, 25% (n=6) underwent surgery and adjuvant chemotherapy, 29% (n=7) underwent surgery and adjuvant RT, and 33% (n=8) underwent surgery and adjuvant chemotherapy and RT. At 3 years median follow, there was no significant difference in overall survival between treatment modalities. The addition of radiation therapy conferred increased DMFS in patients undergoing surgery irrespective of adjuvant chemotherapy (44% vs. 83%, P=0.0211).In patients receiving adjuvant chemotherapy, the significant increase in DMFS persisted with the addition of RT (P=0.0310). Lymph node involvement (n=8) was associated with a lower locoregional recurrence-free survival (38% vs. 92%, P=0.0029).
RT may offer a potential benefit in reducing the rate of distant metastases, though there were no statistically significant improvements in survival metrics.

Eribulin mesylate (EM) is a fully synthetic macrocyclic ketone analogue of the marine natural product halichondrin. EM has been reported to be active in metastatic breast cancer. In this paper, we report efficacy and safety of data of EM in a retrospective, real-world series of patients with poor prognosis, hormone-refractory, or triple-negative metastatic breast cancer.
The analysis was carried out at 4 interrelated oncology centers. EM was delivered at the dose of 1.4 mg/m2 in 100 mL of normal saline over 2 to 5 minutes on days 1 and 8 every 21 days. EM was continued until disease progression or unacceptable toxicity. Side effects were reported every cycle as per standard clinical practice and graded according to NCI-CTCAE, version 4.0. Time-to-progression and overall survival were reported.
In this series of 90 patients the overall response rate was 22%, and 21% and 23% in the hormonal-resistant group and the triple-negative one, respectively. Stable disease was recorded in 24%, 21%, and 27%, respectively, in the whole series, the hormonal-resistant group, and the triple-negative one, respectively. Time-to-progression was 3.5 months (range, 1 to 22 mo) in the whole series and 3.0 months (range, 1 to 14.7 mo) and 3.4 months (range, 2.2 to 16.2 mo) in the hormonal-resistant group and the triple-negative one, respectively. Overall survival reached a median of 11.4 months.
This multicenter study, albeit retrospective, demonstrates the activity of this combination as third-line chemotherapy option in a challenging clinical setting such as triple-negative or hormone-resistant patients with breast cancer progressing after several lines of hormonal manipulations.

We included 1198 pregnant women living in 3 regions of Russia in the observational, multicenter, cross-sectional, continuous study. All pregnant women were tested for serum 25(OH)D levels. We collected biomaterial from August 2018 to December 2019. The end point of the study was the indicator of vitamin D supply in pregnant women and its level depending on the trimester of pregnancy and region of Russia. Statistical analysis included counting and grouping according to vitamin D levels, and calculating the median vitamin D concentration in each region.

Skin cancers were diagnosed and removed intermittently within decades, but with no malignancies in other organs. Oral retinoids combing with topical 5- fluorouracil and photodynamic treatment yielded a desirable outcome. Uterine carcinosarcomas (UCS) are rare tumors that carry a poor prognosis and high recurrence rate. Standard treatment consists of surgical resection and chemotherapy, though the benefit of adjuvant radiotherapy (RT) has yet to be determined. This study assessed survival rates between patients with UCS who underwent surgical resection alone and patients who underwent combinations of surgery, chemotherapy, and RT. We conducted a retrospective review of all patients who underwent surgical resection for UCS between 1993 and 2011 at a single institution. We assessed 3-year disease-free survival, locoregional recurrence-free survival, distant metastases-free survival (DMFS), and overall survival rates and utilized Kaplan-Meier modeling to analyze differences between UCS treatment modalities. Twenty-four patients underwent UCS surgical resection between 1993 and 2011. The mean age was 61 (range 39 to 75 y). Of these patients, 100% (n=24) underwent surgical resection, 25% (n=6) underwent surgery and adjuvant chemotherapy, 29% (n=7) underwent surgery and adjuvant RT, and 33% (n=8) underwent surgery and adjuvant chemotherapy and RT. At 3 years median follow, there was no significant difference in overall survival between treatment modalities. The addition of radiation therapy conferred increased DMFS in patients undergoing surgery irrespective of adjuvant chemotherapy (44% vs. 83%, P=0.0211).In patients receiving adjuvant chemotherapy, the significant increase in DMFS persisted with the addition of RT (P=0.0310). Lymph node involvement (n=8) was associated with a lower locoregional recurrence-free survival (38% vs. 92%, P=0.0029). In this paper, we report efficacy and safety of data of EM in a retrospective, real-world series of patients with poor prognosis, hormone-refractory, or triple-negative metastatic breast cancer. The analysis was carried out at 4 interrelated oncology centers. EM was delivered at the dose of 1.4 mg/m2 in 100 mL of normal saline over 2 to 5 minutes on days 1 and 8 every 21 days. EM was continued until disease progression or unacceptable toxicity. Side effects were reported every cycle as per standard clinical practice and graded according to NCI-CTCAE, version 4.0. Time-to-progression and overall survival were reported. In this series of 90 patients the overall response rate was 22%, and 21% and 23% in the hormonal-resistant group and the triple-negative one, respectively. METHODS The genome composition, nucleotide analysis, codon usage indices, relative synonymous codons usage, and effective number of codons (ENc) were analyzed in the four structural genes; Spike (S), Envelope (E), membrane (M), and Nucleocapsid (N) genes, and two of the most important nonstructural genes comprising RNA-dependent RNA polymerase and main protease (Mpro) of SARS-CoV-2, Beta-CoV from pangolins, bat SARS, MERS, and SARS CoVs. RESULTS SARS-CoV-2 prefers pyrimidine rich codons to purines. Most high-frequency codons were ending with A or T, while the low frequency and rare codons were ending with G or C. SARS-CoV-2 structural proteins showed 5 to 20 lower ENc values, compared with SARS, bat SARS, and MERS CoVs. This implies higher codon bias and higher gene expression efficiency of SARS-CoV-2 structural proteins. SARS-CoV-2 encoded the highest number of over-biased and negatively biased codons. Here we show that TIF-90 binds GTP at threonine 310, and that GTP binding is required for TIF-90-enhanced rRNA synthesis. Overexpression of activated AKT induces TIF-90 T310, but not a GTP-binding site (TIF-90 T310N) mutant, to translocate into the nucleolus and increase rRNA synthesis. Complementing this result, treatment with mycophenolic acid (MPA), an inhibitor of GTP production, dissociates TIF-90 from Pol I and hence abolishes AKT-increased rRNA synthesis by way of TIF-90 activation. Thus, TIF-90 requires bound GTP to fulfill its function as an enhancer of rRNA synthesis. Instead of exploring more aggressive morphine dosing, future research should focus on a multimodal approach to treat breakthrough pain in children.
The brain of chronically ventilated preterm human infants is vulnerable to collateral damage during invasive mechanical ventilation (IMV). Damage is manifest, in part, by learning and memory impairments, which are hippocampal functions. A molecular regulator of hippocampal development is insulin-like growth factor 1 (IGF1). A gentler ventilation strategy is noninvasive respiratory support (NRS). We tested the hypotheses that NRS leads to greater levels of IGF1 messenger RNA (mRNA) variants and distinct epigenetic profile along the IGF1 gene locus in the hippocampus compared to IMV.
Preterm lambs were managed by NRS or IMV for 3 or 21 days. Isolated hippocampi were analyzed for IGF1 mRNA levels and splice variants for promoter 1 (P1), P2, and IGF1A and 1B, DNA methylation in P1 region, and histone covalent modifications along the gene locus.
NRS had significantly greater levels of IGF1 P1 (predominant transcript), and 1A and 1B mRNA variants compared to IMV at 3 or 21 days. NRS also led to more DNA methyl outcomes later in life versus IMV. Also, duration of IMV is directly related to hippocampal damage; however, molecular players remain unknown. NRS, as a gentler mode of respiratory management of preterm neonates, may reduce damage to the immature hippocampus through an epigenetic mechanism.
Dead space is the volume not taking part in gas exchange and, if increased, could affect alveolar ventilation if there is too low a delivered volume. We determined if there were differences in dead space and alveolar ventilation in ventilated infants with pulmonary disease or no respiratory morbidity.
A prospective study of mechanically ventilated infants was undertaken. Expiratory tidal volume and carbon dioxide levels were measured. Volumetric capnograms were constructed to calculate the dead space using the modified Bohr-Enghoff equation. Alveolar ventilation (V
) was also calculated.
Eighty-one infants with a median (range) gestational age of 28.7 (22.4-41.9) weeks were recruited. The dead space [median (IQR)] was higher in 35 infants with respiratory distress syndrome (RDS) [5.7 (5.1-7.0) ml/kg] and in 26 infants with bronchopulmonary dysplasia (BPD) [6.4 (5.1-7.5) ml/kg] than in 20 term controls with no respiratory disease [3.5 (2.8-4.2) ml/kg, p < 0.001]. Minute ventilation was higher in bothlated infants with respiratory distress syndrome or evolving bronchopulmonary dysplasia compared to term controls without respiratory disease. The dead space volume should be considered when calculating the most appropriate volume during volume-targeted ventilation.
Torsion of the testis is an urgent surgical condition that endangers the viability of the gonad and the fertility of the patient. Our aim was to assess potential autoimmune processes and hormonal abnormalities in boys operated on due to that illness.
The authors evaluated the levels of antibodies against sperm and Leydig cells, concentrations of follicle-stimulating, luteinizing and anti-Müllerian hormone, testosterone, oestradiol and vascular endothelial growth factor in the serum in 28 boys operated on due to torsion of the testis. Patients' sexual maturity was assessed according the Tanner scale (group G1, G4 and G5).
No antibodies against sperm or Leydig cells were found in the serum. Statistically significant differences in follicle-stimulating and anti-Müllerian hormone concentrations were observed in the G1, and they were higher in the study than in the control group. There were no statistically significant differences in luteinizing hormone, testosterone, oestradiol and vascular endothelial growe formation of antibodies and hormonal changes due to testicular torsion in boys. It is a novel, prospective study on antibodies against sperms and Leydig cells in the serum and on hormonal processes occurring as a result of the testicular torsion from the prenatal period to the adolescence with division into pubertal groups. Damage is manifest, in part, by learning and memory impairments, which are hippocampal functions. A molecular regulator of hippocampal development is insulin-like growth factor 1 (IGF1). A gentler ventilation strategy is noninvasive respiratory support (NRS). We tested the hypotheses that NRS leads to greater levels of IGF1 messenger RNA (mRNA) variants and distinct epigenetic profile along the IGF1 gene locus in the hippocampus compared to IMV. Preterm lambs were managed by NRS or IMV for 3 or 21 days. Isolated hippocampi were analyzed for IGF1 mRNA levels and splice variants for promoter 1 (P1), P2, and IGF1A and 1B, DNA methylation in P1 region, and histone covalent modifications along the gene locus. NRS had significantly greater levels of IGF1 P1 (predominant transcript), and 1A and 1B mRNA variants compared to IMV at 3 or 21 days. NRS also led to more DNA methyl outcomes later in life versus IMV. Also, duration of IMV is directly related to hippocampal damage; however, molecular players remain unknown. NRS, as a gentler mode of respiratory management of preterm neonates, may reduce damage to the immature hippocampus through an epigenetic mechanism. Dead space is the volume not taking part in gas exchange and, if increased, could affect alveolar ventilation if there is too low a delivered volume. We determined if there were differences in dead space and alveolar ventilation in ventilated infants with pulmonary disease or no respiratory morbidity. A prospective study of mechanically ventilated infants was undertaken. Expiratory tidal volume and carbon dioxide levels were measured. Volumetric capnograms were constructed to calculate the dead space using the modified Bohr-Enghoff equation. Alveolar ventilation (V ) was also calculated. Eighty-one infants with a median (range) gestational age of 28.7 (22.4-41.9) weeks were recruited. METHODS Bowman's layer was mapped with OCT (Optovue Inc., Irvine, CA) before and after removal of the epithelium in normal eyes undergoing photorefractive keratectomy (n = 14) and keratoconic eyes undergoing corneal cross-linking (n = 25). The anterior corneal surface before removal and the underlying Bowman's layer after removal of the epithelium were also mapped with Pentacam (Oculus Optikgeräte, Wetzlar, Germany), and the surface aberrations with ray tracing were computed. RESULTS The agreement of OCT curvatures before and after removal of the epithelium was excellent (intraclass correlation coefficient [ICC] = 0.9). A similar trend was seen between OCT and Pentacam after removal of the epithelium. The agreement of surface wavefront aberrations of the Bowman's layer before and after removal of the epithelium was excellent (ICC = 0.9) between the devices for keratoconic eyes. However, this agreement was relatively inferior in normal eyes (ICC less then 0.5). CONCLUSIONS The virtual OCT curvature and aberrations of the Bowman's layer agreed well with its actual magnitudes on removal of the epithelium in the keratoconic eyes. METHODS This cohort study included 64 eyes of 41 patients who underwent uneventful cataract surgery with implantation of a toric IOL (enVista toric MX60T; Bausch & Lomb, Rochester, NY) to correct preoperative corneal astigmatism. In 30 eyes, a CTR (11 SR model; Videris s.r.o., Prague, Czech Republic) was co-implanted. Analyzed parameters were refraction, visual acuity, and misalignment of toric lenses. RESULTS The mean patient age was 67 years (range 42 to 89 years) and the mean follow-up period was 5 months. Mean manifest astigmatism improved from -1.53 ± 1.15 diopters (D) preoperatively to -0.40 ± 0.61 D postoperatively (P less then .001). Postoperative uncorrected distance visual acuity was 0.10 ± 0.13 logMAR (20/25 Snellen). Mean absolute IOL misalignment was 3.70° with CTR and 3.85° without CTR (P = .683). In eyes with an axial length of 24 mm or greater, IOL axis matched the planned axis in 90.5% of eyes with CTR and 81.8% of eyes without CTR (P = .964). Four eyes (6.25%) needed additional surgical IOL rotation. CONCLUSIONS In eyes after cataract surgery with implantation of a toric IOL, there were no significant differences in the rotational stability of the lens with respect to the presence or absence of CTR. METHODS Patients who underwent a refractive lens exchange with bilateral implantation of the AT LARA 829MP IOL (Carl Zeiss Meditec, Jena, Germany) and attended the 3-month follow-up visit were included in the analysis (N = 351 patients). Demographics, preoperative and postoperative clinical parameters, and patient-reported outcomes were used in a regression model to determine predictors of 3-month postoperative satisfaction. RESULTS The mean age of the study group was 58.2 ± 7.0 years (range 45 to 79 years) and the mean preoperative sphere ranged between -12.50 and +6.75 diopters (D). At 3 months postoperatively, 86.6% of patients were very satisfied or satisfied with outcomes and 93.2% would recommend the procedure to their family or friends. Of all patients, 90.6% achieved binocular uncorrected distance visual acuity of 20/20 or better, 92.0% achieved binocular near vision of 20/50 or better, and 85.5% of eyes were within ±0.50 D of emmetropia. Logistic regression identified postoperative dry eye symptoms, binocular near and distance visual acuity, and glare symptoms as significant independent factors affecting patient satisfaction. CONCLUSIONS Several factors were independently predictive of postoperative satisfaction after EDOF IOL implantation and addressing these may further improve patient satisfaction with the procedure, specifically, proper management of early symptoms of dry eye, ensuring good refractive predictability to maximize unaided vision, and counseling patients about the possibility of visual phenomena in the early postoperative period. Using conventional epithelial thickness map readings (normal-to-the surface tangent) in transepithelial ablations (representing close to parallel vertical sections) may result in induced refractive errors that can be quantified using simple theoretical simulations, because the center-to-periphery progression of the corneal epithelial profile deviates from the progression of the ablated one. METHODS Bowman's layer was mapped with OCT (Optovue Inc., Irvine, CA) before and after removal of the epithelium in normal eyes undergoing photorefractive keratectomy (n = 14) and keratoconic eyes undergoing corneal cross-linking (n = 25). The anterior corneal surface before removal and the underlying Bowman's layer after removal of the epithelium were also mapped with Pentacam (Oculus Optikgeräte, Wetzlar, Germany), and the surface aberrations with ray tracing were computed. RESULTS The agreement of OCT curvatures before and after removal of the epithelium was excellent (intraclass correlation coefficient [ICC] = 0.9). A similar trend was seen between OCT and Pentacam after removal of the epithelium. The agreement of surface wavefront aberrations of the Bowman's layer before and after removal of the epithelium was excellent (ICC = 0.9) between the devices for keratoconic eyes. However, this agreement was relatively inferior in normal eyes (ICC less then 0.5). CONCLUSIONS The virtual OCT curvature and aberrations of the Bowman's layer agreed well with its actual magnitudes on removal of the epithelium in the keratoconic eyes. METHODS This cohort study included 64 eyes of 41 patients who underwent uneventful cataract surgery with implantation of a toric IOL (enVista toric MX60T; Bausch & Lomb, Rochester, NY) to correct preoperative corneal astigmatism. In 30 eyes, a CTR (11 SR model; Videris s.r.o., Prague, Czech Republic) was co-implanted. Analyzed parameters were refraction, visual acuity, and misalignment of toric lenses. RESULTS The mean patient age was 67 years (range 42 to 89 years) and the mean follow-up period was 5 months. Mean manifest astigmatism improved from -1.53 ± 1.15 diopters (D) preoperatively to -0.40 ± 0.61 D postoperatively (P less then .001). Postoperative uncorrected distance visual acuity was 0.10 ± 0.13 logMAR (20/25 Snellen). Mean absolute IOL misalignment was 3.70° with CTR and 3.85° without CTR (P = .683). In eyes with an axial length of 24 mm or greater, IOL axis matched the planned axis in 90.5% of eyes with CTR and 81.8% of eyes without CTR (P = .964). Four eyes (6.25%) needed additional surgical IOL rotation. CONCLUSIONS In eyes after cataract surgery with implantation of a toric IOL, there were no significant differences in the rotational stability of the lens with respect to the presence or absence of CTR. METHODS Patients who underwent a refractive lens exchange with bilateral implantation of the AT LARA 829MP IOL (Carl Zeiss Meditec, Jena, Germany) and attended the 3-month follow-up visit were included in the analysis (N = 351 patients). Demographics, preoperative and postoperative clinical parameters, and patient-reported outcomes were used in a regression model to determine predictors of 3-month postoperative satisfaction. RESULTS The mean age of the study group was 58.2 ± 7.0 years (range 45 to 79 years) and the mean preoperative sphere ranged between -12.50 and +6.75 diopters (D). At 3 months postoperatively, 86.6% of patients were very satisfied or satisfied with outcomes and 93.2% would recommend the procedure to their family or friends. Of all patients, 90.6% achieved binocular uncorrected distance visual acuity of 20/20 or better, 92.0% achieved binocular near vision of 20/50 or better, and 85.5% of eyes were within ±0.50 D of emmetropia. Logistic regression identified postoperative dry eye symptoms, binocular near and distance visual acuity, and glare symptoms as significant independent factors affecting patient satisfaction. CONCLUSIONS Several factors were independently predictive of postoperative satisfaction after EDOF IOL implantation and addressing these may further improve patient satisfaction with the procedure, specifically, proper management of early symptoms of dry eye, ensuring good refractive predictability to maximize unaided vision, and counseling patients about the possibility of visual phenomena in the early postoperative period. Riverine runoff is a significant transport pathway for microplastics (MPs) discharged from land-based sources to marine environments where MPs accumulate. Knowledge of riverine MP-associated biofilms will improve the understanding of the fate and potential effects of MPs in marine environments. This study aimed to characterize the microbial biofilms colonizing MPs in the riverine water of the Pearl River Delta, China, and identify the seasonal, geographical and environmental influences on MP-associated communities. We sampled MPs and the surrounding surface water from eight outlets in three seasons and analyzed their microbial communities by Illumina sequencing of the 16S rRNA gene libraries. Across all sampling seasons and locations, abundant MP-colonizing taxa belonged to the phylum Proteobacteria, which suggested initial biofilm development on those MPs. In this research, Δ14C of Qinghai spruce tree rings collected from Huangzhong (HZ) (36.27°N, 101.67°E, 2982 m amsl) were measured by Accelerator Mass Spectrometry, and a Δ14CO2 time series from 1957 to 2015 was reconstructed. The results show that HZ Δ14C was generally higher than the contemporaneous average level in the mid-high latitudes of the Northern Hemisphere. The peak value of HZ Δ14C occurred in 1964 (as bomb peak) was higher than that of other tree ring records in East Asia at a similar latitude, likely due to the impact of the atmosphere nuclear tests at Semipalatinsk (Kazakhstan). The record shows no obvious disturbance of Lop Nor nuclear weapons tests (in Northwest China) during 1964-1980, except for 1971. A local Suess effect began to appear since 2001, and the estimated atmospheric fossil fuel-derived CO2 (CO2ff) concentration increased from 3.5 ppm to 8.8 ppm from 2006 to 2015. This is associated with the implementation of the "Western Development" strategy in China. HZ Δ14C records document background Δ14C data, useful for regional carbon cycle research and atmospheric CO2ff quantification in the region. These data also provide baseline values for assessment environmental safety connected with nuclear power plants in China.

Besides, 14h could inhibit microtubule assembly and might kill tumor cells by inhibiting VEGF and glycolysis. All experimental results indicate that compound 14h might be a potential drug candidate for the treatment of gastric cancer and was worthy of further study.
Diabetes mellitus (DM) is a risk factor for atrial fibrillation (AF). The effect of antidiabetic medications on AF or the outcomes of catheter ablation (CA) has not been well described. We sought to determine whether metformin treatment is associated with a lower risk of atrial arrhythmias after CA in patients with DM and AF.
A first CA was performed in 271 consecutive patients with DM and AF (age 65 ± 9 years, women 34%; and paroxysmal AF 51%). At a median of 13 months after CA (interquartile range 6-30), 100/182 patients (55%) treated with metformin remained in sinus rhythm without antiarrhythmic drug therapy, compared with 36/89 patients (40%) not receiving metformin (p = .03). There was a significant association between metformin therapy and freedom from recurrent atrial arrhythmias after CA in multivariable Cox hazards models (hazard ratio [HR] 0.66; ±95% confidence interval [CI] 0.44-0.98; p = .04) that adjusted for age, sex, body mass index, AF type (paroxysmal vs. nonparoxysmal), antiarrhythmic medication, obstructive sleep apnea, chronic kidney disease, coronary artery disease, left ventricular ejection fraction, and left atrial diameter. A Cox model that also incorporated other antidiabetic agents and fasting blood glucose demonstrated a similar reduction in the risk of recurrent atrial arrhythmias with metformin treatment (HR 0.63; ±95% CI 0.42-0.96; p = .03).

Conservation crime science, an emerging interdisciplinary field, has the potential to help address this problem. However, its utility depends on serious reflection on the transposition of crime science approaches to conservation contexts, which may differ in meaningful ways from traditional crime contexts. We considered the breadth of crime science approaches being used in conservation as well as the depth of crime science integration in conservation. We used the case of sea cucumber (Holothuria floridana, Isostichopus badionotus) trafficking in Mexico as an example of why the interdisciplinarity of crime and conservation sciences should be deepened and how integration can help ideate new solutions. We first conducted a review of literature to capture the range of interdisciplinarity applications. We identified 6 crime science approaches being applied to the conservation contexts of illegal, unreported, and unregulated fishing; wildlife and plant crime; and illegal logging. We then compared this knowledge base to the case of illegal sea cucumber fishing in Mexico. We identified 5 challenges in the application of these approaches to conservation contexts the relative diffusion of harms and victims in conservation crimes; scalar mismatches in crime, authority, and the conservation issue itself; interactions between legal and illegal networks; communities and their authority to define and control crime; and the role of natural science in the rule of law. Considering these 5 factors may enhance the depth of interdisciplinarity between crime and conservation sciences. Evidence regarding the incidence and clinical outcome of cystic duct perforation (CDP) during endoscopic transpapillary gallbladder drainage (ETGBD) is inadequate. The present study aimed to evaluate the incidence and management of CDP during ETGBD.
Between March 2011 and December 2019, 249 patients underwent initial ETGBD for acute cholecystitis. The incidence of CDP was retrospectively examined and the outcomes between the CDP and non-CDP groups were compared.
CDP during ETGBD occurred in 23 (9.2%) of 249 patients (caused by guidewire in 15 and cannula in 8). ETGBD was successful in 10 patients following CDP. In 13 patients who failed ETGBD, 11 underwent bile duct drainage during the same session; nine patients underwent gallbladder decompression by other methods, such as percutaneous drainage. Clinical resolution for acute cholecystitis was achieved in 20 patients, and no bile peritonitis was noted. We considered the breadth of crime science approaches being used in conservation as well as the depth of crime science integration in conservation. We used the case of sea cucumber (Holothuria floridana, Isostichopus badionotus) trafficking in Mexico as an example of why the interdisciplinarity of crime and conservation sciences should be deepened and how integration can help ideate new solutions. We first conducted a review of literature to capture the range of interdisciplinarity applications. We identified 6 crime science approaches being applied to the conservation contexts of illegal, unreported, and unregulated fishing; wildlife and plant crime; and illegal logging. We then compared this knowledge base to the case of illegal sea cucumber fishing in Mexico. We identified 5 challenges in the application of these approaches to conservation contexts the relative diffusion of harms and victims in conservation crimes; scalar mismatches in crime, authority, and the conservation issue itself; interactions between legal and illegal networks; communities and their authority to define and control crime; and the role of natural science in the rule of law. Considering these 5 factors may enhance the depth of interdisciplinarity between crime and conservation sciences. These findings suggest that the carbon offset research has been evolved from the theoretical exploration at the early stage to a more diversified conversion of research outputs at the practical level in recent years. Interdisciplinary research towards individual and organizational carbon offset behaviors in a broader context of socio-economic development and cooperation among various agents is the emphasis and direction for future study.The regional climate has significantly warmed with erratically declining annual rainfall and intensified downpour within a narrower span of monsoon months, which led to an increased trophic state (≈algae) in most inland waters. Freshwater clupeids vitally control the aquatic food chain by grazing on algae. Despite increasing food availability, IUCN Red List® revealed 16 freshwater clupeids with a decreasing population trend. We investigated one such species' reproductive dependencies, Gudusia chapra (Indian river shad), in the lower Gangetic drainage (India) under a mixed context of climate change and overfishing. Monthly rainfall (≥ 60-100 mm) and water temperature (≥ 31-32 °C) are key breeding cues for females. The regional climate seems inclined to fulfill these through the significant part of the breeding season, and indeed the species has maintained consistent breeding phenology over 20 years. In general, all species exhibited good performances, removing 80.9-99.9% from solutions with 50 μg dm-3, 79.3-98.6% from solutions with 200 μg dm-3, and 69.8-97.7% from solutions containing 500 μg dm-3 of mercury. Among the macroalgae, Ulva intestinalis showed the highest affinity to mercury and it presented an uptake ability up to 1888 μg g-1 of Hg(II) and bioconcentration factors up to 3823, which proved its promising potential on Hg removal.

Although the World Health Organization has recommended the diagnosisand prophylactic treatment of latent tuberculous infection (LTBI) in child household contacts of tuberculosis (TB) cases, the national programs in high-burden TB regions rarely implement adequate screening of this high-risk group, mainly because of resource limitations. We aimed to evaluate the prevalence of LTBI among pediatric household contacts of TB cases in two high-burden provinces in Iran.
We conducted a cohort study in children who had been in household contact with a TB index. All subjects were assessed for active TB disease. For LTBI diagnosis, tuberculin skin test (TST) and QuantiFERON®-TB Gold Plus (QFT-Plus) were performed at the time of the index TB case diagnosis, as well as, 3, 12, and 18 months, if the first results were negative. In addition, interferon-γ-induced protein-10(IP-10) concentrations were measured for all participants.
A total of 230 children were enrolled, who had contact with an index TB case. Three contacts were diagnosed with active TB. According to the TST/QFT-Plus results, 104 (45.2%) children were identified with LTBI during our study. Significantly increased IP-10 levels were found in LTBI patients compared to healthy contacts. Accordingly, more than 50% of LTBI contacts and about 10% of healthy contacts were considered as IP-10-positive.
This study alarmingly illustrates a high prevalence of LTBI among Iranian children exposed to TB cases. We, therefore, emphasize that the children living in close contact with an infectious TB case should be screened effectively and receive prophylactic therapy.

Breast milk is the ideal and complete form of nutrition for infants colostrum contains all the necessary nutrients for infants' growth and development and antibodies that can protect from many childhood illnesses. Understanding the extent of and barriers to colostrum avoidance in Ethiopia is important for learning how to best improve optimal breastfeeding. No single study has been conducted on primigavida mothers in the country. Therefore, this study aimed to assess the rate of colostrum avoidance practice and associated factors among primigavida mothers.
A community- based cross-sectional study was conducted from March to April 2016 among (n = 398) randomly selected primigavida mothers in Bahir Dar city, northwest Ethiopia. Data were collected using a structured interviewer-administered questionnaire and analyzed using SPSS version25. Bivariate and multivariate logistic regression analyses were carried out. Odds ratio with 95% confidence interval was used to measure the strength of association. Statistic 8.8% of primipara mothers. Primipara mothers who were married, unemployed, underwent normal delivery and initiated breastfeeding within 1 h were less likely to discard colostrum. These results suggest that multi-sectorial and multi-disciplinary approaches are needed to decrease colostrum avoidance among primipara mothers in Ethiopia.
The treatment of high-grade (III/IV/V) blunt pancreatic injuries remains controversial. The study aims to summarize and evaluate nonresection management of the pancreas for grade III and IV blunt pancreatic injuries in children.

One child underwent emergency laparotomy for hemorrhagic shock and Roux-en-Y drainage of the distal pancreas. The remaining 10 children and IV blunt pancreatic injuries. Regular long-term follow-up is essential in terms of pancreatic function, especially in patients with pancreatic atrophy.
Anticonvulsant use has been linked to bone deficits in specific patient populations. We studied the association between anticonvulsant use and bone health in a population-based sample of men and women.
Data from 926 men (24-73 yr) and 1070 women (21-94 yr) participating in the Geelong Osteoporosis Study were included. Bone mineral density (BMD, g/cm
) of the PA-spine and total hip was measured using dual-energy X-ray absorptiometry (Lunar). Bone quality was determined using quantitative heel ultrasound (QUS). Anthropometry was conducted and socioeconomic status was determined. Medication and lifestyle information was obtained via questionnaire. Linear regression was used to test associations between anticonvulsant use and bone health before and after adjustment for potential confounders.
Seventeen (1.8%) men and 20 (1.9%) women reported anticonvulsant use. In men, anticonvulsant users had 9.1% lower adjusted mean BMD at the spine and hip compared to non-users. Body mass index was an effect modifier at the spine. Anticonvulsant users also had 1.8% lower speed of sound (SOS), 10.6% lower broadband ultrasound attenuation (BUA) and 13.7% lower stiffness index (SI) compared to non-users. In women, BMD tended to be lower at the hip compared to non-users as with the bone quality measure, BUA. No significant associations were observed at the spine or the other bone quality measures, SOS and SI.
Our data suggest that bone quantity and quality, assessed using BMD and QUS, are lower for men and possibly women who use anticonvulsants. While further exploration into potential mechanisms is needed, our findings suggest that monitoring bone health among users of anticonvulsants is warranted.
