Oxygen evolution reaction (OER) and urea oxidation reaction (UOR) play important roles in the fields of hydrogen energy production and pollution treatment. Herein, a facile one-step chemical etching strategy is provided for fabricating one-dimensional hierarchical nanorods array composed of CoFe layered double hydroxide (LDH)/metal-organic frameworks (MOFs) supported on carbon cloth as efficient and stable OER and UOR catalysts. By precisely controlling the etching rate, the ligands from Co-MOFs are partially removed, the corresponding metal centers then coordinate with hydroxyl ions to generate ultrathin amorphous CoFe LDH nanosheets. The resultant CoFe LDH/MOFs catalyst possesses large active surface area, enhanced conductivity and extended electron/mass transfer channels, which are beneficial for catalytic reactions. Additionally, the intimate contact between CoFe LDH and MOFs modulates the local electronic structure of the catalytic active site, leading to enhanced adsorption of oxygen-containing intermediates to facilitate fast electrocatalytic reaction. As a result, the optimized CoFe LDH/MOF-0.06 exhibits superior OER activity with a low overpotential of 276 at a current density of 10 mA cm-2with long-term durability. Additionally, it merely requires a voltage of 1.45 V to obtain 10 mA cm-2in 1 M KOH solution with 0.33 urea and is 56 mV lower than the one in pure KOH. The work presented here may hew out a brand-new route to construct multi-functional electrocatalysts for water splitting, CO2reduction, nitrogen reduction reactions and so on.Objective.The speed of visual brain-computer interfaces (v-BCIs) has been greatly improved in recent years. However, the traditional v-BCI paradigms require users to directly gaze at the intensive flickering items, which would cause severe problems such as visual fatigue and excessive visual resource consumption in practical applications. Therefore, it is imperative to develop a user-friendly v-BCI.Approach.According to the retina-cortical relationship, this study developed a novel BCI paradigm to detect the fixation point of eyes using a small visual stimulus that subtended only 0.6° in visual angle and was out of the central visual field. Specifically, the visual stimulus was treated as a landmark to judge the eccentricity and polar angle of the fixation point. Sixteen different fixation points were selected around the visual landmark, i.e. different combinations of two eccentricities (2° and 4°) and eight polar angles (0,π4,π2,3π4,π,5π4,3π2and7π4). Twelve subjects participated in this study, and they were asked to gaze at one out of the 16 points for each trial. A multi-class discriminative canonical pattern matching (Multi-DCPM) algorithm was proposed to decode the user's fixation point.Main results.We found the visual stimulation landmark elicited different spatial event-related potential patterns for different fixation points. Multi-DCPM could achieve an average accuracy of 66.2% with a standard deviation of 15.8% for the classification of the sixteen fixation points, which was significantly higher than traditional algorithms (p⩽0.001). The participanand-grasp movements, which also constitutes an advantage with respect to current BCI protocols.Tissue reconstruction requires the utilization of multiple biomaterials and cell types to replicate the delicate and complex structure of native tissues. Various three-dimensional (3D) bioprinting techniques have been developed to fabricate customized tissue structures; however, there are still significant challenges, such as vascularization, mechanical stability of printed constructs, and fabrication of gradient structures to be addressed for the creation of biomimetic and complex tissue constructs. One approach to address these challenges is to develop multimaterial 3D bioprinting techniques that can integrate various types of biomaterials and bioprinting capabilities towards the fabrication of more complex structures. Notable examples include multi-nozzle, coaxial, and microfluidics-assisted multimaterial 3D bioprinting techniques. The model is expanded such that it can be applied for polyenergetic beams. The methodology is validated using Microdosimetric Monte Carlo Damage Simulation code (MCDS). This serves as a bootstrap to determine relevant parameters in the analytical expression, as MCDS is shown to be extensively verified with published empirical data. Double-strand break induction as calculated by this methodology is compared to published proton experiments.

Heterotrophic Proteobacteria are versatile opportunists that have been extensively studied as model organisms in the laboratory, as both pathogens and beneficial symbionts of plants and animals, and as ubiquitous organisms found free-living in many environments. Succeeding in these niches requires an ability to persist for potentially long periods of time in growth-arrested states when essential nutrients become limiting. The tendency of these bacteria to grow in dense biofilm communities frequently leads to the development of steep nutrient gradients and deprivation of interior cells even when the environment is nutrient rich. Surviving within host environments also likely requires tolerating growth arrest due to the host limiting access to nutrients and transitioning between hosts may require a period of survival in a nutrient-poor environment. Interventions to maximise plant-beneficial activities and minimise infections by bacteria will require a better understanding of metabolic and regulatory networks that contribute to starvation survival, and how these networks function in diverse organisms. Here we focus on carbon starvation as a growth-arresting condition that limits availability not only of substrates for biosynthesis but also of energy for ongoing maintenance of the electrochemical gradient across the cell envelope and cellular integrity. We first review models for studying bacterial starvation and known strategies that contribute to starvation survival. We then present the results of a survey of carbon starvation survival strategies and outcomes in ten bacterial strains, including representatives from the orders Enterobacterales and Pseudomonadales (both Gammaproteobacteria) and Burkholderiales (Betaproteobacteria). Finally, we examine differences in gene content between the highest and lowest survivors to identify metabolic and regulatory adaptations that may contribute to differences in starvation survival.Regulation of proinflammatory cytokine expression is critical in the face of single-stranded RNA (ssRNA) virus infections. Many viruses, including coronavirus and influenza virus, wreak havoc on the control of cytokine expression, leading to the formation of detrimental cytokine storms. Understanding the regulation and interplay between inflammatory cytokines is critical to the identification of targets involved in controlling the induction of cytokine expression. In this study, we focused on how the antiviral cytokine interleukin-27 (IL-27) regulates signal transduction downstream of Toll-like receptor 7 (TLR7) and TLR8 ligation, which recognize endosomal single-stranded RNA. Given that IL-27 alters bacterial-sensing TLR expression on myeloid cells and can inhibit replication of single-stranded RNA viruses, we investigated whether IL-27 affects expression and function of TLR7 and TLR8. Analysis of IL-27-treated THP-1 monocytic cells and THP-1-derived macrophages revealed changes in mRNA and protein expression of TLR7 and TLR8. Although treatment with IL-27 enhanced TLR7 expression, only TLR8-mediated cytokine secretion was amplified. Furthermore, we demonstrated that imiquimod, a TLR7 agonist, inhibited cytokine and chemokine production induced by a TLR8 agonist, TL8-506. Delineating the immunomodulatory role of IL-27 on TLR7 and TLR8 responses provides insight into how myeloid cell TLR-mediated responses are regulated during virus infection.During both short- and long-duration spaceflight, several health problems can occur, including those of the skin. Human skin is an ecosystem composed of a wide range of habitats for bacteria, fungi, and viruses called microbiome, which not only show a strong skin site-specific preference but also serve as microbial fingerprints that are highly unique to individuals. These human skin-associated microorganisms make a substantial contribution to the microbial ecosystems that inhabit the closed environments in space. On the other hand, human skin microbiome is also subject to change during spaceflight, which may lead to skin infections or the flare up of skin diseases. This review highlights some of the interactions between the space environment and the skin.Pierson syndrome (PIERSS) is a rare autosomal recessive disorder characterized by the combination of congenital nephrotic syndrome (CNS) and extrarenal symptoms including ocular malformations and neurodevelopmental deficits. The course was additionally complicated by tubulopathy, life-threatening infections, severe hypertension, erythropoietin-resistant anemia, generalized muscular hypotonia, neurogenic bladder, profound neurodevelopmental delay, epilepsy, gastrointestinal problems, secondary hypothyroidism, and necessity of repeated ocular surgery due to microcoria, cataract, and nystagmus. Due to multidisciplinary efforts, at the age of 4 years, the kidney transplantation was possible. Currently, the renal graft has an excellent function; however, the girl presents severe neurodevelopmental delay. The report presents a unique long-term follow-up of severe PIERSS with a few new phenotypical findings. It highlights the clinical problems and challenges in management of this rare condition.
The purpose of this study was to evaluate the effects of nasal surgery on airflow characteristics in patients with obstructive sleep apnea (OSA) by comparing the alterations of airflow characteristics within the nasal and palatopharyngeal cavities.
Thirty patients with OSA and nasal obstruction who underwent nasal surgery were enrolled. A pre- and postoperative 3-dimensional model was constructed, and alterations of airflow characteristics were assessed using the method of computational fluid dynamics. The other subjective and objective clinical indices were also assessed.
By comparison with the preoperative value, all postoperative subjective symptoms statistically improved (p < 0.05), while the Apnea-Hypopnea Index (AHI) changed little (p = 0.492); the postoperative airflow velocity and pressure in both nasal and palatopharyngeal cavities, nasal and palatopharyngeal pressure differences, and total upper airway resistance statistically decreased (all p < 0.01). This study's intent was to test a new system for scoring cardiac thrombotic stability, based on contrast-enhanced ultrasound (CEUS).
We used human whole blood for an in vitro thrombotic model involving 1-h (T

group was monitored for 1h continuously to observe for the formation of a new thrombus on the original thrombus base. Changes in thrombotic CEUS images, histologic features, and shear wave elastography were recorded over time. This study aimed to elucidate the detailed course of the CBT to the facial nucleus through the analysis of lateral medullary infarction (LMI) cases.
The neurological characteristics and magnetic resonance imaging findings of 33 consecutive patients with LMI were evaluated. The location of the lesions was classified rostro-caudally (upper, middle, or lower) and horizontally. Further, we compared the neurological characteristics between the groups with and without central facial paresis (FP).
Eight (24%) patients with central FP ipsilateral to the lesion were identified. Dysphagia and hiccups were more frequently observed in the group with central FP than in the group without central FP. In patients with central FP, middle medullary lesions and those including the ventral part of the dorsolateral medulla were more frequently observed. Contrastingly, patients with lesions restricted to the lateral and dorsal regions of the dorsolateral medulla did not present with central FP.

To evaluate alterations in the serum concentrations of vascular endothelial growth factor (VEGF) and netrin-1 after intravitreal bevacizumab (BCZ) injection for the treatment of diabetic macular edema (DME).
This prospective case-control study included a total of 50 participants assigned to one of three groups, including 10 individuals with DME and non-proliferative diabetic retinopathy (NPDR), 13 with DME, and proliferative diabetic retinopathy (PDR), and 27 healthy individuals as a control group. Serum VEGF and netrin-1 concentrations were measured by enzyme-linked immunosorbent assays (ELISAs) immediately before, as well as 1 week and 1 month after, intravitreal BCZ injection.
The mean VEGF serum concentrations in the PDR and NPDR groups were 388.4 and 196.9 pg/mL at baseline, respectively. After 1 week, these concentrations changed to 193.41 and 150.23 pg/mL, respectively (P = 0.001 and P = 0.005, respectively); after 1 month, the concentrations were 97.89 and 76.46 pg/mL, respectively (P = 0.001 and P = 0.009, respectively). The mean netrin-1 serum concentrations in the PDR patients and NPDR groups were 318.2 and 252.7 pg/mL at baseline, respectively. After 1 week, these concentrations increased to 476.6 and 416.3 pg/mL, respectively (P = 0.033 and P = 0.005, respectively), and after 1 month, they were 676.6 and 747.5 pg/mL, respectively (P = 0.001 and P = 0.005, respectively). The correlation analysis revealed a significant inverse relationship between changes in serum VEGF and netrin-1 concentrations in both the PDR and NPDR groups (r = - 0.685, P = 0.029).

This study's intent was to test a new system for scoring cardiac thrombotic stability, based on contrast-enhanced ultrasound (CEUS). We used human whole blood for an in vitro thrombotic model involving 1-h (T ) and 7-day (T ) subsets. The T group was monitored for 1h continuously to observe for the formation of a new thrombus on the original thrombus base. Changes in thrombotic CEUS images, histologic features, and shear wave elastography were recorded over time. We also studied 28 patients diagnosed with cardiac thrombi, each examined by transthoracic echocardiography and CEUS.Thrombi were scored for substrate (T ) and hardness (T ) based on the visualized degree of contrast penetration into the thrombi. Statistical analyses of T and T reflected thrombolytic time and risk of embolism to other organs. Histologically, the loosely constructed ends of in vitro thrombi solidified over time. In addition, the average Young's modulus of thrombi over time indicated a progressive increase in hardness. Increased rates of international travel have led to a higher demand for healthcare professionals to provide travel health services. Community-based pharmacists are capable of meeting this need.
This study evaluates the impact of pharmacists providing travel health services in a community-based pharmacy on participant understanding and satisfaction of travel education and preparation.
A trained pharmacist met with participants to review their medical history, travel itinerary, and provide education. Indicated immunizations were administered and the participant's primary care provider was contacted if prescription medications were warranted. A questionnaire was administered before and after the travel health consultation assessing participants perceived understanding of travel health information, satisfaction, and perceived monetary value of the service. Data were collected by 5-point Likert-scale responses, with 5 equivalent to strongly agree. Wilcoxon signed-rank test and descriptive statistics were used for evaluation. Participants were included if they had international travel planned within 12 weeks of the consultation.
A total of 12 participants were included. Participant understanding significantly increased for all 5 survey items relating to travel health information with a p value < 0.05 for each item. The largest change was for how to find medical help during international travel (medians and IQR were 3(2-3), and 5(5-5) for pre-and post-consultation, respectively, p = 0.003). Participant satisfaction questions received a median response of 5. Participants' perceived monetary value of the service was a median of $50 (IQR $50-50).

This review characterizes the course of clinical, laboratory, radiological findings during the primary infection period, and the complications post-recovery. Primary care findings are presented for long-COVID care.
Adhering to PRISMA guidelines, 4 databases were searched (PubMed, Embase, CINAHL Plus, Scopus) through December 5, 2020, using the keywords "COVID-19 and/or recovered and/or cardiovascular and/or long-term and/or sequelae and/or sub-acute and/or complication." We included published peer-reviewed case reports, case series, and cross-sectional studies providing the clinical course of COVID-19 infection, and cardiopulmonary complications of patients who recovered from COVID-19, while making healthcare considerations for primary care workers.
We identified 29 studies across 9 countries including 37.9% Chinese and 24.1% U.S. studies, comprising 655 patients (Mean Age = 45) with various ethnical backgroted to establish point-of-care testing, detect early complications, and provide timely treatment.

This study compares the ultrasound (US) guided method with the conventional digital palpation method in radial artery puncture (RAP) for blood gas analysis in septic shock patients.
This is a prospective, single-centre study. Septic shock patients over 18 years of age who needed a RAP sample for blood gas analysis were included. Patients with local infection or trauma at the puncture site, arteriovenous fistula, vascular graft, coagulopathy, a positive Allen test, or did not want to participate were excluded. Patients were randomized into two groups and RAP was obtained with either the US-guided method or the conventional method. The main outcomes were success at first entry, the number of attempts before success, and the time to success.
The 50 eligible patients were randomized into two groups. First entry success rate for the US-guided group and the conventional group was 80% and 42%, respectively. The number of attempts before success and time to success was significantly higher in the conventional group.

Effortful touch by the hand is essential to engaging with and perceiving properties of objects. The temporal structure of whole-body coordination must reflect the prospective control that provides for both the engagement with and perception of properties of the hefted objects. In the present study, we found signatures of multifractality in the time series of fluctuations in Euclidean displacement in the participants' center of pressure (CoP) as they hefted weighted objects to perceive their heaviness and length. Comparisons of widths of the multifractal spectrums of CoP series with 32 Iterative Amplitude Adjusted Fourier Transform (IAAFT) surrogates provided evidence for multiplicative-cascade dynamics and interactivity across scales, through the continuous t-statistic comparing the original and surrogate widths (tMF). After controlling for the linear properties of CoP series and their interactions with the informational variable (i.e., the moment of inertia of the hefted objects), regression modeling of unsigned error in judgments of heaviness and length revealed that the multifractal evidence for nonlinearity (tMF) significantly influenced unsigned error. The two indicators showed opposite, task-specific effects on accuracy accuracy in judgments of heaviness and length decreased and increased, respectively, with greater tMF. These findings suggest that multiplicative-cascade dynamics in posture play a role in prospective coordination during the engagement with objects and perception of their properties via effortful touch by the hand. Future work may elucidate how constraint(s) on exploratory kinematics influence the multifractal behavior in such suprapostural perceptual tasks as effortful touch. Toe joint articulation has been shown to affect gait mechanics, as evidenced by walking simulations, biped robots, and foot prostheses. However, it is not known how parameters such as toe length, foot arch length (i.e., heel-to-toe-joint length) or toe joint axis angle affect human walking. We utilized a previously developed adjustable ankle-toe prosthesis to systematically examine these three foot parameters. We tested ten able-bodied persons walking on a force instrumented-treadmill while wearing a pair of adjustable prostheses attached bilaterally below simulator boots (which fixated their biological ankles). We collected motion and ground reaction force data to compute lower-limb kinematics and kinetics as well as COM power and work. We observed that increasing the foot arch length by 60 mm (35%) increased COM Push-off work by ~5 J, due to increased energy storage and return by the ankle spring. Increasing the toe length by 40 mm (80%) and changing the toe joint axis by ±9° from a neutral angle resulted in negligible effects on COM mechanics and lower limb kinetics. This study provides further insights regarding toe joint function; knowledge which may benefit the design/integration of toe joints into prostheses, exoskeletons and legged robots. INTRODUCTION Flat feet in children negatively affect lower limb alignment and cause adverse health-related problems. The long-term application of foot orthoses (FOs) may have the potential to improve lower limb coordination and its variability. AIM To evaluate the effects of long-term use of arch support FOs on inter-joint coordination and coordination variability in children with flexible flat feet. METHODS Thirty boys with flexible flat feet were randomly assigned to the experimental (EG) and control groups. The EG used medial arch support FOs during daily activities over a four-month period while the control group received a flat 2-mm-thick insole for the same time period. Lower-limb coordination and variability during the 3 sub-stance phases were quantified using a vector coding technique. RESULTS Frontal plane ankle-hip coordination in EG during mid-stance changed to an anti-phase pattern (156.9°) in the post-test compared to an in-phase (221.1°) in the pre-test of EG and post-test of CG (222.7). Frontal plane knee-hip coordination in EG during loading response (LR) changed to an anti-phase pattern (116°) in the post-test compared to an in-phase (35.5°) in the pre-test of EG and post-test of CG (35.3). Ankle inversion/eversion-knee internal/external rotation joint coupling angle in EG changed to an in-phase pattern (59°) in the post-test compared to a proximal phase (89°) in the pre-test. Coupling angle variability increased in the post-test of EG for sagittal plane ankle-hip during push-off, transverse plane ankle-hip during LR and mid-stance, and transverse plane knee-hip during LR and mid-stance compared to pre-test of EG and post-test of CG. We examined whether a general myocontrol skill exists in myocontrolled tasks with and without a prosthesis. Unimpaired, right-handed adults used the sEMG of wrist flexors and extensors to perform several tasks in two experiments. In Experiment 1, twelve participants trained a myoelectric prosthesis-simulator task and a myocontrolled serious game for five consecutive days. Performance was compared between tasks and over the course of the training period. These findings suggest that multiplicative-cascade dynamics in posture play a role in prospective coordination during the engagement with objects and perception of their properties via effortful touch by the hand. Future work may elucidate how constraint(s) on exploratory kinematics influence the multifractal behavior in such suprapostural perceptual tasks as effortful touch. Toe joint articulation has been shown to affect gait mechanics, as evidenced by walking simulations, biped robots, and foot prostheses. However, it is not known how parameters such as toe length, foot arch length (i.e., heel-to-toe-joint length) or toe joint axis angle affect human walking. We utilized a previously developed adjustable ankle-toe prosthesis to systematically examine these three foot parameters. We tested ten able-bodied persons walking on a force instrumented-treadmill while wearing a pair of adjustable prostheses attached bilaterally below simulator boots (which fixated their biological ankles). We collected motion and ground reaction force data to compute lower-limb kinematics and kinetics as well as COM power and work. We observed that increasing the foot arch length by 60 mm (35%) increased COM Push-off work by ~5 J, due to increased energy storage and return by the ankle spring. Increasing the toe length by 40 mm (80%) and changing the toe joint axis by ±9° from a neutral angle resulted in negligible effects on COM mechanics and lower limb kinetics. This study provides further insights regarding toe joint function; knowledge which may benefit the design/integration of toe joints into prostheses, exoskeletons and legged robots. The long-term application of foot orthoses (FOs) may have the potential to improve lower limb coordination and its variability. AIM To evaluate the effects of long-term use of arch support FOs on inter-joint coordination and coordination variability in children with flexible flat feet. METHODS Thirty boys with flexible flat feet were randomly assigned to the experimental (EG) and control groups. The EG used medial arch support FOs during daily activities over a four-month period while the control group received a flat 2-mm-thick insole for the same time period. Lower-limb coordination and variability during the 3 sub-stance phases were quantified using a vector coding technique. RESULTS Frontal plane ankle-hip coordination in EG during mid-stance changed to an anti-phase pattern (156.9°) in the post-test compared to an in-phase (221.1°) in the pre-test of EG and post-test of CG (222.7). Frontal plane knee-hip coordination in EG during loading response (LR) changed to an anti-phase pattern (116°) in the post-test compared to an in-phase (35.5°) in the pre-test of EG and post-test of CG (35.3). Ankle inversion/eversion-knee internal/external rotation joint coupling angle in EG changed to an in-phase pattern (59°) in the post-test compared to a proximal phase (89°) in the pre-test. Coupling angle variability increased in the post-test of EG for sagittal plane ankle-hip during push-off, transverse plane ankle-hip during LR and mid-stance, and transverse plane knee-hip during LR and mid-stance compared to pre-test of EG and post-test of CG. Still, most studies of mucosal immunity are conducted in the absence of semen or without considering its immune impact on the female genital tract. This review discusses the various mechanisms by which semen exposure may influence female genital inflammation and highlights the importance of routine screening for semen biomarkers in vaginal specimens to account for its impact on genital inflammation.Fibrotic diseases are still a serious concern for public health, due to their high prevalence, complex etiology and lack of successful treatments. Myofibroblasts represent the principal cellular mediators of fibrosis, due to their extracellular matrix producing activity, and originate from different types of precursor cells, such as mesenchymal cells, epithelial cells and fibroblasts. Profibrotic activation of myofibroblasts can be triggered by a variety of mechanisms, including the transforming growth factor-β signalling pathway, which is a major factor driving fibrosis. Interestingly, preclinical and clinical studies showed that fibrotic degeneration can stop and even reverse by using specific antifibrotic treatments. Increasing scientific evidence is being accumulated about the role of sirtuins in modulating the molecular pathways responsible for the onset and development of fibrotic diseases. Sirtuins are NAD+ -dependent protein deacetylases that play a crucial role in several molecular pathways within the cells, many of which at the crossroad between health and disease. In this context, we will report the current knowledge supporting the role of sirtuins in the balance between healthy and diseased myofibroblast activity. In particular, we will address the signalling pathways and the molecular targets that trigger the differentiation and profibrotic activation of myofibroblasts and can be modulated by sirtuins.Cancerous inhibitor of protein phosphatase 2A (Cip2A) is an oncoprotein that promotes the development of several types of cancer. A pilot questionnaire was developed to investigate the cognitive tests that clinicians employ in their clinical practice and whether they perceive the Functional Independence Measure (FIM) to add clinical value.
A 10-item pilot questionnaire was created and distributed to 43 health professionals of a single health service in Australia. A snowballing approach to sampling was used.
There were 39 respondents. Less than half of respondents thought the FIM added clinical value, and 15% stated that they would use the FIM to assess cognition on subacute wards. Ninety-seven per cent (97%) of respondents stated that they would rely more heavily on cognitive screening tests than the FIM.
Participant responses to this survey of a single health service raise interesting questions about how useful clinicians perceive the FIM to be, beyond a costing and benchmarking role. Clinicians may prefer cognitive screening tests to the FIM, in clinical practice.

The measurement of progress towards many Sustainable Development Goals (SDG) and other health goals requires accurate and timely all-cause and cause of death (COD) data. However, existing guidance to countries to calculate these indicators is inadequate for populations with incomplete death registration and poor-quality COD data. We introduce a replicable method to estimate national and subnational cause-specific mortality rates (and hence many such indicators) where death registration is incomplete by integrating data from Medical Certificates of Cause of Death (MCCOD) for hospital deaths with routine verbal autopsy (VA) for community deaths.
The integration method calculates population-level cause-specific mortality fractions (CSMFs) from the CSMFs of MCCODs and VAs weighted by estimated deaths in hospitals and the community. Estimated deaths are calculated by applying the empirical completeness method to incomplete death registration/reporting. The resultant cause-specific mortality rates are used to e routine VA in country Civil Registration and Vital Statistics systems.
This integration method facilitates country authorities to use their data to monitor progress with national and subnational health goals, rather than rely on estimates made by external organisations. The method is particularly relevant given the increasing application of routine VA in country Civil Registration and Vital Statistics systems.
Most of the deaths among neonates in low-income and middle-income countries (LMICs) can be prevented through universal access to basic high-quality health services including essential facility-based inpatient care. However, poor routine data undermines data-informed efforts to monitor and promote improvements in the quality of newborn care across hospitals.
Continuously collected routine patients' data from structured paper record forms for all admissions to newborn units (NBUs) from 16 purposively selected Kenyan public hospitals that are part of a clinical information network were analysed together with data from all paediatric admissions ages 0-13 years from 14 of these hospitals. Data are used to show the proportion of all admissions and deaths in the neonatal age group and examine morbidity and mortality patterns, stratified by birth weight, and their variation across hospitals.
During the 354 hospital months study period, 90 222 patients were admitted to the 14 hospitals contributing NBU and generlow-cost interventions, hospitals have high and very variable mortality proportions after stratification by birth weight.
To provide healthcare professionals with comprehensive multidisciplinary expert recommendations for the acute care of severe limb trauma patients, both during the prehospital phase and after admission to a Trauma Centre.
A consensus committee of 21 experts was formed. A formal conflict-of-interest (COI) policy was developed at the onset of the process and enforced throughout. Specifically, Doppler ultrasound of the ophthalmic vessels has become a very useful tool in diagnosing various eye diseases. Doppler ultrasound allows for examining blood flow in the eye even in the presence of eye opacities that impede viewing the posterior segment of the eye. In this review, we describe the principles and techniques of an ocular vascular Doppler ultrasound examination in clinical practice and provide a general approach to the ultrasound characteristics of the most important vascular eye disorders for internists. These include central retinal artery and vein occlusions, chronic retinal ischemic syndrome, anterior optic ischemic neuropathy, and diabetic retinopathy.
Sex differences in the incidence and outcomes of several cancers are well established. Multiple myeloma (MM) is a malignant plasma cell dyscrasia accounting for 2% of all new cancer cases in the UK. There is a clear sex disparity in MM incidence, with 57% of cases in males and 43% in females. The mechanisms behind this are not well understood and the impact of sex on patient outcomes has not been thoroughly explored.
We investigated the association of sex with baseline disease characteristics and outcome in 3894 patients recruited to the phase III UK NCRI Myeloma XI trial, in which treatment exposure to lenalidomide predominated.
Females were significantly more likely to have the molecular lesions t(14;16) and del(17p) and were more likely to meet the cytogenetic classification of high-risk (HiR) or ultra-high-risk disease (UHiR). There was no difference in progression-free survival (PFS) or overall survival (OS) between the sexes in the overall population.
Our data suggest that the genetic lesions involved in the initiation and progression of MM may be different between the sexes.

There is a clear sex disparity in MM incidence, with 57% of cases in males and 43% in females. The mechanisms behind this are not well understood and the impact of sex on patient outcomes has not been thoroughly explored. We investigated the association of sex with baseline disease characteristics and outcome in 3894 patients recruited to the phase III UK NCRI Myeloma XI trial, in which treatment exposure to lenalidomide predominated. Females were significantly more likely to have the molecular lesions t(14;16) and del(17p) and were more likely to meet the cytogenetic classification of high-risk (HiR) or ultra-high-risk disease (UHiR). There was no difference in progression-free survival (PFS) or overall survival (OS) between the sexes in the overall population. Coronary embolism is a rare cause of myocardial infarction (MI). We present a case report which emphasizes the importance of intracoronary imaging in these cases to identify the pathophysiological mechanism of MI.
A 53-year-old male with no past medical history presented to the hospital with typical angina. Electrocardiogram and serum troponin I level trend confirmed non-ST-elevation myocardial infarction. Coronary angiography showed no evidence of any obstructive coronary artery disease, but two small thrombi were noted in the distal first obtuse marginal branch. Optical coherence tomography imaging confirmed this finding in absence of any underlying atherosclerotic plaque rupture or erosion. We describe the case of a 33-year-old female with a history of anomalous left coronary artery of the pulmonary artery who presents with exertional angina. She underwent a Takeuchi repair that was complicated by a baffle leak. She was successfully treated with left internal mammary artery-left anterior descending (LAD) bypass grafting and percutaneous baffle leak closure.
The Takeuchi procedure involves the creation of an aortopulmonary window and an intrapulmonary tunnel that 'baffles' the aorta to the ostium of the anomalous left coronary artery. The most common late complication of the Takeuchi procedure is the presence of a baffle leak. Biventricular Takotsubo cardiomyopathy (BTC) is estimated to occur in 25-42% of those with Takotsubo cardiomyopathy (TC). Little is known about which subset of patients are predisposed to having concomitant right ventricular (RV) involvement, or the pattern of recovery in BTC.
We describe a 69-year-old woman who presented with dyspnoea and was subsequently diagnosed with BTC. We propose that this was triggered by an exacerbation of chronic obstructive pulmonary disease on a background of multiple predisposing factors including recent bereavement, previous excessive alcohol use, status as a current smoker, and anxiety. During her admission, she required non-invasive ventilation and inotropic support to manage her type two respiratory failure and acute heart failure. Serial echocardiograms during the admission allowed us to capture and present the sequential recovery of ventricular systolic function, with the left ventricular (LV) recovery preceding the right ventricle.
Our patient fulfils the International Takotsubo Diagnostic criteria of transient LV dysfunction, emotional and physical triggers, electrocardiogram abnormalities, raised troponin and brain natriuretic peptide and no occlusive coronary artery disease. Cardiac sarcoidosis (CS) and arrhythmogenic right ventricular cardiomyopathy (ARVC) are rare causes of ventricular arrhythmias and are associated with sudden cardiac death. Differentiation between both is important for proper management.
We present a 56-year-old man with sudden cardiac arrest and was diagnosed to have ARVC based on cardiac magnetic resonance imaging (MRI). He developed gradually worsening shortness of breath over the next 1 year. CS was unmasked after a cardiac positron emission tomography (PET). Patient was treated with methotrexate. A repeat cardiac PET scan showed improvement.
The distinction between ARVC and CS is challenging. Both these entities have a patchy involvement and can have similar presentations. ARVC has a predominant right heart involvement. It is diagnosed with the help of an MRI, which shows regional right ventricular wall motion abnormality. These findings can have an overlap with CS. It is important to note that, even though sarcoidosis is a pathologic diagnosis, cardiac biopsy is rarely done owing to its patchy involvement.

These characteristics are contrary to the application independency and versatility of raw Unicode SKOS files, e.g. the custom text arrangement, extensibility or copy & paste editing. We provide an application that adds navigation, auto completion and validity check capabilities on top of a regular Unicode text editor.Precision medicine is an emerging and important field for health care. Molecular tumor boards use a combination of clinical and molecular data, such as somatic tumor mutations to decide on personalized therapies for patients who have run out of standard treatment options. Personalized treatment decisions require clinical data from the hospital information system and mutation data to be accessible in a structured way. Here we introduce an open data platform to meet these requirements. We use the openEHR standard to create an expert-curated data model that is stored in a vendor-neutral format. Clinical and molecular patient data is integrated into cBioPortal, a warehousing solution for cancer genomic studies that is extended for use in clinical routine for molecular tumor boards. For data integration, we developed openEHR Mapper, a tool that allows to (i) process input data, (ii) communicate with the openEHR repository, and (iii) export the data to cBioPortal. We benchmarked the mapper performance using XML and JSON as serialization format and added caching capabilities as well as multi-threading to the openEHR Mapper.The archiving and exchange interface for practice management systems of the Kassenärztliche Bundesvereinigung, defined by FHIR (Fast Healthcare Interoperability Resources) profiles with extensions, describes a new opportunity for medical practitioner to change the system provider. The expectation is to transfer an entire database of a legacy system to another system without data loss. In this paper the potential loss of data is analyzed by comparing parameters. The results show that during an import on average 75% of the parameters per profile are supported and on average only 49% of the reviewed parameters, existing in the exporting system, could be represented based on the interface specification.Data integration is a necessary and important step to perform translational research and improve the sample size beyond single data collections. For health information, the most recent established communication standards is HL7 FHIR. To bridge the concepts of "minimal invasive" data integration and open standards, we propose a generic ETL framework to process arbitrary patient related data collections into HL7 FHIR - which in turn can then be used for loading into target data warehouses. The proposed algorithm is able to read any relational delimited text exports and produce a standard HL7 FHIR bundle collection. We evaluated an implementation of the algorithm using different lung research registries and used the resulting FHIR resources to fill our i2b2 based data warehouse as well an OMOP common data model repository.Sharing data is of great importance for research in medical sciences. Then, secondly, the ontological phenotype representation can be used to generate search queries that are executed by the LHA Data Portal. This enabled us to provide students a data set that is as semantically and clinically realistic as possible to apply patient-level prediction algorithms within the development of clinical decision support systems without putting patient data at any risk.The diagnosis of patients with rare diseases is often delayed. A Clinical Decision Support System using similarity analysis of patient-based data may have the potential to support the diagnosis of patients with rare diseases. This qualitative study has the objective to investigate how the result of a patient similarity analysis should be presented to a physician to enable diagnosis support. We conducted a focus group with physicians practicing in rare diseases as well as medical informatics researchers. To prepare the focus group, a literature search was performed to check the current state of research regarding visualization of similar patients. We then created software-mockups for the presentation of these visualization methods for the discussion within the focus group. Two persons took independently field notes for data collection of the focus group. A questionnaire was distributed to the participants to rate the visualization methods. The results show that four visualization methods are promising for the visualization of similar patients "Patient on demand table", "Criteria selection", "Time-Series chart" and "Patient timeline.

They are used as a model organism in environmental risk assessment of chemicals and soil toxicology. Soil provides physical and nutritive support to agriculture system by regulating biogeochemical cycles, nutrient cycle, waste degradation, organic matter degradation etc. The biggest threat to soil health are pesticides and synthetic chemicals including fertilizers. Earthworms are most severely hit by these xenobiotic compounds leading to a sizeable reduction of their population and adversely affecting soil fertility. Earthworms are incredible soil organisms playing a crucial role in maintaining soil health. Pesticides used in crop management are known to be most over-purchased and irrationally used soil toxicants, simultaneously, used insecticides contribute to a quantum of damage to earthworms and other non-target organisms. LC50 and LD50 studies revealed that earthworms are highly susceptible to insecticides causing immobility, rigidity and also show a significant effect on biomass reduction, growth and reproduction by disrupting various physiological activities leading to loss of earthworm population and soil biodiversity. Intake of Al is by inhalation of aerosols or particles, ingestion of food, water and medicaments, skin contact, vaccination, dialysis and infusions. Toxic actions of Al induce oxidative stress, immunologic alterations, genotoxicity, pro-inflammatory effect, peptide denaturation or transformation, enzymatic dysfunction, metabolic derangement, amyloidogenesis, membrane perturbation, iron dyshomeostasis, apoptosis, necrosis and dysplasia. The pathological conditions associated with Al toxicosis are desquamative interstitial pneumonia, pulmonary alveolar proteinosis, granulomas, granulomatosis and fibrosis, toxic myocarditis, thrombosis and ischemic stroke, granulomatous enteritis, Crohn's disease, inflammatory bowel diseases, anemia, Alzheimer's disease, dementia, sclerosis, autism, macrophagic myofasciitis, osteomalacia, oligospermia and infertility, hepatorenal disease, breast cancer and cyst, pancreatitis, pancreatic necrosis and diabetes mellitus. As part of a Citizen Science Project initiated by the Westerdijk Fungal Biodiversity Institute and the Utrecht University Museum, which aimed to describe novel fungal species from Dutch garden soil, the diversity of Didymellaceae, which is one of the largest families in the Dothideomycetes was investigated. A preliminary analysis of the ITS and LSU sequences from the obtained isolates allowed the identification of 148 strains belonging to the family. Based on a multi-locus phylogeny of a combined ITS, LSU, rpb2 and tub2 alignment, and morphological characteristics, 20 different species were identified in nine genera, namely Ascochyta, Calophoma, Didymella, Juxtiphoma, Nothophoma, Paraboeremia, Phomatodes, Stagonosporopsis, and Xenodidymella. Several isolates confirmed to be ubiquitous plant pathogens or endophytes were for the first time identified from soil, such as Ascochyta syringae, Calophoma clematidis-rectae, and Paraboeremia litseae. They are used as a model organism in environmental risk assessment of chemicals and soil toxicology. Soil provides physical and nutritive support to agriculture system by regulating biogeochemical cycles, nutrient cycle, waste degradation, organic matter degradation etc. The biggest threat to soil health are pesticides and synthetic chemicals including fertilizers. Earthworms are most severely hit by these xenobiotic compounds leading to a sizeable reduction of their population and adversely affecting soil fertility. Earthworms are incredible soil organisms playing a crucial role in maintaining soil health. Pesticides used in crop management are known to be most over-purchased and irrationally used soil toxicants, simultaneously, used insecticides contribute to a quantum of damage to earthworms and other non-target organisms. LC50 and LD50 studies revealed that earthworms are highly susceptible to insecticides causing immobility, rigidity and also show a significant effect on biomass reduction, growth and reproduction by disrupting various physiological activities leading to loss of earthworm population and soil biodiversity. As part of a Citizen Science Project initiated by the Westerdijk Fungal Biodiversity Institute and the Utrecht University Museum, which aimed to describe novel fungal species from Dutch garden soil, the diversity of Didymellaceae, which is one of the largest families in the Dothideomycetes was investigated. A preliminary analysis of the ITS and LSU sequences from the obtained isolates allowed the identification of 148 strains belonging to the family. Based on a multi-locus phylogeny of a combined ITS, LSU, rpb2 and tub2 alignment, and morphological characteristics, 20 different species were identified in nine genera, namely Ascochyta, Calophoma, Didymella, Juxtiphoma, Nothophoma, Paraboeremia, Phomatodes, Stagonosporopsis, and Xenodidymella. Several isolates confirmed to be ubiquitous plant pathogens or endophytes were for the first time identified from soil, such as Ascochyta syringae, Calophoma clematidis-rectae, and Paraboeremia litseae. The physicochemical properties of room temperature ionic liquids (RTILs) consisting of bis(trifluoromethanesulfonyl)amide (TFSA- ) combined with 1-hexyl-1-methylpyrrolidinium (Pyr1,6 + ), 1-(butoxymethyl)-1-methylpyrrolidinium (Pyr1,1O4 + ), 1-(4-methoxybutyl)-1-methyl pyrrolidinium (Pyr1,4O1 + ), and 1-((2-methoxyethoxy)methyl)-1-methylpyrrolidinium (Pyr1,1O2O1 + ) were investigated using both experimental and computational approaches. Pyr1,1O2O1 TFSA, which contains two ether oxygen atoms, showed the lowest viscosity, and the relationship between its physicochemical properties and the position and number of the ether oxygen atoms was discussed by a careful comparison with Pyr1,1O4 TFSA and Pyr1,4O1 TFSA. Ab initio calculations revealed the conformational flexibility of the side chain containing the ether oxygen atoms. In addition, molecular dynamics (MD) calculations suggested that the ion distributions have a significant impact on the transport properties. Based on the previous work of our group and others, this review summarizes the up-to-date progress on anion-modulated PGM compounds (such as RuP2 , IrP2 , PtP2 , RuB2 , Ru2 B3 , RuS2 , etc.) in energy-related electrocatalysis from the origin of their activity and synthetic strategies to electrochemical applications including hydrogen evolution reaction (HER), oxygen evolution reaction (OER), oxygen reduction reaction (ORR), hydrogen oxidation reaction (HOR), N2 reduction reaction (NRR), and CO2 reduction reaction (CO2 RR). The globus pallidus is an area of major iron metabolism and storage in the brain tissue. Calcium, another important metal in the body, is frequently deposited in the globus pallidus as well. Recently, we observed dense paramagnetic deposition with paradoxical calcifications in the globus pallidus and putamen. In this work, we explore detailed MR findings on these structures, and the histological source of the related findings using ex vivo CT and MR images. Ex vivo MR was obtained with a maximum 100 μm3 isotropic resolution using a 15.2 T MR system. 3D gradient echo images and quantitative susceptibility mapping were used because of their good sensitivity to metallic deposition, high signal-to-noise ratio, and excellent contrast to iron and calcium. We found dense paramagnetic deposition along the perforating arteries in the globus pallidus. This paramagnetic deposition was hyperdense on ex vivo CT scans. Histological studies confirmed this finding, and simultaneous deposition of iron and calcium, although more iron dominant, was observed along the vessel walls of the globus pallidus. This was an exclusive finding for the penetrating arteries of the globus pallidus. Thus, our results suggest that several strong and paradoxical paramagnetic sources at the globus pallidus can be associated with vascular degeneration.We report the synthesis of isotactic azobenzene-based polyesters (azopolyesters) with main-chain chirality via highly enantioselective resolution copolymerization of racemic azobenzene-containing epoxides with cyclic anhydrides. This demonstrates that the crystallinity of isotactic azopolyesters can be manipulated via photoinduced reversible isomerization.
