Because inter-subject variation may underline the importance of individualized retraining, a change-point analysis was used for each subject. The tuned change-point application detected major and subtle changes in the time series. No changes were found in the no-biofeedback condition. In the biofeedback condition, a first change in the axial peak tibial acceleration occurred on average after 309 running gait cycles (3'40"). The major change was a mean reduction of 2.45 g which occurred after 699 running gait cycles (8'04") in this group. The time needed to achieve the major reduction varied considerably between subjects. Because of the individualized approach to gait retraining and its relatively quick response due to a strong sensorimotor coupling, we want to highlight the potential of a stand-alone biofeedback system that provides real-time, continuous, and auditory feedback in response to the axial peak tibial acceleration for lower-impact running.The paper presents a theoretical model of the catalytic centers formation processes during annealing of multilayer nanosized metal films for carbon nanotubes growth. The approach to the description of the model is based on the mass transfer processes under the influence of mechanical thermoelastic stresses, which arise due to the difference in the thermal expansion coefficients of the substrate materials and nanosized metal layers. The thermal stress gradient resulting from annealing creates a drop in the chemical potential over the thickness of the film structure. This leads to the initiation of diffusion mass transfer between the inner and outer surfaces of the films. As a result, the outer surface begins to corrugate and fragment, creating separate islands, which serve as the basis for the catalytic centers formation. Experimental research on the formation of catalytic centers in the structure of Ni/Cr/Si was carried out. It is demonstrated that the proposed model allows to predict the geometric dimensions of the catalytic centers before growing carbon nanotubes. It has been demonstrated that obstructive sleep apnea is a significant risk factor for cardiovascular diseases, including arterial hypertension, ischemic heart disease, heart failure, rhythm/conduction disturbances, as well as cerebral stroke. The pathophysiology of these diseases is complex and multifactorial. We present the current state of research on behavioral and environmental factors that influence the relationship between OSA and cardiovascular changes. We discuss the relationship between obesity, alcohol, sedatives, environmental tobacco smoke, allergic diseases and environmental pollution on the one hand and OSA on the other. In this context, the environment should be considered as an important modulator of the relationship between OSA and cardiovascular diseases.

The present study isolated ten compounds including two kaempferol methyl ethers 5-hydroxy-3,7,4'-trimethoxyflavone (1), 3-hydroxy-5,7,4'-trimethoxyflavone (6); four quercetin methyl ethers retusin (5-hydroxy-3,7,3',4'-tetramethoxyflavone) (4), 3,5-dihydroxy-7,3',4'-trimethoxyflavone (5), 3,4'-dihydroxy-5,7,3'-trimethoxyflavone (7), and 3,5,7,3',4'-pentamethoxyflavone (9); β-sitosterol (2); 5-hydroxy-1-(4'-hydroxyphenyl)eicosan-3-one (3); p-hydroquinone (8); and vanillic acid (10) from the rhizomes and fruit of Amomum koenigii J.F.Gmel. (Zingiberaceae). Their structures were determined by MS, NMR, and X-ray spectroscopic techniques. The present study investigates the relationship between vitamin K plasmatic levels and cognitive functions in elderly patients on oral anticoagulant therapy (OAT). Design At the Thrombosis Centre of Haematology, "Sapienza" University of Rome, 85 patients on OAT, aged between 75 and 92, were randomly enrolled in the study. Patients were on OAT with vitamin K antagonists (VKAs). Vitamin K1 concentrations were determined using standardized High-Performance Liquid Chromatography (HPLC). Cognitive functions were assessed using the Milan Overall Dementia Assessment (MODA). Results MODA scores are positively correlated to vitamin K1 concentration. Patients with vitamin K1 below 0.100 μg/L and between 0.100 and 0.400 μg/L and between 0.100 and 0.400 μg/L and between 0.100 and 0.400 p 10 years) does not affect MODA scores. Education seems to exert a greater role on the cognitive status in comparison with aging. Conclusions The study shows a positive association between vitamin K1 concentration and cognitive status in elderly patients (≥75 years) on OAT. Empirical evidence shows that the elderly perceive distances as farther compared with younger adults and that the harder the ground surface to walk, the farther the perceived distance. Such results suggest a general perceptual readaptation promoted by the aging process that is fine-tuned with the decline of the motor resources. However, it is still unknown whether the elderly space perception is affected by interindividual differences in their functional autonomy (FA) and whether the decline of motor resources affects spatial categorization only when distances are judged with reference to the observer's own body or also when they are judged with reference to the body of another agent present in the scene. To this aim, a sample of elderly adults with preserved cognitive functions but different levels of FA, measured through the Instrumental Activity of Daily Living (IADL) scale, were enrolled and tested on the extrapersonal space categorization task. This task requires judging the position of a target as "Near" or "Far" with respect to different reference frames (RFs) centered on the observer's body (Self RF) or centered on external elements, like another body (Other RF) or an object (Object RF). Results indicated that the higher the level of FA, the wider the space categorized as "Near" when adopting as reference frame our own body or the body of another agent in the scene, but not a static object. In conclusion, the individual functional autonomy of elderly individuals, which is strongly influenced by motor resources and efficiency, modulates how the surrounding space is represented, but only when the distance judgment implies an agent body, thus providing new relevant data for recent embodied cognition models of aging. The infant presented in this video tutorial was diagnosed with myocarditis leading to end-stage heart failure and severe mitral valve regurgitation. A bridging therapy was indicated and a Berlin Heart EXCOR® VAD was implanted. This tutorial provides detailed insight on how to perform this procedure. The analysis results showed that the proposed method could be able to accurately predict the concentration of glucose, TC and TG in blood. This method can instantaneous multi-component detection on whole blood.Based on the specific interaction of Ag+ and cytosine-cytosine (C-C) base mismatch and using berberine (Ber) as the fluorescent probe and Exonuclease I (Exo I) as the background fluorescence reducing tool, a label-free Exo I-assisted fluorescence aptamer sensing platform was established for the detection of silver ions with high sensitivity and selectivity. Exo I reduced the fluorescence background of the Ber/Ag+-aptamer complex to a level similar to that of Ber itself in the absence of Ag+. After introducing Ag+ into the sensing system, it induces the aptamer rich in base C to form C-Ag+-C i-motif structure which are resistant to degradation mediated by Exo I. The concentration of Ber, Ag+-aptamer, Exo I and the temperature and reaction time for Exo I were all optimized. Under the optimal experimental conditions, the detection limit of Ag+ was 4.4 nM and the linear range was from 0.0059 μM to 235.48 μM with a coefficient of determination (R2) > 0.99. Moreover, the proposed strategy had been successfully applied to the detection of Ag+ in tap water and human serum with a good recovery ranging from 88.4% to 106.9%.Rare earth-carbon dots (RE-CDs) hybrid nanomaterials with the merits of both RE and CDs have rapidly emerging as highly promising functional materials in biochemical analysis. In this work, a new kind of water-soluble RE-CDs nanocomposite (CDs@CaF2Eu3+) was developed for the ratiometric determination of doxycycline (DOX). The CDs@CaF2Eu3+ under the excitation at 365 nm displayed blue emission of CDs at 440 nm and no obvious emission of Eu3+. With the addition of DOX, substantial fluorescence quenching of the CDs at 440 nm and enhancement of Eu3+ at 613 nm were observed, resulting in a ratiometric fluorescent response toward DOX. A wide linear range from 0.1 µM to 30 µM was achieved in the detection of DOX with a lowest detection limit of 43 nM. For this reason, in this work, we have developed for the first time a nanofiber-based sensing platform for sensitive and on-site DOX assay in just 10 min. This is obtained thanks to printable, porosity and embeddability features of electrospun nanofibrous films (ENFFs) combined with nitrogen and sulfur co-doped carbon dots (NS-CDs) as sensing probes. The assay was done by just pipetting analyte on the hydrophilic spots of the fabricated photoluminescence water-stable ENFFs where the color intensity was being darkened. DOX quenched NS-CDs fluorescence onto ENFFs through inner filter effect. The developed sensor was either coupled with smartphone technology to provide miniaturized, portable and easy-to-use device or an ordinary spectrofluorimeter for solid-state sensing applications (detection limit of 5.4 nM). Moreover, applicability of the designed sensor was evaluated in human serum with satisfactory recoveries. It is more interesting that the fabricated NS-CDs/ENF scaffolds have a high potential to detect the intracellular DOX to enhance cell proliferation leading to be considered as a multimodal tool in biomedical research and clinical diagnostics.
This study aimed to examine the reliability and validity of a newly developed Asthma Numeracy Test (ANT) and to make it available in Arabic.
Patients with asthma who were ≥18 years were seen in 3 outpatient primary care or respiratory medicine clinics in the United Arab Emirates. They completed the ANT of 10 questions assessing participant arithmetic computation, meaning of percentages, estimation, and problem solving. Exo I reduced the fluorescence background of the Ber/Ag+-aptamer complex to a level similar to that of Ber itself in the absence of Ag+. After introducing Ag+ into the sensing system, it induces the aptamer rich in base C to form C-Ag+-C i-motif structure which are resistant to degradation mediated by Exo I. The concentration of Ber, Ag+-aptamer, Exo I and the temperature and reaction time for Exo I were all optimized. Macrolide antibiotics are categorized by the WHO as Highest Priority, Critically Important Antimicrobials due to their recommendation as treatment for severe cases of campylobacteriosis in humans; a self-limiting, rarely life-threatening, zoonotic foodborne infection. Low rates of macrolide resistance in Campylobacter jejuni and the availability of alternative treatments have prompted some regulatory schemes to assign macrolides to a lower importance category. Apart from rare, specific infections, macrolides largely play a supportive role to other drug classes in human medicine. By contrast, although the advent of alternative control methods has seen significant reductions in macrolide use in intensive livestock, they still have a crucial role in the treatment/control of respiratory infections and liver abscesses in cattle. Whilst acknowledging that ongoing surveillance is required to reduce the spread of recently emerged, transferable macrolide resistance among Campylobacter, this article recommends that macrolides should be moved to the WHO Highly Important category.
Although hormonally-derived female sex pheromones have been well described in approximately a dozen species of teleost fish, only a few male sex pheromones have been characterized and the neuroendocrine underpinnings of behavioral responsiveness to them is not understood. Herein, we describe a study that addresses this question using the goldfish, Carassius auratus, an important model species of how hormones drive behavior in egg-laying teleost fishes. Our study had four components. First, we examined behavioral responsiveness of female goldfish and found that when injected with PGF2α, a treatment that drives female sexual receptivity, they became strongly and uniquely attracted to the odor of conspecific mature males, while non-PGF2α-treated goldfish did not discern males from females. Next, we characterized the complexity and specificity of the male pheromone by examining the responsiveness of PGF2α-treated females to the odor of either mature male conspecifics or male common carp odor, as well as their nly unattractive female conspecific water. Lastly, in a test of whether nonhormonal conspecific odor might determine species-specificity, we added AD to water of 3 species of fish and found that while AD made goldfish water strongly attractive, its effects on other species holding water were small. We conclude that circulating PGF2α produced at the time of ovulation induces behavioral sensitivity to a male sex pheromone in female goldfish and that this male pheromone is comprised of AD and a mixture of polar body metabolites. Because PGF2α commonly mediates ovulation and female sexual behavior in egg-laying fishes, and AD is universally produced by male fishes as a precursor to testosterone, we suggest that these two hormones may have similar roles mediating male-female behavior and communication in many species of fish.
People with autoimmune or inflammatory conditions taking immunomodulatory/suppressive medications may have higher risk of novel coronavirus disease 2019 (COVID-19). Chronic disease care has also changed for many patients, with uncertain downstream consequences.
We included participants with autoimmune or inflammatory conditions followed by specialists at Johns Hopkins. Participants completed periodic surveys querying comorbidities, disease-modifying medications, exposures, COVID-19 testing and outcomes, social behaviors, and disruptions to healthcare. We assessed whether COVID-19 risk is higher among those on immunomodulating or suppressive agents and characterized pandemic-associated changes to care and mental health.

The study sought to assess the clinical performance of a machine learning model aiming to identify unusual medication orders.
This prospective study was conducted at CHU Sainte-Justine, Canada, from April to August 2020. An unsupervised machine learning model based on GANomaly and 2 baselines were trained to learn medication order patterns from 10 years of data. Clinical pharmacists dichotomously (typical or atypical) labeled orders and pharmacological profiles (patients' medication lists). Confusion matrices, areas under the precision-recall curve (AUPRs), and F1 scores were calculated.
A total of 12 471 medication orders and 1356 profiles were labeled by 25 pharmacists. Medication order predictions showed a precision of 35%, recall (sensitivity) of 26%, and specificity of 97% as compared with pharmacist labels, with an AUPR of 0.25 and an F1 score of 0.30. Profile predictions showed a precision of 49%, recall of 75%, and specificity of 82%, with an AUPR of 0.60, and an F1 score of 0.59. The model performed better than the baselines. According to the pharmacists, the model was a useful screening tool, and 9 of 15 participants preferred predictions by medication, rather than by profile.

Results Forty (40%) LT- candidates had osteopenia or osteoporosis and 34 (38%) were malnourished. Malnutrition was associated with osteopenia/osteoporosis (odds ratio 3.5, 95% CI 1.4, 9.9). The anvil placement time was 5-14 min, with an average of 6.7 min. The postoperative pain visual analogue scale (VAS) score was 1-4 points, with an average of 2.2 points. The postoperative hospital stay was 7-13 days, with an average of 8.7 days. No complications, such as anastomotic bleeding or stenosis, occurred. Dance-based interventions had a duration of 8-52 weeks, 1-3 sessions per week, with sessions lasting 40-90 min. According to the overall effect size, dance-based interventions significantly improved dual-task performance (-0.48 with a 95% confidence interval from -0.88 to -0.08).Conclusions Dance-based intervention programs may be effective at improving the performance on motor-cognitive dual-task in middle-aged and older adults. Apart from the effects on the cognitive Time Up and Go test, dance interventions may also improve speed walking with a cognitive task. All these conclusions must be taken with caution due to the heterogeneity and the small number of articles. High baseline TNF-α correlated with a more marked decline in BMD (Partial correlation (r) = -0.47, p less then .05) as did high baseline cortisol levels (r = -0.49, p less then .05).Conclusion Malnutrition in liver cirrhosis seems to be associated with osteopenia/osteoporosis, and systemic inflammation (higher TNF-α) and systemic stress (higher cortisol) to bone loss in patients who undergo LT.Objective To evaluate the feasibility and efficacy of the modified reverse puncture device (mRPD) technique for transanal anastomosis in total laparoscopic colorectal resection with natural orifice specimen extraction surgery (NOSES).Methods From August 2015 to September 2017, 34 patients underwent laparoscopic colorectal resection using the mRPD technique to place the anvil in the abdominal cavity and complete transanal anastomosis.Results All patients who underwent total laparoscopic colorectal resection with NOSES were analyzed. The anvil placement time was 5-14 min, with an average of 6.7 min. The postoperative pain visual analogue scale (VAS) score was 1-4 points, with an average of 2.2 points. The postoperative hospital stay was 7-13 days, with an average of 8.7 days. No complications, such as anastomotic bleeding or stenosis, occurred. Dance-based interventions had a duration of 8-52 weeks, 1-3 sessions per week, with sessions lasting 40-90 min. According to the overall effect size, dance-based interventions significantly improved dual-task performance (-0.48 with a 95% confidence interval from -0.88 to -0.08).Conclusions Dance-based intervention programs may be effective at improving the performance on motor-cognitive dual-task in middle-aged and older adults. Apart from the effects on the cognitive Time Up and Go test, dance interventions may also improve speed walking with a cognitive task. All these conclusions must be taken with caution due to the heterogeneity and the small number of articles. Within each cluster, 30 people aged 50+ were selected using compact-segment-sampling. All participants completed smartphone-based audiometry (hearTest). Prevalence was estimated using WHO definitions (PTA of thresholds 0.5, 1, 2, 4 kHz in the better ear of >25 dB HL (any) and >40 dB HL (≥moderate)). Otoscopy and questionnaire were used to assess probable causes. Participants with hearing loss and/or ear disease were asked about care-seeking and barriers.Results Four teams completed the survey in 24 days. 1080 of 1153 (93.7%) participants were examined. The median time to complete the protocol was 24 min/participant. Prevalence of hearing loss was 35.9% (95% CI = 31.6-40.2) (any level); and 10.0% (95% CI = 7.9-12.5) (≥moderate). Here we conducted a comprehensive biophysical characterization of stoichiometry and protein diffusion across the plasma membrane of the epithelial KCNQ1-KCNE2 complex, combining total internal reflection fluorescence (TIRF) microscopy and a series of complementary Fluorescence Fluctuation Spectroscopy (FFS) techniques. Using this approach, we found that KCNQ1-KCNE2 has a predominant 44 stoichiometry, while non-bound KCNE2 subunits are mostly present as dimers in the plasma membrane. At the same time, we identified unique spatio-temporal diffusion modalities and nano-environment organization for each channel subunit. Being the first phylogenetic analyses to include the holopelagic Drieschia, we recovered this species nested among shallow water terminals, suggesting a shallow water ancestry. Based on these results, our phylogenetic reconstructions showed that swimming evolved independently three times in Polynoidae, involving convergent adaptations in morphology and motility patterns across the deep sea (Branchipolynoe), midwater (Drieschia) and anchialine caves (Pelagomacellicephala and Gesiella). Phylogenetic generalized least-squares (PGLS) analyses showed that holopelagic and anchialine cave species exhibit hypertrophy of the dorsal cirri, yet, these morphological modifications are achieved along different evolutionary pathways, i.e., elongation of the cirrophore versus style. During Phase 1, each participant received brief thermal pain stimuli during interactive embodied avatar VR vs. passive VR (no avatar and no interactivity), VR treatment order randomized. After each pain stimulus, participants provided subjective 0-10 ratings of pain. Compared to the passive VR condition, during the interactive avatar VR, participants reported significant reductions in (1) worst pain, (2) pain unpleasantness, (3) time thinking about pain and (4). they had significantly more fun during the pain stimulus (p = .000 for each). We provide a framework which takes into account the energy-consumption required for self-propulsion and address the question of how energy-efficient the emergent interactions are. Upon conditioning, run-and-tumble particles develop an alignment interaction and achieve a higher gain in efficiency than TASEP particles. A point of diminishing returns in efficiency is reached beyond a certain level of conditioning. With recourse to a general formula for the change in energy efficiency upon conditioning, we conclude that the most significant gains occur when there are large fluctuations in mobility to exploit. From a detailed comparison of the emergent effective interaction in a two- versus a three-body scenario, we discover evidence of a screening effect which suggests that conditioning can produce topological rather than metric interactions.The time-dependent variational principle is used to optimize the linear and nonlinear parameters of Gaussian basis functions to solve the time-dependent Schrödinger equation in one and three dimensions for a one-body soft Coulomb potential in a laser field. Based on additional numerical calculations of these outcomes, we discuss possible origins of such differences between two engines and reveal them. Our results imply that, even with an equilibrium bath, a nonequilibrium working fluid brings on the higher performance than what an equilibrium working fluid does.The entanglement of eigenstates in two coupled, classically chaotic kicked tops is studied in dependence of their interaction strength. The transition from the noninteracting and unentangled system toward full random matrix behavior is governed by a universal scaling parameter. Using suitable random matrix transition ensembles we express this transition parameter as a function of the subsystem sizes and the coupling strength for both unitary and orthogonal symmetry classes. The universality is confirmed for the level spacing statistics of the coupled kicked tops and a perturbative description is in good agreement with numerical results. The statistics of Schmidt eigenvalues and entanglement entropies of eigenstates is found to follow a universal scaling as well. Remarkably, this is not only the case for large subsystems of equal size but also if one of them is much smaller.
