  • The lives of lesbian, gay, bi-, pan-, asexual, and transgender (LGBTA+/LGBT) people are not considered to be standard in society, unlike those of heterosexual cisgender people. This can lead to prejudices against LGBT people and may negatively influence their access to high-quality health care. Medical and mental health care have been characterized by attitudes (psycho-)pathologizing LGBT lives and therefore supported the stigmatization of LGBT people in the service of heteronormativity. Mental health professionals (MHPs) largely have transferred principles guiding counseling and psychotherapy with heterosexual (straight) cisgender persons to treatment of LGBT individuals without considering the specific features of LGBT lives. This is true even if the treatment is not exclusively LGBT-related, but can address LGBT-unrelated issues. To counteract this, the present paper aims to provide an insight into ethically sound mental health care for LGBT people. By applying the principles of biomedical ethics, we have analyzed how LGBT individuals can be discriminated against in mental health care and what MHPs may need to offer LGBT-sensitive high-quality mental health care. We argue that MHPs need LGBT-related expertise as well as LGBT-related sensitivity. We argue that if living and deceased donation demonstrate equivalent clinical efficacy and the deceased donor pool is sufficient, living uterus donation should be reevaluated and may no longer be ethically justifiable.Not only do epidemics such as HIV/AIDS, Ebola Virus Disease (EVD), and the current Coronavirus Disease (COVID-19) cause the loss of millions of lives, but they also cost the global economy billions of dollars. Consequently, there is an urgent need to formulate interventions that will help control their spread and impact when they emerge. The education of young girls and women is one such historical approach. They are usually the vulnerable targets of disease outbreaks - they are most likely to be vehicles for the spread of epidemics due to their assigned traditional roles in resource-limited countries. Based on our work and the work of others on educational interventions, we propose six critical components of a cost-effective and sustainable response to promote girl-child education in resource-limited settings.Endometriosis, despite only affecting 10-15% of women of fertile age, is still an enigmatic disease. Recent developments in assisted reproductive technology have contributed to a better understanding of where and how endometriosis could compromise fertility. In this mini-review we will show how the main point of damage in endometriosis is quantitative impairment of the ovaries, if the "less is more" mantra should be applied when considering ovarian surgery, and when fertility preservation prior to ovarian surgery could be considered. Endometrial receptivity, however, does not seem to be affected.The trend in our society to delay procreation increases the difficulty to conceive spontaneously. Thus, there is a growing need to use assisted reproduction technologies (ART) to form a family. With advanced maternal age, ovaries not only produce a lower number of oocytes after ovarian stimulation but also a lower quality-mainly aneuploidies-requiring further complex analysis to avoid complications during implantation and pregnancy. Although there are different options to have a child at advanced maternal age (like donor eggs), this is not the preferred choice for most patients. Unless women had cryopreserved their eggs at a younger age, reproductive medicine should try to optimize their opportunities to become pregnant with their own oocytes, when chances of success are reasonable. Aging has many causes, but telomere attrition is ultimately one of the main pathways involved in this process. Several reports link telomere biology and reproduction, but the molecular reasons for the rapid loss of ovarian function at middle age are still elusive. This review will focus on the knowledge acquired during the last years about ovarian aging and disease, both in mouse models of reproductive senescence and in humans with ovarian failure, and the implication of telomeres in this process. In addition, the review will discuss recent results on ovarian rejuvenation, achieved with stem cell therapies that are currently under study, or ovarian reactivation by tissue fragmentation and the attempts to generate oocytes in vitro.An estimated 37 million people globally suffer from Human Immunodeficiency Virus-1 (HIV-1) infection with 1.7 million newly acquired infections occurring on average each year. Although crucial advances in combined antiretroviral therapy (ART) over the last two decades have transformed an HIV-1 diagnosis into a tolerable and controlled condition, enabling over 20 million people living with HIV-1 to enjoy healthy and productive lives, no cure or vaccine yet exists. Developing a successful cure strategy will require a firm understanding of how viral latency is established and how a persistent and long-lived latent is generated. The latent reservoir remains the primary obstacle for cure development and most putative cure strategies proposed fundamentally address its eradication or permanent suppression.

We describe a case of an 80-year-old man who presented with unilateral right-sided pleural effusion. He underwent therapeutic thoracocentesis and was subsequently diagnosed with a pleuroperitoneal leak through pleural fluid analysis. Peritoneal dialysis was ceased, and he transitioned temporarily to hemodialysis. He was subsequently treated with talc pleurodesis and successfully recommenced on peritoneal dialysis at six weeks after operation. Group II patients scoring 4 or more were counseled to optimize their modifiable risk factors before proceeding with surgery. Group II patients scoring 4 or more were counseled to optimize their modifiable risk factors before proceeding with surgery. Retrospective chart review was done for patients in group I to find out their risk score for the study purpose. Of note, clinical data demonstrate that high level of miR-375 correlates with BM metastasis in NB patients. To do so, the interaction mechanisms of the four subsystems are examined. Results show that (a) only transportation and social carrying capacity had causality between them as per the Granger causality test; (b) all four carrying capacities can support themselves (i.e., the development of economic conditions helps to improve social carrying capacity and the improvement of social carrying capacity helps to improve environmental carrying capacity); and (c) both the four carrying capacities are mostly affected by their own fluctuations. Overall transportation carrying capacity is the most important driving force among the four subsystems that interact with each other; followed by economic carrying capacity, which has promotion effect on the social and environmental aspects; social carrying capacity poses impact on the environmental dimension but not vice versa. On the other hand, biofilm forming and antibiotic producing bacteria, nutrient limiting condition sustaining bacteria, and those capable of synthesizing enzymes prerequisite for active metabolism under stress, increased over time. The shifts in the bacterial community were dependent on mechanisms adopted by the clades to cope with the BW tank conditions. Functional prediction of the bacterial community revealed a significant increase in the core metabolic functions, which enabled the survival of such bacteria. As the voyage progressed, an increase in the total viable bacteria in BW tanks could be attributed to the decrease in the abundance of phytoplankton and zooplankton. At the end of the voyage, the bacterial community in the BW tanks was significantly different, and the species diversity and richness were higher than that of the natural seawater (source water). Pathogenic species were more abundant during mid-voyage than at the end of the voyage, suggesting that voyage duration influences the pathogenic bacterial community.     A descriptive cross-sectional study was conducted between January 2014 and December 2019 on 103 chronic dialysis patients with end-stage renal disease presenting to a tertiary care hospital. Ethical approval was received from the institutional review board (2/(6-11) E2/076/77). Whole sampling was done. Data entry and analysis were done in Microsoft Excel 10.

    The study included 103 patients with 117 cuffed tunneled dialysis catheters placed for hemodialysis. On assessing the outcome of the catheters, the primary and secondary patency rates of the catheters were 5.85±4.87 and 1.21±3.77 months. Thirty-one (30.1%) patients required one intervention, and 11 (10.68%) catheters required 3 or more interventions to maintain patency. Eighteen (17.48%) patients presented with catheter dysfunction while in 11 (10.68%) cases, the catheter was kinked or malpositioned at the notch. In one patient, procedure was abandoned due to severe bleeding and in 2 (1.94%) patients dialysis catheters could not be negotiated into the right atrium and left in brachiocephalic junction.

    Results showed that stress during exam 210 (92.9%) and preparation phase 200 (88.5%) stood out as the maximum stressors for our study group. The least stress-causing element was recorded as terms with seniors 45 (19.9%), adjustment with roommates 52 (23.01%), and competing with peers 69 (30.53%). Length of course 187 (82.74%), understanding the course 173 (76.55%), reading several textbooks 171 (75.66%), and work overload 165 (73.01%) amounted to significant stressors.

    Neonatal sepsis is the most important cause of morbidity and mortality among low birth weight and preterm babies in developing countries. The main objective of this study is to find the level of micro-Erythrocyte sedimentation rate in neonatal sepsis.

All preterm, term and post-term babies with neonatal sepsisdelivered at Kathmandu Medical College Teaching Hospital were enrolled. Convenient sampling method was applied and statistical analysis was done with Statistical package for social sciences 19 version.

The objective of this study is to find out the prevalence of domestic violence among infertile women attending subfertility clinic of tertiary hospital.

Face to face interview was conducted using a structured interview schedule. Data analysis was done in the Statistical Package for Social Sciences. This descriptive cross-sectional study was conducted among infertile women in a tertiary hospital from July to August 2018 after taking ethical approval. Convenient sampling was used. Face to face interview was conducted using a structured interview schedule. Data analysis was done in the Statistical Package for Social Sciences. Descriptive statistics (frequency, percentage) were used to analyze the data. Point estimate at 95% CI was calculated along with frequency and proportion for binary data.   • The question of whether a population will persist or go extinct is of key interest throughout ecology and biology. Various mathematical techniques allow us to generate knowledge regarding individual behaviour, which can be analysed to obtain predictions about the ultimate survival or extinction of the population. A common model employed to describe population dynamics is the lattice-based random walk model with crowding (exclusion). This model can incorporate behaviour such as birth, death and movement, while including natural phenomena such as finite size effects. Hence, we cannot yet discriminate between the quiet-Sun TSI being enhanced or reduced during the Maunder and Dalton sunspot minima, although there is a growing consensus from the combinations of models and observations that it was slightly enhanced. For this reason, a technique developed in the Janashia-Lagvilava matrix spectral factorization method is applied. Numerical simulations illustrate our ideas in simple situations that demonstrate the potential of the method.In the work, we obtain an effective criterion of the stability of the partial indices for matrix polynomials under an arbitrary sufficiently small perturbation. Verification of the stability is reduced to calculation of the ranks for two explicitly defined Toeplitz matrices. Furthermore, we define a notion of the stability of the partial indices in the given class of matrix functions. This means that we will consider an allowable small perturbation such that a perturbed matrix function belong to the same class as the original one. We prove that in the class of matrix polynomials the Gohberg-Krein-Bojarsky criterion is preserved, i.e. new stability cases do not arise. Systemic administration of ELA exerts vasodilatory, antihypertensive, cardioprotective, and renoprotective effects, whereas central application of ELA increases blood pressure and causes cardiovascular remodeling primarily secondary to the hypertension. We performed multiple logistic regression to test the primary and secondary objectives using SAS 9.4.

    Out of 531 patients included in the analysis, 207 (39%) were on ACE-i/ARB at baseline.     Clinical and experimental evidence regarding the influence of heart rate (HR) on arterial stiffness and its surrogate marker carotid-to-femoral pulse wave velocity (cf-PWV) is conflicting. We aimed to evaluate the effect of HR on cf-PWV measurement under controlled haemodynamic conditions and especially with respect to blood pressure (BP) that is a strong determinant of arterial stiffness.

    Fifty-nine simulated cases were created using a previously validated in-silico model. For each case, cf-PWV was measured at five HR values, 60, 70, 80, 90, 100 bpm. With increasing HR, we assessed cf-PWV under two scenarios with BP free to vary in response to HR increase, and with aortic DBP (aoDBP) fixed to its baseline value at 60 bpm, by modifying total peripheral resistance accordingly. Further, we quantified the importance of arterial compliance (C) on cf-PWV changes caused by increasing HR.

    When BP was left free to vary with HR, a significant HR-effect on cf-PWV (0.66 ± 0.24 m/s per 10 bpm, P < 0.001) was observed. This effect was reduced to 0.21 ± 0.14 m/s per 10 bpm (P = 0.048) when aoDBP was maintained fixed with increasing HR. The HR-effect on the BP-corrected cf-PWV was higher in the case of low C = 0.8 ± 0.3 ml/mmHg (0.26 ± 0.15 m/s per 10 bpm, P = 0.014) than the case of higher C = 1.7 ± 0.5 ml/mmHg (0.16 ± 0.07 m/s per 10 bpm, P = 0.045).

    The AHA/ACC-2017 hypertension guideline recommends an age-independent target blood pressure (BP) of less than 130/80 mmHg. In an elderly cohort without established cardiovascular disease (CVD) at baseline, we determined the impact of this guideline on the prevalence of hypertension and associated CVD risk.

    Nineteen thousand, one hundred and fourteen participants aged at least 65 years from the ASPirin in Reducing Events in the Elderly (ASPREE) study were grouped by baseline BP 'pre-2017 hypertensive' (BP ≥140/90 mmHg and/or on antihypertensive drugs); 'reclassified hypertensive' (normotensive by pre-2017 guidelines; hypertensive by AHA/ACC-2017 guideline), and 'normotensive' (BP <130 and <80 mmHg). The objective of this study was to chemically characterize Moro orange juice. The influence of juice intake on metabolic parameters of rats that were obese, diabetic or both by means of body mass management after the activation of anthocyanins during refrigerated storage was also assessed.

    Juice intake reversed most of the metabolic abnormalities exhibited by obese rats, including reduction of body mass and improvement of the biochemical profile. The loss of body mass by diabetic animals and obese and diabetic animals was not attributed to juice intake, but to type I diabetes mellitus. In these animals, no improvement was observed in the biochemical profile, liver enzymes and glycemia, so this treatment model was deemed unfeasible.

    The beneficial effects cannot be explained only by the anthocyanin C3G present in the juice, but rather, by the synergism between all the components. Studies in humans are needed to determine whether the ingestion of this orange can be recommended as an effective strategy to prevent or ameliorate complications of obesity. © 2020 Society of Chemical Industry.

    To determine IQ at 26 to 30 years in very-low-birthweight (VLBW) adults compared with term-born controls; and to examine the stability of IQ in VLBW individuals between 7 to 8 years and 26 to 30 years, identify perinatal and social predictors of IQ, and assess the contribution of brain volume to IQ.

    At 26 to 30 years, 229 VLBW adults (71% survivors of prospectively enrolled national cohort) and 100 term-born controls were tested on the Wechsler Abbreviated Scale of Intelligence. For VLBW, IQ at 7 to 8 years, perinatal and social data were extracted from the data set, and 150 adults underwent volumetric cranial magnetic resonance imaging (MRI).

    At 26 to 30 years, the mean adjusted difference between VLBW and controls for total IQ was 9.4 (95% CI 6.5-12.4) points. In VLBW individuals the correlation between IQ scores at 7 to 8 years and 26 to 30 years was 0.78. On multiple regression analysis, parental education was the strongest predictor of verbal and total IQ at both ages. Birthweight was a strong predictor of perceptual and total IQ. We further show that the data are explainable by a simple model in which the wing traits are effectively neutral at least within a range of phenotypic values but their evolutionary rates are differentially reduced because mutations affecting these traits are purged owing to their different pleiotropic effects on other traits that are under stabilizing selection.
