  • Multiple-choice questions (MCQs) are useful in assessing student performance, covering a wide range of topics in an objective way. Its reliability and validity depend upon how well it is constructed. Defective Item detected by item analysis must be looked for item writing flaws and optimized. The aim of this study was to evaluate the MCQs for difficulty levels, discriminating power with functional distractors by item analysis, analyze poor items for writing flaws, and optimize.

    This was a prospective cross-sectional study involving 120 MBBS students writing formative assessment in Ophthalmology. It comprised 40 single response MCQs as a part of 3-h paper for 20 marks. Items were categorized according to their difficulty index, discrimination index, and distractor efficiency with simple proportions, mean, standard deviation, and correlation. The defective items were analyzed for proper construction and optimized.

    The mean score of the study group was 13.525 ± 2.617. Mean difficulty index, discrimination index, and distractor efficiency were 53.22, 0.26, and 78.32, respectively. Among 40 MCQs, twenty-five MCQs did not have non-functioning distractor; 7 had one, 5 had two, and 3 had three. Of the 20 defective items, 17 were optimized and added to the question bank, two were added without modification, and one was dropped.

    National Accreditation Board for Hospitals and Healthcare Providers operates the health-care accreditation program in India. Research on impact of accreditation on eye-care centers is scarce. This article was conceptualized to scientifically evaluate the changes in documentation brought about by accreditation and its effects on staff in the Ophthalmology Department of an Indian Medical College.

    This was conducted as a quasi-experimental study in four steps. First, a point-based evaluation of case sheets in both pre-accreditation and postaccreditation phase, along with statistical analysis of the results, was done. Step two involved analysis of quality indicators and patient safety indices in successive years from inception of data to post-accreditation phase. Step three comprised a survey of staff employing a validated tool, and finally, face-to-face semistructured interviews with designated authorities, including finance departmental head, completed the study.

    Retrospective study of 11 patients for their demographic, clinical features and imaging including SD OCT (tumour location, extent and interface) and OCT-A (surface and intrinsic vascularity) were reviewed. Flow rate percentage (FR %) was calculated over the lesion and compared to fellow eye and similar pigmented lesions.

    The average age was 52.8 ± 10.9 years. ONH-MCT tumors occupied 3-tissue spaces- optic disc (n = 2), retinal layer (n = 5) and retina-choroidal layers (n = 4). SD OCT (11 eyes) showed elevated hyper reflective disorganized retinal layers with posterior shadowing (9 eyes) and hyper reflective dots within the tumor (all eyes). Microvascular features on OCT-A (8 eyes) in radial peripapillary capillary slab showed surface vascularization (7 eyes) and intrinsic vascularity in choroidal slab (8 eyes) with surrounding hypo reflective boundary. The mean FR % was higher at 65.1 ± 3.77% (CI 61.9-68.2) compared to mean FR at 60.4 ± 1.06% (CI 59.5-61.2) in the fellow eye (p = 0.01). Comparison with nevus and melanoma SD OCT showed a high reflective choroidal layer with normal or irregular outer retinal layers respectively; OCT-A showed hypo reflective area at the center with hyper reflective boundary and iso reflective area at center with hyper reflective boundary respectively.

    The aim of this study was to evaluate the efficacy and safety of intravitreal moxifloxacin injections in the treatment of acute post-cataract surgery endophthalmitis with visual acuity equal to or greater than hand movements.

    Fifty two patients with post-cataract surgery endophthalmitis who presented within 6 weeks with visual acuity equal to or greater than hand movements received two intravitreal moxifloxacin injections 48 h apart. Patients with prior history of ocular disease or treatment were excluded. Patients were followed up to 3 months either for resolution of endophthalmitis or worsening of disease. Number of patients who achieved visual acuity equal to or better than 20/40 and 20/200 at the third-month follow-up visit were compared with the number of patients at presentation, using Chi-square test. In addition, pretreatment LogMAR visual acuity at the time of presentation was compared with posttreatment visual acuity at end of third month using paired t test.

    Fifty (96.15%) patients showed complete resolution of endophthalmitis while three patients required vitrectomy.   • While quality improvement (QI) and clinical research embody two distinct scientific approaches, they have the same ultimate goal-to improve health and patient care outcomes. By leveraging their respective strengths there is a higher likelihood of achieving and sustaining health improvements. Our objective was to create recommendations to enhance the collaboration of the Canadian emergency medicine QI and clinical research communities.

    An expert panel of eight ED clinicians with diverse QI and clinical research expertise drafted a list of recommendations based on their professional expertise and a scoping review of the literature. These recommendations were refined through consultation with national stakeholders and reviewed at the 2020 CAEP Virtual Academic Symposium, where feedback was received through several virtual platforms.

    The final six recommendations include that all emergency medicine providers should (1) understand the role and application of both clinical research and QI science; that academic emergency medicine physicians should (2) contribute to both local adoption and broad dissemination of project findings, (3) leverage QI methodologies in research projects to improve knowledge translation, and (4) ensure that project outcomes prioritize patient care; and that academic leaders should (5) enhance the infrastructure for oversight of research and QI projects, and (6) encourage collaboration between researchers and QI experts by ensuring that academic and operational infrastructures align and support both.

We isolated, identified and extracted of their secondary metabolites of carbapenem resistant Gram-negative bacteria derived from cancer patients. Various methods have been used to determine hepatotoxicity, nephrotoxicity, tumorigenesis, inflammatory and sperm abnormalities in the albino rats injected with extracts. In comparison with the normal animals group, all extracts induced hepatotoxicity which was evidenced by the significant elevation in the activity of the serum alanine aminotransferase, aspartate aminotransferase, gamma-glutamyltransferase and alkaline phosphatase; also, nephrotoxicity that was indicated through the marked increase in the serum urea and creatinine levels; tumorigenesis was achieved from the sharp elevation in serum levels of alpha fetoprotein, carcinoembryonic antigen and lactate dehydrogenase values as tumor markers; as well as severe inflammatory characteristics were monitored from the marked raise of tumor necrosis factor alpha and interleukin-1beta. Furthermore, the proportion of micronuclei in polychromatic erythrocytes and sperm abnormalities were statistically significant in all groups compared to control group. Various kinds of head abnormalities and coiled tail were noted. Histopathological examination of hepatic tissue came in line with the biochemical and cytological findings. It could conclude that the extracts of Serratia sp. Esraa 1, Stenotrophomonas sp. Esraa 2, Acinetobacter sp. Esraa 3, Escherichia sp. Esraa 4 and Pseudomonas sp. The present prospective randomized crossover study aimed to assess the changes in driving pressure in response to variations in the level of assistance delivered by PSV vs NAVA.

    16 intubated adult patients, recovering from hypoxemic acute respiratory failure (ARF) and undergoing assisted ventilation, were randomly subjected to six 30-min-lasting trials. Traditionally, research on this aspect of memory has focused on the contributions of particular brain regions to recognition responses, but the interaction between regions may be of even greater importance to a full understanding. In this study, we examined patterns of network connectivity during retrieval in a recognition memory task. We estimated connectivity between brain regions from electroencephalographic signals recorded from twenty healthy subjects. A multivariate autoregressive model (MVAR) was used to determine the Granger causality to estimate the effective connectivity in the time-frequency domain. https://www.selleckchem.com/products/blebbistatin.html We used GPDC and dDTF methods because they have almost resolved the previous volume conduction and bivariate problems faced by previous estimation methods.   • Dermatology being a visual branch, there is a need to add a visual element in learning and assessment of dermatology. This study compares the utility of image-based assessment (IBA) as a new tool compared to routinely used semi-structured viva (SSV) in dermatology formative assessment at undergraduate level.

    Comparison was made between batches of students in year 2018 who underwent clinical posting term ending assessment by IBA with the retrospective cohort of batch of students in year 2015 who underwent assessment by SSV. The students' marks in this assessment and their attendance were collected. Feedback was taken from batch of students who had undergone IBA assessment. Faculty feedback was also taken.

    Correlation of attendance with marks was higher in IBA batch compared to SSV. IBA is better able to assess the diagnostic skills which requires visual element and prescription writing skill. SSV can do an authentic assessment of clinical reasoning skills. IBA had higher variability in marks allotted to students suggesting that it was more objective tool whereas with narrow range of marks SSV was found to be more subjective. Both IBA and SSV had similar acceptability by students and faculty. IBA was more resource intensive at preparation stage while SSV was so in conduction stage. IBA had better educational impact, as it promoted learning through exposure to actual patients.

    Sports Medicine is an upcoming postgraduate speciality in India. A MD Sports Medicine specialist is expected to contribute in the prevention of sports injuries, sportsmen training and enhancement of performance apart from being involved in planning of conduct of sports events amongst many other roles and responsibilities. This requires hands-on training and acquisition of skill sets required to perform these roles. The National Medical Council of India highlights the need for a competency based curriculum and has laid down guidelines for the course. There was a felt need to develop Entrustable Professional Activities (EPAs) and milestones based on the prescribed curricula and develop a portfolio for continuous monitoring of the achievement of these EPAs.

    A five step model was done by experts in the field to prepare the EPAs, milestones and portfolio. This consisted of faculty development, identification of the EPAs and milestones and portfolio preparation.

There is a need to develop an innovative IPE based module on communication skills for postgraduates stressing upon the existing lacunae and requirements detected by a validated assessment tool. We assessed the residents' appraisal of communication skills by a validated questionnaire to identify key areas to improve their communication skills and to determine perceptions of all the stakeholders i.e. faculty, nursing staff and paramedical staff.

    A descriptive correlational design using an online self-administered questionnaire on a sample of a total of 200 participants comprising residents, paramedical staff, nursing staff and faculty of tertiary care hospital. Study was approved by the Institutional Ethical Committee. Data was analysed by SPSS Version 22.0 used. (p value<0.05 significant).

    The participants included 113 residents, 17 paramedical staff, 33 Nursing staff and 37 faculty members. The present study found a significant difference of scores of means between the residents, faculty, paramedical staff and nursing staff p value<0.05 except for alter centrism, interaction management and environmental control. The mean scores were also significantly different between the first, second and third year residents.

    Dermatology being a visual branch, there is a need to add a visual element in learning and assessment of dermatology. This study compares the utility of image-based assessment (IBA) as a new tool compared to routinely used semi-structured viva (SSV) in dermatology formative assessment at undergraduate level. Comparison was made between batches of students in year 2018 who underwent clinical posting term ending assessment by IBA with the retrospective cohort of batch of students in year 2015 who underwent assessment by SSV. The students' marks in this assessment and their attendance were collected. Feedback was taken from batch of students who had undergone IBA assessment. Faculty feedback was also taken. Correlation of attendance with marks was higher in IBA batch compared to SSV. IBA is better able to assess the diagnostic skills which requires visual element and prescription writing skill. SSV can do an authentic assessment of clinical reasoning skills. IBA had higher variability in marks allotted to students suggesting that it was more objective tool whereas with narrow range of marks SSV was found to be more subjective. Both IBA and SSV had similar acceptability by students and faculty. IBA was more resource intensive at preparation stage while SSV was so in conduction stage. IBA had better educational impact, as it promoted learning through exposure to actual patients. IBA fared better in terms of validity, reliability, acceptability, and educational impact. The National Medical Council of India highlights the need for a competency based curriculum and has laid down guidelines for the course.   • Compared with the traditional honeycomb protection structure, the energy absorption of the initially designed composite honeycomb sandwich structure was improved effectively.Strawberry, belonging to the Fragaria genus, is an important crop that produces popular fruits globally. F. vesca, known as wild strawberry, has great characteristics, such as a robust and powerful aroma, making it an important germplasm resource. The present study aims to establish an efficient regeneration method for the in vitro propagation of F. vesca. Firstly, leaf explants were used to induce callus formation on a Murashige and Skoog medium with combinations of cytokinins (thidiazuron (TDZ) and 6-benzylaminopurine (BA)) and auxin (2,4-dichlorophenoxyacetic acid (2,4-D)). Among them, 0.45-4.54 µM TDZ combined with 0.45-4.53 µM 2.4-D exhibited a high induction rate after 4 weeks of culturing. Different explants were examined for their ability to form a callus, and whole leaves on the medium containing 2.27 µM TDZ and 2.27 µM 2,4-D showed the highest callus induction rate at 100% after 2 weeks of culturing in darkness. A protocol for the efficient regeneration of wild strawberry was established, which might be useful for their large-scale propagation or obtaining transgenic plants in the future.While dengue virus (DENV) infection imposes a serious challenge to the survival of humans worldwide, severe acute respiratory syndrome-coronavirus (SARS-CoV) remains the most devastating pandemic in human history. A significant number of studies have shown that plant-derived substances could serve as potential candidates for the development of safe and efficacious remedies for combating these diseases. Different scientific databases were used to source for literature on plants used against these infections. Thirty-five studies described the traditional use of 25 species from 20 families for treating DENV infection with Carica papaya and Euphorbia hirta were the most widely used across different regions. 13 in vivo studies, 32 in vitro studies, and eight clinical studies were conducted on 30 species from 25 families against different DENV serotypes, while plants from 13 families were reported to inhibit different forms of SARS-CoV, all of which were investigated through in vitro studies. Phytoconstituents belonging to various chemical classes were identified to show a wide range of antiviral activity against these infections. Extensive studies on the potentials of medicinal plants are needed to confirm their efficacy. This paper reveals the capabilities of medicinal plants and their phytochemicals in inhibiting DENV and SARS-CoV infections.In this paper, we report the preparation of bio-based polyurethane (PU) from renewable vegetable oil. The PU was synthesized through the reaction between jatropha oil-based polyol and isocyanate in a one-shot method. Then, lithium perchlorate (LiClO4) salt was added to the polyurethane system to form an electrolyte film via a solution casting technique. The solid polymer electrolyte was characterized through several techniques such as nuclear magnetic resonance (NMR), Fourier transforms infrared (FTIR), electrochemical studies, thermal studies by differential scanning calorimetry (DSC), and dynamic mechanical analysis (DMA). The NMR analysis confirmed that the polyurethane was successfully synthesized and the intermolecular reaction had occurred in the electrolytes system. The FTIR results show the shifting of the carbonyl group (C=O), ether and ester group (C-O-C), and amine functional groups (N-H) in PU-LiClO4 electrolytes compared to the blank polyurethane, which suggests that interaction occurred between the oxygen and nitrogen atom and the Li+ ion as they acted as electron donors in the electrolytes system. DSC analysis shows a decreasing trend in glass transition temperature, Tg and melting point, Tm of the polymer electrolyte as the salt content increases. Further, DMA analysis shows similar behavior in terms of Tg. The ionic conductivity increased with increasing salt content until the optimum value. The dielectric analysis reveals that the highest conducting electrolyte has the lowest relaxation time. The electrochemical behavior of the PU electrolytes is in line with the Tg result from the thermal analysis.Police officers in South Korea can be summoned to incidents involving individuals with mental health problems. Therefore, for officers to communicate effectively in such situations, education is necessary. Accordingly, this study obtained frontline police officers' perceptions of such educational programs and their suggestions regarding supplementary field manuals. Data were collected from 471 frontline police officers from 8 July until 9 August 2020. Data analysis incorporated frequency analysis, cross tabulation, text mining, and meaning network analysis. Participation in educational programs related to people with mental health problems depended on officers' field experience with such persons (χ2 = 7.432, p = 0.006). Among officers who received educational programs, most expressed satisfaction with the programs (χ2 = 72.243, p less then 0.001) and believed that these facilitated problem-solving (χ2 = 7.574, p = 0.023), improved understanding of people with mental health problems (χ2 = 10.220, p = 0.006), enabled better communication with such individuals (χ2 = 21.588, p less then 0.001), and improved confidence in clarity of verbal expression in conversations with them (χ2 = 6.634, p = 0.036). An on-site response manual for communicating with people with mental health problems would represent an effective educational intervention to improve police judgment and responses.

  • Parkinson's Disease (PD) is a common neurodegenerative disease which impacts millions of people around the world. In clinical treatments, freezing of gait (FoG) is used as the typical symptom to assess PD patients' condition. Currently, the assessment of FoG is usually performed through live observation or video analysis by doctors. Considering the aging societies, such a manual inspection based approach may cause serious burdens on the healthcare systems. In this study, we propose a pure video-based method to automatically detect the shuffling step, which is the most indistinguishable type of FoG. Firstly, the RGB silhouettes which only contain legs and feet are fed into the feature extraction module to obtain multi-level features. 3D convolutions are used to aggregate both temporal and spatial information. Then the multi-level features are aggregated by the feature fusion. Skip connections are implemented to reserve information of high resolution and period-wise horizontal pyramid pooling is utilized to fuse both global context and local features. To validate the efficacy of our method, a dataset containing 268 normal gait samples and 362 shuffling step samples is built, on which our method achieves an average detection accuracy of 90.8%. Besides shuffling step detection, we demonstrate that our method can also assess the severity of walking abnormity. Our proposal facilitates a more frequent assessment of FoG with less manpower and lower cost, leading to more accurate monitoring of the patients' condition.In recent studies, collaborative intelligence (CI) has emerged as a promising framework for deployment of Artificial Intelligence (AI)-based services on mobile/edge devices. In CI, the AI model (a deep neural network) is split between the edge and the cloud, and intermediate features are sent from the edge sub-model to the cloud sub-model. In this article, we study bit allocation for feature coding in multi-stream CI systems. We model task distortion as a function of rate using convex surfaces similar to those found in distortion-rate theory. Using such models, we are able to provide closed-form bit allocation solutions for single-task systems and scalarized multi-task systems. Moreover, we provide analytical characterization of the full Pareto set for 2-stream k -task systems, and bounds on the Pareto set for 3-stream 2-task systems. Analytical results are examined on a variety of DNN models from the literature to demonstrate wide applicability of the results.In this study, we propose a novel RGB-T tracking framework by jointly modeling both appearance and motion cues. First, to obtain a robust appearance model, we develop a novel late fusion method to infer the fusion weight maps of both RGB and thermal (T) modalities. The fusion weights are determined by using offline-trained global and local multimodal fusion networks, and then adopted to linearly combine the response maps of RGB and T modalities. Second, when the appearance cue is unreliable, we comprehensively take motion cues, i.e., target and camera motions, into account to make the tracker robust. We further propose a tracker switcher to switch the appearance and motion trackers flexibly. Numerous results on three recent RGB-T tracking datasets show that the proposed tracker performs significantly better than other state-of-the-art algorithms.We propose a neural network model to estimate the current frame from two reference frames, using affine transformation and adaptive spatially-varying filters. The estimated affine transformation allows for using shorter filters compared to existing approaches for deep frame prediction. The predicted frame is used as a reference for coding the current frame. Since the proposed model is available at both encoder and decoder, there is no need to code or transmit motion information for the predicted frame. By making use of dilated convolutions and reduced filter length, our model is significantly smaller, yet more accurate, than any of the neural networks in prior works on this topic. Although frequency-domain DAS can partially resolve this issue, such an algorithm is not suitable for imaging the evolution of bubble activity in real time and for cases in which cavitation events occur asynchronously. This study investigates a transient angular spectrum (AS) approach for PCM. The working principle of this approach is to backpropagate the received signal to the domain of interest and reconstruct the spatial-temporal wavefield encoded with the bubble location and collapse time. The transient AS approach is validated using an in silico model and water bath experiments. It is found that the transient AS approach yields similar results to DAS, but it is one order of magnitude faster. The results obtained by this study suggest that the transient AS approach is promising for fast and accurate PCM.Training convolutional neural networks (CNNs) for segmentation of pulmonary airway, artery, and vein is challenging due to sparse supervisory signals caused by the severe class imbalance between tubular targets and background. We present a CNNs-based method for accurate airway and artery-vein segmentation in non-contrast computed tomography. It enjoys superior sensitivity to tenuous peripheral bronchioles, arterioles, and venules. The method first uses a feature recalibration module to make the best use of features learned from the neural networks. Spatial information of features is properly integrated to retain relative priority of activated regions, which benefits the subsequent channel-wise recalibration. Then, attention distillation module is introduced to reinforce representation learning of tubular objects. Fine-grained details in high-resolution attention maps are passing down from one layer to its previous layer recursively to enrich context. Anatomy prior of lung context map and distance transform map is designed and incorporated for better artery-vein differentiation capacity.

We model task distortion as a function of rate using convex surfaces similar to those found in distortion-rate theory. Using such models, we are able to provide closed-form bit allocation solutions for single-task systems and scalarized multi-task systems. Moreover, we provide analytical characterization of the full Pareto set for 2-stream k -task systems, and bounds on the Pareto set for 3-stream 2-task systems. Analytical results are examined on a variety of DNN models from the literature to demonstrate wide applicability of the results.In this study, we propose a novel RGB-T tracking framework by jointly modeling both appearance and motion cues. First, to obtain a robust appearance model, we develop a novel late fusion method to infer the fusion weight maps of both RGB and thermal (T) modalities. The fusion weights are determined by using offline-trained global and local multimodal fusion networks, and then adopted to linearly combine the response maps of RGB and T modalities. Second, when the appearance cue is unreliable, we comprehensively take motion cues, i.e., target and camera motions, into account to make the tracker robust. We further propose a tracker switcher to switch the appearance and motion trackers flexibly. Numerous results on three recent RGB-T tracking datasets show that the proposed tracker performs significantly better than other state-of-the-art algorithms.We propose a neural network model to estimate the current frame from two reference frames, using affine transformation and adaptive spatially-varying filters. The estimated affine transformation allows for using shorter filters compared to existing approaches for deep frame prediction. The predicted frame is used as a reference for coding the current frame. Since the proposed model is available at both encoder and decoder, there is no need to code or transmit motion information for the predicted frame. By making use of dilated convolutions and reduced filter length, our model is significantly smaller, yet more accurate, than any of the neural networks in prior works on this topic. This study investigates a transient angular spectrum (AS) approach for PCM. The working principle of this approach is to backpropagate the received signal to the domain of interest and reconstruct the spatial-temporal wavefield encoded with the bubble location and collapse time. The transient AS approach is validated using an in silico model and water bath experiments. It is found that the transient AS approach yields similar results to DAS, but it is one order of magnitude faster. The results obtained by this study suggest that the transient AS approach is promising for fast and accurate PCM.Training convolutional neural networks (CNNs) for segmentation of pulmonary airway, artery, and vein is challenging due to sparse supervisory signals caused by the severe class imbalance between tubular targets and background. We present a CNNs-based method for accurate airway and artery-vein segmentation in non-contrast computed tomography. It enjoys superior sensitivity to tenuous peripheral bronchioles, arterioles, and venules. The method first uses a feature recalibration module to make the best use of features learned from the neural networks. Spatial information of features is properly integrated to retain relative priority of activated regions, which benefits the subsequent channel-wise recalibration. Then, attention distillation module is introduced to reinforce representation learning of tubular objects. Fine-grained details in high-resolution attention maps are passing down from one layer to its previous layer recursively to enrich context. Anatomy prior of lung context map and distance transform map is designed and incorporated for better artery-vein differentiation capacity.

A clinical study was conducted in 290 healthy infants aged from 6 to 19 weeks. Infants received either a control infant formula or one of the two investigational infant formulas (control formula with 0.8 g/100 ml scGOS/lcFOS and Bifidobacterium breve M-16V at either 1 × 104 cfu/ml or 1 × 106 cfu/ml). Exclusively breastfed infants were included as a reference. Analyses were performed on intention-to-treat groups and all-subjects-treated groups. After 6 weeks of intervention, the synbiotics at two different doses significantly increased the bifidobacteria proportions in healthy infants. The synbiotic supplementation also decreased the prevalence (infants with detectable levels) and the abundance of C. difficile. Closer to the levels in the breastfed reference group, fecal pH was significantly lower while L-lactate concentrations and acetate proportions were significantly higher in the synbiotic groups. All formulas were well tolerated and all groups showed a comparable safety profile based on the number and severity of adverse events and growth. Small RNA-seq identified 1739 conserved and 774 novel microRNAs (miRNAs). Transcriptome-seq characterised the expression of 66,559 genes while degradome-seq profiled the miRNA-guided mRNA cleavage dynamics. Differentially expressed miRNAs and genes were identified, with significant regulatory patterns subject to trans- and inter-generational stress. Integrated analysis using three omics platforms revealed significant biological interactions between stress-responsive miRNA and targets, with transgenerational stress tolerance potentially contributed via pathways such as hormone signalling and nutrient metabolism.

  • Midbrain dopamine neurons (DANs) regulate various brain functions such as motor control and motivation. Alteration of spiking activities of these neurons could contribute to severe brain disorders including Parkinson's disease and depression. Previous studies showed important roles of somatodendritic voltage-gated K+ channels (Kv) of DANs in governing neuronal excitability and dopamine release. However, it remains largely unclear about the biophysical properties and the function of Kv channels distributed at DAN axons. We performed whole-cell recordings from the axons of DANs in acute mouse midbrain and striatal slices. We detected both rapidly activating/inactivating Kv current (i.e. A-current) and rapidly activating but slowly inactivating current (i.e. D-current) in DAN axons. Pharmacological experiments with channel blockers revealed that these currents are predominantly mediated by Kv1.4 and Kv1.2 subunits, respectively. Blocking these currents could substantially prolong axonal action potentials (APs) via a reduction of their repolarization slope. Together, our results show that Kv channels mediating A- and D-currents shape AP waveforms in midbrain DAN axons, through this regulation they may control dopamine release at the axonal terminals. Therefore, these axonal Kv channels could be drug targets for brain disorders with abnormal dopamine release.Multi-contrast (**) Magnetic Resonance Imaging (MRI) of the same patient usually requires long scanning times, despite the images sharing redundant information. Through a convolutional network, the new method incorporates a learnable regularization unit that is capable of extracting, fusing, and mapping shareable information among different contrasts. Specifically, a dilated inception block is proposed to promote multi-scale feature extractions and increase the receptive field diversity for contextual information incorporation. Lastly, an optimal ** information feeding protocol is built through the design of a complementary feature extractor block. Comprehensive experiments demonstrated the superiority of the proposed network, both qualitatively and quantitatively.During meiosis, homologous chromosomes exchange genetic material. Any defects in its formation produce failures in meiotic recombination, chromosome segregation and meiosis completion. It has been proposed that DNA repair events that will be resolved by crossover between homologous chromosomes are predetermined by the SC. Hence, structural analysis of the organization of the DNA in the SC could shed light on the process of crossover interference.     This research aimed to provide a deeper insight into the gender-specific barriers to smoking cessation and gender-specific preferences for interventions in primary care, in order to contribute to better aligned cessation care for women.

    Qualitative study using focus groups.

    Regularly smoking female and male adults were recruited from four different general practices in The Hague (The Netherlands).

    A total of 11 women and nine men participated. Participants included were regular smokers with a minimum age of 18 and sufficient command of the Dutch language, who were willing to talk about smoking cessation. Inclusion ended when saturation was reached for both women and men. Participants were selected by means of purposeful sampling, whereby looking at age, educational level and experience with quitting.

    The main barriers to smoking cessation in women were psychological factors, such as emotion and stress, compared with environmental factors in men. Women indicated they were in need of support and positivity, and both women and men expressed the desire for assistance without judgement. Contrary to men, women were not drawn to restrictions and (dis)incentives.

    When counselling smokers, in women the focus should be on perceived internal problems, as opposed to more external obstacles in men. Contrary to men, female smokers seem to prefer non-coercive interventions, such as a group intervention offering support and positivity. Future research should focus on these gender differences, and how they could improve treatment in primary care.

Using the minimal MM risk group as the reference, female (relative risk ratio (RRR)=4.57; 95% CI (1.64 to 12.75); p =0.004) and older (RRR=1.08; 95% CI (1.04 to 1.12); p<0.001) participants were more likely to belong to the concordant MM group, while tobacco users (RRR=8.41; 95% CI (1.93 to 36.69); p=0.005) and older (RRR=1.09; 95% CI (1.03 to 1.15); p=0.002) participants had a high likelihood of belonging to the discordant MM group.

    The primary outcome was the proportion of patients who received a red cell transfusion postoperatively and within 7 days of index surgery. Secondary outcomes included proportion of patients receiving a non-red cell blood component transfusion and number of units transfused within 7 days and during index hospital stay, quality of life and cost-effectiveness analyses.
