Splenic artery aneurysm (SAA) is a relatively rare disease. Most patients with SAA have no symptoms, and detection is incidental detection. The incidence of rupture is not particularly high, but the mortality rate of ruptured SAAs is high. The main treatment for gastric cancer is gastrectomy with lymph node dissection, with dissection around the celiac artery suggested to be the most important. A 68-year-old woman with early gastric cancer in the lesser curvature of the lower gastric corpus was referred to our hospital. CT showed no remarkable findings except for a saccular SAA (diameter, 1.5 cm). We planned laparoscopic distal gastrectomy. However, because the SAA was close to the surgical field and its saccular shape created a rupture risk, we performed interventional radiology for SAA before surgery. One month later, laparoscopic distal gastrectomy with D1+ was performed successfully. The patient has remained disease-free in the 51 months since the operation.Esophageal leiomyomas are common benign tumors. Although surgical resection is warranted in symptomatic patients, the procedure used to enucleate a giant, circumferential tumor is complicated. A 38-year-old man was referred to our institution with a diagnosis of submucosal esophageal tumor. An endoscopic examination revealed a protruding submucosal mass in the lower third of the esophagus. Computed tomography scans demonstrated a circumferential mass measuring 90 × 40 mm. Examination of the biopsy specimens resulted in a diagnosis of leiomyoma of the esophagus, and thoracoscopic enucleation of the tumor via the right thorax with the patient in the prone position was planned. Histopathological and immunohistochemical staining of the surgical specimen confirmed the preoperative diagnosis of benign leiomyoma. The patient was discharged on postoperative day 7 without any complications.Small molecule probe development is pivotal in biomolecular science. Endocan is a proteoglycan that is regarded as a novel marker of endothelial dysfunction. Endothelial dysfunction in pulmonary vascular bed is known to play an important role for the pathogenesis of COPD.
This study aimed to determine serum endocan levels in patients with stable COPD and acute exacerbation of COPD (AECOPD) and to test the relationship between serum endocan levels and exacerbations.
This study enrolled a total of 55 COPD patients, 24 of which had AECOPD and 31 had stable COPD. All patients' basic demographic and clinical data were recorded and blood samples were collected.
Serum endocan levels were significantly higher in the AECOPD group compared to the stable COPD and control groups (for both p<0.001) and stable COPD group had higher levels than the control group (p<0.005). Additionally, serum endocan levels were negatively correlated with FVC, FEV1, partial oxygen pressure and oxygen saturation (r=-0.30, p=0.03; r=-0.34, p=0.01; r=-0.34, p=0.01 and r=-0.36, p=0.007 respectively), and positively correlated with disease duration and systolic pulmonary artery pressure (r=0.47, p<0.001; r=0.31, p=0.02 respectively). A cut-off value of 434.29pg/ml for endocan predicted exacerbation with a sensitivity of 79% and a specificity of 84% (AUC 0.778, 95% Cl 0.648-0.909; p<0.001). Logistic regression analysis revealed that increased endocan levels was independent predictor of COPD exacerbation (OR=9.32, 95%CI, 1.64-52.95; p=0.01).

Endocan may be a novel biomarker for detection of endothelial dysfunction and prediction of exacerbations in patients with COPD.Alternative splicing (AS) leads to transcriptome diversity in eukaryotic cells and is one of the key regulators driving cellular differentiation. Although AS is of crucial importance for normal hematopoiesis and hematopoietic malignancies, its role in early hematopoietic development is still largely unknown. Here, by using high-throughput transcriptomic analyses, we show that pervasive and dynamic AS takes place during hematopoietic development of human pluripotent stem cells (hPSCs). We identify a splicing factor switch that occurs during the differentiation of mesodermal cells to endothelial progenitor cells (EPCs). Perturbation of this switch selectively impairs the emergence of EPCs and hemogenic endothelial progenitor cells (HEPs). Mechanistically, an EPC-induced alternative spliced isoform of NUMB dictates EPC specification by controlling NOTCH signaling. Furthermore, we demonstrate that the splicing factor SRSF2 regulates splicing of the EPC-induced NUMB isoform, and the SRSF2-NUMB-NOTCH splicing axis regulates EPC generation. The identification of this splicing factor switch provides a new molecular mechanism to control cell fate and lineage specification.
Studies have demonstrated that antinuclear antibodies (ANAs) may be correlated with neuromyelitis optica spectrum disorder (NMOSD). However, the conflicting results of studies about the value of ANAs in AQP4 antibody-positive NMOSD patients need to be further investigated.
Case data were collected from 143 patients with AQP4 antibody-positive NMOSD. Patients were divided into two groups based on the ANA test results. The analysis of clinical characteristics, laboratory tests, and MRI examination results were compared between two groups the NMOSD patients with ANA (+) and with ANA (-).
Disease duration of NMOSD is shorter in the ANA (+) patients with EDSS<4 than in the ANA (-) patients (12.05±16.73 versus 29.43±41.03, p-value=.013). The median time from disease onset to an EDSS score of 4.0 is significantly longer in the ANA (-) NMOSD patients than in the ANA (+) patients (48.2months versus 24months, p=.04). In addition, ANA (RR, 2.234; 95% CI, 1.078-4.629; p-value=.031) can predict the severity of NMOSD.

Our findings suggest that, with a salient correction provided by a credible source, people are capable of updating their knowledge with new information that reverses what they previously thought.It is essential for healthcare institutions to accommodate the needs of the growing population of visually impaired patients. Hospital leaders must implement strategies to guarantee these patients' autonomy. We reviewed the literature and identified measures hospitals have implemented to accommodate visually impaired patients. To bridge gaps in the literature, we interviewed healthcare managers from five different hospitals in the United States. These interviews reveal that, while different levels of accommodations are already offered by those institutions, accommodations are generally extremely basic, and leave enormous room for improvement to ensure that the visually impaired receive proper care and dignified treatment.There has been **** talk about decolonizing global health lately. Using existing literature and personal reflections, we reflect on two widely known illustrations of global health interventions implemented in sub-Saharan Africa - distribution of contraceptives and dissemination of Ebola virus prevention and treatment devices - featuring different temporal backdrops. We show how these solutions have most often targeted the superficial dimensions of global health problems, sidestepping the structures and mental models that shape the actions and reactions of African populations. Lastly, we question the ways through which the decolonial approach might indeed offer a credible positioning for rethinking global health interventions.EsxA, secreted by the ESAT-6 secretion system 1 (ESX-1) secretion system, is considered the major Mycobacterium tuberculosis (Mtb) virulence determinant. Iatrogenic laryngotracheal stenosis (iLTS) is characterized by fibroinflammatory narrowing of the upper airway and is most commonly caused by intubation injury. Evidence suggests a key role for CD4 T cells in its pathogenesis. The objective of this study is to validate emerging multiplex immunofluorescence (mIF) technology for use in the larynx and trachea while quantitatively characterizing the immune cell infiltrate in iLTS. In addition to analyzing previously unstudied immune cell subsets, this study aims to validate previously observed elevations in the immune checkpoint PD-1 and its ligand PD-L1 while exploring their spatial and cellular distributions in the iLTS microenvironment.

Our findings suggest that, with a salient correction provided by a credible source, people are capable of updating their knowledge with new information that reverses what they previously thought.It is essential for healthcare institutions to accommodate the needs of the growing population of visually impaired patients. Hospital leaders must implement strategies to guarantee these patients' autonomy. We reviewed the literature and identified measures hospitals have implemented to accommodate visually impaired patients. To bridge gaps in the literature, we interviewed healthcare managers from five different hospitals in the United States. These interviews reveal that, while different levels of accommodations are already offered by those institutions, accommodations are generally extremely basic, and leave enormous room for improvement to ensure that the visually impaired receive proper care and dignified treatment.There has been much talk about decolonizing global health lately. Glucocorticoids (GC) remain integral to large vessel vasculitis (LVV) and ANCA-associated vasculitis (AAV) treatment. We aimed to assess real-world GC tapering trajectories among patients referred for LVV or AAV and identify factors associated with 'delayed' tapering.
Patients first assessed at a vasculitis clinic July 2017-August 2019 for LVV or AAV and taking GC were included. Delayed tapering was defined as prednisone >10 mg above target based on tapering recommendations (2010 British Society of Rheumatology Guidelines for Giant Cell Arteritis, 2015 CanVasc AAV Recommendations). We compared characteristics of patients with delayed and appropriate tapering and assessed barriers to timely tapering though chart reviews and referring physician surveys.
160 patients (65 LVV, 95 AAV) were taking GC at their first visit. Among the 42 (26%) patients with delayed tapering, mean daily prednisone dose was 39.2 mg (SD 14) compared to a target of 15.2 mg (SD 15). Pulse GC were administered to 19/42 (45%) patients with delayed tapering compared to 26/118 (22%) with appropriate tapering (p<0.05). Mean Birmingham Vasculitis Activity Score at treatment onset and GC duration were not significantly different between the two groups. Vision loss and/or stroke was more frequent in LVV referrals who experienced delayed (9/21, 43%) vs. appropriate (6/44, 14%) tapering (p<0.05). Managing risk of vasculitis flare was the most common challenge to tapering GC among surveyed referring physicians.

Rheumatoid arthritis (RA) and fibromyalgia syndrome (FM) are common diagnoses encountered in rheumatology practice, but do not enjoy the same status. We aimed to examine physician's illness perceptions regarding these two rheumatologic disorders and to evaluate how they correlate with their relationship with these patients.
Forty-five rheumatologists were enrolled in the study. Demographic data were registered. Measures collected included the Brief Illness Perception Questionnaire (BIPQ) and the Difficult Doctor- Patient Relation Questionnaire (DDPRQ-10). Both were recorded twice, related to FM and RA. Empathy and burnout were also assessed.
Of 45 physicians included in the study, only 53% were willing to accept FM patients. FM was considered a more severe disease than RA (FM-BIPQ mean score 54, SD 5.5 versus RA-BIPQ mean 45.6 SD 6.5, p<0.00) in terms of treatment control, understanding and emotional response generated by the disease. Doctor-patient relationship was perceived more difficult with FM patients compared to RA patients (FM-DDPRQ mean score 35.1, SD 9.2 versus RA-DDPRQ mean 19.6, SD 7.1, p<0.00), and was significantly correlated to the patient's concern about the illness (p<0.034) and patient's emotional response (p<0.036). Resistance to accept FM patients was largely influenced by difficult doctor-patient relationship. Higher levels of empathy were found in physicians experiencing less difficulty with FM patients.

Baricitinib, an oral Janus kinase (JAK) 1-2 inhibitor, is currently used along biologic DMARDs (bDMARDs) after the failure of methotrexate (MTX) in rheumatoid arthritis (RA). We investigated the efficacy and safety of baricitinib in real life.
We prospectively enrolled 446 RA patients treated with baricitinib from 11 Italian centres. Patients were evaluated at baseline and after 3, 6, and 12 months. They were arrayed based on previous treatments as bDMARD-naïve and bDMARD-insufficient responders (IR) after the failure or intolerance to bDMARDs. A sub-analysis differentiated the effects of methotrexate (MTX) and the use of oral glucocorticoids (OGC).
Our cohort included 150 (34%) bDMARD-naïve and 296 (66%) bDMARD-IR patients, with 217 (49%) using baricitinib as monotherapy. Considering DAS-28-CRP as the primary outcome, at 3 and 6 months, 114/314 (36%) and 149/289 (51.6%) patients achieved remission, while those in low disease activity (LDA) were 62/314 (20%) and 46/289 (15.9%), respectively; finally at 12 months 81/126 (64%) were in remission and 21/126 (17%) in LDA. At all-timepoints up to 12 months, bDMARDs-naïve patients demonstrated a better clinical response, independently of MTX. A significant reduction in the OGC dose was observed at 3 and 12 months in all groups. Myofascial decompression (MFD), or cupping, and self-myofascial release (SMR) are common techniques utilized to treat soft tissue injuries and increase flexibility. MFD is a negative pressure soft tissue treatment technique using suction to manipulate the skin and underlying soft tissues. One method of SMR is a foam roller, where a patient rolls his/her bodyweight over a dense foam cylinder in a self-massaging fashion to mobilize soft tissues for the body part treated.
The purpose of this investigation was to examine the acute effects on hamstring flexibility and patient-rated outcome measures comparing two soft tissue treatments, 1) MFD, and 2) a moist heat pack with SMR using a foam roller in patients with diagnosed hamstring pathology.

Seventeen collegiate athletes [13 males (20.6+/- years; 184.9+/-cm; 90.8+/-kg) and 4 females (20.5+/-years; 167.1+/-cm; 62.7+/-kg)] with diagnosed hamstring pathology (mild strain and/or symptoms of tightness, pain, decreased D group for the GROC values.
The findings suggest that both treatments are beneficial in increasing hamstring length. Patients though felt an enhanced treatment effect using MFD over SMR for perceived benefits to hamstring flexibility.

The Nordic hamstring exercise (NHE) is an effective strategy to prevent hamstring strain injuries in soccer players. The current literature recommends a 10-week training program with three sessions per week, but the short preseason period and the congested schedule make difficult for high-performance soccer teams to apply the NHE as recommended.
The purpose of this study was to examine the effect of a pragmatic NHE training program during a four-week preseason period on eccentric knee flexor strength of high-performance soccer players.
Quasi-experimental clinical trial.
This study included 25 under-20 male soccer players from a premier league club. They performed eight sessions of NHE (3 sets of 6-10 repetitions, twice a week) during the four-week preseason period. The eccentric knee flexor strength was evaluated during the NHE execution on a custom-made device, before and after the training program.
The NHE training program significantly increased the players' eccentric knee flexor strength in both right (Δ = 13%; p<0.001; effect size = 0.97) and left limbs (Δ = 13%; p<0.001; effect size = 0.92). Individual analysis identified 76% of the players as responders to the NHE training program (Δ = 16%; effect size = 1.60), and 24% as non-responders (Δ = 3%; effect size = 0.24).
A four-week training program with NHE performed twice a week is feasible in the real-world of high-performance soccer clubs and increases the eccentric knee flexor strength of male soccer players.

Myofascial decompression (MFD), or cupping, and self-myofascial release (SMR) are common techniques utilized to treat soft tissue injuries and increase flexibility. MFD is a negative pressure soft tissue treatment technique using suction to manipulate the skin and underlying soft tissues. One method of SMR is a foam roller, where a patient rolls his/her bodyweight over a dense foam cylinder in a self-massaging fashion to mobilize soft tissues for the body part treated. The purpose of this investigation was to examine the acute effects on hamstring flexibility and patient-rated outcome measures comparing two soft tissue treatments, 1) MFD, and 2) a moist heat pack with SMR using a foam roller in patients with diagnosed hamstring pathology. Pilot randomized controlled trial study. Seventeen collegiate athletes [13 males (20.6+/- years; 184.9+/-cm; 90.8+/-kg) and 4 females (20.5+/-years; 167.1+/-cm; 62.7+/-kg)] with diagnosed hamstring pathology (mild strain and/or symptoms of tightness, pain, decreased D group for the GROC values. The findings suggest that both treatments are beneficial in increasing hamstring length. The Nordic hamstring exercise (NHE) is an effective strategy to prevent hamstring strain injuries in soccer players. The current literature recommends a 10-week training program with three sessions per week, but the short preseason period and the congested schedule make difficult for high-performance soccer teams to apply the NHE as recommended. The purpose of this study was to examine the effect of a pragmatic NHE training program during a four-week preseason period on eccentric knee flexor strength of high-performance soccer players. Quasi-experimental clinical trial. This study included 25 under-20 male soccer players from a premier league club. They performed eight sessions of NHE (3 sets of 6-10 repetitions, twice a week) during the four-week preseason period. The eccentric knee flexor strength was evaluated during the NHE execution on a custom-made device, before and after the training program. The NHE training program significantly increased the players' eccentric knee flexor strength in both right (Δ = 13%; p<0.001; effect size = 0.97) and left limbs (Δ = 13%; p<0.001; effect size = 0.92). Individual analysis identified 76% of the players as responders to the NHE training program (Δ = 16%; effect size = 1.60), and 24% as non-responders (Δ = 3%; effect size = 0.24). After 3 years it increased to 5.1 (±1.75, range 3-8) and further to 7 (±1.8, range 4-9) at the last follow-up. The mean postoperative TESS was 80.8 (±6.4, range 75.7-91.4) at the latest follow-up. In this prospective study, agroup of 12 elementary school children aged between 7 and 10years without neurological or orthopedic problems participated. The measurements included aclinical examination, three-dimensional gait analysis with electromyographic recordings and measurement of postural sway on aforce plate.
The backpack load, on average 15% of the body weight, led to aslower walking speed, shorter step length and increased double-support phase. Increased anterior pelvic and trunk tilt, as well as hip flexion were also observed. Furthermore, the muscle activity and postural sway of the children were affected by the increased load.
School backpacks weighing 4 kg caused changes in gait, muscle activity, posture and stability in elementary school children. Due to the high altitude and cold climate in the region, the vegetation ecosystem has become very sensitive to environmental changes. In recent decades, due to the impact of climate change and human activities, the grassland degradation and desertification in this region have become very serious. In order to study the changes in aeolian desertification of grassland (ADG) in Sanjiangyuan, the Landsat images and GIS technology were used to monitor the dynamics of ADG from 1975 to 2015, and the driving factors behind this were analyzed. The results revealed that from 1975 to 2000, the area of ADG increased by 2855.8 km2, and the growth rate was 114.23 km2 a-1. In contrast, the ADG was restored from 2000 to 2015, with a decrease of 1286.54 km2 and a rate of 85.77 km2 a-1. The main reasons for the expansion of ADG in the early stage were the rising temperature, the fluctuation of precipitation and wind speed, and the increase in intensified human activities. In the marine realm, marine protected areas (MPAs) were designated to counteract regional pressures, but they might be ineffective to protect vulnerable species and habitats, if their distribution is affected by global climate change. We used six Species Distribution Models (GLM, MARS, FDA, RF, GBM, MAXENT) to project changes in the distribution of eight benthic indicator and key species under climate change in the North Sea MPAs for 2050 and 2099. The projected distribution area of most species will be stable or even increase within the MPAs between 2001 and 2050. Thereafter, the distribution area decreased, especially within MPAs in the central North Sea by 2099, and some key species even disappeared from the MPAs. Video games and surgical procedures share similar skills such as visuospatial abilities and hand-eye coordination; therefore, video games can be a valuable tool for surgical training amongst medical students. However, comprehensive, up-to-date systematic reviews are necessary to confirm.
A systematic literature search of PubMed, MEDLINE, and EMBASE was performed in April 2020 with no limits set on the date of publication. Observational and randomized controlled studies were included. Quality and bias were assessed using the Newcastle-Ottawa Scale for nonrandomized studies and the Grading of Recommendations Assessment, Development and Evaluation system for randomized studies.
A total of 575 participants from 16 studies were included. The most common surgical skills tested were laparoscopy (n= 283, from 8 studies) and robotic surical heterogeneity amongst included studies limit the ability to make conclusive decisions; thus, future studies with long-term follow-up, larger sample sizes, outcomes stratified by video-game characteristics, and up-to-date technology are necessary.
Knowledge about the impact of coronavirus disease 2019 (COVID-19) on kidney transplant recipients (KTRs) concerning viral shedding and humoral immune response against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is limited. The aim of this study is to analyze viral dynamics and the antibody response to SARS-CoV-2 in KTRs with COVID-19 and study their association with clinical data.
Consecutive KTRs diagnosed with COVID-19 at our center were evaluated for clinical presentation and outcome; duration of viral shedding and viral burden by reverse transcription-polymerase chain reaction assay cycle threshold; and magnitude of seroconversion to SARS-CoV-2.
Six KTRs identified with COVID-19 were hospitalized. Presenting symptoms were similar to those in the general population. Four patients had severe disease and, of these, 2 required mechanical ventilation, 4 had acute kidney injury, and 3 had secondary bacterial infections. Immunosuppression was reduced in all patients. Five patients were treated with hydroxychloroquine. No patient required dialysis or died. Patients with severe disease had a longer duration of viral shedding, which lasted more than 40 days, and had IgG antibodies against SARS-CoV-2, which were detected from 3 weeks to as long as 10 weeks after symptom onset. In patients with less severe disease no IgG antibodies where detected between 9 and 14 weeks after symptom onset.
In our series, KTRs with severe COVID-19 had prolonged viral shedding and a stronger humoral immune response to SARS-CoV-2. These preliminary data need to be confirmed with further studies and over a longer period of time.

Active antibody-mediated rejection (aABMR), particularly late aABMR, remains a major challenge for long-term renal allograft survival. This single-center retrospective study aimed to compare clinical features between early vs late aABMR and to identify risk factors for allograft failure among patients with aABMR.
Forty-one patients diagnosed with aABMR at our hospital were included and were divided into 2 groups early aABMR (≤6 months; n= 10) vs late aABMR (>6 months; n= 31) based on the time from transplant to diagnosis. We used six Species Distribution Models (GLM, MARS, FDA, RF, GBM, MAXENT) to project changes in the distribution of eight benthic indicator and key species under climate change in the North Sea MPAs for 2050 and 2099. The projected distribution area of most species will be stable or even increase within the MPAs between 2001 and 2050. Thereafter, the distribution area decreased, especially within MPAs in the central North Sea by 2099, and some key species even disappeared from the MPAs. Quality and bias were assessed using the Newcastle-Ottawa Scale for nonrandomized studies and the Grading of Recommendations Assessment, Development and Evaluation system for randomized studies. A total of 575 participants from 16 studies were included. The most common surgical skills tested were laparoscopy (n= 283, from 8 studies) and robotic surical heterogeneity amongst included studies limit the ability to make conclusive decisions; thus, future studies with long-term follow-up, larger sample sizes, outcomes stratified by video-game characteristics, and up-to-date technology are necessary. Knowledge about the impact of coronavirus disease 2019 (COVID-19) on kidney transplant recipients (KTRs) concerning viral shedding and humoral immune response against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is limited. The aim of this study is to analyze viral dynamics and the antibody response to SARS-CoV-2 in KTRs with COVID-19 and study their association with clinical data. Consecutive KTRs diagnosed with COVID-19 at our center were evaluated for clinical presentation and outcome; duration of viral shedding and viral burden by reverse transcription-polymerase chain reaction assay cycle threshold; and magnitude of seroconversion to SARS-CoV-2. Six KTRs identified with COVID-19 were hospitalized. Presenting symptoms were similar to those in the general population. Four patients had severe disease and, of these, 2 required mechanical ventilation, 4 had acute kidney injury, and 3 had secondary bacterial infections. Immunosuppression was reduced in all patients. Five patients were treated with hydroxychloroquine. No patient required dialysis or died. Patients with severe disease had a longer duration of viral shedding, which lasted more than 40 days, and had IgG antibodies against SARS-CoV-2, which were detected from 3 weeks to as long as 10 weeks after symptom onset. In patients with less severe disease no IgG antibodies where detected between 9 and 14 weeks after symptom onset. In our series, KTRs with severe COVID-19 had prolonged viral shedding and a stronger humoral immune response to SARS-CoV-2. Due to limited space and resources, it can be difficult to train students on audiological procedures adequately. In the present study, we compared audiology training outcomes between a traditional approach and a recently developed immersive virtual reality (VR) approach in audiology students. Twenty-nine first-year audiology students participated in the study; 14 received traditional training ("TT group"), and 15 received the VR training ("VRT group"). Pre- and post-training evaluation included a 20-item test developed by an audiology educator. Post-training satisfaction and self-confidence were evaluated using Likert scales. Mean post-training test scores improved by 6.9±9.8 percentage points in the TT group and by 21.1±7.8 points in the VRT group; the improvement in scores was significant for both groups. After completing the traditional training, the TT group was subsequently trained with the VR system, after which mean scores further improved by 7.5 points; there was no significant difference in post-VR training scores between the TT and VRT groups. After training, the TT and VRT groups completed satisfaction and self-confidence questionnaires. Satisfaction and self-confidence ratings were significantly higher for the VR training group, compared to the traditional training group. Satisfaction ratings were "good" (4 on Likert scale) for 74% of the TT group and 100% of the VRT group. Self-confidence ratings were "good" for 71% of the TT group and 92% of the VRT group. Here, we investigate the mechanisms underlying how vaginal bacteria alter PrEP drug levels and impact HIV infection rates ex vivo. Using cervicovaginal lavages (CVLs) from women with or without bacterial vaginosis (BV), we identified microbial metabolism of PrEP drugs in BV samples through LC-MS/MS analysis of soluble drug levels and metabolite formation in dual T-cell cultures. CVL samples were assessed for microbiome analysis using sequencing of bacterial 16S rRNA genes. We also observed non-Lactobacillus bacteria that are associated with BV may potentially impact PrEP efficacy through increased HIV infection rates in co-cultures containing Lactobacillus or BV bacteria, PrEP drugs, CEM-GFP cells, and HIV-1LAI virus. Finally, we used these data to develop a novel predictive mathematical simulation modeling system to predict these drug interactions for future trials. These studies demonstrate how dysbiotic vaginal microbiota may impact PrEP drugs and provides evidence linking vaginal bacteria to PrEP efficacy in women.
Quality-assured medicines are a principal means of achieving health-related Sustainable Development Goals. An example of quality assurance/quality control (QA/QC) procedures in drug procurement is provided by the operation of the Global Drug Facility (GDF) of the Stop TB Partnership, the largest provider of tuberculosis (TB) medicines to the public sector worldwide.
Procedures and results of GDF's quality assurance/quality control (QA/QC) over the five-year period 2013-2017 were analysed retrospectively. 13,999 batches of 51 different medicines had been procured and reviewed within this period. 1,388 of these batches had been analysed in the laboratories of GDF's external quality control agent (QCA). Assay and dissolution results determined by the manufacturers and by the external QCA were compared using Bland-Altman analysis.
All investigated batches of medicines were in specifications at the time of shipment. The costs for QA/QC were 0.8% of purchase costs. The median time required for chemical analysis was 10 working days.

COVID-19 is a very high transmission disease with a variable prognosis in the general population. Patients in hemodialysis therapy are particularly vulnerable to developing an infectious disease, but the incidence and prognosis of hemodialysis patients with COVID-19 is still unclear. The main objective is to describe the experience of our dialysis unit in preventing and controlling the spread of SARS-CoV-2 infection.
Preventive structural and organizational changes were applied to all patients and health care personnel in order to limit the risk of local transmission of SARS-CoV-2 infection.
The Nephrology department at Sant Joan Despí Moises Broggi Hospital-Consorci Sanitari Integral is a reference for two satellite hemodialysis centers caring for 156 patients. We combine our own hemodialysis maintenance program for 87 patients with hospitalized patients from peripheral hemodialysis centers. In this area, the reported incident rate of COVID-19 in these peripherical hemodialysis centers was 9.5% to 19.9% and the death rate 25% to 30.5%. In our hemodialysis program, the incidence rate was 5.7%. Three out of five required hospitalization (60%) and nobody died.
Although the risk of local transmission of the disease was very high due to the increase in hemodialysis patients from peripheral centers admitted to hospital, the incidence rate of COVID-19 was very low in our own hemodialysis patients. We believe that the structural and organizational changes adopted early on and the diagnosis algorithm played an important role in minimizing the spread of the disease.

We previously investigated Lon-binding proteins involved in fungal pathogenesis that interact with PrePL, the smallest Magnaporthe oryzae Lon-binding protein. Here, we show that Lon cleaves PrePL and produces Pc, an extracellular 11-kDa isoform with catalase and peroxidase activity. The ΔPrePL loss-of-function strain showed stronger sporulation and accelerated disease development, suggesting a temporally specific negative regulatory mechanism controlled by PrePL in disease progression. Neither the truncated Pc, nor the full-length PrePL missing the Lon cleavage site complemented the ΔPrePL phenotype, suggesting that full-length PrePL and Pc both function in fungal development. PrePL targeted to the mitochondria undergoes hydrolysis by Lon to produce Pc, which accumulates in the fungal apoplast. Importantly, recombinant Pc induced plant defence responses and cell death after being infiltrated into selected plant leaves, indicating that it functions as an avirulence factor. This work thus reveals a novel pathogenic factor in the fungal Lon-mediated pathway. Additionally, our results provide new insight into the functions of a full-length protein and its cleaved isoform in fungal pathogenesis.Latent infection with herpesviruses constitutively activates inflammasomes, while lytic replication suppresses their activation through distinct mechanisms. However, how Epstein-Barr virus (EBV) lytic replication inhibits the activation of inflammasomes remains unknown. Here, we reveal that the EBV immediate-early protein BRLF1 inhibits inflammasome activation, and BRLF1 deficiency significantly increases the activation of inflammasomes and pyroptosis during early lytic lifecycle. BRLF1 interacts with RNA polymerase III subunits to suppress immunostimulatory small RNA transcription, RIG-I inflammasome activation, and antiviral responses. Consequently, BRLF1-deficient EBV primary infection induces robust T-cell and NK cell activation and killing through IL-1β and IL-18. A BRLF1-derived peptide that inhibits inflammasome activation is sufficient to suppress T-cell and NK cell responses during BRLF1-deficient EBV primary infection in lymphocytes. Here, we analyse the endophytic and total bacterial communities of leaves, rhizomes, and roots of six Red Sea seagrass species and their sediments. The structure of seagrass bacterial communities revealed that the 1% most abundant OTUs accounted for 87.9% and 74.8% of the total numbers of reads in sediment and plant tissue samples, respectively. We found taxonomically distinct bacterial communities in vegetated and bare sediments. Yet, our results suggest that lifestyle (i.e. free-living or host-association) is the main driver of bacterial community composition. Seagrass bacterial communities were tissue- and species-specific and differed from those of surrounding sediments. We identified OTUs belonging to genera related to N and S cycles in roots, and members of Actinobacteria, Bacteroidetes, and Firmicutes phyla as particularly enriched in root endosphere.

The Nephrology department at Sant Joan Despí Moises Broggi Hospital-Consorci Sanitari Integral is a reference for two satellite hemodialysis centers caring for 156 patients. We combine our own hemodialysis maintenance program for 87 patients with hospitalized patients from peripheral hemodialysis centers. In this area, the reported incident rate of COVID-19 in these peripherical hemodialysis centers was 9.5% to 19.9% and the death rate 25% to 30.5%. In our hemodialysis program, the incidence rate was 5.7%. Three out of five required hospitalization (60%) and nobody died. Although the risk of local transmission of the disease was very high due to the increase in hemodialysis patients from peripheral centers admitted to hospital, the incidence rate of COVID-19 was very low in our own hemodialysis patients. We believe that the structural and organizational changes adopted early on and the diagnosis algorithm played an important role in minimizing the spread of the disease. We previously investigated Lon-binding proteins involved in fungal pathogenesis that interact with PrePL, the smallest Magnaporthe oryzae Lon-binding protein. Here, we show that Lon cleaves PrePL and produces Pc, an extracellular 11-kDa isoform with catalase and peroxidase activity. The ΔPrePL loss-of-function strain showed stronger sporulation and accelerated disease development, suggesting a temporally specific negative regulatory mechanism controlled by PrePL in disease progression. Neither the truncated Pc, nor the full-length PrePL missing the Lon cleavage site complemented the ΔPrePL phenotype, suggesting that full-length PrePL and Pc both function in fungal development. PrePL targeted to the mitochondria undergoes hydrolysis by Lon to produce Pc, which accumulates in the fungal apoplast. Importantly, recombinant Pc induced plant defence responses and cell death after being infiltrated into selected plant leaves, indicating that it functions as an avirulence factor. This work thus reveals a novel pathogenic factor in the fungal Lon-mediated pathway. However, how Epstein-Barr virus (EBV) lytic replication inhibits the activation of inflammasomes remains unknown. Here, we reveal that the EBV immediate-early protein BRLF1 inhibits inflammasome activation, and BRLF1 deficiency significantly increases the activation of inflammasomes and pyroptosis during early lytic lifecycle. BRLF1 interacts with RNA polymerase III subunits to suppress immunostimulatory small RNA transcription, RIG-I inflammasome activation, and antiviral responses. Consequently, BRLF1-deficient EBV primary infection induces robust T-cell and NK cell activation and killing through IL-1β and IL-18. A BRLF1-derived peptide that inhibits inflammasome activation is sufficient to suppress T-cell and NK cell responses during BRLF1-deficient EBV primary infection in lymphocytes. These results reveal a novel mechanism involved in the evasion of inflammasome activation and antiviral responses during EBV early lytic infection and provide a promising approach for the manipulation of inflammasomes against infection of oncogenic herpesviruses.Polypeptoids have been prepared and researched for more than 20 years. However, our understanding of the seagrass microbiome lacks behind that of other organisms. Here, we analyse the endophytic and total bacterial communities of leaves, rhizomes, and roots of six Red Sea seagrass species and their sediments. The structure of seagrass bacterial communities revealed that the 1% most abundant OTUs accounted for 87.9% and 74.8% of the total numbers of reads in sediment and plant tissue samples, respectively. We found taxonomically distinct bacterial communities in vegetated and bare sediments. Yet, our results suggest that lifestyle (i.e. free-living or host-association) is the main driver of bacterial community composition. Seagrass bacterial communities were tissue- and species-specific and differed from those of surrounding sediments.
