The aim of the study was to evaluate analysis criteria for the identification of the presence of rectal gas during volumetric modulated arc therapy (VMAT) for prostate cancer patients by using electronic portal imaging device (EPID)-based

The pass rate of 2% dose difference (DD2%) in small and large air cavities was 98.74% and 79.05%, respectively, in the appearance of the air cavity after irradiation three quarter times. In the clinical study, some fractions caused a sharp decline in the DD2% pass rate. The proportion for DD2% < 90% was 13.4% of all fractions. Rectal gas was confirmed in 11.0% of fractions by acquiring kilo-voltage X-ray images after the treatment.

After a median follow-up of 38.48 months, CTRT resulted in a significant improvement of the 5-year PFS (60.9%

The objective of this study was To determine the dose volume parameters predicting acute haematological toxicity in carcinoma cervix patients undergoing concurrent chemoradiotherapy.

Patients diagnosed to have Carcinoma Cervix and planned for concurrent chemoradiation by volumetric modulated arc therapy (VMAT) were included for analysis. Patients were assessed at baseline and every week during treatment for acute haematological toxicities. Dose volume parameters from treatment plans were correlated with RTOG grade of haematological toxicities.
A total of 34 patients diagnosed to have squamous cell carcinoma of cervix were treated by radical radiotherapy by VMAT technique and concurrent chemotherapy. The most common stage of presentation was stage IIB (61.7%). 29 patients (85.2%) completed five cycles of weekly cisplatin. Statistical analysis for sensitivity and specificity of dosimetric parameters was antly correlated with acute haematological toxicity.

Organ dose measurements were done with thermoluminescent dosimeters in Alderson RA NDO male phantom for head & neck (H&N), chest and pelvis protocols of the Elekta Synergy XVI kV CBCT system. From the measured organ dose, effective dose to patients were calculated according to the International Commission on Radiological Protection 103 report recommendations.
For H&N, chest and pelvis scans, the organ doses were in the range of 0.03-3.43 mGy, 6.04-22.94 mGy and 2.5-25.28 mGy, respectively. The calculated effective doses were 0.25 mSv, 5.56 mSv and 4.72 mSv, respectively.
The obtained results were consistent with the most published studies in the literature. Although the doses to patient organs from the kV CBCT system were relatively low when compared with the prescribed treatment dose, the amount of delivered dose should be monitored and recorded carefully in order to avoid secondary cancer risk, especially in pediatric examinations.

Xanthogranuloma of the sellar region is a rare benign lesion, and there are few cases reported in children. Its histogenesis is controversial, and it is difficult to strictly differentiate it from craniopharyngioma (CP), Rathke's cleft cyst, or pituitary adenoma.
A 16-year-old boy presented with a rare xanthogranuloma of the sellar region after complaining of retardation of growth 5 years previously. The ophthalmologic evaluation revealed no visual field disturbance. Endocrinological examination revealed hypopituitarism. Magnetic resonance imaging showed an intrasellar mass extending into the suprasellar region and compressing the optic chiasma, which appeared mixed signals on T1-weighted images. Endonasal transsphenoidal resection of the tumor was performed. https://www.selleckchem.com/products/LDE225(NVP-LDE225).html Histological analysis of the tumor sections demonstrated granulomatous tissue with cholesterol clefts, hemosiderin deposits, fibrous tissues, multinucleated giant cells, and lymphocyte. Our case demonstrates the natural progression of the disease, suggesting that xanthogranuloma of the sellar region without epithelial components may be an independent disease.
Food allergy (FA) in children impacts their own and their family quality of life (QoL). The association of specific FA factors with the various domains of health-related QoL (HRQL) in children is unclear.
The aim of this study was to evaluate FA characteristics in primary school children as determinants of components of HRQL.
Children with FA were recruited from the allergy clinic of a tertiary children's hospital. Demographic and clinical data were retrieved from their records, and 3 HRQL questionnaires were administered the FA QoL Questionnaire-Child Form (FAQLQ-CF), the FA independent measure (FAIM), and the Pediatric QoL Questionnaire (PedsQL™). Stepwise multiple linear regression analysis was carried out to investigate the correlation between FA characteristics and the scores on the HRQL scales. Bonferroni correction for multiple comparisons was set at p < 0.0002.
Of 172 primary schoolchildren with FA invited to take part, 110 participated (response rate 64%), of whom 83 (75.5%) were male, aged 7.5-12.3 years (mean 10.0 ± 1.4) years. From 38 demographic and clinical characteristics, 10 were excluded on initial data analysis and 28 proceeded to bivariate analysis with the scores on FAQLQ-CF, FAIM, PedsQL™, and their subscales. Most of the 28 showed no correlation with the scores; only 4 were entered into multivariate analysis with FAQLQ-CF and PedsQL™ scores, none of which, finally showed significant association.

During the past two decades, the attention of public health has been drawn to premarital genetic screening (PGS) programs to reduce birth defects and avoid genetic disorders. In Jordan, the high rate of genetic hemoglobinopathies compelled the government to implement an obligatory PGS program before marriage. Therefore, the objective of this study was to investigate the knowledge, opinion, and practice of young Jordanians concerning PGS.
Using a pretested questionnaire, this cross-sectional study was conducted on a convenience sample from Jordan. The measures included respondents' demographics, and beliefs/opinions regarding PGS.
A total of 432 participants completed the survey. The majority (87.8%) had a positive attitude toward PGS program. Reasons behind this positive attitude were preventing transmission of genetic diseases, reducing family breakdown/psychosocial problems, and financial burdens of having a child with genetic disease. In fact, 49.8% of participants were willing to change their marriage decision in case of receiving incompatible results. Moreover, most of the participants (75.1%) demanded the implementation of a law that prohibits incompatible marriages. A positive attitude toward PGS was found to be associated with female gender and having a university education.
Young Jordanians have a positive attitude toward the implementation of PGS. Yet, educational programs should be drawn up to the target population before getting married emphasizing the important role of PGS in the wellness of the community.

Incomplete abortion is a common complication of pregnancy. Sesamum indicum L. is a widely used emmenagogue herb.
We designed a clinical trial to evaluate the efficacy of sesame for the removal of retained products of conception (RPOC).
In this randomized, open-label, and controlled trial, 45 patients received sesame powder as an intervention group and 45 patients received expectant management as a control group for 5 days. The primary outcome measure was complete resolution of RPOC assessed by sonography. Secondary outcome measures were severity of patients' vaginal bleeding and pain.
84.1% had complete resolution of RPOC in the sesame group, while 26.2% had complete resolution of RPOC in the control group, which was statistically significant (p < 0.001). Moreover, patients in the sesame group showed a significantly more decreasing trend in pain and vaginal bleeding compared to the control group (p < 0.001).
Sesame had a significant effect on the removal of RPOC and the reduction of pain and vaginal bleeding.

Three months later, a follow-up study revealed left coronary ostial pseudoaneurysm at the anastomotic site together with strong 18F-FDG uptake, leading to commencement of steroid therapy. Eight months later, disappearance of left coronary ostial pseudoaneurysm was found by a follow-up CT scan. Conclusion This is a rare TA case in whom spontaneous resolution of coronary anastomotic aneurysm by steroid therapy was found without reconstructive surgery.Aims The aim of the study was to determine the associations of weight loss or gain with all-cause mortality risk in heart failure with preserved ejection fraction (HFpEF). Methods and Results Non-lean patients from the Americas from the Treatment of Preserved Cardiac Function Heart Failure with an Aldosterone Antagonist study were analyzed (n = 1,515). Weight loss and weight gain were defined as a decrease or increase in weight ≥5% between baseline and 1 year. To determine the associations of weight change and mortality risk, we used adjusted Cox proportional hazards models and restricted cubic spline models. The mean age was 71.5 (9.6) years. Weight loss and gain were witnessed in 19.3 and 15.9% patients, respectively. After multivariable adjustment, weight loss was associated with higher risk of mortality (HR 1.42, 95% CI 1.06-1.89, P = 0.002); weight gain had similar risk of mortality (HR 0.98, 95% CI 0.68-1.42, P = 0.932) compared with weight stability. There was linear relationship between weight change and mortality risk. The association of weight loss and mortality was different for patients with and without diabetes mellitus (interaction p = 0.009). Conclusion Among patients with HFpEF, weight loss was independently associated with higher risk of all-cause mortality, and weight gain was not associated with better survival. However, their joint effects on mortality are not clear. Design and Methods Using administration data from the geriatric department in a tertiary hospital, retrospective longitudinal analyses of patients aged ≥65 years with CKD were consecutively enrolled from February 2010 to November 2015. Baseline CKD stages, HF with reduced and preserved ejection fraction (HFrEF and HFpEF), Rockwood frailty index, handgrip strength (HGS), 6-m walking speed, and timed up-and-go test were used to predict the prevalence of frailty, physical disability, and all-cause mortality. Results Among 331 old patients with CKD, their mean age was 81.3 ± 6.6 years. CKD stages showed the following distributions stage 3, 74.9%; stage 4, 15.7%; stage 5, 9.4%. The prevalence of HF was 23.3%, and Rockwood frailty was 74.3%. Rockwood frailty and HF were both significantly associated with CKD stages. After a mean follow-up period of 3.1 ± 2.1 years, 44 patients died, and a crude analysis showed that stage 4, stage 5 CKD, low HGS, and Rockwood frailty index were associated with mortality. Pathological examination, biochemical analyses, and gene profile analysis revealed evidence of inflammation, fibrosis, cell death, and myofilament dephosphorylation in the myocardium of HFpEF model animals, together with gene expression shifts promoting cardiac remodeling and downregulating metabolic pathways. Furthermore, we noted the activation of several signaling proteins that impact cardiac fibrosis and remodeling, including transforming growth factor-β/SMAD family members 2/3, type I/III/V collagens, phosphatidylinositol 3-kinase, extracellular signal-regulated kinase, matrix metalloproteinases 2 and 9, tissue inhibitor of metalloproteinases 1 and 2, interleukins 6 and 1β, and inhibitor of κBα/nuclear factor-κB. Three months later, a follow-up study revealed left coronary ostial pseudoaneurysm at the anastomotic site together with strong 18F-FDG uptake, leading to commencement of steroid therapy. Eight months later, disappearance of left coronary ostial pseudoaneurysm was found by a follow-up CT scan. Conclusion This is a rare TA case in whom spontaneous resolution of coronary anastomotic aneurysm by steroid therapy was found without reconstructive surgery.Aims The aim of the study was to determine the associations of weight loss or gain with all-cause mortality risk in heart failure with preserved ejection fraction (HFpEF). Methods and Results Non-lean patients from the Americas from the Treatment of Preserved Cardiac Function Heart Failure with an Aldosterone Antagonist study were analyzed (n = 1,515). Weight loss and weight gain were defined as a decrease or increase in weight ≥5% between baseline and 1 year. To determine the associations of weight change and mortality risk, we used adjusted Cox proportional hazards models and restricted cubic spline models. The mean age was 71.5 (9.6) years. Weight loss and gain were witnessed in 19.3 and 15.9% patients, respectively. After multivariable adjustment, weight loss was associated with higher risk of mortality (HR 1.42, 95% CI 1.06-1.89, P = 0.002); weight gain had similar risk of mortality (HR 0.98, 95% CI 0.68-1.42, P = 0.932) compared with weight stability. There was linear relationship between weight change and mortality risk. The association of weight loss and mortality was different for patients with and without diabetes mellitus (interaction p = 0.009). Conclusion Among patients with HFpEF, weight loss was independently associated with higher risk of all-cause mortality, and weight gain was not associated with better survival. However, their joint effects on mortality are not clear. Design and Methods Using administration data from the geriatric department in a tertiary hospital, retrospective longitudinal analyses of patients aged ≥65 years with CKD were consecutively enrolled from February 2010 to November 2015. Baseline CKD stages, HF with reduced and preserved ejection fraction (HFrEF and HFpEF), Rockwood frailty index, handgrip strength (HGS), 6-m walking speed, and timed up-and-go test were used to predict the prevalence of frailty, physical disability, and all-cause mortality. Results Among 331 old patients with CKD, their mean age was 81.3 ± 6.6 years. CKD stages showed the following distributions stage 3, 74.9%; stage 4, 15.7%; stage 5, 9.4%. The prevalence of HF was 23.3%, and Rockwood frailty was 74.3%. Rockwood frailty and HF were both significantly associated with CKD stages. After a mean follow-up period of 3.1 ± 2.1 years, 44 patients died, and a crude analysis showed that stage 4, stage 5 CKD, low HGS, and Rockwood frailty index were associated with mortality. Given rising populist nationalism and multiplying meanings of "right" and "left," this paper assesses whether Europeans who identify as extremely left-wing on the political spectrum hold anti-immigrant attitudes. In contrast to right-wing xenophobes, we further examine whether the political left, who conventionally emphasize class conflict, oppose immigrants less for cultural reasons and more for materialist reasons. We also consider whether socioeconomic status and values traditionally associated with the political left-favoring redistributive policies, egalitarianism, or social rights to benefits and services for immigrants-temper left- more than right-wing xenophobia. We find that a surprisingly large share of those who identify as far left do express extremely xenophobic attitudes, and we profile them in contrast to far right xenophobes. With logistic regression analysis of nine waves of the European Social Survey (2002-2018), we find that, all things equal, socioeconomic status influences far left xenophobia more than far right xenophobia, but inegalitarian values, less support for redistributive policies, and welfare chauvinism can only partially account for far left xenophobia and unexpectedly do not distinguish it from far right xenophobia. This implies that far left parties might adopt anti-immigrant policies to try to retain their loyal voters, even though such policies do not comport with broader left-wing values and may increase racial and ethnic inequality. Controlling for demographic and attitudinal differences reduces the probability of xenophobia among the far left by about sixty percent, but there remains some residual anti-immigrant attitudes among this group still to be explained.This article provides an overview of the UK government policy in relation to relationships and sex education in schools. It focuses on the latest statutory guidance which requires primary and secondary schools in England to teach pupils about different types of relationships, including same-sex relationships. We outline the current policy frameworks and present a rationale for why Lesbian, Gay, Bisexual, Trans and Queer (LGBTQ+) identities and relationships should be present in the curriculum. We critically interrogate the government response and we present a framework to support the implementation of a whole school approach to LGBTQ+ inclusion. We draw on Meyer's model of minority stress to explore risks to children and young people if they are not provided with an LGBTQ+ curriculum.It is hypothesized that in the "more highly evolved" mammals, including the domesticated mammals, that the brainstem and the cerebellum receive arterial blood through the vertebrobasilar system whilst the internal carotid arteries primarily supply the forebrain. In camels, the arterial blood supply to the brain differs from that of ruminants since the internal carotid artery and the rostral epidural rete mirabile (RERM) are both present and the basilar artery contributes a significant proportion of cerebral afferent blood. In this study, we described the anatomical distribution of the vertebrobasilar system arterial supply in the dromedary. Secondly, we determined the direction of blood flow within the vertebral and basilar arteries using transcranial color doppler ultrasonography. Thirdly, we quantified the percentage arterial contributions of the carotid and vertebrobasilar systems to the dromedary brain. Fifty-five heads of freshly slaughtered male Omani dromedaries aged 2-6 years were dissected to determibral and basilar arteries was toward the brain in animals examined in ventral recumbency and when standing. The vertebrobasilar system contributes 34% of the blood supply to the brain. The vertebrobasilar system is the exclusive supply to the medulla oblongata, pons and cerebellum.While the gross skull and dental morphology, masticatory biomechanics, dental eruption patterns, and radiographic dental anatomy has been described in the Tasmanian devil (Sarcophilus harrisii), to date no studies have comprehensively examined the prevalence and appearance of pathologic processes affecting their skulls and dentition. As such, the aim of this study was to describe macroscopic and radiographic anatomy and identify the prevalence of anatomic variations and pathological processes in Tasmanian devil dentition and skulls. To do so, anatomical and pathological findings were documented in Tasmanian devil skulls using photography and dental radiography. Assessment of skull trauma, anatomical and developmental abnormalities, periodontitis, endodontic disease, and tooth resorption was performed. A total of 28 Tasmanian devil skulls containing 1,028 teeth were examined. Evidence of postmortem trauma was common. The most common positional abnormality was palatal or buccal rotation of the premolar teeth. They were grouped according to age, category of dog and time since last antiparasitic treatment. Samples were analyzed by flotation to detect parasitic eggs and cysts/oocysts. Among these, 129 (43%) dogs were also analyzed with the Baermann-technique to detect cardiopulmonary larval stages. Parasite dispersal stages were found in 24 (7.9%, CI 95% 4.9-10.1) of the dogs at flotation, while no dog shed cardiopulmonary larval stages. Giardia sp. cysts were observed in 2.6% (n = 8) of dogs examined, cysts of Sarcocystis spp. were observed in 0.6% (n = 2), oocysts of Cystosisopora ohioensis were found in one dog (0.3%). Eggs of Toxocara canis (2.3%, n = 7), Uncinaria stenocephala (1.3%, n = 4) and Trichuris vulpis (0.3%, one dog) were found. None of the dogs were diagnosed with more than one species.

Prevention of extrauterine growth restriction (EUGR) in preterm neonates is one of the biggest challenges to neonatologists. Lack of uniformity and inconsistency in nutrition practices are the most common causes. We started a quality improvement (QI) initiative with an intention to decrease the proportion of EUGR.
We performed prospective nonrandomized QI from May 2018 to July 2019. Ninety-six neonates (born at <32 weeks and/or <1.2 kg) were compared with 111 historical controls. A continuous feedback loop was maintained, and changes were analyzed as plan-do-study-actcycle.
Although EUGR was lower after the intervention (93.7% vs 87.5%), this change was not statistically significant. However, other measures of in-hospital neonatal growth showed improvement, including median discharge weight percentile from 1% to 3% (P = .003). Median difference in the z-score for weight from birth to discharge also improved significantly from -1.84 to -1.65 (P = .04). Babies in the intervention group regained birtated improved outcomes.
To evaluate the safety and feasibility of left bundle branch area pacing (LBBAP) in patients with valvular interventions.
Eighty-four patients were included in this study. All patients underwent recent surgical or percutaneous valvular interventions. LBBAP was attempted in all patients. Implant success rates, peri- and postprocedure electrocardiogram, pacing parameters, and complications were assessed at implant, and during follow-up.
LBBAP implantation was successful in 80/84 (95%) patients. Mean age was 74.1 ± 13.8 years and 56% patients were male. Prior valvular replacements included percutaneous aortic (26), surgical aortic (36), combined surgical aortic plus mitral (6), MVR (10), tricuspid (1), and pulmonic (1). Average LVEF was 52.6 ± 11%. Majority of patients underwent LBBAP due to atrioventricular block (76%) and sinus node disease (13%). Total procedure duration was 74.1 ± 12.5 min and fluoroscopic duration was 9.7 ± 6.8 min. Pacing parameters were stable during follow-up period of 10.0 ± 6.3 months. Pacing QRS duration was significantly narrower than baseline QRS duration (131.5 ± 31.4 ms vs. 114.3 ± 13.7 ms, p < .001, respectively). No acute complications were observed. Mean follow-up was 10.0 ± 6.3 months (median 8.4 months, min 1 and max 24 months). During follow-up, there were three device infections and two patients had loss of LBBA capture within 1 month of implant.

LBBAP is a feasible and safe pacing modality in patients with prior interventions for valvular heart disease.Coronavirus disease 2019 (COVID-19), after initially being detected in Wuhan, China in late 2019, quickly spread to most regions of the world, meeting World Health Organization criteria for a pandemic. Social distancing along with other measures implemented to control spread had a drastic impact on the provision of healthcare including deferred elective procedures and surgeries as well as delayed care and evaluation for emergent diagnoses such as heart attacks and strokes. In the home nutrition practice, patients began to delay routine laboratory tests and were canceling or deferring annual visits. Our group soon began to rapidly deploy telehealth to meet the needs of the home nutrition patients. Telehealth is not a new concept with descriptions of healthcare being provided at a distance with the use of telephone in the 1870s. https://www.selleckchem.com/products/wortmannin.html However, widespread adoption has been limited because of regulation (licensure, prescriptions, credentialing, and privileges), lack of reimbursement, as well as adoption and availability of technology needed to carry out telehealth visits. As regulations and limits on reimbursements were waived during COVID-19 pandemic, our home nutrition practice began to evaluate core components of the care we provide for our patients and assess which could be successfully transitioned to telehealth. In addition to the history and regulation of telehealth, the current manuscript provides details regarding successful implementation of telehealth visits such as change management, selection of telehealth platform, scheduling and logistics, as well as carrying out the virtual visit including history and physical exam.
Melanoma is one of the most common human malignancies; yet, it is often thought of as a disease of adulthood rather than one affecting adolescents and young adults. This study sought to understand the experiences of adolescents, young adults and their family living with malignant melanoma.
A qualitative study using Interpretive Phenomenological Analysis, through a multi-perspective design.
Data collection was conducted between January and August 2018 in each participant's Primary Care Centre when they were attending for an outpatient appointment. Each young person and a nominated family member were interviewed (n=10) either individually (n=4) or as a dyad (n=6) according to their personal choice. In-depth semi-structured interviews were conducted and audio-recorded with the participant's consent. Interview data were transcribed verbatim and analysed.
The metanarrative 'Life Interrupted' was the core conceptual thread woven throughout the findings. It represents the interconnections and interrelationshs patient group is provided with the same access to service delivery as other adolescents and young adults with cancer.

Serosal adhesions to the mesentery without clinical evidence of obstruction were observed in two horses with two-layer closure.
Prevention of extrauterine growth restriction (EUGR) in preterm neonates is one of the biggest challenges to neonatologists. Lack of uniformity and inconsistency in nutrition practices are the most common causes. We started a quality improvement (QI) initiative with an intention to decrease the proportion of EUGR. We performed prospective nonrandomized QI from May 2018 to July 2019. Ninety-six neonates (born at <32 weeks and/or <1.2 kg) were compared with 111 historical controls. A continuous feedback loop was maintained, and changes were analyzed as plan-do-study-actcycle. Although EUGR was lower after the intervention (93.7% vs 87.5%), this change was not statistically significant. However, other measures of in-hospital neonatal growth showed improvement, including median discharge weight percentile from 1% to 3% (P = .003). Median difference in the z-score for weight from birth to discharge also improved significantly from -1.84 to -1.65 (P = .04). Babies in the intervention group regained birtated improved outcomes. To evaluate the safety and feasibility of left bundle branch area pacing (LBBAP) in patients with valvular interventions. Eighty-four patients were included in this study. All patients underwent recent surgical or percutaneous valvular interventions. LBBAP was attempted in all patients. Implant success rates, peri- and postprocedure electrocardiogram, pacing parameters, and complications were assessed at implant, and during follow-up. LBBAP implantation was successful in 80/84 (95%) patients. Mean age was 74.1 ± 13.8 years and 56% patients were male. Prior valvular replacements included percutaneous aortic (26), surgical aortic (36), combined surgical aortic plus mitral (6), MVR (10), tricuspid (1), and pulmonic (1). Average LVEF was 52.6 ± 11%. Majority of patients underwent LBBAP due to atrioventricular block (76%) and sinus node disease (13%). Total procedure duration was 74.1 ± 12.5 min and fluoroscopic duration was 9.7 ± 6.8 min. Pacing parameters were stable during follow-up period of 10.0 ± 6.3 months. Pacing QRS duration was significantly narrower than baseline QRS duration (131.5 ± 31.4 ms vs. 114.3 ± 13.7 ms, p < .001, respectively). No acute complications were observed. Mean follow-up was 10.0 ± 6.3 months (median 8.4 months, min 1 and max 24 months). In this perspective, both wastewater and residues from process can be valorised for recovering nutrients (N and P), producing value added products (i.e. biopolymers), energy vectors and biofuels (i.e. bio-H2, bio-CH4 and bioethanol). As an additional benefit, changing the approach for WWTPs design and operation will decrease the overall amount of landfilled residues. Clinical teachers perform overlapping tasks in education and patient care. They are therefore expected to juggle many professional identities such as educator and clinician. Yet little is known about how clinical teachers negotiate their professional identities. The present research examined the lived experiences of clinical teachers as they manage and make sense of their professional identities in the context of a faculty development program.
This study adopted interpretative phenomenological analysis, which is an idiographic and inductive methodological approach that enables an in-depth examination of how people conceptualize their personal and social worlds. In-depth semi-structured individual interviews were conducted with six purposively sampled Brazilian clinical teachers who were attending a faculty development program. Each participant's lived experience was analyzed independently. Then, these individual analyses were compared against each other to identify convergence and divergence.
Participanembedding identity and looked for common points between the identities it contained, which allowed them to meaningfully reconcile the different demands from their overlapping professional identities. Thus, this research introduces the notion of embedding identity as a strategy to make sense of many professional identities. Variability in the embedding identities depicted in this investigation suggests the fluid and contextualized character of professional identity development. How participants saw themselves also influenced how they behaved and interacted with others accordingly. Understanding clinical teacher identity development enriches current perspectives of what it is like to be one of these medical professionals. Faculty development programs ought to consider these perspectives to better support clinical teachers in meeting the overlapping demands in education and patient care.The immature platelet fraction (IPF) is a measure of newly released platelets, which has been used as a marker of platelet production in multiple human studies but is not widely available in multispecies analyzers. IPF gates were created using undiluted and diluted (1/10) blood samples obtained from adult and newborn (postnatal day 10, P10) C57BL/6J wild-type (WT) ****, and from 3 murine models of thrombocytopenia c-MPL-/- ****, which lack the thrombopoietin receptor (hyporegenerative); antibody-mediated thrombocytopenia; and acute inflammation-induced thrombocytopenia. P10 **** were chosen because, at their size, we could consistently obtain (by terminal phlebotomy) the blood volume needed to run an undiluted sample. The undiluted blood IPF gate successfully differentiated between mechanisms of thrombocytopenia in both adult and P10 ****. For diluted samples, 2 IPF gates were generated a thrombocytopenic (T) gate, which performed well in samples with platelet counts (PCs) less then 800 × 109/L in adult **** and less then 500 × 109/L in newborn ****, and a non-thrombocytopenic (NT) gate, which performed well in samples with PCs above these thresholds. PCs and IPFs measured in diluted blood using these gates agreed well with those measured in undiluted blood and had good reproducibility. This technique first depends on the information of training data. Besides, one is not capable of using the ranking information in the estimation process. In this paper, we propose a ranked set sampling scheme in which ranking information from multiple sources can be combined and incorporated efficiently into both data collection and estimation. The ranked set sampling data is used for non-parametric and maximum likelihood estimation of ordinal categorical population. Through extensive simulation studies, the performance of estimators is evaluated.

The study was designed to analyze the technical efficiency of input use among the sesame producer farmers' in Maykadra Kebelle, Kafta-Humera district, Tigrai, Ethiopia; identified factors that influence farmers' resource use efficiency. Primary and secondary data sources were used and random sampling method was applied to select 187 sample sesame producers. Primary data were collected using structured questionnaire interview. Tobit two-stage model was employed to estimate farmers' resource efficiency of sesame production. In the first stage, Data Envelopment Analysis (DEA) was used to analyze farmers' technical and scale efficiency. In the second stage, factors that influence farmers' resource use efficiency were identified using the Tobit model. The DEA result indicated that the technical and scale efficiency of sesame producer farmers were 52% and 55% respectively. The result also revealed that under-utilization of the production inputs under consideration, especially land size and amount of seed used. Moreover, farmers' planting method (P = 0.030∗∗), age of the household head (P = 0.042∗∗), land size (P = 0.002∗∗∗), education of the household head (P = 0.001∗∗∗), and total asset owned (P = 0.024∗∗) were associated with farmers optimal input-output mix of sesame production. As a result, it can be concluded that smallholder farmers in the study area were inefficient in using inputs in sesame production. Therefore, the current inefficiency in sesame production could be improved by giving special attention and working on the factors that affect optimal input-output mix at the farm level.The use of organic fertiliser to improve soil health is crucial to halting the downward trend of crop yields in sub-Saharan Africa. If this goal is to be achieved, however, farmers require support to adopt organic fertiliser practices that match their attitudes and decision-making capacity. This study evaluated farmers' attitudes to a set of prevailing organic fertiliser practices and their associated behavioural costs (difficulty). The explanatory Rasch model was applied to a set of primary data from 250 farming households in north-east Ghana. The results showed that the average attitude of farmers was **** less than the difficulty estimate of an average organic fertiliser practice, although the practices generally showed a moderate difficulty. On average, farmers' attitudes matched just three of sixteen practices on the scale, with most (70 %) of the farmers showing very weak attitudes towards the input. Latent regression results revealed that the weak attitude levels were strongly related to key factors in the farmers' background, including education, resource endowment and access to extension services. Participation in determining policies on organic fertiliser use enhances farmers' knowledge and skills concerning use of the input. Hence, access to such policies can replace education for the less-educated majority of farmers. Thus, training programmes are proposed that develop the average farmer's capacity to adopt these practices in this area, especially the less difficult ones. Forty-nine food items were included in the final SFFQ with five open-ended questions for fruit groups. One hundred and six adolescents aged 15-17 years participated in the study. The total sugar intake among the adolescents was 58.80 g/day (52.7 g sucrose; 1.47 g fructose; 1.49 glucose) which contributed to 11.6% of the total energy intake per day. The reliability analysis showed a good agreement between the two administered SFFQs in a one-month interval. The relative validity results, using 6-days food diaries as a reference method, demonstrated a superior ability to rank individuals into the same and adjacent classification and only less then 10% gross misclassification in all sugar intakes. The developed SFFQ in turn has been proven to have moderate to good validity and be applicable for a larger epidemiological study.
Mothers who take care of their disabled children usually suffer from several problems, such as physical, psychological, and social problems. The disability of children can also directly impress the quality of their mothers' lives. This study is going to check out how the disability of children affects the quality of life of the mothers.
This study was conducted to compare the quality of life and resilience of mothers with disabled and typically developed children in Fasa, Iran.
The subjects of this cross-sectional study were 240 mothers (120 mothers having disabled children and 120 mothers with typically developed children) referring to Fasa State Health Centers and Fasa State Welfare Office. They were randomly selected and included in the study. All samples were collected by the convenience sampling method. The data were collected by using the World Health Organization Quality-of-Life Scale (WHOQOL-BREF) and the Connor-Davidson Resilience Scale (CD-RISC). The collected data were then analyzed by the SPresiliency in these two groups. These results suggest the necessity to provide supportive and therapeutic programs for improving the quality of life of mothers with disabled children.

The disability of children can also directly impress the quality of their mothers' lives. This study is going to check out how the disability of children affects the quality of life of the mothers. This study was conducted to compare the quality of life and resilience of mothers with disabled and typically developed children in Fasa, Iran. The subjects of this cross-sectional study were 240 mothers (120 mothers having disabled children and 120 mothers with typically developed children) referring to Fasa State Health Centers and Fasa State Welfare Office. They were randomly selected and included in the study. All samples were collected by the convenience sampling method. The data were collected by using the World Health Organization Quality-of-Life Scale (WHOQOL-BREF) and the Connor-Davidson Resilience Scale (CD-RISC). The collected data were then analyzed by the SPresiliency in these two groups. These results suggest the necessity to provide supportive and therapeutic programs for improving the quality of life of mothers with disabled children. This paper examines the efforts of one ACCHO, which in the absence of dedicated funding, pivoted its operations in response to COVID-19. Gurriny Yealamucka Health Service (Gurriny) is the only primary healthcare service in the discrete Indigenous community of Yarrabah, Far North Queensland.
The research was conducted at the request of the Chief Executive Officer of Gurriny. Using grounded theory methods, thirteen Gurriny staff and five Yarrabah and government leaders and community members were interviewed, transcripts of these interviews and 59 documents were imported into NVIVO-12 and coded, and key concepts were coesses, poorly coordinated communication and resource allocation channels, and bottlenecks in hierarchical funding approval processes.

The full societal impact COVID-19 pandemic is laid bare in urban mobility patterns. This research explored the recently published data on the operation of subway and bike share systems (BSS) during the COVID-19 outbreak in New York city, providing evidence on its impact over urban transport systems, but also on how its different components can work in conjunction. The BSS has proved to be more resilient than the subway system, with a less significant ridership drop (71% vs 90% ridership drop and 50% decrease on the ridership ratio) and an increase on its trips' average duration (from 13 min to 19 min per trip). Moreover, the study found evidence of a modal transfer from some subway users to the bike sharing system. After analyzing the transport volume of n = 15,715 routes in the timeframe of 23.03.2020 to 30.04.2020, a transport volume growth rate expressing the difference of real and expected transport volume was calculated. This ratio was then examined concerning the number of COVID-19 infections per day. The results of this study prove that the increasing freight volume for dry products in retail logistics does not depend on the duration of the COVID-19 epidemy but on the strength quantified through the total number of new infections per day. There is lack of research however, on finding common ground to determine the central tendencies if multiple predictions are accounted, which creates bias in decision making. The findings of this research provide a practical outlook for businesses and governments to better position their financial and policy decisions with regard to fossil fuels which could affect the lives of many. This research uses a novel prediction combination approach to determine an earliest-case peak oil occurrence through qualitative and quantitative methods for bias minimisation. The predictions are sourced from a balance of reputable private and public international agencies. The result interestingly finds commonality of reaching an earliest-case of peak oil in the year 2025 with feasible factors considered. The research discusses the inflection point forecast and financial risk mitigation recommendations for private entities and governments gathered from expert reports and articles in the field of oil supply and demand.The unprecedented global shutdown that resulted from the COVID pandemic presents an opportunity to reconfigure future transport policy and practice for the benefit of the global environment and individual citizens alike. The public health crisis has necessitated an urgent reconsideration of transport and its contribution to post-COVID economic recovery. In recognition of the importance of individual behaviour and collective responsibility in protecting both personal and public health during the crisis, this think piece proposes a new concept of Responsible Transport to help inform and shape transport policy and practice responses to COVID. The novelty of this proposal lies in the fact that it incorporates not only environmental considerations with respect to sustainability but also encompasses considerations of individual and community health and wellbeing. Moreover, it stresses the role of the individual as a responsible autonomous actor in delivering socially desired transport outcomes.The purpose of this paper is to analyze the effects of uncertainty shocks on airline employment in the light of the current global pandemic. The airline industry has faced many threats throughout history, but none quite as rapid and severe as the one posed by the spread of COVID-19. One constant during uncertainty shocks and industry downturns is that airline labor bears the brunt of the decline. As the industry reduces capacity amid the increase in travel restrictions, the post-stimulus impacts to airline labor are not known. Using time series analysis, the dynamics of historical uncertainty shocks to the industry are examined. During periods of uncertainty shocks, the estimated job loss is nearly 7% of the airline workforce with an upper bound of over 13%. Major airline employment is most impacted, while low-cost and regional airline employment is least impacted. The hardest hit employees are ones related to passenger handling and flight operations, while management employees fair slightly better during these uncertain periods.

This research explored the recently published data on the operation of subway and bike share systems (BSS) during the COVID-19 outbreak in New York city, providing evidence on its impact over urban transport systems, but also on how its different components can work in conjunction. The BSS has proved to be more resilient than the subway system, with a less significant ridership drop (71% vs 90% ridership drop and 50% decrease on the ridership ratio) and an increase on its trips' average duration (from 13 min to 19 min per trip). Moreover, the study found evidence of a modal transfer from some subway users to the bike sharing system. The first effects of the free BSS programs aimed at essential service workers were also evaluated.
