Nanoscale phase-change contrast agents (PCCAs) have been found to have great potential in non-invasive extravascular imaging and therapeutic delivery. However, the contrast-to-tissue ratio (CTR) of PCCA images is usually limited because of either physiological motion or incomplete cancelation of tissue signal. Therefore, to improve the CTR of PCCA images in the presence of physiological motion, a new imaging technique, ultrafast inter-frame activation ultrasound (UIAU) imaging, is proposed and validated. Results of studies with controlled motion in tissue-mimicking phantoms indicate UIAU could provide significantly higher CTRs (maximum 17.3 ± 0.9 dB) relative to conventional pulse inversion imaging (maximum CTR 3.4 ± 1.4 dB). UIAU has CTRs up to 16.1 ± 1.0 dB relative to 3.9 ± 2.3 dB for differential imaging in the presence of physiological motion at 20 mm/s. In vivo imaging of PCCAs in the rat liver also reveals the ability of UIAU to enhance PCCA image contrast in the presence of physiological motion. Using ultrasound for the diagnostic workup of thyroid lesions is a widely accepted technique. An inexpensive phantom model of thyroid lesions would be an invaluable tool for engaging medical students in learning the diagnostic algorithm for thyroid lesions and how to perform fine needle aspiration (FNA). The aim of this study was to devise an inexpensive and reproducible training phantom model for thyroid lesion detection, image interpretation and in vitro FNA using ultrasound guidance. A simple phantom model imitating benign cystic lesions, intermediately suspicious lesions and highly suspicious lesions was developed using a chicken breast, red seedless grapes, pimento olives and blackberries. The phantom was constructed for a total cost of $4.09 per unit and constructed in approximately 3 min. Nine models were constructed in total, demonstrating that the model design is replicable. This thyroid FNA phantom is an inexpensive, easy-to-produce model that allows medical students to practice measuring lesions and performing FNAs using ultrasonography. Future studies could be explored to assess this model's role in medical student education. MATERIALS AND METHODS Retrospective data collection was performed, identifying 105 procedures (100 patients) who underwent non-articular MPHF surgery from 2010-2016 19 PBS, 65 IMN, 21 CPF. All patients were followed for at least two years or until death. RESULTS Reoperation rates were similar at one year (10.5%,6.2%,4.8%, p = 737), two years (15.8%,6.2%,9.5%, p = 375), and final evaluation (15.8%,7.7%,14.3%, p = 248). The rate of BI for PBS, IMN, and CPF was 10.5%,0%, and 4.8% (p = 049 PBS/IMN) at one year, 15.8%,0%, and 9.5% (p = 010 PBS/IMN) at two years, and 15.8%,0%, and 14.3% (p = 010 IMN/PBS, p = 013 IMN/CPF) at final evaluation. CONCLUSIONS Reoperation rate was not significantly different at any time point. However, IMN surgery resulted in the lowest rate of broken implants (zero), statistically significant versus PBS at all time periods and versus CPF at final follow-up. PBS may eventually offer selected advantages for MPHF management, but current data suggests fragility must be thoughtfully considered. Here, we demonstrated that EBV-infection amplified EGF-stimulated Ca2+ responses through the promotion of intracellular aggregation of STIM1, which serves as a Ca2+ sensor to activate SOCE. Blockage of EBV-remodeled Ca2+ signaling by STIM1-silencing inhibited cell migration by interrupting epithelial-mesenchymal transition (EMT) in vitro, and suppressed tumor dissemination in zebrafish and lymph node metastasis in ****. In addition, STIM1 expression was upregulated in primary NPC tissues compared with normal nasopharyngeal epithelium and stronger among the patients with advanced lymph node metastatic disease (N2-3 stage). Our findings thus indicate that EBV promotes metastatic potential by enhancing STIM1-dependent Ca2+ signaling that manipulates EMT in NPC cells. EBV-modulated Ca2+ signaling could serve as a candidate anti-metastatic target for NPC treatment. Food sensory imagery - creating a vivid mental image of the sensory experience of eating - can lead to the selection of smaller portions because it serves as a reminder that eating enjoyment does not necessarily increase with portion size. The evidence is mostly limited to adults and to energy-dense foods for which it is particularly difficult to predict the satiating effects of consumption quantity. The objective was to study how food sensory imagery influences portion size selection of foods varying in energy density (brownie and applesauce) by 7- to 11-year-old children. During after-school time, 171 children were randomized into two conditions. Children in the food sensory imagery condition were asked to imagine the taste, smell, and texture of eating palatable foods, i.e., chocolate cereal, chocolate waffle, and chocolate candies. Children in the control condition performed a similar sensory imagery task for non-food-related activities. Children were then asked to choose between the recommended serving size, a 50% larger portion, and a 125% larger portion of either brownie or applesauce. One week later, they were placed in the same condition for the other food. Compared to the control condition, food sensory imagery led children to choose 7.1% less brownie but had no effect on applesauce portion selection. Exploratory findings showed that the food sensory imagery intervention was especially effective at reducing brownie portion size selection among children who were moderately hungry, who usually eat fast, and whose parents pressure them to eat. In conclusion, food sensory imagery may be a useful intervention to nudge children towards healthier portion size choices because it reduces the selected portion size of an energy-dense snack without reducing the selected portion size of a healthier snack. Interpersonal food behaviors, such as food sharing and food offering, are associated with closeness between people and thought to facilitate the formation and strengthening of social bonds. Despite the importance of food and food-related behavior in general and for social bonding specifically, there is relatively little research about the psychological underpinnings of food-related behavior within relationship science. To fill this gap, we conducted three studies focusing on food behaviors and attachment style. In Study 1, we found attachment avoidance to be negatively associated with engagement in food sharing behaviors, and with a lower likelihood of dating a potential partner due to the partner's food preferences. In Study 2, we found that enhancing attachment security increased the tendency to offer one's food to a fellow participant. In Study 3, we found that the tendency to accept food from a fellow participant was positively associated with attachment anxiety, but security priming did not affect this tendency. Implications for attachment, close relationships, and food sciences are discussed. BACKGROUND Premature ventricular contractions (PVC) are known to reduce the percentage of biventricular (BiV) pacing in patients with cardiac resynchronization (CRT), decreasing the clinical response. The aim of this study was to evaluate the prevalence of a high PVC burden, as well as therapeutic action (pharmacotherapy, catheter ablation or device programming), in a large CRT implantable-defibrillator (CRT-D) population. METHODS Patients with a CRT-D device from the UMBRELLA multicenter prospective remote monitoring registry were included. The PVC count was collected from each remote monitoring transmission. Patients were divided into two high (≥1 transmission ≥200/≥400 PVC/h, respectively) and one low (all transmissions less then 200 PVC/h) PVC count groups. The PVC burden following a high PVC count transmission was calculated. RESULTS Of 1268 patients, 135 (11%) and 43 (3.4%) presented high PVC count (≥200/≥400 PVC/h, respectively). The majority of patients in the high PVC groups were not treated (61 [79%] and 32 [74%], respectively. Considering the untreated patients in the high PVC groups, median PVC/h was 199 (interquartile range [IQR] 196) and 271 (IQR 330), respectively. The PVC burden (proportion of time with PVC/h ≥ 200/≥400) was 40% (IQR 70) and 29% (IQR 59), respectively. CONCLUSION A significant proportion of CRT-D patients presented a high PVC count, however, few received treatment. In the untreated patients with a high PVC count, the PVC burden during follow-up varied substantially. Several consecutive recordings of a high PVC count should be warranted before considering therapeutic action such as catheter ablation.

Coordinated assembly of viral and host factors is essential for the successful propagation of viruses as well as the generation of host anti-viral response. Previous studies from our group, as well as from other groups, have identified host proteins interacting with various components of the Hepatitis E virus (HEV). However, the functional relevance of host protein interactions in HEV replication context has been notably overlooked. The present study reports that heterogeneous nuclear ribonucleoproteins (hnRNPs), namely hnRNPK, hnRNPA2B1, hnRNPH, PCBP1, and PCBP2 interact with HEV RNA promoter and RNA dependent RNA polymerase to regulate HEV replication. We found that hnRNPK and hnRNPA2B1 are the virus supportive factors interacting with HEV RNA at promoter regions along with HEV polymerase protein, which are essential for HEV replication in the cells. Contrarily, hnRNPH, PCBP1, and PCBP2 are the anti-viral factors that interact exclusively with HEV genomic promoter and inhibit HEV replication in Huh7 S10-3 cells. In-vitro RNA binding assays revealed that the anti-viral hnRNP proteins hamper the binding of virus supportive hnRNP proteins at HEV genomic promoter. In the binding reaction, the binding of HEV polymerase protein to the genomic promoter is slightly affected by the presence of anti-viral hnRNPH. In an effort of visualizing the subcellular localization of hnRNP proteins in the HEV replication scenario in the Huh7 cells, we showed that hnRNPK, hnRNPA2B1, hnRNPH, PCBP1, and PCBP2 redistribute from nucleus to cytoplasm. In conclusion, our study highlights the importance of hnRNP proteins in HEV replication regulation. Amyotrophic Lateral Sclerosis (ALS) and Frontotemporal Dementia (FTD) constitute aggressive neurodegenerative pathologies that lead to the progressive degeneration of upper and lower motor neurons and of neocortical areas respectively. In the past decade, the identification of several genes that cause these disorders indicated that the two diseases overlap in a multifaceted spectrum of conditions. Genetic evidence has revealed that several genes with a mechanistic role at different stages of the autophagy process are mutated in ALS/FTD patients. Moreover, the three main proteins aggregating in ALS/FTD, including in sporadic cases, are also targeted by autophagy and affect this process. Here, we examine the varied dysfunctions and degrees of involvement of the autophagy-lysosome system that have been discovered in ALS/FTD. Additionally, three assays further confirmed that ApoM was a novel virus particle associated protein. We also showed that ApoM was required for HCV production, especially for the assembly/release step of HCV life cycle.
Coordinated assembly of viral and host factors is essential for the successful propagation of viruses as well as the generation of host anti-viral response. Previous studies from our group, as well as from other groups, have identified host proteins interacting with various components of the Hepatitis E virus (HEV). However, the functional relevance of host protein interactions in HEV replication context has been notably overlooked. The present study reports that heterogeneous nuclear ribonucleoproteins (hnRNPs), namely hnRNPK, hnRNPA2B1, hnRNPH, PCBP1, and PCBP2 interact with HEV RNA promoter and RNA dependent RNA polymerase to regulate HEV replication. We found that hnRNPK and hnRNPA2B1 are the virus supportive factors interacting with HEV RNA at promoter regions along with HEV polymerase protein, which are essential for HEV replication in the cells. Contrarily, hnRNPH, PCBP1, and PCBP2 are the anti-viral factors that interact exclusively with HEV genomic promoter and inhibit HEV replication in Huh7 S10-3 cells. In-vitro RNA binding assays revealed that the anti-viral hnRNP proteins hamper the binding of virus supportive hnRNP proteins at HEV genomic promoter. In the binding reaction, the binding of HEV polymerase protein to the genomic promoter is slightly affected by the presence of anti-viral hnRNPH. In an effort of visualizing the subcellular localization of hnRNP proteins in the HEV replication scenario in the Huh7 cells, we showed that hnRNPK, hnRNPA2B1, hnRNPH, PCBP1, and PCBP2 redistribute from nucleus to cytoplasm. In conclusion, our study highlights the importance of hnRNP proteins in HEV replication regulation. This study aims to determine the impact of credit constraints on wheat farmers' welfare. For this study, data on 575 wheat farmers were collected through a simple random sampling technique. The treatment-effect model was used to find the effect of credit constraints on the farmers' welfare. In addition, to control for the problem of endogeneity, ordinary least squares and logistic regressions were employed. Farmers' welfare was measured by as consumption. The results show that constrained farmers cultivate 2.8-4.1% more area of land than unconstrained farmers but that the spending and income per capita of credit-constrained farmers are 18.9 to 13.8% lower, respectivley, than those of unconstrained farmers. Moreover, the results indicate that the welfare and income of credit-constrained farmers are influenced by age, the interest rate, area of land, and family size. An increase in the area of land enhances wellbeing and returns for the constrained farmer, which is in contrast to the unconstrained farmer. The results show that credit constraints have a negative impact on farmers' welfare and income. Better welfare may only be achieved if credit is supplied to credit-constrained farmers. Furthermore, this study has potentially significant implications. First, the negative impact of the interest rate suggests that the State Bank of Pakistan should modify agricultural credit policies, particularly to design a flexible interest rate for farmers. Second, the central bank should amend the agricultural credit limits based on the current financial needs of the agricultural market when the rate of inflation is high and the impact of agricultural crises is long and drawn-out. Third, the government should launch an agricultural Islamic bank in the study area. This Islamic bank would support religious farmers who are constrained due to objections to interest, and it would relax the farmers' credit constraints as well as help them to increase welfare and income.Changes in the levels of pH, lactic acid, acetic acid, headspace volatile compounds, and spoilage odour intensity during a 35 day refrigerated storage period in two sets of a Balkan-style fresh sausage, a control sausage (CON) and a sausage containing an aqueous hop extract (HOP), packaged under a 20% CO2 and 80% N2 atmosphere were evaluated. Storage resulted in progressive sausage acidification and increased the levels of acetic acid, 1-methylbutanol, ethyl acetate, ethyl hexanoate, and 2-ethylhexanol; all of which are associated with anaerobic microbial metabolism under sugar restricted conditions. Storage decreased the levels of hexanal, heptanal, pentanol, and garlic-derived organosulfur compounds. Hop extract showed oxygen scavenging abilities, and decreased the levels of the volatile compounds derived from lipid auto-oxidation while contributing to the presence of specific terpene compounds. The use of hop extract did not improve the shelf life of sausages packed under anaerobic atmosphere. The spoilage odour appeared in both types of sausages on the 14th day, and it was considered strong from day 21 onwards.Lactobacillus is a beneficial bacteria that could inhibit pathogenic potential of other microorganisms. This study aimed at characterizing and suggesting the reusability potential of AMD sludge to reduce the environmental problem caused by its accumulation so that it could become a valuable material. The sludge was obtained after treating a synthetic AMD with a green oxidant sodium ferrate (VI) (Na2FeO4) that was prepared by a wet oxidation method. Chemical and physical characterization of a dried sludge generated after treatment was then performed using the Fourier Transform-Infrared and X-Ray powder Diffraction spectroscopy. Scanning Electron Microscopy-Energy Dispersive X-ray Spectroscopy also served to identify the surface morphology of the sludge. The sludge presented a high weight percentage of Fe and O and lower concentrations of other metals such as Al, Mn, Si, and Na. Nitrogen adsorption/desorption isotherms or Brunauer-Emmett-Teller (BET) was used to assess the surface area, pore volume and diameter of the sludge. The BET results showed that the surface area of the sludge obtained after treating the synthetic AMD using Na2FeO4 was 31.50 ± 0.03 m2/g with pore diameter and volume of 52.50 nm and 0.41 cm3/g, respectively. However, the produced sludge could serve as an adsorbent to remove pollutants from water or to synthesize different magnetic nanocomposites due to its high surface area (>natural zeolite) and high composition of Fe and O.Illicit use of Sudan dyes, a group of harmful and carcinogenic azo dyes, in the food industry has taken a surge in various parts of the world, especially in Africa. This study also sought to experiment and propose an alternative method for quick, easy, cheap, robust and ecologically safe analysis of Sudan dyes in chilli pepper powder and similar matrices. The optimized method used a 6.0 mL mixture of acetoneacetonitrile (15 v/v) solvent in a modified QuEChERs method for extraction of Sudan dyes I-IV. The simultaneous analysis of the dyes were achieved on Shimadzu prominence UFLC 20AD coupled with SPD 20AX UV detector operated at dual wavelength of 500 and 480 nm. A total of twenty four (24) chilli pepper powder samples from eight different vendors on the Ghana market were analysed using the optimized method.

The catalyst showed excellent catalytic activity toward the oxidation of alcohol to aldehyde in an aqueous medium below the lower critical solution temperature (LCST) and pKa values (7-7.5) of the copolymer. The main advantages of the hybrid catalyst, as compared to the existing catalysts, are outstanding alcohol conversion (up to 99%) for a short reaction time (1 h), small amount of the catalyst (5 mg), and good recyclability equal to at least five times.Iron is crucial to satisfy several mitochondrial functions including energy metabolism and oxidative phosphorylation. Patients affected by Myelodysplastic Syndromes (MDS) and acute myeloid leukemia (AML) are frequently characterized by iron overload (IOL), due to continuous red blood cell (RBC) transfusions. This event impacts the overall survival (OS) and it is associated with increased mortality in lower-risk MDS patients. Accordingly, the oral iron chelator Deferasirox (DFX) has been reported to improve the OS and delay leukemic transformation. However, the molecular players and the biological mechanisms laying behind remain currently mostly undefined. The aim of this study has been to investigate the potential anti-leukemic effect of DFX, by functionally and molecularly analyzing its effects in three different leukemia cell lines, harboring or not p53 mutations, and in human primary cells derived from 15 MDS/AML patients. Our findings indicated that DFX can lead to apoptosis, impairment of cell growth only in a context of IOL, and can induce a significant alteration of mitochondria network, with a sharp reduction in mitochondrial activity. Moreover, through a remarkable reduction of Murine Double Minute 2 (MDM2), known to regulate the stability of p53 and p73 proteins, we observed an enhancement of p53 transcriptional activity after DFX. Interestingly, this iron depletion-triggered signaling is enabled by p73, in the absence of p53, or in the presence of a p53 mutant form. In conclusion, we propose a mechanism by which the increased p53 family transcriptional activity and protein stability could explain the potential benefits of iron chelation therapy in terms of improving OS and delaying leukemic transformation.The aim of this study was to investigate the pressure agglomeration process of wheat straw (WS) and the blends of WS with calcium carbonate (CC) or cassava straw (CS) with a ratio of 6% wt./wt. from seven separate fractions with sizes in the range of 0.21-2.81 mm. The agglomeration was performed at a moisture of 30% wb and a material temperature of 78 °C, with a dose of 0.1 g, in a die of diameter 8 mm and height 80 mm. The effects of the process were evaluated based on the compaction parameters and the pellets' density, tensile strength, and water absorption. The incorporation of additives into the WS improved the pellet process and quality. Refined results were achieved after adding CC, as compared to those achieved after adding CS, and the preferred particle size was in the range of 1.00-1.94 mm. This was because, under the given conditions, the **** pressure in the die chamber significantly increased, allowing the achievement of a single pellet density of 800 kg·m-3. The pellets were resistant to compressive loads and cracked only at tensile strength of 6 MPa and a specific compression work of 6.5 mJ·mm-2. The addition of CC to the WS improved the strength of the adhesive and the cohesive bonds between the particles. The water absorption for the uncrushed pellets was considerably less than that for crushed pellets, which results in the safer storage of uncrushed pellets and excellent moisture absorption of crushed pellets. The addition of CC to the WS offers benefits in the form of pellet strength with a high water absorption capability. Notably, a study of crushed pellet litter under broiler rearing conditions and an analysis of the operational costs of using WS additives are required for implementing this study.Influenza viruses and respiratory syncytial virus (RSV) are respiratory viruses that primarily circulate worldwide during the autumn and winter seasons. Patients affected by Myelodysplastic Syndromes (MDS) and acute myeloid leukemia (AML) are frequently characterized by iron overload (IOL), due to continuous red blood cell (RBC) transfusions. This event impacts the overall survival (OS) and it is associated with increased mortality in lower-risk MDS patients. Accordingly, the oral iron chelator Deferasirox (DFX) has been reported to improve the OS and delay leukemic transformation. However, the molecular players and the biological mechanisms laying behind remain currently mostly undefined. The aim of this study has been to investigate the potential anti-leukemic effect of DFX, by functionally and molecularly analyzing its effects in three different leukemia cell lines, harboring or not p53 mutations, and in human primary cells derived from 15 MDS/AML patients. Our findings indicated that DFX can lead to apoptosis, impairment of cell growth only in a context of IOL, and can induce a significant alteration of mitochondria network, with a sharp reduction in mitochondrial activity. Moreover, through a remarkable reduction of Murine Double Minute 2 (MDM2), known to regulate the stability of p53 and p73 proteins, we observed an enhancement of p53 transcriptional activity after DFX. Interestingly, this iron depletion-triggered signaling is enabled by p73, in the absence of p53, or in the presence of a p53 mutant form. In conclusion, we propose a mechanism by which the increased p53 family transcriptional activity and protein stability could explain the potential benefits of iron chelation therapy in terms of improving OS and delaying leukemic transformation.The aim of this study was to investigate the pressure agglomeration process of wheat straw (WS) and the blends of WS with calcium carbonate (CC) or cassava straw (CS) with a ratio of 6% wt./wt. from seven separate fractions with sizes in the range of 0.21-2.81 mm. The agglomeration was performed at a moisture of 30% wb and a material temperature of 78 °C, with a dose of 0.1 g, in a die of diameter 8 mm and height 80 mm. The effects of the process were evaluated based on the compaction parameters and the pellets' density, tensile strength, and water absorption. The incorporation of additives into the WS improved the pellet process and quality. Refined results were achieved after adding CC, as compared to those achieved after adding CS, and the preferred particle size was in the range of 1.00-1.94 mm. This was because, under the given conditions, the back pressure in the die chamber significantly increased, allowing the achievement of a single pellet density of 800 kg·m-3. The pellets were resistant to compressive loads and cracked only at tensile strength of 6 MPa and a specific compression work of 6.5 mJ·mm-2. The addition of CC to the WS improved the strength of the adhesive and the cohesive bonds between the particles. The water absorption for the uncrushed pellets was considerably less than that for crushed pellets, which results in the safer storage of uncrushed pellets and excellent moisture absorption of crushed pellets. The addition of CC to the WS offers benefits in the form of pellet strength with a high water absorption capability. Notably, a study of crushed pellet litter under broiler rearing conditions and an analysis of the operational costs of using WS additives are required for implementing this study.Influenza viruses and respiratory syncytial virus (RSV) are respiratory viruses that primarily circulate worldwide during the autumn and winter seasons. This study first unravels the role of BAK1 in mediating high light-triggered activation of CATs, thereby degrading H2O2 and regulating plant growth and development in Arabidopsis.We investigated the metabolite changes of Morus roots (MRs) according to different cultivar families (Simheung, Daesim, Cheong-il, Sangchon, Daeseong, Suhong, Suwon, and Igsu) using ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-QTOF-MS) to understand the relationship between different cultivars and metabolite changes. Data were analyzed by partial least squares discriminant analysis (PLS-DA), and samples were successfully separated in PLS-DA scores. Eight metabolites in the electrospray ionization (ESI)-positive mode and 16 metabolites in the ESI-negative mode contributed to the separation in PLS-DA. Our data suggest that comparative analysis of MR metabolites according to different cultivars is useful to better understand the relationship between the different cultivars and metabolite changes. Furthermore, we analyzed the MRs for their ability to improve benign prostatic hyperplasia (BPH). LNCaP cells were used to evaluate the prostate-specific antigen (PSA) inhibitory activity of MRs, and, amongst them, the extract with the highest activity was selected. Igsu demonstrated the highest inhibition effect of prostate-specific antigen (PSA) expression among the MR cultivars. Igsu was also evaluated by administration in a testosterone-induced benign prostatic hyperplasia model in Sprague-Dawley rats. We also measured serum 8-hydroxy-2-deoxyguanosine (8-OHdG) levels (an index of oxidative stress) and carotid intima-media thickness (IMT), a surrogate marker of atherosclerosis. After adjusting them for age and sex, telomere length inversely correlated (p less then 0.05) with NADPH oxidase-mediated superoxide production, with 8-OHdG values, and with carotid IMT. Interestingly, the asymptomatic subjects with plaques have a lower telomere length (p less then 0.05), and higher values of plasma 8-OHdG and superoxide production (p less then 0.05). These data were confirmed in a second population in which patients with coronary artery disease showed lower telomere length and higher 8-OHdG and superoxide production than the asymptomatic subjects. Postoperative adhesions represent the most common cause of acute small bowel obstruction (80%) and are usually a consequence of abdomino-pelvic surgery performed with open technique.
A 45-year-old black man arrived at the emergency room with abdominal pain and distension three months after laparoscopic distal gastrectomy with Roux-en-Y anastomosis performed for benign pyloric stenosis. CT abdominal scan revealed some air-fluid levels in the center of the abdomen with distension of proximal jejunal loops caused by intestinal adhesions. Laparoscopic adhesiolysis was performed to restore the intestinal transit.
The formation of adhesions is more frequent after abdomino-pelvic surgery. CT abdominal scan is very useful tool to identify the level and the aetiology of obstruction and it may predict the need for surgery, the location of different adhesive bands in order to identify wich patients are likely candidates for laparoscopic treatment.
In selected cases, laparoscopic approach for small bowel obstruction is a good surgical option. In patients with adbominal dense adhesions or clinical signs of intestinal ischemia, conversion to laparotomy should be considered an alternative.

We present the case of a 42-years old man who was admitted to the emergency department at our Institution presenting with a 5-days history of right sided abdominal pain. Laboratory blood count reported leukocytosis and alteration of principal inflammation index; at the abdominal ultrasound no signs of perforation or collection were described.
Indication to surgery was posed and an explorative laparoscopy was performed. The presence of local peritonitis at the right colonic flexure secondary to a full thickness bowel perforation caused by a toothpick was found. There was also an acute phlegmonous appendicitis. A laparoscopic appendectomy and a full-thickness double running suture of the perforation were performed.

Patients with upper-tract carcinoma in situ (UT-CIS) that have failed treatment with BCG are recommended for radical nephroureterectomy (RNU). We describe a cohort of patients with BCG-refractory UT-CIS that were treated with docetaxel, a novel agent in the approach to topical therapy.
Patients with pathologically proven UT-CIS from 2012 to 2020 with an imperative indication for organ preservation and history of BCG-refractory disease were included. Each patient underwent ureteroscopy with biopsy and selective cytology pre- and postinduction, and after each maintenance course. Complete response (CR) was defined as the absence of visualized lesions on ureteroscopy, negative selective cytology, and absence of clinical progression. No response (NR) was defined as persistence of lesions after induction or absence of visualized lesions with persistently positive cytology.
Seven patients and 10 renal units were treated. Six of the 10 renal units had initial CR (60%). Three patients with NR went on to have RNU, one of which subsequently died due to cancer-specific mortality. One patient with bilateral disease had NR in 10 renal unit and cure in the other. This patient subsequently developed recurrence in his remaining renal unit. A second patient had CR in both kidneys for 6 years, but 1 year after finishing maintenance regimen developed HG disease in 1 ureter. Average follow-up was 33 months.

Prostate cancer and cardiovascular (CV) disease share several risk factors, with the incidence of both rising with increasing age. We describe a cohort of patients with BCG-refractory UT-CIS that were treated with docetaxel, a novel agent in the approach to topical therapy. Patients with pathologically proven UT-CIS from 2012 to 2020 with an imperative indication for organ preservation and history of BCG-refractory disease were included. Each patient underwent ureteroscopy with biopsy and selective cytology pre- and postinduction, and after each maintenance course. Complete response (CR) was defined as the absence of visualized lesions on ureteroscopy, negative selective cytology, and absence of clinical progression. No response (NR) was defined as persistence of lesions after induction or absence of visualized lesions with persistently positive cytology. Seven patients and 10 renal units were treated. Six of the 10 renal units had initial CR (60%). Three patients with NR went on to have RNU, one of which subsequently died due to cancer-specific mortality. One patient with bilateral disease had NR in 10 renal unit and cure in the other. This patient subsequently developed recurrence in his remaining renal unit. A second patient had CR in both kidneys for 6 years, but 1 year after finishing maintenance regimen developed HG disease in 1 ureter. Average follow-up was 33 months. This study demonstrates efficacy of docetaxel as a treatment option for patients with UT-CIS with a contraindication to RNU after failing BCG. However, the best PCEs of reported ideal-bandgap (1.3-1.4 eV) Sn-Pb PSCs with a higher 33% theoretical efficiency limit are 0.4 V). Herein, it is found that the addition of guanidinium bromide (GABr) can significantly improve the structural and photoelectric characteristics of ideal-bandgap (≈1.34 eV) Sn-Pb perovskite films. GABr introduced in the perovskite films can efficiently reduce the high defect density caused by Sn2+ oxidation in the perovskite, which is favorable for facilitating hole transport, decreasing charge-carrier recombination, and reducing the Voc deficit. Therefore, the best PCE of 20.63% with a certificated efficiency of 19.8% is achieved in 1.35 eV PSCs, along with a record small Voc deficit of 0.33 V, which is the highest PCE among all values reported to date for ideal-bandgap Sn-Pb PSCs. Moreover, the GABr-modified PSCs exhibit significantly improved environmental and thermal stability. Of the 27 peptides, 13 were stable after incubation at 37 °C for 72 hr, but the remaining 14 were less stable. Ex vivo catabolites of these 14 peptides were detected using their theoretical masses generated in silico, their appearance was monitored over the time course of incubation, and their identity was verified by their product ion spectra. Catabolites identified for chemotactic peptide, DALDA, dmtDALDA, deltorphins I and II, Hyp6 -dermorphin, Lys7 -dermorphin and dermorphin analog are novel. A D-amino acid residue at position 2 or 1 of a peptide or next to its C-terminus protected the relevant terminal from degradation by exopeptidases, but such a residue at position 3 did not. A pGlu residue or N-methylation at the N-terminus of a peptide did not protect its N-terminal. Ethylamide at the C-terminus of a peptide provided the C-terminal protection from attacks by carboxypeptidases. A standardized questionnaire collected demographic and medical information; the Medication Adherence Rating Scale (MARS) was used to evaluate self-reported adherence. Questionnaire data were analysed using summary descriptive statistics. Interview data were subject to qualitative content analysis. Programmed cell death protein 1 (PD-1) is a critical inhibitory receptor that limits excessive T cell responses. Cancer cells have evolved to evade these immunoregulatory mechanisms by upregulating PD-1 ligands and preventing T cell-mediated anti-tumor responses. Consequently, therapeutic blockade of PD-1 enhances T cell-mediated anti-tumor immunity, but many patients do not respond and a significant proportion develop inflammatory toxicities. To improve anti-cancer therapy, it is critical to reveal the mechanisms by which PD-1 regulates T cell responses. We performed global quantitative phosphoproteomic interrogation of PD-1 signaling in T cells. By complementing our analysis with functional validation assays, we show that PD-1 targets tyrosine phosphosites that mediate proximal T cell receptor signaling, cytoskeletal organization, and immune synapse formation. PD-1 ligation also led to differential phosphorylation of serine and threonine sites within proteins regulating T cell activation, gene expression, and protein translation. In silico predictions revealed that kinase/substrate relationships engaged downstream of PD-1 ligation. These insights uncover the phosphoproteomic landscape of PD-1-triggered pathways and reveal novel PD-1 substrates that modulate diverse T cell functions and may serve as future therapeutic targets. These data are a useful resource in the design of future PD-1-targeting therapeutic approaches.
The association between obesity and outcomes in patients receiving programmed death-1/programmed death ligand-1 (PD-L1) checkpoint inhibitors has already been confirmed in pre-treated non-small cell lung cancer (NSCLC) patients, regardless of PD-L1 tumor expression.
We present the outcomes analysis according to baseline body mass index (BMI) and BMI variation in a large cohort of metastatic NSCLC patients with a PD-L1 expression ≥50%, receiving first line pembrolizumab. We also evaluated a control cohort of metastatic NSCLC patients treated with first line platinum-based chemotherapy. Normal weight was set as control group.
962 patients and 426 patients were included in the pembrolizumab and chemotherapy cohorts, respectively. Tumor mutational burden (TMB) measurement is limited by low tumor purity of samples, which can influence prediction of the immunotherapy response, particularly when using whole-exome sequencing-based TMB (wTMB). This issue could be overcome by targeted panel sequencing-based TMB (pTMB) with higher depth of coverage, which remains unexplored.
We comprehensively reanalyzed four public datasets of immune checkpoint inhibitor (ICI)-treated cohorts (adopting pTMB or wTMB) to test each biomarker's predictive ability for low purity samples (cut-off 30%). For validation, paired genomic profiling with the same tumor specimens was performed to directly compare wTMB and pTMB in patients with breast cancer (paired-****, n=165) and ICI-treated patients with advanced non-small-cell lung cancer (paired-NSCLC, n=156).

Sterile processing departments (SPDs) play a crucial role in surgical safety and efficiency. SPDs clean instruments to remove contaminants (decontamination), inspect and reorganise instruments into their correct trays (assembly), then sterilise and store instruments for future use (sterilisation and storage). However, broken, missing or inappropriately cleaned instruments are a frequent problem for surgical teams. These issues should be identified and corrected during the assembly phase.
A work systems analysis, framed within the Systems Engineering Initiative for Patient Safety (SEIPS) model, was used to develop a comprehensive understanding of the assembly stage of reprocessing, identify the range of work challenges and uncover the inter-relationship among system components influencing reliable instrument reprocessing.
The study was conducted at a 700-bed academic hospital in the Southeastern United States with two reprocessing facilities from October 2017 to October 2018. Fifty-six hours of direct observations, 36 interviews were used to iteratively develop the work systems analysis.
