For the first time it is shown that the development of resistance to melittin in Acholeplasma laidlawii, a mycoplasma that is widely spread in nature and that is the main contaminant of cell cultures and vaccines, is associated with significant changes in the genomic profile, in cellular and vesicular proteomes, as well as in virulence.The growth-stimulating activity of three selenium nanocomposites (NCs) in various matrices based on arabinogalactan (NC Se/AG, 6.4% Se), starch (NC Se/St, 2% Se), and carrageenan (NC Se/Car, 12% Se) with respect to plants of radish, soybean, and potato was investigated. It was shown that the treatment of plant seeds with NCs stimulated root growth during germination. It was found that the studied NCs affected both the level of lipid peroxidation and the activity of the antioxidant enzyme glutathione peroxidase (GPX). The treatment of radish seeds with NCs stimulated root growth during their germination and reduced the content of diene conjugates (DC) in root tissues. It was shown that soaking seeds in NC Se/AG solution increased the GPX activity in the tissues of the radish root by 40%. Stimulation of soybean root growth under the influence of NC Se/Car may also be associated with the activation of GPX. Furthermore, in potato plants, this NC led to the stimulation of germination; however, this was probably due to the activation of other antioxidant enzymes. The yellow color of C. chinense fruit may be the result of both decreased PSY1 expression and increased PSY2 transcription. Thus, it was demonstrated that the acquired fruit pigmentation retains strict phylogenetic limitations, which, however, can be overcome using artificial selection for the activity of phytoene synthase PSY1.The damage to blood coagulation factor XIII (FXIII) at different stages of its enzymatic activation under the action of various physiological amounts of hypochlorite ion was studied. The results obtained by HPLC-MS/MS, SDS-PAGE, and colorimetry showed that, during the conversion of FXIII to FXIIIa, the vulnerability of FXIII to hypochlorite-induced oxidation increased. Cancer poses a substantial health and economic burden on patients and caregivers in Canada. Previous reviews have estimated the indirect cost burden as work-related productivity losses associated with cancer. However, these estimates require updating and complementing with more comprehensive data that include relevant dimensions beyond labor market costs, such as patient time, lost leisure time and home productivity losses.
A systematic review of the literature was conducted to identify studies published from 2006 to 2020 that measured and reported the indirect costs borne by cancer patients and their caregivers in Canada, from the patient, caregiver, employer, and societal perspectives. Study characteristics and cost estimation methods were extracted from relevant studies. Costs estimates were reported and converted to 2020 *** for the following categories lost earnings, caregiving time costs, home production losses, patient time (leisure), morbidity-, disability-, premature mortality-related costs, friction costs, and overall productivity losses.

Specialised exercise has proven beneficial for cancer patients who are therefore offered a supervised six-week exercise programme at hospitals in Denmark. This quality assurance study investigated whether the distance between home and hospital and other demographic variabilities affected patients' participation in and completion of the programme.
Data were collected on cancer patients' participation in a specialised exercise programme at the Department of Oncology, Vejle Hospital, Denmark, between January 2014 and June 2017. Data covering the same period were collected on all patients referred to the department. Demographics and information on cancer diagnosis were retrieved from the patient administrative systems. The distance by car from patients' residential postcode to hospital was measured using a map service. The potential relationship between distance to hospital and participation was investigated by univariate and multivariate regression analyses.
Travel distance had a significant impact (p less than 0.001) on participation. Similarly, gender (p = 0.001), intention of treatment (p = 0.001) and type of cancer (p = 0.001) were of significance to completion of the programme.
Travel distance tended to compromise participation, but for patients who chose to participate, travel distance did not affect completion of the programme.

Pandemics are known to cause stress and anxiety in pregnant women. During the coronavirus disease 2019 (COVID-19) lockdown of the Danish society, pregnant women were considered to be at increased risk, and access to antenatal care changed.
On 8 April 2020A, a questionnaire was sent to 332 pregnant women previously sampled by general practitioners in two Danish regions. The women were contacted via secured e-mail (e-Boks), and questionnaires were returned until 6 May.
The questionnaire was returned by 257 women (77%). More than half believed that they were at a high risk of infection with COVID-19, and a third of the women were concerned about the risk of serious disease - especially for their unborn child. Almost 90% isolated at home most of the time. The majority were worried about possible consequences of the pandemic for antenatal care, but very few had actually missed a scheduled preventive consultation with their general practitioner, and only 15% had missed an appointment with their midwife. The majority of the women preferred normal consultations and found no added safety in shifting the consultation from the normal clinical setting.
The COVID-19 pandemic and lockdown have had a major impact on Danish pregnant women. The extended physiotherapy examination scheme (EES) was developed to assist general practitioners in their assessment of patients with complex low-**** pain (LBP). The aim was to compare LBP patients in the EES and patients in current physiotherapy practice (CP) with respect to characteristics, healthcare utilisation and sick leave.
Data from three cohorts were included, one covering the EES and two covering CP for LBP patients in Danish primary care physiotherapy. Baseline questionnaire data were collected, and 78-week follow-up was conducted in Danish registries. Patients were stratified according to their risk profile by the STarT **** Tool, and regression analyses were used to analyse healthcare utilisation and sick leave.
A total of 746 patients in the EES and 361 in CP were included. Patients in the EES had sick leave more often and were characterised by a longer pain duration and more disability than CP patients. The probability of contact to secondary care was doubled among patients in the EES with a high-risk profile and patients in the EES showed a trend towards a higher degree of sick leave.
LBP patients in the EES are more burdened than patients in CP. Consequently, for some subgroups in the EES, a higher referral rate and more sick leave were detected. These findings may be the result of vague referral criteria, lack of appropriate screening tools or inadequate monitoring of the EES.

The municipality of Copenhagen offers general health assessment (GHA) to all newly resettled refugees, conducted at the Section of Immigrant Medicine, Hvidovre Hospital. This study described their disease burden and sociodemographic characteristics.
In this cross-sectional study, all adult individuals assessed from 1 January 2017 to 30 January 2019 were included. Doctors performed the GHA, including a structured questionnaire, clinical examination and blood testing.
In total, 160 refugees were included. Few suffered from communicable diseases (e.g., 1% hepatitis B virus) or other somatic diseases (4% diabetes Type 2). However, deficiencies such as vitamin D deficiency (76%), vitamin B12 deficiency (31%) and anaemia (12%) were frequent. The majority reported headache (54%) or other pain (53%). Furthermore, signs of post-traumatic stress disorder were frequent (33%) and significantly associated with experience of torture, prison and persecution.
The population presented with pertinent health issues such as vitamin deficiencies, mental health issues and symptoms of pain. Few suffered from non-communicable or communicable diseases. Our results suggest that an offer of specialised services at municipality level for all newly resettled refugees may be beneficial. Furthermore, the study underlines the need for more research within the field of refugee health.

In a multiple-tier prehospital emergency system, knowing the response time of supplemental prehospital units may aid the ambulance personnel in deciding whether to remain at the scene and initiate treatment or to load the patient and head towards the hospital. We sought to correlate the actual to the predicted response time indicated at the GPS display in the vehicles of the supplemental prehospital resources.
From December 2016 to February 2017, all emergency runs with lights and sirens performed by the mobile emergency care units in Odense were registered. For each emergency run, the physician registered the actual response time, the distance to the incident when travelling along the route suggested by the GPS and the predicted time to arrival. These registrations of time variables served as the basis for a linear regression analysis. A correlation between estimated and actual response time was calculated.
A total of 617 runs were registered. In all, 189 runs were excluded. Thus, a total of 428 runs were included.

The current dietary recommendation for protein intake in the UK is 0.75 g/kg/day, however, this population-wide recommendation does not necessarily reflect altered requirements for older adults to maintain muscle protein synthesis, nor does it encompass the potential impact of intake timing. Optimal muscle protein synthesis in older adults requires both higher intake requirements and a distribution of protein intake above a 25 g threshold, three times across the day. This study aimed to describe the protein intake of older adults in a UK region and compare the results to recommendations. The study re-assessed two existing datasets with rich diet information for older adults in the South Yorkshire area. Data were extracted from food diaries of 256 adults aged between 65 and 89 years old (mean ± SD 72.4 ± 5.3 years). Fewer than 50% of the participants met current UK recommendations (0.75 g/kg/day) and fewer than 15% met the ESPEN 1.2 g/kg/day age-specific recommendation. The following welding parameters have been used 500 rpm tool rotation speed, 150 mm/min tool traverse speed, tool tilt angle 2°, pressure force 17 kN. The welded material was investigated in three different configurations HT0, HT1, and HT2. The first type of weld (HT-0) was made using AA2519 alloy in non-precipitation hardened state and examined in such condition. The second type of weld (HT-1) has been performed on AA2519-T62, that corresponds to precipitation hardening condition. The last type of weld (HT2) was performed on annealed AA2519 and the obtained welds were subjected to the post-weld precipitation hardening process. The heat treatment was carried out in two stages solution heat treatment (530°C/2h + cooling in cold water) and aging (165oC/10h). Residual stresses were measured using X-Ray diffraction patterns obtained from Bruker D8 Discover X-Ray diffractometer utilizing the concepts of Euler cradle and polycapillary primary beam optics. The current dietary recommendation for protein intake in the UK is 0.75 g/kg/day, however, this population-wide recommendation does not necessarily reflect altered requirements for older adults to maintain muscle protein synthesis, nor does it encompass the potential impact of intake timing. Optimal muscle protein synthesis in older adults requires both higher intake requirements and a distribution of protein intake above a 25 g threshold, three times across the day. This study aimed to describe the protein intake of older adults in a UK region and compare the results to recommendations. The study re-assessed two existing datasets with rich diet information for older adults in the South Yorkshire area. Data were extracted from food diaries of 256 adults aged between 65 and 89 years old (mean ± SD 72.4 ± 5.3 years). The welded material was investigated in three different configurations HT0, HT1, and HT2. The first type of weld (HT-0) was made using AA2519 alloy in non-precipitation hardened state and examined in such condition. The second type of weld (HT-1) has been performed on AA2519-T62, that corresponds to precipitation hardening condition. The last type of weld (HT2) was performed on annealed AA2519 and the obtained welds were subjected to the post-weld precipitation hardening process. The heat treatment was carried out in two stages solution heat treatment (530°C/2h + cooling in cold water) and aging (165oC/10h). Residual stresses were measured using X-Ray diffraction patterns obtained from Bruker D8 Discover X-Ray diffractometer utilizing the concepts of Euler cradle and polycapillary primary beam optics. MicroRNAs (miRNAs) are small RNA molecules with important gene regulatory roles in normal and pathophysiological cellular processes. Burkitt lymphoma (BL) is an ****driven lymphoma of germinal center B (GC-B) cell origin. To gain further knowledge on the role of miRNAs in the pathogenesis of BL, we performed small RNA sequencing in BL cell lines and normal GC-B cells. This revealed 26 miRNAs with significantly different expression levels. For five miRNAs, the differential expression pattern was confirmed in primary BL tissues compared to GC-B cells. MiR-378a-3p was upregulated in BL, and its inhibition reduced the growth of multiple BL cell lines. RNA immunoprecipitation of Argonaute 2 followed by microarray analysis (Ago2-RIP-Chip) upon inhibition and ectopic overexpression of miR-378a-3p revealed 63 and 20 putative miR-378a-3p targets, respectively. Effective targeting by miR-378a-3p was confirmed by luciferase reporter assays for MAX Network Transcriptional Repressor (MNT), Forkhead Box P1 (FOXP1), Interleukin 1 Receptor Associated Kinase 4 (IRAK4), and lncRNA Just Proximal To XIST (JPX), and by Western blot for IRAK4 and MNT. Overexpression of IRAK4 and MNT phenocopied the effect of miR-378a-3p inhibition. In summary, we identified miR-378a-3p as a miRNA with an oncogenic role in BL and identified IRAK4 and MNT as miR-378a-3p target genes that are involved in its growth regulatory role.Lack of sensitive diagnostic tests impairs the understanding of the epidemiology of histoplasmosis, a disease whose burden is estimated to be largely underrated. Broad-range PCRs have been applied to identify fungal agents from pathology blocks, but sensitivity is variable. In this study, we compared the results of a specific Histoplasma qPCR (H. qPCR) with the results of a broad-range qPCR (28S qPCR) on formalin-fixed, paraffin-embedded (FFPE) tissue specimens from patients with proven fungal infections (n = 67), histologically suggestive of histoplasmosis (n = 36) and other mycoses (n = 31). The clinical sensitivity for histoplasmosis of the H. qPCR and the 28S qPCR was 94% and 48.5%, respectively. Samples suggestive for other fungal infections were negative with the H. qPCR. The 28S qPCR did not amplify DNA of Histoplasma in FFPE in these samples, but could amplify DNA of Emergomyces (n = 1) and Paracoccidioides (n = 2) in three samples suggestive for histoplasmosis but negative in the H. qPCR. In conclusion, amplification of Histoplasma DNA from FFPE samples is more sensitive with the H. qPCR than with the 28S qPCR. The test results are subsequently analyzed using a detection algorithm to detect the jaw activity based on the captured audio signals and to classify them into four main categories, namely chewing, swallowing, coughing and talking. The mean power associated with each category of activity is then calculated by using the pressure signals as measured by a water-inflated earplug subjected to earcanal dynamic movement. The results show that 3.8 mW of power, achieved mainly by the chewing movement, is readily available on average from within the earcanal.Cataracts account for over half of global blindness. Cross-treatment experiments using one type of AS to treat the other two kinds of wastewater were conducted. The results of the cross-treatment experiments and qPCR both suggest the functional adaptation of the microbial community. We revealed selection strategies for the adaptation of bacteria and archaea in AS during environmental changes, providing a theoretical basis for petrochemical wastewater treatment.The mechanism of surfactants in surfactant-in situ chemical oxidation (S-ISCO) coupled process for trichloroethene (TCE) degradation was firstly reported. The performance of TCE solubilization and inhibition of TCE degradation in three nonionic surfactants (TW-80, Brij-35, TX-100) in PS/Fe(II)/citric acid (CA) system was compared and TW-80 was evaluated to be the optimal surfactant in S-ISCO coupled process due to the best TCE solubilizing ability and minimal inhibition for TCE degradation (only 31.8% TCE inhibition in the presence of 1 g L-1 TW-80 surfactant). The inhibition mechanism in TCE degradation was also demonstrated by comparing the strength of ROSs and PS utilization. The results showed that the composition of RAS was predominantly organic acids, ketones, polypeptides, esters, phenols, amino acids and intermediate metabolic products. The safety concentrations (SCs) of FAS to Carassius auratus, Ctenopharyngodon idellus and Hypophthalmichthys molitrix were 1.92 × 1010 cells/L, 1.58 × 1011 cells/L and 1.30 × 1011 cells/L, respectively. The SCs of the RAS were significantly lower than those of the FAS (p less then 0.05), with the values of 1.25 × 109 cells/L, 8.8 × 109 cells/L and 9.7 × 109 cells/L, for each species, respectively. This work was set out to evaluate the half-life (t1/2) of direct photodegradation of 36 micropollutants under controlled conditions of light exposure close to solar radiation. The results allowed to classify the micropollutants into three groups (fast, medium and slow). Seven micropollutants were classified in the fast group with t1/2 between 0.05 h and 0.79 h, 24 in the medium group with t1/2 between 5.3 h and 49.7 h, and five in the slow group with t1/2 between 56 h and 118 h. The t1/2 values obtained in laboratory were compared with those from a CW receiving treated wastewater. Correction factors were calculated to adjust the in situ data for the light intensity in laboratory and improved the correspondence especially for the micropollutants of the fast and medium groups. Cross-treatment experiments using one type of AS to treat the other two kinds of wastewater were conducted. The performance of TCE solubilization and inhibition of TCE degradation in three nonionic surfactants (TW-80, Brij-35, TX-100) in PS/Fe(II)/citric acid (CA) system was compared and TW-80 was evaluated to be the optimal surfactant in S-ISCO coupled process due to the best TCE solubilizing ability and minimal inhibition for TCE degradation (only 31.8% TCE inhibition in the presence of 1 g L-1 TW-80 surfactant). The inhibition mechanism in TCE degradation was also demonstrated by comparing the strength of ROSs and PS utilization. The results showed that the composition of RAS was predominantly organic acids, ketones, polypeptides, esters, phenols, amino acids and intermediate metabolic products. The safety concentrations (SCs) of FAS to Carassius auratus, Ctenopharyngodon idellus and Hypophthalmichthys molitrix were 1.92 × 1010 cells/L, 1.58 × 1011 cells/L and 1.30 × 1011 cells/L, respectively. The SCs of the RAS were significantly lower than those of the FAS (p less then 0.05), with the values of 1.25 × 109 cells/L, 8.8 × 109 cells/L and 9.7 × 109 cells/L, for each species, respectively. This work was set out to evaluate the half-life (t1/2) of direct photodegradation of 36 micropollutants under controlled conditions of light exposure close to solar radiation. The results allowed to classify the micropollutants into three groups (fast, medium and slow). Seven micropollutants were classified in the fast group with t1/2 between 0.05 h and 0.79 h, 24 in the medium group with t1/2 between 5.3 h and 49.7 h, and five in the slow group with t1/2 between 56 h and 118 h. The t1/2 values obtained in laboratory were compared with those from a CW receiving treated wastewater. Correction factors were calculated to adjust the in situ data for the light intensity in laboratory and improved the correspondence especially for the micropollutants of the fast and medium groups. Adrenal angiosarcoma is an extremely rare malignancy with few reported cases in the literature. Patients may be asymptomatic or have nonspecific complaints at presentation. There are no pathognomonic imaging findings, while histopathology can be confounding due to overlap with other disease processes. We present a case of a 38-year-old woman with a long history of cocaine abuse who had metastatic adrenal angiosarcoma at the time of presentation. The adrenal tumor was an incidental finding on imaging. CT demonstrated a heterogeneous mass in the right adrenal gland with central calcification, and MRI identified central necrosis in the mass. Histopathology demonstrated sheets of epithelioid cells, dilated anastomotic vascular spaces, and abundant necrosis, and immunohistochemistry was positive for various vascular markers. The findings were consistent with adrenal angiosarcoma. The patient underwent adrenalectomy and is now receiving adjuvant chemotherapy. Due to the aggressive nature of adrenal angiosarcoma, timely diagnosis and treatment is critical. This case adds to the sparse literature surrounding this disease by highlighting crucial imaging and histopathologic findings that will aid in more efficient diagnosis. Although rare, the disease should be considered in the context of suspicious adrenal lesions. In the future, structured review of all reported cases of adrenal angiosarcoma can help inform diagnosis and therapy for this rare disease. Given the role of the APC protein in development of the central nervous system, we hypothesized that patients with FAP would show differences in cognitive functioning compared to controls. Methods Matched case-control study designed to evaluate cognitive function using the Test of Nonverbal Intelligence-4, the Bateria III Woodcock-Munoz, and the Behavior Rating Inventory of Executive Functions-Adult. Twenty-six individuals with FAP (mean age = 34.2 ± 15.0 years) and 25 age-gender and educational level matched controls (mean age = 32.7 ± 13.8 years) were evaluated. Results FAP-cases had significantly lower IQ (p = 0.005). Across all tasks of the Batería III Woodcock-Muñoz, FAP-cases performed significantly lower than controls, with all of the summary scores falling in the bottom quartile compared to controls (p less then 0.0001). Patients with FAP scored within the deficient range for Long-Term Retrieval and Cognitive Fluency. Conclusion APC protein has an important role in neurocognitive function. The pervasive nature of the observed cognitive dysfunction suggests that loss or dysfunction of the APC protein impacts processes in cortical and subcortical brain regions. In vitro, randomized control trials reporting flexural strength of titanium dioxide nanoparticle reinforced resin were selected. The review was formulated based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses- Protocol (PRISMA-P) guidelines. Quality assessment was performed according to Consolidated Standards Of Reporting Trials (CONSORT) guidelines and risk of bias Cochrane tool. Six articles in the category of 1%, 3%, 5% weight fractions of titanium dioxide nanoparticles were subjected to a meta-analysis. A meta-analysis was performed using random-effects at a 95% confidence interval. Within the limitations of the study, it can be assumed that there is no precise conformity on the ideal titanium dioxide nanoparticle concentration required to improve the flexural strength of the polymer. Stringent use of standard ISO guidelines may help in obtaining consistent results.

Kawasaki disease (KD) was first reported by Dr. Tomisaku Kawasaki in 1967 and is classified as an autoimmune vasculitis of small- and medium-sized arteries. It is usually a self-limited condition occurring in childhood, but it can have complications such as coronary artery aneurysms, myocardial ischemia, and arrhythmias with significant morbidity and mortality presenting later in life. We report a case of a KD presenting in adulthood with late cardiovascular sequelae managed with coronary artery bypass graft.Objectives Hypertension is strongly related to body mass index (BMI). Obesity has been the single main contributor to hypertension. Furthermore, the clinical manifestations are normally associated with BMI in hypertensive patients. This study aimed to evaluate the relationship between clinical features and BMI among hypertensive patients in both males and females. Methodology A retrospective cross-sectional study was conducted among 296 patients having a self-reported history of hypertension and on anti-hypertensive medication. The study was conducted in the medical outpatient department of a secondary care hospital in Karachi during six months (January 2020 to June 2020). A detailed history was taken from each patient about hypertension-related symptoms, and clinical examination was performed. Blood pressure was measured using a sphygmomanometer with a stethoscope. Results Of the 296 patients, 156 (52.2%) were males and 140 (47.3%) were females; 16 (5.4%) of them were underweight, 91 (30.7%) were normal weight, 129 (43.6%) were overweight, and 60 (20.3%) were obese; in addition, 106 (35.8%) reported edema and 71 (24.0%) reported nausea and so on. As far as the association of clinical features and BMI was concerned, our study results showed that only edema (p=0.017) and nausea (p=0.044) were significantly associated with the BMI of the patients. Patients with edema were more likely to be obese than those without edema (29.2% vs. 15.3%), whereas patients with nausea were more likely to be overweight than those without nausea (57.7% vs. 39.1%). Conclusions Our study showed that among the clinical features, edema and nausea were significantly associated with the BMI of the patients, whereas the relationship with others was insignificantly related to BMI of the patients in both male and female patients.Spinal arteriovenous malformations (AVMs) are a rare form of spinal blood vessel defect that results in vessel engorgement leading to clinical signs secondary to mass effect and ischemia. We present the patient's clinical course following suspicion of spinal AVM along with a review of current classification and imaging modalities.Depressive disorder and neurodegenerative diseases are two different clinical entities. Depression is a common psychiatric disorder in the general population. However, when present concomitantly with neurodegenerative disorders, its diagnosis becomes challenging. In many cases, patients remain undiagnosed and hence, untreated, worsening the prognosis of the neurodegenerative diseases and impairing the quality of life. One of the possible reasons for the difficulties in diagnosis in such cases is that both conditions affect the central nervous system, so there might be an overlap of symptoms leading to a missed diagnosis of depression in a neurodegenerative disease patient and vice versa. Symptoms such as irritability, apathy, and decreased cognition are common to both types of disorders. Some neurodegenerative diseases, especially Alzheimer's disease, can initially present as a depressive prodrome. This may cause a difficulty in differentiating between these two conditions and a diagnosis of either conditions may be missed; hence an opportunity for timely intervention and improved outcomes is missed. An approach towards analyzing and comparing the pathological mechanisms common to both disease types will create a better understanding of depression and neurodegenerative diseases, identify their similarities, and develop improved clinical criteria to help clinicians make a timely diagnosis of these conditions present together. We present a 66-year-old female with history of thrombocytopenia and nonalcoholic cirrhosis who presented with severe headache. Head CT demonstrated a right temporal lobe intraparenchymal hemorrhage with sylvian fissure subarachnoid hemorrhage. Cerebral angiogram showed a 1.5mm x 1.5mm right middle cerebral artery (MCA) bifurcation aneurysm. The patient underwent Y-stent coiling from the right M1 into the right M2 superior division and the right M1 into the right M2 inferior division, with a 1mm x 1cm coil. Given the patient's thrombocytopenia, only aspirin monotherapy was administered peri-procedural. Shortly thereafter, the patient developed left hemiparesis. Computed tomography angiogram (CTA) demonstrated thrombus within the stent. Thrombectomy was performed with thrombolysis in cerebral infarction (TICI) 3 revascularization and improvement to neurologic baseline. However, that evening she became acutely hypotensive, unresponsive, and ultimately expired due to hemorrhagic cause. Antiaggregate therapy among neuroendovascular procedures is debated with no clear standard of care. This case highlights the difficult decisions that must be made to balance the risks associated with the use of antiplatelets with ruptured aneurysms.A 32-year-old man presented with profound jaundice, rigors and decreased appetite. It has been observed that LFTs typically improve upon ketamine cessation, but this case demonstrated escalating hyperbilirubinaemia, despite ketamine cessation. Recurrent cholangitis and biliary duct stricturing were demonstrated on magnetic resonance cholangiopancreatography (****). Conclusions Our study showed that among the clinical features, edema and nausea were significantly associated with the BMI of the patients, whereas the relationship with others was insignificantly related to BMI of the patients in both male and female patients.Spinal arteriovenous malformations (AVMs) are a rare form of spinal blood vessel defect that results in vessel engorgement leading to clinical signs secondary to mass effect and ischemia. We present the patient's clinical course following suspicion of spinal AVM along with a review of current classification and imaging modalities.Depressive disorder and neurodegenerative diseases are two different clinical entities. Depression is a common psychiatric disorder in the general population. However, when present concomitantly with neurodegenerative disorders, its diagnosis becomes challenging. In many cases, patients remain undiagnosed and hence, untreated, worsening the prognosis of the neurodegenerative diseases and impairing the quality of life. One of the possible reasons for the difficulties in diagnosis in such cases is that both conditions affect the central nervous system, so there might be an overlap of symptoms leading to a missed diagnosis of depression in a neurodegenerative disease patient and vice versa. Symptoms such as irritability, apathy, and decreased cognition are common to both types of disorders. Some neurodegenerative diseases, especially Alzheimer's disease, can initially present as a depressive prodrome. This may cause a difficulty in differentiating between these two conditions and a diagnosis of either conditions may be missed; hence an opportunity for timely intervention and improved outcomes is missed. An approach towards analyzing and comparing the pathological mechanisms common to both disease types will create a better understanding of depression and neurodegenerative diseases, identify their similarities, and develop improved clinical criteria to help clinicians make a timely diagnosis of these conditions present together. Head CT demonstrated a right temporal lobe intraparenchymal hemorrhage with sylvian fissure subarachnoid hemorrhage. Cerebral angiogram showed a 1.5mm x 1.5mm right middle cerebral artery (MCA) bifurcation aneurysm. The patient underwent Y-stent coiling from the right M1 into the right M2 superior division and the right M1 into the right M2 inferior division, with a 1mm x 1cm coil. Given the patient's thrombocytopenia, only aspirin monotherapy was administered peri-procedural. Shortly thereafter, the patient developed left hemiparesis. Computed tomography angiogram (CTA) demonstrated thrombus within the stent. Thrombectomy was performed with thrombolysis in cerebral infarction (TICI) 3 revascularization and improvement to neurologic baseline. In the ESGO/ESTRO/ESP guidelines for endometrial carcinoma management, the risk category of clear cell carcinoma (CCC) is not well defined. In fact, while p53-abnormal (p53abn) CCC are known to be aggressive, the prognosis of mismatch repair-deficient (MMRd) and p53-wild-type (p53wt) CCCs is less clear.
To assess the prognostic value of the MMRd and p53wt groups in CCC through a systematic review and meta-analysis.
Electronic databases were searched from their inception to February 2021. All studies reporting p53 expression, MMR proteins expression and survival outcomes in endometrial CCC (either pure or mixed) were included. Kaplan-Meier and Cox regression survival analyses with hazard ratio (HR) for overall survival (OS) were performed by using the p53abn group as reference; a significant p-value<0.05 was adopted.
Six studies with 136 CCC (114 pure and 22 mixed) were included. Five-year OS was 95.7 ± 4.3% in the MMRd group, 48.4 ± 8.4% months in the p53wt group and 40.6 ± 10.4% in the p53abn group. The hazard of death was significantly lower in the MMRd group than in the p53abn group (HR = 0.062; p = 0.007), while it did not significantly differ between the p53wt and the p53abn group (HR = 0.673; p = 0.222). The POLEmut group could not be analyzed due to the absence of deaths. Similar results were observed in the pure CCC and mixed CCC subgroups.

Assessing the association between endometriosis and/or adenomyosis and ovarian cancer.
We identified all women with histological proven endometriosis (51,544 women) and/or adenomyosis (85,015 women) from the Dutch pathology database (1990-2015) and matched with women with a benign dermal nevus (132,654 women). Histology results for ovarian cancer were retrieved. We estimated crude and age-adjusted incidence rate ratios (IRR) for ovarian cancer.
We found 1017 (2.0%), 1284 (1.5%) and 471 (0.4%) ovarian cancer cases in the endometriosis, adenomyosis and nevus cohort, respectively. The age-adjusted IRRs were 19.75 (95% CI 16.70-23.35) in the endometriosis cohort and 5.93 (95% CI 4.91-7.16) in the adenomyosis cohort. The highest IRRs were found for endometrioid and clear cell ovarian cancer subtypes. Excluding the first year of follow-up did not result in a significant IRR for ovarian cancer overall but resulted in a statistically significant age-adjusted IRR of 3.92 (95% CI 2.19-7.01) for clear cell ovarian cancer in the endometriosis cohort. This study shows that gynecologist should also be aware of an increased ovarian cancer incidence in women with adenomyosis.
To compare the outcomes of patients with low-risk gestational trophoblastic neoplasia (GTN) treated with 8-day methotrexate (MTX) with two different regimens of folinic acid (FA).
Retrospective cohort study of low-risk GTN followed at Rio de Janeiro Federal University, from January/2000-December/2019 with 8-day MTX with FA at 0.1 mg/kg versus 15 mg fixed dose.
Among 667 patients with low-risk GTN, 323 were treated with FA at 0.1 mg/kg and 142 with FA at 15 mg fixed dose. Administrative data from the National Child Abuse and Neglect Data System Child File for 2406 counties were merged with measures of intergenerational income mobility from Chetty et al. (2014a), including the probability that a child born in the bottom quintile of the national income distribution reaches the top quintile by age thirty.
Weighted least squares analyses were used to empirically estimate the relationship between intergenerational income mobility and child maltreatment report rates. Maltreatment reports were also divided into subgroups by age and metropolitan status.
Counties where children have a greater chance of moving up the income ladder have lower child maltreatment report rates, independent from income inequality and poverty rates.

DESIGN This cohort study included adults without diabetes at baseline, who were followed-up every two years for incident diabetes. Serum ANGPTL6 concentrations were measured at baseline and during oral glucose tolerance tests (OGTTs). A hepatic cell line, HepG2, and diet-induced obesity mouse model were used to evaluate the response of ANGPTL6 expression and secretion to hyperglycemia and the metabolic syndrome. RESULTS We recruited 1103 subjects without diabetes at baseline. During the 4.22-year follow-up, 113 subjects (10.2%) developed incident diabetes. Serum ANGPTL6 was negatively associated with the incidence of diabetes (adjusted HR=0.77, p=0.042). However, serum ANGPTL6 level was higher in subjects with pre-diabetes (p=0.018) and was elevated during OGTT. In HepG2 cells, treatment with glucose, but not insulin, induced ANGPTL6 expression. Hepatic ANGPTL6 expression and serum ANGPTL6 concentrations were significantly higher in **** fed with high-fat diet than in those fed with standard chow (both p less then 0.05). Even the most consistent finding, altered white-matter microstructure in the anterior limb of the internal capsule, is based on a handful studies. The urge for replicable targets to understand the underlying mechanisms of insomnia made us study white-matter fractional anisotropy (FA) across three samples of cases and controls. METHODS 3-Tesla MRI diffusion tensor imaging data of three independent samples were combined for analysis, resulting in n=137 participants, of whom 73 were diagnosed with insomnia disorder and 64 were matched controls without sleep complaints. Insomnia severity was measured with the Insomnia Severity Index (ISI). White matter microstructure was assessed with FA. White matter tracts were skeletonized and analyzed using Tract-Based Spatial Statistics. We performed a region-of-interest analysis using linear mixed-effect models to evaluate case-control differences in internal capsule FA as well as associations between internal capsule FA and insomnia severity. RESULTS FA in the right limb of the anterior internal capsule was lower in insomnia disorder than in controls (β=-9.76e-3; SE=4.17e-3, P=.034). In the entire sample, a higher Insomnia Severity Index score was associated with a lower FA value of the right internal capsule (β=-8.05e-4 FA/ISI point, SE=2.60e-4, P=.008). Ancillary whole brain voxel-wise analyses showed no significant group difference or association with insomnia severity after correction for multiple comparisons. CONCLUSIONS The internal capsule shows small but consistent insomnia-related alterations. Since then the main role of SMU was to continuously maintain, develop and refurbish the standard with related accessory to ensure international equivalence. Since 2002, a re-evaluation of the active volume of the ionizing chamber, correction factors, the key data, due to adoption of ICRU Report 90(1), and changes of the calibrations methods and corrections of the equipment were made to obtain improved parameters. The range was extended for 137Cs gamma ray beams and key comparisons with the Bureau International des Poids et Mesures (BIPM) were performed. Further investigation of the parameters for selecting which patients will benefit most from DIBH is essential. We performed dosimetric comparisons for 21 patients with left-sided ** who had both computed tomography (CT)-based free-breathing (FB) and DIBH plans. The doses to the heart and left anterior descending artery (LAD) and any reduction due to the DIBH technique were analysed. Based on CTFB plans, dosimetric analysis revealed that the irradiation doses to the heart and LAD were significantly correlated with the target volume, the ipsilateral lung volume (ILV) and the total lung volume (TLV). When patients had an ILV ≥ 950 cm3 or a TLV ≥ 2200 cm3, the irradiation doses to the heart and LAD were significantly decreased. Furthermore, the reduction in the mean heart dose (MHD) was correlated to the difference in lung volume between FB and DIBH. The difference in ILV between DIBH and FB of 1.8 indicated that the patients obtained more benefit from the DIBH technique. The data suggest that lung volume (ILV and TLV) measured on a CT-simulation scan and the difference between FB and DIBH could be utilized to help select patients for DIBH. © The Author(s) 2020. In the entire sample, a higher Insomnia Severity Index score was associated with a lower FA value of the right internal capsule (β=-8.05e-4 FA/ISI point, SE=2.60e-4, P=.008). Ancillary whole brain voxel-wise analyses showed no significant group difference or association with insomnia severity after correction for multiple comparisons. CONCLUSIONS The internal capsule shows small but consistent insomnia-related alterations. The findings support a circuit-based approach to underlying mechanisms since this tract connects many brain areas previously implicated in insomnia. © Sleep Research Society 2020. Published by Oxford University Press [on behalf of the Sleep Research Society].The primary standard for air kerma of the Slovak Institute of Metrology (SMU) as a part of national standard of dosimetric quantities of gamma radiation NE028/02, based on cylindrical graphite walled cavity ionization chamber was established in 2002 for the 60Co gamma ray beams. Since then the main role of SMU was to continuously maintain, develop and refurbish the standard with related accessory to ensure international equivalence. Since 2002, a re-evaluation of the active volume of the ionizing chamber, correction factors, the key data, due to adoption of ICRU Report 90(1), and changes of the calibrations methods and corrections of the equipment were made to obtain improved parameters. The range was extended for 137Cs gamma ray beams and key comparisons with the Bureau International des Poids et Mesures (BIPM) were performed. For Permissions, please email journals.permissions@oup.com.The deep inspiration breath-hold (DIBH) technique has been utilized to reduce the cardiac dose in left-sided breast cancer (BC) patients undergoing radiotherapy. Further investigation of the parameters for selecting which patients will benefit most from DIBH is essential. We performed dosimetric comparisons for 21 patients with left-sided BC who had both computed tomography (CT)-based free-breathing (FB) and DIBH plans. The doses to the heart and left anterior descending artery (LAD) and any reduction due to the DIBH technique were analysed. Based on CTFB plans, dosimetric analysis revealed that the irradiation doses to the heart and LAD were significantly correlated with the target volume, the ipsilateral lung volume (ILV) and the total lung volume (TLV). When patients had an ILV ≥ 950 cm3 or a TLV ≥ 2200 cm3, the irradiation doses to the heart and LAD were significantly decreased. Furthermore, the reduction in the mean heart dose (MHD) was correlated to the difference in lung volume between FB and DIBH. The difference in ILV between DIBH and FB of 1.8 indicated that the patients obtained more benefit from the DIBH technique. The data suggest that lung volume (ILV and TLV) measured on a CT-simulation scan and the difference between FB and DIBH could be utilized to help select patients for DIBH. © The Author(s) 2020.
