An optical signal is temporally encoded using gold coded sequences and transmitted through a turbid medium. The encoded signals are captured as a sequence of elemental images by two orthogonal polarized image sensor arrays. Polarimetric and polarization difference imaging are used to suppress the partially polarized and unpolarized background noise such that only the polarized ballistic signal photons are captured at the sensor. Multidimensional integral imaging is used to obtain 4D reconstructed data, and multidimensional nonlinear correlation is performed on the reconstructed data to detect the optical signal. We compare the effectiveness of the proposed polarimetric underwater optical signal detection approach to conventional (non-polarimetric) integral imaging-based and 2D imaging-based signal detection systems. The underwater signal detection capabilities are measured through performance metrics such as receiver operating characteristic (ROC) curves, the area under the curve (AUC), and the number of detection errors. Furthermore, statistical measures, including the Kullback-Leibler divergence, signal-to-noise ratio (SNR), and peak-to-correlation energy (PCE), are also calculated to show the improved performance of the proposed system. Our experimental results show that the proposed polarimetric integral-imaging approach significantly outperforms the conventional imaging-based methods. Here, we overcome this limitation by using radio-frequency interferometry to enhance the Brillouin-induced delay applied to the optical sidebands that carry RF signals, while controlling the phase of the optical carrier with integrated silicon nitride microring resonators. We report a delay tunability over 600 ps exploiting an enhancement factor of 30, over a bandwidth of 1 GHz using less than 1 dB of Brillouin gain utilizing a photonic chip architecture based on Brillouin scattering and microring resonators.We demonstrate an on-chip coherent mode scrambling demultiplexer for polarization multiplexed few mode signals. The device has been fabricated in the standard silicon-on-insulator platform. The mode demultiplexer consists of an array of 2D grating couplers for dual polarization few mode fiber-to-chip coupling and optical hybrids realized by 4×4 MMIs. The array of perfect vertical 2D grating couplers allows us an efficient fiber-to-chip coupling with experimental peak coupling efficiencies of -5.2 dB and -9.0 dB at 1570 nm for LP01 and LP11 modes, respectively, while simulated coupling efficiencies at 1550 nm are -3.6 dB and -3.3 dB for LP01 and LP11, respectively. We successfully performed a ****-to-**** three LP modes division multiplexing transmission experiment with single polarization 32 Gbaud QPSK signals using the fabricated mode demultiplexer relying on offline MIMO DSP techniques.

Taste-assessment study using the electronic-tongue quantified the bitterness threshold of the drug and its aversiveness concentration (2.79 mM). Therefore, a taste-masking strategy would be useful when further formulating PSP as an OD formulation.This study was conducted to investigate the deposition of several mineral elements and the mRNA levels of mineral-related genes across different tissues of cherry valley ducks. The contents of magnesium (Mg), potassium (K), zinc (Zn), and selenium (Se) in ducks' breast muscle, thigh muscle, liver, skin, and tibia at the age of 0, 21, 35, 49, and 63 days, respectively, were measured using an atomic fluorescence spectrophotometer, while the mRNA levels of mineral-related genes were detected by qRT-PCR. The results revealed that the dynamics of Mg and K were generally similar in each tissue, with a significant positive correlation (p less then 0.05). In the breast muscle, thigh muscle, and liver, the contents of almost all mineral elements reached their peak values (p less then 0.05) at the age of 49 to 63 days. Interestingly, the expression of most mineral-related genes was the highest at birth (p less then 0.05). In addition, there was a significant negative correlation between the expression of ATP1A1 and the deposition of K (r = -0.957, p less then 0.05), and a similar result was found for the expression of ATP8 and the deposition of Zn (r = -0.905, p less then 0.05). Taken together, Mg and K could be used as joint indicators for the precise breeding of the high-quality strain of cherry valley ducks, while the age of 49 to 63 days could be used as the reference for the best marketing age. In addition, ATP1A1 and ATP8 could be used as the key genes to detect K and Zn, respectively. Cold-active β-galactosidases are identified in microorganisms endemic to permanently low-temperature environments. While mesophilic β-galactosidases are broadly studied and employed for biotechnological purposes, the cold-active enzymes are still scarcely explored, although they may prove very useful in biotechnological processes at low temperature. This review covers several issues related to cold-active β-galactosidases, including their classification, structure and molecular mechanisms of cold adaptation. Moreover, their applications are discussed, focusing on the production of lactose-free dairy products as well as on the valorization of cheese whey and the synthesis of glycosyl building blocks for the food, cosmetic and pharmaceutical industries.The COVID-19 pandemic has led to a reduction in hip and knee replacement surgery across healthcare systems. When regular operating returns, there will be a large volume of patients and an emphasis on a short hospital stay. Patients will be keen to return home, and capacity will need to maximised. Strategies to reduce the associated risks of surgery and to accelerate recovery will be needed, and so Enhanced Recovery after Surgery (ERAS) should be promoted as the model of care. ERAS protocols are proven to reduce hospital stay safely; however, ERAS pathways may require adaption to ensure both patient and staff safety. The risk of exposure to possible sources of COVID-19 should be limited, and so hospital visits should be minimised. The use of technology such as smartphone apps to provide pre-operative education, wearable activity trackers to assist with rehabilitation, and the use of telemedicine to complete outpatient appointments may be utilised. Also, units should be reminded that ERAS protocols are multi-modal, and every component is vital to minimise the surgical stress response. The focus should be on providing better and not just faster care. Units should learn from the past in order to expedite the implementation of or adaption of existing ERAS protocols. Strong leadership will be required, along with a supportive organisational culture, an inter-professional approach, and a recognised QI method should be used to contextualize improvement efforts.Colistin is considered as a last resort agent for treatment of severe infections caused by carbapenem-resistant Enterobacterales (CRE). Recently, plasmid-mediated colistin resistance genes (mcr type) have been reported, mainly corresponding to mcr-1 producers. Those mcr-1-positive Enterobacterales have been identified not only from human isolates, but also from food samples, from animal specimens and from environmental samples in various parts of the world. The colistin resistance trait was confirmed for all those 20 isolates with a positivity of the Rapid Polymyxin NP (Nordmann-Poirel) test. Minimum inhibitory concentrations (****) of colistin for all MCR-1-producing isolates ranged between 4 and 16 μg/mL. Noticeably, 9 out of the 20 mcr-1-positive isolates produced an extended-spectrum β-lactamase (ESBL), respectively producing CTX-M-9 (n = 2), CTX-M-14 (n = 4), CTX-M-15 (n = 2), and SHV-12 (n = 1). Noteworthy, the fosA4 gene encoding resistance to fosfomycin was found in a single mcr-1-positive E. coli isolate, in which both genes were located on different conjugative plasmids. The pulsed-field gel electrophoresis (PFGE) patterns were identified, corresponding to 10 different sequence types (STs), highlighting the genetic diversity of those different E. coli. Whole-genome sequencing revealed three major types of mcr-1-bearing plasmids, corresponding to IncI2, IncX4, and IncHI2 scaffolds. Constipation is one of the most common complaints of the digestive system indicated with an increase in defecation frequency, difficulty in defecation, and hard and strained defecation. Environmental, personal, and genetic factors may be affecting constipation although the affecting factors have not yet been thoroughly explained. The aim of this study was to investigate constipation frequency and lifestyles in medical students.
The population was selected among medical students for the study, which was planned as a survey study. Demographic data of all the participants and the factors suggested to affect constipation were questioned and analyzed.
The study covered a total of 425 medical students. Among the students reporting constipation, 2.86% were in their first year of medical school, while 7.53% were in the third year and 9.09% were in the sixth year. The rate of students reporting constipation and familial history was statistically significant. While regular eating habits were reported in the first and third years, this rate was **** lower in the sixth year group working at clinical departments. The results of our study did not reveal any significant relationship between daily intake of water and constipation. There was, however, a significant relationship between stress and constipation.
The results of our study showed that medical education did not curb constipation frequency. We believe that stress is significant in constipation. The data we collected indicate that regular eating habits and excess liquid intake are not as effective as suggested in the treatment of constipation.

No consensus exists regarding which procedures should be performed to disinfect endoscopes and working platforms after COVID-19 patients have undergone endoscopy.

For endoscopic preprocessing at the bedside, a key disinfection step is using a multienzyme stock solution. The nucleic acid tests for endoscopists, washers, endoscopes, and working platforms were all negative. Based on our experience with the 11 COVID-19 patients who had undergone endoscopy, we provide an endoscopic reprocessing method for the bedside endoscopic diagnosis and treatment of COVID-19 patients for reference.

The accurate diagnosis of biliary strictures remains problematic. The aim of the present study was to prospectively evaluate the clinical utility of probe-based confocal laser endomicroscopy (pCLE) under the direct view of peroral cholangioscopy (POCS) for the diagnosis of biliary strictures.
Consecutive patients with biliary strictures were included. We investigated sensitivity, specificity, and accuracy to diagnose malignancy for (1) ERCP alone, (2) POCS, (3) pCLE under the direct view of POCS, and (4) tissue sampling under the direct view of POCS.
A total of 30 patients (17 with malignant lesions) were prospectively enrolled. (1) ERCP alone showed 88.2% sensitivity, 46.2% specificity, and 70% (95% confidence interval (CI), 52.1%-83.3%) accuracy. (2) POCS showed 100% sensitivity, 76.9% specificity, and 90% (95% CI, 74.4%-96.5%) accuracy. (3) pCLE under the direct view of POCS showed 94.1% sensitivity, 92.3% specificity, and 93.3% (95% CI, 78.7%-98.8%) accuracy. (4) Tissue sampling under the direct view of POCS showed 82.4% sensitivity, 100% specificity, and 90% (95% CI, 74.4%-96.5%) accuracy.

Environmental, personal, and genetic factors may be affecting constipation although the affecting factors have not yet been thoroughly explained. The aim of this study was to investigate constipation frequency and lifestyles in medical students. The population was selected among medical students for the study, which was planned as a survey study. Demographic data of all the participants and the factors suggested to affect constipation were questioned and analyzed. The study covered a total of 425 medical students. Among the students reporting constipation, 2.86% were in their first year of medical school, while 7.53% were in the third year and 9.09% were in the sixth year. The rate of students reporting constipation and familial history was statistically significant. While regular eating habits were reported in the first and third years, this rate was much lower in the sixth year group working at clinical departments. The results of our study did not reveal any significant relationship between daily intake of water and constipation. There was, however, a significant relationship between stress and constipation. The results of our study showed that medical education did not curb constipation frequency. We believe that stress is significant in constipation. Understanding of anatomical adaptations of various plant organs and tissues to heavy metals and metalloids could greatly contribute to integral and modern approach for investigation of plants in changing environmental conditions. These findings are necessary for understanding of the whole spectra of physiological and biochemical reactions in plants and to maintain the crop productivity worldwide. Moreover, our holistic perception regarding the processes underlying the plant responses to metal(loids) at anatomical level are needed for improving crop management and breeding techniques.Patients with LBBB of unknown onset presenting with chest pain can pose a diagnostic challenge in the ED while Smith-Modified-Sgarbossa (SMS) ECG criteria might facilitate AMI diagnosis. We demonstrate a case of a 79-year-old man that presented to the ED with chest pain. Caregivers play an important role in providing physical, emotional and social support to individuals with hip fracture. This study sought to explore perceptions of caregiving for older adults with hip fractures by comparing experiences of caregivers, providers and decision-makers.
Semi-structured interviews were conducted both in-person and by telephone with caregivers, providers and decision-makers (n=32) from one rural and one urban hospital selected from two health regions in Ontario, Canada. Interviews were audio-recorded and transcribed verbatim. The data were coded descriptively and interpretively, and compared within and across participants types.
Three main themes reflected tensions between caregivers and providers/decision-makers. Firstly, differing expectations of caregiver roles were identified. Several caregivers wanted more participation in the decision-making process but often felt excluded, while others expressed feeling stressed due to the expectations placed on them. Conversely, provide different providers could facilitate more positive caregiving experiences and interactions.Neuronal development, maintenance and function depends on the tight regulation of cytoskeleton organization and dynamics. Following injury, adult central nervous system neurons have a limited ability to regenerate and to recapitulate their robust developmental axon growth. This decreased regenerative capacity is set by their inability to establish regeneration-competent growth cones. Growth cones are actin-enriched structures that regulate axon extension rate and direction. During neuronal development, increasing actin dynamics in the growth cone through the regulation of the activity of specific actin-binding proteins leads to increased axon elongation. Here, we will focus on recent findings showing that enhanced axon regeneration in the adult nervous system can be achieved by promoting actin dynamics, or by decreasing actomyosin contraction in the growth cone. These discoveries underscore the importance of actin organization in the growth cone as a key factor to promote axon (re)growth that should be further explored in the future.
Obsessive-compulsive disorder (OCD) has been linked with elevated risk of suicidality. However, most previous studies have been cross-sectional, and little is known about the aetiology of the association between obsessive-compulsive symptoms (OCS) and suicidality in young adults.
Participants were members of the Child and Adolescent Twin Study in Sweden, at ages 18 (n=9,162) and 24 (n=3,466). Twins completed self-report measures, including assessment of OCS, suicidal ideation, and suicidal attempts. Logistic regression models tested concurrent and prospective associations of total OCS and OCS dimensions with suicidality, with and without adjustment for depression and anxiety symptoms. Genetic models tested the extent to which the main phenotypic associations were accounted for by genetic and environmental influences.
Total OCS were significantly associated with concurrent reports of suicidality at age 18 and 24, even when controlling for depressive and anxiety symptoms. Taboo obsessions (e.g., sexual an-shared environmental effects, suggesting that effective OCS treatment might reduce suicidality risk in this group.
Brothers and sisters growing up together share a large proportion of their genes and rearing environment. However, some siblings thrive whereas others struggle. This study investigated family-wide childhood bonding experiences with mother and father, in addition to individual-specific recollections, in relation to current depressive and anxiety symptom levels in adulthood. We examined whether extraversion and internal locus of control (iLoC) had a protective effect in this.
The sample consisted of 256 families with at least one lifetime depressed or anxious person (N=596; ages 20-78). Multilevel modeling with cross-level interactions was used.
Adult siblings showed moderate to high agreement in their childhood parental bonding (PB) recollections. Over-and-above the association between individual-specific recollections of PB and adult internalizing symptoms, family-wide poor PB was additionally linked to elevated symptom levels. Within families characterized by poor maternal bonding persons with an iLoC were relatively less anxious (but not less depressed), whereas extraversion was not protective in this context.
Although evidence exists that poor childhood PB has an impact on (adult) psychopathology, causality cannot be determined and possible recall bias of PB should be noted. Moreover, next to their moderating effects, extraversion and LoC may also act as mediators.
Our findings extend prior work bydemonstrating the importance of siblings' childhood PB experiences next to a person's own recollections when investigating adult internalizing symptoms, while also elucidating individual differences within families.

This write-up is a brief reflection of a rural doctor couple, Dr. Himmatrao Saluba Bawaskar (HSB) and Dr. Pramodini Himmatrao Bawaskar (PHB), working in the remote area of Maharashtra state of India during COVID-19 pandemic. During the pandemic, rural doctors are routinely exposed to symptomatic COVID-positive cases in the outpatient as well as indoor setting. The authors, both husband and wife, were in compulsory quarantine for twice at home and experienced social stigmas attached to a positive case. Here is a report the details of COVID-19 pattern and its management learned from the published scientific papers on COVID-19, and severe acute respiratory syndrome due to SARS-CoV-2 from December 2019 and their own experience in rural setting and the current literature shared in the form of personal narration. Apart from the personal experience of patients experience regarding quarantine period, COVID-19 is discussed in detail for the benefit of rural practitioners.The world is passing through the unprecedented crisis of COVID 19 pandemic. A large section of the global population has been living under mandatory mass quarantine, the lockdown, as a strategy towards slowing down the expansion of the pandemic. This lockdown is being eased out across world in a phase wise manner. India being one of the most populous countries is hardest hit by the pandemic and soon the number of positive cases is likely to touch one million mark. One of the most significant phenomenons observed during the Indian lockdown, has emerged as the long march of migrant workers from cities to their native places. Bihar, one of the Indian provinces is the major provider of migrant labourers for Indian agriculture and the industry sectors. As depicted on social media and television, the plight of migrants was disturbing and exposed modern Indian democracy's vulnerabilities. Many of them had to walk on foot for thousands of kilometers, with their hungry families, from the industrial cities to their natsh empire. The economy's size and rise in gross domestic product (GDP) are meaningless if ordinary citizens continue to be disfranchised, not protected, and liberated from the colonial processes. For India's sovereign economic development, there is no option but to invest in long-term and rebuild the civilization and build a system of the indigenous Indian knowledge economy based on the core principles and values of the Indian civilization.

Sixty-five VX2 tumor-burdened rabbits were randomly divided into five groups and treated transarterially with apatinib-loaded PIB (Group PA, 0.4 mL, n=13), PIB alone (Group P, 0.4 mL, n=13), iodized oil alone (Group I, 0.4 mL, n=13), apatinib solution (Group A, 0.4 mL, n=13) or saline (Group NS, 0.4 mL, n=13). The dose of apatinib was 2 mg/kg. Tumors were harvested, sectioned and immunohistochemically stained, and the tumor growth rates and survival times in each group were measured. Blood samples and liver tissues were colly reduced the systemic drug concentration. The data were then analyzed in SPSS version 21 by using chi-squared test, paired t-test, and independent t-test. Results According to the findings of the present study, there is no significant difference between the family-centered and the client-centered groups in terms of demographic features. By investigating the mean changes of both groups, There was a significant difference between quality of life score and spiritual-psychological aspect of scores before and after the intervention in the family-centered group. Conclusion The findings of the present study showed that family-centered education can increase the quality of life for people with gastrointestinal ostomy. Spiritual care education was conducted in five sessions of two hours. Quality of life Questionnaire (SF-36) was used in this study. To investigate the underlying mechanism and therapeutic potential of TAE combined with apatinib-loaded p( -isopropyl-acrylamide- -butyl methyl acrylate) temperature-sensitive (PIB) nanogel for the suppression of rabbit VX2 liver tumor growth. During the pandemic, rural doctors are routinely exposed to symptomatic COVID-positive cases in the outpatient as well as indoor setting. The authors, both husband and wife, were in compulsory quarantine for twice at home and experienced social stigmas attached to a positive case. Here is a report the details of COVID-19 pattern and its management learned from the published scientific papers on COVID-19, and severe acute respiratory syndrome due to SARS-CoV-2 from December 2019 and their own experience in rural setting and the current literature shared in the form of personal narration. Apart from the personal experience of patients experience regarding quarantine period, COVID-19 is discussed in detail for the benefit of rural practitioners.The world is passing through the unprecedented crisis of COVID 19 pandemic. A large section of the global population has been living under mandatory mass quarantine, the lockdown, as a strategy towards slowing down the expansion of the pandemic. This lockdown is being eased out across world in a phase wise manner. India being one of the most populous countries is hardest hit by the pandemic and soon the number of positive cases is likely to touch one million mark. One of the most significant phenomenons observed during the Indian lockdown, has emerged as the long march of migrant workers from cities to their native places. Bihar, one of the Indian provinces is the major provider of migrant labourers for Indian agriculture and the industry sectors. As depicted on social media and television, the plight of migrants was disturbing and exposed modern Indian democracy's vulnerabilities. Many of them had to walk on foot for thousands of kilometers, with their hungry families, from the industrial cities to their natsh empire. The economy's size and rise in gross domestic product (GDP) are meaningless if ordinary citizens continue to be disfranchised, not protected, and liberated from the colonial processes. For India's sovereign economic development, there is no option but to invest in long-term and rebuild the civilization and build a system of the indigenous Indian knowledge economy based on the core principles and values of the Indian civilization.

In situ thermal recovery is utilized extensively for unconventional bitumen extraction in the Cold Lake-****** River (CLBR) basin in Alberta, Canada. Public health concerns have been raised over potable groundwater contamination and arsenic release adjacent to these operations within the CLBR basin, which have been linked to subsurface heating of aquifer sediments. Under localized heated conditions, As-bearing aquifer sediments have been shown to undergo water-rock interactions and release constituents at near neutral pH conditions; however, release mechanisms have yet to be conclusively reported. To investigate the hydrogeochemical processes of aquifer heating and solute transport in detail, this study applies a novel heated column design to mimic saturated aquifer materials in contact with a thermal recovery well while constraining flow and geochemical conditions. Two column experiment scenarios were considered using 1) quartz [SiO2] sand with 0.6 wt% pyrite [FeS2]; and 2) aquifer sediments collected from the CLBR region. Heated temperature gradients between 50 °C and 90 °C were maintained within a 0.6 m section of the 3 m column with a flow rate of one pore volume per week. During heated low oxygen ( less then 3 mg L-1) conditions, results generally show increases in pH, Al, As, B, Mn, Mo, Si and Zn concentrations within and downgradient of the column heating section. Constituent release is primarily attributed to thermal desorption from Fe oxides, clay and silicate mineral dissolution, competitive anion exchange, and increased mixing. Overall results suggest that these mechanisms are responsible for increasing constituent concentrations in groundwater adjacent to in situ thermal recovery operations.
This study aims to investigate the effects of tea extracts on biofilm formation by oral streptococci and the potential mechanisms behind the effects.
We examined the effects of five types of tea extracts (green, oolong, black, pu-erh and chrysanthemum tea) on cell surface hydrophobicity and auto-aggregation of three different streptococcal species (Streptococcus mutans, Streptococcus salivarius and Streptococcus mitis) and evaluated their biofilm formation on four disparate hard surfaces (glass, stainless steel, hydroxyapatite and titanium). The correlation between biofilm formation and the cellular properties were investigated in order to study the mechanisms by which the tea extracts affect biofilm formation.
Results show that the tea extracts reduced cell surface hydrophobicity (by up to 57.9 %) and, in some cases, altered cellular auto-aggregation (by up to 12 %) and biofilm formation (by up to 2.61 log CFU cm
). Specifically, oolong tea extract was found to enhance biofilm formation by increasing cellular auto-aggregation and pu-erh tea extract retarded biofilm formation by increasing auto-aggregation. Biofilm formation correlated well to cell surface hydrophobicity and auto-aggregation in combination, but not to either one alone as determined by multiple linear regression analysis.

The study included publications in the English language that addressed the use of antimicrobial peptides in the prevention and treatment of caries. These publications were also searchable on PubMed, Web of Science, Embase, Scopus, the Collection of Anti-Microbial Peptides and the Antimicrobial Peptide Database. A total of 3,436 publications were identified, and 67 publications were included. Eight publications reported seven natural human antimicrobial peptides as bactericidal to Streptococcus mutans. Fifty-nine publications reported 43 synthetic antimicrobial peptides developed to mimic natural antimicrobial peptides, fusing peptides with functional sequences and implementing new designs. The 43 synthetic antimicrobial peptides were effective against Streptococcus mutans, and nine peptides specifically targeted Streptococcus mutans. Ten antimicrobial peptides had an affinity for hydroxyapatite to prevent bacterial adhesion. Six antimicrobial peptides were also antifungal. Four antimicrobial peptides promoted remineralisation or prevented the demineralisation of teeth by binding calcium to hydroxyapatite. In conclusion, this study identified 67 works in the literature that reported seven natural and 43 synthetic antimicrobial peptides for the prevention and treatment of caries. Most of the antimicrobial peptides were bactericidal, and some prevented bacterial adhesion. We investigated alcohol use during lockdown in a population of college students, usually characterized by social and heavy drinking. We also tested the predictive role of pre-lockdown drinking motives on alcohol consumption during lockdown. We collected data from 1951 French-speaking Belgian students during the lockdown period (April 1st - May 3rd, 2020) through a cross-sectional online survey. Participants self-reported their daily alcohol consumption (1) during a typical week in normal circumstances (i.e., before lockdown), and (2) since lockdown onset. We also assessed drinking motives and severity of alcohol use before lockdown. Using GLP-1RAs instead of insulin cost US$54,851 and US$29,115 per case of all-cause mortality and hospitalized hypoglycemia prevented, respectively, from the payer perspective, and saved US$19,391 and US$10,293, respectively, from the healthcare sector perspective. Sensitivity analyses showed that the probability of using GLP-1RAs versus insulin being cost-effective for preventing one case of all-cause mortality or hospitalized hypoglycemia ranged from 60 to 100%. The systematic review revealed a cost-effective profile of using GLP-1RAs versus insulin.

However, it remains to be answered which characteristics of plural leadership in hospital management boards make them impactful. The present study focuses on social determinants of implementation power of hospital boards using Talcott Parsons' sociological concept of adaptation, goal attainment, integration, and latency (AGIL), focusing on the G (goal attainment) and I (integration) factors of this concept. The study aims to test the hypothesis that hospitals with management boards that are oriented toward the quality goal (G) and socially integrated (I) (GI boards) are better at implementing quality management than hospitals with boards lacking these characteristics (non-GI boards).
A cross-sectional mixed-method design was used for data collection in 109 randomly selected hospitals in seven European countries. Data is based on the study "Deepening our understanding of qual board meetings.
Our findings suggest that the implementation power of hospital management boards is higher if there is a sense of unity and purpose within the boards. Thus, to improve quality management, it could be worthwhile to increase boards' social capital and to increase time designated for quality management in board meetings.
Early suspicion followed by assessing lung function with spirometry could decrease the underdiagnosis of chronic obstructive pulmonary disease (COPD) in primary care. We aimed to develop a nomogram to predict the FEV
/FVC ratio and the presence of COPD.
We retrospectively reviewed the data of 4241 adult patients who underwent spirometry between 2013 and 2019. By linear regression analysis, variables associated with FEV
/FVC were identified in the training cohort (n = 2969). Using the variables as predictors, a nomogram was created to predict the FEV
/FVC ratio and validated in the test cohort (n = 1272).
Older age (β coefficient [95% CI], - 0.153 [- 0.183, - 0.122]), male sex (- 1.904 [- 2.749, - 1.056]), current or past smoking history (- 3.324 [- 4.200, - 2.453]), and the presence of dyspnea (- 2.453 [- 3.612, - 1.291]) or overweight (0.894 [0.191, 1.598]) were significantly associated with the FEV
/FVC ratio. In the final testing, the developed nomogram showed a mean absolute error of 8.2% between the predicted and actual FEV
/FVC ratios. The overall performance was best when FEV
/FVC < 70% was used as a diagnostic criterion for COPD; the sensitivity, specificity, and balanced accuracy were 82.3%, 68.6%, and 75.5%, respectively.
The developed nomogram could be used to identify potential patients at risk of COPD who may need further evaluation, especially in the primary care setting where spirometry is not available.

Tumour-associated antigens (TAAs) comprise a large set of non-mutated cellular antigens recognized by T cells in human and murine cancers. Their potential as targets for immunotherapy has been explored for more than two decades1, yet the origins of TAA-specific T cells remain unclear. While tumour cells may be an important source of TAAs for T cell priming2, several recent studies suggest that infection with some viruses, including Epstein-Barr virus and influenza virus can elicit T cell responses against abnormally expressed cellular antigens that function as TAAs3,4. However, the cellular and molecular basis of such responses remains undefined. Here we show that expression of the Epstein-Barr virus signalling protein LMP1 in B cells provokes T cell responses to multiple TAAs. LMP1 signalling leads to overexpression of many cellular antigens previously shown to be TAAs, their presentation on major histocompatibility complex classes I (****I) and II (****II) (mainly through the endogenous pathway) and the upregulation of costimulatory ligands CD70 and OX40L, thereby inducing potent cytotoxic CD4+ and CD8+ T cell responses. These findings delineate a mechanism of infection-induced anti-tumour immunity. Furthermore, by ectopically expressing LMP1 in tumour B cells from patients with cancer and thereby enabling them to prime T cells, we develop a general approach for rapid production of autologous cytotoxic CD4+ T cells against a wide range of endogenous tumour antigens, such as TAAs and neoantigens, for treating B cell malignancies. This work stresses the need to revisit classical concepts concerning viral and tumour immunity, which will be critical to fully understand the impact of common infections on human health and to improve the rational design of immune approaches to treatment of cancers.Successful pregnancies rely on adaptations within the mother1, including marked changes within the immune system2. Genetic deletion of Rank (also known as Tnfrsf11a) in thymic epithelial cells results in impaired thymic involution and blunted expansion of natural regulatory T (Treg) cells in pregnant female ****. Sex hormones, in particular progesterone, drive the development of thymic Treg cells through RANK in a manner that depends on AIRE+ medullary thymic epithelial cells. The depletion of Rank in the mouse thymic epithelium results in reduced accumulation of natural Treg cells in the placenta, and an increase in the number of miscarriages. Thymic deletion of Rank also results in impaired accumulation of Treg cells in visceral adipose tissue, and is associated with enlarged adipocyte size, tissue inflammation, enhanced maternal glucose intolerance, fetal macrosomia, and a long-lasting transgenerational alteration in glucose homeostasis, which are all key hallmarks of gestational diabetes. Transplantation of Treg cells rescued fetal loss, maternal glucose intolerance and fetal macrosomia. In human pregnancies, we found that gestational diabetes also correlates with a reduced number of Treg cells in the placenta. Xq22.3-q23 microdeletion is a rare genomic disorder. The purpose of this study is to emphasize the correlation between clinical phenotype and genotype of proximal deletion on chromosome Xq22.3-q23. A 5 years old boy had a 671Kb microdeletion on Xq23 by chromosomal microarray analysis (CMA), including AMMECR1 and CHRDL1 genes. He presented microsomia, midface hypoplasia, right kidney dysplasia and mildly motor retardation, which have never been reported before to be related with Xq23 deletion. To our knowledge, this is the first case with Xq23 microdeletion. A total of 9 cases with microdeletion at Xq22.3-q23 covered AMMECR1 gene and 2 cases with CHRDL1 mutation were reviewed. These data indicated that Xq23 microdeletion with microsomia, midface hypoplasia, kidney dysplasia, mildly motor retardation was rare. The previous literature showed two novel point mutation in AMMECR1 and CHRDL1 with some phenotype different from the patient. Xq23 microdeletion should be considered for patients with microsomia, midface hypoplasia, kidney dysplasia and growth retardation.The aim of this paper is to introduce basic concepts and methods for calculating sample size in animal studies. In recent years, the developing world has been experiencing unprecedented increases in urbanization, with a correspondingly massive increase in municipal solid waste generation, among other things. Inefficient and inadequate waste collection and management, due to lack of resources and planning, led to significant increases in the volumes of waste on the streets and in open dumps, where it serves as food sources for free-roaming dogs. In this commentary, we discuss examples of poor waste management and the likely impact on rabies control efforts through the sustenance of free-roaming dogs in some dog rabies-endemic countries. We aim to stress the importance of implementing strategies that effectively address this particular issue as an important component of humane dog population management, as it relates to aspirations for the control and elimination of dog rabies per se.Vaccines are arguably a public health success story as well as an incredibly cost-effective medical resource. Despite this, worldwide concerns about their safety are growing, with the risk of increased morbidity and mortality in vaccine-preventable diseases because of vaccine refusal. The global political trend in developed countries is to increasingly reduce mandates and the compulsory nature of vaccination programs. This is due to strong opposition from anti-vaccination movements and groups. While these have existed since the beginnings of vaccinology, they have recently gained a strong foothold through massive exploitation of the media and especially the internet. This has led to widespread misinformation and greater difficulty for governments and health institutions in dealing with parents' concerns and misconceptions. Common strategies in order to maintain a high degree of public acceptance of vaccines include the enhancement of adverse effect reporting systems, the enrichment of scientific literature, and the dissemination of targeted information to parents and health care providers.
Xq22.3-q23 microdeletion is a rare genomic disorder. The purpose of this study is to emphasize the correlation between clinical phenotype and genotype of proximal deletion on chromosome Xq22.3-q23. A 5 years old boy had a 671Kb microdeletion on Xq23 by chromosomal microarray analysis (CMA), including AMMECR1 and CHRDL1 genes. He presented microsomia, midface hypoplasia, right kidney dysplasia and mildly motor retardation, which have never been reported before to be related with Xq23 deletion. To our knowledge, this is the first case with Xq23 microdeletion. A total of 9 cases with microdeletion at Xq22.3-q23 covered AMMECR1 gene and 2 cases with CHRDL1 mutation were reviewed. These data indicated that Xq23 microdeletion with microsomia, midface hypoplasia, kidney dysplasia, mildly motor retardation was rare. The previous literature showed two novel point mutation in AMMECR1 and CHRDL1 with some phenotype different from the patient. Xq23 microdeletion should be considered for patients with microsomia, midface hypoplasia, kidney dysplasia and growth retardation.The aim of this paper is to introduce basic concepts and methods for calculating sample size in animal studies.
