Secondary aim of the study was to fill the gap in research on the impact of copper smelters on aquatic ecosystems. Although the manuscript describes a case study performed near copper smelter in Poland, due to the novel results and cosmopolitan distribution of the species and significant world-wide impact of industry on the environment the results may be interested to broad publicity and find substantial application.Platelets regulate multiple aspects of CD4+ T cell immunity, and may exert distinct regulations among different T cell subsets. Our aim was to investigate how platelets regulate CD4+ central memory T cell (Tcm) responses. αCD3/αCD28-stimulated human CD4+ Tcm cells were cultured without or with platelets or platelet-derived mediators. Polyclonal stimulation induced cell proliferation and Th1 and Treg cell activation of Tcm cells. Platelet factor 4/PF4 neutralization abolished platelet-enhanced Tcm effector responses, whilst TGFβ neutralization only partially inhibited platelet-enhanced Treg cell activation. A total of 26 older patients with a major depressive disorder (MDD) (19 women; age range 65-84 years) and 26 older healthy controls (18 women; age range 65-80 years) participated in the study. The results showed that MDD individuals presented greater Stroop effects than their healthy controls and an absence of the conflict adaptation effect defined as a reduction of the influence of irrelevant stimulus dimensions after incongruent trials. Additionally, our results also showed that the processing of emotional words in depressed participants is more automatic than the processing of emotional faces. These findings suggest that older depressed individuals have greater difficulty in recognizing affective facial expressions than older healthy controls, while the over-learned behavior of word reading greatly reduces differences in the performance of the emotional Stroop task between groups (MDD and healthy controls).Objective To identify occupational risk factors for ALS using well-characterized participants with ALS (P-ALS), sibling controls (S-controls), and matched population controls (P-controls) within the National ALS Registry. Demographic, socioeconomic, and medical data were ascertained from medical records and structured interviews. P-ALS were followed prospectively for 2 years or until death, whichever came sooner. S-controls and age-, sex-, race/ethnicity-, and residential location-matched P-controls were recruited over 3 years. Occupational exposure to lead and agricultural chemicals (ACs) were assigned by an occupational hygienist, blinded to case status. OS biomarkers in urine were measured. Results P-ALS (mean age 62.8 years; 63% males) resided across the United States. Demographic and socioeconomic variables did not differ among P-ALS, S-controls, and P-controls. P-ALS were more likely to report occupations with exposure to lead (adjusted OR (aOR)=2.3, 95% CI 1.1, 4.6) and ACs (aOR = 2.4, 95% CI 1.2, 4.6) compared to pooled controls. Among those with occupations with exposure to both lead and ACs, aOR was 7.2 (95% CI 2.0, 26.1). Urinary 8-oxo-dG was significantly elevated among P-ALS (11.07 ± 5.42 ng/mL) compared to S-controls, P-controls, or pooled controls (pooled 7.43 ± 5.42 ng/mL; p less then 0.0001) but was not associated with occupational exposure to either lead or ACs. Conclusions Findings reveal increased risk of ALS diagnosis among those with occupational exposure to lead and ACs and increased OS biomarkers among cases compared to controls. OS may be an important pathogenic mechanism in ALS.

Purpose Patients who undergo radical surgery for rectal cancer often suffer from low anterior resection syndrome (LARS). Symptoms of this syndrome include frequent bowel movements, gas incontinence, fecal incontinence, fragmentation, and urgency. The aim of this study was to investigate the convergent validity, discriminative validity, and reliability of the Korean version of the LARS score questionnaire. Methods The English LARS score questionnaire was translated into Korean using the forward-and-**** translation method. A total of 146 patients who underwent radical surgery for rectal cancer answered the Korean version of the LARS score questionnaire including an anchor question assessing the impact of bowel function. The participants answered the questionnaire once more after 2 weeks. Results The Korean LARS score questionnaire showed a high convergent validity in terms of a high correlation between LARS score and quality of life (perfect fit 55.5% vs. moderate fit 37.6% vs. no fit 6.8%, respectively, P less then 0.001). The LARS score also showed good discriminative validity between groups of patients differing with regard to gender (29 for male vs. 25 for female, P= 0.014), tumor level (29 for.Purpose To delineate the learning curve (LC) for laparoscopic appendectomy (LA) performed by residents according to seniority in training. Methods Between October 2015 and November 2016, 150 patients underwent LA by three residents (in their first, second, and third year of training) under supervision. The patients were non-randomly assigned to each resident. The data were reviewed and analyzed retrospectively from prospectively collected database. The perioperative outcomes were compared between the three residents including operation time, complication, conversion, and so on. The LCs were evaluated by the moving average method and cumulative sum control chart (CUSUM) for operation time and surgical completion. Results Baseline characteristics and perioperative outcomes were similar except for age and location of the appendix among the three groups. Operation time was not different among the three residents (43.9, 45.3, and 48.4 min for A, B, and C, respectively). The moving average method for operation time showed a decreasing tendency for all residents. CUSUM for operation time showed that the peak points occurred at the 24th, 18th, and 31st cases for resident A, B, and C, respectively. In terms of surgical failure, residents A, B, and C reached steady states after the 35th, 11th, and 16th cases, respectively. Perforation of the appendix base was the only risk factor for surgical failure. Conclusion The LC for LA by residents was 11-35 cases according to multidimensional statistical analyses. The record comprised of 46,704 animals, where the number of the sires used was 1298 and the dams were 38,366 animals. The traits considered were carcass weight (CWT), eye muscle area (EMA), **** fat thickness (BFT), and marbling score (MS). Errors were introduced in the pedigree dataset through randomly assigning sires to all progenies. The error rates substituted were 5, 10, 20, 30, 40, 50, 60, 70 and 80%, respectively. A simulation was performed to produce a population of 1650 animals from the pedigree data. A REML based animal model was applied to estimate the EBV, accuracy of the EBV, expected genetic gain, variance components, and heritability (h2) estimates for carcass traits. Correlation of the simulated data under PEs was also estimated using Pearson's method. Results Beef cattle had significantly higher final body weight (BW) in 15% group, higher average daily gain (ADG) and dry matter intake (DMI) in 5% group, 10% group and 15% group, and higher feed conversion ratio (FCR) in 10% group and 15% group. Significantly higher blood superoxide dismutase (***) concentration was noted in 15% group, higher blood total antioxidant capacity (TAC) in 10% group and 15% group, lower 8-OHdG and malondialdehyde (MDA) in 15% group. Meat had lower pH in 15% group, higher CIE L* in 5% group, 10% group, and 15% group, and lower drip loss in 15% group. Greater concentration of meat polyunsaturated fatty acids (PUFA) was observed in 10% group and 15% group, and docosahexaenoic acid (DHA) in 15% group. Conclusion Diet with 15% B. papyrifera silage could improve performance and increase final BW, ADG, DMI and FCR, enhance the antioxidant functions by decreasing blood 8-OHdG and MDA and increasing blood *** and TAC, improve the meat quality by lowing pH and drip loss and increasing CIE L*, increase the meat PUFA and DHA concentration. Polyphenols and flavonoids might be the main components responsible for the antioxidant activity and anti-biohydrogenation in the B. papyrifera silage. And B. papyrifera silage could be used as a new feedstuff in beef cattle nutrition.

Both sustress and distress might impair normal physiological functions and even lead to pathological conditions, while eustress might benefit health through hormesis-induced optimization of homeostasis. Therefore, an optimal stress level is essential for building biological shields to guarantee normal life processes.Finding an optimal set of nodes, called key players, whose activation (or removal) would maximally enhance (or degrade) certain network functionality, is a fundamental class of problems in network science1,2. Potential applications include network immunization3, epidemic control4, drug design5, and viral marketing6. Due to their general NP-hard nature, those problems typically cannot be solved by exact algorithms with polynomial time complexity7. Many approximate and heuristic strategies have been proposed to deal with specific application scenarios1,2,8-12. Yet, we still lack a unified framework to efficiently solve this class of problems. Here we introduce a deep reinforcement learning framework FINDER, which can be trained purely on small synthetic networks generated by toy models and then applied to a wide spectrum of influencer finding problems. Extensive experiments under various problem settings demonstrate that FINDER significantly outperforms existing methods in terms of solution quality. Moreover, it is several orders of magnitude faster than existing methods for large networks. The presented framework opens up a new direction of using deep learning techniques to understand the organizing principle of complex networks, which enables us to design more robust networks against both attacks and failures.
Evaluation of X-ray reject analysis is an important quality parameter in diagnostic facility. The aim of this study was to find out the radiograph rejection and its causes during the coronavirus disease 2019 (COVID-19) pandemics as there was fear of coronavirus disease infection among the technical staff from the incoming patients in a busy, high volume public sector tertiary care hospital.
This descriptive study was conducted at Radiology Department, Lady Reading Hospital, Peshawar from August to November, 2020. The rejected radiographs and their causes were analyzed.
A total of 15,000 X-ray procedures were conducted during study period out of which 2550 cases were repeated making the total rejection 17%. Rejection in male and female were 74.3 and 25.7%, respectively, while rejection in adults was (80.1%) and (19.9%) in pediatric age group of the total rejection. Therefore, an optimal stress level is essential for building biological shields to guarantee normal life processes.Finding an optimal set of nodes, called key players, whose activation (or removal) would maximally enhance (or degrade) certain network functionality, is a fundamental class of problems in network science1,2. Potential applications include network immunization3, epidemic control4, drug design5, and viral marketing6. Due to their general NP-hard nature, those problems typically cannot be solved by exact algorithms with polynomial time complexity7. Many approximate and heuristic strategies have been proposed to deal with specific application scenarios1,2,8-12. Yet, we still lack a unified framework to efficiently solve this class of problems. Here we introduce a deep reinforcement learning framework FINDER, which can be trained purely on small synthetic networks generated by toy models and then applied to a wide spectrum of influencer finding problems. Extensive experiments under various problem settings demonstrate that FINDER significantly outperforms existing methods in terms of solution quality. Moreover, it is several orders of magnitude faster than existing methods for large networks. The presented framework opens up a new direction of using deep learning techniques to understand the organizing principle of complex networks, which enables us to design more robust networks against both attacks and failures. Evaluation of X-ray reject analysis is an important quality parameter in diagnostic facility. The aim of this study was to find out the radiograph rejection and its causes during the coronavirus disease 2019 (COVID-19) pandemics as there was fear of coronavirus disease infection among the technical staff from the incoming patients in a busy, high volume public sector tertiary care hospital. This descriptive study was conducted at Radiology Department, Lady Reading Hospital, Peshawar from August to November, 2020. The rejected radiographs and their causes were analyzed. While Staphylococcus spp. are the most common organisms isolated, Salmonella spp. are a rare cause of PJI (estimated prevalence less then 0.3 %). We present a case of a 62-year-old patient with a history of previous joint trauma complicated by osteonecrosis, infection and chronic alcohol abuse with late hematogenous prosthetic hip infection due to Salmonella enterica serovar Enteritidis. PJI due to Salmonella spp. should be considered in the differential diagnosis when a patient has risk factors such as malignancy, hemoglobinopathies, diabetes mellitus, human immunodeficiency virus/acquired immunodeficiency syndrome, alcohol dependency or immunosuppressed state, even without significant preceding gastrointestinal symptoms. Our patient had a few of these risk factors and required surgical debridement in addition to antimicrobials for treatment of his PJI.A 36-year-old man with well controlled HIV developed Campylobacter fetus aortitis. To prevent aortic rupture, emergent surgical resection and neo-aortoiliac replacement with his left femoral vein was conducted. After surgical intervention, he was successfully treated with intravenous ertapenem for 6 weeks followed by oral amoxicillin for 3 months.Some HIV-infection diagnostic guidelines and health care providers still rely on the ELISA-Western blot diagnostic algorithm. We present a near misdiagnosis case with discordant test results and a lack of proper counseling. Cefepime is a commonly used broad-spectrum antimicrobial agent for the treatment of nosocomial infections but to date has not been associated with EP. We report the first documented case of EP secondary to cefepime for the treatment of pneumonia. The patient's peripheral eosinophilia and leukocytosis resolved promptly after discontinuation of cefepime and initiation of steroid treatment.
Malignant otitis externa (MOE) is a serious infection of the external auditory canal that is frequently associated with skull base osteomyelitis (SBO) as well as secondary neurological sequelae. Patients with poorly controlled diabetes mellitus or immunosuppression are at increased risk of developing such critical infection for multiple local and systemic factors. , fungal infections are also occasionally encountered, often associated with delayed diagnosis and high morbidity and mortality.
We report a case of a 63 years old man with uncontrolled diabetes mellitus who presented with symptoms and signs of MOE, supported by radiological assessments. The patient was treated presumptively with a prolonged course of antibiotics without clinical improvement, coupled with progression of radiological findings and significant disease extension. Reassessment with biopsies and tissue cultures from external auditory meatus, tempo-mandibular bone, as well as base of the skull grew
. The patient received induction treatment with high dose liposomal amphotericin followed by fluconazole to control disease progression and complications.

Human immunodeficiency virus (HIV) infection and antiretroviral therapy (ART) have been associated with reduced bone mineral density (BMD) in persons with HIV (PWH). BMD provides information only about bone mineral quantity. Trabecular bone score (TBS) is a noninvasive tool that estimates bone microarchitecture. The aim of this study is to measure BMD and TBS of Chinese PWH after one-year ART.
We designed a retrospective study of adult Chinese PWH. Patients with a dual-energy X-ray absorptiometry (DXA) scan prior to ART initiation, and again 48 weeks later were included. Information regarding demographic and clinical history, HIV treatment history, BMD and TBS were collected. We analyzed differences in BMD and TBS over 48 weeks and associations between key risk factors and changes in BMD and TBS.
Our study included 233​PWH (mean age​=​36.6​±â€‹11.1 years). Before ART initiation, 19.3% of PWH had normal BMD but abnormal TBS. Both BMD and TBS decreased after one-year ART. TDF and LPV/r-containing regimens were associated with greater declines in BMD at different site.

Some human behaviors have serious societal consequences, but these consequences tend to be neglected in online research on societally relevant behaviors. For example, human activities contribute to climate change and biodiversity loss, but pro-environmental behavior is often studied using inconsequential self-reports and hypothetical scenarios. Such measures can easily be administered online, but suffer from severe validity problems. To address these problems, we developed a multi-trial web-based procedure for the study of consequential pro-environmental behavior. On the Work for Environmental Protection Task (WEPT), participants can choose to exert voluntary extra efforts screening numerical stimuli in exchange for donations to an environmental organization. They thus have the opportunity to produce actual environmental benefits at actual behavioral costs (i.e., to show actual pro-environmental behavior). In a preregistered validation study (N = 209), we found WEPT performance to systematically vary with these consequences, that is, the implemented costs and benefits were large enough for participants to effectively take them into account. In addition, aggregated WEPT performance was found to be highly reliable and to be correlated to self-reports and objective observations of other pro-environmental behaviors and conceptually related measures. These findings support the validity of the WEPT as an online procedure for the study of actual pro-environmental behavior. We discuss how the WEPT can advance the experimental analysis of pro-environmental behavior, help to address problems of common-method variance in individual difference research, and be adapted for the consequential study of other societally relevant behaviors.We propose a Sankey diagram-based visualization method to illustrate test-takers' action sequences in technology-enhanced, drag-and-drop items. Using the process data from three mathematical items in the National Assessment of Educational Progress, we exemplify how to develop Sankey diagrams, discover frequent actions at each stage of problem solving, deduce test-takers' response strategies, and discuss the relationship among domain (i.e., mathematics) knowledge, response strategy, and item performance. The applications and future extensions of this method can contribute to process-based problem-solving research in educational assessments.
This study aimed to evaluate whether treatment with fingolimod (FTY) may induce functional changes on the macular pre-ganglionic retinal elements in patients affected by relapsing-remitting multiple sclerosis (RR-MS) without optic neuritis (ON).
This case-control observational and retrospective study assessed multifocal electroretinogram (mfERG) responses from 35 healthy controls (mean age 43.58 ± 5.76years), 41 patients with RR-MS without ON (mean age 40.64 ± 4.83years, MS-noFTY group), and from 21 patients with RR-MS without ON (mean age 42.38 ± 12.34years) and treated with fingolimod (Gilenya®, Novartis Europharm, 0.5mg/day) (MS-FTY group). MfERG N1 and P1 implicit times (ITs), and N1-P1 response amplitude densities (RADs) were measured from concentric rings (R) with increasing foveal eccentricity 0-5° (R1), 5-10° (R2), 10-15° (R3), 15-20° (R4), 20-25° (R5). We considered R1 and R2 as "central macular areas" and R3, R4 and R5 as "more eccentric retinal areas". In the MS-FTY group, mfERG recordings wereal elements even in the absence of macular oedema.
To assess the point prevalence of peripheral neuropathy (PN) in children with type 1 diabetes mellitus (T1DM) and to determine their predictors.
In this cross-sectional study, children aged 8-18 y with T1DM on insulin therapy for > 2 y and free from acute complications were enrolled. All participants were evaluated for symptoms of PN with diabetic neuropathy symptom (DNS) score and underwent a detailed neurological examination. Assessment of nerve dysfunction was done using nerve conduction studies (NCS). The disease-related factors that increase the risk of PN were determined.
Fifty children (52% boys) were enrolled with mean age of 12.2 ± 2.8 y and duration of diabetes 5.1 ± 2.1 y. No subject had clinical evidence or DNS score suggestive of PN. Twenty-eight (56%) children demonstrated subclinical neuropathy on NCS. Proportion of children with pure motor, pure sensory, and mixed motor-sensory neuropathy was 40%, 2%, and 14%, respectively. The peroneal nerve was the most common motor nerve affected. Poor glycemic control (HbA1c > 9%) and longer duration of diabetes (> 5 y) were significantly associated with the risk of PN (p value < 0.05).
A large proportion of children with T1DM have subclinical PN. Poor glycemic control and longer duration of diabetes are risk factors for nerve dysfunction. Neurophysiological studies should be performed in these children to facilitate early detection.

Some human behaviors have serious societal consequences, but these consequences tend to be neglected in online research on societally relevant behaviors. For example, human activities contribute to climate change and biodiversity loss, but pro-environmental behavior is often studied using inconsequential self-reports and hypothetical scenarios. Such measures can easily be administered online, but suffer from severe validity problems. To address these problems, we developed a multi-trial web-based procedure for the study of consequential pro-environmental behavior. On the Work for Environmental Protection Task (WEPT), participants can choose to exert voluntary extra efforts screening numerical stimuli in exchange for donations to an environmental organization. They thus have the opportunity to produce actual environmental benefits at actual behavioral costs (i.e., to show actual pro-environmental behavior). In a preregistered validation study (N = 209), we found WEPT performance to systematically vary with these consequences, that is, the implemented costs and benefits were large enough for participants to effectively take them into account. In addition, aggregated WEPT performance was found to be highly reliable and to be correlated to self-reports and objective observations of other pro-environmental behaviors and conceptually related measures. These findings support the validity of the WEPT as an online procedure for the study of actual pro-environmental behavior. We discuss how the WEPT can advance the experimental analysis of pro-environmental behavior, help to address problems of common-method variance in individual difference research, and be adapted for the consequential study of other societally relevant behaviors.We propose a Sankey diagram-based visualization method to illustrate test-takers' action sequences in technology-enhanced, drag-and-drop items. In this cross-sectional study, children aged 8-18 y with T1DM on insulin therapy for > 2 y and free from acute complications were enrolled. All participants were evaluated for symptoms of PN with diabetic neuropathy symptom (DNS) score and underwent a detailed neurological examination. Assessment of nerve dysfunction was done using nerve conduction studies (NCS). The disease-related factors that increase the risk of PN were determined. Fifty children (52% boys) were enrolled with mean age of 12.2 ± 2.8 y and duration of diabetes 5.1 ± 2.1 y. No subject had clinical evidence or DNS score suggestive of PN. Twenty-eight (56%) children demonstrated subclinical neuropathy on NCS. Proportion of children with pure motor, pure sensory, and mixed motor-sensory neuropathy was 40%, 2%, and 14%, respectively. The peroneal nerve was the most common motor nerve affected. There is a robust link between patients' expectations and clinical outcomes, as evidenced by the placebo effect. Expectations depend in large part on the context surrounding treatment, including the patient-provider interaction. Prior work indicates that providers' behavior and characteristics, including warmth and competence, can shape patient outcomes. Yet humans rapidly form trait impressions of others before any in-person interaction. It is unknown whether these first impressions influence subsequent health care choices and expectations.
Our goal was to test whether trait impressions of hypothetical medical providers, based exclusively on facial images, influence the choice of medical providers and expectations about pain and analgesia following hypothetical painful medical procedures.
Across five online experiments, participants (total N=1108) viewed and made judgments about hypothetical healthcare providers. Experiments 1-4 included computer-generated faces that varied in features associated with our experimental simulations suggest that humans develop expectations about pain and health outcomes before even setting foot in the clinic, based exclusively on first impressions. These findings have strong implications for health care, as individuals increasingly rely on digital services to select healthcare providers and even receive treatment, a trend that is exacerbated as the world embraces telemedicine.
Mental health and substance use disorders are strong risk factors for homelessness. Understanding the role of transdiagnostic factors could help inform efforts to reduce homelessness among veterans with a range of disorders. Homeless veterans have high rates of trauma exposure, which can result in the depletion of social and emotional resources that may contribute to housing and employment stability. In this study, we evaluated the role of problems with emotional lability and interpersonal closeness as transdiagnostic socio-emotional factors that might interfere with efforts to achieve housing and employment stability.

Greater attention and more targeted efforts should be directed at helping trauma-exposed veterans build socio-emotional resources during treatment.

The current study seeks to identify which factors -at the community level- predict racial disparities in actual disease manifestation. This study focuses on the extent to which regional demographics and racial attitudes, both implicit and explicit, are associated with prevalence rates of several diseases for Black and White patients in the United States.
Implicit and explicit racial attitudes obtained from Project Implicit (Xu et al., 2017) were aggregated at the county level to predict variation in the prevalence rates of several chronic illnesses among Medicare recipients.
When controlling for economic indicators, Black and White patients who live in areas with high implicit and explicit racial bias tend to exhibit a higher incidence of chronic health problems, including cancer, stroke, asthma, diabetes, and heart failure. These relationships tended to be stronger for Black patients. Additionally, patients in racially diverse and racially segregated regions also tended to exhibit a higher incidence of chronic health problems.
Findings from the study highlight the reliable relationship between both racial biases and regional demographics and the incidence rates of several chronic diseases, particularly in Black patients.

Mortality rate after subarachnoid hemorrhage due to rupture of vertebral artery dissecting aneurysms (VADAs) is high; endovascular coil trapping is the first-line therapy to prevent rerupture. To select optimal treatments, the positional relationship between the VADA and posterior inferior cerebellar artery (PICA) and the morphology of the contralateral vertebral artery must be considered, and outcome predictors of different treatment methods and their possible complications must be identified.
We retrospectively studied 44 patients with ruptured VADAs who had undergone endovascular or surgical treatment. VADA morphology was assessed on conventional preoperative angiograms, and VADAs were categorized based on their site in relation to the PICA. VADA site, treatment method, and complications were used to identify prognostic factors.
The sites of the 44 VADAs were PICA-proximal (n= 3), PICA-distal (n= 22), PICA-absent (n= 7), and PICA-involved (n= 12). Treatments included internal coil trapping (n= 30), proximal coil occlusion (n= 5), and stent placement (n= 3); surgical flow alteration via an occipital artery-PICA bypass and ligation at the proximal vertebral artery and the PICA origin was performed in 6 patients. Periprocedural rebleeding was associated with a poor outcome. Internal coil trapping prevented the rerupture of PICA-proximal and PICA-absent VADAs, and flow alteration prevented rerupture of PICA-involved VADAs; there were no complications directly attributable to these procedures.

Here we focus attention on the role that inflammation and fibrosis play in the progression of age-related pathologies of the cornea and the lens as well as in glaucoma, the formation of epiretinal membranes, and in proliferative vitreoretinopathy.
Syphilis is endemic in the Sub-Saharan zone and disproportionately affects at-risk populations such as men who have sex with men, sex workers and HIV infected individuals. In this study, we measure the impact of syphilis among people living with HIV in the Republic of Chad, where no data are currently available.
Outpatients attending 2 HIV clinics in N'Djamena, Republic of Chad, were tested for syphilis. Subjects who tested positive for both non-treponemal (VDRL) and treponemal (TPHA) received a single dose of Benzathine Penicillin G, 2.4 MU. An additional VDRL test was performed 6 months after treatment to ensure appropriate serological response.
Of 207 patients included, 29 (14%) tested positive for VDRL at the first visit, with moderate/low antibody titers (ranging from 1/2 to 1/8); 24 (82.6%) of these had treponemal immunization confirmed by TPHA test. Six months after Benzathine Penicillin treatment, 22/24 of the patients (91.6%) tested negative for VDRL, and 2 showed a 4-fold titer decline.

Developmental delay, feeding difficulties, joint contractures and a high prevalence of autism spectrum disorders are characteristic of the syndrome. Endocrine abnormalities include mostly various pituitary hormonal deficiencies, presenting as hypoglycemia in 48% of reported cases. Persistent hyperinsulinism was only described in two siblings and responded to diazoxide treatment. We describe a unique case of an infant with Schaaf-Yang syndrome that presented with persistent hyperinsulinism unresponsive to diazoxide. Furthermore, we conducted a literature review of the endocrine abnormalities described in MAGEL2 related disorders. The case presented expands the clinical phenotype of Schaaf-Yang syndrome and emphasizes the importance of endocrine follow-up in these patients. Further investigation into the role of MAGEL2 in the regulation of pancreatic beta-cell insulin secretion, will improve our understanding of the abnormalities in glucose regulation in this syndrome.Alpha-thalassemia X-linked intellectual disability (ATR-X) syndrome is caused by a mutation in ATRX, which is essential for proper chromatin remodeling. ATRX dysfunction leads to dysregulation of many genes due to abnormal chromatin remodeling, and causes a multisystem disorder in patients with ATR-X. Because mitochondrial disorders also show multisystem involvement, whether mitochondrial function is affected in patients with ATR-X is of interest. Here, we report a case of a 4-year-old male with a mutation (NM_000489.4 c.736C > T p.Arg246Cys) in ATRX, who showed mitochondrial dysfunction with complex I deficiency. The results from our study suggest that target genes of the ATRX protein may include those responsible for mitochondrial function, and mitochondrial dysfunction may contribute to some ATR-X phenotypes.

Outpatients attending 2 HIV clinics in N'Djamena, Republic of Chad, were tested for syphilis. Subjects who tested positive for both non-treponemal (VDRL) and treponemal (TPHA) received a single dose of Benzathine Penicillin G, 2.4 MU. An additional VDRL test was performed 6 months after treatment to ensure appropriate serological response. Of 207 patients included, 29 (14%) tested positive for VDRL at the first visit, with moderate/low antibody titers (ranging from 1/2 to 1/8); 24 (82.6%) of these had treponemal immunization confirmed by TPHA test. Six months after Benzathine Penicillin treatment, 22/24 of the patients (91.6%) tested negative for VDRL, and 2 showed a 4-fold titer decline. This first study in the Republic of Chad suggests that syphilis infection is frequent among people living with HIV in this country. Systematic screening of syphilis should be considered in this population. Developmental delay, feeding difficulties, joint contractures and a high prevalence of autism spectrum disorders are characteristic of the syndrome. Endocrine abnormalities include mostly various pituitary hormonal deficiencies, presenting as hypoglycemia in 48% of reported cases. Persistent hyperinsulinism was only described in two siblings and responded to diazoxide treatment. We describe a unique case of an infant with Schaaf-Yang syndrome that presented with persistent hyperinsulinism unresponsive to diazoxide. Furthermore, we conducted a literature review of the endocrine abnormalities described in MAGEL2 related disorders. The case presented expands the clinical phenotype of Schaaf-Yang syndrome and emphasizes the importance of endocrine follow-up in these patients. Further investigation into the role of MAGEL2 in the regulation of pancreatic beta-cell insulin secretion, will improve our understanding of the abnormalities in glucose regulation in this syndrome.Alpha-thalassemia X-linked intellectual disability (ATR-X) syndrome is caused by a mutation in ATRX, which is essential for proper chromatin remodeling. ATRX dysfunction leads to dysregulation of many genes due to abnormal chromatin remodeling, and causes a multisystem disorder in patients with ATR-X. Because mitochondrial disorders also show multisystem involvement, whether mitochondrial function is affected in patients with ATR-X is of interest. Here, we report a case of a 4-year-old male with a mutation (NM_000489.4 c.736C > T p.Arg246Cys) in ATRX, who showed mitochondrial dysfunction with complex I deficiency. The proposed methodology represents a new management approach to support local authorities in defining the area of influence within the water bathing profile through the proper characterization of the point sources of bacterial pollution.Fecal sludge (FS)-derived fertilizer material, Fortifer™ was used in farmers' field to cultivate different crops under varying soil and agro-climatic conditions in Ghana. The aim was to (1) create awareness among smallholder farmers for widespread use of Fortifer™ (2) observe the response of crops to Fortifer™ application by farmers in different agro-ecological zones (3) obtain farmers feedback on the FS-derived product to enhance further dissemination across the country. In total 95 farmers in six locations participated in the farmer-led pilots. Fortifer™ containing up to 3.0% nitrogen, 3.6% phosphorus, 1.3% potassium and 44.3% organic matter was applied to tomato, rice, maize and pepper in comparison to inorganic fertilizers at recommended rates. Subsequently, farmers' perception of, and willingness to use the product were studied. Crop yield was significantly higher (p ≤ 0.05) in the Fortifer™ plots compared to the inorganic fertilizer plots for all the selected crops. Yield was 12% higher for tomato, 27% for rice and maize and 30% for pepper under the Fortifer™ plots. Farmers indicated that, nutrient content was the most important factor they consider when making fertilizer purchasing decision.Indiscriminate dumping may be considered a 'wicked problem', as it is regarded as a complex, intractable, open-ended problem and it includes rights-based and justice issues. An understanding of the dynamics of indiscriminate dumping may assist with improving the management of indiscriminate dumping. From the first round of data collection to the last, no dumpsite disappeared despite the efforts of the community to clean up each month. There were 3 different phases phase I as the control without IBU for 56 days, phase II (40 days), and phase III (60 days) containing IBU at 10 and 5 mg L-1 each. The overall performance of the SBR as well as the extracellular polymeric substances (EPS) in terms of polysaccharides, proteins, and humic acid substances were estimated. Morphological parameters of microbial aggregates in the presence of IBU (phase II and phase III) were assessed by quantitative image analysis (QIA). Removal efficiencies of chemical oxygen demand (COD) and ammonium (NH4+) were significantly reduced by IBU. Loosely bound EPS (LB-EPS) decreased during phase II and phase III, and tightly bound EPS (TB-EPS) was slightly higher in phase II than phase I. TB-EPS proteins were greater in phase II, perhaps to protect microbial cells from IBU exposure. These findings provided insight into both activated sludge stress-responses and EPS composition under long-term IBU exposure. Spearman correlation showed that EPS and morphological parameters significantly affected sludge settleability and flocculation. To identify associations between baseline ultrasound (US) and mammographic features and metastasis free survival (MFS) in women receiving neo-adjuvant chemotherapy (NACT) for breast cancer.
The data were collected as part of an ethically approved prospective study. Women with invasive breast cancer receiving NACT who were metastasis free at diagnosis were included. Baseline US and mammography were performed. Imaging was assessed by an experienced breast radiologist who was blinded to outcomes. US imaging features documented included posterior effect, skin thickening, size and stiffness using shear wave elastography (SWE). The mammographic features documented were spiculation and microcalcification. The development of metastatic disease was ascertained from computer records. Statistical analysis was performed using Kaplan Meier survival curves and Receiver Operator Characteristic (ROC) analysis.
171 women with 172 cancers were included in the study and 55 developed metastatic disease. Mean follow-up was 6.0 years. Women with mammographic calcification had significantly poorer metastasis free survival (MFS) compared to women without calcification (p = 0.043, 6 yr MFS 50 % vs 69 %). Women bearing cancer with distal shadowing had poorer MFS than women without shadowing (p = 0.025, 6 yr MFS 47 % vs. 73 %). Women with US skin thickening had poorer MFS compared to women without skin thickening (p = 0.032, 6 yr MFS 52 % vs. 68 %). Mammographic spiculation, US size and stiffness at SWE had no significant association with MFS.
We have identified mammographic and US features associated with MFS in women receiving NACT. Such information may be useful when counselling patients about the benefits and risks of NACT.

The endoderm is the innermost germ layer that forms the linings of the respiratory and gastrointestinal tracts, and their associated organs, during embryonic development. Xenopus embryology experiments have provided fundamental insights into how the endoderm develops in vertebrates, including the critical role of TGFβ-signaling in endoderm induction,elucidating the gene regulatory networks controlling germ layer development and the key molecular mechanisms regulating endoderm patterning and morphogenesis. With new genetic, genomic, and imaging approaches, Xenopus is now routinely used to model human disease, discover mechanisms underlying endoderm organogenesis, and inform differentiation protocols for pluripotent stem cell differentiation and regenerative medicine applications. In this chapter, we review historical and current discoveries of endoderm development in Xenopus, then provide examples of modeling human disease and congenital defects of endoderm-derived organs using Xenopus.Planar cell polarity (PCP) refers to the coordinated polarization of cells within the plane of a tissue. PCP is a controlled by a group of conserved proteins organized in a specific signaling pathway known as the PCP pathway. A hallmark of PCP signaling is the asymmetric localization of "core" PCP protein complexes at the cell cortex, although endogenous PCP cues needed to establish this asymmetry remain unknown. While the PCP pathway was originally discovered as a mechanism directing the planar organization of Drosophila epithelial tissues, subsequent studies in Xenopus and other vertebrates demonstrated a critical role for this pathway in the regulation of actomyosin-dependent morphogenetic processes, such as neural tube closure. Large size and external development of amphibian embryos allows live cell imaging, placing Xenopus among the best models of vertebrate neurulation at the molecular, cellular and organismal level. This review describes cross-talk between core PCP proteins and actomyosin contractility that ultimately leads to tissue-scale movement during neural tube closure.Neural crest cells are a multipotent embryonic stem cell population that emerges from the lateral border of the neural plate after an epithelium-to-mesenchyme transition. These cells then migrate extensively in the embryo and generate a large variety of differentiated cell types and tissues. Alterations in almost any of the processes involved in neural crest development can cause severe congenital defects in humans. Moreover, the malignant transformation of one of the many neural crest derivatives, during childhood or in adults, can cause the development of aggressive tumors prone to metastasis such as melanoma and neuroblastoma. Collectively these diseases are called neurocristopathies. Here we review how a variety of approaches implemented using the amphibian Xenopus as an experimental model have shed light on the molecular basis of numerous neurocristopathies, and how this versatile yet underused vertebrate animal model could help accelerate discoveries in the field. Using the current framework of the neural crest gene regulatory network, we review the pathologies linked to defects at each step of neural crest formation and highlight studies that have used the Xenopus model to decipher the cellular and molecular aspects of neurocristopathies.The epidermis of the Xenopus embryo has emerged as a powerful tool for studying the development of a ciliated epithelium.
