  • The Internet is a widely used resource for patients seeking health information, yet little editing or regulations are imposed on posted material. We sought to assess the quality and accuracy of information presented on shoulder instability on the online video platform YouTube. We hypothesize that YouTube videos concerning shoulder instability will be of little quality, accuracy, and reliability.

    The first 50 YouTube videos resulting from the keyword query 'shoulder instability' were analyzed. The Journal of American Medical Association (JAMA) benchmark criteria (score range, 0-4) was used to assess video accuracy and reliability, and the Global Quality Score (GQS; score range, 0-5) was used to assess the quality of the video's educational content along with a generated Shoulder-Specific Score (SSS).

    The 50 videos observed collectively had 5,007,486 views, with the mean number of views being 100,149.72±227,218.04. Of all videos observed, 32% were from a medical source and 56% had content relating to pathology information. The mean JAMA score was 2.84±0.74, with the highest scores coming from academic sources. The mean GQS and SSS scores were 2.68±0.84 and 5.30±3.78. The mean GQS score was highest in videos from medical sources (3.3±0.8) and videos about surgical technique/approach (3.2±1.1). Advertisements were negative predictors of the JAMA score (β=-0.324, P =0.014), and academic (β=0.322, P =0.015) and physician sources (β=0.356, P =0.008) were positive predictors.

A total of 1097 patients (51.8%) had at least one MOI, 322 (15.2%) at least two MOIs, and 48 (2.3%) at least three MOIs. Male gender, smoking history, and absence of genital ulcer were significantly associated with MOI in multivariable binary logistic regression. Multivariable ordinal regression analyses confirmed the association between MOI and male gender and smoking, but not the protective effect of genital ulcers. In both regression analyses, we found no significant effects of age, human leucocyte antigen-B51, skin involvement, or joint involvement on MOI.Conclusion Male gender and positive smoking history have a significant influence on the presence of MOI in patients with BS.I am often asked how our mother inspired my sister Moriah and me to want to become scientists. The different machine learning algorithms outperform the various EEG based emotion models for valence and arousal. But the downside is to devote numerous efforts to designing features from the given noisy signals which are also a very time-consuming process. The Deep Learning analysis overcomes the hand-engineered feature extraction and selection problems. In this study, the Database of Emotion analysis using Physiological signals (DEAP) has been visualized to classify High-Arousal- Low-Arousal (HALA), High-Valence-Low-Valence (HVLV), familiarity, Dominance and Liking emotions. The fusion of deep learning models, namely CNN and LSTM-RNN seems to perform better for the analysis of emotions using EEG signals.   • Papillary fibroelastoma (PFE) is a primary, histologically benign endocardial neoplasm. Though PFE has long been reported as the second most common primary cardiac neoplasm, it has since pulled ahead of cardiac myxomas, largely due to evolving cardiac imaging modalities. While PFEs are benign histologically, they have the potential for devastating clinical consequences, transient ischemic attack, stroke, myocardial infarction, syncope, pulmonary, and peripheral embolism. Despite increased detection rate, there remains uncertainty regarding etiology, exact prevalence, and clinical management of PFEs. This paucity of information is reflected by the lack of official guidelines on this matter. In this article, we aim to summarize the current state of understanding regarding PFE and discuss areas of ongoing controversy.
    To investigate the prevalence of potentially inappropriate prescribing (PIP) for cardiovascular system (CVS) and antiplatelet/anticoagulant (AP/AC) drugs among Korean elderly patients, using the Screening Tool of Older Persons' Prescriptions (STOPP) criteria version 2 and to identify the risk factors related to PIP.

    The 2016 National Aged Patient Sample data, comprising National Health Insurance claim records for a random sample of 20% of patients aged ≥ 65 years, were used to calculate PIP prevalence of outpatient prescriptions. For criteria including drug-disease interactions, PIP prevalence per indication was estimated.

    Among 1,274,148 elderly patients and 27,062,307 outpatient prescription claims, 100,085 patients (7.85%) and 341,664 claims (1.27%) had one or more PIP. The most frequent PIP was "non-steroidal anti-inflammatory drug with concurrent antiplatelet agent (s) without proton-pump inhibitor prophylaxis" in the claim-level (0.97%) and patient-level (6.33%) analyses. "Beta-blocker with bradycardia" (16.47% of claims) and "angiotensin receptor blockers in patients with hyperkalaemia" (23.89% of claims) showed the highest PIP prevalence per indication. Logistic regression analysis revealed that, among the patient and health care provider characteristics, female, older age, more severe comorbidities, polypharmacy, higher level of healthcare organization, and specialty of prescriber were significantly associated with a higher risk of PIP.

    Our findings of a high prevalence of PIP for CVS and AP/AC drugs among the elderly suggest that an effective strategy is urgently needed to improve the prescription practices of these drugs.

    Post-operative atrial fibrillation (POAF) is a common yet understudied clinical issue after coronary artery bypass graft (CABG) leading to higher mortality rates and stroke. This systematic review and meta-analysis evaluated the rates of adverse outcomes between patients with and without POAF in patients treated with CABG or combined procedures.

    The search period was from the beginning of PubMed and Embase to May 18
    , 2020 with no language restrictions. The inclusion criteria were (1) studies comparing new onset atrial fibrillation before or after revascularization
    no new onset AF before or after revascularization. The outcomes assessed included all-cause mortality, cardiac death, cerebral vascular accident (CVA), myocardial infarction (MI), repeated revascularization, major adverse cardiac event (****), and major adverse cardiac and cerebrovascular events (MACCEs).

    Of the 7,279 entries screened, 11 studies comprising of 57,384 patients were included. Compared to non-POAF, POAF was significantly associated with higher risk of all-cause mortality (Risk Ratio (RR) = 1.58; 95% Confidence Interval (CI) 1.42-1.76,

Existing methods for tumor phylogeny estimation suffer from either high computational intensity or low-resolution indication of clonal architecture, giving a necessity of developing new methods for efficient and accurate reconstruction of tumor trees. We introduce GRMT (Generative Reconstruction of Mutation Tree from scratch), a method for inferring tumor mutation tree from SCS data. GRMT exploits the k-Dollo parsimony model to allow each mutation to be gained once and lost at most k times. Under this constraint on mutation evolution, GRMT searches for mutation tree structures from a perspective of tree generation from scratch, and implements it to an iterative process that gradually increases the tree size by introducing a new mutation per time until a complete tree structure that contains all mutations is obtained. This enables GRMT to efficiently recover the chronological order of mutations and scale well to large datasets. Extensive evaluations on simulated and real datasets suggest GRMT outperforms the state-of-the-arts in multiple performance metrics. Oncology wards have more communication complexity than general wards; therefore, creating a positive communication environment is important for ensuring quality care. This study aimed to understand the relationship between organizational communication satisfaction and missed nursing care in Taiwan.

    This cross-sectional study was performed to measure organizational communication satisfaction and missed nursing care in six oncology wards at a stand-alone cancer centre hospital in Taipei, Taiwan in December, 2018.

    The study collected data using the Communication Satisfaction Questionnaire and the MISSCARE survey. The data were analysed using descriptive statistics, t test, analysis of variance and Pearson product-moment correlation analysis in December 16, 2018.

    A total of 111 questionnaires were collected, and the response rate was 92.5%. This cross-sectional study was performed to measure organizational communication satisfaction and missed nursing care in six oncology wards at a stand-alone cancer centre hospital in Taipei, Taiwan in December, 2018. Pigs were housed in mixed sex pens (n = 8 SG+6 FG/pen) equipped with feeding stations to record daily feed intake per individual pig. Pigs were assigned to three dietary treatments resulting in a 2 × 3 factorial arrangement at 15 weeks of age. Isoenergetic diets were formulated by increasing the ideal protein profile based on the following SID lysine (Lys) levels 0.92%, 1.18% and 1.45%. Pigs were weighed bi-weekly until 21 weeks of age. Fast-growing pigs were 33.7 kg heavier, gained 255 g/day and consumed 625.5 g/day more than SG pigs (p 0.05).     Using a multicenter approach and harmonized analysis methods, we aimed to shed light on Parkinson's disease stage-specific profiles of pathology, as suggested by in vivo neuroimaging.

    Individual brain MRI and clinical data from 2357 Parkinson's disease patients and 1182 healthy controls were collected from 19 sources. We analyzed regional cortical thickness, cortical surface area, and subcortical volume using mixed-effects models. Patients grouped according to Hoehn and Yahr stage were compared with age- and sex-matched controls. Within the patient sample, we investigated associations with Montreal Cognitive Assessment score.

    Overall, patients showed a thinner cortex in 38 of 68 regions compared with controls (d

This study aimed to explore the relationship between donor highest AST value or first donor GGT value and graft survival. All consecutive patients who underwent a primary LT in a single center with available donor AST (N=1253) and GGT value (N=1152) were included. There was no significant association between donor AST and 90-day graft survival. We found a moderate association between GGT and 90-day graft survival. We found a significant interaction with a donor history of alcohol abuse (HAA). The risk of graft loss was associated with AST and GGT in donors with an HAA but remains unchanged in donors without HAA. There was no difference in graft survival according to donor AST or GGT with a cutoff ≥ 95th percentile (475 UI/L for AST and 170 UI/L for GGT). However, graft survival was significantly decreased when donors combined GGT ≥ 170 UI/L and HAA (61% at one year).   • Our results suggest preventive practices such as equipment sanitation should be implemented to limit the spread of herbicide resistant D. insularis.
    Evidence in our work suggests that gene flow of glyphosate-resistant D. insularis is associated with movement of agricultural machinery, although local selection pressure seems to play an important role in the evolution of herbicide resistance throughout the country. Our results suggest preventive practices such as equipment sanitation should be implemented to limit the spread of herbicide resistant D. insularis.
    The purpose of this study was to use folic acid and zinc nitrate to construct a biocompatible supramolecular hydrogel loaded with the herbicide dicamba as an ultra-low-volume spray formulation. The drift potential of the hydrogel was studied by simulating the field environment in a wind tunnel.

    The three-dimensional network structure of the successfully prepared dicamba hydrogel system was observed using cryo-scanning electron microscopy. A rheological study of the dicamba hydrogel showed that it has shear-thinning and self-healing properties. Using a laser particle size analyzer, it was shown that the droplet size of the dicamba gel (approximately 100 μm) was significantly larger than that of the control group water and dicamba-KOH droplets. Droplet collectors and water-sensitive papers were arranged in the wind tunnel to evaluate the drift-reduction performance of the dicamba gel. Compared with dicamba-KOH aqueous solution, dicamba gel has a good effect in reducing drift.

    Recent reports suggest that buprenorphine is being diverted and used non-medically. However, no apparent studies have reported national-level data on buprenorphine diversion.

    Case report data were drawn from a quarterly survey of prescription drug diversion completed by a national sample of law enforcement and regulatory agencies who engage in drug diversion investigations. Quarterly rates of buprenorphine diversion per 100 000 population and 100 000 prescriptions dispensed were calculated for the period 2002 through 2019. Population-based diversion rates were also calculated by U.S. region.

    In total, 9670 cases of diverted buprenorphine were reported across all 50 states and the District of Columbia during the study period. Buprenorphine diversion rates, per 100 000 population, were characterized by an accelerating increase over time; increases in diversion rates from 1st quarter 2002 through 4th quarter 2006 were not statistically significant, yet from 1st quarter 2007 through 4th quarter 2019, the rate of diversion cases increased by 0.0067 cases per 100 000 per quarter (p < 0.001). Buprenorphine diversion rates per 100 000 prescriptions dispensed indicated a gradual increase over time; from 3rd quarter 2010 through 4th quarter 2019, diversion rates showed a statistically significant increase of 0.28 cases (p=0.037) per quarter on average.     Tumor lysis syndrome (TLS) is a common oncologic emergency among patients with pediatric hematologic malignancies. The mainstay of TLS management is aggressive intravenous hydration. However, the epidemiology of fluid overload (FO) and acute kidney injury (AKI) in this population is understudied. In this study, we aimed to describe the incidence, severity, and complications of FO and AKI among pediatric patients with TLS.

    We completed a single-center retrospective cohort study of pediatric patients with a new diagnosis of hematologic malignancy over a 10-year period. Patients with TLS were analyzed in two groups based on the severity of AKI and FO. Charts were reviewed for complications associated with AKI and FO including hypoxemia, mechanical ventilation, hyponatremia, pulmonary edema, pediatric intensive care (PICU) admission, and need for renal replacement therapy (RRT).

    We analyzed 56 patients with TLS for FO and AKI. We found severe FO (≥10%) occurred in 35.7% (n=20). PICU admission occurred in 35% of patients with severe FO compared to 8.3% in those with mild/moderate FO <10% (p=.013). Complications of hypoxemia (30% vs. 5.6%, p=.012) and pulmonary edema (25% vs. 2.8%, p=.010) were more common among those with severe FO. AKI occurred in 37.5% (n=21) patients and resulted in a significant increase in PICU admission and requirement for RRT (p=.001 and <.001, respectively).

Case report data were drawn from a quarterly survey of prescription drug diversion completed by a national sample of law enforcement and regulatory agencies who engage in drug diversion investigations. Quarterly rates of buprenorphine diversion per 100 000 population and 100 000 prescriptions dispensed were calculated for the period 2002 through 2019. Population-based diversion rates were also calculated by U.S. region. In total, 9670 cases of diverted buprenorphine were reported across all 50 states and the District of Columbia during the study period. Buprenorphine diversion rates, per 100 000 population, were characterized by an accelerating increase over time; increases in diversion rates from 1st quarter 2002 through 4th quarter 2006 were not statistically significant, yet from 1st quarter 2007 through 4th quarter 2019, the rate of diversion cases increased by 0.0067 cases per 100 000 per quarter (p < 0.001). Buprenorphine diversion rates per 100 000 prescriptions dispensed indicated a gradual increase over time; from 3rd quarter 2010 through 4th quarter 2019, diversion rates showed a statistically significant increase of 0.28 cases (p=0.037) per quarter on average.   • In this article, we present the draft genome sequence of Metabacillus niabensis strain 4T19T (= CSUR Q2603 T = DSM 17723 = JCM 16399 = KACC 11279), that is a new Metabacillus species isolated from cotton-waste composts. The genome sequence from Metabacillus niabensis strain 4T19T was assembled into 462 contigs for a total size of 4,987,608 bp with a G + C content of 35.5%.Embryogenesis is a vulnerable stage in elasmobranch development due in part to high predation mortality. Embryonic elasmobranchs respond to potential predators by displaying a freezing behaviour, characterized by the cessation of pharyngeal respiration followed immediately by coiling of the tail around the body. We hypothesized that the duration of this freeze response is limited by the embryo's requirement for oxygen. Here, Scyliorhinus canicula embryos were incubated at either 15°C or 20°C during embryogenesis and tested for the duration of, and metabolic consequence of, the freeze response at their respective incubation temperature. Freeze response duration was negatively impacted by routine metabolic rate; embryos at 20°C had 7-fold shorter freeze duration than those at 15°C, potentially increasing their susceptibility to predation. These data demonstrate the capacity for climate change stressors to affect animal behaviour and suggest that this may occur by eliciting changes in the organism's metabolism. The goals of this study were to explore, describe and compare the bacterial communities in caecal and hard faecal samples from free-ranging black-tailed prairie dogs (n = 36) from KS, USA, using high-throughput sequencing of the V4 region of the 16S rRNA gene and to compare sex and geographic locations. A total of 22 paired faecal and caecal samples were collected post-mortem from free-ranging black-tailed prairie dogs from 5 different geographical locations. The results revealed that the microbiota of both faecal and caecal samples were dominated by the phylum Firmicutes (genera belonging to the Clostridiales order). There was significantly greater richness in faecal compared with caecal samples. There were significant differences between the 5 different geographic regions (P  less then  0.001), specifically in the relative abundances of genera. There were differences in rare members of the microbiome between faecal samples from male and female prairie dogs but with no significant impact on overall community structure. This study provides novel data and expands our knowledge about the gastrointestinal microbiome composition of free-ranging black-tailed prairie dogs, which has potential to inform conservation efforts and improve their captive management.Many reef organisms, such as the giant clams, are confronted with global change effects. We investigated physiological responses of 4-year-old Tridacna maxima to realistic levels of temperature (+1.5°C) and partial pressure of carbon dioxide (pCO2) (+800 μatm of CO2) predicted for 2100 in French Polynesian lagoons during the warmer season. During a 65-day crossed-factorial experiment, individuals were exposed to two temperatures (29.2°C, 30.7°C) and two pCO2 (430 μatm, 1212 μatm) conditions. The impact of each environmental parameter and their potential synergetic effect were evaluated based on respiration, biomineralization and photophysiology. Kinetics of thermal and/or acidification stress were evaluated by performing measurements at different times of exposure (29, 41, 53, 65 days). At 30.7°C, the holobiont O2 production, symbiont photosynthetic yield and density were negatively impacted. High pCO2 had a significant negative effect on shell growth rate, symbiont photosynthetic yield and density. No significant differences of the shell microstructure were observed between control and experimental conditions in the first 29 days; however, modifications (i.e. less-cohesive lamellae) appeared from 41 days in all temperature and pCO2 conditions. No significant synergetic effect was found. Present thermal conditions (29.2°C) appeared to be sufficiently stressful to induce a host acclimatization response. All these observations indicate that temperature and pCO2 are both forcing variables affecting T. maxima's physiology and jeopardize its survival under environmental conditions predicted for the end of this century.

    The aim of this thematic literature analysis was to map current studies in the field of designing ECAs for patients with dementia in order to identify the existing research trend and possible gaps that need to be covered in the near future. The review questions in this study were as follows (1) what research frameworks are used to study the interaction between patients with dementia and ECAs? (2) what and proofs of efficacy of the approach are required. Therefore, by analyzing the main topic of the narrative review, this study underscores the challenge of synchronizing and harmonizing knowledge, efforts, and challenges in the dementia care field and its person-centered paradigm through the user-centered design approach. Enabling strict collaboration between interdisciplinary research networks, medical scientists, technology developers, patients, and their formal and informal caregivers is still a great challenge in the field of technologies for older adults.

Many models for understanding trust exist, some explicitly designed for eHealth, but no studies have used these models to explore trust in the context of using IoT systems to support asthma self-management.

Supported self-management, including an action plan agreed with clinicians, improves asthma outcomes. Internet-of-things (IoT) systems with artificial intelligence (AI) can provide customized support for a range of self-management functions, but trust is vital to encourage patients' adoption of such systems. Many models for understanding trust exist, some explicitly designed for eHealth, but no studies have used these models to explore trust in the context of using IoT systems to support asthma self-management. In this study, we aim to use the McKnight model to explore the functionality, helpfulness, and reliability domains of patients' and clinicians' trust in IoT systems to deliver the 14 components of self-management support defined by the PRISMS (Practical Reviews in Self-Management Support) taxonomy. We used think-aloud techniques in semistructured interviews to explore the views of patients and clinicians. Patients were recruited from research registerste novel advice. Research is needed to ensure that technological capability does not outstrip the trust of individuals using it. As a common mental disease, depression seriously affects people's physical and mental health. According to the statistics of the World Health Organization, depression is one of the main reasons for suicide and self-harm events in the world. Therefore, strengthening depression detection can effectively reduce the occurrence of suicide or self-harm events so as to save more people and families. With the development of computer technology, some researchers are trying to apply natural language processing techniques to detect people who are depressed automatically. Many existing feature engineering methods for depression detection are based on emotional characteristics, but these methods do not consider high-level emotional semantic information. The current deep learning methods for depression detection cannot accurately extract effective emotional semantic information. In this paper, we propose an emotion-based attention network, including a semantic understanding network and an emotion understanding network,e results compared with state-of-the-art methods. The experimental results showed that our model is competitive with the state-of-the-art models. The semantic understanding network module, the emotion understanding network module, and the dynamic fusion strategy are effective modules for depression detection. In addition, the experimental results verified that the emotional semantic information was effective in depression detection. Close to half (48.2%) were police officers, while 35.7% were officers in the Federal Road Safety Corp (FRSC). 45% of respondents had secondary/high school education, while 39.5% had bachelor's degree; with a significant majority in the FRSC. 269(54.2%) respondents had convicted a road user of drunk driving/riding on the basis of suequipment for objectively detecting drunk driving among road users. The objective of our study was to examine the patterns of retention in care among adolescents and young adults with HIV infection by analyzing Medicaid and commercial health insurance claims data.

    We evaluated retention in care for HIV-diagnosed adolescents and young adults aged 13-24 using the 2010-2014 MarketScan Medicaid and MarketScan Commercial Claims health insurance databases. The study period extended 36 months from the date of the first claim with a code for HIV or AIDS. We determined the unweighted proportion retained in care for the Medicaid and Commercial Claims cohorts for months 0-24 and 25-36. We assessed associations between demographic characteristics and retention in care using logistic regression.

Aggregation and sub-setting by characteristics of interest was done in R to calculate means and frequencies of reported general physician payments from industry across different specialties, locations, timeframes, and payment types. Normalization was applied for numbers of physicians or payments. Geospatial statistical hot spot analysis was conducted in ArcGIS. 51.73 million payment records were analyzed. In total, 50,047,930 payments were issued to 771,113 allopathic or osteopathic physicians, representing $8,702,631,264 transferred from industry to physicians over the five-year period between 2014 and 2018. The mean payment amount was $179, with a standard deviation of $12,685. Variability in physicians' financial relationships with industry were apparent across specialties, regions, time, and payment type. A limited match rate between records in the NPPES and Open Payments databases may have resulted in selection bias of trends related to physician characteristics. Further research is necessary, particularly in the context of changing industry payment trends and public perceptions of the appropriateness of these relationships.
