To analyze the clinical data of a case of acute emamectin·chlorfenapyr poisoning in Guangzhou 12th People's Hospital in 2019. The patient developed high fever and night sweats, and gradually became unconscious. The patient died after 5 days of treatment. The toxicity and mortality of emamectin·chlorfenapyr were high. For acute poisoning patients, in addition to conventional symptomatic treatment, early blood purification treatment should be actively carried out.Objective To investigate the changes of neuron-specific enolase (NSE) in serum and cerebrospinal fluid of patients with subacute 1, 2-dichloroethane (DCE) poisoning. Methods Ten patients with subacute 1, 2-DCE poisoning hospitalized in Guangzhou 12th Municipal People's Hospital from December 2014 to March 2019 were taken as the subacute 1, 2-DCE poisoning group, 34 typical acute toxic encephalopathy patients hospitalized at the same time as typical acute toxic encephalopathy group, 40 healthy physical examinees as normal control group. The levels of serum NSE in patients of subacute 1, 2-DCE poisoning and typical acute toxic encephalopathy group during onset and improvement were detected by chemiluminescence method, and the results were analyzed statistically. The level of NSE in cerebrospinal fluid of subacute 1, 2-DCE poisoning group was detected and analyzed its correlation with the level of NSE in serum. Using receiver operator characteristic (ROC) curve to analyze the diagnostic efficacy of NSE in subacute 1, 2-DCE poisoning and typical acute toxic encephalopathy (area under curve, AUC) . Results There was no significant difference between the serum NSE level of the patients with subacute 1, 2-DCE poisoning in the onset group and the normal control group and the improvement group (P>0.05) . Methods In March 2019, by consulting the occupational disease diagnosis records, we investigated the distribution of all 471 occupational noise-induced deafness diagnostic applicants in Guangzhou Occupational Disease Prevention and Treatment Center from 2011 to 2018. Frequency and constituent ratio were used to describe the distribution. Results From 2011 to 2018, there were 471 cases of occupational noise-induced deafness diagnostic applicants and 211 of the applicants were diagnosed as occupational noise-induced deafness (44.8%, 211/471) . The new cases were mainly mild (83.9%, 177/211) , with the predilection age of 40.0-49.0 years old (41.7%, 88/211) and the predilection seniority of 5.0-9.9 years (38.9%, 82/211) . Among the new cases, there were mainly males (88.6%, 187/211) . Most of the new cases were distributed in Nansha district (23.2%, 49/211) , Huangpu district (22.7%, 48/211) and Panyu district (21.8%, 46/211) . Besides, in terms of industry distribution, they were concentrated in manufacturing industry (82.0%, 173/211) . The scale of enterprises was mainly medium-sized (42.7%, 90/211) , and the most of their economic type was domestic-funded (40.8%, 86/211) . Conclusion Although most of the newly diagnosed cases of occupational noise deafness in Guangzhou are mild, they still need to be paid attention to and strengthen the noise industry protection education.Objective To explore the status and influencing factors of occupational pressure among medical staff in a third-level first-class hospital of traditional Chinese medicine in Tianjin, and to provide reference for formulating relevant policies. A total of 191 questionnaires were distributed and recovered, and 189 valid questionnaires were recovered, with an effective recovery rate of 98.95%. The Scale for Occupational Stressor on Clinician was used for investigation. The influence of different characteristics on the occupational pressure of medical staff was analyzed. Results The average total score of occupational pressure was (94.8±15.4) . There were significant differences in occupational pressure among different age groups (P less then 0.05) . To measure the impact of rapid influenza real-time qualitative reverse transcriptase polymerase chain reaction (RT-PCR) on patient management in busy pediatric emergency department (ED) and urgent care clinic settings.
We developed a brief, elective survey that clinicians completed when an influenza RT-PCR order was placed in the ED or urgent care clinic between February 18, 2019, and March 13, 2019. We captured the clinical suspicion for influenza, intended management plans, and actual management plans once influenza RT-PCR results were available.
We evaluated 339 encounters, of which 164 (48.4%) had a positive influenza RT-PCR. Clinical suspicion for influenza was a nonsignificant predictor for influenza PT-PCR positivity (P=.126). After rapid influenza RT-PCR results were available, clinicians changed their original plans in 44.5% of influenza RT-PCR positive vs 92.6% of influenza RT-PCR negative cases (P<.0001). Change in plans for antiviral use was observed in 26% of influenza positive vs 77% of influenza negative cases (P<.0001). A total of 135 antiviral prescriptions were avoided in patients with negative influenza RT-PCR.
Implementation of a rapid and accurate influenza RT-PCR in the acute care setting is important to systematically diagnose influenza in children and improve outpatient management decisions, because clinical suspicion for influenza is inaccurate. To investigate the relationship between intraretinal macular hemorrhage and visual acuity outcomes in eyes with central retinal vein occlusion or hemiretinal vein occlusion managed with aflibercept, bevacizumab, or observation.

Reduced area of hemorrhage by month 6 was observed in 70.7% (116 of 164) of aflibercept-treated eyes, 63.8% (104 of 163) of bevacizumab-treated eyes, and 42.2% (27 of 64) of observation eyes by month 8 (P < .01). To report the incidence and outcomes of hypertensive phase (HP) following Aurolab Aqueous Drainage Implant (AADI) (Aurolab) surgery in adults with refractory glaucoma.
Retrospective, noncomparative, interventional case series.
All eyes that received the AADI and had a minimum of 2-year follow-up were identified, and data of patients who had intraocular pressure (IOP) ≤21mm Hg at 6weeks (ie, the time at which the tube-ligature suture dissolves) were used for statistical analysis. HP was defined as IOP >21mm Hg during the first 3months after the release of the tube ligating suture (with or without medications) in the absence of tube obstruction.
A total of 200 eyes were included in the study, and HP was seen in 64 eyes (32%) with a peak IOP (mean ± SD) of 29.6 ± 7.8mm Hg and peak incidence at 2-3months after surgery. HP resolved within 3months of its onset in 60 of the 64 eyes (94%) with additional IOP-lowering medications. The cumulative success rates were 71.8% (95% CI= 59.3%-81.2%) in HP eyes and 76.4% (95% CI= 68.7%-82.7%) in non-HP eyes (P= .23). Unadjusted Cox proportional hazards analysis showed that eyes experiencing HP had a marginally higher risk of failure (HR= 1.16, 95% CI= 0.6-2.1), but this relationship was not statistically significant (P= .61).

To identify characteristics of patients with early open-angle glaucoma exhibiting greater macular perfusion density (PD) loss compared with macular ganglion cell layer (GCL) thickness loss.
Cross-sectional study.
Optical coherence tomography (OCT) imaging of the optic nerve head and macula was conducted in patients and healthy control subjects. Minimum rim width, retinal nerve fiber layer and GCL thickness, and PD from OCT angiography were derived. Only high-quality images were included. For direct comparison, raw PD and GCL thickness values in patients were converted to relative age-corrected loss values based on data from controls. Demographic and ocular variables related to greater PD loss compared with GCL thickness loss were identified with multivariate logistic regression.

RevMan software 5.3 (Cochrane Collaboration, the Nordic Cochrane Centre, Copenhagen, Denmark) was used for conducting all statistical analyses. Results This meta-analysis demonstrated that LNR > 0.2 significantly correlated with worse survival (hazard ratio [HR] 1.84; 95% CI 1.74-1.94; p ≤ 0.00001) in node-positive pancreatic cancer patients. Oral feeding is one of the most complex maturational skills of infancy. Difficulties with feeding require specialized attention, and if not well managed, may prolong the newborn's hospital length of stay. This is particularly true for prenatally opioid exposed (POE) infants. A paucity of literature exists characterizing feeding behaviors of POE infants, yet feeding problems are common.
The purpose of this integrative review was to synthesize and critically analyze the evidence that characterizes feeding behaviors in full-term, POE infants.
The electronic databases of CINAHL, PubMed, Scopus, and PsycINFO were used. Inclusion criteria were studies in English, conducted from 1970 to 2019, with participant samples consisting of infants with prenatal opioid exposure, born full-term, and between 38 and 40 weeks of gestation. Based on the inclusion criteria, our search yielded 557 articles. Neonatal opioid withdrawal syndrome (NOWS) is a significant and growing health problem that affects more than 23,000 infants annually, with an estimated hospital cost of more than $720 million.
The purpose of this study was to examine factors associated with the need to initiate medication for the treatment of NOWS.
A retrospective review of medical records was conducted of 204 infants born to mothers who used opioids during pregnancy from April 2011 to September 2017. Associations between maternal, infant, and environmental factors and the need for neonatal pharmacological management were examined using χ, t tests, and regression analysis.
Of 204 neonates exposed to opioids prenatally, 121 (59%) developed symptoms of NOWS, requiring treatment with morphine. Neonates requiring morphine had significantly higher gestational ages (37.7 weeks vs 36.4 weeks; P < .001), and mothers were present at the neonate's bedside for a lower proportion of their total hospital stay (57% vs 74% of days; P < .001). Maternal factors associated with the need for neonatal medication treatment included the mother's reason for opioid use (P = .014), primary type of opioid used (P < .001), tobacco use (P = .023), and use of benzodiazepines (P = .003).
This research provides information regarding the proportion of infants exposed to opioids prenatally who develop NOWS that requires treatment, as well as maternal, infant, and environmental factors associated with the need for neonatal medication use.
Future research is needed to examine these relationships prospectively in a larger and more diverse sample.

The current US opioid crisis has resulted in a significant increase in opioid use disorder among pregnant and parenting women. Substance use disorders, in general, are highly stigmatized conditions. Stigma serves as a well-documented global barrier to health-seeking behaviors and engagement in healthcare. While extensive research exists on the stigma of mental illness, few studies have explored the stigma experienced by families impacted by neonatal abstinence syndrome (NAS).
Therefore, the purpose of this article is to explore the role of stigma in the care of families impacted by NAS.
In this article, we present a discussion about the effects of stigma on this patient population and provide exemplars of stigma experiences from our previous research and the existing literature.
Mothers of infants with NAS faced the challenges of overcoming stigma as they were often ostracized, excluded, and shamed. Nurses who provide care for these women and their infants have reported experiencing ethical distress, moral distress, and compassion fatigue.
Greater awareness of the impact of opioid use on the maternal-child population has resulted in numerous educational offerings for healthcare providers; however, this alone is not adequate to end stigma. Fortunately, promising tools and methods have been developed for assisting nurses with addressing stigma in a manner that can be both nonconfrontational and highly effective.
Future research is needed to explore and evaluate the efficacy of various existing strategies for counteracting harmful stigma in this patient population.

Greater leisure-time physical activity (LTPA) associates with healthier lives, but knowledge regarding occupational physical activity (OPA) is more inconsistent. DNA methylation (DNAm) patterns capture age-related changes in different tissues. We aimed to assess how LTPA and OPA are associated with three DNAm-based epigenetic age estimates, namely, DNAm age, PhenoAge, and GrimAge.
The participants were young adult (21-25 yr, n = 285) and older (55-74 yr, n = 235) twin pairs, including 16 pairs with documented long-term LTPA discordance. Genome-wide DNAm from blood samples was used to compute DNAm age, PhenoAge, and GrimAge Age acceleration (Acc), which describes the difference between chronological and epigenetic ages. Physical activity was assessed with sport, leisure-time, and work indices based on the Baecke Questionnaire. Genetic and environmental variance components of epigenetic age Acc were estimated by quantitative genetic modeling.

V.In this study, bioglass (BG)‑iron oxide (Fe3O4) nanocomposite coating was developed to produce bioactive and antibacterial coatings. The nanocomposite coating was embedded in chitosan (CS) matrix and coating was fabricated by electrophoretic deposition (EPD) method. The coating was characterized by using SEM/EDX and XRD respectively. The experiment was performed with three varying concentrations (1, 3, 5 (wt%)) of Fe3O4 nanoparticles prepared by the co- precipitation method in the bioactive glass coating. The antibacterial activity was examined in Escherichia coli and Staphylococcus aureus bacteria which determine that the growth of microorganisms was inhibited with the progressive increment of Fe3O4 nanoparticles. The bioactivity assessment was done in PBS for 7 days and it was detected that the composite coatings improve the bone-bonding ability which was again confirmed by SEM-EDX. The corrosion behavior was evaluated in Ringer's solution by electrochemical test. The corrosion analysis revealed that the BG-1% Fe3O4 nanocomposite coating has superior corrosion resistance as compared to the other coatings. The findings of the research have shown that the BG-Fe3O4-CS nanocomposite coating can be widely used as a suitable material for orthopedic applications. In the present investigation, a lipid hydrolyzing gene RPK01 was cloned from metagenome source of hot spring. Expression and purification of recombinant protein revealed single purified protein band of ~24 KDa on 12% SDS-PAGE, and is well corroborated with the deduced molecular weight of protein as calculated from its amino acid sequence. The purified protein displayed high activity towards short chain fatty acids and was found to be completely stable at 30°C till 3h, it further retained ~40% activity at 50°C and 60°C temperature till 3h. Additionally, the pH stability assay showed its functionality in broad range of pH, with maximum stability observed at pH 2.0, it decreases from pH 4.0 to pH 12.0, and nearly showed 40% activity in these pH values. Biological macromolecule namely cellulose nanofiber was synthesized from agrowaste sugarcane bagasse. Further, hierarchical nano Fe(0) - FeS was anchored on cellulose nanofibers to obtain CNF-Fe(0)@FeS and it was characterized by various spectral techniques. Formation of FeS2 was confirmed by high resolution TEM and XPS techniques. Anchoring on cellulose nano fibers prevented agglomeration of nanocomposite and furthermore, coating of FeS2 helped in the preservation of Fe(0). Thus the prepared nano composite was very effective in the removal of cationic Methylene blue (MB) dye and anionic Congo red (CR) dye with a maximum adsorption capacity of 200.0 and 111.1 mg/g respectively. This study investigates how extraction method impacts the yield, chemical composition, structure characteristics, hypoglycemic and antioxidant activity of polysaccharides from ****** (Zingiber officinale Roscoe) stems and leaves (GSLP). Four extraction methods were employed to obtain the GSLP, including hot water extraction (HWE), ultrasound-assisted extraction (UAE), alkaline solution extraction (ASE), and enzyme-assisted extraction (EAE). The data showed that ASE produced highest extraction yield compared to the other extraction methods. Scanning electron microscopy indicated that GSLP microstructures were greatly influenced by extraction method. Moreover, in vitro hypoglycemic activity and antioxidant activity experiments demonstrated that the biological capacities of ASE-GSLP were superior to GSLPs extracted by the other methods. Taken together, these results indicate that polysaccharides from ****** stems and leaves obtained by alkali, complex enzyme, and ultrasonic-assisted extractions are imbued with different characteristic mechanisms of degradation, despite the uniformity of their main structures. In addition, ASE-GSLP displayed better biological activities probably due to its abundant uronic acid content, higher sulfate radical, and smaller molecular weight. Thus, it can be concluded that ASE has great potential as an effective strategy for obtaining polysaccharides from stems and leaves because of its higher yield and the remarkable bioactivity of its products. V.In this study, bioglass (BG)‑iron oxide (Fe3O4) nanocomposite coating was developed to produce bioactive and antibacterial coatings. The nanocomposite coating was embedded in chitosan (CS) matrix and coating was fabricated by electrophoretic deposition (EPD) method. The coating was characterized by using SEM/EDX and XRD respectively. The experiment was performed with three varying concentrations (1, 3, 5 (wt%)) of Fe3O4 nanoparticles prepared by the co- precipitation method in the bioactive glass coating. The antibacterial activity was examined in Escherichia coli and Staphylococcus aureus bacteria which determine that the growth of microorganisms was inhibited with the progressive increment of Fe3O4 nanoparticles. The bioactivity assessment was done in PBS for 7 days and it was detected that the composite coatings improve the bone-bonding ability which was again confirmed by SEM-EDX. The corrosion behavior was evaluated in Ringer's solution by electrochemical test. The corrosion analysis revealed that the BG-1% Fe3O4 nanocomposite coating has superior corrosion resistance as compared to the other coatings. The findings of the research have shown that the BG-Fe3O4-CS nanocomposite coating can be widely used as a suitable material for orthopedic applications. In the present investigation, a lipid hydrolyzing gene RPK01 was cloned from metagenome source of hot spring. Expression and purification of recombinant protein revealed single purified protein band of ~24 KDa on 12% SDS-PAGE, and is well corroborated with the deduced molecular weight of protein as calculated from its amino acid sequence. The purified protein displayed high activity towards short chain fatty acids and was found to be completely stable at 30°C till 3h, it further retained ~40% activity at 50°C and 60°C temperature till 3h. Additionally, the pH stability assay showed its functionality in broad range of pH, with maximum stability observed at pH 2.0, it decreases from pH 4.0 to pH 12.0, and nearly showed 40% activity in these pH values. Biological macromolecule namely cellulose nanofiber was synthesized from agrowaste sugarcane bagasse. Further, hierarchical nano Fe(0) - FeS was anchored on cellulose nanofibers to obtain CNF-Fe(0)@FeS and it was characterized by various spectral techniques. Formation of FeS2 was confirmed by high resolution TEM and XPS techniques. Anchoring on cellulose nano fibers prevented agglomeration of nanocomposite and furthermore, coating of FeS2 helped in the preservation of Fe(0). Thus the prepared nano composite was very effective in the removal of cationic Methylene blue (MB) dye and anionic Congo red (CR) dye with a maximum adsorption capacity of 200.0 and 111.1 mg/g respectively. Both MB and CR dye followed Pseudo second kinetic with regression coefficients >0.98. Especially for MB dye, it was observed that kinetics of adsorption/degradation was very fast and within 3 min, peak at 370 nm disappeared and 70% reduction in intensity was observed at 660 nm. Further FTIR and XPS revealed evidence for the degradation of adsorbed dye molecules. Considering its high adsorption capacity, degradation potential and ease of preparation using agrowaste makes CNF-Fe(0)@FeS a promising sorbent for the treatment industrial dye effluents. This study investigates how extraction method impacts the yield, chemical composition, structure characteristics, hypoglycemic and antioxidant activity of polysaccharides from ginger (Zingiber officinale Roscoe) stems and leaves (GSLP). Four extraction methods were employed to obtain the GSLP, including hot water extraction (HWE), ultrasound-assisted extraction (UAE), alkaline solution extraction (ASE), and enzyme-assisted extraction (EAE). The data showed that ASE produced highest extraction yield compared to the other extraction methods. Scanning electron microscopy indicated that GSLP microstructures were greatly influenced by extraction method. Moreover, in vitro hypoglycemic activity and antioxidant activity experiments demonstrated that the biological capacities of ASE-GSLP were superior to GSLPs extracted by the other methods. Taken together, these results indicate that polysaccharides from ginger stems and leaves obtained by alkali, complex enzyme, and ultrasonic-assisted extractions are imbued with different characteristic mechanisms of degradation, despite the uniformity of their main structures. In addition, ASE-GSLP displayed better biological activities probably due to its abundant uronic acid content, higher sulfate radical, and smaller molecular weight. Student mental health in higher education has been an increasing concern. The COVID-19 pandemic situation has brought this vulnerable population into renewed focus.
Our study aims to conduct a timely assessment of the effects of the COVID-19 pandemic on the mental health of college students.
We conducted interview surveys with 195 students at a large public university in the United States to understand the effects of the pandemic on their mental health and well-being. The data were analyzed through quantitative and qualitative methods.
Of the 195 students, 138 (71%) indicated increased stress and anxiety due to the COVID-19 outbreak. Multiple stressors were identified that contributed to the increased levels of stress, anxiety, and depressive thoughts among students. These included fear and worry about their own health and of their loved ones (177/195, 91% reported negative impacts of the pandemic), difficulty in concentrating (173/195, 89%), disruptions to sleeping patterns (168/195, 86%), decreased social interactions due to physical distancing (167/195, 86%), and increased concerns on academic performance (159/195, 82%). To cope with stress and anxiety, participants have sought support from others and helped themselves by adopting either negative or positive coping mechanisms.
Due to the long-lasting pandemic situation and onerous measures such as lockdown and stay-at-home orders, the COVID-19 pandemic brings negative impacts on higher education. The findings of our study highlight the urgent need to develop interventions and preventive strategies to address the mental health of college students.

Public health measures that were effective in previous infection outbreaks (eg, wearing a face mask, quarantining) were implemented in this outbreak. Available multidimensional social network data that take advantage of the recent rapid development of information and communication technologies allow for an exploration of disease spread and control via a modernized epidemiological approach. By using spatiotemporal data and real-time information, we can provide more accurate estimates of disease spread patterns related to human activities and enable more efficient responses to the outbreak. Two real cases during the COVID-19 outbreak demonstrated the application of emerging technologies and digital data in monitoring human movements related to disease spread. Although the ethical issues related to using digital epidemiology are still under debate, the cases reported in this article may enable the identification of more effective public health measures, as well as future applications of such digitally directed epidemiological approaches in controlling infectious disease outbreaks, which offer an alternative and modern outlook on addressing the long-standing challenges in population health.

Cognitive function and physical frailty are known to be closely related. Among older adults with dementia, those who wear dentures have a higher mortality rate than those who do not wear them. This suggests the possibility that oral health may affect the cognitive-frailty relationship. This study aims to investigate whether the number of teeth present, acts as a moderating variable in the cognitive function-frailty relationship.
Data were obtained from the cross-sectional baseline study of the Korean Frailty Aging Cohort Study (2016-2017). Cognitive function was assessed using the Mini-Mental State Examination. Frailty score was based on the Cardiovascular Health Study Index. Oral condition was evaluated by the number of teeth present and analyzed using categories of 0-9 teeth, 10-19 teeth, and ≥20. The moderation effect was analyzed using the ordinary least squares (OLS) regression.

PURPOSE Excessive joint line obliquity (JLO) after open-wedge high tibial osteotomy (OWHTO) induces detrimental stress on the articular cartilage. The purpose of this article is to assess the correlation between JLO and the clinical results after OWHTO. METHODS 68 patients were followed up for more than 1 year. JLO was assessed using a long-leg standing anteroposterior radiograph. The knee osteoarthritis outcome score (KOOS) and KSS (Knee Society score) objective knee score were assessed as clinical scores. The Weight-bearing line ratio (WBLR), medial proximal tibial angle (MPTA), lateral distal femoral angle (LDFA), and joint line convergence angle (JLCA) were assessed as radiological parameters. The timed up-and-go (TUG) test and single-leg standing (SLS) test were performed, and the isometric muscle strength of the quadriceps and hamstrings was assessed to evaluate the knee function. The primary outcomes were the correlations between the JLO and the clinical score, radiological parameters and knee function after OWHTO. The secondary objective of this study was to detect the factor with the greatest influence on JLO. RESULTS There were significant correlations between the postoperative JLO and the KOOS in the subcategories of pain, activities of daily living (ADL), and sports and recreation (r = -0.311, -0.302, -0.282, p = 0.011, 0.014, 0.022, respectively). However, the postoperative JLO was not significantly correlated with the KSS, knee function, or muscle strength. The preoperative LDFA and postoperative MPTA were factors influencing increased JLO after OWHTO. DISCUSSION There was no significant correlation between the JLO and the actual knee function. Abstract Les communautés de parasites de Caranx sexfasciatus ont été caractérisées et analysées afin de déterminer si elles connaissent des variations interannuelles dans la structure et/ou la composition des espèces. Au total, 422 C. sexfasciatus ont été collectés dans la baie d’Acapulco, au Mexique, entre mai 2016 et mars 2019. Trente-deux taxons de parasites métazoaires ont été identifiés cinq Monogenea, treize Digenea, un Acanthocephala, un Cestoda, trois Nematoda, sept Copepoda et deux Isopoda. Les monogènes étaient les espèces de parasites les plus fréquentes et les plus abondantes au cours de toutes les années d’échantillonnage. La richesse en espèces de parasites au niveau de la communauté des composants variait considérablement de 8 (mai 2016) à 25 (mars 2019) et était similaire aux rapports précédents pour d’autres espèces de Carangidae. Les communautés et infracommunautés de C. sexfasciatus étaient caractérisées par un faible nombre d’espèces de parasites, une faible diversité et la dominance d’une seule espèce (le monogène Neomicrocotyle pacifica). Thirty-eight, non-replicated, KPC-Kp isolates from colonized patients (either already colonized at admission or colonized during admission), collected during 2016, were subjected to antimicrobial-susceptibility testing and WGS. All isolates were resistant to β-lactams, with the exception of ceftazidime/avibactam (97.4 % susceptible). The second most effective agent was fosfomycin, followed by colistin, trimethoprim/sulfamethoxazole, gentamicin and amikacin (92.1, 86.8, 60.5, 44.7 and 50 % of susceptibility, respectively). A large proportion of isolates (n=18/38, 47.4%) belonged to clonal group (CG) 101, and most of them (n=15) to a new sequence type (ST) designated as ST2502. All the CG101 isolates had a capsule locus type KL17. The ST2502 harboured the genes encoding for the yersiniabactin siderophore and the ArmA methylase, conferring high-level resistance to aminoglycosides. The second most represented lineage of isolates (16/38, 42.1%) belonged to ST512 of CG258. Analysing WGS data, we were able to ascertain the common origin of some isolates imported from other hospitals, and to track several clusters of in-LTACRF cross-transmissions. EnvZ responds to cytoplasmic signals that arise from changes in the extracellular milieu, and OmpR acts canonically (requiring phosphorylation) to regulate the porin genes and noncanonically (without phosphorylation) to activate the acid stress response. Herein, we describe how insights gleaned from stimulus recognition and response in EnvZ are relevant to nearly all sensor kinases and response regulators.

Abstract Les communautés de parasites de Caranx sexfasciatus ont été caractérisées et analysées afin de déterminer si elles connaissent des variations interannuelles dans la structure et/ou la composition des espèces. Au total, 422 C. sexfasciatus ont été collectés dans la baie d’Acapulco, au Mexique, entre mai 2016 et mars 2019. Trente-deux taxons de parasites métazoaires ont été identifiés cinq Monogenea, treize Digenea, un Acanthocephala, un Cestoda, trois Nematoda, sept Copepoda et deux Isopoda. Les monogènes étaient les espèces de parasites les plus fréquentes et les plus abondantes au cours de toutes les années d’échantillonnage. La richesse en espèces de parasites au niveau de la communauté des composants variait considérablement de 8 (mai 2016) à 25 (mars 2019) et était similaire aux rapports précédents pour d’autres espèces de Carangidae. Thirty-eight, non-replicated, KPC-Kp isolates from colonized patients (either already colonized at admission or colonized during admission), collected during 2016, were subjected to antimicrobial-susceptibility testing and WGS. All isolates were resistant to β-lactams, with the exception of ceftazidime/avibactam (97.4 % susceptible). The second most effective agent was fosfomycin, followed by colistin, trimethoprim/sulfamethoxazole, gentamicin and amikacin (92.1, 86.8, 60.5, 44.7 and 50 % of susceptibility, respectively). A large proportion of isolates (n=18/38, 47.4%) belonged to clonal group (CG) 101, and most of them (n=15) to a new sequence type (ST) designated as ST2502. All the CG101 isolates had a capsule locus type KL17. The ST2502 harboured the genes encoding for the yersiniabactin siderophore and the ArmA methylase, conferring high-level resistance to aminoglycosides. The second most represented lineage of isolates (16/38, 42.1%) belonged to ST512 of CG258. Analysing WGS data, we were able to ascertain the common origin of some isolates imported from other hospitals, and to track several clusters of in-LTACRF cross-transmissions. The results revealed that, in peculiar epidemiological settings such as LTACRF, new KPC-Kp clones different from those prevailing in acute-care hospitals and associated with uncommon resistance and virulence determinants can successfully emerge and disseminate.Two-component regulatory systems represent the major paradigm for signal transduction in prokaryotes. EnvZ responds to cytoplasmic signals that arise from changes in the extracellular milieu, and OmpR acts canonically (requiring phosphorylation) to regulate the porin genes and noncanonically (without phosphorylation) to activate the acid stress response. The anesthetic management of patients with severe pulmonary hypertension is different from that of normal, healthy patients, and regional nerve blocks are commonly used for them. Due to the individual variability of the course, distribution, and branching of the nerves below the inguinal ligament, the supra-inguinal fascia iliaca (SIFI) block has a wider and more stable blocking area. In combination with the sacral plexus block, they can satisfy the needs of surgical anesthesia below the hip.
A 46-year-old man with tuberculosis, chronic obstructive pulmonary disease, pulmonary heart disease, World Health Organization (WHO) class III pulmonary hypertension and right heart dysfunction, and American Society of Anesthesiologists physical status class III needed fixation of an intramedullary nail in the left lower extremity. Additionally, he had broken his left lower limb after a recent fall. Both general anesthesia and epidural anesthesia were not appropriate.
The patient had a clear history of tuberculosis, computerized tomography scan displayed destructive pneumonophthisis. Furthermore, he had chronic obstructive pulmonary disease and pulmonary heart disease.
An ultrasound-guided SIFI combined with a sacral plexus block was successfully performed for surgical anesthesia and avoided all hemodynamic fluctuations.

Collagen colitis (CC) is a microscopic colitis diagnosed by mucosal biopsy and is extremely rare in children.
We reported a child with severe persistent diarrhea that could not be relieved with traditional diarrheal treatment. No abnormalities were found after multiple colonoscopies.
A significant increase in total IgE levels was found in the patient's blood. He had a history of mild chronic allergic rhinitis and slightly intermittent wheezing. However, we found that the child had a hyperallergic reaction to multiple respiratory antigens and had mild pulmonary dysfunction. Finally, colonoscopy with biopsy identified the diagnosis of CC.
Considering that a respiratory allergic reaction was one of the causes of diarrhea, anti-allergic treatment was given to the child, and his severe diarrhea was soon relieved. Corticosteroid treatment was suggested to the patient, but his parents firmly refused steroid therapy. According to the patient's specific allergic reaction to mites, desensitization treatment was finally chosen for him.
After 1 year of desensitization for dust mites, the patient's respiratory symptoms improved, total IgE levels decreased, autoantibodies declined, and diarrhea did not reoccur. Colonoscopy with biopsy showed a significant improvement in pathology.

The analyses were consistent with the features of pharmacists' work characteristics, including job demands, job control, and workload playing mediating roles between antecedent variables (personality and working environment) and emotional outcomes (burnout and job satisfaction). On the other hand, job control and workload did not predict emotional exhaustion.This study indicates that personality is a negative predictor of 2 dimensions of burnout (emotional exhaustion and depersonalization), while the working environment, especially job demand, is a predictor of burnout and greater emotional exhaustion. The aim of this study was to explore the differences in blood management in PTUI CS with or without prophylactic IIA BO and to evaluate the safety and efficacy of cell salvage to reduce the need for donor blood transfusion during PTUI CS.This retrospective study included all women who were diagnosed with PPP and PA and underwent PTUI CS from October 1, 2016, to October 31, 2018. Sixty-four patients were included 34 underwent prophylactic IIA BO (IIA group), whereas 30 were treated without prophylactic IIA BO (control group). The primary outcome was a composite measure of perioperative blood management outcomes, including the estimated blood loss (EBL), donor blood transfusion, salvaged blood returned, fresh frozen plasma (FFP), pre- and postoperative serum hemoglobin and hematocrit.

Further studies should examine to identify more accurate prognostic factor with large series.A 60's man came to our hospital for jaundice. Contrast-enhanced CT showed irregular thickening of the hilar bile duct, and the lymph nodes(LN)were swollen from the hilar to the abdominal aorta. These LNs showed similar findings in endoscopic ultrasonography(EUS), and fine needle aspiration cytology(FNA)was performed on the enlarged No.13LN to diagnose LN metastasis of hilar cholangiocarcinoma. Since the peri-aortic LN was also markedly enlarged, it was considered to be metastasis, and was diagnosed as unresectable hilar cholangiocarcinoma with distant LN metastasis. When gemcitabine/cisplatin therapy(GC therapy)was started, tumor markers normalized and LN decreased in 4 months. We performed GC therapy for a total of 12 cycles and did not re-exacerbate. Cholangioscopy revealed that bile duct stenosis at the hilar portion had improved. We have determined that curative resection is possible and performed surgery. We confirmed that No.16b1LN was negative by pathological diagnosis during surgery and performed left hepatic caudate lobectomy, extrahepatic cholangectomy, and biliary reconstruction. Diagnosis was pT2aN1(n8a)M0, fStage ⅢB, and pR0. A colonoscopy revealed advanced cancer in the lower rectum. Computed tomography showed a tumor larger than 5 cm in the lower rectum with metastasis to the right lateral lymph node. The patient was diagnosed with advanced locally rectal cancer, and chemoradiotherapy(35 Gy plus S-1)was added after 6 courses of mFOLFOX6, and laparoscopic abdominal perineal resection and right lateral lymph nodes dissection were performed. Histopathological examination revealed endocrine cell carcinoma(pT3[A], pN0, M0, pStage Ⅱa). Four months after the operation, recurrence was found in the pelvis, lymph nodes, and lungs, and he died 9 months after the operation. This treatment resulted in partial response(PR) after 3 months, with reduction in the sizes of metastatic lymph nodes surrounding the pancreatic head and paraaortic lesion. However, the sizes of metastatic lymph nodes increased after 7 months of chemotherapy. Ramucirumab/nab-paclitaxel was then administered as second-line chemotherapy, and the diameter of the metastatic lymph nodes subsequently decreased after 4 months of the regimen. However, progressive disease was observed at 7 months, and blood transfusion was required because of bleeding from the primary gastric tumor. Therefore, nivolumab was initiated as third-line chemotherapy 14 months after the first treatment. After nivolumab administration, a 28% reduction in metastatic lymph nodes was achieved within 3 months, together with the regression of the primary gastric tumor and improvement in anemia within 6 months. PR was achieved after 12 months of nivolumab administration, and effective disease control was maintained for 16 months without any adverse reaction to nivolumab.A 32-year-old woman was admitted our hospital due to epigastric discomfort. The patient diagnosed as having scirrhous carcinoma of the stomach by upper gastrointestinal scope. Peritoneal dissemination and ovarian metastasis were confirmed by the diagnostic laparoscopy. Therefore, combination chemotherapy with S-1 and intraperitoneal chemotherapy(ip)with docetaxel (DTX) was started. After 2 courses chemotherapy, laparoscopy was performed again. Peritoneal dissemination was scarred, but biopsy showed altered AE1/AE3 positive cells, and increased left ovarian metastasis, so systemic chemotherapy was changed to DCS chemotherapy and added DTX ip. After 4 courses chemotherapy and 7 months after the first diagnosis, subtotal gastrectomy, hysterectomy and bilateral adnexectomy were performed because the cytology and tumor marker remained within normal range. In histopathological diagnosis, the effect of chemotherapy was Grade 2 at the primary site and Grade 3 at the metastatic site. Nine years have passed since the initial diagnosis and she has no relapse with postoperative adjuvant chemotherapy.A 78-year-old man was admitted to our hospital with a diagnosis of esophageal cancer and gastric cancer. Gastroscopy showed a type 2 tumor located in the cardia from the lower esophagus, and a pathological examination showed malignant melanoma. Based on the physical examination and other imaging tests, the patient was diagnosed with primary amelanotic malignant melanoma of the esophagus, but the tumor was unresectable due to extensive lymph node metastasis. According to the guideline, immune checkpoint inhibitor(nivolumab)was used for treatment, but because the tumor progressed after 2 courses and the performance status of the patient worsened, aggressive treatment was ended. Six weeks after finishing treatment, computed tomography showed that the tumor had shrunk to some extent. The patient ultimately died from aspiration pneumonia 4 months after the first consultation. The patient was thought to have had an immune-related adverse event, with the tumor showing pseudoprogression.

The patient was diagnosed with advanced locally rectal cancer, and chemoradiotherapy(35 Gy plus S-1)was added after 6 courses of mFOLFOX6, and laparoscopic abdominal perineal resection and right lateral lymph nodes dissection were performed. Histopathological examination revealed endocrine cell carcinoma(pT3[A], pN0, M0, pStage Ⅱa). Four months after the operation, recurrence was found in the pelvis, lymph nodes, and lungs, and he died 9 months after the operation. Neuroendocrine carcinoma is relatively rare, so the further accumulation of cases and establishment of treatment methods are desired.A 66-year-old man was diagnosed with advanced gastric cancer(L, Less, Type 2, T4a[SE], N2, M1[LYM], H0, P0, cStage Ⅳ)and received treatment with S-1/cisplatin as first-line chemotherapy. This treatment resulted in partial response(PR) after 3 months, with reduction in the sizes of metastatic lymph nodes surrounding the pancreatic head and paraaortic lesion. However, the sizes of metastatic lymph nodes increased after 7 months of chemotherapy. Ramucirumab/nab-paclitaxel was then administered as second-line chemotherapy, and the diameter of the metastatic lymph nodes subsequently decreased after 4 months of the regimen. However, progressive disease was observed at 7 months, and blood transfusion was required because of bleeding from the primary gastric tumor. Therefore, nivolumab was initiated as third-line chemotherapy 14 months after the first treatment. After nivolumab administration, a 28% reduction in metastatic lymph nodes was achieved within 3 months, together with the regression of the primary gastric tumor and improvement in anemia within 6 months. PR was achieved after 12 months of nivolumab administration, and effective disease control was maintained for 16 months without any adverse reaction to nivolumab.A 32-year-old woman was admitted our hospital due to epigastric discomfort. The patient diagnosed as having scirrhous carcinoma of the stomach by upper gastrointestinal scope. Peritoneal dissemination and ovarian metastasis were confirmed by the diagnostic laparoscopy. Therefore, combination chemotherapy with S-1 and intraperitoneal chemotherapy(ip)with docetaxel (DTX) was started. After 2 courses chemotherapy, laparoscopy was performed again. Peritoneal dissemination was scarred, but biopsy showed altered AE1/AE3 positive cells, and increased left ovarian metastasis, so systemic chemotherapy was changed to DCS chemotherapy and added DTX ip. After 4 courses chemotherapy and 7 months after the first diagnosis, subtotal gastrectomy, hysterectomy and bilateral adnexectomy were performed because the cytology and tumor marker remained within normal range. In histopathological diagnosis, the effect of chemotherapy was Grade 2 at the primary site and Grade 3 at the metastatic site. Nine years have passed since the initial diagnosis and she has no relapse with postoperative adjuvant chemotherapy.A 78-year-old man was admitted to our hospital with a diagnosis of esophageal cancer and gastric cancer. Gastroscopy showed a type 2 tumor located in the cardia from the lower esophagus, and a pathological examination showed malignant melanoma. Based on the physical examination and other imaging tests, the patient was diagnosed with primary amelanotic malignant melanoma of the esophagus, but the tumor was unresectable due to extensive lymph node metastasis. According to the guideline, immune checkpoint inhibitor(nivolumab)was used for treatment, but because the tumor progressed after 2 courses and the performance status of the patient worsened, aggressive treatment was ended. Six weeks after finishing treatment, computed tomography showed that the tumor had shrunk to some extent. The patient ultimately died from aspiration pneumonia 4 months after the first consultation.
