To overcome different opinions and choices regarding guided surgery, this article explains an evidence-based literature review to assess its various outcomes and allowing informed choices before using various guided surgical techniques based on its expectation and reality outcomes. This highlights a clinician's choice to guide his successful implant surgery without causing distress in the midway of treatment. An online search was done on PubMed/Medline database to bring in accuracy to the expertise. This review includes reference of publications from 2000 to 2019, which is related to promising outcomes using computer-assisted static or dynamic navigation system for the placement of implant. Out of these, 809 were related to the computer-guided implant placement. Relevant papers were chosen in accordance with the inclusion and exclusion criteria. This review article contemplates to reflect the fact that computer-guided approach is considered to offer more predictable, safer, and faster implant placement with the predetermined final prosthetic outfit. Thus, digital planning and placing of dental implants in the correct position keep escalating to a higher achievement levels than a classical freehand approach. Nevertheless, this guided surgical approach also holds some errors and risks, which must be identified and rectified.COVID-19 pandemic represents an unprecedented global health crisis in the last 100 years. In this article, we review some of the important mathematical models used to support the ongoing planning and response efforts. These models differ in their use, their mathematical form and their scope.Globally, countries have resorted to social distancing, travel restrictions and economic lockdowns to reduce transmission of COVID-19. The socioeconomic costs of these blunt measures are expected to be high, especially in sub-Saharan Africa where many live hand-to-mouth and lack social safety nets. Social Accounting Matrix multiplier model results show that Ghana's urban lockdown, although in force for only three weeks in April 2020, has likely caused GDP to fall by 27.9% during that period, while an additional 3.8 million Ghanaians temporarily became poor. Compared to the government's revised GDP growth rate of 1.5% for 2020, the model predicts a contraction of 0.6 to 6.3% for 2020, depending on the speed of the recovery. The US$200 million budgeted for Ghana's Coronavirus Alleviation Program will close only a small part of the estimated US$ 2.3 billion GDP gap between the fast recovery scenario and government's revised GDP trajectory.This article adds a **** needed microlevel perspective to the literature on interactions between civil society organizations and governments. I argue that a microlevel perspective assists in making connections between two dominant streams in the literature on government-CSO relations an empirical-analytical stream and a critical stream. It aims to better understand the interactions and relations, by analysing the institutional work done by CSOs' members. Adopting this approach puts CSO members in a more agentic position. Interactional processes are brought to the centre of analysis. The Dutch Community Sport Coach programme was used as a case to illustrate the usefulness of the approach. Through a one-year organizational ethnography, the article scrutinizes the way in which members of one CSO enact the organization's service delivery relationship with a municipality. Through a multidimensional perspective on agency, the analysis shows how individual CSO members act as embedded agents that assimilate a public logic into the dominant community logic. It further shows the CSO's members efforts and struggle to maintain their community logic. The article argues that an analysis of the microfoundations of government-civil society organization relations foregrounds the multivocality of the relationship as foundational.
    Nursing quality is one of today's priorities in health care. The COVID-19 pandemic had posed particular challenges for nursing practice. There is currently only limited information available about nursing care in health care institutions during the COVID-19 pandemic.

    Therefore the aim of this study was to describe the quality of nursing care during the COVID-19 pandemic with respect to the available structural circumstances (structure), the applied interventions (process) and the perceived stress of nursing staff (outcome).

    A cross-sectional study using an electronic questionnaire was carried out in different health care institutions. The questionnaire includes general questions as well as questions on structure, process and outcome indicators.

    2605 nurses participated in this online survey. The study showed that nurses were often provided with guidelines and trainings on protective measures. About two thirds of the nurses reported a sufficient availability of face masks, yet half of the participants wore them longer than eight hours.

Our results support the use of TTBEs as dietary supplements for anxiolytic purposes and unveil age and gender-dependent responses.Identifying perturbed pathways at an individual level is important to discover the causes of cancer and develop individualized custom therapeutic strategies. Though prognostic gene lists have had success in prognosis prediction, using single genes that are related to the relevant system or specific network cannot fully reveal the process of tumorigenesis. We hypothesize that in individual samples, the disruption of transcription homeostasis can influence the occurrence, development, and metastasis of tumors and has implications for patient survival outcomes. Here, we introduced the individual-level pathway score, which can measure the correlation perturbation of the pathways in a single sample well. We applied this method to the expression data of 16 different cancer types from The Cancer Genome Atlas (TCGA) database. Our results indicate that different cancer types as well as their tumor-adjacent tissues can be clearly distinguished by the individual-level pathway score. Additionally, we found that there was strong heterogeneity among different cancer types and the percentage of perturbed pathways as well as the perturbation proportions of tumor samples in each pathway were significantly different. Finally, the prognosis-related pathways of different cancer types were obtained by survival analysis. We demonstrated that the individual-level pathway score (iPS) is capable of classifying cancer types and identifying some key prognosis-related pathways.To improve the standard point positioning (SPP) accuracy of integrated BDS (BeiDou Navigation Satellite System)/GPS (Global Positioning System) at the receiver end, a novel approach based on Long Short-Term Memory (LSTM) error correction recurrent neural network is proposed and implemented to reduce the error caused by multiple sources. On the basis of the weighted least square (WLS) method and Kalman filter, the proposed LSTM-based algorithms, named WLS-LSTM and Kalman-LSTM error correction methods, are used to predict the positioning error of the next epoch, and the prediction result is used to correct the next epoch error. Based on the measured data, the results of the weighted least square method, the Kalman filter method and the LSTM error correction method were compared and analyzed. The dynamic test was also conducted, and the experimental results in dynamic scenarios were analyzed. From the experimental results, the three-dimensional point positioning error of Kalman-LSTM error correction method is 1.038 m, while the error of weighted least square method, Kalman filter and WLS-LSTM error correction method are 3.498, 3.406 and 1.782 m, respectively. https://www.selleckchem.com/MEK.html The positioning error is 3.7399 m and the corrected positioning error is 0.7493 m in a dynamic scene. While physical intimate partner violence is covered by a wide range of behaviors, sexual intimate partner violence (SIPV) is often only measured through attempted or completed forced sex, ignoring less obvious forms of sexual intimate partner violence. We explored women's conceptualizations of SIPV by conducting in-depth interviews with 18 Tanzanian women. Using a thematic approach, we identified key features of women's sexual intimate relationships and their perceptions of them. The women clearly defined acts of positive sexual relationships that occurred with mutual consent and seduction and SIPV that included acts of forced sex and sex under the threat of violence. They also identified several acts that were crossing the line, whereby a discrepancy of views existed whether they constituted SIPV, such as having sex when out of the mood, sex being the duty of the wife, sex during the menses, requests for anal sex, having sex to not lose the husband, husband refusing sex and husband having other partners. Women in this study felt violated by a far wider range of sexual acts in their relationships. Future studies need to improve the measurement of sexual intimate partner violence to allow the collection of encompassing, yet comparable, data on this harmful phenomenon.The Internet of Things (IoT) has evolved significantly with advances in gathering data that can be extracted to provide knowledge and facilitate decision-making processes. Currently, IoT data analytics encountered challenges such as growing data volumes collected by IoT devices and fast response requirements for time-sensitive applications in which traditional Cloud-based solution is unable to meet due to bandwidth and high latency limitations. In this paper, we develop a distributed analytics framework for fog-enabled IoT systems aiming to avoid raw data movement and reduce latency. The distributed framework leverages the computational capacities of all the participants such as edge devices and fog nodes and allows them to obtain the global optimal solution locally. To further enhance the privacy of data holders in the system, a privacy-preserving protocol is proposed using cryptographic schemes. Security analysis was conducted and it verified that exact private information about any edge device's raw data would not be inferred by an honest-but-curious neighbor in the proposed secure protocol.

    Our objective was to evaluate the effects of bone-anchored maxillary protraction (BAMP) on the status of the secondary alveolar bone graft in patients with unilateral complete cleft lip and palate (UCLP).

    The experimental group (EG) comprised 26 patients with UCLP, mean age of 11.9years, submitted to secondary alveolar bone grafting (SABG) with recombinant bone morphogenetic protein, and BAMP therapy, using miniplate-borne Class III intermaxillary elastics. Cone beam computed tomography (CBCT) examinations were taken 6months after SABG and before BAMP (T1) and after 18months of BAMP therapy (T2). The control group (CG) was composed of 24 patients with UCLP submitted only to SABG with recombinant bone morphogenetic protein or autogenous bone from iliac crest without BAMP therapy, matched by initial age and sex with the EG. In the CG, CBCT examinations were performed 6months (T1) and 12months (T2) after SABG surgery. CBCT axial sections were analyzed using Garib scores in both time points. Intra- and intergroup comparisons were performed using Wilcoxon and Mann-Whitney tests, respectively (P<0.05).

    No intergroup differences were found at T1 and T2. The EG showed significant improvement of graft status from T1 to T2 at the cervical and middle levels of the alveolar cleft. No significant interphase differences were found for graft scores in the CG.

    A diverse range of outcomes is used in orthodontic research with a focus on measuring outcomes important to clinicians and little consistency in outcome selection and measurement. We aimed to develop a core outcome set for use in clinical trials of orthodontic treatment not involving cleft or orthognathic patient groups.

    A list of outcomes measured in previous orthodontic research was identified through a scoping literature review. Additional outcomes of importance to patients were obtained using qualitative interviews and focus groups with adolescents aged 10-16years. Rating of outcomes was carried out in a 2-round electronic Delphi process involving health care professionals and patients using a 9-point scale. A face-to-face meeting was subsequently held with stakeholders to discuss the results before refining the core outcome set.

    After triangulation, a final list of 34 outcomes grouped under 10 domains was obtained for rating in the e-Delphi surveys. Fifteen outcomes were voted "in" after the second Delphi round involving 274 participants with a further outcome being included after the consensus meeting. These were subsequently refined into a final set of 7 core outcomes, including the impact of self-perceived esthetics, alignment and/or occlusion, skeletal relationship, stability, patient-related adherence, breakages, and adverse effects on teeth or teeth-supporting structures.

Additional outcomes of importance to patients were obtained using qualitative interviews and focus groups with adolescents aged 10-16years. Rating of outcomes was carried out in a 2-round electronic Delphi process involving health care professionals and patients using a 9-point scale. A face-to-face meeting was subsequently held with stakeholders to discuss the results before refining the core outcome set. To standardize the measurement of urine albumin, a reference measurement procedure (RMP) and urine albumin certified reference materials (CRMs) are needed.

    A candidate RMP for urine albumin based on liquid chromatography-isotope dilution tandem mass spectrometry (LC-IDMS/MS) using human serum albumin as calibration standard was developed. Isotope-labeled human albumin was used as internal standard. Urine samples were digested using trypsin and eight resulting "signature" peptides of albumin were quantified by LC-IDMS/MS. The candidate RMP was employed in value assignment of external quality assessment (EQA) samples and certification of urine albumin reference materials. The commutability of the developed CRMs was assessed against patient samples.

    The candidate RMP (recovery 101.5-103.2% and CV 1.2-3.3% at about 7-40mg/L) met optimal performance goal. The lower limit of quantification was 0.03mg/L as determined by signal-to-noise method. The EQA results from clinical laboratories using different immunoturbidimetric methods were generally comparable with assigned target values determined by the candidate RMP, with albumin concentrations ranging from 5 to 226mg/L. Optical and MR imaging was performed to assess the targeted delivery. Biodistribution and histology was performed to determine the distribution of oxaliplatin. Results L-NIR-Fe3O4/OX presented a significant increase of oxaliplatin release (~18%) and lower cell viability after AMF exposure (p less then 0.001). Optical imaging showed a significant release of oxaliplatin among mesenteric vein injected (MV) group of animals. MR imaging on MV injected animals showed R2* changes in the tumor regions at the same regions immediately after infusion compared to the surrounding liver (p less then 0.001). Methods GEPIA, OncoLnc, PROGgeneV2 and HPA database were utilized to analyze the expression (transcriptome and protein levels) and overall survival of YAP1 in PC. Then, we evaluated the risk factors associated with overall survival based on public data from TCGA-PAAD via Cox regression. Besides, LinkedOmics was utilized to identify co-expression genes and the potential regulation network of YAP1. Furthermore, we explored the relationship between YAP1 and immune infiltration using CIBERSORT algorithm and GEPIA database. Results The age, lymph node metastasis status and up-regulated YAP1 expression have been proved to be independent prognostic factors for poor prognosis. The functions of YAP1 and co-expression genes were mainly involved in the angiogenesis, immune response-regulating signaling pathway, regulation of actin cytoskeleton, NOD-like receptor signaling pathway and cytokine-cytokine receptor interaction. Differentiation antagonizing non-protein coding RNA (DANCR), a well-known lncRNA that is aberrant expression in various tumors, including hepatocellular carcinoma, gastric cancer, colorectal cancer, breast cancer, lung cancer and glioma and so on, in which it functions as oncogene mainly, contributing to cancer development and progression. High expressed DANCR is correlated with poor prognosis. In the present review, we summarize recent progression concerning the role, potential clinical utilities and underlying molecular mechanisms of DANCR related to occurrence and development of multiple cancers.Background Pancreatic cancer (PC) is one of the most common digestive malignancy, with severe cancer-related death and disease burden. Yes-associated protein 1 (YAP1) has been reported to be involved in the tumorigenesis and progression of several cancers, thus leading to poor prognosis of patients. However, the relationship between YAP1 and immune microenvironment in PC deserve more scrutiny. Methods GEPIA, OncoLnc, PROGgeneV2 and HPA database were utilized to analyze the expression (transcriptome and protein levels) and overall survival of YAP1 in PC. Then, we evaluated the risk factors associated with overall survival based on public data from TCGA-PAAD via Cox regression. Besides, LinkedOmics was utilized to identify co-expression genes and the potential regulation network of YAP1. Furthermore, we explored the relationship between YAP1 and immune infiltration using CIBERSORT algorithm and GEPIA database. Results The age, lymph node metastasis status and up-regulated YAP1 expression have been proved to be independent prognostic factors for poor prognosis. The functions of YAP1 and co-expression genes were mainly involved in the angiogenesis, immune response-regulating signaling pathway, regulation of actin cytoskeleton, NOD-like receptor signaling pathway and cytokine-cytokine receptor interaction. Specifically, increased YAP1 expression was significantly correlated with immune infiltrating levels of resting CD4+T cells.   • Mutations in collagen XII have been recently described to cause modifications in the connective tissue which phenotypic demonstration resembles that of collagen VI related myopathies. We describe a Caucasian 14-year old girl who complained to her doctor about not being able to practice schools' sports due to recurrent clavicle dislocation when trying to throw a ball. In addition, when questioned, she also reported a notion of lower strength in the upper limbs when compared to same age peers. Based on the clinical exam performed, a joint hypermobility with a slight reduction of the muscular strength of the upper limbs diagnosis was noted. Targeted sequencing identified a heterozygous missense mutation in COL12A1 - c.8336G > A (p. Arg2779His). Algorithms developed to predict the effect of the changes on the protein structure and function do not agree on the potential impact of this modification. This case shows the importance to consider collagen XII-related disorders when in presence of patients with an overlapping phenotype with both muscle and connective tissue abnormalities, once mutations in collagen VI have been excluded.
    Anxiety is characterized by psychological, physiological, and cognitive complaints. Current treatments have significant limitations, and often overlook any potential benefits to common cognitive symptoms, notably attention and executive function issues. The current study aimed to investigate the use of transcranial direct current stimulation (tDCS) on both common anxiety symptoms and executive function abilities in a college aged sample.

    As this is one of the first large scale anxiety studies utilizing tDCS, participants were given a single session of tDCS (anodal, cathodal, or sham) for 20min at 2mA over the left dorsolateral prefrontal cortex (lDLPFC). Participants also completed a series of self-reported anxiety measures and measures of executive functioning (Rey-O Copy and Wisconsin Card Sorting Task).

    While there were no differences in anxiety between tDCS groups, a trend was noted demonstrating better performance on Rey-O Copy for the cathodal group. Anxiety increased pre to post for all groups.

    Overall, results suggest that while anodal stimulation of the lDLPFC may benefit cognitive abilities for this population, targeting psychological symptoms of anxiety likely requires stimulation over other cortex, possibly right DLPFC. Further, the use of tDCS, whether active or sham, may be distressing to patients.

  • An 87-year-old male having a history of C3-7 open-door cervical laminoplasty 20 years ago fell and sustained neck pain and paralysis with complete motor and sensory deficits below C6 (Frankel A). Computed tomography (CT) revealed ankylosis from C2 to C7 due to diffuse idiopathic skeletal hyperostosis (DISH) and a C5/6 fracture with C5 posterior displacement. We performed surgery the day after injury using a posterior approach for stabilization of the spinal column from C3 to T1. Translaminar screws (LS) were placed to the right (hinge side) of C3-7, lateral mass screws (LMS) to the left (open side) of C3-6, and pedicle screws to the left of C7 and bilaterally in T1. Bony fusion was achieved as seen on CT images 6 months after surgery. We conclude that long posterior stabilization using LMS and LS is an effective treatment for cervical fracture in patients with DISH having a history of cervical laminoplasty.Aneurysmal bone cysts (ABCs) are benign bone lesions mainly occurring at the metaphyseal end of long bones and are a rarity in the calvarium. Radiological investigations and histology revealed that the lesion was an ABC. A total surgical excision was achieved despite its intracranial extension along with the involvement of dura. Prognosis is excellent with total removal as a total surgical removal of the lesion is considered curative. The rarity of the lesion along with a good surgical result despite an intracranial extension with dural involvement prompted this report.Medulloblastoma (MB) is very rare malignant primary brain tumor in adults and its location at cerebellopontine angle (CPA) is even rarer. There are only few case reports of CPA MBs in literature. Most of them are reported in pediatric age groups which are mostly intraaxial. Only 11 cases of extra-axial CPA MBs have been reported so far. The author is reporting a very rare case of adult extra-axial CPA MBs with multiple intracranial metastases. We are also reviewing clinical and radiological features and unusual way of its presentation along with surgical management. Keeping MBs as one of the differential diagnosis for extra-axial CPA tumors is going to increase our spectrum of diagnosis which can lead to alteration in management.L4 and L5 fractures are different from those at the thoracolumbar area.     A 59-year-old male diagnosed with stage III carcinoma of the external auditory canal underwent cisplatin chemotherapy. Seven days after the last dose, he presented with a disturbance of consciousness due to basilar artery occlusion, which was associated with chemotherapy administration. The patient recovered consciousness after thrombectomy. Interestingly, an increase in serum von Willebrand factor (vWf) activity was observed. The vWf activity level gradually normalized 5 months after cisplatin administration.

However, the posterior epidural migration is rare. We present a strange situation encountered during surgical decompression of the posterior migrated fragment. A 72-year-old male presented with a history of pain radiating to the left lower limb and Grade 3 power of the extensor hallucis longus. Magnetic resonance imaging revealed a prolapsed intervertebral disc and a possible posterior epidural migration of disc fragment. Routine surgical steps for microdiscectomy were followed after confirmation of level using fluoroscopy. However, the extruded disc fragment was not seen, and both exiting and traversing roots were free with adequate mobility. After extensively searching for a disc in the spinal canal, suction fluid was filtered through a surgical mop used as a sieve. Material collected was sent for histopathological study. Biopsy report confirmed material filtered was indeed the intervertebral disc. Thus, accidental suction of disc material in case of the posterior epidural migrated disc is a possibility, and we should be vigilant about this scenario to avoid disaster.

A 59-year-old male diagnosed with stage III carcinoma of the external auditory canal underwent cisplatin chemotherapy. Seven days after the last dose, he presented with a disturbance of consciousness due to basilar artery occlusion, which was associated with chemotherapy administration. The patient recovered consciousness after thrombectomy. Interestingly, an increase in serum von Willebrand factor (vWf) activity was observed. The vWf activity level gradually normalized 5 months after cisplatin administration. Endothelial injuries could be responsible for cisplatin-associated cerebral infarction. Magnetic resonance imaging revealed a prolapsed intervertebral disc and a possible posterior epidural migration of disc fragment. Routine surgical steps for microdiscectomy were followed after confirmation of level using fluoroscopy. However, the extruded disc fragment was not seen, and both exiting and traversing roots were free with adequate mobility. After extensively searching for a disc in the spinal canal, suction fluid was filtered through a surgical mop used as a sieve. Material collected was sent for histopathological study. Biopsy report confirmed material filtered was indeed the intervertebral disc. Thus, accidental suction of disc material in case of the posterior epidural migrated disc is a possibility, and we should be vigilant about this scenario to avoid disaster.

  • The objective of this study is to report the clinical characteristics and treatment of patients with progressive cerebellar ataxia associated with anti-GAD antibodies. We performed a retrospective review of all patients with anti-GAD ataxia managed at the Sheffield Ataxia Centre over the last 25 years. We identified 50 patients (62% females) with anti-GAD ataxia. The prevalence was 2.5% amongst 2000 patients with progressive ataxia of various causes. Mean age at onset was 55 and mean duration 8 years. Gaze-evoked nystagmus was present in 26%, cerebellar dysarthria in 26%, limb ataxia in 44% and gait ataxia in 100%. Nine patients (18%) had severe, 12 (24%) moderate and 29 (58%) mild ataxia. Ninety percent of patients had a history of additional autoimmune diseases. Family history of autoimmune diseases was seen in 52%. Baseline MR spectroscopy of the vermis was abnormal at presentation in 72%. Thirty-five patients (70%) had serological evidence of gluten sensitivity. All 35 went on gluten-free diet (GFD). Eighteen (51%) improved, 13 (37%) stabilised, 3 have started the GFD too recently to draw conclusions and one deteriorated. Mycophenolate was used in 16 patients, 7 (44%) improved, 2 stabilised, 6 have started the medication too recently to draw conclusions and one did not tolerate the drug. There is considerable overlap between anti-GAD ataxia and gluten ataxia. For those patients with both, strict GFD alone can be an effective treatment. To achieve this, three donated embalmed cadavers were used to simulate a case in which an attempted dismemberment and burning had occurred. Fifty-five pre-burning injuries were manually induced 30 using a machete to inflict chopping trauma, and 25 with a serrated bread knife to inflict sharp force trauma, on the thigh, knee, ankle and wrist. The cadavers were cremated in a furnace at Madrid's Cementerio Sur and the burnt remains were analysed at the Laboratorio de Antropología y Odontología Forense of the Universidad Complutense de Madrid. Not all pre-burning injuries inflicted were visible after the cremation process; only 13% were detected in this experiment. Toolmarks can be masked, modified, destroyed or overlooked from the outset of the procedure due to several factors which influence the post-burning survival and detection of toolmarks and contribute to conceal the evidence of trauma. The expression of VrLELP was higher in leaves and pods and lower in nodule roots relative to other tissues. The expression of VrLELP varied during flower development. The expression of VrLELP also varied during the day, reaching a peak after 12 h of illumination under long-day conditions. In contrast, under short-day conditions, the abundance of VrLELP transcripts changed little throughout the day. In addition, VrLELP delayed flowering time in transgenic Arabidopsis plants by suppressing the expression of the flowering-time genes CO and FT under short-day conditions. However, VrLELP did not affect flowering time under long-day conditions in Arabidopsis.     Of the 151 dengue-infected patients with severe liver injury or ALF, 51% were 7.3% specificity. Regarding mortality prediction, the deterioration of liver function to ALF was the most significant factor related to death in DISH patients (aOR 108.5, 95%CI 5.5-2145.4,
    = 0.002). Other independent factors associated with death included baseline INR (aOR 10.4, 95%CI 2.6-40.5,
