PURPOSE To assess the impact of measurement error on cylinder treatment planning in corneal refractive surgery and to compare the reliability of two indices of cylinder change the Correction Index, based on the surgically induced astigmatism, and the Flattening Index, based on the flattening effect. Targeting undercorrection minimized postoperative cylinder compared with targeting full correction, with the optimal target dependent on the amount of random error in refraction measurement. It was optimal to partially treat 0.25 D of cylinder, even if the presumed level of measurement error was relatively high. CONCLUSIONS The Flattening Index outperforms the Correction Index when accounting for errors in preoperative and postoperative cylinder measurement, particularly when pre-operative cylinder is 1.00 D or less. Treatment nomograms should be adjusted to use the Flattening Index to assess cylinder change, and reporting guidelines should be updated to include the Flattening Index. METHODS A total of 40 eyes from 20 participants were prospectively randomized to receive WFG or WFO PRK with the WaveLight Allegretto Eye-Q 400-Hz excimer laser (Alcon Laboratories, Inc., Fort Worth, TX). The primary outcome measured was uncorrected distance visual acuity (UDVA) at 12 months postoperatively. Secondary outcome measures included corrected distance visual acuity (CDVA), less than 5% and less than 25% contrast visual acuity, changes in higher order aberrations (HOAs), and self-reported quality of vision. Patients completed a validated, Rasch-tested, linear-scaled 30-item questionnaire assessing the quality of vision and visual symptoms preoperatively and at each visit. RESULTS There was no statistically significant difference (P > .05) between the WFG and WFO groups in the frequency of attaining a refractive error within ±0.25 diopters of emmetropia, achieving postoperative UDVA of 20/16 or better or 20/20 or better, losing one or two or more lines or maintaining their preoperative CDVA, or differences in UDVA, CDVA, low contrast acuity, or HOAs. None of the measured quality of vision parameters exhibited statistically significant differences between the groups preoperatively or at 12 months postoperatively (all P > .05). The WFG and WFO groups each showed significant improvement in multiple subjective measures compared to pretreatment values in the same eye. A total of 12.5% of patients preferred their WFG PRK eye, 31.25% preferred their WFO PRK eye, and 56.25% had no preference at 12 months postoperatively. All identified patients were then prospectively examined at their next regular postoperative follow-up visit and presented with the standardized and clinically validated Quality of Vision questionnaire to gauge patient-reported postoperative visual quality. RESULTS A total of 80 eyes (40 patients) were eligible for 11 matching. Mean postoperative follow-up was 27.8 ± 14.3 months in the SMILE gretter uncorrected distance visual acuity, fewer higher order aberrations, and better subjective quality of vision than SMILE. After the sitting position reference axis was registered with IOLMaster 700 (Carl Zeiss Meditec AG), these data were transferred to the Callisto eye system, which was connected to the operating VisuMax microscope (Carl Zeiss Meditec AG). Cyclotorsion was measured by activating the Z-align function and compensated for by repositioning the patient's body or tilting the head until the reference axis from the IOLMaster 700 (0-180) was parallel to a manually drawn reference axis on the screen (0-180) before docking. The visual and refractive results were studied preoperatively and postoperatively.

This study explored the nature and frequency of healthcare utilization for mental health problems among uninsured/uninsurable Latinos who received outpatient care between 2012 and 2015 through an academic hospital-affiliated program that covers primary and specialty services to uninsured patients without regard to documentation status. Encounters for mental health disorders were the most common category, accounting for 14.88% of all visits. Mood (78%) and anxiety disorders (16%) were the most prevalent mental health diagnoses. The most frequent reason to seek care was symptom, signs, and ill-defined conditions (37.47%), and within this subgroup, pain was the leading cause of seeking care (88%), which may indicate high rates of somatization of mental health distress. This study presents a unique opportunity to explore the burden and nature of mental health needs among a population for which healthcare information is rarely attainable and highlights the need for culturally competent screening mechanisms and interventions to address the stressors faced by emergent communities.Chromosome painting is a useful technique for distinguishing specific chromosomes (fragments), elucidating the genetic relationships of different genomes or chromosomes, and identifying chromosomal rearrangements. In tetraploid Ae. ventricosa, both 4D and 4M could be painted; however, the signal intensity of 4M was less compared with 4D. No painted chromosome was observed for the other alien species. This indicated that the relationship among D/M/U was closer than that among D/A/B as well as D with genomes H/R/Ss/Sc/Y/P/N/J. The poor biodegradability of EDTA makes itself a persistent substance in the natural environment. This work provided a fluorescence "on-off-on" strategy for the sequential determination of trace Cu2+ and EDTA. Amino-functionalized graphene quantum dots (afGQDs) were synthetized via the thermal pyrolysis of citric acid. Fluorescence resonance energy transfer (FRET) between afGQDs and 1-(2-pyridylazo)-2-naphthol (PAN) effectively quenched the fluorescence of this carbon-based nanomaterial. The generation of the Cu2+-PAN complex caused the increased FRET efficiency and the further fluorescence decline. The change of the fluorescence intensity sensitively responded to copper ions. The linear range and the limit of detection (LOD) were 1 nM-10 μM and 0.87 nM, respectively. EDTA could decompose the Cu2+-PAN complex and liberate PAN, which weakened the FRET efficiency and led to the fluorescence recovery. The increasing degree of the fluorescence intensity was closely related to EDTA within a concentration range from 10 nM to 10 μM with a LOD at 4 nM. Copper ions in the water and human serum samples and EDTA in the trypsin-EDTA sample were successfully detected based on the proposed fluorescence method.Diagnostic terms in psychiatry like 'schizophrenia' and 'bipolar disorder' are deeply contested in the professional community, by mental health activists and the public. In this paper, we provide a theoretical framework for considering diagnostic terms as ordinary linguistic expressions and illustrate this approach by a corpus linguistic analysis of 'schizophrenia.' Our aim is to show how a focus on language itself can inform current and future debates about psychiatric terminology and provide new insights on relevant processes concerning their actual usage and change over time. We hope that this contributes to enhancing mutual understanding between different discourse spheres and stakeholders.Pressurized intraperitoneal aerosol chemotherapy (PIPAC) represents a novel approach to deliver intraperitoneal chemotherapy. We report our experience with PIPAC in patients with peritoneal metastasis (PM) from gastric cancer (GC). Data from GC patients (n = 20) included in the prospective PIPAC-OPC1 and PIPAC-OPC2 studies are reported. All patients had received prior systemic chemotherapy. The mean peritoneal cancer index (PCI) was 10.5 (range 0-39) and nine patients had diffuse GC. PIPAC with cisplatin 7.5 mg/m2 and doxorubicin 1.5 mg/m2 were administered at 4-6-week intervals. Outcome criteria were objective tumour response, survival and adverse events. Twenty patients had 52 PIPAC procedures with a median follow-up of 10.4 months (3.3-26.5). Median survival from the time of PM diagnosis and after the first PIPAC procedure was 11.5 months and 4.7 months, respectively. Fourteen patients had repeated PIPAC (> 2), and the objective tumour response according to the histological peritoneal regression grading score (PRGS) was observed in 36%, whereas 36% had stable disease.

Lessons learned from experienced trainers represent a vital distillation of first-hand experience into widely applicable knowledge to be used to reduce potential failures and achieve desired outcomes. Findings from this study offer further guidance on best practices for implementing and sustaining HBS programs and provide insight into challenges and successes experienced by HBS trainers.

Findings from this study offer further guidance on best practices for implementing and sustaining HBS programs and provide insight into challenges and successes experienced by HBS trainers.The educational pedagogy surrounding Helping Babies Breathe (HBB) has been transformative in going beyond a curriculum focused only on basic neonatal resuscitation; indeed, it created the framework for an educational program that has served as a model for replication for other impactful programs, such as the Helping Mothers Survive and other Helping Babies Survive curricula. The tenets of HBB include incorporation of innovative learning strategies such as small group discussion, skills-based learning, simulation and debriefing, and peer-to-peer learning, all of which begin the hard work of changing behaviors that may eventually affect health care systems. Allowing for adaptation for local resources and culture, HBB has catalyzed innovation in the development of simplified, pictorial educational materials, in addition to low-tech yet realistic simulators and adjunct devices that have played an important role in empowering health care professionals in their care of newborns, thereby improving outcomes. In this review, we describe the development of HBB as an educational program, the importance of field testing and input from multiple stakeholders including frontline workers, the strategies behind the components of educational materials, and the impact of its pedagogy on learning.Helping Babies Breathe (HBB) addresses a major cause of newborn mortality by teaching basic steps of neonatal resuscitation and improving survival rates of infants affected by intrapartum-related events or asphyxia. Addressing the additional top causes of mortality (infection and prematurity) requires more comprehensive education, including content on thermal and nutritional support, breastfeeding, and alternative feeding strategies, as well as recognition and treatment of infection. Essential Care for Every Baby (ECEB) and Essential Care for Small Babies (ECSB) use educational principles developed with HBB as a model for teaching basic newborn care. The tenets of HBB include incorporation of innovative learning strategies such as small group discussion, skills-based learning, simulation and debriefing, and peer-to-peer learning, all of which begin the hard work of changing behaviors that may eventually affect health care systems. Allowing for adaptation for local resources and culture, HBB has catalyzed innovation in the development of simplified, pictorial educational materials, in addition to low-tech yet realistic simulators and adjunct devices that have played an important role in empowering health care professionals in their care of newborns, thereby improving outcomes. Atomic force microscopy was used to characterize molecular interactions of LOV2 in open and closed states with ZDK at a single-molecule level. The closed state of LOV2 enables strong binding with ZDK, characterized by a 60-fold lower dissociation rate and a ∼1.5-times higher activation energy barrier than for its open state. As-prepared microstructures possess a narrowed recombination region, which dramatically increases the current density by 3 orders of magnitude under pulsed operation, compared with the corresponding thin-film devices, representing a promising solution-processed device platform for electrical pumping. Under programmable electrical excitation, both static and dynamic displays were demonstrated with such microscale LEC arrays as display panels. Here we use two microbial rhodopsins, Gloeobacter rhodopsin and sodium ion pump rhodopsin (NaR), to provide the first experimental and theoretical evidence that the twist direction of the retinal chromophore indeed determines the sign of the ROA spectrum. We disrupt the hydrogen bond responsible for the distortion of the retinal in NaR and show that the sign of the ROA signals of this nonfunctional mutant is flipped. The reported ROA spectra are monosignate, a property that has been seen for a variety of photoreceptors, which we attribute to an energetically favorable gradual curvature of the chromophore.

A machine learning (ML)-based clinical tool to identify responders would prevent unnecessary surgery, while targeting non-responders for early intervention. We used regional left ventricular (LV) contractile injury patterns in ML models to identify IDCM HF non-responders. MRI-based multiparametric strain analysis was performed in 178 test subjects (140 normal subjects and 38 IDCM patients), calculating longitudinal, circumferential, and radial strain over 18 LV sub-regions for inclusion in ML analyses. Patients were identified as responders based upon symptomatic and contractile improvement on medical therapy. We tested the predictive accuracy of support vector machines (SVM), logistic regression (LR), random forest (RF), and deep neural networks (DNN). The DNN model outperformed other models, predicting response to medical therapy with an area under the receiver operating characteristic curve (AUC) of 0.94. The top features were longitudinal strain in (1) basal anterior, posterolateral and (2) mid posterior, anterolateral, and anteroseptal sub-regions. Simultaneously, the clinician provided the evaluation of the tasks based on the MDS-UPDRS scores. One hundred-ninety kinematic features were extracted from the inertial motor data. Data processing and statistical analysis identified a set of parameters able to distinguish between HC and PwPD. Then, multiple feature selection methods allowed selecting the best subset of parameters for obtaining the greatest accuracy when used as input for several predicting regression algorithms. The maximum correlation coefficient, equal to 0.814, was obtained with the adaptive neuro-fuzzy inference system (ANFIS). However, the ways in which these interact to predict behaviour problems have yet to be examined using objective sleep measurement. The lack of objective evidence for these relationships in young children has recently been defined by the World Health Organization (2019) as a gap in the field. We thus aimed to test the interacting effects of screen exposure and objectively measured sleep on behaviour problems in the preschool age. A total of 145 children aged 3-to-6-years participated in this cross-sectional study. Sleep was assessed objectively using actigraphy for 1-week, and subjectively using parent-reported daily sleep diaries. Parents reported the child's daily duration of screen exposure, and completed the Strengths and Difficulties Questionnaire. Results showed that actigraphic sleep duration, timing and efficiency were associated with screen exposure. The link between screen time and behaviour problems was moderated by sleep duration, as it was significant only for children with sleep duration of 9.88 h or less per night. Sleep duration also moderated the relation between screen time and externalizing-but not internalizing-problems. Hence, the combination of increased screen exposure and decreased sleep duration may be particularly adverse for child mental health. While these key relationships should be further examined in longitudinal and experimental investigations, our findings shed light on their complexity, underscoring the importance of the moderating role of sleep.As mental health systems move towards person-centred care, outcome measurement in clinical research and practice should track changes that matter to young people and their families. This study mapped the types of change described by three key stakeholder groups following psychotherapy for depression, and compared the salience of these outcomes with the frequency of their measurement in recent quantitative treatment effectiveness studies for adolescent depression.Using qualitative content analysis, this study identified and categorized outcomes across 102 semi-structured interviews that were conducted with depressed adolescents, their parents, and therapists, as part of a randomized superiority trial. Adolescents had been allocated to Cognitive-Behavioral Therapy, Short-Term Psychoanalytic Psychotherapy, or a Brief Psychosocial Intervention.The study mapped seven high-level outcome domains and 29 outcome categories. On average, participants discussed change in four domains and six outcome categories. The most frequently discussed outcome was an improvement in mood and affect (i.e., core depressive symptoms), but close to half of the participants also described changes in family functioning, coping and resilience, academic functioning, or social functioning. Coping had specific importance for adolescents, while parents and therapists showed particular interest in academic functioning. There was some variation in the outcomes discussed beyond these core themes, across stakeholder groups and treatment arms.Of the outcomes that were frequently discussed in stakeholder narratives, only symptomatic change has been commonly reported in recent treatment studies for adolescent depression. A shift towards considering multiple outcome domains and perspectives is needed to reflect stakeholder priorities and enable more nuanced insights into change processes.

We used regional left ventricular (LV) contractile injury patterns in ML models to identify IDCM HF non-responders. MRI-based multiparametric strain analysis was performed in 178 test subjects (140 normal subjects and 38 IDCM patients), calculating longitudinal, circumferential, and radial strain over 18 LV sub-regions for inclusion in ML analyses. Patients were identified as responders based upon symptomatic and contractile improvement on medical therapy. We tested the predictive accuracy of support vector machines (SVM), logistic regression (LR), random forest (RF), and deep neural networks (DNN). The DNN model outperformed other models, predicting response to medical therapy with an area under the receiver operating characteristic curve (AUC) of 0.94. The top features were longitudinal strain in (1) basal anterior, posterolateral and (2) mid posterior, anterolateral, and anteroseptal sub-regions. However, the ways in which these interact to predict behaviour problems have yet to be examined using objective sleep measurement. The lack of objective evidence for these relationships in young children has recently been defined by the World Health Organization (2019) as a gap in the field. We thus aimed to test the interacting effects of screen exposure and objectively measured sleep on behaviour problems in the preschool age. A total of 145 children aged 3-to-6-years participated in this cross-sectional study. Sleep was assessed objectively using actigraphy for 1-week, and subjectively using parent-reported daily sleep diaries. Parents reported the child's daily duration of screen exposure, and completed the Strengths and Difficulties Questionnaire. Results showed that actigraphic sleep duration, timing and efficiency were associated with screen exposure. The link between screen time and behaviour problems was moderated by sleep duration, as it was significant only for children with sleep duration of 9.88 h or less per night. Sleep duration also moderated the relation between screen time and externalizing-but not internalizing-problems. Hence, the combination of increased screen exposure and decreased sleep duration may be particularly adverse for child mental health. While these key relationships should be further examined in longitudinal and experimental investigations, our findings shed light on their complexity, underscoring the importance of the moderating role of sleep.As mental health systems move towards person-centred care, outcome measurement in clinical research and practice should track changes that matter to young people and their families. Adolescents had been allocated to Cognitive-Behavioral Therapy, Short-Term Psychoanalytic Psychotherapy, or a Brief Psychosocial Intervention.The study mapped seven high-level outcome domains and 29 outcome categories. On average, participants discussed change in four domains and six outcome categories. The most frequently discussed outcome was an improvement in mood and affect (i.e., core depressive symptoms), but close to half of the participants also described changes in family functioning, coping and resilience, academic functioning, or social functioning. Coping had specific importance for adolescents, while parents and therapists showed particular interest in academic functioning. There was some variation in the outcomes discussed beyond these core themes, across stakeholder groups and treatment arms.Of the outcomes that were frequently discussed in stakeholder narratives, only symptomatic change has been commonly reported in recent treatment studies for adolescent depression. Interval-censored data often arise naturally in medical, biological, and demographical studies. As a matter of routine, the Cox proportional hazards regression is employed to fit such censored data. The related work in the framework of additive hazards regression, which is always considered as a promising alternative, remains to be investigated. We propose a sieve maximum likelihood method for estimating regression parameters in the additive hazards regression with case II interval-censored data, which consists of right-, left- and interval-censored observations. We establish the consistency and the asymptotic normality of the proposed estimator and show that it attains the semiparametric efficiency bound. The finite-sample performance of the proposed method is assessed via comprehensive simulation studies, which is further illustrated by a real clinical example for patients with hemophilia.BACKGROUND One of the challenges encountered by clinicians new to implant dentistry is the determination and controlling of implant location. MATERIAL AND METHODS Ten clinicians new to implant dentistry inserted one anterior (central incisor) and one posterior (first molar) implants per protocol in training maxillary models. The FG protocol involved drilling and implant placement through the guide, while the PG protocol controlled the pilot drilling only. The FH implant placement was completed without the aid of any guide. A total of 30 models were used, and 60 implants were inserted. The implant vertical, horizontal neck, horizontal apex, and angle deviations from planned positions were calculated. RESULTS The FG protocol provided the most accurate implant placement in relation to horizontal neck (0.47 mm-0.52 mm), horizontal apex (0.71 mm-0.74 mm), and angle deviations (2.42o-2.61o). The vertical deviation was not significantly different among the different protocols. The PG protocol was generally similar to the FH protocol with a horizontal neck deviation of 1.01 mm-1.14 mm, horizontal apex deviation of 1.02 mm-1.35 mm, and angle deviation of 4.65o-7.79o. The FG protocol showed similarity in the accuracy of the anterior and posterior implants. There was a tendency for inferior accuracy for posterior implants compared with anterior implants for the PG and FH protocols. CONCLUSIONS In the hands of recently introduced clinicians to implant dentistry, it appears that the accuracy of the FG protocol was superior to the other protocols and was not influenced by the position of the implants. Compared with NDS, the DS group showed reduced GM volume in the right inferior frontal orbital gyrus (IFOG) and left caudate (CAU) but increased GM volume in the left calcarine fissure, weaker negative right IFOG-left rectus gyrus (REG) FC, and weaker positive right IFOG-left inferior parietal lobule (IPL) FC. In DS, the GM volume of the right IFOG and left CAU was negatively correlated with NGASR and HRSD scores, respectively; the right IFOG-left IPL FC was negatively correlated with cognitive factor scores; and the GM volume of the right IFOG was positively correlated with IFOG-REG and IFOG-IPL FC. We found that the growth inhibition phenotype including root length, shoot length and fresh weight were significantly improved by 3OC6-HSL under salt stress condition. The physiological and biochemical analysis revealed that the contents of chlorophyll and proline were increased and the contents of MDA and Na+ and Na+/K+ ratios were decreased after 3OC6-HSL treatment in Arabidopsis and wheat under salt stress condition. Molecular analysis showed that 3OC6-HSL significantly upregulated the expression of salt-responsive genes including ABA-dependent osmotic stress responsive genes COR15a, RD22, ADH and P5CS1, ABA-independent gene ERD1, and ion-homeostasis regulation genes SOS1, SOS2 and SOS3 in Arabidopsis under salt stress condition.

We have described a modification of the Blumgart pancreaticojejunostomy. The modification of the Blumgart pancreaticojejunostomy was compared to the Cattel-Warren pancreaticojejunostomy in cohorts of patients matched by propensity scores based on factors predictive of clinically relevant postoperative pancreatic fistula, which was the primary endpoint of this study. Based on a noninferiority study design, 95 open pancreatoduodenectomies per group were needed. Feasibility of the modification of the Blumgart pancreaticojejunostomy in robotic pancreatoduodenectomy was also shown. All pancreaticojejunostomies were performed by a single surgeon.
Between October 2011 and May 2019, there were 415 pancreatoduodenectomies with either a Cattel-Warren pancreaticojejunostomy (n= 225) or a modification of the Blumgart pancreaticojejunostomy (n= 190). There was 1 grade C postoperative pancreatic fistula in 190 consecutive modionversion to open surgery (1.5%), a clinically relevant postoperative pancreatic fistula occurred after 9 procedures (13.6%) with no case of grade C postoperative pancreatic fistula and a 90-day mortality of 3%.

Tumor-associated tissue eosinophilia (TATE) has been associated with outcomes in a variety of solid tumors; however, its role in breast cancer is not well defined. We hypothesized that tumor-associated tissue eosinophilia is associated with a high mutation and neoantigen load, and we assessed its correlation with cancer outcomes.
The Cancer Genome Atlas was analyzed for eosinophil signatures in breast cancer specimens. Descriptive analyses were performed, including the tumor-infiltrating cell composition using CIBERSORT, cytolytic activity score, and gene set enrichment analysis. Overall survival and disease-free survival were calculated using the Kaplan-Meier method.
Out of 1069 cases analyzed, 40 (3.7%) had tissue eosinophils (the tumor-associated tissue eosinophilia group). Tumor-associated tissue eosinophilia was noted in 32.5% luminal, 5% HER2-positive, and 15% triple-negative breast cancer subtypes. The single nucleotide variant-neoantigen load was significantly higher in the tumor-associated tissand was associated with a higher single nucleotide variant-neoantigen load and nonsilent mutation rate, similar to that of tumor-infiltrating lymphocytes in the triple-negative subtype. Tumor-associated tissue eosinophilia (TATE) has been associated with outcomes in a variety of solid tumors; however, its role in breast cancer is not well defined. We hypothesized that tumor-associated tissue eosinophilia is associated with a high mutation and neoantigen load, and we assessed its correlation with cancer outcomes. The Cancer Genome Atlas was analyzed for eosinophil signatures in breast cancer specimens. Descriptive analyses were performed, including the tumor-infiltrating cell composition using CIBERSORT, cytolytic activity score, and gene set enrichment analysis. Overall survival and disease-free survival were calculated using the Kaplan-Meier method. Out of 1069 cases analyzed, 40 (3.7%) had tissue eosinophils (the tumor-associated tissue eosinophilia group). Tumor-associated tissue eosinophilia was noted in 32.5% luminal, 5% HER2-positive, and 15% triple-negative breast cancer subtypes. The single nucleotide variant-neoantigen load was significantly higher in the tumor-associated tissand was associated with a higher single nucleotide variant-neoantigen load and nonsilent mutation rate, similar to that of tumor-infiltrating lymphocytes in the triple-negative subtype. Solubility study was conducted to select the most appropriate oils and surfactants for formulation. Different possible systems were created. Ternary phase diagrams were constructed to select the most promising system for further study. The nanoemulsion intermediate gel of the selected system was evaluated for stability, dilution effect, viscosity, pH, antifungal activity, droplet size, PDI, and zeta potential. In vitro release of the drug from the selected intermediate gel was investigated, and the kinetic model of drug release was determined. Ex vivo permeation of itraconazole was studied, and the amount of drug accumulated in the skin was calculated. Solubility and phase diagrams revealed that the system consisting of 60% cotton seed oil and 40% span 80 provided the nanoemulsion intermediate gel with the highest drug concentration. The selected system had a droplet size of about 236 nm and zeta potential of - 59.8. The viscosity of the corresponding intermediate gel was 1583.47 cp. The system exhibited high stability at 4°C and 25°C for 12 months and improved antifungal activity. In vitro release study showed complete release of itraconazole within 4 h, while the ex vivo permeation study revealed accumulation of the majority of the drug within the skin layers (72.5%).
Erosion of a pacer lead into the chest wall may result in pericardial effusion with cardiac tamponade. Free rupture into the pleura or mediastinum can result in hypotension and cardiac arrest.
We report a unique case of a right ventricular pacer lead which eroded through the right ventricle into the left chest wall and penetrated a rib. The patient presented with a tender chest wall mass without pericardial or pleural effusion. The segment of rib which the pacing lead had penetrated was removed.
The patient tolerated the procedure well and was discharged 1week after the operation. This case adds to the current literature the justification of removal of temporary and non-functional pacing leads.

The objective of the paper was to assess real-life experience in the management of head and neck cancer (HNC) patients during the COVID-19 pandemic in radiotherapy departments and to evaluate the variability in terms of adherence to American Society of Radiation Oncology (ASTRO) and European Society for Radiotherapy and Oncology (ESTRO) recommendations.
In May 2020, an anonymous 30-question online survey, comparing acute phase of outbreak and pre-COVID-19 period, was conducted. Two sections exploited changes in general management of HNC patients and different HNC primary tumors, addressing specific statements from ASTRO ESTRO consensus statement as well.
Eighty-eight questionnaires were included in the demographic/clinical workflow analysis, and 64 were analyzed for treatment management. Forty-eight percent of radiotherapy departments became part of oncologic hubs. First consultations reduced, and patients were addressed to other centers in 33.8 and 18.3% of cases, respectively. Telematic consultations were used in 50% of follow-up visits and 73.9% of multidisciplinary tumor board discussions. There were no practical changes in the management of patients affected by different primitive HNCs. Hypofractionation was not favored over conventional schedules.
Compared to pre-COVID era, the clinical workflow was highly re-organized, whereas there were no consistent changes in RT indications and schedules.

After crack initiation within the FZ, coalescence of cracks leads to crack growth in axial direction and a subsequent reduction of the load-bearing capacity. This behavior is reproduced well by the numerical simulations with the BW criterion, whereas simulations with the RTCL criterion predict fracture initiation at too high displacements. Overall, the results show the strong influence of the ductility of the FZ on the crushing behavior of welded X-profiles.The objectives were to analyze the effect of a gerontogymnastics program on functional ability and fitness on overweight and obese older woman and to understand if sarcopenia mediates its effect. This randomized controlled trial involved 216 overweight and obese women. The experimental group (EG) carried out 12 weeks of a gerontogymnastics program. The assessment was of gait speed, cardiorespiratory fitness, functional capacity, and muscle strength. EG showed significant improvements in almost every test. When the effect of training was adjusted by gait speed, the improvement of the 6 min walk test (MWT) for the trained group was no longer significant (p = 0.127). The improvement of the 6 MWT was significantly and positively associated with the 10 m test (β = -10.087). After including the 10-m test in the equations, the association between the 6MWT and carrying out the training program decreased but remained significant (β = -19.904). The mediation analysis showed a significant, direct and indirect effect with a significant Sobel test value (z = 6.606 ± 7.733; p = 0.000). These results indicate that a gerontogymnastics program improves functional capacity and fitness; and the effect of a gerontogymnastics program on CRF is mediated by sarcopenia in older women who are overweight and obese.To demonstrate the association of the serum creatinine/serum cystatin C ratio (sarcopenia index, SI) with clinical outcomes including cardiovascular and bleeding risk in older patients who underwent percutaneous coronary intervention (PCI), we analyzed a multicenter nation-wide pooled registry. A total of 1086 older patients (65 years or older) who underwent PCI with second-generation drug-eluting stents (DES) were enrolled. The total population was divided into quartiles according to the SI, stratified by sex. The primary clinical outcomes were major adverse cardiovascular events (****, all-cause death, myocardial infarction and target lesion revascularization) and thrombolysis in myocardial infarction major and minor bleeding during a 3-year follow-up period. In the total population, **** occurred within 3 years in 154 (14.2%) patients. The codon usage was driven by mutation pressure and natural selection, although the main contributing factor was natural selection. Additionally, the codon adaptation index (CAI) and deoptimization (RCDI) illustrated the strong adaptability of H7N9 to Gallus gallus. Similarity index (SiD) analysis showed that Homo sapiens posed a stronger selection pressure than Gallus gallus. Thus, we assume that this may be related to the gradual adaptability of the virus to human. In addition, the host strong selection pressure was validated based on CpG dinucleotide content. In conclusion, this study analyzed the usage of codons of two genes of H7N9 and expanded our understanding of H7N9 host specificity. Combined or coupled remediation technologies are one of the efficient processes for the treatment of contaminated soils. In these technologies, two or more soil remediation techniques are applied simultaneously or sequentially, in which one technique complements the other, making the treatment very efficient. Coupling anodic oxidation (AO) and soil remediation for the treatment of soil contaminated with organics has been studied via two configurations (i) soil remediation, ex situ AO, where AO is used as a post-treatment stage for the treatment of effluents from soil remediation process and (ii) soil remediation, in situ AO, where both processes are applied simultaneously. The former is the most widely investigated configuration of the combined processes, while the latter is less common due to the greater diffusion dependency of AO as an electrode process. In this review, the concept of soil washing (SW)/soil flushing (SF) and electrokinetic as soil remediation techniques are briefly explained followed by a discussion of different configurations of combined AO and soil remediation.

Urology Lean work reduced new patient clinic access time (119 to 21 days) and Bacillus Calmette-**érin (BCG) in clinic treatment time (180 to 105 min). The average proportion of urology on-time surgeries was better than the overall surgery on-time surgeries (71% v 61%). CONCLUSIONS Urology Lean work successfully applied DMS in a service specific yet holistic approach. Urology Lean work improved resident engagement in quality and safety endeavors and served as a DMS model throughout perioperative and clinic areas. We use data on fatal police shootings from Washington Post's "Fatal Force Database" (2015-2017). Using Census population estimates, we examine rates of fatal police shootings, stratified by race/ethnicity (White, Black, Hispanic), across urban and rural areas using five different classification schemes. Two classification schemes-from the National Center for Health Statistics and the US Department of Agriculture-use counties as the basic unit. Three classification schemes-from the National Center for Education Statistics, the US Census Bureau, and the website "FiveThirtyEight" use zip codes. There were just under 1000 fatal police shootings per year from 2015 to 2017, a rate of 0.31 per 100,000 population. Black victimization rates were more than twice those for Whites, with Hispanic victimization rates in between. Across all classification schemes there was little difference in rates of fatal police shootings between urban and rural areas, with suburbs having somewhat lower rates. Among Whites, rates of fatal police shooting victimization were higher in rural areas compared to urban areas, while among Blacks the rates were higher in more urban areas. Our results suggest that efforts to reduce police shootings of civilians should include rural and suburban as well as urban areas. In the current study, the impact of microbial transglutaminase (MTGase)-mediated crosslinking on the thermally denatured egg white protein (TD-EWP) made in alkaline pH in relation to their gelling properties of in gel and emulsion gel systems was investigated. Unlike native EWP that was not a good substrate for MTGase due to its compact structure, SDS-PAGE and gel solubility data showed the TD-EWP was sufficiently cross-linked by MTGase. The MTGase significantly (p less then 0.05) increased the gel strength, fracture stress, fracture strain, and TPA parameters and decreased the frequency dependence of elastic modulus and stress dependence of creep compliance of TD-EWP gel and emulsion gel samples. The enhanced mechanical properties of the TE-EWP gelled samples treated with MTGase may be suitable for delivering sensitive compounds as well as in fabricating tailored scaffolds in tissue engineering and biomedical products. The synthesized catalysts were applied for the simultaneous reduction/degradation of mixture of 4-NP-MB, 4-NP-RB and 4-NP-MB-RB mixture to check the practical applicability. Catalytic recyclability of CA-Ce/Zr@Cu0 catalyst dropped till 5th cycle which is due to the leaching of Cu0 NPs. Highly monodisperse spherical nanocrystalline cellulose (SNCC) was prepared via a rapid single-step heterogeneous hydrolysis technique. The production process was observed to follow first-order kinetics with the reaction rate constant of 42.88 × 10-2 min-1 and the reaction rate of 10.08 × 10-5 mol·L-1·min-1. The obtained OPF-CNC was characterized via complementary analyses. The produced OPF-CNC showed a high crystallinity index value (60%) and high BET surface area (26.10 m2 g-1) as compared to α-cellulose (crystallinity index 54% and BET surface area7.14 m2g-1). The surface analyses via scanning electron microscope (SEM) and transmission electron microscopy (TEM) demonstrated that the OPF-CNC has a smooth surface with a needle-like shape, where the average length and diameter are 95.09 nm and 6.81 nm, respectively. The corrosion analyses via electrochemical impedance spectroscopy (EIS) and potentiodynamic polarization (PD) illustrate that the coated mild steel with the inclusion of 0.5 wt% OPF-CNC has managed to sharply reduce the corrosion (99%). Urology Lean work reduced new patient clinic access time (119 to 21 days) and Bacillus Calmette-Guérin (BCG) in clinic treatment time (180 to 105 min). The average proportion of urology on-time surgeries was better than the overall surgery on-time surgeries (71% v 61%). CONCLUSIONS Urology Lean work successfully applied DMS in a service specific yet holistic approach. Urology Lean work improved resident engagement in quality and safety endeavors and served as a DMS model throughout perioperative and clinic areas. The rupestral species Triatoma jatai Gonçalves et al., 2013, Triatoma costalimai Verano & Galvão, 1959 and Triatoma williami Galvão et al., 1965, which are morphologically similar, have been found naturally infected with Trypanosoma cruzi (Chagas, 1909) in wild, peridomestic, and intradomestic environments, representing a risk of new outbreaks of Chagas disease. This study presents morphological description complementation of these species, with an emphasis on the structures of the female external genitalia, using scanning electron microscopy.
The females of T. jatai and T. costalimai (n = 10 of each) were captured in the Brazilian municipalities of Paranã and Aurora do Tocanti structures to differentiate T. costalimai, T. jatai and T. williami. SEM analysis contributes to and corroborates, together with other tools morphological and molecular, the distinction of the three species.
High-grade serous ovarian cancer (HGSOC) is a major unmet need in oncology. The remaining uncertainty on its originating tissue has hampered the discovery of molecular oncogenic pathways and the development of effective therapies.
We used an approach based on the retention in tumors of a DNA methylation trace (OriPrint) that distinguishes the two putative tissues of origin of HGSOC, the fimbrial (FI) and ovarian surface epithelia (OSE), to stratify HGSOC by several clustering methods, both linear and non-linear. The identified tumor subtypes (FI-like and OSE-like HGSOC) were investigated at the RNAseq level to stratify an in-house cohort of macrodissected HGSOC FFPE samples to derive overall and disease-free survival and identify specific transcriptional alterations of the two tumor subtypes, both by classical differential expression and weighted correlation network analysis. We translated our strategy to published datasets and verified the co-occurrence of previously described molecular classification ofossible origins for human HGSOC, whose derived subtypes are both molecularly and clinically distinct. These results will help define a new roadmap towards rational, subtype-specific therapeutic inroads and improved patients' care.

Dysregulation of long non-coding RNAs (lncRNAs) is responsible for cancer initiation and development, positioning lncRNAs as not only biomarkers but also promising therapeutic targets for cancer treatment. A growing number of lncRNAs have been reported in hepatocellular carcinoma (HCC), but their functional and mechanistic roles remain unclear.
Gene Set Enrichment Analysis was used to investigate the molecular mechanism of UPK1A antisense RNA 1 (UPK1A-AS1). Cell Counting Kit-8 assays, EdU assays, flow cytometry, western blotting, and xenograft assays were used to confirm the role of UPK1A-AS1 in the proliferation of HCC cells in vitro and in vivo. Bioinformatics analyses and quantitative polymerase chain reaction (qRT-PCR) were performed to explore the interplay between UPK1A-AS1 and enhancer of zeste homologue 2 (EZH2). RNA immunoprecipitation (RIP), RNA pull-down assays, western blotting, and qRT-PCR were conducted to confirm the interaction between UPK1A-AS1 and EZH2. The interaction between UPK1A-AS1 y upregulated in HCC, and the upregulation of UPK1A-AS1 predicted poor prognosis for patients with HCC.
Our study revealed that UPK1A-AS1 promotes HCC development by accelerating cell cycle progression through interaction with EZH2 and sponging of miR-138-5p, suggesting that UPK1A-AS1 possesses substantial potential as a novel biomarker for HCC prognosis and therapy.

Every item of trial design is selected in the best medical interest of participating patients. Care trials can eventually show what constitutes good medical practice based on patient outcomes. In the meantime, care trials give clinicians and patients the scientific methods necessary for optimization of medical care when no one really knows what to do.We report the progress of 9 randomized care trials that were used to guide the endovascular or surgical management of 1212 patients with acute stroke, intracranial aneurysms, and arteriovenous malformations in a single center in an elective or acute care context. Care trials were used to address long-standing dilemmas regarding rival medical, surgical, or endovascular management options or to offer innovative instead of standard treatments. https://www.selleckchem.com/products/rmc-9805.html The trial methodology, by replacing unrepeatable treatment decisions by 11 randomized allocation whenever reliable knowledge was not available, had an immediate impact, transforming unverifiable dogmatic medical practice into verifiable outcome-based medical care.
