Remarkable coincidences across two millennia are found on five basic criteria, which hints at a certain historical stability of some of the most fundamental features of a concept of "science" broadly construed.John Norton's Material Theory of Induction (Norton, 2003, 2005, 2008, forthcoming) has a two-fold, negative and positive, goal. The negative goal is to establish that formal logics of induction fail if they are understood as universally applicable schemas of induction. The positive goal is to establish that it is material facts that enable and justify inductive inferences. I argue in this paper that Norton is more successful with his negative than with his positive ambition. While I do not deny that facts constitute an important type of enabler and justifier of inductions, they are by no means the only type. I then defend this thesis against the most influential objections put forward by the critics of RCT. In doing so, I explicate the implications of my thesis for the ongoing philosophical debate concerning the explanatory potential of RCT and the comparative merits of widely endorsed accounts of explanation.Philosophers of science are increasingly arguing for and addressing the need to do work that is socially and scientifically engaged. However, we currently lack well-developed frameworks for thinking about how we should engage other expert communities and what the epistemic benefits are of doing so. In this paper, I draw on Collins and Evans' concept of 'interactional expertise' - the ability to speak the language of a discipline in the absence of an ability to practice - to consider the epistemic benefits that can arise when philosophers engage scientific communities. As Collins and Evans argue, becoming an interactional expert requires that one 'hang out' with members of the relevant expert community in order to learn crucial tacit knowledge needed to speak the language. Building on this work, I argue that acquiring interactional expertise not only leads to linguistic fluency, but it also confers several 'socio-epistemic' benefits such as the opportunity to cultivate trust with scientific communities. These benefits can improve philosophical work and facilitate the broader uptake of philosophers' ideas, enabling philosophers to meet a variety of epistemic goals. As a result, having at least some philosophers of science acquire interactional expertise via engagement will likely enhance the diversity of epistemic capacities for philosophy of science as a whole. For some philosophers of science, moreover, the socio-epistemic benefits identified here may be more important than the ability to speak the language of a discipline, suggesting the need for a broader analysis of interactional expertise, which this paper also advances.

  • Hypertension is an important disease of public health concern. Awareness and medication adherence with diet modification have an important effect on the control of blood pressure and its associated morbidity and mortality. Therefore, this study was conducted to assess the awareness of hypertension, medication adherence, and dietary pattern in hypertensive population of western Rajasthan.

    The study was hospital based cross-sectional. Blood pressure measurements were taken in a sitting position in right arm after a 5-min rest using nonmercury sphygmomanometer and required cuff size. A prevalidated and pretested questionnaire for the assessment of awareness of hypertension was used.

    Out of the total 384 patients, the majority of the patients were males (62.5%). There was a statistically significant difference found in awareness of hypertension among rural and urban patients. Nonadherence to antihypertensive medications was seen more in males (60.0%) as compared to females (40.0%). https://www.selleckchem.com/products/esi-09.html The most common reason for nonadherence was found to be forgetfulness (27.6%) followed by poor knowledge about the hypertension and ignorance of long-term treatment (22.9%).     Ramadan is the ninth month in the lunar Islamic calendar and lasts for about 29-30 days. It is mandatory for all adult individuals of the Islamic faith to fast during the holy month of Ramadan. During this period, healthy Muslims do not eat or drink (i.e., total abstinence from food and fluids) and refrain from tobacco, daily from pre-dawn until dusk (21). Very few studies in the recent years have been conducted on this subject and for primary care physicians, it is of great help in understanding the physiological changes that occur in various populations with different rituals in Muslims.

    The present study included healthy adult male Muslim volunteers from different mosques in and around Jammu city. The number of subjects in the study was 100. Blood samples from all 100 subjects were collected twice during the study-once in the week prior to the beginning of Ramadan and then again in the last week of Ramadan (from 98 subjects as 2 subjects opted out). Following biochemical parameters were taken into consideration (a) Serum total cholesterol (b) Serum triglycerides (c) Serum HDL cholesterol.

    It was observed that compared to pre-fasting levels, total cholesterol (TC) and triglycerides (TG) were significantly decreased (P = 0.000) and high-density lipoprotein cholesterol (HDL-C) level had significantly increased (P = 0.000).

    "Aging India" has become a phenomenon of public health importance. Old age is beset with physical, mental, and social challenges. Among these, mental health concerns are least prioritized in most of the developing countries with depression being the most common and easy to screen.

    A cross-sectional study was conducted in July-August 2018 in village Kirpind in north India.

    162 subjects aged 60 years or more, both males and females participated in the study. Depression was assessed using the 15-item Geriatric Depression Scale and those with a GDS score >=5 were categorized as depressed.

    Pearson's Chi-square test and binary logistic regression were used for analysis.

    Nearly 59.3% of subjects had no depression, 33.9% were suffering from mild to moderate depression whereas 6.8% were severely depressed. The mean age of subjects was 69 (±7.4) years. Chi-square test was used to study the association of various factors with depression and only female gender showed a positive statistical association. On using binary logistic regression analysis, being female again emerged to be a significant predictor of depression while no other factor was significantly associated with the outcome.

    There is a need to sensitize primary care workers and physicians to identify and manage geriatric depression early. It also points towards the need for multicentric, longitudinal studies evaluating various aspects of geriatric depression.

    Chronic migraine (CM) poses a significant burden for patients, and it has multiple diagnostic and managemental challenges, particularly among primary health-care physicians (PCPs).

    The objective of this study is to assess the levels of knowledge regarding CM diagnosis and management among PCPs and to explore the factors associated with poor knowledge.

    A cross-sectional study was conducted among PCPs working at primary health-care centers in Jeddah, Saudi Arabia. A semi-structured questionnaire comprising 20 items related to CM diagnosis and treatment was used.

    A total of 136 PCPs responded (aged 36.4 ± 8.1, 68.4% were females). The overall median (P75) knowledge score was 13.0 (15.0), and 45.6% of the participants had adequate knowledge (score > median). The majority of respondents (>50%) could not identify the lower limits of CM duration, the necessity of identifying at least eight migraine attacks in a month, the importance of managing medication overuse, and the indication of topiramate as the most efficacious agent in CM.

  • A method for expanding the measurement range of low-coherence digital holography up to several times longer than the coherence length is proposed. The method was implemented with a multireflection reference mirror composed of partially and highly reflective mirrors, in conjunction with the Fourier transform method with spatial filtering for single-shot complex amplitude imaging, making it useful for observing a moving and deforming object. One of the features of the reference arm is that the measurement range is simply controlled by adjusting the position and angle of the highly reflective mirror. The measurement of objects with a general curved shape and a large step height was demonstrated.A stable, 22.9 W, 671 nm single-frequency laser using a type II noncritically phase-matched external-cavity frequency doubling is demonstrated. The output power of the fundamental laser is 32.1 W; the corresponding conversion efficiency of frequency doubling from 1342 to 671 nm is calculated to be 71.3%. The M2 factors are measured to be 1.10 and 1.08 in the x and y directions, respectively. To the best of our knowledge, 22.9 W is the highest power obtained for a 671 nm single-frequency laser.We report a new polymer/colloidal-quantum-dot (CQD) film with a nanostructured interface, which is fabricated through a template-assisted photopolymerization method, toward the use of amplified spontaneous emission. First, for carrying channel header information, we transmit a 10 Gb/s on-off keying (OOK) data channel as a Gaussian beam and add to it a 10 Mb/s OOK header carried by an OAM beam with the mode order ℓ=3. We recover the header and use it to drive a switch and select the output port. Secondly, for orthogonal channel coding, we configure transmitters to generate orthogonal spatial codes (orthogonal spatial beam profiles of OAM modes), each carrying an independent data stream. We measure the correlation between the OAM codes and demonstrate their use in a multiple access system carrying two 10 Gb/s OOK data channels. At the end of this Letter, we combine the concepts of using OAM modes for carrying channel header information and orthogonal channel coding in one experiment. We transmit a 10 Gb/s OOK data channel as a Gaussian beam and add to it two 10 Mb/s OOK header waveforms carried by different OAM codes. In the routing node, we recover one of the headers to drive the switch.In this Letter, we experimentally demonstrate low noise 300 GHz wave generation based on a Kerr microresonator frequency comb operating in the soliton regime. The spectral purity of a 10 GHz GPS-disciplined dielectric resonant oscillator is transferred to the 300 GHz repetition rate frequency of the soliton comb through an optoelectronic phase-locked loop. Two adjacent comb lines beat on a uni-traveling carrier photodiode emitting the 300 GHz millimeter-wave signal into a waveguide. In an out-of-loop measurement, we measure the 300 GHz power spectral density of phase noise to be -88dBc/Hz, -105dBc/Hz at 10 kHz, and 1 MHz Fourier frequency, respectively. Phase-locking error instability reaches 2×10-15 at 1 s averaging time. It requires only three binarized intensity patterns to illuminate the full field, which can be implemented at ∼22kHz using a digital micromirror device. Each diffraction pattern at the Fourier plane is acquired in a single shot without high-dynamic-range synthesis, resulting in three intensity-only images corrupted with underexposed pixels. We develop an adaptive phase retrieval algorithm to adaptively remove the negative influence of underexposure and recover both the object's amplitude and phase. Both simulations and experiments validate that the method enables fast and high-fidelity complex-field imaging.A tunable light absorption of graphene using topological interface states (TISs) is presented. The monolayer graphene is embedded in the interface of asymmetric topological photonic crystals (ATPCs). A strong absorption phenomenon occurs by the excitation of TISs. It is found that the absorption spectra are intensively dependent on the chemical potential of graphene and the periodic number of the ATPCs. Furthermore, the absorption can be rapidly switched in a slight variation of chemical potential, which is modulated by the applied gate voltage on graphene. This study not only opens up a new approach for enhancing light-monolayer graphene interactions, but also provides for practical applications in high absorption optoelectronic devices.     A method for expanding the measurement range of low-coherence digital holography up to several times longer than the coherence length is proposed. The method was implemented with a multireflection reference mirror composed of partially and highly reflective mirrors, in conjunction with the Fourier transform method with spatial filtering for single-shot complex amplitude imaging, making it useful for observing a moving and deforming object. One of the features of the reference arm is that the measurement range is simply controlled by adjusting the position and angle of the highly reflective mirror. The measurement of objects with a general curved shape and a large step height was demonstrated.A stable, 22.9 W, 671 nm single-frequency laser using a type II noncritically phase-matched external-cavity frequency doubling is demonstrated. The output power of the fundamental laser is 32.1 W; the corresponding conversion efficiency of frequency doubling from 1342 to 671 nm is calculated to be 71.3%. The spectral purity of a 10 GHz GPS-disciplined dielectric resonant oscillator is transferred to the 300 GHz repetition rate frequency of the soliton comb through an optoelectronic phase-locked loop. Two adjacent comb lines beat on a uni-traveling carrier photodiode emitting the 300 GHz millimeter-wave signal into a waveguide. In an out-of-loop measurement, we measure the 300 GHz power spectral density of phase noise to be -88dBc/Hz, -105dBc/Hz at 10 kHz, and 1 MHz Fourier frequency, respectively. We develop an adaptive phase retrieval algorithm to adaptively remove the negative influence of underexposure and recover both the object's amplitude and phase. The monolayer graphene is embedded in the interface of asymmetric topological photonic crystals (ATPCs). A strong absorption phenomenon occurs by the excitation of TISs. It is found that the absorption spectra are intensively dependent on the chemical potential of graphene and the periodic number of the ATPCs. Furthermore, the absorption can be rapidly switched in a slight variation of chemical potential, which is modulated by the applied gate voltage on graphene. This study not only opens up a new approach for enhancing light-monolayer graphene interactions, but also provides for practical applications in high absorption optoelectronic devices.     Prior opioid use can influence outcomes for patients with musculoskeletal disorders. The purpose of this study was to compare downstream medical utilization-based outcomes (costs, visits, recurrent episodes) after an initial diagnosis of patellofemoral pain based on pre-injury utilization of opioids.

    A total of 85,7880 consecutive patients were followed for a full 12months before and 24months after an initial diagnosis of patellofemoral pain (January 2009 to December 2013). Data were sourced from the Military Health System Data Repository, a single-payer closed government system. Opioid prescription fills were identified, and medical visits and costs were calculated for all knee-related medical care, to include recurrence rates in the 2-year surveillance period.

    A relatively small number of individuals filled an opioid prescription in the year prior (n = 1746; 2.0%); however, these individuals had almost twice the mean costs of knee-related medical care ($1557 versus %802) and medical visits (8.4 versusithout prior opioid use. • Prior opioid use has predicted higher costs and poor outcomes after surgery, but this is the first study to confirm similar findings in non-surgical patients.This study was aimed to analyze the readability and quality of patient education websites on esophageal cancer. The first 50 websites resulting in each keyword search were evaluated using validated FRES, LIDA, and DISCERN scores to assess readability, usability, and reliability, and quality of information, respectively. Non-parametric tests were used for statistical analysis. A total of 108 eligible websites were included in the analysis. The addition of KCN decreased NO3- uptake and assimilation, whereas a negligible impact was observed in NH4+ treatments. Enzymatic assays showed a marked activities enhancement of glutamine synthetase (GS), glutamate synthase (GOGAT), and glutamate dehydrogenase (GDH) in NH4+-fed seedlings after KCN exposure. A semi-field experiment was conducted for 420 days in southern Brazil, testing continuous disposal of washing pulverization tanks in biobeds, Typic Haploperox or Typic Hapludults. In addition, different biobeds received a single dose (1 L) of Lorsban® 480 BR to simulate an accidental spillage. Chronic ecotoxicity tests were performed using Folsomia candida, Eisenia andrei, and Enchytraeus crypticus in different sampling times for both experiments. F. candida was the most sensitive species. The biobed system was able to eliminate effects from residues of both pesticides over time in all species, which did not happen in both natural soils. In accidental spillage simulation, even 420 days after contamination, F. candida did not show reproduction. The biobeds can be a feasible alternative for the disposal and treatment residues of pesticides, also for handling and washing pesticides activities. The system was efficient in promoting degradation and reducing ecotoxicity effects posed by Lorsban® 480 BR and Dithane® NT for soil fauna. It is a safe alternative to avoid soil contamination.

  • More than 800 people from 61 countries registered for at least one part of the meeting, which was held mostly in the Remo.co video-conferencing platform.More than 50 years of efforts to identify the major cytokine responsible for red blood cell (RBC) production (erythropoiesis) led to the identification of erythropoietin (EPO) in 1977 and its receptor (EPOR) in 1989, followed by three decades of rich scientific discovery. We now know that an elaborate oxygen-sensing mechanism regulates the production of EPO, which in turn promotes the maturation and survival of erythroid progenitors. Engagement of the EPOR by EPO activates three interconnected signaling pathways that drive RBC production via diverse downstream effectors and simultaneously trigger negative feedback loops to suppress signaling activity. Together, the finely tuned mechanisms that drive endogenous EPO production and facilitate its downstream activities have evolved to maintain RBC levels in a narrow physiological range and to respond rapidly to erythropoietic stresses such as hypoxia or blood loss. Examination of these pathways has elucidated the genetics of numerous inherited and acquired disorders associated with deficient or excessive RBC production and generated valuable drugs to treat anemia, including recombinant human EPO and more recently the prolyl hydroxylase inhibitors, which act partly by stimulating endogenous EPO synthesis. Ongoing structure-function studies of the EPOR and its essential partner, tyrosine kinase JAK2, suggest that it may be possible to generate new "designer" drugs that control selected subsets of cytokine receptor activities for therapeutic manipulation of hematopoiesis and treatment of blood cancers.The treatment landscape for bladder cancer has undergone a rapid evolution in the past five years with the approval of seven new agents.     Angiogenesis is a critical step in the growth of pancreatic neuroendocrine tumors (PNETs) and may be a selective target for PNET therapy. However, PNETs are robustly resistant to current anti-angiogenic therapies that primarily target the VEGFR pathway. Thus, the mechanism of PNET angiogenesis urgently needs to be clarified.

    Dataset analysis was used to identify angiogenesis-related genes in PNETs. Immunohistochemistry was performed to determine the relationship among Neuropilin 2 (NRP2), VEGFR2 and CD31. Cell proliferation, wound-healing and tube formation assays were performed to clarify the function of NRP2 in angiogenesis. The mechanism involved in NRP2-induced angiogenesis was detected by constructing plasmids with mutant variants and performing Western blot, and immunofluorescence assays. A mouse model was used to evaluate the effect of the NRP2 antibody in vivo, and clinical data were collected from patient records to verify the association between NRP2 and patient prognosis.

    This review discusses the current status of mesenchymal stem cell (MSC) therapy for SCI, criteria to considering for the application of ****therapy and novel biological therapies that can be applied together with **** to enhance its efficacy. Bone marrow-derived **** (BMSCs), umbilical cord-derived **** (UC-****) and adipose tissue-derived **** (ADSCs) have been trialed for the treatment of SCI. Application of **** may minimize secondary injury to the spinal cord and protect the neural elements that survived the initial mechanical insult by suppressing the inflammation. The aims of this review are to discuss these newly approved agents for bladder cancer and to feature promising drugs and combinations-including immune checkpoint inhibitors, targeted therapies, and antibody-drug conjugates-that are in development.Right heart catheterization is an essential diagnostic modality in the evaluation of pulmonary hypertension. The coronavirus disease 2019 pandemic has resulted in deferral of elective procedures including right heart catheterization. The benefits of proceeding with right heart catheterization, such as further characterization of hemodynamic subtype and severity of pulmonary hypertension, initiation of targeted pulmonary arterial hypertension therapy, as well as further hemodynamic testing, need to be carefully balanced with the risk of potentially exposing both patients and health care personnel to coronavirus disease 2019 infection. This review article aims to provide best clinical practices for safely performing right heart catheterization in pulmonary hypertension patients during the coronavirus disease 2019 pandemic. Angiogenesis is a critical step in the growth of pancreatic neuroendocrine tumors (PNETs) and may be a selective target for PNET therapy. However, PNETs are robustly resistant to current anti-angiogenic therapies that primarily target the VEGFR pathway. Thus, the mechanism of PNET angiogenesis urgently needs to be clarified. Dataset analysis was used to identify angiogenesis-related genes in PNETs. Immunohistochemistry was performed to determine the relationship among Neuropilin 2 (NRP2), VEGFR2 and CD31. Cell proliferation, wound-healing and tube formation assays were performed to clarify the function of NRP2 in angiogenesis. The mechanism involved in NRP2-induced angiogenesis was detected by constructing plasmids with mutant variants and performing Western blot, and immunofluorescence assays. A mouse model was used to evaluate the effect of the NRP2 antibody in vivo, and clinical data were collected from patient records to verify the association between NRP2 and patient prognosis. This review discusses the current status of mesenchymal stem cell (MSC) therapy for SCI, criteria to considering for the application of MSC therapy and novel biological therapies that can be applied together with MSCs to enhance its efficacy. Bone marrow-derived MSCs (BMSCs), umbilical cord-derived MSCs (UC-MSCs) and adipose tissue-derived MSCs (ADSCs) have been trialed for the treatment of SCI. Application of MSCs may minimize secondary injury to the spinal cord and protect the neural elements that survived the initial mechanical insult by suppressing the inflammation. Additionally, MSCs have been shown to differentiate into neuron-like cells and stimulate neural stem cell proliferation to rebuild the damaged nerve tissue. The effusion is most commonly unilateral, with the right and left side being affected in 50% and 33.3% of the patients, respectively. Only 16.66% of cases present a bilateral effusion. The underlying etiology can be classified as spontaneous or traumatic. The diagnosis is made through pleural fluid analysis and imaging studies. The following article presents two cases of left spontaneous chylothorax a 26-year-old male presenting with a chylous pleural effusion due to a non-Hodgkin lymphoma and a 47-year-old patient from a tropical area with a chylous pleural effusion attributed to filariasis.     ADHD is one of the most common psychiatric disorders in children and adolescents. Altered functional connectivity has been associated with ADHD symptoms. This study aimed to investigate abnormal changes in the functional connectivity of resting-state brain networks (RSNs) among adolescent patients with different subtypes of ADHD.

    The data were obtained from the ADHD-200 Global Competition, including fMRI data from 88 ADHD patients (56 patients of ADHD-Combined, ADHD-C and 32 patients of ADHD-Inattentive, ADHD-I) and 67 typically developing controls (TD-C). Group ICA was utilized to research aberrant brain functional connectivity within the different subtypes of ADHD.

    In comparison with the TD-C group, the ADHD-C group showed clusters of decreased functional connectivity in the left inferior occipital gyrus (p = 0.0041) and right superior occipital gyrus (p = 0.0011) of the dorsal attention network (DAN), supplementary motor area (p = 0.0036) of the executive control network (ECN), left supramarginal gyr/impulsivity, and unconscious movements.
    The results suggested that decreased functional connectivity within the DMN and DAN was responsible, at least in part, for the symptom of inattention in ADHD-I patients. Similarly, we believed that the impaired functional connectivity within networks may contribute to the manifestations of ADHD-C patients, including inattention, hyperactivity/impulsivity, and unconscious movements.

Treatment reduces adverse maternal and perinatal outcomes although uncertainty remains about the optimal diagnostic criteria. The GEMS Trial aims to assess whether detection and treatment of women with GDM using the lower International Association of Diabetes in Pregnancy Study Groups diagnostic criteria compared with the higher criteria recommended in New Zealand reduces infant morbidity without increasing maternal morbidity.

    GEMS is a multicentre, randomised trial. Women with a singleton pregnancy at 24 to 34 weeks' gestation are eligible who give written informed consent. Women are randomly allocated to the Lower Criteria Group or the Higher Criteria Group. Women with a normal OGTT by their allocated criteria receive routine care (Higher criteria fasting plasma glucose < 5.5mmol/L, AND 2 hour < 9.0mmol/L; Lower criteria fasting plasma glucose < 5.1mmol/L, AND 1 hour < 10.0mmol/L, AND 2 hour < 8.5m practice. If use of the lower diagnostic criteria results in significantly fewer large for gestational age infants and/or improves maternal and perinatal outcomes these criteria should be recommended for diagnosis of gestational diabetes.

    Natural Language Processing techniques are constantly being advanced to accommodate the influx of data as well as to provide exhaustive and structured knowledge dissemination. Within the biomedical domain, relation detection between bio-entities known as the Biomedical relation extraction (BRE) task has a critical function in knowledge structuring. Although recent advances in deep learning-based biomedical domain embedding have improved BRE predictive analytics, these works are often task selective or employ external knowledge-based pre/post processing. In addition, deep learning-based models do not account for local syntactic contexts, which have improved data representation in many kernel classifier-based models. In this study, we propose a universal BRE model, i.e. LBERT, which is a Lexically-aware Transformer-based Bidirectional Encoder Representation model, and which explores both local and global contexts representations for sentence level classification tasks.

    This paper presents one of the most exhaustive BRE studies ever conducted over five different bio-entity relation types. Our model outperforms state-of-the-art deep learning models in protein-protein (PPI), drug-drug (DDI) and protein-bio-entity (REL) relation classification tasks by 0.02%, 11.2% and 41.4% respectively. LBERT representations show a statistically significant improvement over BioBERT in detecting true bio-entity relation for large corpora like PPI. Our ablation studies clearly indicate the contribution of the lexical features and distance-adjusted attention in improving prediction performance by learning additional local semantic context along with bi-directionally learned global context.

    Subunit interactions at the cytoplasmic domain interface (CD-I) have recently been shown to control gating in inward rectifier potassium channels. Here we report the novel KCNJ2 variant p.Glu293Lys that has been found in a patient with Andersen-Tawil Syndrome type 1 (ATS1), causing amino acid substitution at the CD-I of the inward rectifier potassium channel subunit Kir2.1. Neither has the role of Glu293 in gating control been investigated, nor has a pathogenic variant been described at this position. This study aimed to assess the involvement of Glu293 in CD-I subunit interactions and to establish the pathogenic role of the p.Glu293Lys variant in ATS1.

    The p.Glu293Lys variant produced no current in homomeric form and showed dominant negative effect over wild type (WT) subunits. Immunocytochemical labelling showed the p.Glu293Lys subunits to distribute in the subsarcolemmal space. Salt bridge prediction indicated the presence of an intersubunit salt bridge network at the CD-I of Kir2.1, with the involveme also identifies disturbed interactions between the cytoplasmic domains of Kir2.1 subunits as the molecular mechanism of loss-of-function in the p.Glu293Lys variant. Adult patients with multiple sclerosis (n = 632) were identified from the Comprehensive Longitudinal Investigations in Multiple Sclerosis at Brigham study (CLIMB), and paediatric patients (n = 132) from the US Network of Paediatric Multiple Sclerosis Centers (NPMSC) registry. Change in EDSS was defined as the difference in EDSS between attack and follow-up. Change in EDSS at follow-up compared to baseline was significantly lower in children compared to adults (P = 0.001), as were several functional system scores. Stratification by decade at onset for change in EDSS versus age found for every 10 years of age, EDSS recovery is reduced by 0.15 points (P  less then  0.0001). A larger proportion of children versus adults demonstrated improvement in EDSS following an attack (P = 0.006). For every 10 years of age, odds of EDSS not improving increase by 1.33 times (P  less then  0.0001). Younger age is associated with improved recovery from relapses. Age-related mechanisms may provide novel therapeutic targets for disability accrual in multiple sclerosis.
    To increase the safety of aortic valve replacement, we developed the 'Caput medusae' method, where the prosthesis is prefixed with circumferential tourniquets prior to knot tying. We assumed that an even distribution of forces may help reduce tissue damage. To confirm this theoretically, we compared forces between knots and tourniquets.

    The experimental set-up included a device with movable acrylic plates, a mounted valve and a set of sutures. Traction forces were measured with a luggage scale. Two different tourniquets were compared individually and as bundles of 15. Force-path curves were generated. Knotting and tourniquet forces of 18 staff surgeons were then compared. Both modalities were measured 10 times on 2 days, resulting in 40 observations per surgeon, or 360 observations per modality.

    Polyvinyl chloride tourniquets were stiffer than silicone, expressed by a 1.5- to 1.7-fold higher regression-line slope. Fifteen simultaneous tubes produced force increments 7.9-8.9 times higher than their single counterparts.

Change in EDSS at follow-up compared to baseline was significantly lower in children compared to adults (P = 0.001), as were several functional system scores. Stratification by decade at onset for change in EDSS versus age found for every 10 years of age, EDSS recovery is reduced by 0.15 points (P  less then  0.0001). A larger proportion of children versus adults demonstrated improvement in EDSS following an attack (P = 0.006). Two different tourniquets were compared individually and as bundles of 15. Force-path curves were generated. Knotting and tourniquet forces of 18 staff surgeons were then compared. Both modalities were measured 10 times on 2 days, resulting in 40 observations per surgeon, or 360 observations per modality. Polyvinyl chloride tourniquets were stiffer than silicone, expressed by a 1.5- to 1.7-fold higher regression-line slope. Fifteen simultaneous tubes produced force increments 7.9-8.9 times higher than their single counterparts.

Uranium reclamation from aqueous systems is of great significance, not only in the usage of nuclear power but also for the remediation of nuclear leakage pollution. Recently, metal-organic frameworks (MOFs) with high porosity, regular pore structure and modifiable functional groups, have been considered as one of the most effective uranium adsorption materials. Specifically, the post-grafted functional groups possess strong affinity to uranium. This frontier summarizes the recent progresses on grafting functional groups in MOFs for U(vi) sorption from aqueous solutions. The adsorption performance, interaction mechanisms and the grafted functional groups are assessed in this study. Clinical decision making and communication with patient/next of kin were based on "pot" classification. Settings/Subjects Palliative medicine consults were placed on all COVID-19 positive patients admitted to the ICU between March 29, 2020, and May 1, 2020.

The objectives of this study were to describe the perceived ineffectiveness of common pharmacological treatments for dysmenorrhea and investigate factors associated with perceived treatment ineffectiveness. Materials and Methods In this cross-sectional study, 678 women with dysmenorrhea (aged 14-42) provided data on perceived treatment ineffectiveness, dysmenorrhea symptom-based phenotypes, demographics, clinical factors, and psychobehavioral characteristics. We used Fisher's exact tests to compare treatment ineffectiveness across three symptom-based phenotypes. We used logistic regressions to explore associations of phenotype, demographic, clinical, and psychobehavioral correlates of perceived treatment ineffectiveness. Results Percentages perceiving treatments as ineffective were 29.3%-3 effectiveness, so treatments can be tailored to individuals.The action of guanine nucleotide exchange factors (GEFs) on the ADP-ribosylation factor (ARF) family of small GTPases initiates intracellular transport pathways. Because the combination of these actions results in Grp1 being recruited preferentially to the recycling endosome rather than to the plasma membrane, our findings reveal the complexity of recruitment mechanisms that need to be coordinated in localizing an ARF GEF to an intracellular compartment to initiate a transport pathway.   • Growing high quality silicon nanowires (SiNWs) at elevated temperature on cooler polymer films seems to be contradictive but highly desirable for building high performance flexible and wearable electronics. In this work, we demonstrate a superfast (vnw > 3.5 μm·s-1) growth of high quality SiNWs on polymer/glass substrates, powered by self-selected laser at 808 nm heating of indium catalyst droplets that absorb amorphous Si layer to produce SiNWs. Because of the tiny heat capacity of the nanodroplets, the SiNW growth can be quickly heated up and frozen via rapid laser ON/OFF switching, enabling a deterministic diameter modulation in the ultralong SiNWs. Finally, prototype field effect transistors are also fabricated upon the laser-droplet-heating grown SiNWs with a high Ion/Ioff ratio of >104 and reasonable subthreshold swing of 386 mV·dec-1, opening a generic new route to integrate high-quality NW channels directly upon large area and lightweight polymer substrates for developing high-performance flexible electronics.

Thus, we decided to introduce eHealth technology to farmers' musculoskeletal health care. Using a mobile application (app), we aim to provide a tailored self-exercise program for shoulder, knee, and **** pain in female farmers in rural areas after a musculoskeletal health check-up.

    This study is planned as 2 randomized control studies (MObile Delivered self-Exercise [MODE] phase I and phase II). We plan to recruit 200 female farmers aged 41 to 70 years. Initially, the shoulders, knees, and low **** will be evaluated to provide individualized exercise programs. In MODE-I (single-blinded evaluator), the subjects will be randomly allocated to experimental (n = 100) and control (n = 100) groups using a computer-generated sequence. Both groups will perform a 3-month self-exerc to the degree of participation over 12 months to confirm the effect of self-exercise. Our study should aid in managing musculoskeletal health for farmers living in rural areas and help promote health in the "untact" era.

    Jehovah's Witnesses (JWs) refuse blood transfusions due to religious issues. This situation may impact kidney transplantation (KT) outcomes in case of hemorrhagic complications. We evaluated demographic characteristics of this population, hematologic safety, and graft outcomes.

    This was a retrospective, single-center study comparing KT outcomes in JW patients versus a non-JW control group. Hematologic endpoints included clinical indication for blood transfusion (hemoglobin <7 g/dL), decrease of hemoglobin >2 g/dL or hematocrit >5% in the first week after KT, hemorrhagic complications requiring surgery, and de novo prescription of erythropoiesis-stimulating agents. Secondary endpoints included delayed graft function, treated biopsy-proven acute rejection, renal function, mortality, and graft survival at 12 months.

    From January 1989 to September 2018, we identified 143 JW (10 pediatric) and selected 142 matched control (non-JW) patients. There were no differences in the incidence of clinical indiduring the first year after transplantation compared with non-JW patients.
    The effects of atrial fibrillation (AF) catheter ablation on the left atrium (LA) are poorly understood.

    To examine short- and long-term associations of AF catheter ablation with LA function using cardiac magnetic resonance (CMR).

    Fifty-one AF patients (mean age 56 ± 8 years) underwent CMR at baseline, 1 day (n = 17) and 11 ± 2 months after ablation (n = 38). LA phasic volumes, emptying fractions (LAEF), and longitudinal strain were measured using feature-tracking CMR. LA fibrosis was quantified using late gadolinium enhancement (LGE).

    There were no acute changes in volume; however, active, total LAEF, and peak LA strain decreased significantly compared to the baseline. During long-term follow-up, there was a decrease in maximum but not minimum LA volume (from 99 ± 5.2 ml to 89 ± 4.7 ml; p = .009) and a decrease in total LAEF (from 43 ± 1.8% to 39 ± 2.0%; p = .001). In patients with AF recurrence, LA volumes were unchanged. However, total LAEF decreased from 38 ± 3% to 33 ± 3%; p = .015. Patients without AF recurrence had no changes in LA functional parameters during follow-up. The amount of LA LGE at long-term follow-up was higher compared to the baseline, however, was significantly less compared to immediately post-procedure (37 ± 1.9% vs. 47 ± 2.8%; p = .015). A higher increase in LA LGE extent compared to the baseline was associated with a greater decrease in total LAEF (r = -.59; p < .001).

Compared to 7DDRs, SFFQs underestimated energy intake, macronutrients, and sodium intake, but overestimated some micronutrients. The mean of Spearman correlation coefficients was 0.66 (range 0.38 to 0.88) between 46 energy-adjusted nutrients estimated from 7DDRs and the final SFFQ, de-attenuated for within-person variation in the 7DDRs. These deattenuated correlations were similar using ASA24s as the comparison. Relative to biomarkers, SFFQs underestimated energy, sodium, and protein intakes, and the sodiumpotassium ratio. The energy-adjusted correlations between the final SFFQ and the biomarkers were slightly lower than the correlations between the SFFQ and 7DDRs.
    Growing high quality silicon nanowires (SiNWs) at elevated temperature on cooler polymer films seems to be contradictive but highly desirable for building high performance flexible and wearable electronics. In this work, we demonstrate a superfast (vnw > 3.5 μm·s-1) growth of high quality SiNWs on polymer/glass substrates, powered by self-selected laser at 808 nm heating of indium catalyst droplets that absorb amorphous Si layer to produce SiNWs. Because of the tiny heat capacity of the nanodroplets, the SiNW growth can be quickly heated up and frozen via rapid laser ON/OFF switching, enabling a deterministic diameter modulation in the ultralong SiNWs. Finally, prototype field effect transistors are also fabricated upon the laser-droplet-heating grown SiNWs with a high Ion/Ioff ratio of >104 and reasonable subthreshold swing of 386 mV·dec-1, opening a generic new route to integrate high-quality NW channels directly upon large area and lightweight polymer substrates for developing high-performance flexible electronics.     Total mass, fat mass, lean mass (total, percentage and index) and the android/gynoid (A/G) ratio were similar in both groups; however, bone mineral content was lower (P<.001) in the POI group. Lean and fat mass indexes were 14.19±1.63 and 11.04±3.58, respectively, and the percentage of gynoid to android fat was higher (51.17±6.71% versus 47.74±9.19%; A/G ratio=0.93±0.15) in the POI group. In addition, the increase in total mass correlated positively with glucose and ALT levels and negatively with HDL-cholesterol. Increased A/G ratio was the measurement most frequently associated with cardiovascular risk markers.

    Neonates have lower levels of antithrombin (AT) due to immature liver synthetic function. AT deficiency may lead to inadequate anticoagulation with heparin during cardiac surgery resulting in consumption of coagulation factors and increased blood transfusion. The goal of this study is to examine the effect of AT level on the transfusion requirements of neonates and infants undergoing open heart surgery.

    This is a prospective, observational study at a tertiary pediatric referral center. Neonates and infants up to 6 months of age undergoing congenital heart surgery with cardiopulmonary bypass (CPB) were enrolled. Demographic, intraoperative, transfusion, and complications data were collected. Preoperative AT level was measured after induction of anesthesia. Prior to separation from CPB, a second blood sample was drawn and AT, thrombin antithrombin complex (TAT), D-dimer, and anti-Xa levels were measured. Linear and logistic regression were performed for data analysis.

    Preoperative low AT level was significantly associated with increased transfusion of red blood cells (RBCs) and fresh frozen plasma (FFP) during CPB, but not after separation from CPB. The incidence of thrombosis and re-operation were not associated with preoperative AT levels.     Antibodies against ganglioside complexes (GSCs) are associated with various clinical features and subtypes of Guillain-Barré syndrome (GBS).

    One-hundred patients were evaluated for antibodies to GSCs formed by combination of GM1, GM2, GD1a, GD1b, GT1b, and GQ1b using manual enzyme linked immuno-sorbent assay (ELISA).

    Twenty-six patients were GSC antibody-positive, most frequent being against GM1-containing GSC (76.9%). Gender distribution, mean age, symptom-duration, antecedent events, electrophysiological subtypes, requirement for mechanical ventilation, and median duration of hospital stay were comparable between the GSC antibody-positive and negative groups. There was no association between specific GSC antibody and electrophysiological subtypes or clinical variants. After controlling for false discovery rate (FDR) using the Benjamini-Hochberg method, the number of subjects who improved in overall disability sum score, modified Erasmus GBS outcome score, and neuropathy symptom score at discharge was significantly higher in the GSC antibody-positive group.
