    Underreporting of harms in randomized controlled trials (RCTs) may lead to incomplete or erroneous assessments of the perceived benefit-to-harm profile of an intervention. To compare benefit with harm in clinical practice and future clinical studies, adverse event (AE) profiles including severity need to be understood. Even though patients report harm symptoms earlier and more frequently than clinicians, rheumatology RCTs currently do not provide a reporting framework from the patient's perspective regarding harms. Our objective for this meta-research project was to identify AEs in order to determine harm clusters and whether these could be self-reported by patients. Our other objective was to examine reported severity grading of the reported harms.

    We considered primary publications of RCTs eligible if they were published between 2008 and 2018 evaluating pharmacological interventions in patients with a rheumatic or musculoskeletal condition and if they were included in Cochrane reviews. We extracted dataof clinician- or patient-reported severity in the primary articles of the 93 trials. However, we identified 33 terms suggesting severity, but severity grading was discernible in only 9%, precluding a breakdown by severity in this systematic review.

    Our results support the need for a standardized framework for patients' reporting of harms in rheumatology trials. Reporting of AEs with severity should be included in future reporting of harms, both from the patients' and investigators' perspectives.

However, its psychometric properties have not been examined in a clinical sample of parents of children with disruptive behavior. Examining the psychometric properties of the PCS in this population is important given research linking parent attribution with childhood disruptive behavior. The present study aimed to (a) examine the psychometric properties of the PCS in a sample of parents whose children were clinic-referred for disruptive behavior problems; and (b) investigate the concurrent validity of the PCS and its factors using correlations with parent reports of children's emotional and behavioral difficulties, and parenting skills (i.e., discipline, supervision). A confirmatory factor analysis was run on 225 parents' responses on the PCS, and revealed that a two-factor structure of the PCS fit the data well. Significant correlations were found between Parent Causal Attributions (Factor 1) and parent-reported parenting difficulties. Child Responsible Attributions (Factor 2) were correlated with elevations in children's emotion, attention, and conduct difficulties. The results provide information on the utility of the PCS for parents of children with disruptive behavior and its potential clinical relevance.Pathological worry is characterized by an inability to distract or disengage from worry, and this uncontrollability is the defining feature of generalized anxiety disorder (GAD). The present study assessed a novel computerized strategy that targets these attention difficulties. Worry Disengagement Training (WDT), which involves alternating between writing about one's worry and positive topics, was evaluated in a sample with elevated worry (N = 50), most of whom met for GAD diagnosis (66%). Compared to waitlist, WDT led to increased ability to disengage from in vivo worry on a breath focus task, resulting in fewer negative intrusions (β = -.29, p = .02; sr2 = .08). Relative to waitlist, WDT also led to lower self-reported general worry (β = -.36, p = .001, sr2 = .14) and depressive symptoms (β = -.25, p = .02, sr2 = .07). These effects remained in the subset of participants meeting criteria for GAD. WDT did not impact anxious arousal, suggesting some specificity of effects. These findings provide preliminary support for WDT as an effective strategy to increase disengagement ability and reduce worry and depression. Therefore, we analyzed predictors of outcome in an open trial (n=30) of ICBT for OCD using the OCD-NET platform, which consists of 10 online modules delivered with therapist support. Hibernating myocardium is characterized by reduced regional contractile function and blood flow, which both recover after reperfusion or revascularization. Short-term myocardial hibernation is an adaptation of contractile function to the reduced blood flow such that energy and substrate metabolism recover during the ongoing ischaemia. Chronic myocardial hibernation is characterized by severe morphological alterations and altered expression of metabolic and pro-survival proteins. Myocardial stunning is observed clinically and must be recognized but is rarely haemodynamically compromising and does not require treatment. Myocardial hibernation is clinically identified with the use of imaging techniques, and the myocardium recovers after revascularization. Several trials in the past two decades have challenged the superiority of revascularization over medical therapy for symptomatic relief and prognosis in patients with chronic coronary syndromes. A better understanding of the pathophysiology of myocardial stunning and hibernation is important for a more precise indication of revascularization and its consequences. Therefore, this Review summarizes the current knowledge of the pathophysiology of these characteristic reperfusion phenomena and highlights their clinical implications.

Twenty-five percent of prescribing in patients on excess CS was judged avoidable; attributed to either prescribing long-term CS without evidence of consideration of alternative strategies, prescribing error or miscommunication. More patients received immunomodulatory therapy (IMT) in the group treated with CS above threshold than below threshold (p < 0.001) but there was no significant difference in doses of IMT.

    Gastric cancer (GC) with hepatic metastasis has a poor prognosis. Understanding the molecular mechanisms involved in hepatic metastasis may contribute to the development of sensitive diagnostic biomarkers and novel therapeutic strategies.

    We performed transcriptome analysis of surgically resected specimens from patients with advanced GC. One of the genes identified as specifically associated with hepatic metastasis was selected for detailed analysis. GC cell lines with knockout of the candidate gene were evaluated in vitro and in vivo. Expression of the candidate gene was analysed in GC tissues from 300 patients.

    Ethanolamine kinase 2 (ETNK2) was differentially upregulated in GC patients with hepatic metastasis. ETNK2 expression was elevated in GC cell lines derived from haematogenous metastases. ETNK2 knockout significantly suppressed proliferation, invasion, and migration; increased apoptosis; reduced Bcl-2 protein expression; and increased phosphorylated p53 expression. In mouse xenograft models, ETNK2 knockout virtually abolished hepatic metastasis. Stratification of GC patients based on ETNK2 mRNA level revealed significant associations between high ETNK2 tumour expression and both hepatic recurrence and worse prognosis.

  • In this work, several attempts were made to prepare nanoceramic zirconate (Nd₂Zr₂O7) powder for pigment applications. The single pyrochlore phase, Nd₂Zr₂O7 was successfully synthesized with the aid of flux at relatively low temperature (1000 °C) upon milling the reaction mixture made from Nd₂O₃ and ZrO₂ in a tungsten carbide vial. Several characterizations were done on the thermal analysis (TG/DTA), X-ray diffraction (XRD), High-resolution scanning electron microscope (HRSEM), energy dispersive X-ray (EDX) analysis, Fourier-transform infrared (FT-IR) analysis, and near-infrared (NIR spectroscopy). A single-phase nanocrystalline (˜28 nm) pyrochlore structure (Fd3m) was confirmed through XRD analysis. A highly uniform particle in the size of ˜110 nm was observed for the Nd₂Zr₂O7 phase in the microimages. The vibrational (FT-IR) peaks at 423 cm-1 and 510 cm-1 in FTIR spectroscopic study confirmed the formation of pyrochlore structure. Higher NIR reflectivity recorded for this material in the 750-2500 nm region shows its novel application in color cool pigments for energy-saving paints reminiscent of Bi2-xY x Ce²O7, Bi²Ce2-xTbxO7, and Gd²Ti²O7.Magnetic CoFe₂O₄ nanoparticles were successfully manufactured through the process of nitrate combustion using anhydrous ethanol as fuel, they together with their intermediate were characterized by thermo gravimetric (TG) analysis, selected area electron diffraction (SAED), transmission electron microscope (TEM), vibrating sample magnetometer (VSM), and X-ray diffraction (XRD). These results indicated a phenomenon that the magnetic CoFe₂O₄ nanoparticles could be formed at 400 °C, the average grain size, the specific magnetization, and the specific surface area of magnetic CoFe₂O₄ nanoparticles fabricated at 400 °C for 2 h with 30 mL anhydrous ethanol were corresponding 20 nm, 78.0 Am²/kg and 83.2 m²/kg. Magnetic CoFe₂O₄ nanoparticles were in application to adsorb methyl blue (MB) of wastewater, and their adsorption performances and electrochemical properties were investigated, the adsorption process data well agreed with the pseudo-second-order kinetics model in concentration ranging from 100 mg/L to 400 mg/L of MB. Compared with Freundlich model, Langmuir model (correlation coefficient R² = 0.9976) could evaluate the adsorption equilibrium state of MB onto CoFe₂O₄ nanoparticles at indoor temperature, so the monomolecular layer adsorption mechanism was demonstrated to be the mechanism of the MB molecules' adsorption onto CoFe₂O₄ nanoparticles.ZrC was produced by the combustion synthesis technology using Cu, Zr, and graphite as the starting element powders. The synthesis mechanism of ZrC was investigated by the combustion wave quenching experiment. Furthermore, the effects of sizes of C and Cu on the combustion synthesis behavior and products were also explored. Results revealed that ZrC was fabricated through the displacement reaction between C and Cu-Zr liquid. The Cu size hardly affected the combustion temperature and resultant products, indicating that the preparation cost of ZrC could be decreased by employing coarse Cu powders. With increasing C size, the burning temperature and ceramic particle size reduced. Graphite with size of 2.6 μm was used as the C source, and only ZrC nanoparticles and Cu were obtained. The products could be employed to prepare nano-sized ZrC/Cu composites without the elimination of by-products.The present work describes the preparation and the investigation of the room temperature electrical and optical properties of a series of liquid nanocomposites (lnC) prepared with different concentrations of multiwalled carbon nanotubes (MWCNT) in a variety of liquid matrices glycerin, Vaseline, glucose, propylene glycol and silicone oil (SIO). Special attention is deserved to the SIO matrix, owing to its convenient electrical properties for our purposes. We verified that a small percent fraction of MWCNT dispersed along the SIO matrix is capable of improving the electrical conductivity of the matrix by orders of magnitude, indicating that the MWCNT strongly participates in the electrical conduction mechanism.     In this work, several attempts were made to prepare nanoceramic zirconate (Nd₂Zr₂O7) powder for pigment applications. The single pyrochlore phase, Nd₂Zr₂O7 was successfully synthesized with the aid of flux at relatively low temperature (1000 °C) upon milling the reaction mixture made from Nd₂O₃ and ZrO₂ in a tungsten carbide vial. Several characterizations were done on the thermal analysis (TG/DTA), X-ray diffraction (XRD), High-resolution scanning electron microscope (HRSEM), energy dispersive X-ray (EDX) analysis, Fourier-transform infrared (FT-IR) analysis, and near-infrared (NIR spectroscopy). A single-phase nanocrystalline (˜28 nm) pyrochlore structure (Fd3m) was confirmed through XRD analysis. A highly uniform particle in the size of ˜110 nm was observed for the Nd₂Zr₂O7 phase in the microimages. The vibrational (FT-IR) peaks at 423 cm-1 and 510 cm-1 in FTIR spectroscopic study confirmed the formation of pyrochlore structure.     Broken appointments are an important cause of waste in health care. Patients who fail to attend incur costs to providers, deny trainees learning opportunities, and impact their own health as well as that of other patients who are waiting for care.

    A total of 410,000 appointment records over 3 years were extracted from our electronic health record. We conducted exploratory data analysis and assessed correlations between appointment no-shows and other attributes of the appointment and the patient. The University of Michigan Medical School's Committee on Human Research reviewed the study and deemed that no Institutional Review Board oversight was necessary for this quality improvement project that was, retrospectively, turned into a study with previously de-identified data.

    The patient's previous attendance record is the single most significant correlation with attendance. We found that patients who said they are "scared" of dental visits were 62% as likely to attend as someone reporting "no problem." Patients over 65 years of age have better attendance rates. There was a positive association between receiving email/text confirmation and attendance. A total of 94.9% of those emailed a reminder and 92.2% of those who were texted attended their appointment.

    We were able to identify relationships of several variables to failed and attended appointments that we were previously unknown to us. This knowledge enabled us to implement interventions to support better attendance at Dental Clinics at the University of Michigan, improving patient health, student training, and efficient use of resources.

    This work evaluated antimicrobial photodynamic therapy (PDT), sonodynamic therapy (SDT), and the association of both therapies (sonophotodynamic therapy [SPDT]), mediated by curcumin (Cur) against Staphylococcus aureus biofilm. Next, additional strategies for these treatments were assessed.

    S. aureus biofilms received PDT, SDT, and SPDT, mediated by Cur (80 µM), LED light (450 nm), and 1 MHz ultrasound. The same treatments were also performed adding a strategy Cur with sodium dodecyl sulfate (SDS), Cur with potassium iodide (KI) or a pre-treatment with ultrasound. Cell viability was determined and biofilm architecture was evaluated under confocal microscopy.

    SPDT was more effective to inactivate the bacteria than PDT and SDT. SDS achieved the greatest viability reductions, followed by KI and ultrasound pre-treatment. Confocal images revealed biofilm disruption and a reduced number of cells in all treatments. However, SPDT exhibited a pronounced effect and it was greater using SDS.

Second, the ARIMA and ANN's performance was compared statistically between 6912 Seasonal ARIMA (SARIMA) and 110,592 feedforward ANN with different model structures, based on the mean square error (MSE) distributions depicted by boxplots. In a statistical view, the ANN models obtained a significantly lower median value and a more concentrated distribution of validation MSEs, which indicated lighter overfitting and better generalization ability. Furthermore, the optimal SARIMA models' performance is inferior to even the median of the ANN models in the case study. In contrast with the previous comparisons among selected models, the statistical comparison in this study shows lower uncertainty.The aim of this study was to evaluate the effect of pulp and leaves of Glycyrrhiza glabra to reduce the ruminal biogas production in sheep. Five experimental diets of two levels of Glycyrrhiza glabra pulp (GGP) and Glycyrrhiza glabra leaves (GGL) at 150 and 300 g/kg dry matter (DM) were assessed for biogas production and fermentation parameters. Diets were control (diet without GGP or GGL), GGP15 (diet contains GGP at 150 g/kg DM), GGP30 (diet contains GGP at 300 g/kg DM), GGL15 (diet contains GGL at 150 g/kg DM), and GGL30 (diet contains GGL at 300 g/kg DM). Inclusion of 150 and 300 g/kg GGP and 300 g/kg GGL decreased (P less then 0.0001) asymptotic biogas production (A), fermentation rate (μ), biogas production at 24 h of incubation (GP24), apparent degraded substrate (ADS), in vitro organic matter disappearance (OMD), and metabolizable energy (ME). Microbial protein biomass (MP) was improved (P = 0.003) by GGP15, GGL15, and GGL30 versus control. Total VFAs (P = 0.003), acetate (P = 0.009), and butyrate (P = 0.002), CH4 (mmol and mL/g OMD), CO2 (mmol and mL/g OMD) (P = 0.0003 and P = 0.0002, respectively), were decreased in GGP15, GGP30, and GGL30 diets versus control. Acetate to propionate ratio (Ac/Pr) was decreased (P = 0.038) in GGL30 diet compared to other diets. In this research, we collected Qinglong antimony mine tailings (8-meter deep) and then analyzed the content changes, geochemical behavior, and ecological risk assessment of 7 heavy metals (Sb, As, Cr, Cd, Cu, Zn, Pb) in the tailing profile, providing a theoretical basis for strengthening the source control and risk control of heavy metals. In addition, the chemical forms of Sb and As were analyzed, and the relationship between their forms and their physical and chemical properties was analyzed by redundancy analysis (RDA). The results showed that the concentrations of Sb (671.97-13896.62 mg/kg), As (287.38-657.36 mg/kg), Cu (27.61-74.48 mg/kg), and Cd (0.49-1.76 mg/kg) in the tailings greatly exceeded their background values, those of Pb (15.67-125.74 mg/kg) and Cr (22.69-185.88 mg/kg) moderately exceed their background values, while that of Zn (41.66-94.48 mg/kg) was slightly below its background value. Among the chemical forms of Sb and As, the residual fraction (F4) had the highest concentration. RDA showed that the pH and tailing particle size were significantly correlated with the chemical species content of Sb and As (p Cr. The potential environmental risk index showed that Sb and As imposed a serious risk and Cr, Cd, Cu, Pb, and Zn imposed a low risk, which was basically consistent with the results of the improved Igeo values. Currently, the restoration of cut slope mainly focuses on the aboveground landscapes and slope stability. Yet, it remains elusive about the belowground ecosystem functions at the early stage of restoration. In this study, we evaluated the belowground ecosystem functions of cut slopes that had been restored approximately 3 years using soil enzymatic activities, microbial biomass, and stoichiometry as the proxies. The results indicated that the phenol oxidase activity was higher in cut slopes, while the activities of β-1,4-glucosidase, β-1,4-N-acetylglucosaminidase, leucine aminopeptidase, and acid phosphatase were lower in cut slope soils compared with natural soils. Soil nitrogen availabilities (total and/or ammonium nitrogen) showed high negative correlations with the phenol oxidase activity and positive correlations with the activities of almost all other enzymes. These results suggested that soil nitrogen was the key factor in driving the shifts of enzymatic activities across two types of soils. Moreover, we found the imbalance of soil nutrients in cut slope soils, especially the carbon vs. nitrogen and the nitrogen vs. phosphorus. By applying the vector analysis, we found that the vector A values were more than 45° in all samples, suggesting that microbial phosphorus limitation occurred in both cut slope and natural soils. These findings suggested that maintaining the balance of soil nutrient supplies is important to the recovery of the below-ground ecosystem functions at the early restoration stage of cut slopes. This study provided new insights into designing the ecological restoration strategies for cut slopes by considering the belowground ecosystem functions.

In this study, we evaluated the belowground ecosystem functions of cut slopes that had been restored approximately 3 years using soil enzymatic activities, microbial biomass, and stoichiometry as the proxies. The results indicated that the phenol oxidase activity was higher in cut slopes, while the activities of β-1,4-glucosidase, β-1,4-N-acetylglucosaminidase, leucine aminopeptidase, and acid phosphatase were lower in cut slope soils compared with natural soils. Soil nitrogen availabilities (total and/or ammonium nitrogen) showed high negative correlations with the phenol oxidase activity and positive correlations with the activities of almost all other enzymes. These results suggested that soil nitrogen was the key factor in driving the shifts of enzymatic activities across two types of soils. Moreover, we found the imbalance of soil nutrients in cut slope soils, especially the carbon vs. nitrogen and the nitrogen vs. phosphorus. By applying the vector analysis, we found that the vector A values were more than 45° in all samples, suggesting that microbial phosphorus limitation occurred in both cut slope and natural soils. These findings suggested that maintaining the balance of soil nutrient supplies is important to the recovery of the below-ground ecosystem functions at the early restoration stage of cut slopes.     Iowa is among several rural Midwestern states with the highest proportions of contralateral prophylactic mastectomy (CPM) in women < 45 years of age. We evaluated the role of rurality and travel distance in these surgical patterns.

    Women with unilateral breast cancer (2007-2017) were identified using Iowa Cancer Registry records. Patients and treating hospitals were classified as metro, nonmetro, and rural based on Rural-Urban Continuum Codes. Differences in patient, tumor, and treatment characteristics and median travel distance (MTD) were compared. Characteristics associated with CPM were evaluated with multivariate logistic regression.

    22,158 women were identified 57% metro, 26% nonmetro and 18% rural. Young rural women had the highest proportion of CPM (52%, 39% and 40% for rural, metro, nonmetro women < 40 years). Half of all rural women had surgery at metro hospitals; these women had the longest MTD (62 miles). Among all women treated at metro hospitals, rural women had the highest proportion of CPM (17% rural vs 14% metro/nonmetro, p = 0.007). On multivariate analysis, traveling ≥ 50 miles (ORs 1.43-2.34) and rural residence (OR = 1.29) were independently predictive of CPM. Other risk factors were young age (< 40 years OR = 7.28, 95% CI 5.97-8.88) and surgery at a metro hospital that offers reconstruction (OR = 2.30, 95% CI 1.65-3.21) and is not NCI-designated (OR = 2.34, 95% CI 1.92-2.86).

Several methods have been reported to synthesize AuNPs; however, most of them were not ecofriendly. In the present study, the green synthesis of AuNPs has been carried out. Using the green synthesis method, AuNPs of T. indica were synthesized at room temperature (25 °C) by mixing 5 mL of HAuCl4 (1 mM) with 1 mL of T. indica seed extract solution. This extract solution was prepared by taking 5 gm dry seeds in 100 mL of double deionized water with continuous stirring for up to 24 h at 80 °C. The stability of AuNPs was confirmed with the help of relevant experimental techniques including ultraviolet-visible (UV/Vis) showing maximum absorbance at 535-540 nm, Fourier transform infrared showing a broad signal at 3464 cm-1 which can be attributed to either amide or hydroxyl functionalities and atomic force microscopy analysis showed that the biomaterial surrounding AuNPs was agglomerated which proves the formation of discrete nanostructutres. These AuNPs have been evaluated for their antibacterial potential. The results revealed good antibacterial activity of the samples against. Klebsiella pneumonia, Bacillus subtilis and Staphylococcus epidermidis with 10-12 mm zone of inhibition range. The AuNPs were also found stable at high temperature, over a range of pH and in 1 mM salt solution. Patients with these diseases have increased rates of COVID-19 aggravation and mortality. Because periodontopathic bacteria have been isolated from the bronchoalveolar lavage fluid of patients with COVID-19, periodontitis may be a risk factor for COVID-19 aggravation. However, the molecular links between periodontitis and COVID-19 have not been clarified. In this study, we found that the culture supernatant of the periodontopathic bacterium Fusobacterium nucleatum (CSF) upregulated the SARS-CoV-2 receptor angiotensin-converting enzyme 2 in A549 alveolar epithelial cells. In addition, CSF induced interleukin (IL)-6 and IL-8 production by both A549 and primary alveolar epithelial cells. The results revealed a significant decrease in the cortisol baseline at the end of the AAI program, suggesting that the activities carried out with humans and in a different environment could improve the welfare of dogs housed in kennels. However, the molecular links between periodontitis and COVID-19 have not been clarified. In this study, we found that the culture supernatant of the periodontopathic bacterium Fusobacterium nucleatum (CSF) upregulated the SARS-CoV-2 receptor angiotensin-converting enzyme 2 in A549 alveolar epithelial cells. In addition, CSF induced interleukin (IL)-6 and IL-8 production by both A549 and primary alveolar epithelial cells. CSF also strongly induced IL-6 and IL-8 expression by BEAS-2B bronchial epithelial cells and Detroit 562 pharyngeal epithelial cells. Saliva samples were collected every two weeks in three conditions at the kennel (baseline), after transportation and at the end of the working sessions. The results revealed a significant decrease in the cortisol baseline at the end of the AAI program, suggesting that the activities carried out with humans and in a different environment could improve the welfare of dogs housed in kennels. The groEL gene was amplified and sequenced to perform genetic characterization and phylogenetic analysis. A total of 188 blood samples were collected from different wild ungulates species showing an overall prevalence of 63.8% (88.7% in wild ruminants and 3.6% in wild boars). The prevalence of A. phagocytophilum DNA in ticks (manly Ixodes ricinus), and keds collected from wild ruminants was high, reflecting the high infection rates obtained in their hosts. Among ticks collected from wild boars (Hyalomma marginatum and Dermacentor marginatus) no DNA was detected. Phylogenetic analysis demonstrated the presence of ecotype I and II. To date, this is the first Italian report of ecotype I in alpine chamois, mouflon, and wild boar species. During lockdown aimed at containing the coronavirus disease (COVID-19) spread, children have been forced to stay at home. The present study aimed at investigating the lifestyles of outpatients (aged 5-17 years) with complicated obesity enrolled in the day-hospital food education program at the Children's Hospital Bambino Gesù in Rome. A survey was performed based on a structured questionnaire, investigating dietary habits and lifestyles. The questionnaire answers were rated as "yes/no/sometimes" or "often/never/sometimes". Eighty-eight families correctly completed the questionnaire between March and May 2020. The results highlighted that 85.2% (N = 75) of the patients ate breakfast regularly, and 64.3% (N = 72) consumed fruit as an afternoon snack. However, 21.6% (N = 19) did just "often" home workouts, and 50.0% (N = 44) reported an increase of feeling hungry with "sometimes" frequency. There is a significant relationship of feeling hungry with gender (p less then 0.0001) and age (p = 0.048) and, also, between gender with having breakfast (p = 0.020) and cooking (p = 0.006). RA-loaded nanovesicles (5, 10 and 20 mg/kg) or free RA (20 mg/kg) were orally administered from three days prior to colitis induction and during days 1, 3, 5 and 7 of DSS administration. Additionally, the organelle-specific cell death pathway was investigated in cells treated with PHMG-p. PHMG-p-FITC co-localized with the endoplasmic reticulum (ER), and PHMG-p induced ER stress in A549 cells and ****. The ER stress inhibitor tauroursodeoxycholic acid (TUDCA) was used as a pre-treatment to verify the role of ER stress in PHMG-p-induced cytotoxicity. The cells treated with PHMG-p showed apoptosis, which was inhibited by TUDCA. Geometrical parameters (layer thickness and surface roughness) and physical parameters (direct optical bandgap, free carrier concentration, mobility, and specific electrical resistivity) were determined from spectroscopic ellipsometry data modeling. Specific electrical resistivity determined from the Drude oscillator corresponds well with the results from electrical measurements.

METHODS A systematic search was conducted on databases like PubMed, Scopus, Web of Science, and Embase with components "air pollution" and "myocardial infarction". The summary relative risk (RR) and 95% confidence intervals (95%CI) were also calculated to assess the association between the PM2.5 and MI. RESULTS Twenty-six published studies were ultimately identified as eligible candidates for the meta-analysis of MI until Jun 1, 2018. The results illustrated that a 10-μg/m 3 increase in PM2.5 was associated with the risk of MI (RR = 1.02; 95% CI 1.01-1.03; P ≤ 0.0001). The heterogeneity of the studies was assessed through a random-effects model with p  less then  0.0001 and the I2 was 69.52%, indicating a moderate degree of heterogeneity. We also conducted subgroup analyses including study quality, study design, and study period. Accordingly, it was found that subgroups time series study design and high study period could substantially decrease heterogeneity (I2 = 41.61, 41.78). CONCLUSIONS This meta-analysis indicated that exposure - response between PM2.5 and MI. The aim of this study was to identify the prevalence of and risk factors for frailty among elderly patients in China. STUDY DESIGN AND SETTING This cross-sectional study surveyed 9996 elderly patients from 6 tertiary-level hospitals in China. The prevalence of frailty among patients from selected wards was surveyed by trained investigators. A mixed-effects Poisson regression model was used to analyse the factors associated with frailty among elderly patients. RESULTS The mean age of all subjects was 72.47 ± 5.77 years. The prevalence rate of frailty in this study was 18.02%. After adjustments were made for the confounding effect of the clustering of hospital wards, a mixed-effects Poisson regression model showed that the associated factors of frailty included the following age (OR 1.016, 95% CI 1.012-1.020), BMI  28 (OR 0.897, 95% CI 0.856-0.940); higher education level, including middle school (OR 0.915, 95% CI 0.857, 0.977) and diploma and above (OR 0.891, 95% CI 0.821, 0.966); and current alcohol use (OR 0.869, 95% CI 0.815, 0.927). CONCLUSION We identified a relatively high prevalence of frailty among elderly patients, and there are several associated factors among the population derived from this investigation of a large-scale, multicentre, nationally representative Chinese elderly inpatient population. RESULTS Th2 cells, T helper cells, and Mast cells were identified as prognostic immune cells in both TCGA and GEO groups. Then SUPV3L1 and SLC22A17 were identified as hub genes which may affect immune cell infiltration by correlation analysis. Survival analysis further proved that hub genes and prognostic immune cells are associated with the prognosis of gastric cancer. In gastrointestinal tumors, hub genes and prognostic immune cells also found differences in non-tumor and tumor tissues. CONCLUSIONS We found that three immune cells infiltration are associated with the prognosis of gastric cancer and further identify two hub genes. These two key genes may affect immune cell infiltration, result in the different prognosis of patients.BACKGROUND Intravenous remifentanil patient-controlled analgesia (RPCA) is an alternative for epidural analgesia (EA) in labor pain relief. However, it remains unknown whether RPCA is superior to EA in decreasing the risk of intrapartum maternal fever during labor. METHODS According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic review and meta-analysis was performed by searching PubMed, EMBASE and the Cochrane Central Register of Controlled Trials from inception to April 2019. All randomized controlled trials (RCTs) investigating the risk of intrapartum maternal fever with RPCA compared with EA alone or EA in combination with spinal analgesia during labor were included. https://www.selleckchem.com/products/sch772984.html RESULTS A total of 825 studies were screened, and 6 RCTs including 3341 patients were identified. Compared with EA, RPCA was associated with a significantly lower incidence of intrapartum maternal fever (risk ratio [RR] 0.48, P = 0.02, I2 = 49%) during labor analgesia. After excluding 2 trials via the heterogeneity analysis, there was no difference in the incidence of intrapartum fever between patients receiving RPCA and those receiving EA. Satisfaction with pain relief during labor was lower in the RPCA group than that in the EA group (- 10.6 [13.87, - 7.44], P  less then  0.00001, I2 = 0%). The incidence of respiratory depression was significantly greater in the RPCA group than that in the EA group (risk ratio 2.86 [1.65, 4.96], P = 0.0002, I2 = 58%). It is vital decision makers implement effective strategies to help improve air pollution, especially in developing countries or prevent exposure to PM2.5 to protect human health.BACKGROUND To date, most previous studies of frailty among hospitalized elderly Chinese patients have been conducted based on small samples, which cannot represent the elderly patient population. The aim of this study was to identify the prevalence of and risk factors for frailty among elderly patients in China. STUDY DESIGN AND SETTING This cross-sectional study surveyed 9996 elderly patients from 6 tertiary-level hospitals in China. The prevalence of frailty among patients from selected wards was surveyed by trained investigators. A mixed-effects Poisson regression model was used to analyse the factors associated with frailty among elderly patients. RESULTS The mean age of all subjects was 72.47 ± 5.77 years. The prevalence rate of frailty in this study was 18.02%. After adjustments were made for the confounding effect of the clustering of hospital wards, a mixed-effects Poisson regression model showed that the associated factors of frailty included the following age (OR 1.016, 95% CI 1.012-1.020), BMI  28 (OR 0.897, 95% CI 0.856-0.940); higher education level, including middle school (OR 0.915, 95% CI 0.857, 0.977) and diploma and above (OR 0.891, 95% CI 0.821, 0.966); and current alcohol use (OR 0.869, 95% CI 0.815, 0.927). CONCLUSION We identified a relatively high prevalence of frailty among elderly patients, and there are several associated factors among the population derived from this investigation of a large-scale, multicentre, nationally representative Chinese elderly inpatient population. METHODS This article presented research linking gastric cancer to immune cells, based on RNA-seq data of Stomach adenocarcinoma (STAD) and gene expression profile of GSE84437, 24 kinds of tumor-infiltrating immune cells were quantified by single-sample gene set enrichment analysis. RESULTS Th2 cells, T helper cells, and Mast cells were identified as prognostic immune cells in both TCGA and GEO groups. Then SUPV3L1 and SLC22A17 were identified as hub genes which may affect immune cell infiltration by correlation analysis. Survival analysis further proved that hub genes and prognostic immune cells are associated with the prognosis of gastric cancer. In gastrointestinal tumors, hub genes and prognostic immune cells also found differences in non-tumor and tumor tissues. CONCLUSIONS We found that three immune cells infiltration are associated with the prognosis of gastric cancer and further identify two hub genes. These two key genes may affect immune cell infiltration, result in the different prognosis of patients.BACKGROUND Intravenous remifentanil patient-controlled analgesia (RPCA) is an alternative for epidural analgesia (EA) in labor pain relief. However, it remains unknown whether RPCA is superior to EA in decreasing the risk of intrapartum maternal fever during labor. METHODS According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic review and meta-analysis was performed by searching PubMed, EMBASE and the Cochrane Central Register of Controlled Trials from inception to April 2019. All randomized controlled trials (RCTs) investigating the risk of intrapartum maternal fever with RPCA compared with EA alone or EA in combination with spinal analgesia during labor were included. https://www.selleckchem.com/products/sch772984.html RESULTS A total of 825 studies were screened, and 6 RCTs including 3341 patients were identified. Compared with EA, RPCA was associated with a significantly lower incidence of intrapartum maternal fever (risk ratio [RR] 0.48, P = 0.02, I2 = 49%) during labor analgesia. After excluding 2 trials via the heterogeneity analysis, there was no difference in the incidence of intrapartum fever between patients receiving RPCA and those receiving EA. Satisfaction with pain relief during labor was lower in the RPCA group than that in the EA group (- 10.6 [13.87, - 7.44], P  less then  0.00001, I2 = 0%). The incidence of respiratory depression was significantly greater in the RPCA group than that in the EA group (risk ratio 2.86 [1.65, 4.96], P = 0.0002, I2 = 58%). The incidence of Apgar scores less then  7 at 5 min in the RPCA group was equivalent to that in the EA group. CONCLUSION There is no solid evidence to illustrate that the incidence of intrapartum maternal fever is lower in patients receiving intravenous RPCA than in patients receiving EA.
