  • Globally, harmful algal blooms (HABs) are on the rise, as is evidence of their toxicity. The impacts associated with blooms, however, vary across Nation states, as do the strategies and protocols to assess, monitor, and manage their occurrence. In Canada, water quality guidelines are standardized nationally, but the management strategies for HABs are not. Here, we explore current strategies to understand how to better communicate risks associated with HABs to the public. Our team conducted an environmental scan on provincial and territorial government agency protocols around HABs. Results suggest that there are variations in the monitoring, managing, and communicating of risk to the public British Columbia, Manitoba, New Brunswick, and Quebec have well-established inter-agency protocols, and most provinces report following federal guidelines for water quality. Notably, 3 northern territories have no HABs monitoring or management protocols in place. More populous provinces use a variety of information venues (websites, social media, on site postings, and radio) to communicate risks associated with HABs, whereas others' communications are limited. To induce more collaboration on HABs monitoring and management and reduce the associated risks, creating a coherent system with consistent messaging and inter-agency communication is suggested.
    Coronavirus disease (COVID-19) is caused by the severe acute respiratory syndrome coronavirus 2 (SARS-COV2). COVID-19 is highly contagious, potentially fatal, and a global public health concern. Combining optimized personal protective equipment (PPE) use and hand hygiene is the best strategy for preventing COVID-19 in health care workers (HCWs).

    We conducted a national cross-sectional web-based survey of HCWs in the infection control program (IPC) in Thailand between May 5, 2020 and May 15, 2020. The primary objective was the prevalence of optimized PPE use amongst HCWs. The secondary objective was identification of the independent predictors of optimized PPE use.

    We received a response from 46% of HCWs (756/1650), and all those who responded were nurse or HCWs who were registered in the IPC network. Five HCWs were excluded because of missing data, and 751 were included in the final analysis. The prevalences of PPE use were 22% (168/751) for optimized PPE use, 78% (583/751) for non-optimized PPE use, 35d PPE use were COVID-19 knowledge-based factors and education level. The central objective was to use a postfeminist lens to critically explore how wine mom culture is part of the consumption, (re)production and (re)configuration of the ideologies of 'good' and 'bad' motherhood occurring in online gendered spaces. The findings demonstrated the ubiquity of wine mom culture and its contribution to normalized images and meanings of 'liberated motherhood' that may have problematic sociocultural and health implications related to women's alcohol consumption.
    To investigate macular vascular changes and underlying influencing factors using optical coherence tomography angiography (OCTA) after phacoemulsification in patients with cataracts.

    Patients diagnosed with cataracts at the First Affiliated Hospital of Anhui Medical University from March to September 2019 were included. The macular retinal thickness, the microvascular density in the foveal, parafoveal, and perifoveal area, and the area of the foveal avascular zone (FAZ) were measured using OCTA at baseline, 1-week, 1-month, and 3-month post-operation.

    Forty-seven cataract patients (58 eyes) were eligible for this study. Compared with baseline thickness, the foveal, parafoveal, and perifoveal thicknesses, particularly in the inner retina, significantly increased at 1- and 3-month post-operation (P<0.05). There was a nonsignificant difference in the microvascular density of the foveal and perifoveal areas at 1-week, 1-month, and 3-month post-operation (P>0.05). At 1-month post-operation, the deep vessel density at the perifovea significantly increased (P<0.05). The FAZ area significantly diminished at 1- and 3-month post-operation compared with the baseline data (P<0.05).

  • This study evaluated the influence of diet and environmental conditions on intake behavior and activity of feedlot steers. Feedlot rations used were comprised of a main concentrate (1) corn or (2) barley. A GrowSafe system measured individual animal intake and behavior and HOBO accelerometers measured steer standing time. An Onset weather station collected on site weather data. Steer daily intake displayed a diet by temperature class interaction (p ≤ 0.05). Relative temperature change had no effect on variation in intake (p = 0.60); however, diet influenced variation of intake (p less then 0.01), where corn-fed steers had a greater coefficient of variation (CV) than barley-fed steers (21.89 ± 1.46 vs. 18.72 ± 1.46%). Time spent eating (min d-1) and eating rate (g min-1) both displayed a diet by temperature class interaction (p ≤ 0.05). Diet did not affect steer lying activity (p ≥ 0.12), however, time spent lying (min d-1) and frequency of lying bouts (bouts d-1) increased on relatively cold days while the duration of lying bouts (min bout-1; p less then 0.01) decreased. Short-term environmental temperature changes interacted with diet influencing feedlot beef cattle intake behavior; however, they did not interact with basal diet in respect to steer activity.The COVID-19 pandemic has had a negative impact on education. A study of the fundamentals of α → ω phase transformations induced by high-pressure torsion (HPT) in Ti-Nb-based alloys is presented in the current work. Before HPT, a Ti-3wt.%Nb alloy was annealed at two different temperatures in order to obtain the α-phase state with different amounts of niobium. X-ray diffraction analysis, scanning electron microscopy (SEM) and transmission electron microscopy (TEM) were applied for the characterisation of phase transitions and evolution of the microstructure. A small amount of the β-phase was found in the initial states, which completely transformed into the ω-phase during the HPT process. During HPT, strong grain refinement in the α-phase took place, as did partial transformation of the α- into the ω-phase. Therefore, two kinds of ω-phase, each with different chemical composition, were obtained after HPT. The first one was formed from the β-phase, enriched in Nb, and the second one from the α-phase. It was also found that the transformation of the α-phase into the ω-phase depended on the Nb concentration in the α-Ti phase. The less Nb there was in the α-phase, the more of the α-phase was transformed into the ω-phase.The development and commercialization of new drugs is an articulated, lengthy, and very expensive process that proceeds through several steps, starting from target identification, screening new leading compounds for testing in preclinical studies, and subsequently in clinical trials to reach the final approval for therapeutic use. Here, we report the use of iPSCs as a disease model for drug development in the contest of neurological disorders, including Alzheimer's (AD) and Parkinson's disease (PD), Amyotrophic lateral Sclerosis (ALS), and Fragile X syndrome (FRAX).Epigenetic modifications of the nuclear genome, including DNA methylation, histone modifications and non-coding RNA post-transcriptional regulation, are increasingly being involved in the pathogenesis of several human diseases. Recent evidence suggests that also epigenetic modifications of the mitochondrial genome could contribute to the etiology of human diseases. In particular, altered methylation and hydroxymethylation levels of mitochondrial DNA (mtDNA) have been found in animal models and in human tissues from patients affected by cancer, obesity, diabetes and cardiovascular and neurodegenerative diseases. Moreover, environmental factors, as well as nuclear DNA genetic variants, have been found to impair mtDNA methylation patterns. Some authors failed to find DNA methylation marks in the mitochondrial genome, suggesting that it is unlikely that this epigenetic modification plays any role in the control of the mitochondrial function. On the other hand, several other studies successfully identified the presence of mtDNA methylation, particularly in the mitochondrial displacement loop (D-loop) region, relating it to changes in both mtDNA gene transcription and mitochondrial replication. Overall, investigations performed until now suggest that methylation and hydroxymethylation marks are present in the mtDNA genome, albeit at lower levels compared to those detectable in nuclear DNA, potentially contributing to the mitochondria impairment underlying several human diseases.

  • Retinal capillary is vulnerable to diabetes, whether gestational diabetes mellitus (GDM) eyes without clinical retinopathy have capillary abnormalities has not been well studied. To observe the microvasculature changes in eyes of GDM women compared with normoglycemic pregnant women and non-pregnant women by optical coherence tomography-angiography (OCT-A).

    GDM women, age-matched normoglycemic pregnant women and non-pregnant women were included in this study. All subjects were examined by OCT-A, vascular density and macular foveal parameters were measured automatically.

    Thirty eight non-pregnant women (NC group), thirty pregnant women without GDM (PC group), and thirty one GDM women (GDM group) were included in this study. There was a significant reduction of vascular density in superficial capillary layer, but an increase in deep capillary layer in PC and GDM groups (P < 0.001). When in terms of the average vascular density, the difference was insignificant among these three groups. Although all the measurements were similar between PC and GDM groups, more capillary "dropout" changes were detected in GDM group. Unexpectedly, the abnormal changes of central macular thickness thinning and foveal avascular zone enlargement seen during pregnancy were improved when compared to PC group.

    The changes of vascular density implied the redistribution of capillary network from superficial to deep layer under pregnancy and GDM states. Although the transient hyperglycemia aggravates the changes of capillary "dropout", GDM group revealed the improvement of central macular thickness thinning and foveal avascular zone enlargement during pregnancy.

Misperceptions and malpractices regarding neonatal care and neonatal complications are the leading causes of neonatal deaths in Bangladesh.     A qualitative study was conducted in Netrakona district of Bangladesh from April to June 2015. Three sub-districts (Upazilas) including Purbadhala, Durgapur and Atpara of Netrakona district were selected purposively. Five focus group discussions (FGDs) and twenty in-depth interviews (IDIs) were conducted in the rural community. Themes were identified through reading and re-reading the qualitative data and thematic analysis was performed.

    We aimed to test which life course model best described the association between leisure-time physical activity (LTPA) and multimorbidity at age 55. We analyzed data from birth to age 55 using the database from the 1958 National Child Development Survey.

    Multimorbidity was considered as the presence of more than one chronic condition. LTPA was measured through questionnaires from 1965 (age 7) to 2013 (age 55), which were applied in eight different occasions. We compared the fit of a series of nested adjusted logistic regression models (representing either the critical, accumulation or sensitive period models) with a fully saturated model. Data were reported as odds ratio (OR) and 95% confidence interval (CI).

    Retinal capillary is vulnerable to diabetes, whether gestational diabetes mellitus (GDM) eyes without clinical retinopathy have capillary abnormalities has not been well studied. To observe the microvasculature changes in eyes of GDM women compared with normoglycemic pregnant women and non-pregnant women by optical coherence tomography-angiography (OCT-A). GDM women, age-matched normoglycemic pregnant women and non-pregnant women were included in this study. All subjects were examined by OCT-A, vascular density and macular foveal parameters were measured automatically. Thirty eight non-pregnant women (NC group), thirty pregnant women without GDM (PC group), and thirty one GDM women (GDM group) were included in this study. There was a significant reduction of vascular density in superficial capillary layer, but an increase in deep capillary layer in PC and GDM groups (P < 0.001). When in terms of the average vascular density, the difference was insignificant among these three groups. Although all the measurements were similar between PC and GDM groups, more capillary "dropout" changes were detected in GDM group. Unexpectedly, the abnormal changes of central macular thickness thinning and foveal avascular zone enlargement seen during pregnancy were improved when compared to PC group. The changes of vascular density implied the redistribution of capillary network from superficial to deep layer under pregnancy and GDM states. Although the transient hyperglycemia aggravates the changes of capillary "dropout", GDM group revealed the improvement of central macular thickness thinning and foveal avascular zone enlargement during pregnancy. More recently, emotion recognition software has been developed that detects emotions from video recordings of human faces. However, its validity and comparability to EMG measures is unclear. The aim of the current study was to compare the Affectiva Affdex emotion recognition software by iMotions with EMG measurements of the zygomaticus mayor and corrugator supercilii muscle, concerning its ability to identify happy, angry and neutral faces. Twenty participants imitated these facial expressions while videos and EMG were recorded. Happy and angry expressions were detected by both the software and by EMG above chance, while neutral expressions were more often falsely identified as negative by EMG compared to the software. Overall, EMG and software values correlated highly. In conclusion, Affectiva Affdex software can identify facial expressions and its results are comparable to EMG findings. Our research aims to demonstrate that the theory of the death drive does not represent a complete change in direction for Freud. It is present, in essence, in his earliest work, to the extent that the "principle of inertia" described in 1895 in A Project for a Scientific Psychology (Freud, 1895) can be seen as a precursor to the death drive. Based on a reading of Freud's early formulations of his ideas, we aim to bring to light how certain aporias that seem inherent to the concept of the death drive can be overcome if we consider them in the context of an epistemological model that draws on the paradigms of physics which were conveyed by the Helmholtz School. Namely, we can consider the idea of death drive in reference to the principle of entropy and the laws of thermodynamics. Data for listed SMEs for the years 2016-2018 are analyzed through the panel regression technique. Significantly, the study demonstrates that CEO age is positively interlinked with the growth and CSR activity of the firm. Moreover, the empirical underpinning of the results also reveals that state-owned enterprises and firms with high total assets prefer cognitive CEOs, who accelerate the firm's value and invigorate CSR activity. The number of independent directors is analyzed as a moderator and is concluded to be an intensifier for both SME growth and CSR activity.   • Rigorous characterization of biotherapeutics, and monoclonal antibodies in particular, is a challenging task in terms of ensuring safety, efficacy, and potency of a therapeutic agent because of structural heterogeneity during cell culture, purification and storage. In this work, we used microfluidic capillary electrophoresis-mass spectrometry to analyze intact monoclonal antibody and assess the root cause of increases in acidic and basic variants under stress at high temperature. The antibody was analyzed at multiple levels, including its intact state under native conditions, and subunit and peptide levels. The normal and degraded antibodies at different time points were characterized and compared with each other. We concluded that the basic variants in the unstressed sample were produced C-terminal amidation, while the acidic variants were produced by deamidation. In stressed samples, change in the acidic and main peaks were caused by deamidation, and changes in the basic peaks were caused by both deamidation and oxidation. These results demonstrate that microfluidic capillary electrophoresis-mass spectrometry (CE-MS) is a powerful direct and generic tool for separation and identification of charge heterogeneity of biotherapeutics.Invasive intraductal papillary mucinous neoplasms (inv-IPMNs) have a better prognosis than regular pancreatic ductal adenocarcinoma (PDAC), but no association with status of surgical margins and microscopic infiltration patterns has previously been described. The aim of this study is to review patterns of invasion and the predictive value of clinical guidelines in terms of rates of resection of high-grade dysplasia (HGD) and cancer among intraductal papillary mucinous neoplasms (IPMNs). Consecutively, resected IPMNs between 2011 and 2017 were analyzed. Data were obtained from a prospectively maintained database. A total of 132 patients were identified. Out of these, 38 patients with inv-IPMNs, initially identified as solid lesions suspicious of cancer, were compared with a control group of 101 patients with ordinary PDAC. Lower rates of vascular invasion, perineural invasion, lymph node metastasis, advanced T stage, and R1 status were characteristic of the inv-IPMNs in addition to better overall survival (OS) for a low tumor stage. Furthermore, as novel findings, the PDACs presented with resection margin involvement of 3 or more positive margins (31.3% vs. 9.5%, p = 0.044), associated with poor OS. Of the patients presenting as pT3, the inv-IPMN less often invaded more than one extrapancreatic anatomical structure (40.1% vs. 63.9%, p = 0.03). Regarding the predictive value of clinical guidelines, the frequency of resected HGD in IPMNs with high-risk stigmata (n = 54) and IPMNs with worrisome features was 30.7%, and the frequency of invasive carcinoma was 5.7%. In conclusion, we report a low resection rate of high-risk IPMNs and present novel findings describing inv-IPMNs as a less infiltrative phenotype compared with regular PDAC.Candida albicans is a fungal component of the human gut microbiota and an opportunistic pathogen. Regarding the predictive value of clinical guidelines, the frequency of resected HGD in IPMNs with high-risk stigmata (n = 54) and IPMNs with worrisome features was 30.7%, and the frequency of invasive carcinoma was 5.7%. Here, we explore how transient hormonal signaling coordinates a neural activity state transition and maturation of associative learning. We identify spontaneous, asynchronous activity in a Drosophila learning and memory brain region, the mushroom body. This activity declines significantly over the first week of adulthood. Moreover, this activity is generated cell-autonomously via Cacophony voltage-gated calcium channels in a single cell type, α'/β' Kenyon cells. Juvenile hormone, a crucial developmental regulator, acts transiently in α'/β' Kenyon cells during a young adult sensitive period to downregulate spontaneous activity and enable subsequent enhanced learning. Hormone signaling in young animals therefore controls a neural activity state transition and is required for improved associative learning, providing insight into the maturation of circuits and behavior.

  • Two different dewetting methods, namely pulsed laser-induced dewetting (PLiD)-a liquid-state dewetting process and thermal dewetting (TD)-a solid-state dewetting process, have been systematically explored for Ag thin films (1.9-19.8 nm) on Si substrates for the fabrication of Ag nanoparticles (NPs) and the understanding of dewetting mechanisms. The effect of laser fluence and irradiation time in PLiD and temperature and duration in TD were investigated. A comparison of the produced Ag NP size distributions using the two methods of PLiD and TD has shown that both produce Ag NPs of similar size with better size uniformity for thinner films ( less then 6 nm), whereas TD produced bigger Ag NPs for thicker films (≥8-10 nm) as compared to PLiD. As the film thickness increases, the Ag NP size distributions from both PLiD and TD show a deviation from the unimodal distributions, leading to a bimodal distribution. The PLiD process is governed by the mechanism of nucleation and growth of holes due to the formation of many nano-islands from the Volmer-Weber growth of thin films during the sputtering process. The investigation of thickness-dependent NP size in TD leads to the understanding of void initiation due to pore nucleation at the film-substrate interface. We then accessed The Genomics of Drug Sensitivity in Cancer database to predict the therapeutic drug responses of signature-defined high- and low-risk ** patients in the TCGA cohort. Greater sensitivity to chemotherapy was revealed in the low-risk group. Finally, we conducted gene set enrichment analysis of the signature genes and established, by applying the ESTIMATE algorithm, distinct correlations between the two risk groups and the presence of stromal and immune cell types in the tumor microenvironment. Knocking down circITGA7 can significantly inhibit the proliferation of glioma cells and reduce cell metastasis. Through analysis and dual-luciferase report assay, we found that circITGA7 acts as a sponge for miR-34a-5p targeting VEGFA in glioma. Our study showed that circITGA7 regulates the proliferation and metastasis of glioma cell lines (SW1783&U373) by regulating the miR-34a-5p/VEGFA pathway. In conclusion, our study revealed a regulatory loop for the circITGA7/miR-34a-5p/VEGFA axis to regulate glioma development.

  • RESULTS Forty modeling studies were identified and categorized into 4 groups Markov disability/recurrence (k = 29); CI (k = 2); discrete event simulation (k = 4), and other (k = 5). Only 2 modeling studies included CI as an outcome, and both focused on narrow populations at risk of intracranial aneurysm. None of the models allowed for disease progression in the absence of a stroke recurrence. None of the included studies carried out any sensitivity analysis in relation to model design or structure. CONCLUSIONS Current stroke models used in economic evaluation are not adequate to model poststroke CI or dementia, and will require adaptation to be used for this purpose. © 2020 S. Karger AG, Basel.BACKGROUND Risk factors for post-ERCP complications are well-studied. However, risk factors for complications and success after endoscopic sphincterotomy (EST) for duct stones, are poorly-determined. This study aims to verify risk factors for mortality, complications and success after EST. METHODS A multivariate analysis was carried out in a dataset of ERCPs performed during 17 years. RESULTS 5226 ERCPs were performed, of which 2137 were in patients with bile duct stones (1458 women and 679 men; mean age = 57 years) who underwent EST with attempted stone removal. There were 171 (8%) complications being pancreatitis in 87 (4.1%), bleeding in 48 (2.2%), other complications in 36 (1.8%), and mortality of 0.6%. Successful stone(s) removal was obtained in 2028 cases (94.9%). On multivariate analysis, mortality was associated with age > 60 yr. (1 vs. 0.2%), cholangitis (4.3 vs.0.3%) and EST--related complications (5.8 vs. 0.2%). Complications were associated with unsuccessful stone removal (13.4 vs. 7.5%) and difficult cannulation (13.9 vs. 5.4%). An unsuccessful EST was independently related to difficult cannulation (86.2 vs. 98.7%), precutting (79.4 vs. 96.4%) and complications (86.5 vs. 95.6%). CONCLUSIONS Risk factors for complications after EST for stones are delayed bile duct cannulation and failed stone retrieval. Mortality is higher in older patients, those who presented with an EST-related complication or those who presented initially with cholangitis. Difficult cannulation, EST-related complications and pre-cut were associated with an unsuccessful procedure. In this series, outpatient EST with attempted stone retrieval showed to be as safe as performing the procedure in hospitalized patients. © 2020 S. [Chest 2002;122;1535-1542] by reviewing the effectiveness of air purification for the treatment of home-related allergic asthma (dust mite, dog, cat, and cockroach). We analysed the trials included by McDonald et al. as well as studies published since 2000. Data on asthma symptoms scores (ASS), medication use, forced expiratory volume in 1 s as a percentage of the predicted value (FEV1 %pred), histamine provocative concentration causing a 20% reduction in FEV1 (PC20), Asthma Quality of Life Questionnaire (AQLQ) scores, and fractional exhaled nitric oxide (FeNO) levels were extracted. The effectiveness was examined using metafor (registered in Prospero CRD42019127227). Ten trials including a total of 482 patients (baseline characteristics mean FEV1 %pred 83.2%, I2 = 96.7%; mean PC20 4.93 mg/mL, I2 = 44.0%; mean AQLQ 4.67 [max. 7], I2 = 93.7%; mean FeNO 36.5 ppb, I2 = 0%) were included. We assessed the mean differences in the AQLQ scores as +0.36 (95% CI 0.10 to 0.62, p = 0.01, n = 302, I2 = 0%) and the FeNO levels as -6.67 ppb (95% CI -10.56 to -2.77, p = 0.0008, n = 304, I2 = 0%). The standardised mean differences in all other health outcomes were not significant (ASS -0.68, p = 0.20; medication use -0.01, p = 0.94; FEV1 %pred -0.11, p = 0.34; PC20 +0.24, p = 0.53). We found statistically significant mean differences in the AQLQ scores and FeNO levels in patients with predominantly mild to moderate asthma at baseline. A large trial reported great improvement in the subgroup of patients receiving Global Initiative for Asthma step 4 therapy. Karger AG, Basel.INTRODUCTION Although signet ring cell carcinoma (SRC) is a poorly differentiated cancer subtype, recent studies suggest that endoscopic resection can be applied in small, mucosal early gastric SRC. However, other studies report frequent positive lines at the lateral resection margin after endoscopic treatment. Subepithelial spread beneath normal mucosa can exist in SRC, and such lesions may be the cause of positive margins after endoscopic resection. Thus, we conducted a retrospective study in order to evaluate the significance of subepithelial spread in early gastric SRC. METHOD Medical records of early gastric SRC patients who underwent surgery or endoscopic resection from January 2011 to December 2016 at a single tertiary hospital (Daejeon, South Korea) were reviewed to examine subepithelial spread and clinical datum. 2 expert pathologists reviewed all pathologic specimens and only patients showing a pure SRC component were included. RESULTS 86 patients were initially enrolled and subepithelial spread existed in 62 patients (72.1%). The mean distance of subepithelial spread was 1132.1µm, and maximal the maximal distance was 6000µm. Only discoloration was significantly associated with the presence of a subepithelial spread (p less then 0.05, chi-square test and logistic regression test). Distance of subepithelial spread did not correlate with total lesion size. CONCLUSION Subepithelial spread of early gastric SRC occurs frequently, and can reach up to 6 mm. Lesion discoloration may be associated with the presence of subepithelial spread. Our results suggest that careful decision of margin is needed when performing endoscopic resection of early gastric SRC.  . © 2020 S. We aimed to study the pattern of X inactivation and its effect on a translocated autosome and to find out an explanation of the involvement of chromosome 14 in 2 different structural chromosomal abnormalities. We report on a girl with frontal bossing, hypertelorism, low-set ears, micrognathia, cleft palate, congenital heart disease, and abnormal skin pigmentations. The patient displayed iris, choroidal, and retinal coloboma and agenesis of the corpus callosum and cerebellar vermis hypoplasia. Cytogenetic analysis revealed a karyotype 45,X,der(X)t(X;14)(q24;q11)[85]/46,XX,rob(14;14)(q10;q10),+14[35]. Array-CGH for blood and buccal mucosa showed high mosaic trisomy 14 and an Xq deletion. MLPA detected trisomy 14 in blood and buccal mucosa and also showed normal methylation of the imprinting center. FISH analysis confirmed the cell line with trisomy 14 (30%) and demonstrated the mosaic deletion of the Xq subtelomere in both tissues.

We assessed the mean differences in the AQLQ scores as +0.36 (95% CI 0.10 to 0.62, p = 0.01, n = 302, I2 = 0%) and the FeNO levels as -6.67 ppb (95% CI -10.56 to -2.77, p = 0.0008, n = 304, I2 = 0%). Subepithelial spread beneath normal mucosa can exist in SRC, and such lesions may be the cause of positive margins after endoscopic resection. Thus, we conducted a retrospective study in order to evaluate the significance of subepithelial spread in early gastric SRC. METHOD Medical records of early gastric SRC patients who underwent surgery or endoscopic resection from January 2011 to December 2016 at a single tertiary hospital (Daejeon, South Korea) were reviewed to examine subepithelial spread and clinical datum. 2 expert pathologists reviewed all pathologic specimens and only patients showing a pure SRC component were included. RESULTS 86 patients were initially enrolled and subepithelial spread existed in 62 patients (72.1%). The mean distance of subepithelial spread was 1132.1µm, and maximal the maximal distance was 6000µm. Only discoloration was significantly associated with the presence of a subepithelial spread (p less then 0.05, chi-square test and logistic regression test). Distance of subepithelial spread did not correlate with total lesion size. CONCLUSION Subepithelial spread of early gastric SRC occurs frequently, and can reach up to 6 mm. Lesion discoloration may be associated with the presence of subepithelial spread. We report on a girl with frontal bossing, hypertelorism, low-set ears, micrognathia, cleft palate, congenital heart disease, and abnormal skin pigmentations. The patient displayed iris, choroidal, and retinal coloboma and agenesis of the corpus callosum and cerebellar vermis hypoplasia. Cytogenetic analysis revealed a karyotype 45,X,der(X)t(X;14)(q24;q11)[85]/46,XX,rob(14;14)(q10;q10),+14[35]. Array-CGH for blood and buccal mucosa showed high mosaic trisomy 14 and an Xq deletion. MLPA detected trisomy 14 in blood and buccal mucosa and also showed normal methylation of the imprinting center. FISH analysis confirmed the cell line with trisomy 14 (30%) and demonstrated the mosaic deletion of the Xq subtelomere in both tissues.

  • Transcranial Doppler ultrasound (TCD) is noninvasive and highly sensitive and specific for the diagnosis of patent foramen ovale (PFO). We evaluated the diagnostic implications of the TCD with a saline agitation test as a routine work-up for ischemic stroke patients.

    A TCD bubble study was performed in all consecutive ischemic stroke patients as a routine work-up. We evaluated the prevalence of microembolic signals (MES) for each stroke etiology and the optimal number of MES for predicting the PFO-attributable stroke.

    Subjects (N = 499) with acute ischemic stroke were enrolled. A significant fraction of patients had MES during both normal respiration (5.7-44.4%) and the Valsalva maneuver (19.5-55.6%) across all stroke etiology categories. The optimal MES threshold for the diagnosis of PFO-attributable stroke confirmed by transesophageal echocardiography was 46 MES during the Valsalva maneuver (96% sensitivity and 95% specificity). Applying ≥46 MES during the Valsalva maneuver as a threshold effectively increased the ability to differentially diagnose PFO-attributable stroke from other etiologies. The number of MES during the Valsalva maneuver was negatively correlated with increasing age (r = -.108; P = .016).

    Many children with intellectual disability live with medical comorbidities. This study examined the impacts of comorbidities on quality of life (QOL) of children with intellectual disabilities and whether impacts varied with caregiver perceptions that medical needs had been met.

    Primary caregivers of 447 children (aged 5-19 years) with an intellectual disability reported on their child's medical comorbidities and the extent to which they perceived their child's medical needs had been met in a cross-sectional observational study. The Quality of Life Inventory-Disability was used to measure QOL on a 100-point scale. Linear regression models including interaction terms were used to evaluate their associations.

    Parent-reported recurrent child pain (-4.97, 95% CI -8.21, -1.72), night-time sleep disturbances (-4.98, 95% CI -7.23, -2.73), daytime somnolence (-8.71, 95% CI -11.30, -2.73), seizures that occurred at least weekly (-7.59, 95% CI -13.50, -1.68) and conservatively managed severe scoliosis (-7.39, 95% CI -12.97, -1.81) were negatively associated with child QOL. Despite the majority of parents (~70%) perceiving that their child's medical needs had been met to a great extent, this did not significantly moderate the association between any comorbidities and QOL.

    Comorbidities were common and had marked associations with QOL. Evaluation and management of pain and sleep disturbance continue to be high priorities in improving QOL of young people with intellectual disabilities. The findings suggest that strict patience-display rules were constructed as an occupational impetus that exacts a price by increasing emotional load. Patience was regulated continually by normalizing practices that were culturally embedded in the organizations, by reframing patients' abusive behaviours, and by diffusing undesired emotions using detachment and disengagement practices. The findings contribute to the scarce empirical research on emotional labour of nurse's aides, and to the conceptualization and empirical study of nurses' normalizing regulation practices as cultural-organizational artefacts.

    Respiratory syncytial virus (RSV) is one of the main viral pathogens causing acute respiratory infections in children under 5 years of age but has seldom been studied in Central African Republic (CAF). Taking advantage of the national influenza surveillance network in CAF, this study aimed at providing the first insights into RSV prevalence and seasonality over 4 years of surveillance and the clinical manifestations of RSV in this population in CAF.

    A total of 3903 children under 5 years matching the influenza-like illness (ILI, 68.5%) or severe acute respiratory infection (SARI, 31.5%) case definitions were recruited from January 2015 to December 2018. The presence of RSV viral RNA in nasopharyngeal samples was assessed by RT-PCR, followed by RSV-A and RSV-B typing and Sanger sequencing on a subset of samples. Phylogenetic analyses were carried on partial G protein sequences. Associations between RSV and demographic or clinical manifestations were investigated by statistical analyses.

    Schizophrenia (SCZ) is an incurable neuropsychiatric disorder generally described by impaired social behavior and altered recognition of reality. For the first time, this study explored serum levels of antioxidants (vitamin A, E, and C), malondialdehyde (MDA), macro-minerals (calcium, potassium, and sodium), and trace elements (zinc, iron, and selenium) in Bangladeshi patients with SCZ and thereby, discovering any pathophysiological correlation.

    This case-controlled study evaluated 63 patients with SCZ as cases and 63 healthy individuals as controls. Vitamin A and E levels were defined by RP-HPLC. MDA and vitamin C levels were measured by using UV spectrophotometry, and macro and trace elements by atomic absorption spectroscopy.

    Clinical support systems are widely used in pediatric care. The aim of this study was to assess the support for drug treatments used at pediatric cardiac wards and intensive care units in Sweden.

    Drug information, such as type of drug, indication, dose, and route of administration, for all in-hospital pediatric cardiac patients, was included in the study. Treatments were classified as either on-label (based on product information) or off-label. Support for off-label treatment was stratified by the use of clinical support systems (the national database on drugs, local, or other clinical experience guidelines).

These data provide clear evidence that will help supplement both internal and industry-wide initiatives focused on using prior knowledge for the creation of modular claims for small virus retentive filters and allow better allocations of resources typically spent on potentially unnecessary studies.The dropping of glass vials based on negligence or accidental events that occur during the preparation or mixing of injectable drugs are examples of instances of occupational exposures occurring in a clinical setting. To reduce such risks, several types of glass vial packaging have been developed. We herein compared the resistance of base- and cup-type packaged vials to breakage and scattering of contents during falls with control vials. The falling heights at which test products were dropped were set to 70, 135, and 180 cm. Compared with the control group, appearance changes were inhibited in the cup-type groups. Significant differences were found between the cup-type and control groups at heights of 135 and 180 cm. Next, resistance of packaging to spilling and scattering of solution from the vial was determined. In so doing, the packaging and the packaged drug product are qualified as being suited for their intended use. It is reasonable to propose that the extent of chemical characterization required to qualify the packaging and the packaged drug product depends on the risk that leached substances could adversely affect drug product quality; the higher the risk, the more extensive and rigorous the required qualification. Although regulatory guidance supports and advocates such a risk-based approach to chemical characterization, the existing guidance is founded on an overly simplified approach to risk assessment, leading to incongruous risk classifications for certain classes of drug products. Furthermore, the existing guidance no longer links risk to current requirements concerning the extent of chemical characterization necessary to secure regulatory approval of drug product applications. To address these circumstances, this manuscript proposes and justifies a risk classification process (risk evaluation matrix) for drug products and packaging and a risk-based approach to chemical characterization requirements, linking risk to the degree and rigor of the chemical characterization process and establishing chemical characterization requirements for individual risk classes.

  • A growing body of research has documented the determinants of healthcare expenditure, but no known empirical research has focused on investigating the spatial effects between economic policy uncertainty (EPU) and healthcare expenditure. This study aims to explore the spatial effects of EPU on healthcare expenditure using the panel data of 29 regions in China from 2007 to 2017. Our findings show that healthcare expenditure in China has the characteristics of spatial clustering and spatial spillover effects. Our study also shows that EPU has positive spatial spillover effects on healthcare expenditure in China; that is, EPU affects not only local healthcare expenditure but also that in other geographically close or economically connected regions. Our study further indicates that the spatial spillover effects of EPU on healthcare expenditure only exist in the eastern area. The findings of this research provide some key implications for policymakers in emerging markets.Background The 2016 wildfires in Fort McMurray (Alberta, Canada) led to a massive displacement of 88,000 people and destroyed 2,400 homes. Although no direct human fatality resulted, many individuals feared for their lives or those of their loved ones. Objectives (1) To estimate the prevalence of post-traumatic stress, major depressive, insomnia, generalized anxiety, and substance use disorders in the adult population of Fort McMurray 1 year after the evacuation; (2) To identify pre-, peri-, and post-disaster correlates of mental health disorders. Methods A phone survey using random digit sampling was used to survey evacuees. A total of 1,510 evacuees (response rate = 40.2%, 55.5% women, mean age = 44.11, SD = 12.69) were interviewed between May 9th and July 28th, 2017. Five validated scales were administered the PTSD Symptoms Checklist (PCL-5), the Insomnia Severity Index (ISI), the depression and anxiety subscales of the Patient Health Questionnaire (PHQ-9, GAD-7), and the CAGE Substance Abuse Screening Toovacuees had clinically significant psychological symptoms, including those of insomnia, post-traumatic stress, depression, anxiety, and substance use. This study helped identify individuals more at risk for mental health issues after a natural disaster and could guide post-disaster psychosocial support strategies.The purpose of this study was to characterize different ethnic groups in Kyrgyzstan regarding cardiovascular disease (CVD) and mental distress, and to investigate the association between CVD and mental distress. The mental distress was measured in terms of sleep disturbance, burnout, and stress. Materials and Methods A cross-sectional study was carried out among six ethnic groups in Kyrgyzstan, aged 18 years and above. The sample was stratified for age, education, family status, and income. We used the Karolinska Sleep Questionnaire to assess sleep disturbance, the physical and emotional subscale of the Shirom Melamed Burnout Questionnaire to assess burnout, and the 10-item Perceived Stress Scale to assess perceived stress. We aimed to investigate whether ambulatory orchidopexy can improve the timely repair rate. Methods A retrospective analysis was conducted from 2012 to 2019 at our medical center. Ambulatory orchidopexy was started at our medical center on March 24, 2016. Boys born on or after September 24, 2015 were classified into the "with ambulatory medical resource" group, and boys born before September 24, 2014, were classified into the "without ambulatory medical resource" group. The timely repair rates were calculated and compared. Results A total of 4,972 cryptorchidism cases were included in the final study. Approximately 33.0% of cryptorchid boys received trchid boys, and it is worthy of promotion in developing countries and regions.Background Esophageal diverticulum (ED) is an extremely rare complication of congenital esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) surgery. We aimed to investigate feasible methods for the treatment of this rare complication. Methods We retrospectively reviewed all patients with EA/TEF at Beijing Children's Hospital from January 2015 to September 2019. The clinicopathological features of patients with ED after EA/TEF surgery were recorded. Follow-up was routinely performed after surgery until December 2020. Results Among 198 patients with EA/TEF, ED only occurred in four patients (2.02%; one male, three female). The four patients had varying complications after the initial operation, including anastomotic leakage (3/4), esophageal stenosis (3/4), and recurrence of TEF (1/4). The main clinical symptoms of ED included recurrent pneumonia (4/4), coughing (4/4), and dysphagia (3/4). All ED cases occurred near the esophageal anastomosis. Patients' age at the time of diverticulum repair was 6.6-16.8 months. All patients underwent thoracoscopic esophageal diverticulectomy (operation time 1.5-3.5 h). Anastomotic leakage occurred in one patient and spontaneously healed after 2 weeks. The other three patients had no peri-operative complications. All patients were routinely followed up after surgery for 14-36 months. During the follow-up period, all patients could eat orally, had good growth and weight gain, and showed no ED recurrence or anastomotic leakage on esophagogram. Conclusions ED is a rare complication after EA/TEF surgery and is a clear indication for diverticulectomy. During the midterm follow-up, thoracoscopic esophageal diverticulectomy was safe and effective for ED after EA/TEF surgery.

  • The nuclear pore complex (NPC) serves as a central gate for mRNAs to transit from the nucleus to the cytoplasm. The ability for mRNAs to get exported is linked to various upstream nuclear processes including co-transcriptional RNP assembly and processing, and only export competent mRNPs are thought to get access to the NPC. While the nuclear pore is generally viewed as a monolithic structure that serves as a mediator of transport driven by transport receptors, more recent evidence suggests that the NPC might be more heterogenous than previously believed, both in its composition or in the selective treatment of cargo that seek access to the pore, providing functional plasticity to mRNA export. In this review, we consider the interconnected processes of nuclear mRNA metabolism that contribute and mediate export competence. Furthermore, we examine different aspects of NPC heterogeneity, including the role of the nuclear basket and its associated complexes in regulating selective and/or efficient binding to and transport through the pore.     The current study examined whether changes in core beliefs over time predict positive symptoms, negative symptoms, social functioning, and role functioning within individuals at clinical high risk for psychosis (CHR).

    CHR participants (N=73) completed the Brief Core Schemas Scale (BCSS) at baseline, with a subset of participants (n=54) completing study procedures at a 12-month follow-up. Attenuated psychotic symptoms, social, and role functioning were assessed at both timepoints.

    Hierarchical regression analyses showed that changes in self-beliefs between baseline and follow-up predicted worsening social and role functioning at 12-month follow-up, with trends suggesting they also predicted negative and positive symptoms at 12-month follow-up. The independent effect of an increase in negative self-beliefs marginally or significantly predicted all outcomes of interest.

    Individuals at clinical high-risk (CHR) for psychosis experience high rates of bullying. There is little research on the differences between CHR who did and did not experience bullying. However, there is evidence that bullying may be related to negative schemas and social impairment.

    To examine differences in core schemas, asocial and defeatist beliefs, and social functioning between those who did and did not report bullying experiences in a large sample of CHR individuals. We hypothesized that bullying in CHR youth would be associated with poorer social functioning, increased maladaptive beliefs, and negative core schemas.

    CHR participants (N=203) were split into those who did and did not report experiencing bullying. The two groups were compared on demographic characteristics, social functioning, and belief variables, using the Brief Core Schemas Scale, the Asocial Beliefs Scale, the Defeatist Performance Attitudes Scale, and the First Episode Social Functioning Scale.

    72.9% reported experiencing bullying. These participants had greater severity of negative schemas about others and asocial and defeatist performance beliefs, and lower social functioning scores.

    Prevalence of bullying among CHR participants is high. Bullying may be a risk factor for increased asocial and defeatist beliefs, negative core schemas, and poor social functioning. Targeting maladaptive schemas and beliefs during treatment may serve to improve functional outcomes in this group.
    Prevalence of bullying among CHR participants is high. Bullying may be a risk factor for increased asocial and defeatist beliefs, negative core schemas, and poor social functioning. The combination of liquid FE-SEM and in situ heating techniques revealed temperature-driven solution restructuring for ions/water systems with different water states and their critical point behavior expressed in a rapid switch between thermal expansion and shrinkage of liquid microphases at temperatures of ≈100-130 °C, which was directly recorded on electron microscopy videos. Automation of FE-SEM video analysis by a neural network approach allowed quantification of the morphological changes in ions/water systems during heating on the basis of thousands of images processed with a speed almost equal to the frame rate of original electron microscopy videos. Tracking and evolution of the micro-heterogeneous domains, hypothesized in the Ioliomics concept, was mapped and quantified for the first time.
    The nuclear pore complex (NPC) serves as a central gate for mRNAs to transit from the nucleus to the cytoplasm. The ability for mRNAs to get exported is linked to various upstream nuclear processes including co-transcriptional RNP assembly and processing, and only export competent mRNPs are thought to get access to the NPC. While the nuclear pore is generally viewed as a monolithic structure that serves as a mediator of transport driven by transport receptors, more recent evidence suggests that the NPC might be more heterogenous than previously believed, both in its composition or in the selective treatment of cargo that seek access to the pore, providing functional plasticity to mRNA export. In this review, we consider the interconnected processes of nuclear mRNA metabolism that contribute and mediate export competence. Furthermore, we examine different aspects of NPC heterogeneity, including the role of the nuclear basket and its associated complexes in regulating selective and/or efficient binding to and transport through the pore. The current study examined whether changes in core beliefs over time predict positive symptoms, negative symptoms, social functioning, and role functioning within individuals at clinical high risk for psychosis (CHR). CHR participants (N=73) completed the Brief Core Schemas Scale (BCSS) at baseline, with a subset of participants (n=54) completing study procedures at a 12-month follow-up. Attenuated psychotic symptoms, social, and role functioning were assessed at both timepoints. Hierarchical regression analyses showed that changes in self-beliefs between baseline and follow-up predicted worsening social and role functioning at 12-month follow-up, with trends suggesting they also predicted negative and positive symptoms at 12-month follow-up. The independent effect of an increase in negative self-beliefs marginally or significantly predicted all outcomes of interest. There is limited and inconsistent reporting on the association between Life's Simple 7 (LS7) and dementia in the elderly population.
