This study aimed to evaluate treatment response, survival, safety profiles, and predictive factors to chimeric antigen receptor T cell (CAR-T) therapy in Chinese patients with relapsed or refractory B cell acute lymphoblast leukemia (R/R B-ALL). 39R/R B-ALL patients who underwent CAR-T therapy were included. Baseline data were collected from patients' electronic medical records. Patients' peripheral bloods, bone marrow aspirates, and biopsies were obtained for routine examination, and treatment response and survival profiles as well as adverse events were evaluated. The rates of complete remission (CR), CR with minimal residual disease (MRD) negative/positive, and bridging to hematopoietic stem-cell transplantation (HSCT) were 92.3%, 76.9%, 15.4%, and 43.6%, respectively. The median event-free survival (EFS) was 11.6 months (95% confidence interval (CI) 4.0-19.2 months) and median overall survival (OS) was 14.0 months (95% CI 10.9-17.1 months). Bridging to HSCT independently predicted better EFS and OS, while high bone marrow blasts level independently predicted worse EFS. The incidence of cytokine release syndrome (CRS) was 97.4%, and refractory disease as well as decreased white blood cell independently predicted higher risk of severe CRS. Other common adverse events included hematologic toxicities (grade I 5.1%, grade II 7.7%, grade III 17.9%, grade IV 69.2%), neurotoxicity (28.2%), infection (38.5%), and admission for intensive care unit (10.3%). In conclusion, CAR-T therapy presents with promising treatment response, survival and safety profiles, and higher disease burden predicts worse survival as well as increased risk of severe CRS in Chinese R/R B-ALL patients.Gastric cancer (GC) has a high mortality rate, and metastasis is the main reason for treatment failure. The review presents the data concerning the role of metabolic microenvironment of tumor cells from the point of tumor-host interaction in order to employ these parameters to working out the methods of diagnosis and prognosis of disease outcome in patients with gastric cancer. Special attention has been paid to hypoxia as a key factor of metabolic microenvironment that positively affects tumor progression, stimulating its aggressiveness, metastasis and resistance to therapy and is regarded as a factor of unfavorable prognosis. Rapamycin, an mTOR kinase inhibitor, and also a potent autophagy inducer, could not only effectively reverse glucocorticoid resistance, but also promote autophagy in the ALL cells. Autophagy has been suggested to play a paradoxical role in cancer treatment. The aim of this study was to address the role of the rapamycin-induced autophagy in the leukemia treatment. MATERIALS AND METHODS Cell proliferation was detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay in ALL cell lines of CEM-C1 and CEM-C7. Western Blot analysis was performed to test protein expressions. RESULTS Inhibition of mTOR by rapamycin could reverse glucocorticoid resistance in CEM-C1 cells, and also induce autophagy in these cells by up-regulation of LC3-II and Beclin-1 expressions. This autophagy played a pro-survival role since its inhibition by 6-amino-3-methylpurine or chroloquine could enhance rapamycin-induced cell death. Rapamycin increased the expression of intracellular ferritin, and this effect could be totally blocked by 6-amino-3-methylpurine and chroloquine, suggesting that the protective role of autophagy might be mediated through up-regulation of ferritin, the major iron-binding stress protein. Ciclopirox olamine, an iron chelator, could enhance rapamycin's anti-leukemia effect by down-regulation of intracellular ferritin expression. CONCLUSIONS All these findings would suggest that rapamycin-induced autophagy plays a pro-survival role in leukemia cells and this effect might be mediated by up-regulation of intracellular ferritin expression. We hypothesize that the combination of mTOR pathway inhibitors and autophagy inhibition is rational and would induce strong anti-leukemia effects in ALL.

Forty-six patients who underwent planned day-surgery PCNL via multiple tracts for the treatment of complex renal stones were retrospectively reviewed. All procedures were performed by an experienced surgeon. The outcomes were recorded. Results The mean stone size and burden were 4.8 cm and 990.2 mm2, respectively. There were 26 (56.5%) and 20 (43.5%) patients with staghorn stones and multiple stones, respectively. Totals of two, three, and more than three tracts (with up to 7 tracts) were established in 22, 11, and 13 patients, respectively. The tract sizes ranged from 14 to 24 Fr. One to four nephrostomy tubes were placed in most patients, and a tubeless process was accomplished in only 3 (6.5%) patients. The mean surgery time was 116 min with a hemoglobin drop of 22.1 ± 16.8 g/L. Eight (17.4%) patients developed postoperative complications, with severe complications (Clavien grades III-IV) in two cases (4.4%). 39 (84.8%) patients were discharged within 24 h after surgery, and 7 (15.2%) patients were fully admitted. Only 1 (2.2%) patient required readmission. The stone clearance rate was 84.8%. The NLRP3 inflammasome involvement in ERS-induced inflammation has not been completely described. The intersection of ERS with multiple inflammatory pathways can initiate and aggravate chronic diseases. Accumulating evidence suggests that ERS-induced activation of NLRP3 inflammasome is the pathological basis of various inflammatory diseases. In this review, we have discussed the networks between ERS and NLRP3 inflammasome, with the view to identifying novel therapeutic targets in inflammatory diseases. The hazard ratios (HRs) of investigator-assessed PFS and independently assessed PFS were recorded, and a discrepancy index was obtained by calculating the ratio of their respective HRs. Moreover, we investigated possible factors of discrepancy by analyzing the trials in different groups (by year, by tumor type, by drug type, by study design). Results We analyzed 28 RCTs meeting the search criteria. The estimated mean discrepancy index was 0.98 (confidence interval 0.927-1.032 (n = 32)). Subgroup analysis showed that the confidence intervals in all cases included the value 1, except in the subgroup of studies started in the period 2003-2006.

Conclusion Considering recently reported skin reactions associated with teriflunomide, neurologists and patients should be careful on potential warning symptoms and signs of cutaneous drug reactions of this drug.Background At present, coronavirus disease 2019 (COVID-19) has spread in many countries. We conducted this study to help pediatricians understand the conditions of COVID-19 in children. Methods We retrospectively summarized the characteristics, treatment and outcomes of pediatric cases in Wuhan Children's Hospital which was the only designated hospital for children with COVID-19 in Hubei Province. A Cox proportional hazards regression analysis was used to evaluate factors associated with clinical outcomes. Results As of February 29, 75 children had been discharged, of which only one was has severe pneumonia and one was critical cases. Children younger than 2 years were more susceptible to COVID-19. All patients have received interferon-α nebulization, and eight cases including the severe and critical cases were co-administrated ribavirin. Five patients with mild pneumonia were given arbidol. Twenty-three patients were given traditional Chinese medicine (TCM). The average length of stay (LOS) and the time of SARS-CoV-2 clearance were 10.57 and 6.39 days, respectively. None of the factors was associated with LOS or time of SARS-CoV-2 clearance. Conclusions The severity of COVID-19 in pediatric cases were milder than adults. Material and methods The VALGENT4 panel comprises 998 consecutive SurePath cervical samples from routine screening and 297 SurePath samples enriched for disease (100 ASC-US, 100 LSIL, 97 HSIL). The cobas HPV test is a real-time PCR assay which detects HPV16 and 18 individually and 12 other high-risk (hr) HPV genotypes in one bulk. Results The clinical performance of the cobas test was assessed relative to that of the comparator assay GP5+/6 + PCR Enzyme ImmunoAssay (GP-EIA) by a non-inferiority test. The relative sensitivity for ≥ CIN2 was 1.00 (95% CI 0.97-1.04) and relative specificity for the control group was 1.02 (95% CI 1.01-1.04). The cobas test was found non-inferior to that of GP-EIA for both sensitivity and specificity (p-0.0006 and p less then 0.0001, respectively). The type specific performance of the cobas test was evaluated using the GP5+/6 + PCR with Luminex genotyping (GP-LMNX) as comparator. The cobas test showed excellent to good concordance (Kappa 0.70 to 0.90) with GP-LMNX for all three genotype groups in the overall VALGENT population but good to moderate concordance in the Screening population (kappa from 0.56 to 0.80). We conducted this study to help pediatricians understand the conditions of COVID-19 in children. Methods We retrospectively summarized the characteristics, treatment and outcomes of pediatric cases in Wuhan Children's Hospital which was the only designated hospital for children with COVID-19 in Hubei Province. A Cox proportional hazards regression analysis was used to evaluate factors associated with clinical outcomes. Results As of February 29, 75 children had been discharged, of which only one was has severe pneumonia and one was critical cases. Children younger than 2 years were more susceptible to COVID-19. All patients have received interferon-α nebulization, and eight cases including the severe and critical cases were co-administrated ribavirin. Five patients with mild pneumonia were given arbidol. Twenty-three patients were given traditional Chinese medicine (TCM). The average length of stay (LOS) and the time of SARS-CoV-2 clearance were 10.57 and 6.39 days, respectively. None of the factors was associated with LOS or time of SARS-CoV-2 clearance. Conclusions The severity of COVID-19 in pediatric cases were milder than adults. In other pathogenic infections, in vivo imaging systems (IVIS) have proven extremely useful in studying pathogenesis, tissue tropism and in vivo drug efficacy. IVIS requires the use of transgenic parasites expressing a florescent reporter. Developing a method to produce transgenic filarial parasites expressing a florescent reporter would permit IVIS to be applied to the study of tissue tropism and provide a non-invasive way to screen for in vivo drug efficacy against these parasites. METHODOLOGY/PRINCIPAL FINDINGS We report the development of a dual luciferase reporter construct in a piggyBac backbone that may be used to stably transfect Brugia malayi, a causative agent of human filariasis. Parasites transfected with this construct were visible in IVIS images obtained from infected gerbils. The signal in these infected animals increased dramatically when the transgenic parasites matured to the adult stage and began to produce transgenic progeny microfilaria. We demonstrate that the IVIS system can be used to develop an effective method for cryopreservation of transgenic parasites, to non-invasively monitor the effect of treatment with anti-filarial drugs, and to rapidly identify transgenic F1 microfilariae. CONCLUSIONS To our knowledge, this represents the first application of IVIS to the study of a human filarial parasite. This method should prove useful in studies of tissue tropism and as an efficient in vivo assay for candidate anti-filarial drugs.BACKGROUND Recent initiatives in psychiatry emphasize the utility of characterizing psychiatric symptoms in a multidimensional manner. We utilized nine self-report symptom measures drawn from a large cellular biobanking study that enrolled individuals with mood and psychotic disorders, as well as healthy controls. To summarize questionnaire results we used word embeddings, a technique to represent words as numeric vectors preserving semantic and syntactic meaning. A low-dimensional approximation to the embedding space was used to derive the proposed succinct summary of symptom profiles. The family includes Sm and Sm-like proteins, which play important roles in pre-mRNA splicing. Most research on the Sm proteins have been conducted in herbaceous plants, and less in woody plants such as Dimocarpus longan (longan). And the embryo development status significantly affects the quality and yield of longan. In this study, we conducted a genome-wide analysis of longan Sm genes (DlSm) to clarify their roles during somatic embryogenesis (SE) and identified 29 Sm genes. Phylogenetic analysis deduced longan Sm proteins clustered into 17 phylogenetic groups with the homologous Sm proteins of Arabidopsis thaliana. We also analyzed the gene structures, motif compositions, and conserved domains of the longan Sm proteins. The promoter sequences of the DlSm genes contained many light, endosperm development, hormone, and temperature response elements, which suggested their possible functions. In the non-embryogenic callus(NEC) and during early SE in longan, the alternative splicing(AS) events of DlSm genes indicated that these genes may influence SE development by changing gene structures and sequences. The kinetin(KT) hormone, and blue and white light treatments affected the differentiation and growth of longan embryonic callus(EC) probably by affecting the AS events of DlSm genes. Expression profiles showed the possible functional divergence among Sm genes, and different hormones and light qualities affected their expression levels. The expression trends of the DlSm genes determined by RNA sequencing as fragments per kilobase of exon model per million mapped reads (FPKM) and by real-time quantitative PCR(qRT-PCR) during early SE in longan showed that the expression of the DlSm genes was affected by the growth and differentiation of longan SE, and decreased as the somatic embryo differentiation progressed. The results will contributed to understanding the longan Sm gene family and provide a basis for future functional validation studies.

BACKGROUND AND OBJECTIVES The aim of this study was to develop measures to assess implicit memory associations for video gaming and to examine the relationship between implicit memory associations, video gaming involvement, and problem video gaming. METHODS A survey of online panelists from across Canada was conducted that included 166 problem video gamers who were recruited for participation in a larger study examining the relationship between gambling, video gaming, and collectible card play. Memory associations were assessed using a 10-item measures of word associations and behaviour associations. Video gaming involvement was assessed via self-report of time and frequency of play, and problem video gaming was assessed using the Behavioral Addiction Measure for Video Gaming (BAM-VG). https://www.selleckchem.com/products/d-ap5.html RESULTS Significant Kendall tau-b correlations ranging from 0.297 to 0.405 were found between measures of memory associations and both level of video gaming involvement and problem video gaming. CONCLUSIONS There is a robust relationship between memory associations for video gaming with level of video gaming involvement and problem video gaming. The results suggest that implicit associations may have utility in the assessment of problem video gaming. BACKGROUND There is growing concern regarding problematic smartphone overuse. Practiced mindfulness, the state of being aware of the present moment, may protect against problematic smartphone use by reducing the strength of risk factors. PURPOSE We hypothesized that trait mindfulness can reduce the impact of risk factors on a) objective smartphone use and b) subjective problematic smartphone use. METHODS Our sample (n = 135, Mage = 19.15, 68% female) consisted of college students from a large university. Participants completed self-report measures of boredom proneness, impulsivity, technology-related anxiety (nomophobia), trait mindfulness, smartphone use frequency, and problematic use. RESULTS Higher mindfulness was significantly associated with lower boredom proneness, impulsivity, and problematic use (F = 12.12, p less then .01). Hierarchical regression revealed that the positive relationships between nomophobia, and problematic use decreased as mindfulness levels increased. A similar protective effect was observed for boredom proneness, although the effect dissipated as impulsivity rose. A second regression revealed no significant predictors of weekly smartphone use. CONCLUSIONS Effects of nomophobia and boredom proneness on problematic smartphone use diminish with increased mindfulness, but impulsivity may interfere with this. Risk and protective factors for 'addiction' appear unrelated to smartphone use frequency. Future research should examine benefits of mindfulness-based interventions in promoting emotional and cognitive self-regulation, focusing on those who use smartphones in dysfunctional ways. INTRODUCTION Burns are one of the most common injuries sustained globally. Low- and middle-income countries (LMICs) are disproportionately affected by burn injury morbidity and mortality; African children have the highest burn mortality globally. In high-income countries, early surgical intervention has shown to improve survival. However, when applied to burn victims in LMICs, improved survival in the early excision cohort (≤5 d) was not seen. Therefore, we aimed to determine the magnitude of the effect of surgical intervention on burn injury survival. METHODS A retrospective analysis of a prospectively collected data, utilizing the Kamuzu Central Hospital Burn Database from May 2011 to July 2019, was performed. Pediatric patients (≤12 y) were included. Patients were excluded if they underwent surgical intervention for nonacute burn care management. Bivariate analyses stratifying by type of surgical intervention was performed, comparing demographics, burn characteristics, surgical intervention, and patient moodds ratio 5.24, 95% confidence interval 2.40-11.44, P = 0.003) when compared with patients not undergoing surgery. CONCLUSIONS In this propensity-weighted analysis, surgical intervention following burn injury increases the odds of survival by a factor of 5.24 when compared with patients not undergoing surgical intervention. Efforts to enhance burn infrastructure to deliver surgical care is imperative to attenuate burn mortality in resource-poor settings. BACKGROUND The American College of Surgeons Commission on Cancer has incorporated documentation of critical elements outlined in Operative Standards for Cancer Surgery into revised standards for cancer center accreditation. This study assessed the current documentation of critical elements in partial mastectomy (PM) and sentinel lymph node biopsy (SLNB) operative reports. MATERIALS AND METHODS Operative reports for PM + SLNB at a single academic institution from 2013 to 2018 were reviewed for compliance and surveyor interobserver reliability with the Oncologic Elements of Operative Record defined in Operative Standards and compared with a nonredundant American Society of Breast Surgeons Mastery of Breast Surgery (MBS) quality measure for specimen orientation. RESULTS Ten reviewers each evaluated 66 PM + SLNB operative reports for 13 Oncologic Elements and one MBS measure. No operative records reported all critical elements for PM + SLNB or PM alone. Residents completed 36.4% of operative reports Element documentation was similar for PM but varied significantly for SLNB between resident and attending authorship. Combined reporting performance and interrater reliability varied across all elements and was highest for the use of SLNB tracer (97.1% and κ = 0.95, respectively) and lowest for intraoperative assessment of SLNB (30.6%, κ = 0.43). MBS specimen orientation had both high proportion reported (87.0%) and interrater reliability (κ = 0.84). CONCLUSIONS Adherence to reporting critical elements for PM and SLNB varied. Whether differential compliance was tied to discrepancies in documentation or reviewer abstraction, clarification of synoptic choices may improve reporting consistency. Evolving techniques or technologies will require continuous appraisal of mandated reporting for breast surgery.

The results suggest that implicit associations may have utility in the assessment of problem video gaming. BACKGROUND There is growing concern regarding problematic smartphone overuse. Practiced mindfulness, the state of being aware of the present moment, may protect against problematic smartphone use by reducing the strength of risk factors. PURPOSE We hypothesized that trait mindfulness can reduce the impact of risk factors on a) objective smartphone use and b) subjective problematic smartphone use. METHODS Our sample (n = 135, Mage = 19.15, 68% female) consisted of college students from a large university. Participants completed self-report measures of boredom proneness, impulsivity, technology-related anxiety (nomophobia), trait mindfulness, smartphone use frequency, and problematic use. RESULTS Higher mindfulness was significantly associated with lower boredom proneness, impulsivity, and problematic use (F = 12.12, p less then .01). Hierarchical regression revealed that the positive relationships between nomophobia, and problematic use decreased as mindfulness levels increased. A similar protective effect was observed for boredom proneness, although the effect dissipated as impulsivity rose. A second regression revealed no significant predictors of weekly smartphone use. CONCLUSIONS Effects of nomophobia and boredom proneness on problematic smartphone use diminish with increased mindfulness, but impulsivity may interfere with this. Risk and protective factors for 'addiction' appear unrelated to smartphone use frequency. Future research should examine benefits of mindfulness-based interventions in promoting emotional and cognitive self-regulation, focusing on those who use smartphones in dysfunctional ways. INTRODUCTION Burns are one of the most common injuries sustained globally. Low- and middle-income countries (LMICs) are disproportionately affected by burn injury morbidity and mortality; African children have the highest burn mortality globally. In high-income countries, early surgical intervention has shown to improve survival. However, when applied to burn victims in LMICs, improved survival in the early excision cohort (≤5 d) was not seen. Therefore, we aimed to determine the magnitude of the effect of surgical intervention on burn injury survival. METHODS A retrospective analysis of a prospectively collected data, utilizing the Kamuzu Central Hospital Burn Database from May 2011 to July 2019, was performed. Pediatric patients (≤12 y) were included. Patients were excluded if they underwent surgical intervention for nonacute burn care management. Bivariate analyses stratifying by type of surgical intervention was performed, comparing demographics, burn characteristics, surgical intervention, and patient moodds ratio 5.24, 95% confidence interval 2.40-11.44, P = 0.003) when compared with patients not undergoing surgery. CONCLUSIONS In this propensity-weighted analysis, surgical intervention following burn injury increases the odds of survival by a factor of 5.24 when compared with patients not undergoing surgical intervention. Efforts to enhance burn infrastructure to deliver surgical care is imperative to attenuate burn mortality in resource-poor settings. BACKGROUND The American College of Surgeons Commission on Cancer has incorporated documentation of critical elements outlined in Operative Standards for Cancer Surgery into revised standards for cancer center accreditation. This study assessed the current documentation of critical elements in partial mastectomy (PM) and sentinel lymph node biopsy (SLNB) operative reports. MATERIALS AND METHODS Operative reports for PM + SLNB at a single academic institution from 2013 to 2018 were reviewed for compliance and surveyor interobserver reliability with the Oncologic Elements of Operative Record defined in Operative Standards and compared with a nonredundant American Society of Breast Surgeons Mastery of Breast Surgery (MBS) quality measure for specimen orientation. RESULTS Ten reviewers each evaluated 66 PM + SLNB operative reports for 13 Oncologic Elements and one MBS measure. No operative records reported all critical elements for PM + SLNB or PM alone. Residents completed 36.4% of operative reports Element documentation was similar for PM but varied significantly for SLNB between resident and attending authorship. Combined reporting performance and interrater reliability varied across all elements and was highest for the use of SLNB tracer (97.1% and κ = 0.95, respectively) and lowest for intraoperative assessment of SLNB (30.6%, κ = 0.43). MBS specimen orientation had both high proportion reported (87.0%) and interrater reliability (κ = 0.84). CONCLUSIONS Adherence to reporting critical elements for PM and SLNB varied. Whether differential compliance was tied to discrepancies in documentation or reviewer abstraction, clarification of synoptic choices may improve reporting consistency. Sleep paralysis (SP) is a psychobiological phenomenon caused by temporary desynchrony in the architecture of rapid eye movement (REM) sleep. It affects approximately 7.6% of the general population during their lifetime. The aim of this study was to assess (1) the prevalence of SP among Polish students in Lublin (n = 439) using self-reported online surveys, (2) the frequency of SP-related somatic and psychopathologic symptoms, and (3) the factors potentially affecting the occurrence of symptoms among people experiencing SP. We found that the incidence of SP in the Polish student population was slightly higher (32%) than the average prevalence found in other student populations (28.3%). The SP clinical picture was dominated by somatic symptomatology 94% of respondents reported somatic symptoms (most commonly tachycardia, 76%), 93% reported fear (most commonly fear of death, 46%), and 66% reported hallucinations (most commonly visual hallucinations, 37%). The number of SP episodes was related to sleep duration and supine position during sleep. The severity of somatic symptoms correlated with lifestyle variables and anxiety symptomatology. Our study shows that a significant proportion of students experience recurrent SP and that this phenomenon is associated with fear and physical discomfort. The scale of the phenomenon requires a deeper analysis.The ongoing studies of the influence of internal defects on fatigue strength of additively manufactured metals adopted an internal crack or notch-like model at which the threshold stress intensity factor is the driving mechanism of fatigue failure. The current article highlights a shortcoming of this approach and offers an alternative based on X-ray microcomputed tomography and cyclic plasticity with a hybrid formulation of Chaboche and Armstrong-Frederick material laws. The presented tessellation and geometrical transformation scheme enabled a significantly more realistic morphological representation of internal defects that yielded a cyclic strain within 2% of the experimental values. This means that cyclic plasticity models have an accurate prediction of mechanical properties without repeating a full set of experiments for additively manufactured arbitrary microstructures. The coupling with a material law that is oriented towards the treatment of cyclic hardening and softening enabled more accurate computation of internal stresses under cyclic loading than ever before owing to the maturity of tessellation and numerical tools since then. The resulting stress-strain distributions were used as input to the Fatemi-Socie damage model, based on which a successful calculation of fatigue lifetime became possible. Furthermore, acting stresses on the internal pores were shown to be more than 450% concerning the applied remote stress amplitude. The results are a pretext to a scale bridging numerical solution that accounts for the short crack formation stage based on microstructural damage.The DMD gene is one of the largest human genes, being composed of 79 exons, and encodes dystrophin Dp427m which is deficient in Duchenne muscular dystrophy (DMD). In some DMD patient, however, small size dystrophin reacting with antibody to N-terminal but not to C-terminal has been identified. The mechanism to produce N-terminal small size dystrophin remains unknown. Intronic polyadenylation is a mechanism that produces a transcript with a new 3' terminal exon and a C-terminal truncated protein. In this study, intronic alternative polyadenylation was disclosed to occur in the middle of the DMD gene and produce the half-size N-terminal dystrophin Dp427m, Dpm234. The 3'-rapid amplification of cDNA ends revealed 421 bp sequence in the downstream of DMD exon 41 in U-251 glioblastoma cells. The cloned sequence composing of the 5' end sequence of intron 41 was decided as the terminal exon, since it encoded poly (A) signal followed by poly (A) stretch. Subsequently, a fragment from DMD exon M1 to intron 41 was obtained by PCR amplification. This product was named Dpm234 after its molecular weight. However, Dpm234 was not PCR amplified in human skeletal and cardiac muscles. Remarkably, Dpm234 was PCR amplified in iPS-derived cardiomyocytes. Accordingly, Western blotting of cardiomyocyte proteins showed a band of 234 kDa reacting with dystrophin antibody to N-terminal, but not C-terminal. Echinococcus granulosus is the causative agent of cystic echinococcosis, which has serious impacts on human and/or animal health, resulting in significant economic losses. Echinococcus granulosus comprises a number of intra-specific variants or strains at the genetic level. In Saudi Arabia, few studies were performed on genetic variations in Echinococcus species. Therefore, the present study aimed to investigate the phenotypic and genetic characterization of hydatid cysts harboured by sheep and camels in Al-Madinah Al-Munawarah. Species identification was based both on morphology and 28S ribosomal RNA gene sequences. We found Fellodistomum agnotum in the White Sea for the first time. Buccinum undatum was proved to be intermediate host of both F. agnotum and Fellodistomum fellis, and metacercariae of F. fellis were registered from two more buccinid species Buccinum scalariforme and Neptunea despecta. We also found metacercariae of F. agnotum and F. fellis producing eggs in the second intermediate host. Two fellodistomids were found in protobranch bivalves sporocysts and cercariae of Steringophorus furciger in Nuculana pernula, and sporocysts with large furcocercous cercariae in Ennucula tenuis. The latter were identified as F. agnotum by molecular analysis; thus, the entire life cycle of this species was reconstructed.Long-term stability is one of the main challenges for translation of therapeutic proteins into commercially viable biopharmaceutical products. During processing and storage, proteins are susceptible to denaturation. The aim of this work was to evaluate the stability of amphiphilic cyclodextrin-based nanoparticles (NPs) containing insulin glulisine. The stability of the NP dispersion was systematically evaluated following storage at three different temperatures (4 °C, room temperature (RT) and 40 °C). While the colloidal parameters of the NPs in terms of size and zeta potential were maintained (109 ± 9 nm, polydispersity index 0.272, negative zeta potential -25 ± 3 mV), insulin degraded over 60 days during storage. To enhance the shelf life of the product and to circumvent the need for cold-chain maintenance, a lyophilized formulation containing insulin glulisine NPs (1.75 mg/mL of NPs) and 25 mg/mL trehalose was produced. The freeze-dried powder extended the stability of the product for up to 30 days at ambient temperature and 90 days at 4 °C (with 95% and >80% insulin recovery, respectively). Addition of hydrogen alongside diesel fuel has been found to improve engine performance and efficiency; however, after a certain limit, hydrogen begins to show adverse effects, mainly because the ratio of oxygen to fuel decreases. This can be overcome by using oxyhydrogen, which is a mixture of hydrogen and oxygen gas. In this study, effects of addition of oxyhydrogen generated by electrolysis, with varying flows at the intake manifold, on a 315 cc compression ignition engine alongside diesel were analyzed. The engine was mounted on a Thepra test bed and torque measurements were taken at predetermined test points for diesel and 6 and 10 standard cubic feet per hour flowrates of oxyhydrogen. H10 showed the maximum improvement in engine performance equating to a 22.4% increase in both torque and power at 3000 r/min, and a 19.4% increase in efficiency at 2600 r/min was recorded. The large increase in engine performance as compared to previous results is because of high oxyhydrogen flowrate to displacement volume ratio. The oxyhydrogen flowrate to displacement ratio is the most important factor as it is directly impacts engine performance. The difference in engine performance because of oxyhydrogen becomes prominent at higher engine speed due to high suction pressure. No experimental flowrates of oxyhydrogen showed any adverse effect on the engine performance.Background In low- and middle-income countries (LMICs), infection is the most frequent complication in surgical procedures, and surgical site infections (SSIs) globally are the most frequent health-care-associated infections (HAIs). Preventing SSI is an important target for overall quality improvement and patient safety as well as supporting the infection prevention and control (IPC) global agenda. Methods In 2018, the World Heath Organization (WHO) presented the first Global Guidelines for the Prevention of Surgical Site Infections. The WHO also simplified SSI surveillance materials and included process measures, critical to addressing the barriers existing in LMICs. Because surveillance activities alone will not lead to improvements and implementation is more challenging than guideline development, the WHO then outlined a novel, step by step approach for implementation based on its tried and tested improvement approach for IPC measures. These documents have been reviewed and summarized to achieve wider reach in the surgical community.

During processing and storage, proteins are susceptible to denaturation. The aim of this work was to evaluate the stability of amphiphilic cyclodextrin-based nanoparticles (NPs) containing insulin glulisine. The stability of the NP dispersion was systematically evaluated following storage at three different temperatures (4 °C, room temperature (RT) and 40 °C). While the colloidal parameters of the NPs in terms of size and zeta potential were maintained (109 ± 9 nm, polydispersity index 0.272, negative zeta potential -25 ± 3 mV), insulin degraded over 60 days during storage. To enhance the shelf life of the product and to circumvent the need for cold-chain maintenance, a lyophilized formulation containing insulin glulisine NPs (1.75 mg/mL of NPs) and 25 mg/mL trehalose was produced. The freeze-dried powder extended the stability of the product for up to 30 days at ambient temperature and 90 days at 4 °C (with 95% and >80% insulin recovery, respectively). Following intra-intestinal administration of the freeze-dried formulation, while no lowering of blood glucose was seen, insulin glulisine was detected in both portal and systemic blood indicating that potential exists for further development of the formulation to simultaneously achieve prolonged stability and therapeutic efficacy.Compression ignition engines are one of the world's largest consumers of fossil oil but have energy extraction efficiency limited to 35%. Addition of hydrogen alongside diesel fuel has been found to improve engine performance and efficiency; however, after a certain limit, hydrogen begins to show adverse effects, mainly because the ratio of oxygen to fuel decreases. This can be overcome by using oxyhydrogen, which is a mixture of hydrogen and oxygen gas. In this study, effects of addition of oxyhydrogen generated by electrolysis, with varying flows at the intake manifold, on a 315 cc compression ignition engine alongside diesel were analyzed. The engine was mounted on a Thepra test bed and torque measurements were taken at predetermined test points for diesel and 6 and 10 standard cubic feet per hour flowrates of oxyhydrogen. H10 showed the maximum improvement in engine performance equating to a 22.4% increase in both torque and power at 3000 r/min, and a 19.4% increase in efficiency at 2600 r/min was recorded. The large increase in engine performance as compared to previous results is because of high oxyhydrogen flowrate to displacement volume ratio. The oxyhydrogen flowrate to displacement ratio is the most important factor as it is directly impacts engine performance. The difference in engine performance because of oxyhydrogen becomes prominent at higher engine speed due to high suction pressure. No experimental flowrates of oxyhydrogen showed any adverse effect on the engine performance.Background In low- and middle-income countries (LMICs), infection is the most frequent complication in surgical procedures, and surgical site infections (SSIs) globally are the most frequent health-care-associated infections (HAIs). Preventing SSI is an important target for overall quality improvement and patient safety as well as supporting the infection prevention and control (IPC) global agenda. Methods In 2018, the World Heath Organization (WHO) presented the first Global Guidelines for the Prevention of Surgical Site Infections. The WHO also simplified SSI surveillance materials and included process measures, critical to addressing the barriers existing in LMICs. Because surveillance activities alone will not lead to improvements and implementation is more challenging than guideline development, the WHO then outlined a novel, step by step approach for implementation based on its tried and tested improvement approach for IPC measures. METHODS All patients undergoing elective standard EVAR between January 2008 and June 2017, and with at least one measurement of contemporary (cTnI) or high sensitivity troponin I (hSTnI) in the first 48h after surgery, were retrospectively analyzed. Post-implantation syndrome was defined as the presence of fever and leukocytosis in the postoperative period in the absence of infectious complications. MINS was defined as thS was found to be significantly associated with post-operative **** (p=0.001), but not MINS. Survival analysis revealed no differences between patients with or without PIS regarding 30-day mortality as well as long-term all-cause mortality. ASA score (HR 2.157; 95% CI 1.07 - 4.33, P=0.031) and heart failure (HR 2.284; 95% CI 1.25 - 4.18, P=0.008) were found to be independently associated with increased long-term all-cause mortality in this cohort of patients. CONCLUSION PIS is a common complication after EVAR, occurring in 11.4% of the patients from this cohort. Of these, 58 patients received adjunctive DCB after the atherectomy (group A) and 57 patients were managed with uncoated balloon after atherectomy (group B). Immediate and late clinical outcomes were compared. RESULTS Baseline clinical and lesion data were comparable between the two groups. However, group A included more uses of rotational atherectomy (43.9% vs. 1.7%, p less then 0.001) or embolization protection filter (53.0% vs. 6.7%, p=0.001), and fewer cases requiring provisional stenting (4.5% vs. 18.3%, p=0.014). Clinical primary patency at 1 year was significantly higher in group A than in group B (76.3% vs. 61.1%, p=0.039). There was a trend towards higher 1-year target lesion revascularization (TLR)-free survival in group A (89.8% vs. 77.9% at 1 year, p=0.275) without statistical significance. Proportional hazards regression analysis indicated that age (HR 0.94, 95% CI 0.90-0.99, p=0.016) and provisional stenting (HR 9.78, 95% CI 2.20-43.46, p=0.003) were independent factors associated with restenosis after combined treatment with atherectomy and DCB. CONCLUSIONS In femoropopliteal artery disease, the combination of atherectomy with adjunctive DCB achieved better clinical outcomes in terms of clinical primary patency compared to atherectomy plus uncoated balloon while TLR-free survival may also be improved. In 3535 patients with a simple arch, 1917 underwent TFCAS and 1619 had TCAR. Mean age was 70.6 (±9.5) years and 2382 (67.4%) patients w. Although the occurrence of in-hospital TIA was no different between the two approaches (P =0.6158), there were significantly fewer strokes in those treated with TCAR (P=0.0132). TCAR (P=0.0146) was associated with improved outcomes. A worse outcome was seen with advancing age (P=0.0003), prior strokes (P=0.01), and a left sided lesion (P=0.0176). CONCLUSION TCAR has improved outcomes of TIA/stroke/death compared to TFCAS in both simple and complex aortic arch anatomy. RESULTS Ten patients (15 limbs) with recanalization of great or small saphenous vein were treated with SA. The median age was 67 years and 60% were men. The median time from the first ablation was 7.3 years (IQR 6.5 - 9.6). For SA, radiofrequency (RFA) was performed in 12 limbs (80%) and Ultrasound-guided foam sclerotherapy (UGFS) in 3 cases (20%). The Great Saphenous Vein was the treated vessel in 14 cases (93.3%). No complications were reported. Median follow-up was 13 months. Preoperative and 1 year follow-up VCSS was 9 (IQR 5-10) and 4 (IQR 2-8), respectively. Doppler Ultrasound evaluation after 1 year showed an occlusion rate of 93.

Efficient and cost-effective method to destroy complex dyes is warranted to combat increasing water pollution. In the present study, homogeneous photocatalytic oxidation (PCO) of Reactive Black 5 (RB5) dye was studied using ferric ions (Fe(III)) under visible light (VL) irradiation and sunlight (SL). In the presence of 5 mM ferric ions and at pH 2.6, more than 80% of initial 20 mg/L RB5 was decolourized in 60 min under artificial VL. Decolourization followed pseudo first-order kinetics with the reaction rate constant 0.0356 min-1. 79% of initial COD was removed at the end of 60 min, suggesting mineralization of RB5 as the main cause of decolourization. Using similar experimental conditions under SL, more than 90% RB5 was decolourized in 15 min with an almost 10-fold increase in the reaction rate constant (0.34 min-1). Rate and extent of RB5 destruction significantly decreased in the presence of •OH scavenger indicating photoreduction of Fe-hydroxo species and generation of •OH as the main mechanism of RB5 degradation. RB5 removal increased from ca. 30% to 84% with the increase in Fe(III) concentration from 0.5 to 5 mM. The corresponding 1st-order rate constants increased linearly from 0.006 to 0.036 min-1. RB5 degradation decreased linearly (R2 = 0.98) from 91.7% to 63.3% with the increase in initial RB5 concentration from 10 to 40 mg/L. Fe(III) induced homogenous PCO appears to be a reliable and low-cost method of advanced oxidation without the need for costly reagent such as H2O2.The COVID-19 economic crisis makes it vitally important that workers who earned defined benefit pensions receive them at retirement. We retrospectively analyzed all consecutive NOTES cholecystectomies performed at our center between June 2009 and January 2018. All procedures were performed using a hybrid transvaginal technique, including an umbilical small-size trocar. End points, calculated at discharge, 30 and up to 90 days postoperatively, included intra- and postoperative morbidity assessed by the validated Clavien-Dindo classification and the Comprehensive Complication Index (CCI). Special focus was held on outcome and necessity of pre- and postoperative gynecological examinations. Results. We performed 571 hybrid NOTES cholecystectomies within 9 years. The vast majority were elective, 9.6% were emergency cholecystectomies. 6.7% of patients developed at least one complication until discharge, most of them minor (≤grade II). 30- and 90-day complication rates were 10.7% and 11%, respectively. Mean CCI at discharge and postoperative days 30 and 90 was 1.45 (±6.4), 2.3 (±7.7), and 2.4 (±7.8), respectively. Major complications (≥grade IIIa) occurred in 1.6% of patients, and 4 patients required emergency reoperation. No mortality was observed. In 9.8%, an additional abdominal trocar was placed. All patients underwent routine gynecological examination, whereof only 5 were rejected for transvaginal access preoperatively. In no case transvaginal access was discontinued intraoperatively due to gynecological disease. Conclusion. Hybrid NOTES transvaginal cholecystectomy represents a safe and feasible alternative to standard laparoscopic cholecystectomy. Preoperative gynecological examination is no longer routinely necessary, as intraoperative assessment is adequate.Chitosan adhesive hydrogels have attracted interest because of their unique properties. However, it is still a challenge to use simple and unmodified chitosan to synthesize multifunctional hydrogels with adhesive, self-healing and strain sensing properties. Here, a multifunctional hydrogel of Ca2+-chitosan/polyacrylamide(Ca2+- CS/PAAm)with adhesive self-healing and strain sensing properties was carried out by one-step preparation method. The hydrogel has the ability to adhere to a variety of materials, wherein the adhesion strength to rubber surface is 48.4 kPa. The hydrogel has excellent strain sensing capability on finger bending or cyclic stretching, the sensitivity factor is 0.41 ± 0.015. The cut hydrogel healed completely at room temperature within 4 h, and the healing hydrogel can be stretched to 5 times of its original length. This makes Ca2+-CS/PAAm hydrogel to be great potential in the application of strain sensors, wound dressing and skin adhesives.Background High frequency chest wall oscillation (HFCWO) has long been used for airway clearance for patients with cystic fibrosis. Only limited research has evaluated this therapy in adult patients with non-cystic fibrosis bronchiectasis (NCFB). Methods Data from 2596 patients from a registry of adult bronchiectasis patients using HFCWO therapy was used to evaluate hospitalization patterns before and after initiation of HFCWO therapy, as well as antibiotic use and self-reported metrics of quality of life. Self-reported outcomes were also reviewed by cross-checking with sampled patient charts and found to be consistent. Results The number of patients who had at least one respiratory-related hospitalization decreased from 49.1% (192/391) in the year before to 24.0% (94/391) in the year after starting HFCWO therapy (p-value less then 0.001). At the same time, the number of patients who required three or more hospitalizations dropped from 14.3% (56/391) to 5.6% (22/391). Patients currently taking oral antibiotics for respiratory conditions decreased from 57.

The hydrogel has excellent strain sensing capability on finger bending or cyclic stretching, the sensitivity factor is 0.41 ± 0.015. The cut hydrogel healed completely at room temperature within 4 h, and the healing hydrogel can be stretched to 5 times of its original length. This makes Ca2+-CS/PAAm hydrogel to be great potential in the application of strain sensors, wound dressing and skin adhesives.Background High frequency chest wall oscillation (HFCWO) has long been used for airway clearance for patients with cystic fibrosis. Only limited research has evaluated this therapy in adult patients with non-cystic fibrosis bronchiectasis (NCFB). Methods Data from 2596 patients from a registry of adult bronchiectasis patients using HFCWO therapy was used to evaluate hospitalization patterns before and after initiation of HFCWO therapy, as well as antibiotic use and self-reported metrics of quality of life. Self-reported outcomes were also reviewed by cross-checking with sampled patient charts and found to be consistent. Results The number of patients who had at least one respiratory-related hospitalization decreased from 49.1% (192/391) in the year before to 24.0% (94/391) in the year after starting HFCWO therapy (p-value less then 0.001). At the same time, the number of patients who required three or more hospitalizations dropped from 14.3% (56/391) to 5.6% (22/391). Serum NT level was not significantly associated with glaucoma severity in either PACG group or POAG group based on mean deviation (MD) (mild, n=30; moderate, n=24; sever, n=19; blinding, n=27, p>0.05, in Table 3 for PACG; mild, n=9; moderate, n=20; sever, n=21; blinding, n=40; p>0.05, in Table 4 for POAG). We examined quantifiable information about biodiversity indicators from the most recent two national reports (i.e., 4th in 2010 and 5th in 2015) to the United Nation's Convention on Biological Diversity (CBD) by the 10-member countries of the Association of Southeast Asian Nations (ASEAN) Brunei Darussalam, Cambodia, Indonesia, Laos, Malaysia, Myanmar, Philippines, Singapore, Thailand and Vietnam. This article presents the number of indicators, their level of development, and detailed lists of indicators for each country, and demonstrates general improvement in indicator use by the highest level of government reporting about implementation of the CBD at the national scale.The dataset presented in this article is the supplementary data for the research article titled "The pyrolysis and combustion of polyoxymethylene in a fluidised bed with the possibility of incorporating CO2" [1], in which possible paths of polyoxymethylene conversion in the fluidised bed made from cenospheres and by means of various fluidising gases (air, N2, CO2) were tested. The use of CO2 as fluidising gas was particularly interesting because above 600°C its incorporation into process products (i.e. CO-rich flue gas) was observed. The gaseous products were detected using Fourier Transform Infrared Spectroscopy (FTIR, Gasmet DX-4000) at intervals of a few seconds. The data on the concentration changes over time will allow to evaluate and verificate of new kinetic models of polyoxymethylene degradation with the possibility of incorporating CO2.Here, we present a proteomics dataset of liver proteins to understand aging in rabbits, which complements the publication "Quantitative proteomics to study aging in rabbit liver" [1]. This dataset was generated to understand the molecular basis and metabolic changes of aging processes in liver, which is the main organ involved in metabolism, detoxification, transport, and signaling. Proteins from young, middle, and old age rabbits were extracted and digested. Generated peptides were labeled with light or heavy dimethyl groups at their N-termini, while lysine amines were labeled with TMT10-plex using a cPILOT workflow [2]. Labeled peptides were fractionated by basic pH reverse phase chromatography and analyzed with online reverse phase LC coupled with tandem mass spectrometry (MS/MS and MS3). The RAW files were generated using a Fusion Lumos Orbitrap mass spectrometer (Thermo Scientific) and processed with Proteome Discoverer (PD) version 2.2 to generate a list of identified and quantified proteins. Data was searched against the Rabbit UniProtKB redundant database. A total of 3,867 proteins were identified corresponding to 2,586 protein groups and 22,229 peptides. Dynamic levels of age-related proteins associated with fat metabolism, mitochondrial dysfunction, and protein degradation were detected. The mass spectrometry proteomics data (RAW files) and processed Proteome Discoverer 2.2 files (MSF files) have been deposited to the Proteomics Identification Database (PRIDE) ProteomeXchange Consortium and can be accessed with the dataset identifier PDX013220 (http//www.ebi.ac.uk/pride/archive/projects/PXD013220).This article presents performance data concerning a 1MW crystalline photovoltaic (PV) plant installed in the semi-arid climate of India. Data includes the daily average samples from January 2012 to February 2016, related to solar irradiance on the plane of the array, electrical energy injected into the grid, reference yield, final yield, and the performance ratio. Furthermore, the decomposition time series for the performance ratio by applying the classical seasonal decomposition (CSD), Holt-Winters seasonal model (HW), and Seasonal and Trend decomposition using Loess (STL) is also provided for quantifying of the degradation rate of the PV system. The data are provided in the supplementary file included in this article. The dataset is related to the paper entitled "Performance and degradation assessment of large-scale grid-connected solar photovoltaic power plant in tropical semi-arid environment of India." [1].
