Interventions targeting the TBW may be able to improve multisensory function and ameliorate social communicative symptoms in clinical populations. In pure-coordination games where there are multiple Nash equilibria, the selection of coordinated responses is inexplicable by rational-choice theory - yet coordination is ubiquitous in daily interactions. The psychological game theory therefore evokes the idea of focal-points some equilibria being chosen due to its salience, as well as through predicting (i.e., mentalizing) other's response. Previous work has been limited to investigating how structural atrophy relates to deficits in coordination, or how brain activations differ between intuitive and deliberated coordination. In this study, we investigated how the strategy of coordination is reflected in the brain, compared to when no coordination is required. Using functional near-infrared spectroscopy (fNIRS), we examined the neural correlates of deriving a response to a category where participants had to either answer freely (i.e., a survey) or try to match their response with another participant (i.e., coordinate). We found that the coordination trials elicited significantly larger changes in oxyhemoglobin [HbO] concentration than survey trials in frontopolar and lateral prefrontal cortex (PFC). Individual differences in behavioral focal index was significantly correlated with [HbO] concentration in lateral PFC. Granger Causality (GC) analysis revealed greater effective connectivity from frontopolar to lateral PFC, and less GC from lateral PFC to frontopolar in the coordination condition. Our findings highlight the crucial role of frontopolar and lateral PFC in human coordination. Behavioural and neuroimaging studies have recently demonstrated that symbolic numerical order processing (i.e., deciding whether numbers are in an increasing/decreasing sequence or not) and symbolic numerical magnitude processing (e.g., deciding which of two numerals is larger) engage different cognitive mechanisms and brain regions. Because of this dissociation, growing interest has emerged to better understand the neurocognitive mechanisms of symbolic numerical order processing and their relationship to individual differences in arithmetic performance. In the present functional imaging work, we further investigated this link in a group of thirty children (7.2-10.25 years) from elementary school, who completed a symbolic numerical order verification (are the numbers going up? e.g., 1-2-3) and a symbolic numerical magnitude comparison task (which is the larger number? e.g., 5-7) inside the scanner, as well as an arithmetic fluency test outside the scanner. Behavioural results demonstrated the unique role of numerical order to predict children's arithmetic skills and confirmed its mediating power to explain the association between numerical magnitude and arithmetic performance. Imaging results showed a significant association between numerical order and arithmetic in the intersection of the right inferior frontal gyrus and insula, as well as the posterior middle temporal gyrus. An age-dependent change in brain activity was found in the left intraparietal sulcus. These findings solidify the developmental importance of symbolic numerical order processing in children and provide new evidence that the semantic control network mediates the relationship with arithmetic performance. During comprehension of a hierarchical structure, semantic integration between sequential, mismatched sentential constituents does not proceed when the later word in the sequence (e.g., the noun in the verb + classifier + noun) can be assigned an alternative role in the sentence (e.g., as a modifier of a subsequent object noun) (Zhang et al., 2011). Using electrophysiological measures, we examined how the availability of an alternative structure and whether the reader is motivated to generate an alternative structure affect semantic integration in a hierarchical structure. The same set of sentences was employed in three experiments in which the semantic congruency between the adjective and the object noun in the local structure and the sentential-contextual expectancy towards a noun were manipulated simultaneously. The reader's motivation was manipulated by tasks in which they made semantic acceptability judgment (Experiment 1), passively viewed sentences for comprehension (Experiment 2), or actively predicted the upcoming word and monitored the compatibility between the anticipated and the actual input (Experiment 3). Stronger contextual expectancy reduced N400 responses to the object noun regardless of semantic congruency, indicating that strengthening the constraints of an alternative argument decreased the effort of semantic integration between incompatible local constituents. Relative to weaker expectancy, the incongruent noun preceded by a stronger expectancy context elicited reduced N400 responses when no active prediction was demanded but showed equal amplitudes when such a demand was high. These findings demonstrate that the semantic failure in a flexible structure is likely to be resolved by the comprehender's motivation to generate an alternative structure based on the active use of contextual expectancy information, thus highlighting the "role-dependent semantic processing" during sentence comprehension. Microglial cells are considered as sensors of brain pathology by detecting any sign of brain lesions, infections, or dysfunction and can influence the onset and progression of neurological diseases. They are capable of sensing their neuronal environment via many different signaling molecules such as neurotransmitters, neurohormones and neuropeptides. The neuropeptide VGF has been associated with many metabolic and neurological disorders. TLQP21 is a VGF-derived peptide and has been shown to signal via C3aR1 and C1qBP receptors. The effect of TLQP21 on microglial functions in health or disease is not known. Studying microglial cells in acute brain slices, we found that TLQP21 impaired metabotropic purinergic signaling. Specifically, it attenuated the ATP-induced activation of a K+ conductance, the UDP-stimulated phagocytic activity and the ATP dependent laser lesion-induced process outgrowth. These impairments were reversed by blocking C1qBP, but not C3aR1 receptors. While microglia in brain slices from male mnclude modulation of phagocytic activity and responses to injury. During the task, novel choice options periodically replaced familiar options. The values of the novel options were unknown, and the monkeys had to explore them to see if they were better than other currently available options. The identity of the chosen stimulus and the reward outcome were strongly encoded in the responses of single OFC neurons. These two variables define the states and state transitions in our model that are relevant to decision making. The chosen value of the option, and the relative value of exploring that option were encous, and reward outcomes, which are necessary for computing the values of novel options. Drawing on their publications and activities, it argues that Irish groups adopted a significant, if circumscribed, response to 'learning disabilities' that was reflective of a broader political and social policy approach during the midcentury, with local organisations supporting parents of 'deficient' children and establishing key services across the country. It highlights the way in which these pioneering actions align with existing norms in the state and explores the effect of this voluntary-driven response for the intellectually disabled. Approached in this way, the actions of these learning disability organisations complicate international research on postwar disability activism while furthering an emergent body of research into the complex realities that precluded transformative change in Irish society during the mid-20th century. © Author(s) (or their employer(s)) 2020. Pilot study data suggest rehabilitation interventions based on behavioural activation (BA) and problem-solving treatment (PST) can facilitate post-treatment role resumption among breast cancer survivors. METHODS AND ANALYSIS This protocol describes a multisite randomised controlled trial comparing a 4-month long, nine-session BA and PST-informed rehabilitation intervention (BA/PS) against a time-matched, attention control condition. The overall objective is to assess the efficacy of BA/PS for enhancing breast cancer survivors' activity participation and quality of life over time. A total of 300 breast cancer survivors reporting participation restrictions after completing curative treatment for stage 1-3 breast cancer within the past year will be recruited across two sites (Dartmouth-Hitchcock Medical Center and University of Alabama at Birmingham). Assessments are collected on enrolment (T1) and 8 (T2), 20 (T3) and 44 (T4) weeks later. ETHICS AND DISSEMINATION Study procedures are approved by the Committee for the Protection of Human Subjects at Dartmouth College, acting as the single Institutional Review Board of record for both study sites (STUDY 00031380). METHODS AND ANALYSIS This prospective study will enrol 662 children (aged 21 days to less then 18 months) seeking treatment for TDD symptoms. Children will be treated with intravenous or intramuscular thiamine (100 mg daily for a minimum of 3 days) alongside other interventions deemed appropriate. Baseline assessments, prior to thiamine administration, include a physical examination, echocardiogram and venous blood draw for the determination of thiamine biomarkers. Follow-up assessments include physical examinations (after 4, 8, 12, 24, 36, 48 and 72 hours), echocardiogram (after 24 and 48 hours) and one cranial ultrasound.

The simplest elasticity model of the foundation underlying a slender beam under flexure was conceived by Winkler, requiring local proportionality between soil reactions and beam deflection. Such an approach leads to well-posed elastostatic and elastodynamic problems, but as highlighted by Wieghardt, it provides elastic responses that are not technically significant for a wide variety of engineering applications. Thus, Winkler's model was replaced by Wieghardt himself by assuming that the beam deflection is the convolution integral between soil reaction field and an averaging kernel. Due to conflict between constitutive and kinematic compatibility requirements, the corresponding elastic problem of an inflected beam resting on a Wieghardt foundation is ill-posed. Here, we present the evidence that somatic cells can attain the expression of pluripotency markers after their introduction into early embryos. Mouse embryonic fibroblasts introduced between blastomeres of cleaving embryos, within two days of in vitro culture, express transcription factors specific to blastocyst lineages, including pluripotency factors. Analysis of donor tissue marker DNA has revealed that the progeny of introduced cells are found in somatic tissues of foetuses and adult chimaeras, providing evidence for cell reprogramming. Analysis of ploidy has shown that in the chimaeras, the progeny of introduced cells are either diploid or tetraploid, the latter indicating cell fusion. The presence of donor DNA in diploid cells from chimaeric embryos proved that the non-fused progeny of introduced fibroblasts persisted in chimaeras, which is evidence of reprogramming by embryonic niche. When adult somatic (cumulus) cells were introduced into early cleavage embryos, the extent of integration was limited and only cell fusion-mediated reprogramming was observed. These results show that both cell fusion and cell interactions with the embryonic niche reprogrammed somatic cells towards pluripotency.
Protein profiling allows the determination of the presence of proteins marking various stages of the disease, and differentiates between people at risk of various diseases. In type 1 diabetes, protein profiling had been previously used to find blood markers other than islet autoantibodies to indicate the pancreatic beta cell destruction process and to reflect the progression of type 1 diabetes mellitus (T1DM). However, T1DM is an auto-immune disease and its clinical presentation changes in time of its duration.
To find differences in protein profiles in patients with type 1 diabetes according to diabetes control (HbA1c > 7%) and with presence of diabetic complications or obesity. It may help to identify subgroups of patients who may need a better clinical supervision and individualized treatment.
A group of 103 patients with auto-immunologically confirmed T1DM, and meeting the following inclusion criteria Caucasian race, duration of diabetes >5 years, were used in the study. Criteria of exclusion atients with type 1 diabetes.

Understanding the genetic basis of salt tolerance is vital to develop tolerant varieties. A diversity panel consisting of 276 lentil accessions was screened in a previous study through traditional and image-based approaches to quantify growth under salt stress. Genotyping was performed using two contrasting methods, targeted (tGBS) and transcriptome (GBS-t) genotyping-by-sequencing, to evaluate the most appropriate methodology. tGBS revealed the highest number of single-base variants (SNPs) (c. 56,349), and markers were more evenly distributed across the genome compared to GBS-t. A genome-wide association study (GWAS) was conducted using a mixed linear model. Carbapenem-resistance in healthcare-associated infections (HCAIs) is of great concern, and it is urgent to improve surveillance. We aimed to describe and analyze HCAIs trends on Gram-negative antimicrobial susceptibility in a city from a developing country, following the implementation of an active surveillance program.
This is an aggregated study describing data from 24 hospitals with intensive care units, including a trend analysis by Joinpoint regression between January 2012 and December 2017.
There were 23,578 pathogens in 39,832 HCAIs, from which 16,225 were Gram-negatives (68.8%). Carbapenem susceptibility was lowest in A. baumannii (15.4-25.9%), K. pneumoniae (51.0-55.9%), and P. aeruginosa (64.9-84.1%) and highest in E. coli (96.5-99.2%). Only K. pneumoniae showed a significant Joinpoint at 95% confidence interval -10.71% (-18.02; -2.75) from 2012 to 2014, p=0.02, and 6.54% (-2.00; 15.83) from 2015 to 2017, p=0.12, which was most influenced by urinary tract infections -9.98% (-16.02; -3.48) from 2012 to 2014, p=0.01, and 9.66% (-1.75; 22.39) from 2015 to 2017, p=0.09.

Beta-thalassemia is a genetic disorder that is inherited in an autosomal recessive pattern. This genetic disease leads to a defective beta-globin hemoglobin chain causing partial or complete beta-globin chain synthesis loss. Beta-thalassemia major patients need a continuous blood transfusion and iron chelation to maintain the normal homeostasis of red blood cells (RBCs) and other systems in the body. Patients also require treatment procedures that are costly and tedious, resulting in a serious health burden for developing nations such as Nepal.
A total of 61 individuals clinically diagnosed to have thalassemia were genotyped with multiplex amplification refractory mutation system-polymerase chain reaction (ARMS-PCR). Twenty-one major mutations were investigated using allele-specific primers grouped into six different panels.

Two histologic alterations found to be significantly different between the 35 and 39 and greater than 40 age subgroups were fetal vascular malperfusion (11% [7 of 65] versus 42% [43 of 103]; P = .001) and delayed villous maturation (1.5% [1 of 65] versus 13% [13 of 103]; P = .02). The pure AMA subgroup showed no statistically significant differences compared with the overall AMA group. Chronic deciduitis was the only statistically significant difference between the overall AMA group and the non-AMA comparison group (14% [23 of 168] versus 30% [18 of 60]; P = .02). OBJECTIVE.— To investigate whether this increase can mask a diagnosis of factor V Leiden (FVL) or protein S deficiency in an actual population of patients undergoing apixaban treatment and hypercoagulation testing. DESIGN.— During a 4.5-year period involving 58 patients, we compared the following 4 groups heterozygous for FVL (FVL-HET)/taking apixaban, wild-type/taking apixaban, heterozygous for FVL/no apixaban, and normal APCR/no apixaban. Patients taking apixaban were also tested for protein S functional activity and free antigen (n = 40). RESULTS.— FVL-HET patients taking apixaban had lower APCR ratios than wild-type patients (P less then .001). Activated protein C resistance in FVL-HET patients taking apixaban fell more than 3 SD below the cutoff of 2.2 at which the laboratory reflexes FVL DNA testing. No cases of FVL were missed despite apixaban. In contrast to rivaroxaban, apixaban did not interfere with the assessment of protein S activity (mean activity 93.9 IU/dL, free antigen 93.1 IU/dL, P = .39). A total 3 of 40 patients (8%) had low free protein S antigen (30, 55, and 57 IU/dL), with correspondingly similar activity results (27, 59, and 52 IU/dL, respectively). Apixaban did not cause a missed diagnosis of protein S deficiency. CONCLUSIONS.— Despite apixaban treatment, APCR testing can distinguish FVL-HET from healthy patients, rendering indiscriminate FVL DNA testing of all patients on apixaban unnecessary. OBJECTIVE.— To analyze routine HER2 DISH performance in the field. DESIGN.— We reviewed our experience with HER2 DISH performed at outside laboratories and referred for patient care. RESULTS.— Of 273 identified retrospective DISH results, 55 had repeated FISH testing at our institution; 7 (13%) were discordant. Additional cases had technical flaws hampering appropriate scoring. In 23 cases (42%), HER2 DISH was performed without immunohistochemistry. Slide review of a prospective cohort of 42 consecutive DISH cases revealed 14 (33%) with technical or interpretative limitations potentially jeopardizing results. Commonly identified problems include lack of or weak signals in most tumor cells, and silver precipitate or red signals outside of nuclei, resulting in false-negative or false-positive interpretations, respectively. Further, 44% (24 of 55) of DISH reports lacked complete data, specifically average HER2 signals/cell. CONCLUSIONS.— While HER2 DISH can be an efficient and effective alternative to FISH, we illustrate pitfalls and reinforce that careful attention to slide quality and technical parameters are critically important. METHODS AND MATERIALS We retrospectively compared 129 consecutive patients (136 lesions) who had BCS with SSM at our centre between 11/2011 and 02/2013 with 138 consecutive patients (144 lesions) who underwent BCS with ISM between 08/2014 and 02/2015.SSM or ISM were performed to confirm the target lesions within the excised specimen and margin adequacy. The utility of SMM and ISM was evaluated considering pathology as gold-standard, using χ2 or Fisher's exact tests for comparison of categorical variables, and non-parametric Mann-Whitney test for continuous variables. RESULTS The two groups did not statistically differ for age (p = 0.20), lesion size (p = 0.29) and morphology (p = 0.82) or tumor histology type (p = 0.65). Handcrafted radiomics is a multistage process in which features based on shape, pixel intensities, and texture are extracted from radiographs. Within this review, we describe the steps starting with quantitative imaging data, how it can be extracted, how to correlate it with clinical and biological outcomes, resulting in models that can be used to make predictions, such as survival, or for detection and classification used in diagnostics.
Two histologic alterations found to be significantly different between the 35 and 39 and greater than 40 age subgroups were fetal vascular malperfusion (11% [7 of 65] versus 42% [43 of 103]; P = .001) and delayed villous maturation (1.5% [1 of 65] versus 13% [13 of 103]; P = .02). The pure AMA subgroup showed no statistically significant differences compared with the overall AMA group. Chronic deciduitis was the only statistically significant difference between the overall AMA group and the non-AMA comparison group (14% [23 of 168] versus 30% [18 of 60]; P = .02). CONCLUSIONS.— Our findings, particularly the high frequency of fetal vascular malperfusion, suggest that AMA should be an independent indication for placental pathologic examination.CONTEXT.— Apixaban causes a false increase in activated protein C resistance (APCR) ratios and possibly protein S activity. OBJECTIVE.— To investigate whether this increase can mask a diagnosis of factor V Leiden (FVL) or protein S deficiency in an actual population of patients undergoing apixaban treatment and hypercoagulation testing. DESIGN.— During a 4.5-year period involving 58 patients, we compared the following 4 groups heterozygous for FVL (FVL-HET)/taking apixaban, wild-type/taking apixaban, heterozygous for FVL/no apixaban, and normal APCR/no apixaban. Patients taking apixaban were also tested for protein S functional activity and free antigen (n = 40). RESULTS.— FVL-HET patients taking apixaban had lower APCR ratios than wild-type patients (P less then .001). Activated protein C resistance in FVL-HET patients taking apixaban fell more than 3 SD below the cutoff of 2.2 at which the laboratory reflexes FVL DNA testing. No cases of FVL were missed despite apixaban. In contrast to rivaroxaban, apixaban did not interfere with the assessment of protein S activity (mean activity 93.9 IU/dL, free antigen 93.1 IU/dL, P = .39). A total 3 of 40 patients (8%) had low free protein S antigen (30, 55, and 57 IU/dL), with correspondingly similar activity results (27, 59, and 52 IU/dL, respectively). Apixaban did not cause a missed diagnosis of protein S deficiency. CONCLUSIONS.— Despite apixaban treatment, APCR testing can distinguish FVL-HET from healthy patients, rendering indiscriminate FVL DNA testing of all patients on apixaban unnecessary. During the last 5+ years, brightfield dual in situ hybridization (DISH) has replaced fluorescence methods (fluorescence in situ hybridization [FISH]) in some laboratories. OBJECTIVE.— To analyze routine HER2 DISH performance in the field. DESIGN.— We reviewed our experience with HER2 DISH performed at outside laboratories and referred for patient care. RESULTS.— Of 273 identified retrospective DISH results, 55 had repeated FISH testing at our institution; 7 (13%) were discordant. Additional cases had technical flaws hampering appropriate scoring. In 23 cases (42%), HER2 DISH was performed without immunohistochemistry. Slide review of a prospective cohort of 42 consecutive DISH cases revealed 14 (33%) with technical or interpretative limitations potentially jeopardizing results. Commonly identified problems include lack of or weak signals in most tumor cells, and silver precipitate or red signals outside of nuclei, resulting in false-negative or false-positive interpretations, respectively. Further, 44% (24 of 55) of DISH reports lacked complete data, specifically average HER2 signals/cell. METHODS AND MATERIALS We retrospectively compared 129 consecutive patients (136 lesions) who had BCS with SSM at our centre between 11/2011 and 02/2013 with 138 consecutive patients (144 lesions) who underwent BCS with ISM between 08/2014 and 02/2015.SSM or ISM were performed to confirm the target lesions within the excised specimen and margin adequacy. The utility of SMM and ISM was evaluated considering pathology as gold-standard, using χ2 or Fisher's exact tests for comparison of categorical variables, and non-parametric Mann-Whitney test for continuous variables. RESULTS The two groups did not statistically differ for age (p = 0.20), lesion size (p = 0.29) and morphology (p = 0.82) or tumor histology type (p = 0.65). Abortion in small ruminants is a significant problem in Iraq and causes severe economic losses in sheep farms. Chlamydia abortus causes enzootic abortion in ewes and is associated with reproductive problems in sheep in Sulaimani province - Northern Iraq. During a lambing season in 2017, abortion was widespread among several sheep flocks in different regions of Sulaimani (Kalar, Said Sadiq, and Chamchamal), and C. abortus was one of the causes. Accordingly, we carried out this study to isolate and identify C. abortus in aborted ewes in these regions. We collected 30 samples of aborted fetuses from five herds in which abortions had been observed. The pathogen isolation was done by inoculation into embryonated chicken eggs and conventional PCR was used to identify C. abortus in clinical specimens. C. abortus was identified in one of the 30 aborted fetuses (3.33%) from the Kalar district, and all the remaining 29 samples (96.66%) were found positive to Brucella abortus. The gene ompA encoding the outer membrane protein of C. abortus was sequenced and got the accession number MK643153 in NCBI GenBank. The sequence was named C. abortus strain Sul/2017. Our isolate showed 99.79% homology with Sul/014 (accession No. KY399850) and differed from the latter by two amino acid substitutions at E115K and K259N. We thus employed microRNAs (miR-431, miR-636) identified by their ability to bind AAV genome and also dysregulate Mitogen-activated protein kinase (MAPK) signaling during vector production, by a global transcriptome study in producer cells. A modified vector assembly protocol incorporating a plasmid encoding these microRNAs was developed. AAV2 vectors packaged in the presence of microRNA demonstrated an improved gene transfer potency by 3.7-fold, in vitro. Furthermore, AAV6 serotype vectors encoding an inducible caspase 9 suicide gene, packaged in the presence of miR-636, showed a significant tumor regression (~2.2-fold, P less then .01) in a syngeneic murine model of T-cell lymphoma. Many substances were described only in method validations or case reports, and data were obtained from small case numbers. On the contrary, clozapine, haloperidol, amitriptyline, nortriptyline, risperidone and its metabolite, methylphenidate, citalopram, chlorpromazine, chlorprothixene, and quetiapine had a well-founded database as these substances were investigated in controlled dose studies with higher case numbers. Given the advancements made in analytical techniques over the past years, gas chromatography-mass spectrometry and liquid chromatography with tandem mass spectrometry techniques were the methods of choice and allowed the detection of chemical compounds at low concentrations. The controversy around a potential use of hair analysis to estimate the dosage remains as dose-concentration studies provided divergent results. A harmonization on the investigated hair length as well as on the extraction protocol would be of favor to achieve better comparability. Due to the endothermic nature of removal of aqueous As, Sb, W, and F by Mg-Fe-OH-LDH, the original high temperatures of the geothermal waters could promote their sorption effectively. Batch sorption experiments demonstrated that over 94% and 80% of the As and W removal amounts could be reached within the first 10 and 20 min, respectively. On-site column experiments confirmed thaived pollution.Understanding the spatial distribution of soil salinity is required to conserve land against degradation and desertification. Against this background, this study is the first attempt to predict soil salinity in the Jaghin basin, in southern Iran, by applying and comparing the performance of four deep learning (DL) models (deep convolutional neural networks-DCNNs, dense connected deep neural networks-DenseDNNs, recurrent neural networks-long short-term memory-RNN-LSTM and recurrent neural networks-gated recurrent unit-RNN-GRU) and six shallow machine learning (ML) models (bagged classification and regression tree-BCART, cforest, cubist, quantile regression with LASSO penalty-QR-LASSO, ridge regression-RR and support vectore machine-SVM). In this paper, a hybrid deep learning model VMD-BiLSTM is constructed, which combines variational mode decomposition (VMD) and bidirectional long short-term memory network (BiLSTM), to predict PM2.5 changes in cities in China. VMD decomposes the original PM2.5 complex time series data into multiple sub-signal components according to the frequency domain. Then, BiLSTM is employed to predict each sub-signal component separately, which significantly improved forecasting accuracy. Through a comprehensive study with existing models, such as the EMD-based models and other VMD-based models, we justify the outperformance of the proposed VMD-BiLSTM model over all compared models. The results show that the prediction results are significantly improved with the proposed forecasting framework. And the prediction models integrating VMD are better than those integrating EMD. An undesirable directional distance function (DDF) dynamic data envelopment analysis (DEA) model was employed for the dynamic analysis as it was able to deal with both desirable and undesirable outputs at the same time. The model examined collected water supply and water leak efficiency data from 30 Chinese provinces/municipalities from 2007 to 2018, from which it was found that (1) Beijing, Gansu, Guangdong, and Ningxia had efficient water supply and water leak losses from 2007 to 2018 and the most improved province was Jiangxi; (2) the eastern provinces, in general, had better water efficiencies and the central and western provinces needed greater improvements; and (3) the lowest water leakage loss efficiencies were in Inner Mongolia, Jiangxi, and Heilongjiang, all of which required significant improvements.

Against this background, this study is the first attempt to predict soil salinity in the Jaghin basin, in southern Iran, by applying and comparing the performance of four deep learning (DL) models (deep convolutional neural networks-DCNNs, dense connected deep neural networks-DenseDNNs, recurrent neural networks-long short-term memory-RNN-LSTM and recurrent neural networks-gated recurrent unit-RNN-GRU) and six shallow machine learning (ML) models (bagged classification and regression tree-BCART, cforest, cubist, quantile regression with LASSO penalty-QR-LASSO, ridge regression-RR and support vectore machine-SVM). To do this, 49 environmental landsat8-derived variables including digital elevation model (DEM)-extracted covariates, soil-salinity indices, and other variables (e.g., soil order, lithology, land use) were mapped spatially. In this paper, a hybrid deep learning model VMD-BiLSTM is constructed, which combines variational mode decomposition (VMD) and bidirectional long short-term memory network (BiLSTM), to predict PM2.5 changes in cities in China. VMD decomposes the original PM2.5 complex time series data into multiple sub-signal components according to the frequency domain. Then, BiLSTM is employed to predict each sub-signal component separately, which significantly improved forecasting accuracy. Through a comprehensive study with existing models, such as the EMD-based models and other VMD-based models, we justify the outperformance of the proposed VMD-BiLSTM model over all compared models. The results show that the prediction results are significantly improved with the proposed forecasting framework. And the prediction models integrating VMD are better than those integrating EMD. Down syndrome (DS) is a common congenital chromosomal disorder related to trisomy 21. Lateral cephalometric radiography studies have shown that patients with DS have characteristic craniofacial morphology; however, no 3-dimensional analysis studies have been performed to investigate the craniofacial features, including volumetric aspects, of patients with DS. The present study was performed to evaluate the craniofacial features, including volumetric aspects, of patients with DS and to compare these findings with control participants using cone beam computed tomography (CBCT).
The study sample consisted of 12 patients with DS and 12 control participants. All participants were examined by means of CBCT; the resulting images were used for evaluation of maxillary and mandibular volume, cranial base, and craniofacial measurements. Differences between patients with DS and control participants were statistically analyzed using Student
test.
Compared to control participants, patients with DS exhibited statistically significant reductions in maxillary and mandibular volumes. Both sagittal and axial cranial base linear measurements were shorter in patients with DS than in control participants. In contrast, the cranial base angle was enhanced in patients with DS, compared with control participants. Moreover, condylion (Co)-gnathion, anterior nasal spine-menton, and Co-subspinale (point A) measurements were shorter in patients with DS than in control participants; the sella-nasion-mandibular plane angle was significantly reduced in patients with DS, compared with control participants.
Our results suggest that patients with DS have distinct skeletal volume and craniofacial morphology features, relative to individuals without DS.

Until 2019, there were no approved treatments for ATTR-CM. Treatment consisted of symptom management and organ transplant. Nonpharmacological and pharmacological treatments focused on the symptoms of heart failure (HF) associated with ATTR-CM. However, there are several emerging therapies recently approved or in development to address the underlying pathophysiology. Treatment classes for ATTR-CM include transthyretin stabilizers, human monoclonal antibodies, gene silencers, and CRISPR/Cas9 gene editing.
ATTR-CM is a complex disease in which amyloidosis causes cardiomyopathy. Underdiagnosis is attributed to the clinical presentation being heterogeneous, indistinguishable from HF caused by other etiologies, and the need for invasive testing modalities, including endomyocardial biopsy. Improved diagnostic approaches along with targeted therapies can slow disease progression and enhance patient quality of life.
Diagnostic modalities along with biomarker and genetic testing could detect disease earlier and target therapy more accurately. Novel therapies demonstrate potential treatment benefits and can help shape the standard of care for these patients.

We consider trials in which clusters prospectively recruit or identify participants as a continuous process over a given calendar period, and ask whether and for how long investigators should collect baseline data as part of the trial, in order to maximise precision.
We show how to calculate and plot the variance of the treatment effect estimator for different lengths of baseline period in a range of scenarios, and offer general advice.
In some circumstances it is optimal not to include a baseline, while in others there is an optimal duration for the baseline. All other things being equal, the circumstances where it is preferable not to include a baseline period are those with a smaller recruitment rate, smaller intracluster correlation, greater decay in the intracluster correlation over time, or wider transition period between recruitment under control and intervention conditions.

Critically ill patients with COVID-19 infection on extracorporeal membrane oxygenation (ECMO) face high morbidity and mortality. Palliative care consultation may benefit these patients and their families. Prior to the pandemic, our institution implemented a policy of automatic palliative care consultation for all patients on ECMO due to the high mortality, medical complexity, and psychosocial distress associated with these cases.
The main objective was to describe the role of the palliative care team for patients on ECMO for COVID-19 infection. The secondary objective was to describe the clinical outcomes for this cohort.
Case series.
All patients age 18 or older infected by the novel coronavirus who required cannulation on ECMO from March through July of 2020, at an urban, academic medical center in the United States. Inter-disciplinary palliative care consultation occurred for all patients.
Twenty-three patients (median age 43 years [range 28-64], mean body mass index 34.9 kg/m2 [SD 9.2], 65% Hispanic ethnicity) were cannulated on ECMO.

Prior to the pandemic, our institution implemented a policy of automatic palliative care consultation for all patients on ECMO due to the high mortality, medical complexity, and psychosocial distress associated with these cases. During COVID-19 in Sierra Leone, we did not notice a significant drop in malaria diagnosis in children, suggesting that a proactive approach in the management of malaria in endemic countries during COVID-19 may have had a positive impact. A comprehensive approach that include also educational activities to sensitize the local population, was useful to guarantee successful malaria diagnosis and treatment, and prevents excess of malaria deaths due to potential disruption of the local health systems related to the SARS-CoV-2 pandemic.

Activities to control human African trypanosomiasis (HAT) in Guinea were severely hampered by the Ebola epidemic that hit this country between 2014 and 2016. Active screening was completely interrupted and passive screening could only be maintained in a few health facilities. At the end of the epidemic, medical interventions were progressively intensified to mitigate the risk of HAT resurgence and progress towards disease elimination.
A retrospective analysis was performed to evaluate the medical activities that were implemented in the three most endemic prefectures of Guinea (Boffa, Dubreka and Forecariah) between January 2016 and December 2018. Passive screening using rapid diagnostic tests (RDTs) was progressively resumed in one hundred and one health facilities, and active screening was intensified by visiting individual households and performing RDTs, and by conducting mass screening in villages by mobile teams using the Card Agglutination Test for Trypanosomiasis. A total of 1885, 4897 and 8023 clinthe number of HAT cases, confirming the fear that the disease could resurge as a result of impaired control activities during the Ebola epidemic. On the other hand, the decrease in disease prevalence that was observed between 2016 and 2018 is encouraging, as it suggests that the current strategy combining enhanced diagnosis, treatment and vector control is appropriate to progress towards elimination of HAT in Guinea.
Better drug regimens for mass drug administration (MDA) could accelerate the Global Programme to Eliminate Lymphatic Filariasis (LF). This community study was designed to compare the safety and efficacy of MDA with IDA (ivermectin, diethylcarbamazine and albendazole) or DA (diethylcarbamazine and albendazole) in India.
This two-armed, open-labelled, block randomised, community study was conducted in LF endemic villages in Yadgir district, Karnataka, India. Consenting participants ≥5 years of age were tested for circulating filarial antigenemia (CFA) and microfilaremia (Mf) before treatment with a single oral dose of IDA or DA. Adverse events (AEs) were monitored actively for two days and passively for five more days. IDA had an acceptable safety profile and was more effective for clearing Mf than DA. With adequate compliance and medical support to manage AEs, IDA has the potential to accelerate LF elimination in India.

Reference interval (RI) for hematological parameters is used to interpret laboratory test results in the diagnosis, management and monitoring of hematologic disorders. Several factors including sex, age, dietary patterns, pregnancy status, ethnicity and geographic location affect hematological RIs. However, manufacturers derived reference value is currently in use in most developing countries including Ethiopia. This study aimed to establish hematological RIs for adult population living in Dire Dawa town, East Ethiopia.
In this cross-sectional study, 513 apparently healthy adults of Dire Dawa town were enrolled from January to March 2019. From these, 342 (171 males and 171 non-pregnant females) were aged 18-65 years while 171 were pregnant women aged 15-49 years. After obtaining written informed consent, 5ml fresh whole blood was collected of which 2ml was used for hematologic analysis using Mindray **-3000plus hematology analyzer and 3ml for serological tests. Although the application of ML algorithms to the transportation planning processes-like mode choice, and traffic forecasting and demand modeling-have received **** attention in research and abound in literature, scanty attention is paid to its application to vehicle ownership modeling especially in the context of small to medium cities in developing countries. Therefore, this study attempts to fill this gap by modeling vehicle ownership in the Greater Tamale Area (GTA), a typically small to medium city in Ghana. Using a cross sectional survey of formal sectors workers, data was collected between Juneonthly expenditure on transport, duration of travel to workplace, occupational rank, age, household size and marital status were significant in predicting vehicle ownership for most of the classifiers. These findings could help policies makers carve out strategies that would reduce vehicle ownership but improve personal mobility.
