To describe the management, complications and prognosis of lacrimalprobing in patients with congenital nasolacrimal duct obstruction (CNLDO).
Retrospective, observational and analytical study. We studied the prevalence, symptoms and signs, both before and after probing, in patients with CNLDO diagnosed between 2010 and 2017 at Instituto Fundación Conde deValenciana.
The prevalence at our hospital of congenital nasolacrimal duct obstruction in children less than 1 year of age was 16.1%. 71 eyes of 56 patients undergoing probing were analyzed. 61% of patients were male. The right eye was affected in 46.4%.The average age of diagnosis of CNLDO was 10.2±7.6 months. 84.5% consulted for excessive tearing and 73.2% because of eye discharge. On ophthalmological examination, 71.8% had increased tear meniscus and 45.1% had discharge from punctum when digital pressure was applied to the lacrimal sac. The average age at the time of probing was 14.1±8.4 months. Subsequent to the probing, only 21.1% reported presence of tearing and 5.6% reported ocular discharge, both with a statistically significant reduction (p<0.05 from pre-op). After probing, only 16.9% of eyes had increased tear meniscus and 11.3% presented discharge from punctum after pressure at the lacrimal sac. Both decreases were statistically significant (p<0.05). 2 eyes (2.82%)of 2 patients required a second probing to obtain full resolution.

Data regarding demographic characteristics, vision acuity, involved eye, clinical features, conjunctival lesion type, duration of symptoms, preoperative clinical diagnosis, history, other eye disease, associated systemic diseases, occupation as well as treatment modalities of the patients and its outcome were taken.
Out of 40 cases, females were 21(52.5%) vesus 19(47.5%) male. Amongthem, 19 patients had keratinized stratified squamous epithelium, 17 had conjunctiva intraepithelial squamous neoplasia ansfactory. Further studies with prolonged follow-up are recommended to evaluate the risk of recurrence and other adverse effects.
To determine the surgical and visual outcomes of posteriorly dislocated lens fragments in the vitreous cavity in patients undergoing cataract surgery.
A total of 149 eyes of 149 patients from 2013 to 2018 were included in the study. The primary cataract surgery was performed either at the base hospital and its peripheral centres or referred from elsewhere. Pars plana vasectomy and nucleus removal was performed along with implantation of intraocular lens, wherever possible. Success was defined as best corrected visual acuity (BCVA) ≥ 6/12 at 3 months follow up. Poor visual outcome was defined as per WHO guidelines as BCVA ≤ 3/60.
Posterior capsular rupture and dislocation into vitreous cavity most frequently occurred during phaco-fragmentation in cases of phacoemulsification and during nucleus delivery in cases of small incision cataract surgery. Early vitrectomy was performed within 3 days in 36.2% of cases and within 14 days in 63.8% of cases. Successful visual outcome was achieved in 85.2% of patientsns can be successfully managed in majority of cases with vitreoretinal surgical intervention. The timing of vitrectomy whether performed early or late did not affect the visual outcome. The most important predictorof final visual acuity after PPV for retained lens fragments is a less complicated clinical course without any associated complications such as retinal detachment, cystoidmacula edema and glaucoma. Expertise of the primary cataract surgeon could not be assessed in this study, though surgeon grade with more experience is an important factor in the assessment of complications during the cataract surgery.

Examination included visual acuity testing using Snellens chart, slit lamp biomicroscopy, fund us examination with direct ophthalmoscope, indirect ophthalmoscope and refraction. Students were examined for refractive errors and findings compared with pre-admission data. Changes in spherical, cylindrical power analysed.
Prevalence of Myopia increased from 40% to 48%. Incidence of new refractive errors in emmetropes was 8%. Myopic change found was -0.75D to -1.0D.
Students in medical colleges have high prevalence of myopia, high incidence of adult onset myopia and statistically significant progression. Parental history, prolonged eye strain, excessive use of electronic gadgets, decreased outdoor activity and urban background had significant association with prevalence of myopia.

Intraocular lymphoma is rare. There are very few studies on intraocular lymphoma published from Asian countries.
To report our case series of intraocular lymphoma patients from a tertiary eye centre in Singapore.
Nine patients with intraocular lymphoma managed between January 2005 and December 2014 were identified from Ocular Autoimmune Systemic Inflammatory Infectious Study (OASIS) database. Demographic characteristics, clinical presentation, investigations performed and outcomes recorded.
There were almost equal distribution between males (four patients) and females (five patients) with mean age of presentation was 60.3 years. Five patients had bilateral involvement and vitreo-retina was the most common site of infiltration. All of our patients had central nervous system involvement although four of them had presented with ocular manifestations initially. Anterior chamber fluid cytology, as a less invasive alternative to vitreous analysis was proven to be useful. The time from ocular presentation to diagnosis of ocular lymphoma was variable; from one day to 18 months.

Soft contact lenses have generated growing interest in ocular drug delivery due to their potential to enhance drug bioavailability in ocular tissues. Commercially available soft contact lenses offer several advantages for ocular drug delivery as they are manufactured on a large scale, which guarantees the availability of a consistent and reproducible product, and their favorable safety profile is well-established through broad clinical use. Here we review the rationale for using commercially available soft contact lenses for ocular drug delivery; summarize the evolution of the materials used in contact lens fabrication; and explore various methods used to improve the drug release characteristics and its tissue penetration. While significant progress has been made, several issues still require further attention for the commercial launch of a viable drug-eluting contact lens product, including control of initial burst release, shelf-life stability, and drug loss during processing or storage.

Body image changes are common among patients living with inflammatory bowel disease (IBD) because of the illness and related treatments. Currently, there is little known about how those patients experience and perceive their altered body image in the literature. The aim of this study was to explore the experiences of body image changes in patients with IBD in China and to describe how those changes influence patients' perception of body and self.
This study used a descriptive qualitative approach. A total of 16 participants from three tertiary hospitals in southeast China were recruited through purposive sampling combined with maximum variation strategy. Data were collected through semistructured interviews and analyzed using conventional content analysis method.
The following six themes were extracted (1) "being a constrained person," (2) "being a flawed person," (3) "being a disliked person," (4) "being an alienated person," (5) "being a reconciled person," and (6) "being a blessed person."
With the diagnosis of IBD and its side effects of treatments, patients' life changed significantly and they were struggling to reinterpret their body and self. As a result, negative self-evaluations and/orpositive self-evaluations towards themselves generated. Further studies are required to explore the underlying mechanism and related influencing factors about how those different images were produced.

This study aims at realizing unsupervised term discovery in Chinese electronic health records (EHRs) by using the word segmentation technique. The existing supervised algorithms do not perform satisfactorily in the case of EHRs, as annotated medical data are scarce. We propose an unsupervised segmentation method (GTS) based on the graph partition principle, whose multi-granular segmentation capability can help realize efficient term discovery.
A sentence is converted to an undirected graph, with the edge weights based on n-gram statistics, and ratio cut is used to split the sentence into words. The graph partition is solved efficiently via dynamic programming, and multi-granularity is realized by setting different partition numbers. A BERT-based discriminator is trained using generated samples to verify the correctness of the word boundaries. The words that are not recorded in existing dictionaries are retained as potential medical terms.
We compared the GTS approach with mature segmentation systems for both word segmentation and term discovery. MD students manually segmented Chinese EHRs at fine and coarse granularity levels and reviewed the term discovery results. The proposed unsupervised method outperformed all the competing algorithms in the word segmentation task. In term discovery, GTS outperformed the best baseline by 17 percentage points (a 47% relative percentage of increment) on F1-score.
In the absence of annotated training data, the graph partition technique can effectively use the corpus statistics and even expert knowledge to realize unsupervised word segmentation of EHRs. Multi-granular segmentation can be used to provide potential medical terms of various lengths with high accuracy.

We propose a novel optical 1×2 power splitter based on an asymmetric ladder-shaped multimode interference (MMI) coupler in silicon-on-insulator (SOI) which has an ultra-compact size of 3.3 µm×2.4 µm. A trapezoid with a small region is removed from the bottom left corner of the MMI coupler to achieve variable splitting ratio. The comparison with the asymmetric rectangular 1×2 splitter is numerically analyzed. By carefully optimizing the width of input taper, the proposed splitter shows a low phase deviation for the two output ports while keeping both of a low-loss performance and feasible splitting ratio. The simulated results show that the splitter can operate with an insertion loss less than 0.67 dB, a large range of splitting ratio from 5050 to 1189 and an ultra-low phase deviation less than 2.8° among the C band spectra.Water scarcity is still a pressing issue in many regions. The application of membrane technology through water desalination to convert brackish to potable water is a promising technology to solve this issue. This study compared the performance of templated TEOS-P123 and ES40-P123 hybrid membranes for brackish water desalination. The membranes were prepared by the sol-gel method by employing tetraethyl orthosilicate (TEOS) for the carbon-templated silica (soft template) and ethyl silicate (ES40) for the hybrid organo-silica. Both sols were templated by adding 35 wt.% of pluronic triblock copolymer (P123) as the carbon source. The silica-templated sols were dip-coated onto alumina support (four layers) and were calcined by using the RTP (rapid thermal processing) method. The prepared membranes were tested using pervaporation set up at room temperature (~25 °C) using brackish water (0.3 and 1 wt.%) as the feed. It was found that the hybrid membrane exhibited the highest specific surface area (6.72 m2·g-1), pore size (3.67 nm), and pore volume (0.45 cm3·g-1). The hybrid ES40-P123 was twice thicker (2 μm) than TEOS-P123-templated membranes (1 μm). Lastly, the hybrid ES40-P123 displayed highest water flux of 6.2 kg·m-2·h-1. Both membranes showed excellent robustness and salt rejections of >99%.The MoSi2-ZrB2 coatings were prepared on Nb-Si based alloy by atmospheric plasma spraying with the spraying power 40, 43 and 45 kW. The effect of spraying power on the microstructure and oxidation resistance of MoSi2-ZrB2 coating at 1250 °C were studied. We propose a novel optical 1×2 power splitter based on an asymmetric ladder-shaped multimode interference (MMI) coupler in silicon-on-insulator (SOI) which has an ultra-compact size of 3.3 µm×2.4 µm. A trapezoid with a small region is removed from the bottom left corner of the MMI coupler to achieve variable splitting ratio. The comparison with the asymmetric rectangular 1×2 splitter is numerically analyzed. By carefully optimizing the width of input taper, the proposed splitter shows a low phase deviation for the two output ports while keeping both of a low-loss performance and feasible splitting ratio. The simulated results show that the splitter can operate with an insertion loss less than 0.67 dB, a large range of splitting ratio from 5050 to 1189 and an ultra-low phase deviation less than 2.8° among the C band spectra.Water scarcity is still a pressing issue in many regions. The application of membrane technology through water desalination to convert brackish to potable water is a promising technology to solve this issue. This study compared the performance of templated TEOS-P123 and ES40-P123 hybrid membranes for brackish water desalination. The membranes were prepared by the sol-gel method by employing tetraethyl orthosilicate (TEOS) for the carbon-templated silica (soft template) and ethyl silicate (ES40) for the hybrid organo-silica. Both sols were templated by adding 35 wt.% of pluronic triblock copolymer (P123) as the carbon source. The silica-templated sols were dip-coated onto alumina support (four layers) and were calcined by using the RTP (rapid thermal processing) method. The prepared membranes were tested using pervaporation set up at room temperature (~25 °C) using brackish water (0.3 and 1 wt.%) as the feed. It was found that the hybrid membrane exhibited the highest specific surface area (6.72 m2·g-1), pore size (3.67 nm), and pore volume (0.45 cm3·g-1). The hybrid ES40-P123 was twice thicker (2 μm) than TEOS-P123-templated membranes (1 μm). Lastly, the hybrid ES40-P123 displayed highest water flux of 6.2 kg·m-2·h-1. Both membranes showed excellent robustness and salt rejections of >99%.The MoSi2-ZrB2 coatings were prepared on Nb-Si based alloy by atmospheric plasma spraying with the spraying power 40, 43 and 45 kW. The effect of spraying power on the microstructure and oxidation resistance of MoSi2-ZrB2 coating at 1250 °C were studied. We collected the data from medical records and investigated their clinical manifestation and laboratory examination and IABP support, as well as 28-day (from diagnosis of cardiogenic shock) mortality retrospectively. Kaplan-Meier survival analysis was used to compare the 28-day survival rates of patients of different genders/with or without IABP treatment. Adjustment for age, systolic blood pressure, ST segment elevation myocardial infarction, dual antiplatelet, coma, APACHEâ…¡ score and SAPSâ…¡ score, Cox regression analysis was used to detect the affect of IABP treatment on the risk of all-cause mortality in different crowd, and using Z test to evaluate the modificive IABP, female patients who did not receive IABP and female patients who received IABP (all P less then 0.05). After multiple regression analysis and adjustment of confounding factors, it was found that IABP implantation can significantly reduce the 28-day mortality risk in male patients (HR=0.44, 95%CI 0.25-0.77 P=0.004). While it had no inpact on the death risk in female patients(P= 0.401). The impact of IABP implantation in patients of different genders was significantly different (Z=-2.32, P=0.020). The percentage of biventricular pacing at 6-month follow-tients were responders of CRT as assessed by 6-month efficacy and echocardiographic efficacy. The use of Stereotactic Body Radiotherapy (SBRT) is controversial in Ultra-Central lung tumors, a subset of central lung tumors characterized by proximity to critical mediastinal structures. This is of interest in oligometastatic (≤3 metastases) patients, who can yield survival benefit from local treatments. The aim of our study is to assess the determinants of efficacy and toxicity in this setting.
Clinical and dosimetric parameters were reviewed in a cohort of oligometastatic patients treated with SBRT for ultra-central tumors. Local control rate (LC) and toxicity were assessed. Statistical Analysis was carried out to assess the impact of those predictors on local recurrence and adverse events.
One-hundred-nine consecutive patients were included. A median Biologic Effective Dose (BED) of 105 (75-132) Gy10 was prescribed. At a median follow-up of 17 (range 3-78) months, 2-year LC was 87%. To determine the effect of Cystus® tea (Naturprodukte Dr. Pandalis GmbH & Co. KG) as mouthwash compared to sage tea on oral mucositis in patients undergoing radio(chemo)therapy for head and neck cancer.
In this randomized, prospective phaseIII study, 60head and neck cancer patients with primary or postoperative radio(chemo)therapy were included between 04/2012 and 06/2014. They received either sage or Cystus® tea for daily mouthwash under therapy. Mucositis was scored twice aweek following the Radiation Therapy Oncology Group and the European Organization for Research and Treatment Cancer (RTOG/EORTC) scoring system. Dental parameters were also recorded. Statistical evaluation of the primary endpoint was performed using t‑test and log rank test.
Data from 57patients could be evaluated. Patient characteristics showed no significant difference between the two groups (n = 27sage; n = 30Cystus®). A total of 55patients received the prescribed dose (60-66 Gy postoperative; 70-76.8 Gy primary). Mucositis grade3 was observed in 23patients (n = 11sage; n = 12Cystus®) and occurred between day16 and 50 after start of therapy. There was no significant difference between the two groups in latency (p = 0.75) and frequency (p = 0.85) of the occurrence of mucositis grade3. The self-assessment of the oral mucosa and the tolerability of the tea also showed no significant differences. Occurrence of dental pathologies appeared to increase over time after radiotherapy.

The tibial tuberosity-trochlear groove (TTTG) distance varies with the position of the knee in the MR or CT scanner. We present and assess a simple method for adjustment of adduction or abduction of the knee.
MRI of the knee encompassing a three-dimensional (3D) sagittal sequence including ≥ 8cm of the proximal tibia was analyzed (29 females, 17 males; median age 45years). Using 3D visualization software, the central longitudinal axis of the proximal tibia (TA) was constructed, and the TTTG distance was measured before and after alignment of the TA. Observer reliability was assessed with inter- and intra-class correlation coefficient (ICC) and Bland-Altman plots.

The aim of our study is to assess the determinants of efficacy and toxicity in this setting. Clinical and dosimetric parameters were reviewed in a cohort of oligometastatic patients treated with SBRT for ultra-central tumors. Local control rate (LC) and toxicity were assessed. Statistical Analysis was carried out to assess the impact of those predictors on local recurrence and adverse events. One-hundred-nine consecutive patients were included. A median Biologic Effective Dose (BED) of 105 (75-132) Gy10 was prescribed. At a median follow-up of 17 (range 3-78) months, 2-year LC was 87%. We present and assess a simple method for adjustment of adduction or abduction of the knee. MRI of the knee encompassing a three-dimensional (3D) sagittal sequence including ≥ 8cm of the proximal tibia was analyzed (29 females, 17 males; median age 45years). Using 3D visualization software, the central longitudinal axis of the proximal tibia (TA) was constructed, and the TTTG distance was measured before and after alignment of the TA. Observer reliability was assessed with inter- and intra-class correlation coefficient (ICC) and Bland-Altman plots. Adduction of the knee occurred in 26 examinations, mean 2.7° (range 0.0° to 9.4°), and abduction in 20 examinations, mean 2.6° (range 0.0° to 7.2°). The SARS-CoV-2 infection has widely spread to become the greatest public health challenge to date, the COVID-19 pandemic. Different fatality rates among countries are probably due to non-standardized records being carried out by local health authorities. The Spanish case-fatality rate is 11.22%, far higher than those reported in Asia or by other European countries. A multicentre retrospective study of demographic, clinical, laboratory and immunological features of 584 Spanish COVID-19 hospitalized patients and their outcomes was performed. The use of renin-angiotensin system blockers was also analysed as a risk factor.
In this study, 27.4% of cases presented a mild course, 42.1% a moderate one and for 30.5% of cases, the course was severe. Ages ranged from 18 to 98 (average 63). Almost 60 % (59.8%) of patients were male. Interleukin 6 was higher as severity increased. On the other hand, CD8 lymphocyte count was significantly lower as severity grew and subpopulations CD4, CD8, CD19, and NK showed concordangards to COVID-19 severity in our series. The objective of the present study was to investigate the effect of shikonin on proinflammatory cytokines and chemokine in patients with AD. Ten patients with AD who were allergic to house dust mite (HDM) and seven healthy controls were recruited in this study. Peripheral blood mononuclear cells were isolated, and CD14+ cells were further selected and differentiated to dendritic cells. Dendritic cells stimulated using Der p 2, the major HDM allergen, were cotreated with shikonin for 24 hours, and dexamethasone was used as a control. Culture supernatants were collected, and proinflammatory cytokine and chemokine concentrations were analyzed using a multiplex assay system. Shikonin significantly inhibited Der p 2-induced expression of interleukin (IL)-6, IL-9, and IL-17A; monocyte chemoattractant protein (MCP)-1; macrophage inflammatory protein (MIP)-1α; MIP-1β; and Chemokine (C-C motif) ligand 5 (RANTES). The inhibitory effects of shikonin on IL-9, MIP-1β, and RANTES expression were stronger than those of dexamethasone. Therefore, Shikonin can be considered a promising drug for AD treatment because it inhibits different inflammatory cytokines expression.

A comprehensive literature search of randomized controlled trials using QDD to treat hypertension was conducted in 7 electronic databases, including Chinese databases. Subjects and abstracts of the trials were read in NoteExpress for preliminary screening, and the full text was read for further screening. The data extraction table was made for the selected 19 trials, and risk of bias was assessed by using the Cochrane collaboration tool, followed by data analysis using Rev Man 5.3.
The antihypertensive efficacy of QDD is 1.45 times that of antihypertensive drugs and 1.56 times that of conventional therapies, which can also reduce the endothelin level. QPAD exhibits an antihypertensive effect, and its clinical efficacy is 1.34 times and 1.61 times that of antihypertensive drugs, which can not only significantly lower the diastolic blood pressure but also reduce the 24 h mean ambulate blood pressure.

Detroit Research on Cancer Survivors data were utilized from survivors who completed their baseline survey within 18 months of cancer diagnosis (n = 1145); 18% (n = 356) reported smoking at the time of cancer diagnosis, and 57% of these (n = 203) continued to smoke after their diagnosis. Logistic regression models were used to assess factors associated with continued smoking. Living with a smoker (odds ratio [OR] = 2.78, 95% confidence interval [CI] 1.64, 4.70), higher cumulative years of smoking (OR = 1.03, 95% CI 1.01, 1.05, for each year), and a prostate cancer diagnosis (OR = 7.35, 95% CI 3.89, 13.89) were all associated with increased odds of continued smoking. Survivors with higher social well-being scores (measured by the Functional Assessment of Cancer Therapy, a quality of life assessment) were more likely to quit smoking after diagnosis (OR = 0.96, 95% CI 0.93, 1.00). These findings highlight the continued need for personalized cessation strategies to be incorporated into treatment plans for cancer survivors.
As many as one-quarter of all residents in nursing facilities have cardiopulmonary resuscitation (CPR) as a documented choice in the medical record, despite the likelihood of limited medical benefit in this setting.
The aim of this study was to understand the perspectives of healthcare providers and nursing facility residents regarding CPR decisions.
We used qualitative interviews to examine the perspectives of residents with a documented decision for CPR in the medical record. We then compared residents' views with those of healthcare providers who routinely conduct advance care planning (ACP) conversations in the nursing facility setting.
Five themes emerged from the interviews (a) Resident versus Provider Concerns, (b) Offering Information versus Avoidance, (c) Lack of Understanding of CPR, (d) Lack of Awareness, and (e) 'Don't Keep Me on Machines'. Residents held misconceptions about CPR and/or exhibited an overall poor understanding of the relationship between their own health status and the likeACP discussions.
ACP communication models and training should be designed not only to explore nursing facility residents' goals, values, and preferences, but also to elicit any underlying differences in perceptions that may affect communication. Healthcare providers can identifying the primary concerns of residents and assist them with integrating or reframing these issues as a part of ACP discussions.This study examines the longer term relationship between public health insurance expansions and health behaviors. I leverage geographic and temporal variation in the implementation of the Affordable Care Act-facilitated Medicaid expansions and provide the first estimates of the expansions' behavioral impacts during their first 5 years. Using national survey data from the 2010 to 2018 Behavioral Risk Factors Surveillance System and a difference-in-differences regression design, I show that the Medicaid expansions increase utilization of certain forms of preventive care, while reducing heavy drinking. I also find suggestive evidence that the expansions reduce smoking and increase the probability of exercise. These results stand in contrast with earlier studies that used only 2 or 3 years of postexpansion data and found no detectable effect of the Medicaid expansions on health behaviors in the short run. My results, combined with evidence from previous studies, suggest that public insurance expansions may not prompt an immediate change in health behaviors, but newly eligible populations do increase investments in healthy behaviors over time. In the long run, Medicaid expansions may help reduce engagement in risky behaviors like drinking and smoking among low-income people.Inflammatory bowel disease (IBD) is a heterogeneic disease with a variety of treatments targeting different mechanisms. A multistate, mechanistic, mathematical model of IBD was developed in part 1 of this two-part article series. In this paper, application of the model to predict response of key clinical biomarkers following different treatment options for Crohn's disease was explored. Five therapies, representing four different mechanisms of action, were simulated in the model and longitudinal profiles of key clinical markers, C-reactive protein and fecal calprotectin were compared with clinical observations. Model simulations provided an accurate match with both central tendency and variability observed in biomarker profiles. We also applied the model to predict biomarker and clinical response in an experimental, combination therapy of existing therapeutic options and provide possible mechanistic basis for the increased response. Survivors with higher social well-being scores (measured by the Functional Assessment of Cancer Therapy, a quality of life assessment) were more likely to quit smoking after diagnosis (OR = 0.96, 95% CI 0.93, 1.00). These findings highlight the continued need for personalized cessation strategies to be incorporated into treatment plans for cancer survivors. As many as one-quarter of all residents in nursing facilities have cardiopulmonary resuscitation (CPR) as a documented choice in the medical record, despite the likelihood of limited medical benefit in this setting. The aim of this study was to understand the perspectives of healthcare providers and nursing facility residents regarding CPR decisions. We used qualitative interviews to examine the perspectives of residents with a documented decision for CPR in the medical record. We then compared residents' views with those of healthcare providers who routinely conduct advance care planning (ACP) conversations in the nursing facility setting. Five themes emerged from the interviews (a) Resident versus Provider Concerns, (b) Offering Information versus Avoidance, (c) Lack of Understanding of CPR, (d) Lack of Awareness, and (e) 'Don't Keep Me on Machines'. Residents held misconceptions about CPR and/or exhibited an overall poor understanding of the relationship between their own health status and the likeACP discussions. ACP communication models and training should be designed not only to explore nursing facility residents' goals, values, and preferences, but also to elicit any underlying differences in perceptions that may affect communication. Healthcare providers can identifying the primary concerns of residents and assist them with integrating or reframing these issues as a part of ACP discussions.This study examines the longer term relationship between public health insurance expansions and health behaviors. I leverage geographic and temporal variation in the implementation of the Affordable Care Act-facilitated Medicaid expansions and provide the first estimates of the expansions' behavioral impacts during their first 5 years. Using national survey data from the 2010 to 2018 Behavioral Risk Factors Surveillance System and a difference-in-differences regression design, I show that the Medicaid expansions increase utilization of certain forms of preventive care, while reducing heavy drinking. I also find suggestive evidence that the expansions reduce smoking and increase the probability of exercise. These results stand in contrast with earlier studies that used only 2 or 3 years of postexpansion data and found no detectable effect of the Medicaid expansions on health behaviors in the short run. My results, combined with evidence from previous studies, suggest that public insurance expansions may not prompt an immediate change in health behaviors, but newly eligible populations do increase investments in healthy behaviors over time. In the long run, Medicaid expansions may help reduce engagement in risky behaviors like drinking and smoking among low-income people.Inflammatory bowel disease (IBD) is a heterogeneic disease with a variety of treatments targeting different mechanisms. A multistate, mechanistic, mathematical model of IBD was developed in part 1 of this two-part article series. In this paper, application of the model to predict response of key clinical biomarkers following different treatment options for Crohn's disease was explored. Five therapies, representing four different mechanisms of action, were simulated in the model and longitudinal profiles of key clinical markers, C-reactive protein and fecal calprotectin were compared with clinical observations. Model simulations provided an accurate match with both central tendency and variability observed in biomarker profiles. We also applied the model to predict biomarker and clinical response in an experimental, combination therapy of existing therapeutic options and provide possible mechanistic basis for the increased response. Patients diagnosed with IgAN (n = 194) were divided into three groups representing undetectable (Group 1), below-median (Group 2) and above-median (Group 3) levels of ucMet/creatinine (ucMet/Cr). Many kidney transplant recipients lack the knowledge, abilities, and support they need for self-care. On the other hand, most kidney transplant centers do not have a well-planned and specific training program for them, and educational interventions for kidney transplant recipients have not been adequately effective. The tools used in this study including demographic information, Schering's emotional intelligence questionnaire (α = 0.75-0.85), the 36-item Short Form Health Survey Questionnaire (α = 0.75), and body image (α = 0.77-0.91) were used to collect data. The aim of this study was to prioritize and develop a model to illustrate the relationship between faculty recruitment challenges in medical sciences universities.
Interpretive structural modeling (ISM) is a system design method initially introduced by Warfield (1974). This method helps create order in the complex interconnections between components of a system by interpreting the opinions of a group of experts. It both determines the priority of elements influencing one another and uncovers the association between the elements of a multipart set in a hierarchical structure.
In this method, the identified challenges were built into a paired comparison questionnaire to be completed by policymakers and experts. By the same token, the obtained results were analyzed with the ISM technique.
The four steps include identified variables related to the issue, structural self-interaction matrix, initial reachability matrix, and final reachability matrix was used for analysis. According to these steps, the ISM model was portrayed.
The ISM model was developed in ten levels that divided into three parts including key challenges, strategic challenges, and dependent challenges.

This quasi-experimental, two-group study (standardized patient-based training and feedback lecture) was performed on 67 midwives (licensed by the office) in Mashhad in 2018. Midwives' knowledge and practice before and 2 weeks after training (a 4-h training program) were assessed by the Objective Structured Clinical Examination and a questionnaire. The enzyme β-carotene oxygenase 2 (BCO2) converts carotenoids into more polar metabolites. Studies in mammals, fish, and birds revealed that BCO2 controls carotenoid homeostasis and is involved in the pathway for vitamin A production. However, it is controversial whether BCO2 function is conserved in humans, because of a 4-amino acid long insertion caused by a splice acceptor site polymorphism. We here show that human BCO2 splice variants, BCO2a and BCO2b, are expressed as pre-proteins with mitochondrial targeting sequence (MTS). The MTS of BCO2a directed a green fluorescent reporter protein to the mitochondria when expressed in ARPE-19 cells. Removal of the MTS increased solubility of BCO2a when expressed in Escherichia coli and rendered the recombinant protein enzymatically active. The expression of the enzymatically active recombinant human BCO2a was further improved by codon optimization and its fusion with maltose-binding protein. Introduction of the 4-amino acid insertion into mouse Bco2 did not impede the chimeric enzyme's catalytic proficiency. We further showed that the chimeric BCO2 displayed broad substrate specificity and converted carotenoids into two ionones and a central C14-apocarotendial by oxidative cleavage reactions at C9,C10 and C9',C10'. Thus, our study demonstrates that human BCO2 is a catalytically competent enzyme. In this study we present the 3D structures of the soluble catalytic domain of CmsA/Ktr4, as determined by X-ray crystallography at a resolution of 1.95 Å, as well as the enzyme and Mn2+/GDP complex to 1.90 Å resolution. The CmsA/Ktr4 protein not only contains a highly conserved binding pocket for the donor substrate, GDP-mannose, but also has a unique broad cleft structure formed by its N- and C-terminal regions and is expected to recognize the acceptor substrate, a mannan chain. Based on these crystal structures, we also present a 3D structural model of the enzyme-substrate complex generated using docking and molecular dynamics simulations with α-Man-(1→6)-α-Man-(1→2)-α-Man-OMe as the model structure for the acceptor substrate. This predicted enzyme-substrate complex structure is also supported by findings from single amino acid substitution CmsA/Ktr4 mutants expressed in ΔcmsA/ktr4 A. fumigatus cells. Based on previous X-ray crystal structures, we mutated three conserved residues in the Nef dimer interface (Leu112, Tyr115, and Phe121) and demonstrated attenuated homodimer formation in a cell-based fluorescence complementation assay. Point mutations at each position reduced the infectivity of HIV-1 produced from transfected 293T cells, the Jurkat TAg T-cell line, and donor mononuclear cells in a SERINC5-dependent manner. In SERINC5-transfected 293T cells, virion incorporation of SERINC5 was increased by dimerization-defective Nef mutants, whereas down-regulation of SERINC5 from the membrane of transfected Jurkat cells by these mutants was significantly reduced. A BRAFdel was detected in 7/18 (39%), 6/18 (33%) and 3/18 (17%) LCH lesions using WES, pyrosequencing and Sanger, respectively, with good concordance between the WES and pyrosequencing results (Kappa-coefficient=0.88).
