    This study aims to evaluate the efficacy of 2 versions of a self-guided, smartphone-based meditation app-the Healthy Minds Program (HMP)-which includes training in mindfulness (Awareness), along with practices designed to cultivate positive relationships (Connection) or insight into the nature of the self (Insight).

    A three-arm, fully remote RCT compared 8 weeks of one of 2 HMP conditions (Awareness+Connection and Awareness+Insight) with a waitlist control. Adults (≥18 years) without extensive previous meditation experience were eligible. The primary outcome was psychological distress (depression, anxiety, and stress). Secondary outcomes were social connection, empathy, compassion, self-reflection, insight, rumination, defusion, and mindfulness. Musion, and empathy) of changes in distress were identified.

    In recent years, both suicide and overdose rates have been increasing. Many individuals who struggle with opioid use disorder are prone to suicidal ideation; this may often result in overdose. However, these fatal overdoses are difficult to classify as intentional or unintentional. Intentional overdose is difficult to detect, partially due to the lack of predictors and social stigmas that push individuals away from seeking help. These individuals may instead use web-based means to articulate their concerns.

    This study aimed to extract posts of suicidality among opioid users on Reddit using machine learning methods. The performance of the models is derivative of the data purity, and the results will help us to better understand the rationale of these users, providing new insights into individuals who are part of the opioid epidemic.

    Reddit posts between June 2017 and June 2018 were collected from r/suicidewatch, r/depression, a set of opioid-related subreddits, and a control subreddit set.   • Western medicine developed as an expression of Christian charity and played a large role in the growth of the early church. Despite its original foundation in Christian moral principles, modern medicine has deviated from its origins. The principles of human dignity, solidarity, and subsidiarity have been subjugated to a materialist and transactional construct that forms the basis of the contemporary medical delivery and financing systems. The dehumanization of both healthcare practitioners and patients by the partnership of governmental and corporate entities, and the use of health care as a political instrument, has debased the original mission of the medical profession and represents an affront to the principles of Catholic social teaching (CST). This essay explores the ways in which the US medical delivery and financing systems violate the principles of CST by means seldom recognized due to the inurement of the public and medical professionals. By examining the prevailing healthcare model through the lens of CST, the author illustrates the ways in which CST principles are systematically violated. This analysis serves as the foundation of a Catholic response to the question of how faithful Christians might live out their calls to holiness through the exercise of their professional vocations. A vision of an invigorated model of medicine as vocation, along with illustrative examples, is presented. By exemplifying the principles of human dignity, solidarity and subsidiarity in health care, Christians can seize a golden opportunity for evangelization by rearticulating the historical spiritual mission of Western medicine.Half of the medical professionals in the United States are experiencing symptoms of burnout. From the perspective of theological anthropology, this dehumanizing aspect of the field is not reducible to ethical failures, for it is rooted in the radically new worldview known as self-creation. As an implicit denial of Christian understanding of creation, self-creation entails a rejection of relationality and dependence-both proper to the Revelation of ************. This article proposes that this lost Christian patrimony is intimately connected to the increasingly unhealthy dependence we place upon modern medicine. Relying on theologian Joseph Ratzinger, we will come to see that a recovery of relational dependence is not only necessary for the salvation of man-but the very health of the medical world at large.Although Christian ethics and contemporary utilitarian ethics both employ terms such as "love" and "compassion" in their efforts to deal with human suffering, they are in fact polar opposite ethical views. This fact is not at all easy to discern. One key to perceiving the radical opposition between them lies in clarifying their respective concepts of love and suffering and the relation between the two. In Christian personalism, suffering is always understood as the suffering of individual persons, while in utilitarianism, suffering is primarily understood as a quantifiable entity detached from the individuals who experience it. This detachment of suffering from individuals leads to the depersonalizing and commodifying recommendations of utilitarianism. The dignity of persons as understood in Christian anthropology serves as the foundation of Christian ethics and is the only basis on which ethics can avoid commodifying people. The article begins with an explanation of the utilitarian approach to suffering and its concept of love. It then proceeds to express the view of love and suffering that flows from the Christian perspective. The article concludes by exposing the inherently self-defeating structure of utilitarian ethics and offers the hope-filled, if challenging, approach of Christian personalism. Although Christian anthropology and ethics developed within the historical context of Christianity, and in fact could only have developed there, the arguments here are primarily philosophical elucidations of the differences between the two opposing schools of thought discussed, while here and there including occasional theological points.

These arguments seem to fall into two major categories. Utilitarianism suggests that suspension of civil liberties and constitutional rights is a necessary response, while Libertarianism supports individual decision-making and greatly reduced government mandates. Protesters around the country have been vocal about one or the other points of view. First, we consider in detail the potential harms of severe acute respiratory syndrome virus-2 (SARS-CoV-2) if left unchecked by NPIs. Second, we look at harms due to restricted social and economic activity on human morbidity and mortality. Finally, we offer a framework based on the four pillars of Catholic Social Teaching and the principle of double effect that offers a more humane solution than Utilitarian or Libertarian principles alone.Burnout is highly prevalent among physicians and is associated with negative patient outcomes. Furthermore, medical training is a particularly vulnerable time as studies show that medical students, residents, and fellows experience burnout and emotional exhaustion at higher rates than both the general population and physicians in practice. Can Catholic academic physicians transfer these protective benefits of religion and spirituality to their trainees, who are at the highest risk, and who may or may not share their faith? An ancient Catholic tradition, the Seven Spiritual Works of Mercy, may hold the key. The Spiritual Works of Mercy are listed by the US Conference of Catholic Bishops as Counseling the Doubtful, Instructing the Ignorant, Admonishing the Sinner, Comforting the Sorrowful, Forgiving Injuries, Bearing Wrongs Patiently, and Praying for the Living and the Dead. Using this as a framework, examples of evidenced-based actions from the literature that have been shown to either prevent burnout or to improve the day-to-day experience of medical trainees were discussed.

    Coronavirus disease 2019 is a pandemic disease, requiring persons around the world to take immediate action to reduce the risk of infection. This study was aimed to summarize the patterns and determinants of coronavirus disease 2019 knowledge, attitude, and practice among general populations and health workers.

    A cross-sectional study from PubMed, HINARI, and Scopus were searched from March 16 to July 30, 2020. The review was done in line with the Preferred Reporting Items for Systematic Reviews and Meta-analyses-2009.

    We found 56 articles upon the initial search. Finally, 21 studies were filtered to be studied in this systematic review. Overall, the majority of the articles that were previously published had good knowledge about coronavirus disease 2019 that lies in the ranges from 40% to 99.5%. A good attitude lies in the ranges from 70% to 97.1%. Among impact of coronavirus disease 2019 on mental health, only anxiety was reported that ranges from 24.6% to 96.3%. A cross-sectional study from PubMed, HINARI, and Scopus were searched from March 16 to July 30, 2020. The review was done in line with the Preferred Reporting Items for Systematic Reviews and Meta-analyses-2009. We found 56 articles upon the initial search. The trial included 182 adults with diabetes-related foot ulcers (94/88 in the telemedicine/standard care groups) in 42 municipalities/districts, recruited from three clinical sites in Wesanalyses, differences between treatment groups were small and non-significant for the health and well-being scale scores, as well as for diabetes-related distress and foot ulcer-specific quality of life.

    There were no significant differences in changes in scores for the patient reported outcomes between the intervention and control group, indicating that the intervention did not affect the participants' health, well-being and quality of life.

    To evaluate the accuracy of a computer numerical control (CNC) milled surgical guide for implant placement in edentulous jaws.

    Edentulous patients seeking implants treatment were recruited in this prospective cohort study. Radiographic guides with diagnostic templates were fabricated from wax-up dentures. Patients took cone-beam computed tomography (CBCT) wearing the radiopaque radiographic guides. Implant positions were virtually designed in the planning software based on the CBCT data, and the radiographic templates were converted into surgical guides using CNC milling technique. Forty-four implants were placed into 12 edentulous jaws following guided implant surgery protocol. Post-surgery CBCT scans were made for each jaw, and the deviations between the planned and actual implant positions were measured. Deviation of implant position was compared between maxilla and mandible, and between cases with and without anchor pins using independent t-test.

    Nine patients (3 males and 6 females) with 12 edentulous jaws were recruited. The mean age of patients was 59.2 ± 13.9years old. All 44 implants was placed without complication and survived, the mean three dimensional linear deviation of implant position between virtual planning and actual placement was 1.53 ± 0.48mm at the implant neck and 1.58 ± 0.49mm at the apex. The angular deviation was 3.96 ± 3.05 degrees. No significant difference was found in the deviation of implant position between maxilla and mandible (P = 0.28 at neck, 0.08 at apex), nor between cases with and without anchor pins (P = 0.87 at neck, 0.06 at apex).

    The guides fabricated using the CNC milling technique provided comparable accuracy as those fabricated by Stereolithography. The displacement of the guides on edentulous arch might be the main contributing factor of deviation.

    Loss of the genomic stability jeopardize genome stability and promote malignancies. A fraction of ovarian cancer (OvCa) arises from pathological mutations of DNA repair genes that result in highly mutagenic genomes. However, it remains elusive why the ovarian epithelial cells are particularly susceptible to the malfunction of genome surveillance system.

    To explore the genotoxic responses in the unique context of microenvironment for ovarian epithelium that is periodically exposed to high-level steroid hormones, we examined estrogen-induced DNA damage by immunofluorescence in OvCa cell lines, animal and human samples.

    We found that OvCa cells are burdened with high levels of endogenous DNA damage that is not correlated with genomic replication. The elevation of damage burden is attributable to the excessive concentration of bioactive estrogen instead of its chemomimetic derivative (tamoxifen). Induction of DNA lesions by estrogen is dependent on the expression of hormone receptors, and occurs in G1 and non-G1 phases of cell cycle. Moreover, depletion of homologous recombination (HR) genes (BRCA1 and BRCA2) exacerbated the genotoxicity of estrogen, highlighting the role of HR to counteract hormone-induced genome instability. Finally, the estrogen-induced DNA damage was reproduced in the epithelial compartments of both ovarian and fallopian tubes.

    Taken together, our study disclose that estrogen-induced genotoxicity and HR deficiency perturb the genome stability of ovarian and fallopian epithelial cells, representing microenvironmental and genetic risk factors, respectively.

We see techniques for measuring, understanding and overcoming the domain shift as a crucial step towards reliable use of deep learning in the future clinical pathology applications.The problem of effectively exploiting the information multiple data sources has become a relevant but challenging research topic in remote sensing. In this article, we propose a new approach to exploit the complementarity of two data sources hyperspectral images (HSIs) and light detection and ranging (LiDAR) data. Specifically, we develop a new dual-channel spatial, spectral and multiscale attention convolutional long short-term memory neural network (called dual-channel A³CLNN) for feature extraction and classification of multisource remote sensing data. Spatial, spectral, and multiscale attention mechanisms are first designed for HSI and LiDAR data in order to learn spectral- and spatial-enhanced feature representations and to represent multiscale information for different classes. In the designed fusion network, a novel composite attention learning mechanism (combined with a three-level fusion strategy) is used to fully integrate the features in these two data sources. Finally, inspired by the idea of transfer learning, a novel stepwise training strategy is designed to yield a final classification result. Our experimental results, conducted on several multisource remote sensing data sets, demonstrate that the newly proposed dual-channel A³CLNN exhibits better feature representation ability (leading to more competitive classification performance) than other state-of-the-art methods.This article considers iterative learning control (ILC) for a class of discrete-time systems with full learnability and unknown system dynamics. First, we give a framework to analyze the learnability of the control system and build the relationship between the learnability of the control system and the input-output coupling matrix (IOCM). The control system has full learnability if and only if the IOCM is full-row rank and the control system has no learnability almost everywhere if and only if the rank of the IOCM is less than the dimension of system output. Second, by using the repetitiveness of the control system, some data-based learning schemes are developed. It is shown that we can obtain all the needed information on system dynamics through the developed learning schemes if the control system is controllable. Third, by the dynamic characteristics of system outputs of the ILC system along the iteration direction, we show how to use the available information of system dynamics to design the iterative learning gain matrix and the current state feedback gain matrix. And we strictly prove that the iterative learning scheme with the current state feedback mechanism can guarantee the monotone convergence of the ILC process if the IOCM is full-row rank. Finally, a numerical example is provided to validate the effectiveness of the proposed iterative learning scheme with the current state feedback mechanism.Active learning (AL) aims to maximize the learning performance of the current hypothesis by drawing as few labels as possible from an input distribution. Generally, most existing AL algorithms prune the hypothesis set via querying labels of unlabeled samples and could be deemed as a hypothesis-pruning strategy. However, this process critically depends on the initial hypothesis and its subsequent updates. This article presents a distribution-shattering strategy without an estimation of hypotheses by shattering the number density of the input distribution. For any hypothesis class, we halve the number density of an input distribution to obtain a shattered distribution, which characterizes any hypothesis with a lower bound on VC dimension. Our analysis shows that sampling in a shattered distribution reduces label complexity and error disagreement. With this paradigm guarantee, in an input distribution, a Shattered Distribution-based AL (SDAL) algorithm is derived to continuously split the shattered distribution into a number of representative samples. An empirical evaluation of benchmark data sets further verifies the effectiveness of the halving and querying abilities of SDAL in real-world AL tasks with limited labels. Experiments on active querying with adversarial examples and noisy labels further verify our theoretical insights on the performance disagreement of the hypothesis-pruning and distribution-shattering strategies. Finally, a numerical example is provided to validate the effectiveness of the proposed iterative learning scheme with the current state feedback mechanism.Active learning (AL) aims to maximize the learning performance of the current hypothesis by drawing as few labels as possible from an input distribution. Generally, most existing AL algorithms prune the hypothesis set via querying labels of unlabeled samples and could be deemed as a hypothesis-pruning strategy. However, this process critically depends on the initial hypothesis and its subsequent updates. This article presents a distribution-shattering strategy without an estimation of hypotheses by shattering the number density of the input distribution. For any hypothesis class, we halve the number density of an input distribution to obtain a shattered distribution, which characterizes any hypothesis with a lower bound on VC dimension. Our analysis shows that sampling in a shattered distribution reduces label complexity and error disagreement. With this paradigm guarantee, in an input distribution, a Shattered Distribution-based AL (SDAL) algorithm is derived to continuously split the shattered distribution into a number of representative samples. An empirical evaluation of benchmark data sets further verifies the effectiveness of the halving and querying abilities of SDAL in real-world AL tasks with limited labels. Experiments on active querying with adversarial examples and noisy labels further verify our theoretical insights on the performance disagreement of the hypothesis-pruning and distribution-shattering strategies.     To develop and validate a patient-reported outcome (PRO) measure of quality of life (QoL), specific to patients with Neurofibromatosis Type 1 (NF1)-associated plexiform neurofibromas (pNFs), suitable for use in clinical efficacy trials. The study was conducted in parallel in the UK and US.

    Qualitative interviews were conducted with patients to generate questionnaire content. Face and content validity of the draft scale was assessed by cognitive debriefing interviews (CDIs). A postal validation survey was conducted to identify the final version of the questionnaire (the PlexiQoL), establish its unidimensionality, and assess its psychometric properties.

    Thematic analysis was performed on 42 interview transcripts. Thirty-one CDIs revealed that patients found the draft scale to be comprehensible, relevant, and easy to complete. The postal validation survey was completed by 273 patients. Rasch analysis identified an 18-item unidimensional scale that showed excellent internal consistency, reproducibility, and sensitivity to differences in patient-perceived pNF severity, general health, and the use of pain medication.

    Workplace violence perpetrated by patients and their families towards nurses has become a global problem.

    The present study explores associations between individuals' having witnessed violent incidents in the past and holding attitudes justifying violence in the present, and their intention to behave violently in a nurse-patient interaction at a healthcare facility.

    A cross-sectional study sampled 1,350 participants from among the general public in Israel.

    A self-administered questionnaire measured attitudes regarding violence towards nurses and confronted the participants with two vignettes eliciting verbal and physical violence towards nurses. Multiple logistic regression was conducted to explore the association between attitudes and intention to act violently. A mediation analysis (using the PROCESS macro) was conducted to explore the mediation factors.

    Over half of the participants witnessed an incident of verbal violence in health care and substantially fewer witnessed a physically violent evenor violence in order to minimize such events. In order to prevent violence towards healthcare staff, a holistic multisystem approach should be implemented, involving a focus by sociocultural elements on social values and structures, as violence in healthcare reflects violence in society at large.
    Nurses should be encouraged to report incidents of violence, while healthcare and judicial systems must address this issue seriously. It is recommended that systems alerting for risk indicators be applied, to identify patients with a potential for violence in healthcare facilities. Healthcare policymakers and workers must act to promote an environment of zero tolerance for violence in order to minimize such events. Serum potassium, sodium, chloride, glucose, ALP, and ALT levels were assessed every 2weeks. After a 2-week washout period, each cat was given the opposite topical preparation, and the study process was repeated.

    Hybrid approach to pulmonary valve replacement (PVR) in the paediatric population has been reported, although data in infants and small children are limited. Several strategies are now possible. The aim of this study is to review our hybrid PVR strategy in a complex patient cohort, outlining a variety of approaches employed in our centre.

    We performed a retrospective review of infants and small children who underwent hybrid PVR between May 2017 and April 2019 in a single tertiary cardiology centre. Medical records were reviewed to ascertain demographic, clinical and outcome data.

    Ten patients with a median (interquartile range) age of 1.5 years (1.1-1.9) and weight of 8.8 kg (8-10.6) were managed with hybrid pulmonary valve insertion. Eight patients had perventricular approach (4 sternotomy and 4 subxiphoid) and 2 patients had surgically sutured valve. However, the survival curve of these patients overlapped with that of some patients in the N2 categories. Our study focused on the prognostic significance of different subclassifications for N1 patients.

Several aspects related to the design and development of PEHs and TEHs as the energy harvesting devices were investigated, covering the following topics construction and mechanism of the energy harvesters; electrical characteristics of the fabricated piezoelectric and triboelectric materials; and characterization of the energy harvesters. At a 4 mm gap width between the cover plate and the stopper (the mechanical actuation components of both energy harvesters) and a cover plate pressing frequency of 2 Hz, PEH generated 27.64 mW, 1.90 mA, and 14.39 V across an optimal resistive load of 7.50 kΩ, while TEH generated 1.52 mW, 8.54 µA, and 177.91 V across an optimal resistive load of 21 MΩ. Understanding how ligands bind to GPCRs and which conformational changes they induce is a fundamental step toward the development of more efficient and specific drugs. Moreover, it is emerging that the dynamic of the ligand-receptor interaction contributes to the specificity of both ligand recognition and effector recruitment, an aspect that is missing in structural snapshots from crystallography. We describe here biochemical and biophysical techniques to address ligand-receptor interactions in their structural and dynamic aspects, which include mutagenesis, crosslinking, spectroscopic techniques, and mass-spectrometry profiling. With a main focus on peptide receptors, we present methods to unveil the ligand-receptor contact interface and methods that address conformational changes both in the ligand and the GPCR. The presented studies highlight a wide structural heterogeneity among peptide receptors, reveal distinct structural changes occurring during ligand binding and a surprisingly high dynamics of the ligand-GPCR complexes.Improving health and safety at work has been an important issue for the European Union since the 1980s. The existing literature supports that shift work is associated with multiple indicators of poor health but frequently neglects the potential impact of occupational hazards. This study aims at describing and comparing the exposure to different workplace hazards among shift and other workers before and after 1980. Exposure to different workplace hazards (noise, dust, pollutant, and other physical stressors) were analyzed among 119,413 participants from the UK Biobank cohort. After stratifying the analyses before and after 1980, exposure was compared between shift and other workers. Potential confounding variables (sex, age, ethnicity, education level, occupational category, and neuroticism) were adjusted for in the log-binomial regression. Shift workers had a higher prevalence ratio (PR) than other workers of being exposed to almost all identified hazards both before or after 1980. They were also more likely to be exposed to multiple hazards compared to other workers, both before 1980 (PR 1.25; 95% CI 1.21-1.30) and after 1980 (PR 1.34; 95% CI 1.30-1.38). The prevalence of all measured risk factors was higher after 1980 than before 1980 among shift workers. Of note, the work environment has improved overall for other workers. Our findings suggest that changes at the workplace have benefited other workers more than shift workers as they are still more exposed to all occupational hazards.
