A wide variation of flow parameters was noted in meningitis irrespective of the degree of ventricular dilatation. A significant difference in peak velocity and stroke volume was noted in Group II as compared to controls. Viral meningitis showed milder alteration of CSF flow dynamics as compared to bacterial and tuberculous etiologies. At a cutoff value of 3.57 cm/s in peak CSF velocity, the specificity was 100% and sensitivity was 22.7% to differentiate between viral and non-viral meningitis. Conclusion Alteration of CSF flow dynamics on PCMRI can improve segregation of patients into viral and non- viral etiologies, especially in those in whom contrast is contraindicated or not recommended. The possibility of using technology to activate and manage diagnostic (preliminary diagnosis) and intervention processes should be explored. Two developed telemedicine working models for diagnosis and intervention, including synchronous and asynchronous transmissions, are presented. They are proposals not yet supported by the data. The diagnosis step is composed by two different and consecutives phases (A) pre-specialistic consultation (PSC) and (B) specialistic assessment. The intervention step implemented well-recognized evidence-based models for preschoolers, school-aged, and older children in an online format. Parents' support is also included. The described working models have the purpose of carrying out preliminary specialistic answers to the families without aiming to replace preferable in-person assessment. Based on previous research findings, the telemedicine approach is accepted by parents, increases their sense of competence, increases the parent intervention adhesion, and improves the social communication competencies for children with ASD. In conclusion, the presented working models must be considered partial responses to the current emergency status and at the same time as possible integrations into traditional approaches.The brown alga Fucus vesiculosus is common to the intertidal zones of the Baltic Sea, where it is exposed to high fouling pressures by microorganisms. Our previous studies showed, repeatedly, the consistent antimicrobial activity of F. vesiculosus crude extracts against human pathogens, while untargeted metabolomics analyses have revealed a variety of metabolites. In this study, we applied the UPLC-QToF-MS/MS-based "bioactive molecular networking" (BMN) concept on the most bioactive n-hexane and n-butanol subextracts of Baltic F. vesiculosus coupled with in silico dereplication tools to identify the compounds responsible for antimicrobial activity. The first antimicrobial cluster identified by BMN was galactolipids. Our targeted isolation efforts for this class led to the isolation of six monogalactosyldiacylglycerol (MGDG) derivatives (1-6) and one digalactosyldiacylglycerol (DGDG, 7). The MGDGs 5 and 6 and the DGDG 7 exhibited activity against Staphylococcus aureus. The second compound class with high bioactivity was phlorotannins. In particular, phlorethol-type phlorotannins showed high correlations with antimicrobial activity based on the BMN approach, and two phlorotannins (8-9) were isolated. This study shows that antimicrobial components of F. vesiculosus reside in the algal cell walls and membranes and that BMN provides a complementary tool for the targeted isolation of bioactive metabolites.Oral cancer-a type of head and neck cancer-is estimated to be the fifth most common cancer in Taiwan. These results are the first to demonstrate that JIB extract exhibits anticancer capacity and synergizes with cisplatin to treat oral cancer. Perceived susceptibility, seriousness, self-efficacy, benefits, health motivation, and perceived barriers were found to have strong associations with screening behaviors (p less then 0.05). In this study, we found that few women performed breast self-examination, had clinical breast examination and mammography.

Conclusion A 39-year-old Caucasian woman developed chronic carbonation dysgeusia after initiation of topiramate following a generalized seizure.Background It has been shown that modern life style with reduced physical activity can lead to lower bone accumulation. Also a decline trend in external skeletal robustness in children and young adolescents, measured by the Frame Index (FI), seems to have a parallel trend with the increase in overweight and obesity. Based on these findings we estimate that likely, the FI should be changed after a decade in the Argentinean population as well as in others population of the world. Thereby, the aim of the present study was to describe, using the FI, the pattern of external skeletal robustness in school children aged 6-14 years from two cohorts of Argentina (Puerto Madryn, Chubut) and to compare them with the European reference (Czech Republic, Germany, Poland and Russia). Methods Elbow breadth and height of Puerto Madryn school children were collected in two cross-sectional studies conducted between 2001-2006 (cohort 1 = C1) and 2014-2016 (cohort 2 = C2). Percentiles (P) values, for males and females, from C1 andC1 (P50 and P97), and positive for the remaining values. Finally, height registered, in both sexes, negative values in ER-C1 (except P97 in females), ER-C2 and C1-C2. Conclusions After ten years of the first study carried on in Puerto Madryn, school children show a negative trend in the external skeletal robustness. Additionally, the children of both Argentinean cohorts have lower values compared to the European reference, and mainly the actual cohort. Results 2 St. Nicolas House Analysis suggested that the growth of male sex were more sensitive to population crowding, and that crowding contributes to the greater height dimorphism in taller populations. We start from a full three-dimensional (3D) description of the tissue and derive an effective 2D model that captures, through the curvature of the apical surface, both the apical-basal asymmetry and the spatial geometry of the tissue. Crucially, variations of active stresses across the apical-basal axis lead to active torques that can drive curvature transitions. By identifying four distinct sources of activity, we find that bulk active stresses arising from actomyosin contractility and growth compete with boundary active tensions due to localized actomyosin cables and lamellipodial activity to generate the various states spanning the morphospace of a planar epithelium. Our treatment hence unifies 3D shape deformations through the coupled mechanics of apical curvature change and in-plane expansion/contraction of substrate-adhered tissues. Conclusion A 39-year-old Caucasian woman developed chronic carbonation dysgeusia after initiation of topiramate following a generalized seizure.Background It has been shown that modern life style with reduced physical activity can lead to lower bone accumulation. Also a decline trend in external skeletal robustness in children and young adolescents, measured by the Frame Index (FI), seems to have a parallel trend with the increase in overweight and obesity. Based on these findings we estimate that likely, the FI should be changed after a decade in the Argentinean population as well as in others population of the world. Thereby, the aim of the present study was to describe, using the FI, the pattern of external skeletal robustness in school children aged 6-14 years from two cohorts of Argentina (Puerto Madryn, Chubut) and to compare them with the European reference (Czech Republic, Germany, Poland and Russia). Methods Elbow breadth and height of Puerto Madryn school children were collected in two cross-sectional studies conducted between 2001-2006 (cohort 1 = C1) and 2014-2016 (cohort 2 = C2). Percentiles (P) values, for males and females, from C1 andC1 (P50 and P97), and positive for the remaining values. Finally, height registered, in both sexes, negative values in ER-C1 (except P97 in females), ER-C2 and C1-C2. The latter may pass the neurobehavioral effects of parental sevoflurane exposure to the offspring, who may be affected even at levels of anesthesia that are not harmful to the exposed parents. The large number of patients who require general anesthesia, the even larger number of their future unexposed offspring whose health may be affected, and a growing number of neurodevelopmental disorders of unknown etiology underscore the translational importance of investigating the intergenerational effects of GAs. In this mini review, we discuss emerging experimental findings on neuroendocrine, epigenetic, and intergenerational effects of GAs.Background Total joint arthroplasty is one of the most common surgeries performed in the United States with total knee arthroplasty (TKA) being one of the most successful surgeries for restoring function and diminishing pain. Even with the demonstrated success of TKA and a higher prevalence of arthritis and arthritis related disability among minorities, racial and gender disparity remains a constant issue in providing care for the adult reconstruction patient. Aim To assess the role of demographics and expectations on differences in perioperative patient reported outcomes (PRO) following TKA. Methods One hundred and thirty-three patients scheduled for primary unilateral TKA secondary to moderate to severe osteoarthritis were enrolled in this two-institution prospective study. Validated PRO questionnaires were collected at four time points. Statistical analysis was conducted to determine the impact of gender, ethnic background and expectation surveys responses to assess PRO at these time points. Results Femalee patients. No postoperative differences in outcomes between these groups.Background Surgical site infections are a major cause of morbidity and mortality following orthopedic surgery. Recent efforts to identify sources of contamination in the operating rooms have implicated mobile phones. Aim To investigate microbial colonization on the mobile phones of health care professionals in the orthopedic operating room. Methods We conducted a cross-sectional study involving culture and sensitivity analysis of swabs taken from the mobile phones of orthopedic and anesthesia attendings, residents, technicians and nurses working in the orthopedic operating rooms over a period of two months. Demographic and cell phone related factors were recorded using a questionnaire and the factors associated with contamination were analyzed. https://www.selleckchem.com/ Results Ninety-three of 100 mobile phones were contaminated. Species isolated were Coagulase-negative Staphylococcus (62%), Micrococcus (41%) and Bacillus (26%). Conclusion Mobile phones belonging to health care workers are frequently contaminated with pathogenic bacteria with the potential of transferring drug resistance to nosocomial pathogens. Studies investigating the relationship to surgical site infections need to be conducted. The use of biochar in management of ear rot has not been established. Efficacy of biochar aganst the disease was therefore investigated. Efficacy of biochars produced from poultry faecal waste (Bpw) and sawdust (Bsd) against pathogenic Fusarium verticillioides (Fv) causing ear rot in maize was determined using biochar treatment combinations (Bpw, Bsd, Bpw + Bsd, Bpw + Fv, Bsd + Fv, Bpw + Bsd + Fv, Fv and control) as soil amendments. Additional treatments consisted of fungicide (Cibaplus), poultry feacal waste (Pw), sawdust (Sd), Bpw + Fungicide, Bsd + Fungicide, Bpw + Bsd + Fungicide, Fungicide + Fv, and Pw + Sd. The Bpw and Pw at 1, 2 and 3 kg/m2 each, Bsd and Sd (0.50, 1.00 and 1.50 kg/m2) and fungicide (0.25, 0.50 and 1.00 g/L) were applied. Inoculation of pathogenic F. verticillioides strain was conducted at 7th week after planting and ear rot severity assessed at harvest. Residual effects of treatments were examined in the second season. Data gathered were subjected to ANOVA at α 0.05. Maize treated with Sd, Bpw and Bpw + Fungicide scored 1-3% severity; Bpw + Bsd, Bsd, Fungicide, Pw + Sd, Bsd + Fv, Bsd + Fungicide, Bpw + Bsd + Fungicide, Bpw + Fv, Bpw + Bsd + Fv and Fungicide + Fv scored 4-10 %. Claiming about 50 percent of the world population is infected with H. pylori while therapies for its eradication have failed because of many reasons including the acquired resistance against its antibiotics. Hence, the need to find new anti-H.pylori medications has become a hotspot with the urge of searching for alternative, more potent and safer inhibitors. In the recent drug technology scenario, medicinal plants are suggested as repositories for novel synthetic substances. Hitherto, is considered as ecofriendly, simple, more secure, easy, quick, and less toxic traditional treatment technique. This review is to highlight the anti-H. pylori medicinal plants, secondary metabolites and their mode of action with the aim of documenting such plants before they are effected by cultures and traditions that is expected as necessity.

  • BACKGROUND Current management of patients with atrial fibrillation (AF) is limited by low detection of AF, non-adherence to guidelines, and lack of consideration of patients' preferences, thus highlighting the need for a more holistic and integrated approach to AF management. OBJECTIVE The objective of this study was to determine whether a mobile health (mHealth) technology-supported AF integrated management strategy would reduce AF-related adverse events, compared with usual care. METHODS This is a cluster randomized trial of patients with AF older than 18 years of age who were enrolled in 40 cities in China. https://www.selleckchem.com/products/myk-461.html Recruitment began on June 1, 2018 and follow-up ended on August 16, 2019. Patients with AF were randomized to receive usual care, or integrated care based on a mobile AF Application (mAFA) incorporating the ABC (Atrial Fibrillation Better Care) Pathway A, Avoid stroke; B, Better symptom management; and C, Cardiovascular and other comorbidity risk reduction. The primary composite outcome was a composite or patients receiving the mAFA intervention compared with usual care (all p  less then  0.05). CONCLUSIONS An integrated care approach to holistic AF care, supported by mHealth technology, reduces the risks of rehospitalization and clinical adverse events. Many treatment options are available to address the problem of missing second premolars. Extraction of retained deciduous second molars and subsequent space closure are often appropriate because this option alleviates the need for implants or other restorations. This article describes the conventional orthodontic closure of unilateral and bilateral congenitally missing second premolar spaces after the extraction of retained deciduous second molars. This case report illustrates the successful nonsurgical treatment of an adult with a skeletal Class III malocclusion exacerbated by a functional anterior shift that resulted in a severe overclosure of the mandible and a reverse smile line. To maximize the soft tissue and smile esthetics while idealizing the occlusion, active clockwise rotation of the mandible was induced along with mandibular molar uprighting and sequential leveling. In the maxilla, full arch distalization was achieved after second molar extraction. The treatment provided a satisfying esthetic and functional outcome and has remained stable. This case report describes the successful orthodontic treatment for a healthy woman with maxillary canine-premolar transposition associated with rare hypodontia of the contralateral canine. The treatment included extraction of the transposed maxillary right and deciduous maxillary left canines and 2 mandibular second premolars. The objectives of the treatment were to (1) correct the upper midline deviation and the occlusal plane cant, (2) move the maxillary first premolars into the canine position, (3) retract the mandibular incisors to improve the facial profile, and (4) reduce the labial protrusion. Orthodontic treatment was carried out with the lingual technique in the maxillary arch and esthetic ceramic brackets in the mandibular arch. The segmented mechanics were effective, fast, and consistently promoted the necessary movement without side effects. The smile line was improved, resulting in an ideal and esthetic gingival exposition, with the borders of the maxillary teeth following the lower lip smile curvature. The patient achieved ideal dentofacial esthetics, achieved better dental and functional occlusion, was pleased with her improved facial contour, and as a result, showed an improved self-image. INTRODUCTION This study aimed to evaluate whether viewing digital treatment simulations influenced orthodontic treatment planning decisions or practitioners' confidence in their selected plans. METHODS Records of 6 patients representing different case types (eg, missing teeth, crowding, sagittal discrepancies) were collected. A total of 22 orthodontists and 7 orthodontic residents viewed these records and formulated treatment plans for each case, indicating their most recommended plan and up to 2 alternative plans. After treatment planning each case, digital setups of each treatment plan indicated by the practitioner were shown. The practitioners were then asked if they still recommended their original plan, or if they would now recommend a different plan. Their confidence levels in the success of their plans were recorded before and after viewing the setups. RESULTS After viewing the digital setups, there was a significant change in the treatment plan for 9.2% of the cases. These included modifications like changing the extraction pattern or proposing space closure rather than opening space for an implant. In an additional 14.4% of the cases, treatment plans underwent partial changes, like adding interproximal reduction or temporary anchorage devices. Practitioner confidence levels increased after viewing the setups. In cases where the treatment plan changed, the practitioner's confidence level in the plan increased the most, and the final confidence level was uniformly high among all practitioners. Practitioners reported the most helpful features of digital setups were the ability to superimpose the setup with the original model, determine the amount of tooth movement needed, check the final incisal relationship (overjet and overbite), and establish the amount of interproximal reduction required. CONCLUSIONS Viewing digital setups resulted in changes to the treatment plans in about 24% of the cases. The use of digital setups was associated with higher levels of confidence in the selected plans. INTRODUCTION The purpose of this study was to evaluate the occlusal contact area and cast-radiograph evaluation (CRE) score changes in patients with 3 different retention protocols after a 1-year retention period. METHODS A total of 90 patients with acceptable final occlusion according to American Board of Orthodontics CRE were equally divided into 3 groups according to the retention protocol upper bonded retainer and lower Essix or bonded retainer (Essix group); upper bonded retainer and Hawley or lower bonded retainer (Hawley group); and upper bonded retainer and lower bonded retainer (bonded retainer group). Digital models were used to assess occlusal contact area changes after a 1-year retention period. The follow-up occlusion models were assessed with the American Board of Orthodontics CRE. RESULTS Occlusal contact areas increased significantly in the Hawley and bonded retainer groups for all teeth except incisors.     BACKGROUND Current management of patients with atrial fibrillation (AF) is limited by low detection of AF, non-adherence to guidelines, and lack of consideration of patients' preferences, thus highlighting the need for a more holistic and integrated approach to AF management. OBJECTIVE The objective of this study was to determine whether a mobile health (mHealth) technology-supported AF integrated management strategy would reduce AF-related adverse events, compared with usual care. This article describes the conventional orthodontic closure of unilateral and bilateral congenitally missing second premolar spaces after the extraction of retained deciduous second molars. This case report illustrates the successful nonsurgical treatment of an adult with a skeletal Class III malocclusion exacerbated by a functional anterior shift that resulted in a severe overclosure of the mandible and a reverse smile line. To maximize the soft tissue and smile esthetics while idealizing the occlusion, active clockwise rotation of the mandible was induced along with mandibular molar uprighting and sequential leveling. In the maxilla, full arch distalization was achieved after second molar extraction. The treatment provided a satisfying esthetic and functional outcome and has remained stable. This case report describes the successful orthodontic treatment for a healthy woman with maxillary canine-premolar transposition associated with rare hypodontia of the contralateral canine. The treatment included extraction of the transposed maxillary right and deciduous maxillary left canines and 2 mandibular second premolars. The objectives of the treatment were to (1) correct the upper midline deviation and the occlusal plane cant, (2) move the maxillary first premolars into the canine position, (3) retract the mandibular incisors to improve the facial profile, and (4) reduce the labial protrusion. Orthodontic treatment was carried out with the lingual technique in the maxillary arch and esthetic ceramic brackets in the mandibular arch. The segmented mechanics were effective, fast, and consistently promoted the necessary movement without side effects. The smile line was improved, resulting in an ideal and esthetic gingival exposition, with the borders of the maxillary teeth following the lower lip smile curvature. The patient achieved ideal dentofacial esthetics, achieved better dental and functional occlusion, was pleased with her improved facial contour, and as a result, showed an improved self-image. INTRODUCTION This study aimed to evaluate whether viewing digital treatment simulations influenced orthodontic treatment planning decisions or practitioners' confidence in their selected plans. METHODS Records of 6 patients representing different case types (eg, missing teeth, crowding, sagittal discrepancies) were collected. A total of 22 orthodontists and 7 orthodontic residents viewed these records and formulated treatment plans for each case, indicating their most recommended plan and up to 2 alternative plans. After treatment planning each case, digital setups of each treatment plan indicated by the practitioner were shown. The practitioners were then asked if they still recommended their original plan, or if they would now recommend a different plan. Their confidence levels in the success of their plans were recorded before and after viewing the setups. RESULTS After viewing the digital setups, there was a significant change in the treatment plan for 9.2% of the cases. These included modifications like changing the extraction pattern or proposing space closure rather than opening space for an implant. In an additional 14.4% of the cases, treatment plans underwent partial changes, like adding interproximal reduction or temporary anchorage devices. Practitioner confidence levels increased after viewing the setups. In cases where the treatment plan changed, the practitioner's confidence level in the plan increased the most, and the final confidence level was uniformly high among all practitioners. Practitioners reported the most helpful features of digital setups were the ability to superimpose the setup with the original model, determine the amount of tooth movement needed, check the final incisal relationship (overjet and overbite), and establish the amount of interproximal reduction required. CONCLUSIONS Viewing digital setups resulted in changes to the treatment plans in about 24% of the cases. The use of digital setups was associated with higher levels of confidence in the selected plans. INTRODUCTION The purpose of this study was to evaluate the occlusal contact area and cast-radiograph evaluation (CRE) score changes in patients with 3 different retention protocols after a 1-year retention period. METHODS A total of 90 patients with acceptable final occlusion according to American Board of Orthodontics CRE were equally divided into 3 groups according to the retention protocol upper bonded retainer and lower Essix or bonded retainer (Essix group); upper bonded retainer and Hawley or lower bonded retainer (Hawley group); and upper bonded retainer and lower bonded retainer (bonded retainer group). Digital models were used to assess occlusal contact area changes after a 1-year retention period. The follow-up occlusion models were assessed with the American Board of Orthodontics CRE. RESULTS Occlusal contact areas increased significantly in the Hawley and bonded retainer groups for all teeth except incisors. All 43 eyes underwent cataract surgery and vitrectomy. The aqueous and vitreous humor were collected at the surgery. The anaphylatoxin concentrations were measured by using a cytometric beads array, and the respective C3a, C4a, and C5a concentrations were 2.003 ± 0.679 (mean ± standard deviation) ng/ml, 1.389 ± 0.419 ng/ml, and 0.003 ± 0.004 ng/ml in the aqueous humor, and 1.236 ± 0.642 ng/ml, 1.250 ± 0.542 ng/ml, and 0.048 ± 0.069 ng/ml in the vitreous humor. The mean C3a concentration in the aqueous humor was significantly higher than in the vitreous humor in 43 eyes of iMH and iERM (P  less then  0.001). The mean C4a concentration showed no significant difference between the aqueous humor and vitreous humor (P = 0.282), and the mean C5a in the aqueous humor was significantly lower than in the vitreous humor overall (P  less then  0.001). Participants who were able to recognize at least 5 risk factors, 3 warning signs, and defined calling the ambulance in the first 3 hours of a suspected incident were considered knowledgeable. RESULTS The average age was (27.7 ± 11.5) with a female predominance. The majority (59.3%) were able to correctly define 5 or more risk factors, 68.3% of participants were able to identify at least 3 warning signs of stroke whilst 9.5% were not able to identify any warning signs. More than 70% of participants were willing to call ambulance within 3 hours while 86.2% preferred counseling doctors within 24 hours. Participants who intended to call the ambulance in the first 3 hours were significantly younger (76.1%), mainly students (45.4%) and were able to identify at least 5 risk factors (64.5%) and 3 warning signs of stroke (74.1%). CONCLUSIONS Despite reporting a relatively higher level of public's knowledge regarding stroke, there is a persistent gap in their knowledge of stroke risk factors, and the importance of seeking emergency medical care. Patients who finished the baseline brain computed tomography (CT) examination within 6 h post-ictus and the follow-up CT within 48 h after initial CT were included in the study. Hematoma expansion was delimited as an enlargement of hemorrhage volume over 33% or 12.5 mL between baseline and subsequent CT. Inhibition of growth during antibiotic exposure was evaluated in 24-h automated time-lapse microscopy experiments. Combinations that showed positive interactions were subsequently evaluated in static time-kill experiments. RESULTS All strains carried multiple (≥9) resistance genes as determined by whole-genome sequencing. In the initial screening the combination of polymyxin B and minocycline was most active with enhanced activity compared with the single antibiotics detected against all strains.

Method Twenty-four university students voluntarily took part in this study (Mage = 20.81 years [± 1.76]). Four different 3 × 3 basketball games were played (A) free game, (B) seven seconds possession, (C) three passes maximum per attack phase, (D) conditions B and C combined. Results Statistical analysis showed that constraints B, C and D produce more mental load, provoke differences in the affect response and the performance is worse. Conclusion These results demonstrate the mental cost of restricting the time and number of passes, separately and combined to decide and act.This meta-analysis evaluated theoretical predictions from balanced identity theory (BIT) and evaluated the validity of zero points of Implicit Association Test (IAT) and self-report measures used to test these predictions. Twenty-one researchers contributed individual subject data from 36 experiments (total N = 12,773) that used both explicit and implicit measures of the social-cognitive constructs. The meta-analysis confirmed predictions of BIT's balance-congruity principle and simultaneously validated interpretation of the IAT's zero point as indicating absence of preference between two attitude objects. Statistical power afforded by the sample size enabled the first confirmations of balance-congruity predictions with self-report measures. Beyond these empirical results, the meta-analysis introduced a within-study statistical test of the balance-congruity principle, finding that it had greater efficiency than the previous best method. The meta-analysis's full data set has been publicly archived to enable further studies of interrelations among attitudes, stereotypes, and identities.To reach the most vulnerable individuals in under-resourced countries, health communication interventions increasingly move towards the community level. However, little is known about how health information spreads through local social networks. This paper maps the health information network of a rural trading centre in Uganda. As part of a five-year ethnographic study of sustainable community health resources, ego networks were obtained for 231 village residents in March 2014. Using both ethnographic and social network data, we analyze how the village social network is structured, and how this structure may influence the transmission of health information. Results show a network with low average proximity, with a small number of individuals, notably key administrative officials, **** closer connected to many other community members than average. However, because of social partitioning in the village network, a number of people are outside the social clusters in which the top influencers are located.Background Energy-storage and return feet incorporate various design features including split toes. The model used shows high correlations with the measured data for all feet. Conclusions The ability of prosthetic feet to adapt to uneven ground is design-dependent. The split-toe feet adapted better to cross slopes than those with continuous carbon forefeet. Joints enhance this further by allowing for additional inversion and eversion. The influence on biomechanical and clinical parameters should be assessed in future studies. Clinical relevance Knowing foot-specific ability to adapt to uneven ground may help in selecting an appropriate prosthetic foot for persons with a lower limb amputation. Faster and more comprehensive adaptations to uneven ground may lower the need for compensations and therefore increase user safety.Purpose The aim of the current study was to determine the incidence, clinical characteristics, and risk factors associated with the recurrence of first unprovoked seizure in children. Methods A retrospective, observational study was conducted at King Abdullah University Hospital in Jordan. Children aged from 1 month to 16 years old who attended the hospital between January 2013 to December 2017 were evaluated on the basis of medical records, from the first visit and for a 1-year follow-up period. Results During the study period, a total of 290 cases with first unprovoked seizure were included. The incidence of first unprovoked seizure was 441 cases per 100 000 patient visits to the pediatric clinic. More than half of the cases developed a second attack (55.3%). Children with parental consanguinity were almost 3 times more likely to develop a second attack of seizure compared to those without parental consanguinity (odds ratio [OR] = 2.785, 95% confidence interval [CI] = 1.216-6.378, P = .015) and patients who had a history of focal type of seizure were almost twice as likely to develop seizure recurrence (OR = 1.798, 95% CI = 1.013-3.193, P = .045). Conclusions The current results showed a high incidence of first unprovoked seizure among children in Jordan. Parental consanguinity and focal seizure were associated with the increased risk of recurrent attack. This finding highlights the need for public education regarding the outcomes of parental consanguinity to improve the patient's quality of life.

The influence on biomechanical and clinical parameters should be assessed in future studies. Clinical relevance Knowing foot-specific ability to adapt to uneven ground may help in selecting an appropriate prosthetic foot for persons with a lower limb amputation. Faster and more comprehensive adaptations to uneven ground may lower the need for compensations and therefore increase user safety.Purpose The aim of the current study was to determine the incidence, clinical characteristics, and risk factors associated with the recurrence of first unprovoked seizure in children. Methods A retrospective, observational study was conducted at King Abdullah University Hospital in Jordan. Children aged from 1 month to 16 years old who attended the hospital between January 2013 to December 2017 were evaluated on the basis of medical records, from the first visit and for a 1-year follow-up period. Results During the study period, a total of 290 cases with first unprovoked seizure were included. The incidence of first unprovoked seizure was 441 cases per 100 000 patient visits to the pediatric clinic. More than half of the cases developed a second attack (55.3%). Children with parental consanguinity were almost 3 times more likely to develop a second attack of seizure compared to those without parental consanguinity (odds ratio [OR] = 2.785, 95% confidence interval [CI] = 1.216-6.378, P = .015) and patients who had a history of focal type of seizure were almost twice as likely to develop seizure recurrence (OR = 1.798, 95% CI = 1.013-3.193, P = .045). Conclusions The current results showed a high incidence of first unprovoked seizure among children in Jordan. Parental consanguinity and focal seizure were associated with the increased risk of recurrent attack. This finding highlights the need for public education regarding the outcomes of parental consanguinity to improve the patient's quality of life.

  • Background Treatment of patients with solid tumors and KRAS mutations remains disappointing. One option is the combined inhibition of pathways involved in RAF-MEK-ERK and PI3K-AKT-mTOR. Methods Patients with relapsed solid tumors were treated with escalating doses of everolimus (E) 2.5-10.0 mg/d in a 14-day run-in phase followed by combination therapy with sorafenib (S) 800 mg/d from day 15. KRAS mutational status was assessed retrospectively in the escalation phase. Extension phase included KRAS-mutated non-small-cell lung cancer (NSCLC) only. Pharmacokinetic analyses were accompanied by pharmacodynamics assessment of E by FDG-PET. Efficacy was assessed by CT scans every 6 weeks of combination. Results Of 31 evaluable patients, 15 had KRAS mutation, 4 patients were negative for KRAS mutation, and the KRAS status remained unknown in 12 patients. Dose-limiting toxicity (DLT) was not reached. The maximum tolerated dose (MTD) was defined as 7.5 mg/d E + 800 mg/d S due to toxicities at previous dose level (10 mg/d E + 800 mg/d S) including leucopenia/thrombopenia III° and pneumonia III° occurring after the DLT interval. The metabolic response rate in FDG-PET was 17% on day 5 and 20% on day 14. No patient reached partial response in CT scan. Median progression free survival (PFS) and overall survival (OS) were 3.25 and 5.85 months, respectively. Conclusions Treatment of patients with relapsed solid tumors with 7.5 mg/d E and 800 mg/d S is safe and feasible. Early metabolic response in FDG-PET was not confirmed in CT scan several weeks later. The patient's isokinetic muscle strength test was performed in the first, third, and the sixth month before and after the operation. The knee hamstring peak torque (PT value), quadriceps peak torque (PT value), and total work were mainly measured. The knee joint was evaluated at the hospital for special surgery score, range of motion and other knee function standards, and then healthy limbs and normal people were tested with the same method. Statistical data was used to analyze and deal with the data, evaluate the muscle strength and motor function changes with time progressing, then compare the differences to the healthy limb. From P less then 0.05, we can see that the differmpared with normal people. Conclusions Knee joint strength and knee function after TKA are significantly improved compared with preoperative function, which is of great significance for the treatment of knee osteoarthritis. The constant velocity muscle strength test system has the advantages of safety, accuracy, repeatability and easy operation. The averaged ODC provided systematically higher capillary oxygen tensions than when assuming a 'fixed' standard arterial ODC. When the averaged and measured arterial ODC were used for constructing the capillary ODC, similar values were obtained during resting breathing, but not when the arterial ODC was modulated by hypocapnia.   • fluoride of affected areas.Leptolyngbya boryana (L. boryana) is a typical filamentous cyanobacterium that is widely distributed in aquatic ecosystems and is considered to play an important role in the arsenic biogeochemical cycle. Our results showed that L. boryana resisted arsenite (As(III)) and arsenate (As(V)) concentrations up to 0.25 mM and 5 mM, respectively. When exposed to 100 μM As(III) or As(V) for 4 weeks, L. boryana accumulated as **** arsenic as 570.0 mg kg-1 and 268.5 mg kg-1, respectively. After the 4-week exposure to As(III) and As(V), organoarsenicals including dimethylarsenate (DMAs(V)) and oxo-arsenosugar-phosphate (Oxo-PO4) were detected in the cells of L. boryana, while inorganic arsenic, especially As(V), was still the main species in both the cells and medium. Furthermore, arsenic oxidation was observed to be solely caused by L. boryana cells and was considered the dominant detoxification pathway. In conclusion, due to its powerful arsenic accumulation, biotransformation, and detoxification abilities, L. boryana might play an important role in arsenic remediation in aquatic environments.We investigate the causal relationship between economic policy uncertainty (EPU) and energy consumption and carbon (CO2) emissions in G7 countries. We employ a bootstrap panel Granger causality test developed by Kónya (Econ Model 23978-992, 2006), using a yearly data set spanning from 1998 to 2018. Our test results provide significant support for a unidirectional causality running from EPU to energy consumption in Japan; from EPU to CO2 emissions in the USA and Germany; and from EPU to both energy consumption and CO2 emissions in Canada. In Italy, causality runs from CO2 emissions to EPU, but a bidirectional causality between EPU and energy consumption exists as well. We also explore a unidirectional causality that runs from energy consumption to CO2 in the USA. Based on the overall findings, we draw important implications for policymakers and we strongly recommend for G7 countries to take into account possible negative effects of EPU on energy conservation policies, which should be embarked upon to reduce energy consumption and CO2 emissions, as committed in their recent climate mandate.Applying the climatological water balance (WB) concept to describe the relationship between climatic seasonality and surface water quality according to different forms of land use and land cover (LULC) is an important issue, but little explored in the literature. In this paper, we evaluate the influence of WB on surface water quality and its impacts when interacting with LULC. We monitored 11 sampling points during the four seasons of the year, from which we estimate WQI (water quality index) and TSI (trophic state index). We found an effect of the seasonality factor on both WQI values (F(3,30) = 12.472; p less then 0.01) and in TSI values (F(3,30) = 6.967; p less then 0.01). We noticed that LULC interferes in the way that the water balance influences the WQI and TSI values since in sampling points closest to higher urban density, with little or no riparian protection, the correlation between water balance and water quality was lower. In the stations that had the lowest water surplus and deficit, there was positive linearity between water balance and WQI. However, in the seasons when the surplus and water deficit recorded were extreme, there was no linearity. We conclude that water deficiency impairs the quality of surface water. In the extreme surplus water season, the homogeneity of WQI samples was lower, suggesting a higher interaction between rainwater and LULC. This study contributes to design management strategies of water resources, considering the climatic seasonality for optimization.The WRF-Chem (Weather Research and Forecasting with Chemistry) model is implemented and validated against ground-based observations for meteorological and atmospheric variables for the first time in Northern Vietnam. The WRF-Chem model was based on HTAPv2 emission inventory with MOZCART chemical-aerosol mechanism to simulate atmospheric variables for winter (January) and summer (July) of 2014. The model satisfactorily reproduces meteorological fields, such as temperature 2 m above the ground and relative humidity 2 m above the ground at 45 NCHMF meteorological stations in January, but lower agreement was found in those simulations of July. PM10 and PM2.5 concentrations in January showed good temporal and spatial agreements to observations recorded at three CEM air monitoring stations in Phutho, Quangninh, and Hanoi, with correlation coefficients of 0.36 and 0.59. However, WRF-Chem model was underestimated with MFBs from - 27.9 to - 118.7% for PM10 levels and from - 34.2 to - 115.1% for PM2.5 levels. It has difficulty in capturing day-by-day variation of PM10 and PM2.5 concentrations at each station in July, but MFBs were in the range from - 27.1 to - 40.2% which is slightly lower than those in January. It suggested that further improvements of the model and local emission data are needed to reduce uncertainties in modeling the distribution of atmospheric pollutants. Assessment of biomass burning emission on air quality in summer was analyzed to highlight the application aspect of the WRF-Chem model. Using this method, we developed a new Tier-1 risk assessment system for ectoparasitoid, successfully avoided "host/prey quality-mediated effect," and improve the accuracy of safety evaluation.   • OBJECTIVE Zambia is among the world's top 10 countries with higher fertility rate (5.5 births/woman); unmet family planning need for births spacing (14%) and limiting births (7%). Women in rural Zambia (24%) are reported to have unmet need for family planning than those in urban areas (17%). This study was conducted to ascertain factors associated with modern contraceptive use among rural Zambian women. DESIGN Cross-sectional study. SETTING Rural Zambia. PARTICIPANTS Secondary data of 4903 married or cohabiting rural women (15-49 years) after filtering out the pregnant, urban based and unmarried women from 2013 to 2014 Zambian Demographic and Health Survey (ZDHS) were analysed using SPSS V.22. Multiple logistic regression, Pearson's χ2 and descriptive statistics were performed to examine factors associated with modern contraceptive use. The success rate was influenced by type of specimen formalin-fixed paraffin embedded (FFPE block vs FFPE slides), by origin of the sample (surgery vs biopsy) and by time of fixation ( less then 5 years vs ≥5 years). The most frequent subgroups of effective reports derived from colorectal cancer (25 samples), non-small-cell lung cancer (16 samples), ovarian cancer (10 samples), biliary tract cancer (9 samples), breast cancer (7 samples), gastric cancer (7 samples).
    OBJECTIVE Zambia is among the world's top 10 countries with higher fertility rate (5.5 births/woman); unmet family planning need for births spacing (14%) and limiting births (7%). Women in rural Zambia (24%) are reported to have unmet need for family planning than those in urban areas (17%). This study was conducted to ascertain factors associated with modern contraceptive use among rural Zambian women. DESIGN Cross-sectional study. SETTING Rural Zambia. PARTICIPANTS Secondary data of 4903 married or cohabiting rural women (15-49 years) after filtering out the pregnant, urban based and unmarried women from 2013 to 2014 Zambian Demographic and Health Survey (ZDHS) were analysed using SPSS V.22. Multiple logistic regression, Pearson's χ2 and descriptive statistics were performed to examine factors associated with modern contraceptive use. Without any intervention, lower/middle-income countries (LMICs) will be most affected due to already higher AMR levels compared with higher income countries and due to the far higher burden of diseases in the LMICs. Studies have consistently shown that inappropriate use of antimicrobials is the major driver of AMR. This study aimed to examine whether job strain (a combination of high job demands and low control at work) and/or burnout, the outcome of a gradual depletion of energetic resources resulting from chronic exposure to work-related stress, are associated with IBS. METHODS Fifty-five patients fulfilling the Rome III criteria for IBS and 214 matched healthy controls (HC) participated in this cross-sectional study. All participants completed a job strain measure, the Shirom - Melamed Burnout Measure (SMBM), and dietary and health questionnaires. RESULTS There was no significant difference in the prevalence of job strain between IBS patients and HC (25.5% vs. 23.0%, respectively). Job strain was not associated with increased IBS prevalence (adjusted OR = 1.99, 95% CI 0.54-7.33). In contrast, the mean burnout score in the IBS group was significantly higher than in HC (2.9 ± 1.1 vs. 2.1 ± 0.8, p  less then  .001). Burnout was associated with a 2.41-fold elevated prevalence of IBS (95% CI 1.16-5.02), after adjusting for potential confounding variables including job strain. The secondary dorsal-ventral axis is conditionally specified later in development. Dorsal-ventral polarity is very liable during the early cleavages, though more recent data argue that its direction may be oriented by maternal asymmetry. In this review, we focus on the role of maternal factors in initial embryonic patterning during the first cleavages of sea urchin embryos before activation of the embryonic genome. Unexpected sudden cardiac death (SCD), sudden infant death syndrome (SIDS) and sudden intrauterine unexplained death (SIUD) are major unsolved, devastating forms of death that occur frequently. Obstructive sleep apnea (OSA) has been associated with increased cardiovascular and cerebrovascular morbidity and mortality, including sudden cardiac death (SCD). This editorial will review the pathology of SCD, including sudden infant death syndrome (SIDS) and sudden intrauterine unexplained death (SIUD); OSA with its cardiovascular consequences; the possible link between SCD and OSA, discussing the potential mechanisms underlying these two frequent, but yet overlooked pathologies. Finally, the possible preventive benefits of treating OSA and identifying patients at common risk for OSA and SCD and SIDS-SIUD to prevent unexpected deaths will be discussed. Post-mortem examination is of great importance in every case of SCD sine materia, with examination of the brainstem and cardiac conduction system on serial sections, when general autopsy fails, but it should be stressed that also the investigations of patients suffering from OSA should focus on the possibility of pathological findings in common with cases of SCD. To characterize metanephric tumours in children, we performed a literature review investigating paediatric metanephric adenomas (MA), metanephric stromal tumours (MST) and metanephric adenofibromas (MAF). Including two patients from our own institution (MA, MAF), 110 individual cases (41 MA, 20 MAF, 49 MST) were identified. Additionally, fifteen composite tumours were identified, with areas of MA/MAF and Wilms tumour (WT) or papillary carcinoma. No distinct clinical or radiological features could be defined. In pure metanephric tumours, histologically proven distant metastases were reported once (MA), relapse was reported once (MST) and one tumour-related death occurred (MST). Somatic BRAF-V600E mutations were tested in 15 cases, and identified in 3/6 MA, 3/3 MAF, and 6/6 MST. In our institution the MA harboured a somatic KRAS-G12R mutation. Overall, paediatric metanephric tumours are difficult to discriminate from other renal tumours at presentation, behave relatively benign, and the occurrence of composite tumours warrants analysis of underlying (genetic) pathways. Immunotherapy is emerging as a novel therapeutic approach. This systematic review assesses clinical data regarding immunotherapy efficacy and safety. METHODS Data from 11 clinical trials testing immunotherapy in HNSCC were assessed. We performed the meta-analysis to correlate the overall survival (OS), response rate (RR), adverse effects, HPV status, and PD-L1 expression. RESULTS Immunotherapy extended OS (hazard ratio = 0.77, p  less then  0.0001) and RR significantly (risk ratio = 1.41, p = 0.02). Patients with HPV-positive HNSCC exhibited a better RR (risk ratio = 1.29, p = 0.24) and OS (11.5 vs. 6.3 months). PD-L1 positive tumors showed a higher OS (9.9 vs. 6.5 months). Moreover, immunotherapy caused less adverse effects than standard therapy. CONCLUSION Our results indicate the benefit of immunotherapy for improving RR and OS of HNSCC patients. The benefit is higher in patients with HPV and PD-L1 positive tumors.
