The study group comprised 19 patients who underwent a combination of long- and short-axis alar sacroplasty techniques under fluoroscopic guidance. The control group comprised the remaining 25 patients who underwent short-axis alar sacroplasty under fluoroscopic guidance. Visual analog scale (VAS) scores, operation times, injected cement volumes, and postoperative complications were recorded.

    The VAS score for pain decreased in both groups; however, no significant difference was noted between the study and control groups in injected cement volume (3.55 ± 0.96 vs 2.94 ± 0.89 mL). The operation time was longer in the study group than in the control group (32 ± 7.1 vs 28.04 ± 4.99 min; P = 0.046). No major complications were noted.

    Totally endoscopic technique has been widely used in cardiac surgery, and minimally invasive totally endoscopic mitral valve surgery has been developed as an alternative to median sternotomy for many patients with mitral valve disease. In this study, we describe our experience about a modified minimally invasive totally endoscopic mitral valve surgery and reported the preliminary results of totally endoscopic mitral valve surgery. The aim of this retrospective study is to evaluate the results of totally endoscopic technique in mitral valve surgery.

    We retrospectively reviewed the profiles of 188 patients who were treated for mitral valve disease by modified totally endoscopic mitral valve surgery at our institution between January 2019 and December 2020. The procedure was performed under endoscopic right minithoracotomy and with femoro-femoral cannulation using the single two-stage venous cannula.

    A total of 188 patients underwent total endoscopic mitral valve surgery. Fifty-six patients had concomitant tricuspid valvuloplasty, 11 patients underwent concomitant ablation of atrial fibrillation and atrial septal defect repair was performed in three patients. Only one patient postoperatively died of multi-organ failure. Two patients were converted to median sternotomy. Except for one patient underwent operation to stop the bleeding from the incision site, no other serious complications nor reintervention occurred during the follow-up period.

    Posterior hemivertebra resection and short-segment fusion with pedicle screws are an established treatment in congenital scoliosis, which require pediatric-specific instrumentation. The purpose of this study was to report the results of utilizing cervical lateral mass screws instead of pedicle screws in the treatment of congenital scoliosis in children younger than 5 years old.

    In an IRB-approved retrospective chart review study, patients <5 years old with congenital scoliosis who underwent posterior hemivertebra resection and fusion with lateral mass screws from 2013 to 2017 were included. Demographic information, pre- and post-operative radiographs, complications, and outcomes were extracted from the charts.

    Twenty-three patients were included in the final analysis with a mean age of 40 months, of which 14 were female. Patients were followed for a mean of 51.3±13.2 months. The mean blood loss was 210ml, and patients were hospitalized for a mean of 4 days post-operatively. The correction rate of the are similar and comparable to pediatric-specific pedicle screws, in addition to being low-profile and less bulky compared to adult implants.

    We used data from the 2017-2018 Ghana multiple indicator cluster survey and our analysis was restricted to 3466 mothers who had a live birth within 2 years. Multivariable logistic regression was used to estimate the factors associated with timely initiation of breastfeeding whilst adjusting for potential confounders, and accounted for clustering, stratification, and sample weights.

  • BACKGROUND Alzheimer's disease (AD) is a widespread dementia-related disease affecting mankind worldwide. A cholinergic hypothesis is considered the most effective target for treating mild to moderate AD. Present study aims to identify new scaffolds for inhibiting acetylcholinesterase activity. METHODS To find Acetylcholinesterase (AChE) inhibitors, we computationally designed and chemically synthesized a series of cation-π inhibitors based on novel scaffolds that potentially block AChE. The cytotoxic effect of inhibitors were determined by MTT. AChE inhibition experiment was performed by Ellman and the Amplex red method in the SH-SY5Y cell line. Further, the experimental data on designed compounds corroborate with various computational studies that further elucidate the binding mode of interactions and binding affinity. RESULTS The inhibitors were designed to promote dual binding and were incorporated with groups that may facilitate any of the cation- π, hydrophobic and hydrogen-bonding interactions with the conserved and hot-spot residues in the binding site. The inhibitors possessing pyridine-N-methylated pyridinium group and thereby involved in cation- π interactions are highly active relative to the marketed drug Donepezil as well as the designed analogs that lack the group. In vitro enzymatic Ellman assay and Amplex red assay on SH-SY5Y cell line estimated IC50 of the designed compounds in nM range with one having binding affinity higher than Donepezil. Compounds exhibit no significant toxicity up to µM range. CONCLUSIONS Compounds possessing methylidenecyclohexanone scaffolds, with characteristic dual-binding and involving strong cation-π interactions, serves as new leads for AChE and opens a new direction for drug discovery efforts.The high burden of heart failure in nursing-home populations is due to advanced age and comorbidities. We review the use of natriuretic peptide biomarkers for screening heart failure in nursing-home residents. The study was performed in accordance with recommendations from the Cochrane Collaboration using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement (PRISMA) and is registered in PROSPERO Register of Systematic Reviews. Databases PubMed, Embase, and Trip were searched from 2000 to March 2019, supplemented by hand-searching of references. Studies investigating the nursing-home population were included. The prevalence of heart failure among nursing-home residents was higher than in the general population of comparable age (23% vs 10%, respectively). The rate of misdiagnosis in nursing homes ranged from 25 to 76%. NT-proBNP was the most commonly used natriuretic peptide biomarker for heart failure screening. The mean value of NT-proBNP was significantly higher in residents with heart failure than in residents overall (pooled means of 2409 pg/mL vs 1074 pg/mL, respectively). In comparison with current guidelines, the proposed cut-off values for ruling out heart failure were higher in the analyzed studies, with ranges of 230-760 pg/mL for NT-proBNP and 50-115 pg/mL for BNP. NT-proBNP and BNP are used for screening heart failure in the nursing-home population. The current screening cut-off values are probably too low for use in nursing homes. RECENT FINDINGS Associated chronic pain represents a significant detractor from the quality of life experienced by these patients, affecting over half of those with SCD on more days than not. Current treatment typically is centered upon preventing and responding to acute vasoocclusive crises, presumably because this is the most common reason for hospitalization in these patients. The lack of management of chronic pain symptoms leaves many with SCD in a state of suffering. In this review, the treatment methodologies of SCD patients are examined including alternative treatments, both pharmaceutical and non-pharmaceutical, as well as procedural approaches specifically aimed at reducing chronic pain in these patients.Atrial fibrillation (AF) is highly prevalent among patients with chronic kidney disease (CKD), and also associated with unfavorable outcome. The main reason for not prescribing DOACs is often a reduction in renal function, even if mild. Aim of this review was therefore to evaluate the impact of DOAC therapy in the setting of CKD, from a nephrological perspective, by comparing available evidence on the role of DOACs in patients with CKD and AF with that emerging from traditional warfarin-based therapy.   • In a patient with variant angina of the proximal left anterior descending coronary artery, myocardial ischemia changed the QRS-ST-T configurations without J-waves into those resembling "lambda" waves at maximal ST-segment elevation, and couplets or triplets of supraventricular extrasystole (SVE) changed the ischemia-induced "lambda" waves into QRS-ST-T configurations resembling a "tombstone" morphology or "monophasic QRS-ST complex." The aim of this study was to elucidate the risk of pulmonary embolism (PE) from id-DVT and to investigate the need for anticoagulants.We identified hospitalized patients with id-DVT diagnosed by lower-extremity ultrasonography (LEUS) from January 2013 to December 2013 in our institute. The exclusion criteria were the simultaneous detection of PE, a history of PE and/or DVT, and administration of anticoagulants before DVT detection. We retrospectively investigated the patient characteristics, treatments, occurrence of PE, and bleeding events between the groups with and without anticoagulation.A total of 151 patients met the criteria. The median (IQR) age was 74 (67, 80) years old, and there were 60 (39.7%) men. The median (IQR) observation period was 571 (160, 721) days. Significant differences in patient characteristics were observed for hypertension, operation time, consultation with experts, and follow-up LEUS. During the observation period, only one patient in the no-anticoagulation group who had traumatic cerebral hemorrhaging and was bedridden developed PE (non-massive type).   • Germplasm preservation of animals, whether they are valuable domestic breeds or rare species, is the main goal of gamete cryobanking. Dogs and cats act as models for this purpose thanks to the wide availability of biological material which can be employed to experiment protocols that can then be applied to wild animals. This review is focused on spermatozoa, oocytes and gonadal tissues cryobanking in small domestic animals, which is still an unsolved case. Like in a courtroom, evidences of cryoinjuries affecting cellular structures will be presented, penalties as loss of functionality due to cellular alterations will be described, and appeal as strategies to protect gametes from damages or rescue their functionality will be discussed. Differences and similarities between single cell or tissue cryopreservation will be highlighted, together with the rationale for the choice of one type of preservation or another and the fundamental principles which they are based on. To address this imperative, mammals have developed overlapping and independent mechanisms for evading maternal anti-fetal immune responses that could result in pregnancy loss. Studies of the unusual component of equine invasive trophoblast in the epitheliochorial placenta have illuminated aspects of immune memory and tolerance that have relevance to fertility in the horse and other mammalian species. INTRODUCTION Nodular regenerative hyperplasia (NRH) is a rare hepatic vascular disorder which is often associated with wide variety of systemic diseases. Intrahepatic microvascular injury and subsequent altered perfusion state leads to development of non-cirrhotic portal hypertension in many of these patients. Diagnosis of NRH often remains unsuspected clinically and liver biopsy is essential for the diagnosis and exclusion of fibrosis. We herein, present clinicopathological features of 22 NRH cases. In addition we assessed CK7 and CD34 expression in hepatocytes and sinusoidal endothelial cells respectively. RESULTS Most of the cases were associated with systemic disorders, predominantly immunological, inflammatory and drug-related injuries. Signs and symptoms of portal hypertension were found in 86.4 % patients. Majority of the patients showed a predominant mild cholestatic pattern of liver function tests. Nearly all the (21/22) cases showed CK7 positivity in centrizonal hepatocytes which ranged from less then 10 % cells to diffuse perivenular positivity extending into the midzonal areas. CD34 positivity in sinusoidal endothelial cells was seen in all the cases, which was prominent in periportal areas in all cases; while perivenular (n = 20) and midzonal (n = 14) areas also showed CD34 positive sinusoidal endothelial cells. CONCLUSION This study highlights the role of pathologist in the diagnosis of NRH and stresses upon the need for awareness of NRH as a cause of unexplained portal hypertension in patients with underlying systemic diseases. The altered perfusion state in NRH leads to phenotypic shift in centrizonal atrophic hepatocytes and sinusoidal endothelial cells (as depicted by IHC) which may be responsible for development of portal hypertension. ECM was analyzed for protein composition using mass spectrometry with subsequent Ingenuity Pathway Analysis (IPA) to identify predicted pathways and upstream regulators. qPCR and RNA-sequencing (RNA-Seq) were performed to corroborate these predicted pathways. RESULTS 3034 proteins were differentially regulated between sham and SBR, of which 95 were significant (P  less then  0.05). IPA analysis predicted PPARα agonism to be an upstream regulator of the observed proteomic changes (P  less then  0.001). qPCR and RNA-Seq with KEGG analysis confirmed significant engagement of the PPAR pathway (P  less then  0.05). CONCLUSION Transcriptional signatures of adapting bowel predict subsequent ECM changes after SBR. How ECM communicates with surrounding cells to drive adaptation and vice versa merits further investigation. The global prevalence of glaucoma in people aged 40 to 80 years is estimated to be 3.5%. With the growing number and proportion of older persons in the population, it is projected that 111.8 million people will have glaucoma in 2040. Currently available treatments cannot reverse glaucomatous damage to the visual system; however, early diagnosis and treatment can prevent progression of the disease. In most cases, glaucoma is a chronic condition that requires lifelong management. This article reviews the pathophysiology, classification, clinical manifestations, diagnosis, and management of glaucoma.Age-related macular degeneration (AMD) is a leading cause of blindness. The main risk factor is advancing age, with the severity of vision loss ranging from mild to severe. There is a 25% risk of early AMD and 8% risk of late AMD in patients over the age of 75, with the number of cases expected to increase because of the aging population. Diagnosis of the disease requires a dilated fundus examination. Physicians should be aware of the symptoms, risk factors, and treatment options for AMD to refer appropriately for ophthalmologic evaluation.   • OBJECTIVE This aim of the study was to determine the effectiveness of guided imagery techniques to decrease pain intensity in patients with breast cáncer. METHOD This was a quantitative research with a quasi-experimental design with a total sampling of all breast cancer patients who experience pain in ambulatory surgery in the treatment room and the chemo room using Numeric Rating Scale (NRS) and the guided imagery as an intervention. https://www.selleckchem.com/products/Staurosporine.html RESULTS The results indicated guided imagery for pain intensity reduction was (p=0.000), pulse rate (p=0.005), and systolic blood pressure (p=0.000). We obtained seven articles that met the inclusion criteria consisting of 4 cross-sectional articles, one cross-sectional cohort, one prospective study. RESULTS The prevalence of PI varied incontinence patients, starting from 8.0% of 276 patients, 16.3% of 176,689 patients, 16.9% of 261 patients 30.3% of 8365 patients, 33.3% of 120 patients, 40.6% of 832 patients and 17.1% of 5342 patients. CONCLUSION Our study suggests that the prevalence of PI in patients with varied incontinence, and highest in patients with double incontinences (urine and feces). OBJECTIVE Muscle pain is a musculoskeletal disorder that is mostly encountered in the adult population and affects productivity. Muscle pain in teachers can reduce their performances. One of factors that causes muscle pain is work stress. This study aimed to determine relationship between work stress levels and muscle pain among high school teachers in Makassar. METHOD The method used is the cross-sectional method. A total of 37 people participated in this study. Muscle pain data were obtained through the Nordic Body Map questionnaire. Data analysis used the Spearman's test. RESULTS The results showed that 67.6% experienced low-level muscle pain, 29.7% experienced moderate level muscle pain, and the rest experienced high-level muscle pain. Data analysis showed that there was a statistically significant correlation between work stress and muscle pain among high school teacher (p=0.002; r=0.495). CONCLUSION Work stress has a moderate correlation with muscle pain among high school teachers. METHOD This was a cross-sectional design with sampling techniques using convenience purposive sampling. The process of translating the questionnaire instrument uses standard procedures to translate the original version of the instrument into Indonesian. Begin with the forward translation by translating the original version of the instrument, followed by the synthesis process of the translation results. The synthesis results are continued in the **** translation process then the synthesis process is carried out again to evaluate the changes in meaning between the original version and the results of the translation. The next process is a pilot study and validity test with content validity while the reliability test with internal consistency (Cronbach's alpha). RESULTS There were no significant changes from the original instrument although there were some differences in the choice of word usage in the SCSES instrument. The Content Validity Index (CVI) was obtained at 0.949. The results of the internal consistency test were obtained at 0.825. CONCLUSION The Indonesian version of the SCSES is declared as a valid and reliable instrument to be applied to the ostomate. SCSES can identify the level of self-efficacy of ostomate on the ability to treat stoma. RESULTS The Cohen's kappa revealed that the inter-rater reliability for depth (0.76-0.80), size (0.80-0.86), size score (0.87-0.89), inflammation/infection (0.69-0.75), proportion of granulation tissue (0.73-0.79), type of necrotic tissue (0.65-0.73), proportion of necrotic tissue (0.87-0.89), proportion of slough (0.58-0.65), maceration (0.79-0.85), type of wound edge (0.91-0.93), and tunneling (0.90-0.91). CONCLUSION This study confirmed DFUAS has an adequate reliability between different wound nurses (inter-rater reliability). OBJECTIVE To observe the influence of demonstration methods education on the knowledge of tooth brushing in primary school children in the 10-12 years age group. METHODS The type of research used in this study is quasi-experimental research design with One-Group Pre-Test-Post Test Design. This research uses cross-sectional. RESULTS Showed an increase in the level of knowledge of tooth brushing in students aged 10-12 years after education with the demonstration method, where for pre-treatment students were still found with a sufficient level of knowledge. CONCLUSION Education with the demonstration method influenced the knowledge of tooth brushing in elementary school children in the 10-12 year age group. METHOD This study used the Interpretative Structural Modeling Analysis (ISM) model, which was used to analyze complex matters of a system through patterns designed using graphics. This analysis model was conducted through the stages, namely (1) compiling a structural interaction matrix, (2) collecting a reachability matrix table, (3) compiling a power-dependent matrix driver, and (4) composing a structural model of each sub-element. RESULTS The result showed that 2 out of 10 sub-elements had DP value equal to 1.00, which were the growth of sectoral-centric attitudes and weak inter-institutional commitment. While among institutions, there were three of 11 institutions that play a crucial role in reducing DHF, namely the Office of Health, Community health centers, and hospitals (DP=1.00). CONCLUSION It can be concluded that strengthening the institutional system can help in reducing cases of dengue hemorrhagic fever (DHF).

  • A direct coupling of arylboronic acids with allylic fluorides was carried out in water without additives using a rhodium(III) catalyst (Cp*RhCl2)2. The transformation proceeded with excellent γ-selectivity to afford major allyl-aryl coupling products (Z) γ-substituted α,β-unsaturated amides. The reactions of α-chiral allylic fluorides took place with excellent α-to-γ chirality transfer to give allylated arenes with a stereogenic center at the benzylic and allylic position.The stability and the structure of adducts formed between four substituted phosphanes (PX3, XH, F, Cl, and NMe2) and 11 different carbenes have been investigated by DFT calculations. In most cases, the structure of the adducts depends strongly on the stability of the carbene itself, exhibiting a linear correlation with the increasing dissociation energy of the adduct. Carbenes of low stability form phosphorus ylides (F), which can be described as phosphane → carbene adducts supported with some ****-bonding. The most stable carbenes, which have high energy lone pair, do not form stable F-type structures but carbene → phosphane adducts (E-type structure), utilizing the low-lying lowest unoccupied molecular orbital (LUMO) of the phosphane (with electronegative substituents), benefiting also from the carbene-pnictogen interaction. Especially noteworthy is the case of PCl3, which has an extremely low energy LUMO in its T-shaped form. Although this PCl3 structure is a transition state of rather high energy, the large stabilization energy of the complex makes this carbene-phosphane adduct stable. Most interestingly, in case of carbenes with medium stability both F- and E-type structures could be optimized, giving rise to bond-stretch isomerism. Likewise, for phosphorus ylides (F), the stability of the adducts G formed from carbenes with hypovalent phosphorus (PX-phosphinidene) is in a linear relationship with the stabilization of the carbene. Proteome-wide comparisons of lysine methylation patterns reveal that DTPs display an increase in methylation on K116 of PRC member Jarid2, an event that helps stabilize and recruit PRC2 to chromatin. We also find that EZH2, in addition to methylating histone H3K27, also can methylate G9a at K185, and that methylated G9a better recruits repressive complexes to chromatin. These complexes are similar to complexes recruited by histone H3 methylated at K9. Finally, a detailed histone post-translational modification (PTM) analysis shows that EZH2, either directly or through its ability to methylate G9a, alters H3K9 methylation in the context of H3 serine 10 phosphorylation, primarily in a cancer cell subpopulation that serves as DTP precursors. This work presents a combination strategy of pretreatment of MDA-MB-231/MDR1 cells with quercetin (QU) followed by doxorubicin (DOX) to overcome MDR, which can be delivered by mixed micelles composed of the reduction-sensitive hyaluronic acid-based conjugate and d-α-tocopheryl poly(ethylene glycol) 1000 succinate. The combination strategy can enhance the cytotoxicity of DOX on MDA-MB-231/MDR1 cells by increasing intracellular DOX accumulation and facilitating DOX-induced apoptosis. The probable MDR reversal mechanisms are that the pretreatment cells with QU-loaded mixed micelles downregulate P-glycoprotein expression to decrease DOX efflux as well as initiate mitochondria-dependent apoptotic pathways to accelerate DOX-induced apoptosis.     A direct coupling of arylboronic acids with allylic fluorides was carried out in water without additives using a rhodium(III) catalyst (Cp*RhCl2)2. The reactions of α-chiral allylic fluorides took place with excellent α-to-γ chirality transfer to give allylated arenes with a stereogenic center at the benzylic and allylic position.The stability and the structure of adducts formed between four substituted phosphanes (PX3, XH, F, Cl, and NMe2) and 11 different carbenes have been investigated by DFT calculations. In most cases, the structure of the adducts depends strongly on the stability of the carbene itself, exhibiting a linear correlation with the increasing dissociation energy of the adduct. Carbenes of low stability form phosphorus ylides (F), which can be described as phosphane → carbene adducts supported with some back-bonding. The most stable carbenes, which have high energy lone pair, do not form stable F-type structures but carbene → phosphane adducts (E-type structure), utilizing the low-lying lowest unoccupied molecular orbital (LUMO) of the phosphane (with electronegative substituents), benefiting also from the carbene-pnictogen interaction. Especially noteworthy is the case of PCl3, which has an extremely low energy LUMO in its T-shaped form.     A higher incidence of venous thromboembolism [both pulmonary embolism and deep vein thrombosis (DVT)] in patients with coronavirus disease 2019 (COVID-19) has been described. But little is known about the true frequency of DVT in patients who attend emergency department (ED) and are diagnosed with COVID-19.

    We investigated the incidence, risk factors, clinical characteristics and outcomes of DVT in patients with COVID-19 attending the ED before hospitalization.

    We retrospectively reviewed all COVID patients diagnosed with DVT in 62 Spanish EDs (20% of Spanish EDs, case group) during the first 2 months of the COVID-19 outbreak. We compared DVT-COVID-19 patients with COVID-19 without DVT patients (control group). Relative frequencies of DVT were estimated in COVID and non-COVID patients visiting the ED and annual standardized incidences were estimated for both populations. Sixty-three patient characteristics and four outcomes were compared between cases and controls.

Once considered an uncommon complication, its diagnosis has increased due to increased attention and advances in medical technology. Common symptoms of the syrinx are a sensory loss of the dissociated type with pain and temperature loss and the preservation of fine touch and vibratory sensation. Eventually, a deterioration of motor function with muscle wasting may occur. CASE PRESENTATION We present the case of a 36-year-old woman who sustained a sport accident in 1996, resulting in AIS A, T7 paraplegia. She underwent a magnetic resonance imaging (MRI) examination because of neck and left shoulder pain that resolved after a short anti-inflammatory treatment. The MRI showed a large cavity involving the cord beneath T6 and the medulla. Septations were present at both the spinal cord and medulla levels. With regard to vertebral status, the MRI showed the presence of severe kyphosis at the fracture level together with spinal cord compression. The neurological examination was normal except for the pre-existing paraplegia and of a slight heat and pain sensation deficit in the C8 dermatome. DISCUSSION We discuss the need of regular follow-up examinations as even large syrines with involvement of the brainstem may be asymptomatic. We also discuss the possible pathogenetic factors including the type of treatment of the vertebral lesion.The intrinsic apoptotic pathway is controlled by the BCL-2 family of proteins, which exhibit either a pro-death or pro-survival function. The weights of several tissues including the heart, tibialis anterior, and kidney were likewise reduced in Mcl-1+/-Bcl-2+/- ****. Yet, during modelling and optimization of these devices, as well as in clinical practice, vascular sections are idealized, possibly compromising the effectiveness of the intervention. In this study, we challenge the commonly used approximation of the circular cross-section of the aorta and identify the implications of this approximation to the mechanical assessment of stent grafts. Using computed tomography angiography (CTA) data from 258 AAA patients, the lumen of the aneurysmal neck was analysed. The cross-section of the aortic neck was found to be an independent variable, uncorrelated to other geometrical aspects of the region, and its shape was non-circular reaching elliptical ratios as low as 0.77. These results were used to design a finite element analysis (FEA) study for the assessment of a ring stent bundle deployed under a variety of aortic cross-sections. Results showed that the most common clinical approximations of the vascular cross-section can be a source of significant error when calculating the maximum stent strains (underestimated by up to 69%) and radial forces (overestimated by up to 13%). Nevertheless, a less frequently used average approximation was shown to yield satisfactory results (5% and 2% of divergence respectively).We applied continuous intraoperative neuromonitoring (CIONM) during video-assisted thoracoscopic surgery (VATS) lobectomy for left lung cancer and evaluated its safety and usefulness. An electrode was attached to a double-lumen tube, and placed at vocal cord level to detect the EMG signal evoked by vocal cord movement. Before 4 L lymph node dissection, an automatic periodic stimulation device was applied to the vagus nerve to stimulate vagus nerve continuously. Surgery was suspended if the amplitude decreased lower than the threshold and was resumed when the amplitude recovered. Ten patients (6 male, 4 female) were enrolled. CIONM was successfully performed in all patients without technical failure, and there was no hemodynamic instability. Amplitude decreased below the threshold in four patients. One patient did not recover amplitude and experienced transient vocal cord palsy. In the three other patients, the amplitude recovered above the threshold and no vocal cord palsy occurred. The six patients who did not exhibit amplitude decrease experienced no vocal cord palsy.

The neurological examination was normal except for the pre-existing paraplegia and of a slight heat and pain sensation deficit in the C8 dermatome. Body composition analysis demonstrated that while fat mass was similar to wild-type controls, lean mass was significantly reduced in Mcl-1+/-, Bcl-2+/-, and, most strikingly in Mcl-1+/-Bcl-2+/- mice. The weights of several tissues including the heart, tibialis anterior, and kidney were likewise reduced in Mcl-1+/-Bcl-2+/- mice. Here, we have investigated the functional significance of this accumulation at a genome-wide level. Depletion of DAZL causes a block in maturation and widespread disruption in the pattern of ribosome loading on maternal transcripts. In addition to decreased translation, DAZL depletion also causes translational activation of a distinct subset of mRNAs both in quiescent and maturing oocytes, a function recapitulated with YFP-3'UTR reporters. DAZL binds to mRNAs whose translation is both repressed and activated during maturation. Injection of recombinant DAZL protein in DAZL-depleted oocytes rescues the translation and maturation to MII. Mutagenesis of putative DAZL-binding sites in these mRNAs mimics the effect of DAZL depletion. These findings demonstrate that DAZL regulates translation of maternal mRNAs, functioning both as the translational repressor and activator during oocyte maturation.Stent graft devices for the treatment of abdominal aortic aneurysms (AAAs) are being increasingly used worldwide. Yet, during modelling and optimization of these devices, as well as in clinical practice, vascular sections are idealized, possibly compromising the effectiveness of the intervention. In this study, we challenge the commonly used approximation of the circular cross-section of the aorta and identify the implications of this approximation to the mechanical assessment of stent grafts. Using computed tomography angiography (CTA) data from 258 AAA patients, the lumen of the aneurysmal neck was analysed. The cross-section of the aortic neck was found to be an independent variable, uncorrelated to other geometrical aspects of the region, and its shape was non-circular reaching elliptical ratios as low as 0.77. These results were used to design a finite element analysis (FEA) study for the assessment of a ring stent bundle deployed under a variety of aortic cross-sections. Results showed that the most common clinical approximations of the vascular cross-section can be a source of significant error when calculating the maximum stent strains (underestimated by up to 69%) and radial forces (overestimated by up to 13%). Nevertheless, a less frequently used average approximation was shown to yield satisfactory results (5% and 2% of divergence respectively).We applied continuous intraoperative neuromonitoring (CIONM) during video-assisted thoracoscopic surgery (VATS) lobectomy for left lung cancer and evaluated its safety and usefulness. An electrode was attached to a double-lumen tube, and placed at vocal cord level to detect the EMG signal evoked by vocal cord movement. Before 4 L lymph node dissection, an automatic periodic stimulation device was applied to the vagus nerve to stimulate vagus nerve continuously. Surgery was suspended if the amplitude decreased lower than the threshold and was resumed when the amplitude recovered. Ten patients (6 male, 4 female) were enrolled. CIONM was successfully performed in all patients without technical failure, and there was no hemodynamic instability. Amplitude decreased below the threshold in four patients. One patient did not recover amplitude and experienced transient vocal cord palsy. In the three other patients, the amplitude recovered above the threshold and no vocal cord palsy occurred. The six patients who did not exhibit amplitude decrease experienced no vocal cord palsy.
