  • The goal of this Views and Interviews series was to bring together the thought leaders in the field and envision what the laboratory will look like in the future. This consensus piece strives to take the thoughts of those leaders and develop themes and concepts that will be significant in the laboratory in the coming years.Non-Indigenous older Australians (aged 75+ years) are encouraged to undertake an annual health assessment (75+HA) to identify overlooked medical conditions, holistically assess the management of current chronic conditions and highlight potential risk factors for poor health. Uptake in the 2000s has been previously reported to be slowly increasing, but still very low. This study aimed to identify the uptake of 75+HA between 2011 and 2019, comparing State and National trends over a 9-year period.     We conducted semi-structured in-depth interviews with 20 clients in two Chinese cities in 2018. Participants were recruited through purposive sampling. The content of the interviews was analysed using thematic content analysis in ATLAS.ti.

    Overall, just a minority of participants were aware of nPEP. A majority expressed willingness to use nPEP. Potential barriers to nPEP uptake and adherence included adverse drug reactions, price, concerns of drug efficacy, privacy issues, and forgetting to take the drugs. Almost all participants expressed the need to promote nPEP among clients. Participants suggested that the promotion of nPEP should be at hospitals, online, and be integrated with HIV/AIDS health education.

    Our findings suggested that nPEP guidelines should be formulated and implementation strategies should be developed to address barriers to uptake and adherence in order to successfully tap into the potential of nPEP as an effective HIV prevention tool.

    Among 1348 HIV-infected male patients who were HCV seronegative at baseline, 64 (4.7%) subsequently tested positive for HCV, giving an incidence of 0.88 per 100 person-years of follow-up (PYFU) (95% confidence interval (CI) 0.69-1.13). The incidence rate of HCV seroconversion increased from 0.33 (95% CI 0.12-0.71) per 100 PYFU in 2010-2012 to 1.93 (95% CI 1.36-2.67) in 2016-2018. Independent factors associated with incident HCV infection were younger age groups at HIV diagnosis versus ≥45 years, HIV acquisition via MSM or via both sexual contact and intravenous drug use versus heterosexual transmission, HIV diagnosis in later periods versus 2006-2009, and recent syphilis acquisition.

Use of contraceptives and HIV testing in Thailand was also investigated to understand if this influences utilisation of SRH services in Sweden.
    A cross-sectional study using postal questionnaire to all Thai-born women (age 23-60) in Stockholm, residing in Sweden since 2014. The response rate was 52.3% (n= 266). Bivariate and multivariate logistic regression analyses were used.

    The majority reported a significant need for information related to SRH services. Most of the women using contraception (70%) bought their contraceptives in Thailand. In total, 60% of the women had been HIV-tested at some point in their lives; the majority were tested in Thailand. Women who had been HIV-tested in Thailand were more likely to also have been tested in Sweden.

  • The formation of transient pores in their membranes is a well-known mechanism of permeabilization of cells exposed to high-intensity electric pulses. However, the formation of such pores is not able to explain all aspects of the so-called electroporation phenomenon. In particular, the reasons for sustained permeability of cell membranes, persisting long after the pulses' application, remain elusive. The complete resealing of cell membranes takes indeed orders of magnitude longer than the time for electropore closure as reported from molecular dynamics (MD) investigations. Lipid peroxidation has been suggested as a possible mechanism to explain the sustainable permeability of cell membranes. However, theoretical investigations of membrane lesions containing excess amounts of hydroperoxides have shown that the conductivities of such lesions were not high enough to account for the experimental measurements. Here, expanding on these studies, we investigate quantitatively the permeability of cell membrane lesions that underwent secondary oxidation. MD simulations and free energy calculations of lipid bilayers show that such lesions provide a better model of post-pulse permeable and conductive electropermeabilized cells. These results are further discussed in the context of sonoporation and ferroptosis, respectively a procedure and a phenomenon, among others, in which, alike electroporation, substantial lipid oxidation might be triggered.Noninvasive EEG signals provide neural activity information at high resolution, of which human mental status can be properly detected. However, artefacts always exist in brain oscillatory EEG signals and thus impede the accuracy and reliability of relevant analysis, especially in real-world tasks. Moreover, the use of a mature artefact identification method cannot assure impeccable artefact separation; this leads to a trade-off between removing contaminated information and losing valuable information. This study addresses this problem by investigating a simulator-based driving behaviour analysis using a car-following scenario to correlate the EEG-based mental features with behavioural responses. The study develops an architecture for an artefact composition pool and proposes three integrated prediction models to evaluate the removal compositions of the EEG artefacts. Three errors (mean absolute, root mean square, mean absolute percentage) and R-squared index are considered for measuring the performance of the models. The results show that the best-performing composition outperformed the no-removal and all-removal cases by 11.75% and 4.28% improvements, respectively. Specifically, we investigate different common artefacts including eye blinks, horizontal eye movements, vertical eye movements, generic discontinuities and muscle artefacts. The gained knowledge on artefact removal, EEG spectral features and stimuli-response patterns can be further applied to properly manipulate real-world EEG signals and develop an effective brain-computer interface.As the era of intelligent connected vehicles (ICVs) is approaching, a number of studies have investigated the potential benefits of ICVs, including the safety effects. Although previous studies agree that ICVs would significantly improve traffic safety, its quantified safety effects at different stages are still being debated. This study aims to estimate the ICVs' safety effects by market penetration rate (MPR) adopting a meta-analysis approach. The results from the meta-analysis indicate that the number of conflicts is exponentially reduced as the MPR goes up. For example, compared to the environment without ICVs, 4.2% and 17.4% of conflicts would decrease at the MPR of 10% and 50%, respectively. The effects are more obvious at higher MPR-43.4% of conflicts are expected to decrease at the MPR of 90%. From the case study in the United States based on the meta-analysis, it is expected that the MPR would reach 17-20% in the near future (2025) and 40-48% in 2035. The anticipated reduction in the number of fatal crashes would be 5% and 13%, in 2025 and 2035, respectively. The findings from this study will be useful for both public and private sectors to establish strategic plans to promote ICVs and identify their benefits at different MPRs.One challenge faced by the random-parameter count models for crash prediction is the unavailability of unique coefficients for out-of-sample observations. The means of the random-parameter distributions are typically used without explicit consideration of the variances. In this study, by virtue of the Taylor series expansion, we proposed a straightforward yet analytic solution to include both the means and variances of random parameters for unbiased prediction. We then theoretically quantified the systematic bias arising from the omission of the variances of random parameters. Eleven patients had surgeries at The University of Texas MD Anderson Cancer Center. Four additional cases were provided from an external collaborator. Of the 15 cases, there were 5 male and 10 female patients, with a median age of 52 years (mean 54 years, range 28-84 years). Clinical presentation included adrenal incidentaloma (n = 9), Cushing syndrome (n = 4, including 1 as a part of Carney complex), and subclinical Cushing syndrome (n = 2, including 1 with bilateral macronodular adrenal hyperplasia).

  • COVID-19 will undoubtedly change the future landscape of medical and surgical education. The economic and environmental advantages of virtual learning are clear, while access to a wider range of resources and subject specialists makes the adoption of virtual learning within surgical education an attractive prospect.

    This literature review aims to evaluate evidence on the effectiveness of virtual education in orthopaedics and how we might implement positive changes to educational practice in the future, as a result of lessons learned during the COVID-19 pandemic.

    We performed a review of the literature reporting on efficacy of learning outcomes achieved as a result of virtual education within orthopaedic surgery. Electronic searches were performed using NICE healthcare databases from the date of inception to March 2021. Relevant studies were identified, data extracted, and qualitative synthesis performed.

    14 manuscripts with a total of 1548 participants (orthopaedic trainees or medical students) were iner in mind, and an awareness of best practice for virtual teaching and learning.

This study aimed to measure overall levels of grit among hospital doctors and medical students and to compare levels of Grit across specialties and career grades.

    ology A cross-sectional survey was used to measure GRIT levels using the validated Short Grit Scale (GRIT-S). Hospital doctors and medical students at University Hospital Galway were asked to complete the questionnaire. Gender, age, grade, education, and speciality were recorded. We provide practical strategies to empower learners with the necessary skills to solicit, receive, and reflect on feedback.We conducted a meta-analysis of IOTA (international ovarian tumor analysis) ADNEX (Assessment of Different NEoplasias in the adneXa) as ultrasound system and MRI (magnetic resonance imaging) ADNEX scoring systems as MR system to assess their diagnostic test accuracy for differentiating benign from malignant adnexal masses of the ovary. We performed an electronic search for relevant publications in the English language up to February 2021 using PubMed, CENTRAL (Cochrane Central Register of Controlled Trials), Web of Science, and Google scholar databases and search engines. We computed the pooled sensitivity, pooled specificity, and summary receiver operating characteristics curve (SROC) using the statistical software STATA (Version 13, College Station, TX, StataCorp LP). Based on 11 studies using IOTA-ADNEX, we observed pooled sensitivity, specificity, area under curve, and diagnostic odds ratio were 96% (95% CI, 94% to 97%), 79% (95% CI, 70% to 86 %), 97% (95% CI, 95% to 98%), and 88 (95% CI, 43 to 180). Based on five studies using MR-ADNEX scoring system the pooled sensitivity, specificity, area under curve and diagnostic odds ratio were 91 % (95% CI, 87% to 94 %), 95% (95% CI, 92% to 97 %), 98% (95% CI, 96% to 99%), and 189 (95% CI, 90 to 396) respectively. Our meta-analysis results demonstrate that the MR-ADNEX scoring system had higher specificity however bit lower sensitivity compared to the IOTA-ADNEX scoring system for discriminating benign from malignant ovarian tumors.
    The Covid-19 pandemic ushered a sudden need for residency programs to develop innovative socially distant and remote approaches to effectively promote their program. Here we describe our experience using the social virtual reality (VR) platform Mozilla Hubs for the pre-interview social during the 2020-2021 radiology residency virtual recruitment season, provide results of a survey sent to assess applicants' attitudes towards the VR pre-interview social, and outline additional use-cases for the emerging technology.

    COVID-19 will undoubtedly change the future landscape of medical and surgical education. The economic and environmental advantages of virtual learning are clear, while access to a wider range of resources and subject specialists makes the adoption of virtual learning within surgical education an attractive prospect. This literature review aims to evaluate evidence on the effectiveness of virtual education in orthopaedics and how we might implement positive changes to educational practice in the future, as a result of lessons learned during the COVID-19 pandemic. We performed a review of the literature reporting on efficacy of learning outcomes achieved as a result of virtual education within orthopaedic surgery. Electronic searches were performed using NICE healthcare databases from the date of inception to March 2021. Relevant studies were identified, data extracted, and qualitative synthesis performed. 14 manuscripts with a total of 1548 participants (orthopaedic trainees or medical students) were iner in mind, and an awareness of best practice for virtual teaching and learning. We provide practical strategies to empower learners with the necessary skills to solicit, receive, and reflect on feedback.We conducted a meta-analysis of IOTA (international ovarian tumor analysis) ADNEX (Assessment of Different NEoplasias in the adneXa) as ultrasound system and MRI (magnetic resonance imaging) ADNEX scoring systems as MR system to assess their diagnostic test accuracy for differentiating benign from malignant adnexal masses of the ovary. We performed an electronic search for relevant publications in the English language up to February 2021 using PubMed, CENTRAL (Cochrane Central Register of Controlled Trials), Web of Science, and Google scholar databases and search engines. We computed the pooled sensitivity, pooled specificity, and summary receiver operating characteristics curve (SROC) using the statistical software STATA (Version 13, College Station, TX, StataCorp LP). Based on 11 studies using IOTA-ADNEX, we observed pooled sensitivity, specificity, area under curve, and diagnostic odds ratio were 96% (95% CI, 94% to 97%), 79% (95% CI, 70% to 86 %), 97% (95% CI, 95% to 98%), and 88 (95% CI, 43 to 180).   • Schizophrenia (SZ) is typically preceded by a prodromal (i.e. pre-illness) period characterized by attenuated positive symptoms and declining functional outcome. Negative symptoms are prominent among individuals at clinical high-risk (CHR) for psychosis (i.e. those with prodromal syndromes) and predictive of conversion to illness. Mechanisms underlying negative symptoms are unclear in the CHR population.

    The current study evaluated whether CHR participants demonstrated deficits in the willingness to expend effort for rewards and whether these impairments are associated with negative symptoms and greater risk for conversion. Participants included 44 CHR participants and 32 healthy controls (CN) who completed the Effort Expenditure for Reward Task (EEfRT).

    Compared to CN, CHR participants displayed reduced likelihood of exerting high effort for high probability and magnitude rewards. Among CHR participants, reduced effort expenditure was associated with greater negative symptom severity and greater probability of conversion to a psychotic disorder on a cross-sectional risk calculator.

    This study aimed to explore the spatial distribution of brain metastases (BMs) from small cell lung cancer (SCLC) a homogenous sample, and to identify the metastatic risk levels in brain regions.

Wind assistance and thermal uplift are thought to reduce the energetic costs of flight, humidity, air pressure and temperature may affect the migrants' water balance, and clouds may impede navigation. Recent advances in animal-borne long-distance tracking enable evaluating the importance of these factors in determining animals' flight altitude.

    Here we determine the effects of wind, humidity, temperature, cloud cover, and altitude (as proxy for climbing costs and air pressure) on flight altitude selection of two long-distance migratory shorebirds, far eastern curlew (Numenius madagascariensis) and whimbrel (Numenius phaeopus). To reveal the predominant drivers of flight altitude selection during migration we compared the atmospheric conditions at the altitude the birds were found flying with conditions elsewhere in the air column using conditional logistic mixed effect models.

    Our results demonstrate thortant secondary factor. Our approach and findings can assist in predicting climate change effects on migration and in mitigating bird strikes with air traffic, wind farms, power lines, and other human-made structures.

Our aim was to compare Particle Enhanced Immunoturbidimetry (PEIT) method with High Performance Liquid Chromatography (HPLC) for HbA1c analysis.

    All blood samples were analyzed by HPLC assay on a Bio-Rad D-10 analyzer and PEIT on an Erba XL-200 analyzer. Precision studies were undertaken and Coefficient of Variation (%CV) calculated. Systemic Error (SE), Random Error (RE) and Total Error (TE

  • The formation of transient pores in their membranes is a well-known mechanism of permeabilization of cells exposed to high-intensity electric pulses. However, the formation of such pores is not able to explain all aspects of the so-called electroporation phenomenon. In particular, the reasons for sustained permeability of cell membranes, persisting long after the pulses' application, remain elusive. The complete resealing of cell membranes takes indeed orders of magnitude longer than the time for electropore closure as reported from molecular dynamics (MD) investigations. Lipid peroxidation has been suggested as a possible mechanism to explain the sustainable permeability of cell membranes. However, theoretical investigations of membrane lesions containing excess amounts of hydroperoxides have shown that the conductivities of such lesions were not high enough to account for the experimental measurements. Here, expanding on these studies, we investigate quantitatively the permeability of cell membrane lesions that underwent secondary oxidation. MD simulations and free energy calculations of lipid bilayers show that such lesions provide a better model of post-pulse permeable and conductive electropermeabilized cells. These results are further discussed in the context of sonoporation and ferroptosis, respectively a procedure and a phenomenon, among others, in which, alike electroporation, substantial lipid oxidation might be triggered.Noninvasive EEG signals provide neural activity information at high resolution, of which human mental status can be properly detected. However, artefacts always exist in brain oscillatory EEG signals and thus impede the accuracy and reliability of relevant analysis, especially in real-world tasks. Moreover, the use of a mature artefact identification method cannot assure impeccable artefact separation; this leads to a trade-off between removing contaminated information and losing valuable information. This study addresses this problem by investigating a simulator-based driving behaviour analysis using a car-following scenario to correlate the EEG-based mental features with behavioural responses. The study develops an architecture for an artefact composition pool and proposes three integrated prediction models to evaluate the removal compositions of the EEG artefacts. Three errors (mean absolute, root mean square, mean absolute percentage) and R-squared index are considered for measuring the performance of the models. The results show that the best-performing composition outperformed the no-removal and all-removal cases by 11.75% and 4.28% improvements, respectively. Specifically, we investigate different common artefacts including eye blinks, horizontal eye movements, vertical eye movements, generic discontinuities and muscle artefacts. The gained knowledge on artefact removal, EEG spectral features and stimuli-response patterns can be further applied to properly manipulate real-world EEG signals and develop an effective brain-computer interface.As the era of intelligent connected vehicles (ICVs) is approaching, a number of studies have investigated the potential benefits of ICVs, including the safety effects. Although previous studies agree that ICVs would significantly improve traffic safety, its quantified safety effects at different stages are still being debated. This study aims to estimate the ICVs' safety effects by market penetration rate (MPR) adopting a meta-analysis approach. The results from the meta-analysis indicate that the number of conflicts is exponentially reduced as the MPR goes up. For example, compared to the environment without ICVs, 4.2% and 17.4% of conflicts would decrease at the MPR of 10% and 50%, respectively. The effects are more obvious at higher MPR-43.4% of conflicts are expected to decrease at the MPR of 90%. From the case study in the United States based on the meta-analysis, it is expected that the MPR would reach 17-20% in the near future (2025) and 40-48% in 2035. The anticipated reduction in the number of fatal crashes would be 5% and 13%, in 2025 and 2035, respectively. The findings from this study will be useful for both public and private sectors to establish strategic plans to promote ICVs and identify their benefits at different MPRs.One challenge faced by the random-parameter count models for crash prediction is the unavailability of unique coefficients for out-of-sample observations. The means of the random-parameter distributions are typically used without explicit consideration of the variances. In this study, by virtue of the Taylor series expansion, we proposed a straightforward yet analytic solution to include both the means and variances of random parameters for unbiased prediction. We then theoretically quantified the systematic bias arising from the omission of the variances of random parameters. Although the improvements in abdominal imaging have increased identification of myelolipomatous adrenal cortical adenomas radiologically, due to the rarity of this lesion, the clinical pathological features of these lesions are unclear and can sometimes cause diagnostic difficulty. Eleven patients had surgeries at The University of Texas MD Anderson Cancer Center. Four additional cases were provided from an external collaborator. Of the 15 cases, there were 5 male and 10 female patients, with a median age of 52 years (mean 54 years, range 28-84 years). The results from the meta-analysis indicate that the number of conflicts is exponentially reduced as the MPR goes up. For example, compared to the environment without ICVs, 4.2% and 17.4% of conflicts would decrease at the MPR of 10% and 50%, respectively.     Anxious-depressive attack (ADA) is a symptom complex that comprises sudden intense feelings of anxiety or depression, intrusive rumination of regretful memories or future worries, emotional distress due to painful thoughts, and coping behaviors to manage emotional distress. Although the importance of ADA treatment has been indicated, a scale to measure the severity of ADA has not been developed. We then employed these risk factors to construct and validate a nomogram for predicting mandatory ICU admission after gastrectomy, which may identify those who require ICU indeed and improve ICU utilization.

    A number of 999 gastric cancer patients undergoing gastrectomy from January 2010 to June 2019 were included in the retrospective study. Forty-three patients were classified into mandatory ICU admission groups, and the remaining 956 patients were allocated into the no need for ICU admission group. The candidate variables, including patient demographic characteristics, preoperative laboratory tests and surgical variables, were compared between the two groups. We then carried out univariate and multivariate logistic regression analyses to find out risk factors for mandatory ICU admission. In order to develop the predictive model, we used Akaike informatioedict mandatory ICU admission after gastrectomy for gastric cancer was constructed and validated. Clinicians could apply this predictive model to improve usage of limited ICU resources effectively.

    The databases PubMed, Embase, Medline, Ovid, and Springer were searched to retrieve randomized controlled trials (RCTs) on KOA treatment by acupotomy combined with acupuncture and moxibustion. The search time was set as from the date the database was established to 31 December 2020. The Cochrane Handbook for Systematic Reviews of Intervention 5.0.2 was used to conduct bias risk assessment on the included literature, and Review Manager 5.3 software was used for meta-analysis.

    A total of 10 RCTs were included in this study, including 1,073 participants. Meta-analysis results showed that compared with the control group, the clinical treatment efficiency of the experimental group was higher [mean difference (MD) =5.72; 95% confidence interval (CI) 3.39 to 9.64; Z=6.54; P<0.00001], and the postoperative visual analogue scale (VAS) scores were reduced (MD =-1.72; 95% CI -2.41 to -1.03; Z=4.86; P<0.00001).

Four undescribed pyrazinopyrimidine-type alkaloids, including three natural products pyrasplorines A-C and an artifact deg-pyrasplorine B, as well as a biogenetically related versicoloid A were discovered from the extract of a mangrove-derived fungus Apergillus verisicolor HDN11-84. Pyrasplorine A contains unique spiral-type skeleton (composed of cyclopentenone ring with the pyrazino[1,2-a] pyrimidine core) which is unprecedented in pyrazinopyrimidine-type alkaloids. The deg-pyrasplorine B could be spontaneously converted from pyrasplorine B in mild conditions. Their structures including absolute configurations were elucidated based on NMR spectroscopic analysis, computational calculations and Marfey's method. The absolute configuration of versicoloid A was re-assigned in this study.     Fifteen individuals after stroke and 17 healthy participants were assessed with an instrumented exerciser paired with an instrumented sitting surface while they executed isometric bilateral pushes with the upper limbs. The anteroposterior, vertical and mediolateral forces were recorded at the handles, the thighs and the feet. Force values at maximal bilateral pushing efforts at each segment and inter-limb coordination between sides were compared.

    During the isometric pushes, the paretic maximal forces at the handles for stroke participants were lower than the nonparetic side and lower than both sides of the control participants (p<0.036). The control and stroke participants had moderate to good coordination for the anteroposterior forces (hands and thighs). While they used similar postural strategies to the controls except for a decreased weight on the paretic foot, vertical forces were less coordinated at the handles and feet in the stroke group (p<0.050). The inter-trial variability was also higher in the stroke group.

    Around half of anterior cruciate ligament (ACL) injuries are treated through reconstruction, but the literature lacks mechanical investigation of reconstructions in a dynamic athletic task and rupture environment. The current objective was to ascertain the feasibility of investigating ACL reconstructions in a rupture environment during simulated landing tasks in a validated mechanical impact simulator.

    Four cadaveric lower extremities were subjected to simulated landing in a mechanical impact simulator. External joint loads that mimicked magnitudes recorded from an in vivo population were applied to each joint in a stepwise manner. Simulations were repeated until ACL failure was achieved. Repeated measures design was used to test each specimen in the native ACL and hamstrings, quadriceps, and patellar tendon reconstructed states.

    ACL injuries were generated in 100% of specimens. Graft substance damage occurred in 58% of ACLRs, and in 75% of bone tendon bone grafts. Bone tendon bone and quadriceps grafts survived greater simulated loading than hamstrings grafts, but smaller simulated loading than the native ACL.
