The patient had no repetitive CMAP. He had a dramatic response to fluoxetine at low-moderate doses (40 mg/day), increasing over months Being wheelchair bound, he could walk independently after treatment. Rare cases may offer insight into the pathological gating mechanism leading to CMS. SCCMS should be suspected even without a repetitive CMAP. Fluoxetine at relatively low doses can be a very effective treatment.
Skeletal muscle injuries are frequent clinical challenges due to associated fibrosis and disability. Regenerative medicine is an emerging promising strategy for such cases. Articular cartilage repair has been a challenge in orthopedic practice due to the limited self-regenerative capability. Optimal treatment method for cartilage defects has not been defined. We investigated the effect of decellularized human placental (DHP) scaffold, mesenchymal stem cells (MSC) and platelet-rich plasma (PRP) on hyaline cartilage regeneration in a rat model.
An osteochondral defect was created in trochlea region of the femur in all groups, bilaterally. No additional procedure was performed in control group (n = 14). Only the DHP scaffold was applied to the P group (n = 14). The DHP scaffold and 1 × 10

The evidence on these two agents has been studied through different methods of analysis that span from narrative approaches to network meta-analysis. In the present study, we assessed the performance of a narrative approach combined with the application of the restricted mean survival time (RMST).
We studied the two pivotal placebo-controlled trials focused on evolocumab and alirocumab. Our original framework of comparative assessment employed the RMST. Our objective was to show that in the context of a narrative review, the RMST can be an efficient although simple tool to make indirect comparisons. The endpoint was event-free survival, expressed in months.

Through their lifetimes, a total of 98 HCM-related morbid events occurred in 72 patients. Survival analysis revealed that patients with sarcomere gene mutations experienced those morbid events significantly more frequently, and this tendency was more prominent for lethal arrhythmic events.

We prepared a diet containing mulberry leaves powder and soybean flour at different ratios, fed them to fifth instar silkworm larvae, and observed their growth. We selected the diet with 11 ratio of mulberry powder and soybean flour, named MS-11, and used for further experiments. MS-11 diet was available for oral administration of drugs in silkworm hyperglycemic model and infection model. The availability of a simple artificial diet for experiments that require feeding silkworms will enhance the use of silkworms for biological, biotechnological, and pharmacological researches.The management of neurological infections due to non-tubercular mycobacteria is extremely challenging because of scarce literature, issues with penetration, lack of easily available susceptibility platforms and adverse effects associated with long term therapy. We report a case of a young girl with neurological infection due to rapidly growing mycobacteria to discuss the factors that should be considered while choosing the therapy for such rare and persistent infections.
The COVID-19 has spread to more than 200 countries and territories. But less is known about the knowledge, protection behavior and anxiety regarding the outbreak among the general population.
A cross-sectional, population-based online survey was conducted in China and abroad from January 28 to February 1, 2020. Socio-demographic information was collected and knowledge scores, practice scores, anxiety scores and perceived risk were calculated. General linear model and binary logistic regression were used to identify possible associations.
We included 9,764 individuals in this study, and 156 (1.6%) were from Hubei Province. The average knowledge score was 4.7 (standard deviation, 1.0) (scored on a 6-point scale); 96.1% maintained hand hygiene, and 90.3% of participants had varying levels of anxiety. People in Hubei Province were the most anxious, followed by those in Beijing and Shanghai. People who had experienced risk behaviors did not pay more attention to wearing masks and hand hygiene.
The public had high awareness on knowledge of COVID-19 outbreak, and a high proportion of people practiced good hand hygiene behavior. Many people claimed anxiety, especially in heavily affected areas during pandemic, suggesting the importance of closing the gap between risk awareness and good practice and conduct psychological counseling to public and patients.

Depending on its anatomical location, cholangiocarcinoma is categorized as intrahepatic (iCCA), perihilar (pCCA), or distal (dCCA). Nearly two-thirds of patients with biliary tract cancer present with advanced disease at diagnosis and in 68-86% of resections the cancer eventually recurs either locoregionally or at a distance. Chemotherapy is the first-line therapy for advanced or recurrent BTC. Reports on the effectiveness of pneumococcal conjugate vaccines (PCVs) on pediatric parapneumonic effusion are limited. We report the changes in cases and etiologies of pediatric parapneumonic effusion in a children's hospital before and after national PCV13 vaccination programs.
We screened medical records of children 0-18 years admitted to the National Taiwan University Hospital with diagnoses of lobar pneumonia and parapneumonic effusion between 2008 and 2017. Patients with effusion analyses were included. Results of blood, pleural fluid, and respiratory specimens surveyed as standard care were analyzed.
Diagnostic testing revealed at least a pathogen in 85% of 202 children with lobar pneumonia and parapneumonic effusion. After national PCV13 immunization, pneumococcal empyema decreased by 72% among 2- to 5-year olds. Mycoplasma pneumoniae was the second most common etiology. There were marked differences in effusion characteristics, metabolic, and respiratory parameters between children infected with pneumococcus and M. pneumoniae.
The effectiveness of the national PCV13 immunization programs on pneumococcal empyema was evident and remained substantial after 4 years in Taiwan. Continuous surveillance is important to monitor the emergence of other pathogens including non-PCV serotypes and M. pneumoniae.

We used real-ear insertion gain (REIG), with the international speech test signal (ISTS), to evaluate the amplifying characteristics of hearing aids, set for patients who have been wearing such aids for a long time in a stable manner. We further compared this to the target values of the DSLv5 and NAL-NL2 methods.
The subjects were adults with moderate sensorineural hearing loss. We examined 40 ears in 25 individuals (15 people wearing hearing aids in both ears and ten people wearing aid in only one ear). Fit assessments were performed based on the speech performance-intensity functions and tolerance of environmental noise, and the ears studied were categorized as either ears with sufficient benefit or ears with insufficient benefit. Additionally, we evaluated the REIG for international speech test signals at 65-dB and 80-dB sound pressure level (SPL). We compared the REIG and target values for voice input at 65-dB and 80-dB SPL, calculated from the DSLv5 and NAL-NL2 methods.
Among the 40 ears, 34 receivnd DSLv5 are appropriate, even for Japanese-speaking individuals, at mid-pitch sounds. Although it is necessary to investigate the necessity for low-pitch and high-pitch gains further in the future, this study provides meaningful data regarding the amplifying characteristics in Japanese-speaking individuals who have been wearing hearing aids in a stable manner.An approach is made to the psychological and psychosocial characteristics that the COVID-19 pandemic is acquiring in the countries of our socio-cultural environment. The scarcity of research in this regard and the necessary acceptance of uncertainty to face the situation, both socially, as well as health and psychological, are discussed. The Assistive Technology Device Outcomes Research (ATDOR) checklist was developed as a reporting guideline for researchers to enhance the quality of research in this field. The checklist contains 13 items that cover outcome domains unique to assistive technology devices (ATDs). The ATDOR was intended to be an adjunct to existing publication guidelines for outcomes research.
The aim of this investigation was to examine the ability of the ATDOR checklist to identify strengths and weaknesses in ATD outcomes research publications that may not be detected using another publication guideline designed for outcomes research.
Twenty original ATD outcome studies were scored using the Template for Intervention Description and Replication (TIDieR) checklist, and the ATDOR in two evaluation rounds. In the first round, articles were scored using the TIDieR alone. In the second round, they were scored using the TIDieR and ATDOR together. The difference in percentage scores between the two evaluation rounds was examine capture the future needs of this research area.
When used alongside the TIDieR, the ATDOR adds significant value to evaluations of reporting quality on assistive technology outcomes research. As this field continues to grow, researchers are invited to join in efforts to standardise reporting to promote healthier outcomes for ATD users. Implications for rehabilitation Reporting guidelines that evaluate research studies enhance their reporting quality and promote healthier outcomes for ATD users. The Assistive Technology Device Outcomes Research (ATDOR) checklist was shown to be a useful tool for achieving a minimum standard of reporting in the field of assistive technology. As the field of assistive technology continues to explore different methodologies, ongoing efforts to develop and update reporting guidelines are necessary in order to capture the future needs of this research area.
Cardiac conduction system disturbances potentially leading to permanent pacemaker implantation are significant postoperative complications after aortic valve replacement. The aim of this study was to assess the impact of sutureless prosthetic valve oversizing on permanent pacemaker implantation rate.
This multicenter retrospective study included 306 patients who underwent minimally invasive aortic valve replacement with the Perceval sutureless valve. Oversizing was determined by the implanted valve size indexed to body surface area. Data were analyzed with a multivariable logistic regression model.
This study confirmed excellent postoperative results for minimally invasive aortic valve replacement with right anterior minithoracotomy approach and rapid deployment sutureless valves. Our patient recovered without any complications and had excellent long-term follow-up.Objective To evaluate 11.5% polyacrylic acid (PA) containing 0.3% methylene blue (MB) dye as a photosensitizer for photodynamic therapy (PDT) of carious dentin. Methods One hundred twenty molars were selected and the dentin was exposed for cariogenic challenge, where the molars were placed in brain heart infusion medium containing a standard strain of Streptococcus mutans (ATCC). Samples were randomly divided into eight groups (n = 15) S saline, PA, MB MB 0.3%, PA+MB PA containing 0.3% MB + LLL irradiation with low-level laser, PDT (MB) MB 0.3% + laser, PDT (PA) PA + laser, and PDT (PA+MB) PA containing 0.3% MB + laser. Carious dentin was collected before and after exposure to S. mutans. All samples of carious dentin were homogenized, diluted, and seeded in mitis salivarius bacitracin medium, and the cultures were incubated at 37°C for 15 days in anaerobic jars. The Wilcoxon test was used for analysis. Results The percent microbial reduction achieved with each treatment was as follows PDT (MB), 53.62%; PDT (PA+MB), 50.47%; PDT (PA), 46.73%; PA, 38.51%; MB, 19.75%; PA+MB, 17.18%; LLL, 12.83%; S, 5.99%. The Assistive Technology Device Outcomes Research (ATDOR) checklist was developed as a reporting guideline for researchers to enhance the quality of research in this field. The checklist contains 13 items that cover outcome domains unique to assistive technology devices (ATDs). The ATDOR was intended to be an adjunct to existing publication guidelines for outcomes research. The aim of this investigation was to examine the ability of the ATDOR checklist to identify strengths and weaknesses in ATD outcomes research publications that may not be detected using another publication guideline designed for outcomes research. Twenty original ATD outcome studies were scored using the Template for Intervention Description and Replication (TIDieR) checklist, and the ATDOR in two evaluation rounds. In the first round, articles were scored using the TIDieR alone. In the second round, they were scored using the TIDieR and ATDOR together. The difference in percentage scores between the two evaluation rounds was examine capture the future needs of this research area. When used alongside the TIDieR, the ATDOR adds significant value to evaluations of reporting quality on assistive technology outcomes research. As this field continues to grow, researchers are invited to join in efforts to standardise reporting to promote healthier outcomes for ATD users. The aim of this study was to assess the impact of sutureless prosthetic valve oversizing on permanent pacemaker implantation rate. This multicenter retrospective study included 306 patients who underwent minimally invasive aortic valve replacement with the Perceval sutureless valve. Oversizing was determined by the implanted valve size indexed to body surface area. Data were analyzed with a multivariable logistic regression model. This study confirmed excellent postoperative results for minimally invasive aortic valve replacement with right anterior minithoracotomy approach and rapid deployment sutureless valves. Mortality rate was 1%. Eighteen (5.9%) patients received a new permanent pacemaker. Methods One hundred twenty molars were selected and the dentin was exposed for cariogenic challenge, where the molars were placed in brain heart infusion medium containing a standard strain of Streptococcus mutans (ATCC). Samples were randomly divided into eight groups (n = 15) S saline, PA, MB MB 0.3%, PA+MB PA containing 0.3% MB + LLL irradiation with low-level laser, PDT (MB) MB 0.3% + laser, PDT (PA) PA + laser, and PDT (PA+MB) PA containing 0.3% MB + laser. Carious dentin was collected before and after exposure to S. mutans. All samples of carious dentin were homogenized, diluted, and seeded in mitis salivarius bacitracin medium, and the cultures were incubated at 37°C for 15 days in anaerobic jars. The Wilcoxon test was used for analysis. Soil microorganisms play an important role in regulating the feedback of Alpine steppe ecosystems to future climate change. However, the interaction effect of warming and increasing precipitation on soil microorganisms remains unclear, in the face of an ongoing warmer and wetter climate on the Tibetan Plateau. In this study, we investigate the multi-factorial effects on soil microbial diversity, community structure, and microbial interactions in a three-year climate change experiment established in an Alpine steppe on the Tibetan Plateau, involving warming (+2 °C), +15% increasing precipitation and +30% increasing precipitation. Compared to warming, warming plus increasing precipitation alleviated the decrease in microbial diversity, and increased the dissimilarities in microbial community structures, largely influenced by water and substrate availability. We further observed differences in moisture increased the differences in microbial diversity and dissimilarities in microbial community structures across different precipitation levels under ambient temperature. Interestingly, warming plus increasing precipitation could create more ecological niches for microbial species to coexist but may lessen the strength of microbial interactions in contrast to increasing precipitation alone. Collectively, our findings indicate that microbial responses to future climate change in Alpine steppe soils will be more complex than those under single-climate-factor conditions.
Living in green environments has been associated with various health benefits, but the evidence for positive effects on respiratory health in children is ambiguous.
To investigate if residential exposure to different types of green space is associated with childhood asthma prevalence in Belgium.
Asthma prevalence was estimated from sales data of reimbursed medication for obstructive airway disease (OAD) prescribed to children between 2010 and 2014, aggregated at census tract level (n=1872) by sex and age group (6-12 and 13-18 years). Generalized log-linear mixed effects models with repeated measures were used to estimate effects of relative covers of forest, grassland and garden in the census tract of the residence on OAD medication sales. Consistent associations between OAD medication sales and relative covers of grassland and garden were observed (unadjusted parameter estimates per IQR increase of relative coveeas with high grass cover (grasslands, but also residential gardens) may negatively impact child respiratory health. Potential allergic and non-allergic mechanisms that underlie this association include elevated exposure to grass pollen and fungi and reduced exposure to environmental biodiversity. Reducing the dominance of grass in public and private green space might be beneficial to reduce the childhood asthma burden and may simultaneously improve the ecological value of urban green space.Welders have an increased susceptibility to airway infections with non-typeable Haemophilus influenzae (NTHi), which implicates immune defects and might promote pneumonia and chronic obstructive pulmonary disease (COPD). In this study, an efficient approach to the preparation of Egeria najas-derived biochar supported nZVI composite is first developed for Cr(VI) removal in wastewater. The adsorption behavior and mechanism of Cr(VI) removal on the as-prepared Egeria najas-derived biochar/nZVI (EN@nZVI) composite were investigated. The results of FTIR and XPS indicate that the EN@nZVI surface had many functional groups such as R-COOH, R-OH, R-NH2 and R-C-O-C, etc, which could provide active sites during the adsorption process. #link# The BET results showed that the EN@nZVI had large specific surface area and average pore, which were 142.49 m2/g and 9.85 nm, respectively. EN@nZVI demonstrated high reactivity for Cr(VI) removal. Compared with nZVI, Cr(VI) removal efficiency by EN@nZVI is 50% higher than that of nZVI within 0.5 h. Furthermore, the maximum adsorption capacity of Cr (VI) was 56.79 mg/g and the energy of activation (Ea) was 31.30 kJ/mol. The adsorption process was well described by the pseudo-second order model and Sips adsorption isotherm model. The reaction mechanism of Cr(VI) removal was a multi-step removal mechanism, involving adsorption, surface complex formation, reduction and ion exchange reaction.

Objective To understand the characteristics of spatiotemporal clustering on hepatitis A in Gansu province and to provide evidence for hepatitis A prevention and control. Methods Data related to hepatitis A were retrieved from National Notifiable Disease Report System, ArcGIS 10.3 and SaTScan 9.1 in Gansu province from 2004 to 2018. Results The annual average report incidence rate of hepatitis A was 10.91/100 000, showing a descending trend with no periodic or seasonal features. After the implementation of national expanded immunization program, high annual incidence rates had been seen in Linxia Hui autonomous prefecture and Gannan Tibetan autonomous prefecture. From 2004 to 2012, the lowest RR value appeared in the 0-9 age group (P=0.000) while the highest RR value was in the over 60 age group during 2013-2018 except for the age 0-9 group in 2015. The annual average incidence rate was increasing from south to north and west to east, across the territory. Results from the temporal scanning program revealed that the incidence of hepatitis A was temporally aggregated from 2004 to 2018. For spatio-temporal scanning of 2004-2008, data showed one most likely cluster area (radius 91.95 km, Time frame 2004-2005), apparel mainly in Linxia and Longnan cities. Results from the spatio-temporal scanning program of 2009-2018 also showed that the most likely cluster areas (radius 183.26 km, Time frame 2009-2012) were in Gannan, Linxia, Dingxi and Longnan areas. Conclusions The reported incidence rates of hepatitis A were declining, without significant periodic or seasonal pattern in Gansu province from 2004 to 2018. In the 0-9 years-old group, the incidence rate showed the lowest, while the highest was in the 60 year-olds group. The annual decreases of the incidences of congenital syphilis, primary syphilis and secondary syphilis were all obvious, which were 43.47%, 21.38% and 14.19% respectively. The proportion of latent syphilis cases increased with year. Except for Lishui, the incidences of syphilis in the remaining 10 prefectures showed declining trends. The incidence rates in both men and women showed declining trends with the average rates of 4.80% and 6.45% respectively. The incidence peaks occurred in old men aged ≥60 years and in sexually active women aged 20-34 years, and the syphilis cases in age group ≥60 years increased significantly. The cases were mainly farmers, accounting for 43.00%. Methods This study was based on a 4-year longitudinal cohort of children from Bengbu, Anhui province. Pattern of sugar-sweetened beverages was determined by group-based trajectory modeling (GBTM) through annual child-reported questionnaire. Association between sugar-sweetened beverages pattern with plague index and gingivitis was explored through multivariate linear and logistic regression. Results A total of 997 children including 418 boys and 579 girls were included in this study, with an average age of (11.00±0.70) years at the final wave. Four consumption patterns on sugar-sweetened beverages were identified by GBTM persistent low group (80.70%), gradually decreasing group (12.40%), decreasing after increasing group (2.20%) and gradually increasing group (4.70%). Compared with the persistent low group (1.08±0.70), the Plague index among gradually increasing group (1.73±0.80) was significantly higher (t=4.00, P less then 0.001). The prevalence of gingivitis was significantly higher among children in the gradually increasing group (12.80%) than that in the persistent low group (3.00%) (χ(2)=12.40, P less then 0.001). After controlling for related confounders, results suggested that Plague index increased by 0.58 (95%CI 0.27-0.89) and 0.38 (95%CI 0.03-0.73) in the gradually increasing group and "decreasing follow the increase" group, respectively when compared with the persistent low group. Association between sugar-sweetened beverages pattern with plague index and gingivitis was explored through multivariate linear and logistic regression. Results A total of 997 children including 418 boys and 579 girls were included in this study, with an average age of (11.00±0.70) years at the final wave. Four consumption patterns on sugar-sweetened beverages were identified by GBTM persistent low group (80.70%), gradually decreasing group (12.40%), decreasing after increasing group (2.20%) and gradually increasing group (4.70%). Compared with the persistent low group (1.08±0.70), the Plague index among gradually increasing group (1.73±0.80) was significantly higher (t=4.00, P less then 0.001). The prevalence of gingivitis was significantly higher among children in the gradually increasing group (12.80%) than that in the persistent low group (3.00%) (χ(2)=12.40, P less then 0.001). After controlling for related confounders, results suggested that Plague index increased by 0.58 (95%CI 0.27-0.89) and 0.38 (95%CI 0.03-0.73) in the gradually increasing group and "decreasing follow the increase" group, respectively when compared with the persistent low group. Contamination of durum wheat grain by cadmium (Cd) threatens food safety and is of increasing concern because regulations concerning Cd are becoming stricter due to its toxicity. This work aimed at using soil variables and cultivar types to build models to predict whether durum wheat grain Cd will conform with current and possibly lower regulatory thresholds. We combined multiple Gaussian and logistic regressions and the random forest algorithm to take advantage of their strength. Models tested using cross-validation produced excellent performances including for the lowest regulatory threshold of 0.1 mg Cd/kg, half of the current one 79-85% of the non-conformity cases were detected and the reliability of predictions was 69-82%. The models enabled identification of a x1.4 variability in grain Cd content between cultivars that do not have the low Cd accumulation allele of the Cdu1 gene. The models confirmed that for the grain Cd content, the between-cultivar variability had **** less influence than the phytoavailability of Cd in soil, the critical contexts of which were characterized by the models. In Ethiopia, maternal mortality remains an important public health concern. High maternal mortality is attributed in part to the poor quality of obstetric care. #link# This study was designed to investigate perceptions of midwives about the quality of emergency obstetric care provided at hospitals in the Harari region of Ethiopia.
An explanatory qualitative study was conducted from December 2018 to February 2019 at public and private hospitals in the Harari region, Ethiopia. The data were obtained through in-depth interviews with 12 midwives working in maternity units. The interviewers took notes and audio-recorded the respondents' descriptions. Braun and Clarke's thematic analysis method was employed to analyse the data using Nvivo 12 qualitative data analysis software.
Poorly designed infrastructure, including a scarcity of beds, rooms and ambulances challenged the provision of quality obstetric services. Midwives working at hospitals were inadequate in number and training opportunities were scarce. Languagesources and service delivery management need to be strengthened.
The pedunculopontine nucleus (PPN) is considered a promising target to alleviate gait disorders. We aimed to evaluate the effects of PPN stimulation on motor symptoms and gait disorders in patients with Parkinson's disease (PD) to help assess the potential role of PPN-DBS treatment in gait disorders.
Studies were searched for low-frequency PPN stimulation to treat gait disorders and freezing of gait (FOG) in the PubMed, Embase, Cochrane Library, Web of Science, and ClinicalKey up to April 2020. Outcomes of Unified Parkinson's Disease Rating Scale (UPDRS) part III, subitems 27-30; UPDRS subitems 13 and 14; the Freezing of Gait Questionnaire (FOGQ), and the Gait and Falls Questionnaire (GFQ) were extracted and evaluated during PPN On-stimulation compared to preoperation or Off-stimulation in both Off- and On-medication states.
There was a significant improvement in subitems 27-30 with PPN On-stimulation versus Off-stimulation in Off-medication and On-medication states, but no improvement in UPDRS part III. The occurrence of FOG and falls also declined between PPN On-stimulation and presurgery, with a significant improvement in subitem 13 and subitem 14 in Off-medication and On-medication states, GFQ, and FQGQ. Heterogeneity in stimulation frequency, follow-up, electrode location, and unilateral or bilateral stimulation existed among the included studies.

We investigated functional connectivity based on EEG using graph theoretical analysis in patients with newly diagnosed juvenile myoclonic epilepsy (JME), and whether it could play a role as a biomarker predicting antiepileptic drug (AED) response.
We consecutively enrolled 38 patients with JME and 40 normal controls. The initial EEG was undertaken at the time of diagnosis of JME in a drug-naïve state. The second EEG was done after at least 12 months from the time of the initial EEG. We classified the patients with JME into two groups according to AED response at the time of taking the second EEG. We investigated functional connectivity based on graph theoretical analysis using connectivity measures of the coherence and phase locking value.
In the analysis of functional connectivity using coherence as a connectivity measure, the global efficiency and local efficiency in the AED poor responders (N = 4) decreased, whereas the small-worldness index increased.
