  • Study Design Systematic review. Objective To assess the current literature regarding the relationship between the shoulder and the spine with regard to (1) overlapping pain pathways; (2) differentiating history, exam findings, and diagnostic findings; (3) concomitant pathology and optimal treatments; and (4) cervical spine-based etiology for shoulder problems. Methods A systematic literature search was performed according to the guidelines set forth by the Cochrane Collaboration. Studies were included if they examined the clinical, anatomical, or physiological overlap between the shoulder and cervical spine. Two reviewers screened and selected full texts for inclusion according to the objectives of the study. Quality of evidence was graded using OCEBM (Oxford Center for Evidence Based Medicine) and MINORS (Methodological Index for Nonrandomized Studies) scores. Results Out of 477 references screened, 76 articles were included for review and grouped into 4 main sections (overlapping pain pathways, differentiat referral patterns, shoulder kinematics, and examination techniques are invaluable to the clinician in making an appropriate diagnosis and guiding treatment. The purpose of this article was to summarize and critically analyze the available evidence related to current strategies in the prevention of SSIs after spine surgery. Methods A literature search utilizing Medline database was performed. Relevant studies from all the evidence levels have been included. Recommendations to decrease the risk of SSIs have been provided based on the results from studies with the highest level of evidence. Results SSI prevention occurs at each phase of care including the preoperative, intraoperative, and postoperative periods. Meticulous patient selection, tight glycemic control in diabetics, smoking cessation, and screening/eradication of Staphylococcus aureus are some of the main preoperative patient-related preventive strategies. Currently used intraoperative measures include alcohol-based skin preparation, topical vancomycin powder, and betadine irrigation of the surgical site before closure. Postoperative infection prophylaxis can be performed by administration of silver-impregnated or vacuum dressings, extended intravenous antibiotics, and supplemental oxygen therapy. Conclusions Although preventive strategies are already in use alone or in combination, further high-level research is required to prove their efficacy in reducing the rate of SSIs in spine surgery before evidence-based standard infection prophylaxis guidelines can be built. © The Author(s) 2019.Study Design Retrospective cohort. Methods The 2016 American College of Surgeons-National Surgical Quality Improvement Program (ACS-NSQIP) Pediatric Spinal Fusion Procedure Targeted database records, merged with original 2016 ACS-NSQIP Pediatric file variables, were used for this study. Univariate and multivariate analyses were used to identify independent risk factors associated with admission, as well as a prolonged length of stay in the ICU following surgery. Results A total of 1398 (39.5%) patients required an ICU admission following the procedure-out of whom 416 (29.8%) stayed for more than 2 days. Following adjusted analysis, patient and surgical factors independently associated with an ICU admission were black/African American versus white race, anterior fusion, combined fusion, nonidiopathic scoliosis, preoperative ventilator dependence, asthma, having structural pulmonary abnormality, developmental delay, having a neuromuscular disorder, requiring nutritional support and a total operative time >270 minutes. The only significant factors associated with a prolonged length of ICU stay >2 days were preoperative ventilator dependence, nutritional support requirement, and undergoing anterior or combined fusion. Conclusions This study is the first of its kind to identify significant patient- and procedure-level factors associated with an ICU admission, and also explores predictors for a prolonged stay in the ICU. The study compares the maintenance of correction of thoracolumbar (TL) trauma patients who underwent MISS with facet fusion (FF) and without facet fusion (WOFF) and evaluates instrumentation loosening and failure. Methods TL trauma patients who underwent MISS between 2006 and 2013 were identified and stratified into FF and WOFF groups. To evaluate progressive kyphosis and loss of correction, Cobb angles were measured at immediate postoperative, short-term, and long-term follow-up. Evidence of >2 mm of radiolucency on radiographs indicated screw loosening. If instrumentation was removed, postremoval kyphosis angle was obtained. Results Of the 80 patients, 24 were in FF and 56 were in WOFF group. Between immediate postoperative and short-term follow-up, kyphosis angle changed by 4.0° (standard error [SE] 1.3°) in the FF and by 3.0° (SE 0.4°) in the WOFF group. The change between immediate postoperative and long-term follow-up kyphosis angles was 3.4° (S.E 1.1°) and 5.2° (S.E 1.6°) degrees in the FF and WOFF groups, respectively. Facet fusion had no impact on the change in kyphosis at short term (P = .49) or long term (P = .39). The screw loosening rate was 20.5% for the 80 patients with short-term follow-up and 68.8% for the 16 patients with long-term follow-up. There was no difference in screw loosening rate. Fifteen patients underwent instrumentation removal-all from the FF group.   • Background Hysteroscopic adhesiolysis (HA) remains the mainstay of treatment for intrauterine adhesions (IUA). In cases of moderate or severe IUA, the assistance of various adjunctive aids are usually sought to improve HA's success rate. Among these, intraoperative transabdominal ultrasound (TAS) is the most common; however, it has certain limitations. Preoperative three-dimensional transvaginal ultrasound (3D-TVUS) has been accepted as a non-invasive way to provide accurate information about the uterine cavity. This prospective, non-randomized controlled study will assess the effects of pre-operative 3D-TVUS prior to HA in improving the surgeon's intraoperative judgement. Methods A total of 362 patients, who met the inclusion criteria, aged between 18 and 45 years and diagnosed with moderate or severe IUA underwent HA at our hospital from March 2018 to December 2018. Participants were divided into 2 groups; the study group; n=182 performed 3D-TVUS evaluation prior to HA, and the control group; n=180 underwenrly visible fallopian tube ostia (P less then 0.0001) following HA. Conclusions This study showed that preoperative 3D-TVUS evaluation helped the hysteroscopists with their intraoperative decision-making while carrying out HA. In comparison to those who did not perform preoperative 3D ultrasound, those who underwent 3D-TVUS evaluation had a better surgical success rate in retrieving the fallopian tube ostia and the restoration of normal uterine cavity morphology. 2020 Annals of Translational Medicine. This study aimed to preliminary the alteration in diagnostic and curative aspects of these patients, so as to provide a certain reference for the clinical management of such conditions. Methods Ten patients with CI combined with moderate to severe IUAs were diagnosed, treated and followed up at the Third Xiangya Hospital of Central South University from September 2017 to August 2019, their medical records and the pregnancy outcomes were retrospectively analyzed. Results All 10 patients had a previous history of typical painless cervical dilatation during the second trimester. All patients were moderate to severer IUAs, and the mean AFS score of IUAs was 9.80±1.08 (range, 8 to 12). Preoperatively, inlower uterine segment IUAs, it is necessary to separate the adhesion prior to evaluating the cervical competency with the No. 7 Hegar dilator, to confirm the diagnosis. However, when the cervix is too loose, laparoscopic cervical cerclage is exceptionally carried out first and then IUAs is treated. Results The results revealed that GH administration promoted the proliferation, cell cycle, migration, and growth hormone receptors (GHRs) expression of the hEGC. Thus, it is meaningful to identify more cathelicidins from vertebrate species. Cathelicidins from ranid frogs possess special structural characteristics and activities, but to date only 12 ranid frog cathelicidins have been identified. In the present study, two novel cathelicidins (PN-CATH1 and 2) were identified from the black-spotted frog, Pelophylax nigromaculata. PN-CATHs possess low sequence similarity with the known cathelicidins. They exhibited moderate, but broad-spectrum and rapid antimicrobial activities against the tested bacteria. They kill bacteria by mainly inducing bacterial membrane disruption and possibly generating intracellular ROS formation. They also possess potent anti-biofilm and persister cell killing activity, indicating their potential in combating infections induced by biofilms-forming bacteria. Besides direct antimicrobial activity, they exhibited potent anti-inflammatory activity by effectively inhibiting the LPS-induced production of pro-inflammatory cytokines in mouse macrophages, which could be partly ascribed to their direct LPS-neutralizing ability. Furthermore, PN-CATHs demonstrated powerful in vitro free radical scavenging activities. Ultraviolet radiation significantly increased their in vivo gene expression in frog skin. In the present study, two novel cathelicidins (PN-CATH1 and 2) were identified from the black-spotted frog, Pelophylax nigromaculata. PN-CATHs possess low sequence similarity with the known cathelicidins. They exhibited moderate, but broad-spectrum and rapid antimicrobial activities against the tested bacteria. They kill bacteria by mainly inducing bacterial membrane disruption and possibly generating intracellular ROS formation. They also possess potent anti-biofilm and persister cell killing activity, indicating their potential in combating infections induced by biofilms-forming bacteria. Here, using a diet-induced liver fibrosis murine model (choline-deficient amino acid-defined, high fat diet), we characterized a specific population of HSCs organized as clusters presenting simultaneously hypertrophy of retinoid droplets, quiescent and activated HSC markers. We showed that hypertrophied HSCs co-localized with fibrosis areas in space and time. Importantly, we reported the existence of this phenotype and its association with collagen deposition in three other mouse fibrosis models, including CCl4-induced fibrosis model. Moreover, we have also shown its relevance in human liver fibrosis associated with different etiologies (obesity, non-alcoholic steatohepatitis, viral hepatitis C and alcoholism). In particular, we have demonstrated a significant positive correlation between the stage of liver fibrosis and HSC hypertrophy in a cohort of obese patients with hepatic fibrosis. These results lead us to conclude that hypertrophied HSCs are closely associated with hepatic fibrosis in a metabolic disease context and may represent a new marker of metabolic liver disease progression.Riverbed terracing has been introduced in ancient times to retain water and soil, to reduce hydrological connectivity and erosion and to increase primary and secondary productivity of agro-ecological systems. These presently abandoned human-made landscapes have become novel ecosystems and a potential source of ecosystem services to humans in drylands. We use a mathematical-modeling approach to study factors that regulate terraced riverbeds and affect community and ecosystem attributes such as productivity, functional diversity and resilience to droughts. We introduce a model that captures the relationships between rainfall pattern, runoff coupling between adjacent terraces, and vegetation growth, taking into account competition for water and light. We found that a large number of weak rainfall events results in lower total biomass and functional diversity across the terraced riverbed compared with a few strong rainfall events. We further analyzed the filtering of species traits from pools of functional groups that make different tradeoffs between investment in above-ground biomass to capture canopy resources and investment in below-ground biomass to capture soil resources. Pools characterized by concave tradeoffs give rise to higher functional diversity, lower biomass production and lower resilience to droughts, as compared with convex pools. New empirical studies are needed to test these model predictions.Recycling cycles cause a decrease in mechanical paper properties due to cellulose fiber degradation. The use of cellulose micro/nanofibers (CMF/CNF) to reinforce paper strength has been well studied, although it has been found to have negative effects on drainage. However, the application of CMF/CNF as paper reinforcement is affected by the nanocellulose type. Thus in this study mechanical and chemical treatments in CNF production were compared. Old corrugated container (OCC) pulp used to produce recycled cartonboard was reinforced with 1) CMF from never-dried northern bleached softwood kraft pulp (NBSK) highly refined in a 16-inch low consistency refiner at 1200 rpm and 25 kW of net power; and 2) CNF from NBSK pulp treated by TEMPO-mediated oxidation and homogenization at 600 bars. CMF/CNF and OCC were pulped at the same time and handsheets formed with cationic starch (CS) as retention system. Mechanical, drainage and flocculation properties were evaluated and compared. Data were also compared with other sources of TEMPO CNF. Results show an improvement in mechanical properties, drainage and flocculation when OCC is reinforced with CMF obtained with LCR. Affiliation among group members who stayed near former opponents during aggressions did not increase after aggressions compared to that under control conditions. We introduce a model that captures the relationships between rainfall pattern, runoff coupling between adjacent terraces, and vegetation growth, taking into account competition for water and light. We found that a large number of weak rainfall events results in lower total biomass and functional diversity across the terraced riverbed compared with a few strong rainfall events. We further analyzed the filtering of species traits from pools of functional groups that make different tradeoffs between investment in above-ground biomass to capture canopy resources and investment in below-ground biomass to capture soil resources. Pools characterized by concave tradeoffs give rise to higher functional diversity, lower biomass production and lower resilience to droughts, as compared with convex pools. Old corrugated container (OCC) pulp used to produce recycled cartonboard was reinforced with 1) CMF from never-dried northern bleached softwood kraft pulp (NBSK) highly refined in a 16-inch low consistency refiner at 1200 rpm and 25 kW of net power; and 2) CNF from NBSK pulp treated by TEMPO-mediated oxidation and homogenization at 600 bars. CMF/CNF and OCC were pulped at the same time and handsheets formed with cationic starch (CS) as retention system. Mechanical, drainage and flocculation properties were evaluated and compared. Data were also compared with other sources of TEMPO CNF. Results show an improvement in mechanical properties, drainage and flocculation when OCC is reinforced with CMF obtained with LCR.   • Protein O-GlcNAcylation is a dynamic post-translational modification involving the attachment of N-acetylglucosamine (GlcNAc) to the hydroxyl groups of Ser/Thr residues on numerous nucleocytoplasmic proteins. Two enzymes are responsible for O-GlcNAc cycling on substrate proteins O-GlcNAc transferase (OGT) catalyzes the addition while O-GlcNAcase (OGA) helps the removal of GlcNAc. O-GlcNAcylation modifies protein functions; therefore, dysregulation of O-GlcNAcylation affects cell physiology and contributes to pathogenesis. In this report, we present three cases of patients diagnosed with esophago-gastric junction cancer who were scheduled for elective esophagectomy with a curative intention during second wave of COVID-19 pandemic in a single high-volume tertiary center. Despite all available safety measures, one of the patients developed COVID-19 pneumonia on post-operative day two, leading to an impaired respiratory function and increased pleural fluid collection from the chest tube, resulting in a prolonged time of hospital stay. Finding a good balance between the COVID-19-related perioperative risks and consequences of delaying surgical treatment in patients diagnosed with esophago-gastric cancer is a challenge. In order to achieve the best possible outcome, care must be taken to ensure availability of necessary treatment options and to reduce the risk of SARS-Cov-2 transmission perioperatively.
    Population groups to be prioritized for COVID-19 vaccinations in the U.S. have been determined at the Federal level, but there is variation in how States have implemented guidance. This review examines how the position of population groups in vaccine priority lists varies between Federal guidance and State practice.

    An online search of State vaccination prioritization plans was conducted. Data were extracted on each population group included and their relative position. A standardized ranking method was applied to provide a directional measure of variability in prioritization between State and Federal guidance, for each population group.

    Healthcare workers and those in long-term care facilities were largely prioritized in line with Federal guidance. Aside from early education staff, essential workers were often excluded at State level. Almost all States included the 65-74 year age group and most assigned them to a higher position than recommended in Federal guidance. Those with underlying medical conditions were similarly highly prioritized, although there was more variability across States. Some socially vulnerable groups (not included in Federal guidance) were highly prioritized by many States.

In Spain, during the initial eight weeks after the State of Emergency was decreed on 14 March 2020, in the weekly reports of the Health Ministry, an average decrease of 20% was observed between 11 and week 25 compared with the 2018 donation. We aimed to investigate the impact of the COVID-19 pandemic on blood donations and blood distribution in four autonomous communities, and to explore the evolution of the consumption of blood components (BCs) in ten hospitals of six autonomous communities. We performed a prospective study of grouped cohorts on the donation and distribution of blood in four regional transfusion centers in four autonomous communities in Spain, and a retrospective study of the consumption of blood components in ten hospitals in six autonomous communities. Regarding donations, there was no significant decrease in donations, with differences between autonomous communities, which started between 1 and 15 March 2020 (-11%). The increase in donations in phase II (from 26 May 2020) stands out. Regarding consumption, there was a significant reduction in the consumption of packed red blood cells (RBCs) (24.5%), plasma (45.3%), and platelets (25.3%) in the central period (16 March-10 May). The reduction in the consumption of RBCs was significant in the period from 1-15 March. The sample was made up of 38 professional women soccer players. To measure the external load, the best time, worst time, average time, individual speed, sprint decrement, and power were assessed. In connection with the internal load, the desaturation (sprint) and re-saturation (recovery) rates, as well as the oxygen extraction (∇%SmO2) in the gastrocnemius muscle and maximum heart rate (%HRmax) were measured. This review examines how the position of population groups in vaccine priority lists varies between Federal guidance and State practice. An online search of State vaccination prioritization plans was conducted. Data were extracted on each population group included and their relative position. A standardized ranking method was applied to provide a directional measure of variability in prioritization between State and Federal guidance, for each population group. Healthcare workers and those in long-term care facilities were largely prioritized in line with Federal guidance. Aside from early education staff, essential workers were often excluded at State level. Almost all States included the 65-74 year age group and most assigned them to a higher position than recommended in Federal guidance. Those with underlying medical conditions were similarly highly prioritized, although there was more variability across States. Some socially vulnerable groups (not included in Federal guidance) were highly prioritized by many States. The prioritization of groups for COVID-19 vaccination has been highly variable despite clear Federal guidance. Future guidance must be relevant to local needs, values, and constraints, to minimize any unwarranted heterogeneity in vaccine access across populations. In Spain, during the initial eight weeks after the State of Emergency was decreed on 14 March 2020, in the weekly reports of the Health Ministry, an average decrease of 20% was observed between 11 and week 25 compared with the 2018 donation. We aimed to investigate the impact of the COVID-19 pandemic on blood donations and blood distribution in four autonomous communities, and to explore the evolution of the consumption of blood components (BCs) in ten hospitals of six autonomous communities. We performed a prospective study of grouped cohorts on the donation and distribution of blood in four regional transfusion centers in four autonomous communities in Spain, and a retrospective study of the consumption of blood components in ten hospitals in six autonomous communities. Regarding donations, there was no significant decrease in donations, with differences between autonomous communities, which started between 1 and 15 March 2020 (-11%). The increase in donations in phase II (from 26 May 2020) stands out. Regarding consumption, there was a significant reduction in the consumption of packed red blood cells (RBCs) (24.5%), plasma (45.3%), and platelets (25.3%) in the central period (16 March-10 May). The reduction in the consumption of RBCs was significant in the period from 1-15 March. Therefore, the objective of this study was to analyze muscle oxygenation dynamics during repeated sprint ability (RSA) (8 sprint × 20 s recovery) by near-infrared spectroscopy (NIRS). The sample was made up of 38 professional women soccer players. To measure the external load, the best time, worst time, average time, individual speed, sprint decrement, and power were assessed. In connection with the internal load, the desaturation (sprint) and re-saturation (recovery) rates, as well as the oxygen extraction (∇%SmO2) in the gastrocnemius muscle and maximum heart rate (%HRmax) were measured. A repeated measures statistic was applied based on the inter-individual response of each subject from the baseline versus the other sprints, with linear regression and nonlinear regression analyses between variables. Treatment of cultured luteal steroidogenic cells with a pharmacological inhibitor of NR5A2 decreased progesterone production and tended to decrease abundance of HSD3B1 mRNA. Luteal NR5A2 mRNA increased and NR5A2 protein tended to increase between days 4 and 6 of the estrous cycle, coincident with increased steroidogenic capacity of the CL. Luteal NR5A2 mRNA decreased by 8 h after prostaglandin (PG) F2A injection. During early pregnancy, luteal NR5A2 mRNA was less on days 20 and 23 compared to day 14, but protein abundance did not change. Neither 1 nor 10 ng/mL interferon tau (IFNT) altered NR5A2 abundance in cultured luteal steroidogenic cells, but 10 ng/mL PGF2A decreased NR5A2.     Confirmed COVID-19 cases have been registered in more than 200 countries, and as of July 28, 2020, over 16 million cases have been reported to the World Health Organization. This study was conducted during the epidemic peak of COVID-19 in Italy. The early identification of individuals with suspected COVID-19 is critical in immediately quarantining such individuals. Although surveys are widely used for identifying COVID-19 cases, outcomes, and associated risks, no validated epidemiological tool exists for surveying SARS-CoV-2 infection in the general population.

    We evaluated the capability of self-reported symptoms in discriminating COVID-19 to identify individuals who need to undergo instrumental measurements. We defined and validated a method for identifying a cutoff score.

    The objectives of this study were to assess the prevalence of probable depression during the COVID-19 stay-at-home period and to test three hypothesized risk reduction or risk elevation mechanisms, namely social capital-accrual, escape-from-self, and time-displacement effects.

    This study took place from March to May 2020 at the early stage of the pandemic. The study adopted a prospective design, with an online survey administered to 573 UK and 474 US adult residents at two assessment points 2 months apart.

  • To characterize the clinical features of idiopathic intracranial hypertension (IIH) in patients >50 years of age compared to the typical IIH population and existing data for this older cohort.

    Retrospective, clinical cohort study.

    Medical records of 65 patients >50 years of age at first diagnosis of IIH were reviewed based on the Modified Dandy Criteria from 4 academic centers. Each center provided randomly selected controls from IIH patients <50 years of age for each study patient at their location. Data recorded included patient demographics, presenting symptoms, medications, coexisting medical conditions, cerebrospinal fluid (CSF) opening pressure, treatments, and neuro-ophthalmic data from initial and final visits.

    Compared to controls, the older cohort showed the following characteristics fewer females (n= 51 [78.5%] vs. controls n= 60 [92.3%]; P= .045), fewer headaches (n= 33 [50.8%] vs. controls 52 [80.0%]; P= .001), more frequent incidental discoveries of papilledema (n= 19 [29.2%] vs. controls 7 [10.8%]; P= .015), and lower CSF opening pressure [median 33cm H

    We retrospectively identified 153 adults in a clinical practice with cyclodeviation assessed using double-Maddox rods, of whom 105 were also assessed using fusible synoptophore targets, 73 using nonfusible synoptophore targets, 118 using single-Maddox rod, and 43 using fundus photography. Relationships between double-Maddox rod and other tests were evaluated by calculating mean differences with 95% confidence intervals (CI), intraclass correlation coefficients (ICC), and Bland-Altman plots with linear regression.

    Synoptophore cross-in-circle targets and the largest (of right or left) single-Maddox rod values were similar to double-Maddox values (mean differences-1.2° and 0.1°, respectively; ICC 0.79 and 0.82, respectively). Synoptophore house targets measured less excyclodeviation (mean difference-2.7°; ICC 0.71). Mean summed single-Maddox rod values were somewhat similar to double-Madrue cyclodeviation and relationships with symptoms.
    Peritoneal recurrence of gastric cancer after curative surgical resection is common and portends a poor prognosis. Early studies suggest that extensive intraoperative peritoneal lavage (EIPL) might reduce the risk of peritoneal recurrence and improve survival. We aimed to evaluate the survival benefit of EIPL in patients with gastric cancer undergoing curative gastrectomy.

    In this open-label, phase 3, multicentre randomised trial, patients aged 21-80 years with cT3 or cT4 gastric cancer undergoing curative resection were enrolled at 22 centres from South Korea, China, Japan, Malaysia, Hong Kong, and Singapore. Patients were randomly assigned to receive surgery and EIPL (EIPL group) or surgery alone (standard surgery group) via a web-based programme in random permuted blocks in varying block sizes of four and six, assuming equal allocation between treatment groups. One is that of transforming our links with nature and, particularly, wildlife given the likely COVID-19 origin from illegal wildlife trading. Similar to vector borne diseases (VBD, diseases transmitted by vectors), the COVID-19 pandemic follows related patterns (e.g. no effective or available vaccines, difficult to diagnose, highly localized infection geographical foci, non-human reservoirs) for which we urgently need preventive measures. Towards this aim, governments worldwide must strive to prevent further devastation of natural environments that serve as buffer areas to humans against zoonotic agents (among other health risks), protecting biodiversity and its concomitant causes (e.g. global change), and banning use of wildlife of illegal origin. We herein state that some VBD prevention strategies could also be applied to zoonotic disease prevention, including COVID-19 or any type likely to be related to environmental conditions. The occurrence of future pandemic occurrence will depend on whether governments embrace these aims now.Increasing evidence exisits for the role that shellfish play in the epidemiology of Toxoplasma gondii in marine environment. However, limited information is available on the level of T. gondii infection in wild marine snails, which can play a role in the transmission of T. gondii to other marine organisms and humans. In this study, the prevalence of T. gondii DNA in wild marine snails collected from three coastal cities in China was determined. Between January 2018 and November 2019, 1,206 wild marine snails were randomly collected and examined for the presence of T. gondii DNA using a nested polymerase chain reaction (PCR) targeting T. gondii B1 gene. The amplified products were genotyped using multilocus PCR-restriction fragment length polymorphism analysis. We also examined whether species of snail, sampling region, sampling season, surface runoff near samplic site, residential water discharge near samplic site, and proximity to livestock farms are associated with the occurrence of T. gondii DNA in marine snails.

Similar to vector borne diseases (VBD, diseases transmitted by vectors), the COVID-19 pandemic follows related patterns (e.g. no effective or available vaccines, difficult to diagnose, highly localized infection geographical foci, non-human reservoirs) for which we urgently need preventive measures. Towards this aim, governments worldwide must strive to prevent further devastation of natural environments that serve as buffer areas to humans against zoonotic agents (among other health risks), protecting biodiversity and its concomitant causes (e.g. global change), and banning use of wildlife of illegal origin. We herein state that some VBD prevention strategies could also be applied to zoonotic disease prevention, including COVID-19 or any type likely to be related to environmental conditions. However, limited information is available on the level of T. gondii infection in wild marine snails, which can play a role in the transmission of T. gondii to other marine organisms and humans. In this study, the prevalence of T. gondii DNA in wild marine snails collected from three coastal cities in China was determined. Between January 2018 and November 2019, 1,206 wild marine snails were randomly collected and examined for the presence of T. gondii DNA using a nested polymerase chain reaction (PCR) targeting T. gondii B1 gene. The amplified products were genotyped using multilocus PCR-restriction fragment length polymorphism analysis. We also examined whether species of snail, sampling region, sampling season, surface runoff near samplic site, residential water discharge near samplic site, and proximity to livestock farms are associated with the occurrence of T. gondii DNA in marine snails.

We investigated viltolarsen's ability to induce dystrophin expression and examined its safety in DMD patients.

    In this open-label, multicenter, parallel-group, phase 1/2, exploratory study, 16 ambulant and nonambulant males aged 5-12years with DMD received viltolarsen 40 or 80mg/kg/week via intravenous infusion for 24weeks. Primary endpoints were dystrophin expression and exon 53 skipping levels.

    In western blot analysis, mean changes in dystrophin expression (% normal) from baseline to Weeks 12 and 24 were-1.21 (P=0.5136) and 1.46 (P=0.1636), respectively, in the 40mg/kg group, and 0.76 (P=0.2367) and 4.81 (P=0.0536), respectively, in the 80mg/kg group. The increase in mean dystrophin level at Weeks 12 and 24 was significant in the 80mg/kg group (2.78%; P=0.0364). Patients receiving 80mg/kg showed a higher mean exon 53 skipping level (42.4%) than those receiving 40mg/kg (21.8%). All adverse events were judged to be mild or moderate in intensity and none led to study discontinuation.

    Treatment with viltolarsen 40 or 80mg/kg elicited an increasing trend in dystrophin expression and exon 53 skipping levels, and was safe and well tolerated. The decline in motor function appeared less marked in patients with higher dystrophin levels; this may warrant further investigation.     Dose falloff outside the target was studied retrospectively in 102 SBRT VMAT plans of lung tumor. Plans having R50% and/or D2cm higher than recommended tolerances in RTOG protocols 0813 and 0915 were replanned with new optimization constraints using novel shell structures and novel constraints. Violations in the RTOG R50% value can be rectified with a dose constraint to a novel shell structure ("OptiForR50"). The construction of structure OptiForR50% and the novel optimization criteria translate the RTOG goals for R50% into direct inputs for the optimizer. Violations in the D2cm can be rectified using constraints on a 0.5cm thick shell structure with inner surface 2cm from the PTV surface. Wilcoxon signed-rank test was used to compare differences in dose conformity, volume of hot spots, R50%, D2cm of the target in addition to the OAR doses. Following acute myotoxin injury, denervated muscles formed new muscle fibers in situ. On average, our Italian individuals carried 16.6 protein-truncating variants (PTVs), with 2.5% of the population having a PTV in one of the 59 American College of Medical Genetics (ACMG) actionable genes. Lastly, we looked for PTVs that are likely to cause Mendelian diseases. We found four heterozygous PTVs in haploinsufficient genes (KAT6A, PTCH1, and STXBP1) and three homozygous PTVs in genes causing recessive diseases (DPYD, FLG, and PYGM). Comparing frequencies from our data set to other public databases, like gnomAD, we showed the importance of population-specific databases for a more accurate assessment of variant pathogenicity.
