At least 961 patients recruited in at least 20 centers over 4 years will be needed for the randomized portion of the study. In previous cases, an isolated lesion at the dorsal side of the medulla oblongata induced intractable hiccups. Case descriptions We herein describe a patient with a perimedullary arteriovenous fistula (PMAVF) at the craniocervical junction (CCJ) who had intractable hiccups. A 70-year-old male presented with a 3-year history of intractable hiccups that continued for a few days every week. An initial examination failed to identify the underlying cause, and neither medicine nor self-treatment attenuated his symptoms. Intracranial T2-weighted magnetic resonance imaging showed a hyperintensity area within the dorsolateral medulla and flow voids along the dorsal side of the cervical spine. Angiography revealed PMAVF fed by the left C1 radiculomedullary artery. However, data on invasive brain tissue oxygen monitoring is unknown. Case description We report a case of a pediatric patient with cerebral malaria who developed encephalopathy and cerebral edema and describe the pathophysiology of this disease process with invasive ICP and brain tissue oxygen multimodality neuromonitoring. The utilization of both ICP and brain tissue oxygen monitoring allowed prompt diagnosis and successful treatment of severe intracranial hypertension and low brain tissue oxygenation crisis. The patient was discharged to home in good neurological condition. Conclusion Multimodality neuromonitoring may be considered in pediatric patients who have cerebral edema and encephalopathy from CM.Background Carotid artery stenting (CAS) is an established procedure for the treatment of atherosclerotic disease affecting the extracranial internal carotid artery. Recent population-based studies have suggested that long-term survival after CAS may be limited, thereby questioning its efficacy in a real-world scenario. Methods We retrospectively reviewed outcomes of patients undergoing CAS for asymptomatic or symptomatic carotid stenosis by a neurosurgeon or interventional neuroradiologist at our institution between 2008 and 2018. Patient and disease characteristics were recorded, as was the incidence of peri-procedural and overall ischemia and mortality after CAS. Risk factors for recurrent ischemia and mortality were identified using a Cox proportional hazards model. Results There were 238 patients who met inclusion criteria. Mean age was 69.7 years and the majority of patients were male (69.7%). 62.2% underwent CAS for symptomatic carotid stenosis. The utilization of CAS for treatment of asymptomatic stenosis declined over the study period (p = 0.006). Fourteen patients (5.9%) experienced new or recurrent ipsilateral ischemia during follow-up, with eight (3.4%) experiencing a stroke with permanent neurologic deficit. 59 patients (24.8%) died during follow-up with a median to time to death of 111.3 months (95% CI 95.1 - 133.6) on Kaplan-Meier analysis. Increasing age at time of CAS (Unit Risk ratio (1.05, 95% CI 1.01-1.10, p = 0.011) and comorbid congestive heart failure (RR 2.40, 95% CI 1.39-4.13; p = 0.002) were independent risk factors for mortality during follow-up. However, it is often difficult to detect a CNS-SFT/HPC before surgery. Herein, we describe the case of a presumed left trigeminal schwannoma, which was histopathologically shown to be an HPC. Case description A 65-year-old male presented with paresthesia of the second and third branches of the left trigeminal nerve. Magnetic resonance imaging showed a tumoral lesion in the left cerebellopontine angle and Meckel's cave. We suspected a schwannoma and performed surgery, which resulted in only a partial resection because of tumor bleeding and unexpected intraoperative histological findings. Unfortunately, we could not achieve a definitive diagnosis. However, we observed local recurrence and disseminated lesions 5 and 6.

Previous research investigating individual differences between users and non-users of these platforms has typically focused on one platform. However, individuals typically use a combination of Facebook-owned platforms. Therefore, we aim (1) to identify the relative prevalence of different patterns of social media use, and (2) to evaluate potential between-group differences in the distributions of age, gender, education, and Big Five personality traits. Data collection was performed using a cross-sectional design. Specifically, we administered a survey assessing participants' demographic variables, current use of Facebook-owned platforms, and Big Five personality traits. In N = 3003 participants from the general population (60.67% females; mean age = 35.53 years, SD = 13.53), WhatsApp emerged as the most widely used application in the sample, and hence, has the strongest reach. A pattern consisting of a combined use of WhatsApp and Instagram appeared to be most prevalent among the youngest participants. Further, individuals using at least one social media platform were generally younger, more often female, and more extraverted than non-users. Small differences in Conscientiousness and Neuroticism also emerged across groups reporting different combinations of social media use. Interestingly, when examined as control variables, we found demographic characteristics partially accounted for differences in broad personality factors and facets across different patterns of social media use. In Studies 1 and 2, we found evidence for a correlation with external criteria, that is, teachers with higher test scores reported providing more emotional support for students and having better teacher-student relationships. Therefore, we aim (1) to identify the relative prevalence of different patterns of social media use, and (2) to evaluate potential between-group differences in the distributions of age, gender, education, and Big Five personality traits. Data collection was performed using a cross-sectional design. Specifically, we administered a survey assessing participants' demographic variables, current use of Facebook-owned platforms, and Big Five personality traits. In N = 3003 participants from the general population (60.67% females; mean age = 35.53 years, SD = 13.53), WhatsApp emerged as the most widely used application in the sample, and hence, has the strongest reach. A pattern consisting of a combined use of WhatsApp and Instagram appeared to be most prevalent among the youngest participants. Further, individuals using at least one social media platform were generally younger, more often female, and more extraverted than non-users. Small differences in Conscientiousness and Neuroticism also emerged across groups reporting different combinations of social media use. Interestingly, when examined as control variables, we found demographic characteristics partially accounted for differences in broad personality factors and facets across different patterns of social media use.   • BACKGROUND This study aimed to investigate the efficacy and safety of cytokine-induced killer (CIK)/dendritic cell combined with CIK (DC-CIK) cell therapy in advanced gastrointestinal cancer (GIC). METHODS The PubMed, Cochrane library, and Embase were searched to conduct a meta-analysis of clinical controlled trials to evaluate the efficacy and safety of CIK/DC-CIK cell therapy in advanced GIC. The pooled risk ratios (RRs) or weighted mean difference (WMD) with 95% confidence intervals (95% CIs) were calculated. RESULTS A total of nine studies with 1113 patients were identified. The overall survival (RR = 1.84, 95% CI = 1.41-2.40, Pheterogeneity = 0.654, I2 = 0%), progression-free survival (RR = 1.99, 95% CI = 1.52-2.60, Pheterogeneity = 0.727, I2 = 0%), and quality of life (WMD = 16.09, 95% CI = 1.66-30.52, Pheterogeneity  less then  0.001, I2 = 98.8%) were significantly improved in patients who received chemotherapy combined with CIK/DC-CIK cells, and no severe adverse events were reported. CONCLUSION This meta-analysis suggested that the combination of CIK/DC-CIK immunotherapy and chemotherapy was safe and applicable for patients with advanced GIC. Study-related assessments included the timing of Phe-restricted diets, maternal weight gain, gestational age, pregnancy complications, and blood Phe concentrations (both pre-conception and during pregnancy), obstetrical data, and offspring outcomes(e.g. anthropomorphic measurements and developmental quotients [DQs]). RESULTS A total of six offspring were successfully delivered. The mean ± SD (range) age of the mother at delivery was 26.3 ± 4.7 (range 21.1-32.5) years. The mean duration of Phe control before pregnancy was 5.5 ± 1.3(range 3.1-6.5) months. During pregnancy, the proportion of blood Phe concentrations within the clinically-recommended target range (120-360 μmol/L) ranged from 63.2-83.5%. Low birth weight ( less then  2500 g) offspring occurred in two women who experienced suboptimal metabolic control. In addition, offspring DQ was related to the proportion of blood Phe levels per trimester that were within the recommended range (r = 0.886, p = 0.016). CONCLUSION This is the first report of women in China with PKU who successfully gave birth to clinically healthy babies. Infant outcomes were related to maternal blood Phe management prior to and during pregnancy. METHOD This study used a Grounded Theory methodology. Data from 13 women choosing homebirth and 15 choosing freebirth were collected between 2010 and 2014 and analysed over this time. RESULTS The core category was 'wanting the best and safest,' which describes what motivated the women to birth outside the system. The basic social process, which explains the journey women took as they pursued the best and safest, was 'finding a better way'. Women who gave birth outside the system in Australia had the countercultural belief that their knowledge about what was best and safest had greater authority than the socially accepted experts in maternity care. The women did not believe the rhetoric about the safety of hospitals and considered a biomedical approach towards birth to be the riskier birth option compared to giving birth outside the system. Previous birth experiences taught the women that hospital care was emotionally unsafe and that there was a possibility of further trauma if they returned to hospital. Giving birth outside the system presented the women with what they believed to be the opportunity to experience the best and safest circumstances for themselves and their babies. CONCLUSION Shortfalls in the Australian maternity care system is the major contributing factor to women's choice to give birth outside the system. Genetic relatedness of the cows in the herds was the cause of such small Fst values.     While the benefit of mechanical thrombectomy (MT) for patients with anterior circulation acute ischemic stroke with large-vessel occlusion (AIS-LVO) has been clearly established, difficult vascular access may make the intervention impossible or unduly prolonged. In this study, the authors evaluated safety as well as radiographic and functional outcomes in stroke patients treated with MT via direct carotid puncture (DCP) for prohibitive vascular access.

    The authors retrospectively studied patients from their prospective AIS-LVO database who underwent attempted MT between 2015 and 2018. Patients with prohibitive vascular access were divided into two groups 1) aborted MT (abMT) after failed transfemoral access and 2) attempted MT via DCP. Functional outcome was assessed using the modified Rankin Scale at 3 months. Associations with outcome were analyzed using ordinal logistic regression.

    Of 352 consecutive patients with anterior circulation AIS-LVO who underwent attempted MT, 37 patients (10.5%) were deemeller infarct volumes (11 vs 48 ml, p = 0.04), a greater reduction in NIHSS score (-4 vs +2.9, p = 0.03), and better functional outcome (shift analysis for 3-month modified Rankin Scale score adjusted OR 5.2, 95% CI 1.02-24.5; p = 0.048).

    Traditional stereo-electroencephalography (sEEG) entails the use of orthogonal trajectories guided by seizure semiology and arteriography. Such trajectories provide access to components of seizure networks not readily sampled using orthogonal trajectories. However, the dogma regarding the relative safety and predictability of orthogonal and azimuth-based trajectories persists, given the absence of data regarding the safety and efficacy of oblique sEEG trajectories.     The authors report the outcomes of an institutional case of congenital GBM diagnosed within the first 3 months of life, and performed a comprehensive literature search for published cases from 2000 onward for an integrated survival analysis. For this purpose, a group of Sprague-Dawley male and female rats were fed either with a hypercaloric diet as model of the metabolic syndrome or with a standard chow diet as controls. Subsequently, spatial learning and memory (Morris water maze) as well as short and long-term memory (passive avoidance task) were evaluated. Body weight, blood pressure, triglycerides, and total cholesterol significantly increased (F(1,36) =94.89, P less then 0.001) in rats fed with hypercaloric diet compared to control rats. Furthermore, cognitive impairment was observed in spatial learning and spatial memory on male rats but not on female rats fed with hypercaloric diet. In addition, a long-term memory impairment was observed in both groups fed with hypercaloric diet in comparison to their respective control group (F(1,32)=10.61, P= 0.0027). Immunohistochemistry results showed no changes in the number of positive cells for NeuN, GFAP and Ox-42. In males fed with a hypercaloric diet, a decrease in testosterone levels was observed, whereas estradiol levels decreased in females when compared with their respective control group (P less then 0.0001). In this MetS animal model, metabolic and cognitive differences were observed in males and females, which demonstrates that sex hormones play a significant role in metabolic regulation and neuroprotection related to the CA1 region of the hippocampus. We developed a computer simulation framework for modeling the residence time distribution (RTD) of integrated continuous downstream processes based on a unit-by-unit modeling approach in which unit operations are simulated one-by-one across the entire processing time, and then combined into an integrated RTD model. The framework allows for easy addition or replacement of new unit operations, as well as quick adjustment of process parameters during evaluation of the RTD model. With this RTD model, the start-up phase to reach steady state can be accelerated, the effects of process disturbances at any stage of the process can be calculated, and virtual tracking of a section of the inlet material throughout the process is possible. The aim of this study was to describe the annual incidence, type and severity of injuries and illnesses among Swedish Paralympic athletes and to assess risk factors. METHODS Swedish Paralympic athletes (n=107) self-reported SRIIPS every week during 52 weeks using an eHealth application. Incidence proportions (IP) and incidence rates (IR) were used as measures of disease burden. Time-to-event methods (Kaplan Meier and Cox regression) were used to identify risk factors. RESULTS The annual IP for injury was 68% and for illness 77%. The injury IR was 6.9/1000 hours and the illness IR 9.3/1000 hours. The median time to injury was 19 weeks (95% CI 10.5-27.4) and to illness 9 weeks (95% CI 1.4-16.6). Most injuries occurred during training and 34% were classified as severe (≥21 days of time loss). An increased injury risk was observed among athletes in team sports (HR 1.88; 95% CI 1.19-2.99), athletes with a previous severe injury (HR 2.37; 95% CI 1.47-3.83) and male athletes (HR 1.76; 95% CI 1.06-2.93). The most common illness type was infection (84%). Athletes in team sports (HR 1.64; 95% CI 1.05-2.54) and males with a previous illness (HR=2.13; 95% CI 1.04-4.36) had a higher illness risk. CONCLUSION Paralympic athletes report a high incidence of injuries and illnesses over time.   • Aldose reductase (AR) is the first enzyme of the polyol pathway that has physiological importance under hyperglycaemic conditions. The article has been focussed on AR enzyme inhibition by selected compounds. For this purpose, the in vitro inhibitory effects of various compounds on commercially available recombinant human AR (rAR) enzyme activity were investigated. The IC50 values of compounds on rAR inhibition effect were found for 6-hydroxy flavone, syringic acid, diosmetin, 6-fluoroflavone, 7-hydroxy-4'-nitroisoflavone, myricetin as 2.05, 2.97, 15.75, 16.1, 49.5, and 63 µM, respectively. 6-Hydroxy flavone and syringic acid competitively inhibited rAR with respect to the NADPH with Ki values 0.509 ± 0.036 and 0.842 ± 0.012 µM. In addition, docking studies were performed to evaluate the potential enzyme binding positions of the compounds. Our in vitro and in silico results indicated that the 6-hydroxy flavone may be a good lead compound in the development of AR inhibitors to prevent diabetic complications.Objective This study aimed to clarify whether plasma acrolein level actually increases in rheumatoid arthritis (RA) patients, and to elucidate whether any relationship exists between the levels and the RA background variables.Methods Plasma levels of protein-conjugated acrolein (PC-Acro) in 84 patients (RA group) and 298 normal individuals (Control group) were measured by enzyme-linked immunosorbent assay procedures. The data were statistically analyzed with Wilcoxon rank-sum test, multiple logistic regression analyses and Spearman's rank correlation coefficient.Results The RA group showed significantly higher PC-Acro levels than the Control group (median [interquartile range] 80.5 [63.2-105.2] and 65.9 [58.9-78.1] nmol/ml, respectively). Of background factors giving influence to PC-Acro level in the combination of the two groups, 'diagnosis of RA positive' indicated strong correlation to high PC-Acro level (odds ratio 2.96; 95% confidence interval 1.54-5.71). These increases of PC-Acro in the RA patients did not correlate to their disease duration and/or inflammatory variables PC-Acro level could elevate even in early RA patients showing negative inflammatory findings.Conclusion Plasma levels of PC-Acro increased with RA, but the levels did not correlate with RA background variables. It was detected at least one complication in 53 (34.0%) eyes at presentation. Method Participants included 22 children with DS and 22 children with TD between 22 and 63 months of age and their mothers. Each mother-child dyad participated in a 7-min naturalistic shared book reading activity. Results Compared to mothers of children with TD, mothers of children with DS used significantly more utterances with less grammatical complexity, but a similar range of vocabulary diversity. Mothers of children with DS used more questions, descriptions, gestures, and labels, whereas mothers of children with TD used nearly half of their utterances to read directly from books.

The data were statistically analyzed with Wilcoxon rank-sum test, multiple logistic regression analyses and Spearman's rank correlation coefficient.Results The RA group showed significantly higher PC-Acro levels than the Control group (median [interquartile range] 80.5 [63.2-105.2] and 65.9 [58.9-78.1] nmol/ml, respectively). Also examined are the potential effects of yoga programming for people who are incarcerated, for those working within carceral settings, and on carceral environments generally. We share recommendations for implementing yoga programming in carceral institutions and discuss policy implications. Fifty-three patients were female and 40 were male. Mean age at onset of the uveitis was 9.54 ± 4.29 years. The mean follow-up period was 29.88 ± 28.97 months. Anterior uveitis (49.5%) was the most common anatomic type followed by panuveitis (21.5%), intermediate uveitis (18.3%), and posterior uveitis (10.7%). Juvenile idiopathic arthritis (JIA) was the most common leading systemic disease (18.3%) followed by Behçet disease (11.8%). It was detected at least one complication in 53 (34.0%) eyes at presentation. Method Participants included 22 children with DS and 22 children with TD between 22 and 63 months of age and their mothers. Each mother-child dyad participated in a 7-min naturalistic shared book reading activity. Results Compared to mothers of children with TD, mothers of children with DS used significantly more utterances with less grammatical complexity, but a similar range of vocabulary diversity. Mothers of children with DS used more questions, descriptions, gestures, and labels, whereas mothers of children with TD used nearly half of their utterances to read directly from books.

  • Enterobacterales and other non-fermenting Gram-negative bacteria have become a threat worldwide owing to the frequency of multidrug resistance in these pathogens. On the other hand, efficacious therapeutic options are quickly diminishing. The aims of this study were to describe the susceptibility of 50 multiresistant Gram-negative bacteria, mostly pan-resistant, against old and less-used antimicrobial drugs and to investigate the presence of antimicrobial resistance genes.

    A total of 50 genetically distinct isolates were included in this study, including 14 Acinetobacter baumannii (belonging to ST79, ST317, ST835 and ST836), 1 Pseudomonas aeruginosa (ST245), 8 Serratia marcescens and 27 Klebsiella pneumoniae (belonging to ST11, ST340, ST258, ST16, ST23, ST25, ST101, ST234, ST437 and ST442). The isolates were submitted to antimicrobial susceptibility testing and whole-genome sequencing to evaluate lineages and resistance genes.

Recovery studies were conducted by spiking OTA in real food samples, including white wine, red wine, cereal drink, coffee beverage and tea beverage, and confirmed desirable accuracy and practical applicability of our method. We here report a further remarkable resolution improvement in the spectra of four fully protonated proteins and a small drug molecule, by pushing the MAS rotation frequency even higher, to 150 kHz, than the more routinely used 100 kHz. We observe a reduction of the average homogeneous linewidth by a factor 1.5 and a decrease in the observed linewidth by a factor 1.25. Materials and methods This study was a retrospective review of patient records. Fifty consecutive pediatric patients (aged 20 ng/mL, respectively. Associations between serum 25(OH)D3 levels and pulmonary infection were analyzed. Results Of 50 pediatric patients who underwent LDLT, 19 (38%) developed pulmonary infections in the early postoperative period. The mean serum 25(OH)D3 level in these subjects was 18.7±17.2 ng/mL (range 3.0-70.0 ng/mL). Twenty patients (40%) had severe vitamin D deficiency ( less then 10 ng/mL). The mean serum 25(OH) D3 level was significantly decreased (9.3±7.4 vs 24.5 ± 19.1 ng/mL, p = 0.002) in patients with pulmonary infection compared to those without pulmonary infection. Serum 25(OH)D3 level as a continuous variable (odds ratio (OR) 0.90, 95% confidence interval (CI) 0.84-0.97, P = 0.008) and a classification variable (≤10 ng/mL) (OR 7.42, 95% CI, 2.06-26.79, P = 0.002) were significantly associated with pulmonary infection in univariate analysis. After adjusting for other significant predictors (age, weight and pediatric end-stage liver disease (PELD) score), severe 25(OH)D3 deficiency at presentation was independently associated with a higher risk of developing pulmonary infection in the early postoperative period (OR 5.11, 95% CI, 1.30-20.16, p = 0.02). Objectives of this study was to determine effects of exogenous Spd on changes of endogenous polyamines and γ-aminobutyric acid metabolism, oxidative damage, senescence response, and heat shock proteins (HSPs) expression in white clover under heat stress. Physiological and molecular methods including colorimetric assay, high performance liquid chromatography, and qRT-PCR were applied. Results showed that exogenous Spd significantly alleviated heat-induced stress damage. Spd application not only increased endogenous putrescine (Put), Spd, spermine (Spm), and total polyamine (PAs) accumulation, but also accelerated PAs oxidation and improved glutamic acid decarboxylase activity leading to GABA accumulation in leaves under heat stress. Spd-pretreated white clover maintained significantly higher chlorophyll (Chl) content than untreated plants under heat stress, which could be related to roles of Spd in up-regulating genes encoding Chl synthesis (PBGD and Mg-CHT) and maintaining lower Chl degradation (PaO and CHLASE) during heat stress. In addition, Spd up-regulated HSP70, HSP70B, and HSP70-5 expression that could have function of stabilizing denatured protein and helping proteins to folding correctly in white clover under high temperature stress. After adjusting for other significant predictors (age, weight and pediatric end-stage liver disease (PELD) score), severe 25(OH)D3 deficiency at presentation was independently associated with a higher risk of developing pulmonary infection in the early postoperative period (OR 5.11, 95% CI, 1.30-20.16, p = 0.02). Spermidine (Spd) is small aliphatic amine and acts as ubiquitous plant growth regulator to regulate plant growth, development, and stress tolerance. Objectives of this study was to determine effects of exogenous Spd on changes of endogenous polyamines and γ-aminobutyric acid metabolism, oxidative damage, senescence response, and heat shock proteins (HSPs) expression in white clover under heat stress. Physiological and molecular methods including colorimetric assay, high performance liquid chromatography, and qRT-PCR were applied. Results showed that exogenous Spd significantly alleviated heat-induced stress damage. Spd application not only increased endogenous putrescine (Put), Spd, spermine (Spm), and total polyamine (PAs) accumulation, but also accelerated PAs oxidation and improved glutamic acid decarboxylase activity leading to GABA accumulation in leaves under heat stress. Spd-pretreated white clover maintained significantly higher chlorophyll (Chl) content than untreated plants under heat stress, which could be related to roles of Spd in up-regulating genes encoding Chl synthesis (PBGD and Mg-CHT) and maintaining lower Chl degradation (PaO and CHLASE) during heat stress. In addition, Spd up-regulated HSP70, HSP70B, and HSP70-5 expression that could have function of stabilizing denatured protein and helping proteins to folding correctly in white clover under high temperature stress.
