Although the field of natural product extraction with ionic liquids (ILs) is already a crowded one, the guiding rules regarding the selection of suitable ILs for extraction are still lacking. In this study, the extraction capacity of 32 ILs was investigated using Puerariae lobatae as a testing material, namely, 11 [BMIM]-based, 9 [CMIM]-based, 8 [C n MIM][Br] and 8 [C n MIM][BF4] ILs (n = 2, 4, 6, 8, 10, 12, 14, 16) were studied. Three series of concentrations (0.02, 0.50, 2.00 M) were investigated, and the results demonstrated that the extraction capacity increased with the rising IL concentrations except for 1-butyl-3-methyl-imidazolium tosylate ([BMIM][Tos]). Generally, all the 0.50 and 2.00 M ILs led to the best extraction results. At low concentrations, the IL extraction capacity is strongly dependent on the anions and cations. However, the relationship between extraction capacity and anions or cations gradually becomes weak as increasing IL concentrations. Finally, the solution of ILs was prepared in aqueous or 60% methanol depending on the solubility. The pH can vary from strong acid to weak alkaline. Extraction capacity of 32 ILs varying in concentrations has a good negative correlation with the pH, except for five [BMIM]-ILs. Here, evaluations of LiMBO3 and NaMBO3 (M = Mn, Fe, Co, Ni) as cathode materials of Li-ion and Na-ion batteries, respectively, are performed in the density functional theory (DFT) framework. The structural properties, structural stability after deintercalation, cell voltage, electrical conductivity, and rate capability of the cathodes are assessed. As a result, Li compounds have more structural stability and energy density than Na compounds in the C2/c frame structure. Cell voltage is increased by increasing the atomic number of the transition metal (TM). A noble approach is used to evaluate electrical conductivity and rate capability. M = Fe compounds exhibit the lowest band gaps (BGs), and M = Mn compounds exhibit almost the highest one. The large differences of Stokes shifts among diTyr residues, CdSQDs, and AuNCs ensure their emission at a single excitation wavelength. The nanoprobes can be facilely integrated using two-step synthetic reactions. DiTyr residues and AuNCs are formed and bound to the protein cage through the redox reaction between Au3+ and tyrosine residues of BSA, and the CdSQDs are followed to be conjugated to the modified BSA cage-templated CdS combination reaction. With established benign biocompatibility, the nanoprobes can ratiometrically detect intracellular glutathione by significantly enhancing the green emission of the conjugated CdSQDs. The alkali activators were used to fully hydrate the calcium and silicon raw materials, which further improved the comprehensive performance of the CCSBs made of four pure industrial solid wastes. Within the range of dosage in this study, single doping of different proportions of the alkali activator improved the flexural strength of the CCSB. Based on this, the mechanical properties of the CCSB were further improved as the compounded alkali activator was optimized. Flexural strength is improved when the average pore diameter was refined. The freeze-thaw cycle test shows that a compound-doped alkali activator can effectively reduce the mass loss and strength loss, thereby improving the frost resistance of this material. This research discussed an economically affordable approach to prepare the CCSB material made of industrial solid waste. The oxygen evolution photocatalyst BiVO4 was prepared by the microwave-assisted hydrothermal method. The method is fast and simple, as compared to conventional hydrothermal synthesis. One-third of all deaths in hospitals are caused by sepsis. Despite its demonstrated prevalence and high case fatality rate, antibiotics remain the only target-oriented treatment option currently available. Starting from results showing that low-dose anthracyclines protect against sepsis in ****, we sought to find new causative treatment options to improve sepsis outcomes.
Sepsis was induced in ****, and different treatment options were evaluated regarding cytokine and biomarker expression, lung epithelial cell permeability, autophagy induction, and survival benefit. Results were validated in cell culture experiments and correlated with patient samples.
Effective low-dose epirubicin treatment resulted in substantial downregulation of the sphingosine 1-phosphate (S1P) degrading enzyme S1P lyase (SPL). Consequent accumulation and secretion of S1P in lung parenchyma cells stimulated the S1P-receptor type 3 (S1PR3) and mitogen-activated protein kinases p38 and ERK, reducing tissue damage via increased disease tolerance. The protective effects of SPL inhibition were absent in S1PR3 deficient ****. Sepsis patients showed increased expression of SPL, stable expression of S1PR3, and increased levels of mucin-1 and surfactant protein D as indicators of lung damage.
Our work highlights a tissue-protective effect of SPL inhibition in sepsis due to activation of the S1P/S1PR3 axis and implies that SPL inhibitors and S1PR3 agonists might be potential therapeutics to protect against sepsis by increasing disease tolerance against infections.

Gene therapy has held promises for treating specific genetic diseases. However, the key to clinical application depends on effective gene delivery.
Using a large animal model, we developed two pharmaceutical formulations for gene delivery in the pigs' vagina, which were made up of poly (β-amino ester) (PBAE)-plasmid polyplex nanoparticles (NPs) based two gel materials, modified montmorillonite (mMMT) and hectorite (HTT).
By conducting flow cytometry of the cervical cells, we found that PBAE-GFP-NPs-mMMT gel was more efficient than PBAE-GFP-NPs-HTT gel in delivering exogenous DNA intravaginally. Next, we designed specific CRISPR/SpCas9 sgRNAs targeting porcine endogenous retroviruses (PERVs) and evaluated the genome editing efficacy in vivo. We discovered that PERV copy number in vaginal epithelium could be significantly reduced by the local delivery of the PBAE-SpCas9/sgRNA NPs-mMMT gel. Comparable genome editing results were also obtained by high-fidelity version of SpCas9, SpCas9-HF1 and eSpCas9, in touth Africa under BRICS Multilateral Joint Call for Proposals; grant 17-54-80078 from the Russian Foundation for Basic Research.

In China, practitioners have been using this test with the assumption that the published UK norms are valid for Chinese children. However no systematic investigation has previously been undertaken to check this assumption. 2185 children aged 3-10 years old from a national representative sample in China were therefore recruited to the current study. Performance on the Movement ABC-2 was assessed and compared with the UK standardization norms. Gender differences were also examined. The comparisons revealed that Chinese children were generally better in Manual Dexterity and Balance tasks compared to their UK peers; while UK children were better in Aiming & Catching tasks. Further analysis showed an interaction of country and age with mixed results. For both countries, girls were generally better in Manual Dexterity and Balance tasks, and boys were generally better in Aiming & Catching. Possible explanations for the country differences are discussed. The results suggest that local norms for the Movement ABC-2 Test are needed in China.The mechanisms behind nitrogen removal by the submersed macrophyte-biofilm complex in wetlands remain to be fully elucidated. This study investigated the role of Myriophyllum spicatum and the biofilm on their leaves in nitrogen removal in mesocosm experiments. 15N tracking showed that 61.9% and 30% of the 15N, respectively, was removed from the system and assimilated by the macrophyte-biofilm complex after loading with 5.4 mg L-115N labelled NH4+ for 17 days. Nitrogen budget results showed that about 0.2%, 0.2% and 3.6% of the nitrogen were emitted as water-, HCl- and NaOH-soluble nitrogen-gas species, respectively. Bacteria (76.7-91.8%) were the predominant domain in all samples, followed by eukaryotes (8.0-23.0%), archaea and viruses. Network analyses showed that there were positive- and negative-correlative relationships among nitrogen-cycling genes and nitrifiers and denitrifiers. Our data highlight the important role of biofilm on submersed macrophytes for nitrogen removal.RAS is the most frequently mutated oncogene in human cancers, with mutations in about 30% of all cancers. Mutated RAS is constitutively activated and persistently turned "on" thereby enhancing downstream signaling and leading to tumorigenesis. Various attempts to inhibit Kras in the past were unsuccessful. Recently, several small molecules (AMG510, MRTX849, JNJ-74699157, and LY3499446) have been developed to specifically target K-RAS G12C. Additionally, various other approaches including, SHP2, SOS1 and eIF4 inhibition, have been utilized to abrogate tumor growth in K-RAS mutant cells, resulting in a renewed interest in this pathway.

Quantitative characterization of intracellular H2O2 content, which is still difficult by the conventional biochemical methods due to the lack of real-time and non-invasive technique of single cell measurement, is a useful solution for cell state assessment. Based on the surface enhanced Raman scattering (SERS), we construct a novel boric acid (BA) nanoprobe to perform quantitative characterization of H2O2 content, in which the p-thiol benzene boric acid (4-MPBA) reporter molecule modified with gold nanorods (AuNRs) is employed for Raman signal enhancement. The achieved result demonstrates obvious advantages of the synthesized AuNRs/4-MPBA/BA nanoprobe in measurement sensitivity of H2O2 content. Shown, that the range of 1306-1250cm-1 in the IR spectrum of blood serum is diagnostically significant for breast cancer. This range corresponds to the vibrations of several functional groups of DNA and RNA, which play a key role in discrimination in breast cancer screening using ATR-FTIR spectroscopy. It is shown that the proposed method has advantages in ease of use for clinical diagnosis and gives good results for the identification of breast cancer. The values of sensitivity (92.3%) and specificity (87.1%) obtained using the PCR method are close to those of mammography and ultrasound. In patients with low HFC proportion ( less then 35%), severe anxiety could significantly decrease HFC proportion. The HFC proportion positively correlated with immediate and delayed memory in depressed patients. Further analysis showed that patients with low HFC proportion may have worse delayed memory. LIMITATIONS The lack of prior exposure to the monitoring equipment and procedure could have generated artefacts that would have disappeared after habituation. CONCLUSIONS These results support a positive correlation between the HFC proportion and memory in depressed patients. Further research is required to explore the clinical implications of these findings. V.BACKGROUND Anxiety disorders have been related to cardiovascular diseases via low-grade inflammation, but longitudinal studies on the association between generalized anxiety disorder (GAD) and inflammatory biomarkers are sparse. Furthermore, no studies have examined the association between GAD and the "cardio-protective" adipocytokine adiponectin in this context so far. METHODS In a Swiss population-based sample of 2,415 adults participating in baseline and follow-up exams (mean follow-up duration=5.5 years), we diagnosed a total of 55 persons (2.3%) with GAD using a validated semi-structured psychiatric interview. We prospectively examined the relation between GAD and circulating levels of inflammatory biomarkers (i.e., C-reactive protein, interleukin (IL)-1β, IL-6, tumor necrosis factor-α, and adiponectin), in linear regression models, statistically controlled for the baseline inflammatory marker, socioeconomic status, cardiovascular risk factors, health behaviors, and psychiatric disorders. RESULTS Compared to those without GAD, individuals with GAD had lower IL-6 (β=-0.249, 95%-CI -0.493-(-0.004), p=0.046), and adiponectin (β=-0.264, 95%-CI -0.482-(-0.045), p=0.018) levels at follow-up after adjustment for all covariates. Moreover, GAD was unrelated to several other inflammatory measures. CONCLUSION Individuals with GAD do not seem to exhibit chronic low-grade inflammation, suggesting different underlying biobehavioral mechanisms to those from other anxiety disorders. Low adiponectin levels may be linked to symptoms of GAD through brain areas directly involved in the processing of fear and anxiety. Secondly, we assessed the association between S-DMI and O-DMI. Finally, we explored sociodemographic, clinical, and psychosocial factors related to decision-making impairment status. METHOD One hundred and nine people with MDD were assessed with the Iowa Gambling Task to identify "O-DMI". The item 9 of the **** Depression Inventory was used to compose the variable "S-DMI". Psychiatric history, medication adherence, childhood trauma, physical and psychological pain, and negative life experiences were also collected. RESULTS Forty-six percent of participants have O-DMI and 65.1% S-DMI. From 49.5% showing a discrepancy between both, 68,6% have positive discrepancy and 31,4% negative (i.e. under and overestimation respectively). O-DMI and S-DMI were not associated. Binary logistic regressions showed that the number of negative life events in the past 6 months was predictor of O-DMI, while depression severity and medication adherence predicted S-DMI. Finally, medication adherence and depression severity predicted the type of discrepancy. LIMITATIONS The variable S-DMI was obtained through a single item. CONCLUSION These results confirm the lack of association between objective and subjective measures of cognition in MDD, and for the first-time concerning decision-making. Primary total knee arthroplasty (TKA) may be of benefit in elderly patients with a combination of osteoporotic bone and metaphyseal comminution. However, there continues to be conflicting evidence on the use of TKA for primary treatment of tibial plateau fracture. This systematic review was performed to quantify the outcomes and perioperative complication rates of TKA for primary treatment of tibial plateau fracture. Materials and methods A comprehensive search of MEDLINE, Embase, and PubMed databases from inception through March 2018 was performed in accordance with PRISMA guidelines. Two reviewers independently screened papers for inclusion and identified studies featuring perioperative complications and outcomes of primary TKA for tibial plateau fracture. Weighted means and standard deviations are presented for each outcome. Primary total knee arthroplasty (TKA) may be of benefit in elderly patients with a combination of osteoporotic bone and metaphyseal comminution. However, there continues to be conflicting evidence on the use of TKA for primary treatment of tibial plateau fracture. This systematic review was performed to quantify the outcomes and perioperative complication rates of TKA for primary treatment of tibial plateau fracture. Materials and methods A comprehensive search of MEDLINE, Embase, and PubMed databases from inception through March 2018 was performed in accordance with PRISMA guidelines. Two reviewers independently screened papers for inclusion and identified studies featuring perioperative complications and outcomes of primary TKA for tibial plateau fracture. Weighted means and standard deviations are presented for each outcome. In the matched cohort for cT1a cancers, ER had similar survival to esophagectomy [hazard ratio (HR) 0.85, 95% confidence interval (CI) 0.70-1.04, P = 0.1]. For cT1b cancers, there was no statistically significant difference in overall survival between the treatment groups (HR 0.87, 95% CI 0.66-1.14, P = 0.3). Results Baseline characteristics were similar in the intervention (n = 85) and control (n = 111) group. Median time to functional recovery was 7 and 9 days in the intervention and control group (P less then 0.001), respectively. Length of hospital stay was 8 versus 10 days (P less then 0.001), respectively. Thirty-day postoperative complication rate was significantly reduced in the intervention group (57.6% vs 73.0%, P = 0.024). hen designing exercise studies to promote bone strength. Bioelectrochemical systems (BES) have long been viewed as a promising wastewater treatment technology. However, in reality, the performance of bioelectrochemical systems fed with real (and therefore complex) wastewaters is often disappointing. We have sought to investigate the combined impacts of complex substrates and presence of electron acceptors. In particular, this study illustrates and systematically evaluates the disparity in performance between a BES acclimatised with acetate and those acclimatised with more complex carbohydrates (glucose, sucrose or starch) and in the presence and absence of sulphate. Relative to acetate only, operating with complex carbohydrates reduced current by 73%-87% and coulombic efficiency by 4%-50%. Acclimation with complex carbohydrates seriously impeded the colonisation anode by Geobacteraceae, resulting in substantially reduced capacity to produce current (60.2% on average). Combined acclimation with sulphate further reduced current by 35% on average, and resulted in a total reduction of 83%-93% relative to the acetate control. However, the presence of an electrogenic sulphide-sulphur shuttle meant sulphate had little effect on the coulombic efficiency of the BES. The results indicate that a reduction in current and coulombic efficiency is, at present, an unavoidable consequence of operating a BES fed with complex wastewater. Researchers, designers and policy makers should incorporate such losses in both their plans and their prognostications. Recreational water quality guidelines protect the public from health risks associated with water recreation by helping to prevent unacceptable concentrations of pathogenic organisms in ambient water. However, illness risk is associated with both the concentration of pathogens in the water and the degree of contact with those pathogens. Different recreational activities can result in different levels of contact with ambient water containing water-borne pathogens. We conducted a systematic literature review and meta-analysis to evaluate risks of illness associated with different recreational activities and different levels of contact to ambient surface waters. We screened 8,618 potentially relevant studies for quantitative measures of risk using inclusion/exclusion criteria established in advance. We categorized recreational activities as swimming, sports-related contact, minimal contact, and sand contact. We combined relative risks using a random effects meta-analysis for adverse health outcome categories representing gastrointestinal illness, respiratory illness, skin, eye, ear, nose, throat, and cold/flu illness. We identified 92 studies meeting our inclusion criteria. Pooled risk estimates indicate significant elevation of gastrointestinal illness with the recreational activity categories swimming (2.19, 95% CI 1.82, 2.63) and sports-related contact (2.69, 95% CI 1.04, 6.92), and nonsignificant elevation of gastrointestinal illness with minimal contact (1.27, 95% CI 0.74, 2.16). We also found a significant elevation of respiratory illness with swimming (1.78, 95% CI 1.38, 2.29) and sports-related contact (1.49, 95% CI 1.00, 2.24), and no elevation of respiratory illness with minimal contact (0.90, 95% CI 0.71, 1.14). This study suggests that exposures associated with different types of recreational activities are important characteristics of the exposure pathway when assessing illness risk associated with recreation in ambient surface waters. OBJECTIVES To investigate the mechanisms of susceptibility to inflammation in Yusho patients, peripheral immune cell fractions and concentrations of inflammatory cytokines were evaluated in blood samples collected from both Yusho patients and age-matched healthy subjects undergoing medical examination in Nagasaki. METHODS To exclude diagnostic uncertainty, serum levels of polychlorinated biphenyl (PCB), polychlorinated quarterphenyl (PCQ), and polychlorinated dibenzofuran (PCDF) were measured. Immune cell (e.g. natural killer and regulatory T cell) populations were analyzed by flow cytometry. Serum cytokines involved in immune cell activation were measured by ELISA. RESULTS The relative proportion of natural killer cells was higher in Yusho patients than in healthy subjects, while the proportion of regulatory T cells did not differ between groups. Serum concentrations of IL-36 and IFN-γ were significantly lower in Yusho patients than in healthy subjects. Conversely, serum cytotoxic T lymphocyte-associated antigen-4 (CTLA-4), which is a cytokine related to activated NK cells, was higher in Yusho patients than in healthy subjects and was positively correlated with PCDF blood levels. CONCLUSION Increased numbers of NK cells in Yusho patients suggests that the innate immune response has been activated in Yusho patients. hen designing exercise studies to promote bone strength. Bioelectrochemical systems (BES) have long been viewed as a promising wastewater treatment technology. However, in reality, the performance of bioelectrochemical systems fed with real (and therefore complex) wastewaters is often disappointing. We have sought to investigate the combined impacts of complex substrates and presence of electron acceptors. In particular, this study illustrates and systematically evaluates the disparity in performance between a BES acclimatised with acetate and those acclimatised with more complex carbohydrates (glucose, sucrose or starch) and in the presence and absence of sulphate. Relative to acetate only, operating with complex carbohydrates reduced current by 73%-87% and coulombic efficiency by 4%-50%. Acclimation with complex carbohydrates seriously impeded the colonisation anode by Geobacteraceae, resulting in substantially reduced capacity to produce current (60.2% on average). Combined acclimation with sulphate further reduced current by 35% on average, and resulted in a total reduction of 83%-93% relative to the acetate control. However, the presence of an electrogenic sulphide-sulphur shuttle meant sulphate had little effect on the coulombic efficiency of the BES. The results indicate that a reduction in current and coulombic efficiency is, at present, an unavoidable consequence of operating a BES fed with complex wastewater. Researchers, designers and policy makers should incorporate such losses in both their plans and their prognostications. Recreational water quality guidelines protect the public from health risks associated with water recreation by helping to prevent unacceptable concentrations of pathogenic organisms in ambient water. However, illness risk is associated with both the concentration of pathogens in the water and the degree of contact with those pathogens. Different recreational activities can result in different levels of contact with ambient water containing water-borne pathogens. We conducted a systematic literature review and meta-analysis to evaluate risks of illness associated with different recreational activities and different levels of contact to ambient surface waters. We screened 8,618 potentially relevant studies for quantitative measures of risk using inclusion/exclusion criteria established in advance. We categorized recreational activities as swimming, sports-related contact, minimal contact, and sand contact. We combined relative risks using a random effects meta-analysis for adverse health outcome categories representing gastrointestinal illness, respiratory illness, skin, eye, ear, nose, throat, and cold/flu illness. We identified 92 studies meeting our inclusion criteria. Pooled risk estimates indicate significant elevation of gastrointestinal illness with the recreational activity categories swimming (2.19, 95% CI 1.82, 2.63) and sports-related contact (2.69, 95% CI 1.04, 6.92), and nonsignificant elevation of gastrointestinal illness with minimal contact (1.27, 95% CI 0.74, 2.16). We also found a significant elevation of respiratory illness with swimming (1.78, 95% CI 1.38, 2.29) and sports-related contact (1.49, 95% CI 1.00, 2.24), and no elevation of respiratory illness with minimal contact (0.90, 95% CI 0.71, 1.14). This study suggests that exposures associated with different types of recreational activities are important characteristics of the exposure pathway when assessing illness risk associated with recreation in ambient surface waters. More than 50 years have passed since the Yusho incident, and although inflammatory disorders such as suppuration have been observed in Yusho patients, the etiology of this inflammation susceptibility remains obscure. OBJECTIVES To investigate the mechanisms of susceptibility to inflammation in Yusho patients, peripheral immune cell fractions and concentrations of inflammatory cytokines were evaluated in blood samples collected from both Yusho patients and age-matched healthy subjects undergoing medical examination in Nagasaki. METHODS To exclude diagnostic uncertainty, serum levels of polychlorinated biphenyl (PCB), polychlorinated quarterphenyl (PCQ), and polychlorinated dibenzofuran (PCDF) were measured. Immune cell (e.g. natural killer and regulatory T cell) populations were analyzed by flow cytometry. Serum cytokines involved in immune cell activation were measured by ELISA. RESULTS The relative proportion of natural killer cells was higher in Yusho patients than in healthy subjects, while the proportion of regulatory T cells did not differ between groups. Serum concentrations of IL-36 and IFN-γ were significantly lower in Yusho patients than in healthy subjects. Conversely, serum cytotoxic T lymphocyte-associated antigen-4 (CTLA-4), which is a cytokine related to activated NK cells, was higher in Yusho patients than in healthy subjects and was positively correlated with PCDF blood levels. CONCLUSION Increased numbers of NK cells in Yusho patients suggests that the innate immune response has been activated in Yusho patients. Results Out of a total of 104 patients, the mean age was 24 years, the sex ratio was 0.51 (women 66.3%), twenty (19.2%) patients received medical treatment alone, and 84 (80.8%) patients required medical and surgical treatment. Surgery was required when the size of the lymphadenopathies was greater than or equal to 3 cm (p = 0.005), when the patient presented with an abscess and/or fistula(p = 0.005), when the patient presented with resistance to antibacillary drugs(p = 0.032), or developed a paradoxical upgrading reactions (p = 0.001), or when the patient had a recurrence of lymph node tuberculosis(p = 0.008) on multivariate analysis. But, emergency resection of stage III tumors had significantly decreased 3-years disease free and overall survival rates although there was no significant increase in cancer specific mortality rate. Conclusions Complicated colonic cancers present at a more advanced stage with a lower resectability rate, and higher postoperative morbidity and mortality rates when compared with uncomplicated ones. Main outcome measures Central hemodynamics were estimated with applanation tonometry; cardiac performance was assessed using standard M-mode two-dimensional Doppler, tissue-doppler imaging and speckle-tracking echocardiography. First-phase ejection fraction was calculated as the percentage change in left ventricular volume from end-diastole to the time of peak aortic blood flow. Results Conventional indices of systolic function and cardiac performance were similar pre- and post-race while aortic systolic blood pressure decreased by 9 ± 8 mmHg (P less then 0.001) and first-phase ejection fraction increased by approximately 48% from 16.3 ± 3.9% to 22.9 ± 2.5% (P less then 0.001). The ratio of left ventricular transmitral Doppler early velocity (E) to tissue-doppler imaging early annular velocity (e') increased from 5.1 ± 1.8 to 6.2 ± 1.3 (P less then 0.01). Conclusion In recreational marathon runners, there is a marked increase in first-phase ejection fraction after the race despite no other significant change in cardiac performance or conventional measure of systolic function. More detailed physiological studies are required to elucidate the mechanism of this increase.

.The present study explored the clinical value of plasma microRNA-21 as a novel biomarker for early prediction of stable and unstable angina patients and its relationship with aging. A total of 255 participants, 123 patients with chronic stable angina, 82 patients with unstable angina, and 50 healthy subjects, were included in our study. Stable coronary and unstable coronary patients were confirmed following AHA/ACC clinical protocols. Total RNA was extracted from plasma by using miRNA-based TRIzol reagent. Plasma miR-21 expression levels were determined by real-time polymerase chain reaction. To evaluate the diagnosis accuracy, the receiver operating characteristic (ROC) curves were used. Plasma microRNA-21 concentration levels were significantly elevated in stable and unstable angina patients as compared with control subjects (P less then 0.001). The area under the ROC curves of circulating microRNA-21 was accurately distinguished in stable angina patients (AUC 0.921) and unstable angina patients (AUC 0.944) from healthy subjects. MicroRNA-21 expression gradually elevated with increasing aging in all the populations. Moreover, the current study also demonstrated that the expression of plasma miR-21 levels was significantly associated with different age groups within healthy subjects and stable and unstable angina patients (P less then 0.001). This research finding suggested that plasma microRNA-21 may be considered as a suitable new biomarker for early detection of stable and unstable angina patients, and it has a strong correlation with aging. We aimed to observe the short-term safety and efficacy of combining cryoballoon ablation (CBA) with LAAC in paroxysmal (PAF) patients. Method and Results. From Jan 2016 to Dec 2017, 304 patients diagnosed with nonvalvular, drug-refractory PAF were included, who underwent either CBA alone (n = 262) or combined procedure (n = 42). Instant pulmonary vein isolation (PVI) with CBA was achieved in all patients, while successful LAAC achieved in 41 (97.6%) of combined procedure patients. 1-year freedom of AF rate was lower in combined procedure group (84.7% vs 70.7%, p = 0.04), with unadjusted hazard ratio (HR = 1.97) and 95% confidence interval (CI) 1.03-3.77. However, the multivariate COX model revealed left atrial diameter (p = 0.002, HR = 1.10, and 95% CI 1.04, 1.17), rather than procedure type (p = 0.51, HR = 1.34, and 95% CI 0.57, 3.17), was the predictor for freedom of AF. Only 2 patients in the CBA group had stroke, contributing to the nonsignificant higher stroke incidence (p = 1.00). Transoesophageal echochardiography (TEE) achieved in 35 patients (83.3%) showed complete occlusion with no obvious residual flow (>3 mm), Device-related thrombosis, or pericardial perfusion. All-cause mortality, rehospitalization, and complication rates were similar. Conclusion Combining CBA with LAAC in a single procedure is a feasible strategy for PAF patients, with comparable short-term safety and efficacy to CBA alone. To provide a reference for clinical treatment, we analyzed the potential mechanisms of cognitive dysfunction induced by hypertension and hyperlipidemia, the clinical research and controversy of antihypertensive and lipid-lowering therapies on cognitive function, and the clinical value of combined antihypertensive and lipid-lowering therapy. It is currently believed that hypertension and elevated blood cholesterol levels in middle-aged people may be related to cognitive impairment or dementia in the elderly. Some studies suggest that intensive antihypertensive or lipid-lowering therapies are better than standard antihypertensive or lipid-lowering therapy, yet further tests are needed to confirm their effects on cognitive function. Only 2 patients in the CBA group had stroke, contributing to the nonsignificant higher stroke incidence (p = 1.00). Transoesophageal echochardiography (TEE) achieved in 35 patients (83.3%) showed complete occlusion with no obvious residual flow (>3 mm), Device-related thrombosis, or pericardial perfusion. All-cause mortality, rehospitalization, and complication rates were similar. Conclusion Combining CBA with LAAC in a single procedure is a feasible strategy for PAF patients, with comparable short-term safety and efficacy to CBA alone. It is currently believed that hypertension and elevated blood cholesterol levels in middle-aged people may be related to cognitive impairment or dementia in the elderly. Method The methodological process of cultural adaptation used was based on international literature guidelines consisting of forward translation, synthesis, back translation, review by an expert committee, and pretest. Psychometric analyses were conducted with 140 patients. These included measuring internal consistency (Cronbach's alpha), construct validity with item-total correlations, and convergent construcwith different forms of tachyarrhythmia. Background Hypertriglyceridaemia (HTG) is an important component of residual risk. The knowledge regarding its treatment might not be at a desired level, which might prevent patients from receiving the maximum benefit. We aimed to investigate the knowledge and attitudes of Turkish cardiologists who responded to a survey regarding HTG treatment. Methods A multiple-choice survey was conducted to analyse Turkish cardiologists' management of HTG. The questionnaire was submitted by the Turkish Society of Cardiology to all its members. Results A total of 160 cardiologists responded to the survey. The mean age was 37.5 ± 8.5 years, and 35 (21.9%) of the participants were female. Most of the participants (88%) thought HTG was a risk factor, and 75% of them felt confident in diagnosing and treating HTG. Patient compliance (41%), polypharmacy (33%), and lack of treatment options (15%) were the most common problems obstructing treatment of HTG. A proportion of 96% of the participants knew about non-high-density lipoprotein cholesterol, which is a good surrogate marker of atherogenic dyslipidaemia; however, only 39% were using it as a treatment goal. In the case of low-density lipoprotein cholesterol at goal but with HTG (residual risk), the first choice for treatment was fibrates (94%). Half of cardiologists had never used omega-3 fatty acids as a treatment option. Conclusions Although most of the participating cardiologists felt competent treating HTG, there was a knowledge gap in the treatment of atherogenic dyslipidaemia and management of residual risk. Evidence of the benefit of lowering triglycerides from cardiovascular outcome trials is eagerly awaited. Method Using a clinical method and interpretative phenomenological analysis, eight clinical interviews were conducted at home with four family caregivers (two interviews per caregiver) from two separate families (two caregivers per family), recruited at the neurology unit in the internal medicine department at Yaoundé military hospital. Results Throughout the whole care process, family caregivers have a difficult experience, with painful affects and psychological fatigue. They are forced into subjugation by unconscious alliances through the quality of the bond they have with the parent who has had the stroke. The second part provides a recap of the theoretical foundations, while the third and final part presents the results of what the workshops participants produced.While complexity theory has gradually influenced the field of health and social sciences, it has also had an impact on nursing care by introducing a wealth of terminology into the field. The terms “complex patient,” “complex case,” “complex care,” “complex practice,” and “complex needs” have been proposed to describe different aspects of complexity in nursing care. As these qualifiers reflect, nurses become actors in multidefined care and must integrate complexity into their reflective practice. By way of a narrative literature review, this article aims to offer a new perspective on nursing by explaining the different terms used in the discipline, using a multi-level approach. At the end of this review, the authors propose a new integrative conceptual framework for complexity in nursing practice.Nursing research continues to grow and become self-sustaining. It was in this context that we received funding from the Hospital Program for Nursing and Paramedical Research for our research project on schizophrenia. Schizophrenia is a mental disorder that affects 1% of the general population, and is a crippling disease both cognitively and socially. Cognitive remediation and therapeutic education are nursing practices deemed suitable for the psychosocial rehabilitation of schizophrenia patients. We hypothesized that a therapeutic education program placed upstream of cognitive remediation would have beneficial effects. We planned to include eighty patients aged eighteen to sixty, randomized into two groups, one combining cognitive remediation and therapeutic education, the second using just cognitive remediation. Each patient was assessed using a range of neuropsychological scales. Due to difficulties encountered in including and following up with participants, our statistical results could not be used. We therefore turned our focus to the factors that facilitate and hinder the setting up of a nursing research project. We organized these factors into three areas those related to the nurse ; those related to the care facility and the organization ; and those related to the research. The results were consistent with the literature and show how important it is to encourage nurses to be more involved in a scientific approach.

BACKGROUND The use of mobile social media enables individuals to learn about others' lives, which may lead to upward social comparison. Evidence has shown that upward social comparison on mobile social media can have negative effect for individuals' mental health, such as a higher risk of depression. Previous studies on this issue have mainly focused on adolescents and college students, but less on married adults' well-being. Moreover, mechanisms underlying this association have not been well-documented. METHODS A total of 514 Chinese married adults (62% female) were recruited from 26 regions in China. They completed measures of upward social comparison on mobile social media, envy, marriage quality and depression. RESULTS The results showed a positive association between upward social comparison on mobile social media and depression, which was completely mediated by envy. The direct effect of upward social comparison on depression, and the relation between upward social comparison and envy, were both moderated by marital quality. More specifically that upward social comparison on mobile social media was more strongly associated with depression and envy in adults with lower marital quality. LIMITATIONS This study only focused on the interpersonal factor (i.e., marriage quality) as a moderating variable, not investigating the potential moderating roles of personality factors and other interpersonal factors. In addition, the cross-sectional in design could not examine the causal relationships among the variables. CONCLUSIONS High marriage quality can protect married adults from the adverse effects of upward social comparison on mobile social media. This paper investigates the mental health promotion and protection (MHPP) model of reducing depression. Data are from the Clergy Health Initiative Longitudinal Survey of United Methodist ministers in North Carolina that included the Mental Health Continuum Short Form (****SF) for positive mental health and the Patient Health Questionnaire (PHQ-9) for depression in 2014 and 2016 (N = 955). The promotion hypothesis predicts reduced risk of depression in 2016 among clergy whose mental health increased to flourishing and the increased risk of depression in 2016 for clergy who stayed not flourishing. The protection hypothesis predicts increased risk of depression in 2016 for clergy who were flourishing in 2014 but went down to 'not flourishing' in 2016. The reference group is clergy who stayed flourishing. We used modified Poisson regression models for binary outcomes to estimate Prevalence Ratios (PR) and to estimate Incidence Rate Ratios (IRR) of depression in 2016 associated with changes in mental health status. Results support both hypotheses. Compared to clergy who stayed flourishing, clergy who improved to flourishing were as likely, while clergy who stayed not flourishing were nearly seven times more likely, to have depression in 2016. Clergy who declined to not flourishing were six times more likely to have depression in 2016 compared to those who stayed flourishing. Similar patterns were observed when the sample was restricted to clergy without depression in 2014. These findings suggest focusing on MHPP as a complementary approach to treatment to reduce the incidence, prevalence and burden of depression. METHODS Three common single-nucleotide polymorphisms (SNPs) in the promoter (rs168924 A/G; rs2242446 T/C) and 5'-untranslated region (5'-UTR) (rs2397771 G/C) of the NET gene were genotyped in 2,317 Han-Chinese participants (791 GAD patients and 1,526 controls; age 20-65). Potential confounding factors, such as gender, stress levels and psychiatric comorbidities, were included as covariates. RESULTS An interaction between age and NET genotypes and haplotypes was found for the risk of GAD. In the younger participants, rs168924 minor allele G homozygotes had the lowest incidence of GAD; however, older subjects displayed an inverse pattern, with homozygous G/G carriers presenting the highest prevalence of GAD. Additionally, younger individuals carrying 2 copies of the GGT haplotype composed of rs2397771-rs168924-rs2242446 had the lowest rate of GAD. However, those with 2 copies of the same haplotype exhibited the highest risk of GAD in the older groups. LIMITATIONS Only 3 common SNPs in the promoter and 5'-UTR of the NET gene were analyzed. CONCLUSIONS Our findings are the first to demonstrate that potentially functional SNPs in the NET promoter and 5'-UTR are associated with an increased risk of GAD, and that such associations are determined in an age-specific way. Considering the close relationship between sleep quality and memory consolidation, this study sought to investigate the potential of high-frequency CPC as a novel biomarker for objective evaluation of memory impairment in MDD. METHODS A total of 64 depressed patients and 35 healthy controls were included in this cross-sectional study. High-frequency coupling (HFC) was assessed by electrocardiogram-based CPC analysis using a portable sleep-respiration monitor during sleep for one night. The next day, subjects completed the cognition assessment with the Assessment of Neuropsychological Status (RBANS). The 17-Item Hamilton Depression Rating Scale (HAMD17) and the Hamilton Rating Scales for Anxiety (HAMA) were used to evaluate the severity of depression and anxiety in each patient, respectively. RESULTS There was no significant difference in the proportion of HFC between depressed patients and healthy controls.
