  • Two Sulfonate substituted rhodamine hydrophilic fluorescent probes RbS1 and RbS2 were designed and synthesized for specific detection of Fe3+. It was found that the probe RbS2 was stronger than RbS1 in the water solubility test. Both of them displayed responses to Fe3+ with a apparent fluorescence enhancement at 585 nm, accompanied with a distinct fluorescence change to pink. Upon addition of Fe3+ ions (0-16 μM), the emission intensity of RbS1 and RbS2 increased to 40 and 70 fold, which exhibited a good linear relationship with the concentration of Fe3+. The detection limits of RbS1 and RbS2 for sensing Fe3+ were 0.64 μM and 0.56 μM, respectively. The binding ratios of the RbS1 and RbS2 to Fe3+ were 11 and the recycling ability for Fe3+ was reasonable. Due to the large differences in the content of different substances, it is difficult to directly use the RMSE to evaluate the quantitative effect of the model. This study is the first to propose conducting deviation standardization (DS) for the determination results of various substances. To further improve the performance of the model, we use the successive projections algorithm (SPA) to optimize feature extraction and combine it with the better-performing PLSR model for training. The results show that the optimized DOSC-SPA-PLSR-DS quantitative model has better determination results for 101 sheep serum samples. The average RMSEp* of the concentration of the six substances decreased from 0.0408 to 0.0387, the Rp2 increased from 0.9758 to 0.9846, and the running time was reduced from 0.1659 to 0.0008 s. And the determination performance of lipase (LPS) enzymatic activity has also been improved. The results of this research show that sheep serum Raman spectroscopy combined with DOSC-SPA-PLSR-DS optimization can efficiently monitor the concentration (/enzyme activity) of seven indicators in real time and provide a new strategy for future intelligent supervision of animal husbandry.Invasive Aspergillosis is a challenging infection that requires convenient, efficient, and cost-effective diagnostics. This study addresses the potential of infrared spectroscopy to satisfy this clinical need with the aid of machine learning. Two models, based on Partial Least Squares-Discriminant Analysis (PLS-DA), have been trained by a set of infrared spectral data of 9 Aspergillus-spiked and 7 Aspergillus-free plasma samples, and a set of 200 spectral data simulated by oversampling these 16 samples. Two further models have also been trained by the same sets but with auto-scaling performed prior to PLS-DA. These models were assessed using 45 **** samples, simulating the challenging samples of patients at risk of Invasive Aspergillosis, including the presence of drugs (9 tested) and other common pathogens (5 tested) as potential confounders. The simple model shows good prediction performance, yielding a total accuracy of 84.4%, while oversampling and autoscaling improved this accuracy to 93.3%. The results of this study have shown that infrared spectroscopy can identify Aspergillus species in blood plasma even in presence of potential confounders commonly present in blood of patients at risk of Invasive Aspergillosis.
    Hepatocellular carcinoma (HCC) is characterized by marked phenotypic and molecular heterogeneity. Clinico-morphologic phenotypes and associations are important surrogate markers of molecular aberrations; therefore have immense relevance for targeted therapy. There is paucity of published literature on critical analysis of HCC heterogeneity and morphological alliance.

    To assess the heterogeneity and dominance of histomorphological features, and to explore clinicopathological associations in HCC.

    Retrospective cross-sectional study of 217 HCC tissue specimens was performed for the assessment of prevalence of major histological patterns, cytological features, and clinicopathological correlation.

    Homogeneous architecture with a single dominant histological pattern was a rarity. Single pattern constituting ≥50 % of the tumour was found in less than 1/5th of the cases.     Volumetric modulated arc therapy (VMAT) can acquire projection images during rotational irradiation, and cone-beam computed tomography (CBCT) images during VMAT delivery can be reconstructed. The poor quality of CBCT images prevents accurate recognition of organ position during the treatment. The purpose of this study was to improve the image quality of CBCT during the treatment by cycle generative adversarial network (CycleGAN).

    Twenty patients with clinically localized prostate cancer were treated with VMAT, and projection images for intra-treatment CBCT (iCBCT) were acquired. Synthesis of PCT (SynPCT) with improved image quality by CycleGAN requires only unpaired and unaligned iCBCT and planning CT (PCT) images for training. We performed visual and quantitative evaluation to compare iCBCT, SynPCT and PCT deformable image registration (DIR) to confirm the clinical usefulness.

    We demonstrated suitable CycleGAN networks and hyperparameters for SynPCT. The image quality of SynPCT improved visually and quantitatively while preserving anatomical structures of the original iCBCT. The undesirable deformation of PCT was reduced when SynPCT was used as its reference instead of iCBCT.

    The noise generated in ultra-high-resolution computed tomography (U-HRCT) images affects the quantitative analysis of emphysema. In this study, we compared the physical properties of reconstructed images for hybrid iterative reconstruction (HIR) and deep learning reconstruction (DLR), which are reconstruction methods for reducing image noise. Using clinical evaluation, we evaluated the correlation between low attenuation volume (LAV) % obtained by CT and forced expiratory volume in 1 s per forced vital capacity (FEV
    /FVC) obtained by respiratory function tests.

    CT data obtained by HIR and DLR were used for analysis (matrix size 1024´1024, slice thickness 0.25 mm). A pseudo-PET image was obtained using a pseudo-CT image, which was used for the AC of the no-AC PET image. The coefficient of similarity between the real and generated pseudo-images was estimated using the peak signal-to-noise ratio (PSNR) , the structural similarity (SSIM), and the dice similarity coefficient (DSC). The average values of PSNR, SSIM, and DSC of the pseudo-CT were 31.0 dB, 0.87, and 0.89, and those of the pseudo-PET were 35.9 dB, 0.90, and 0.95, respectively. The AC for the whole-body PET image could be accomplished using the pseudo-CT image generated via the GAN. The proposed method would be established as the CT-less PET/CT examination.

  • Carbon-based single-atom catalysts (CSACs) have recently received extensive attention in catalysis research. However, the preparation process of CSACs involves a high-temperature treatment, during which metal atoms are mobile and aggregated into nanoparticles, detrimental to the catalytic performance. Herein, an ion-imprinting derived strategy is proposed to synthesize CSACs, in which isolated metal-nitrogen-carbon (Me-N4 -Cx ) moiety covalently binds oxygen atoms in Si-based molecular sieve frameworks. Such a feature makes Me-N4 -Cx moiety well protected/confined during the heat treatment, resulting in the final material enriched with single-atom metal active sites. As a proof of concept, a single-atom Fe-N-C catalyst is synthesized by using this ion-imprinting derived strategy. Experimental results and theoretical calculations demonstrate high concentration of single FeN4 active sites distributed in this catalyst, resulting in an outstanding oxygen reduction reaction (ORR) performance with a half-wave potential of 0.908 V in alkaline media.2D magnetism plays a key role in both fundamental physics and potential device applications. However, the instability of the discovered 2D magnetic materials has been one main obstacle in deep research and potential application of 2D magnetism. Here, a localized magnetic moment induced by Pt vacancies in air-stable type-II Dirac semimetal PtSe2 flakes is reported. The localized magnetic moments give rise to the Kondo effect, evidenced by logarithmic increment of resistance with decreasing temperature and isotropic negative longitudinal magnetoresistance. Additionally, the induced magnetic moment and Kondo temperature appear to depend on thickness in the thinner samples ( less then 10 nm). The small magnetocrystalline anisotropy revealed by first-principles calculation indicates that the magnetic moments are randomly localized instead of long-range ordered. The findings demonstrate a new means to induce magnetism in 2D non-magnetic materials.The diagnosis and classification of rhabdomyosarcoma (RMS) has undergone several shifts over the last 30 years.     Carbon-based single-atom catalysts (CSACs) have recently received extensive attention in catalysis research. However, the preparation process of CSACs involves a high-temperature treatment, during which metal atoms are mobile and aggregated into nanoparticles, detrimental to the catalytic performance. Herein, an ion-imprinting derived strategy is proposed to synthesize CSACs, in which isolated metal-nitrogen-carbon (Me-N4 -Cx ) moiety covalently binds oxygen atoms in Si-based molecular sieve frameworks. Such a feature makes Me-N4 -Cx moiety well protected/confined during the heat treatment, resulting in the final material enriched with single-atom metal active sites. As a proof of concept, a single-atom Fe-N-C catalyst is synthesized by using this ion-imprinting derived strategy. Additionally, the induced magnetic moment and Kondo temperature appear to depend on thickness in the thinner samples ( less then 10 nm). The small magnetocrystalline anisotropy revealed by first-principles calculation indicates that the magnetic moments are randomly localized instead of long-range ordered.   • Sera from white-tailed deer (WTD, Odocoileus virginianus) hunter-harvested throughout New York State (NYS), 2007-2015, were tested by plaque reduction neutralization for antibodies against nine mosquito-borne viruses from the families Peribunyaviridae, Flaviviridae, and Togaviridae. Overall, 76.1% (373/490) of sampled WTD were seropositive against at least one virus, and 38.8% were exposed to multiple viruses. The seropositivity rate in adult WTD (78.0%) was significantly greater (P less then 0.0001) than that in fawns (47.7%). Neutralizing antibodies against California serogroup viruses were most common in WTD sampled across all regions (67.1%), followed by the Bunyamwera serogroup (BUN) (37.6%). Jamestown Canyon and Cache Valley orthobunyaviruses were responsible for most California and BUN infections, respectively. Seroprevalence rates to West Nile virus were higher in samples originating from Long Island (LI) (19.0%) than in those originating from the central (7.3%), western (5.0%), and Hudson Valley (4.4%) regions of NYS. Antibodies to Eastern equine encephalitis virus were seen primarily in WTD from central NYS (5.1%), where annual enzootic activity occurs, but low rates were documented in western NYS (1.4%) and LI (1.7%). Low rates of Potosi and LaCrosse orthobunyavirus, and Highlands J virus antibodies were detected over the course of this investigation. St. Louis encephalitis virus (or a closely related virus) antibodies were detected in samples collected from central and western NYS, suggesting local virus transmission despite a lack of evidence from routine mosquito surveillance. We performed an economic appraisal of PBO nets compared with conventional LLINs in 2019 US$ from prevention and health systems perspectives (including treatment cost offsets). We used data from a pragmatic randomized 2012-2014 trial in Nigeria with epidemiological outcomes in an area with confirmed pyrethroid resistance. Each village had 50 months of epidemiologic data, analyzed by village by month, using negative binomial regression. Compared with LLINs, although adding $0.90 per net delivered, PBO nets reduced symptomatic malaria cases by 33.4% (95% CI 10.2-50.6%). From a prevention perspective, the incremental cost-effectiveness ratio was $11 (95% CI $8-$37) per disability-adjusted life year averted. From the health systems perspective, PBO nets were significantly cost-saving relative to conventional LLINs. The benefit-cost analysis found that the added economic benefits of PBO nets over LLINs were $201 (95% CI $61-$304) for every $1 in incremental costs. Growing pyrethroid resistance is likely to strengthen the economic value of PBO nets over LLINs. Results indicated that the Perceptual Reasoning Index, Working Memory Index, and Processing Speed Index scores (except Verbal Comprehension Index score), and all subtest scores significantly predicted giftedness. Furthermore, correct classification rates among gifted children were 93% for index scores and 97% for subtest scores. Furthermore, we discuss the diagnostic relevance of COVID-19 serology in a retrospective cohort of 145 COVID-19 cases in which repetitive molecular and serological SARS-CoV-2 tests were applied. Results The interpretation of the pooled sensitivity of IgM/A and IgG resulted in the highest values (range 14-71% on day 2-7; 88-94% on day 8-18). Overall, the specificity of the assays was high (range 79-100%). Among 145 retrospective cases, 3 cases (2%) remained negative after sequential molecular testing but positive on final SARS-CoV-2 serology. Conclusion Sensitivity of COVID-19 serological diagnosis was variable but consistently increased at >7 days after symptom onset. Specificity was high. A 41-year-old female with a recent excess use of cannabis presented to the emergency department for evaluation of recurrent excruciating epigastric pain accompanied by severe nausea and vomiting. She had similar, milder symptoms a year ago and underwent endoscopic evaluation which was negative except for mild reflux esophagitis for which she was started on a proton pump inhibitor. On this presentation, basic laboratory workup, EKG, and CT scan of abdomen and pelvis were unremarkable. A detailed abdominal exam was only positive for mild epigastric tenderness. She was instructed to resolution of symptoms with topical capsaicin, our case supports this promising intervention and provides an alternate approach to antiemetics and narcotics routinely used in patients with cannabis hyperemesis syndrome.Traumatic bone cyst (TBC), a "pseudocyst" that usually affects long bones, is a rare lesion among cystic lesions in the jaws. The most commonly affected site is the posterior mandible. Most of the time, TBC is asymptomatic and discovered during routine radiographic examination. The treatment recommended for TBC is surgical exploration followed by curettage of the bony walls, which also serves as a diagnostic procedure. A 27-year-old Caucasian male with a noncontributory medical history was referred to our department for the endodontic evaluation of the mandibular right first and second molars, which were connected to an extensive asymptomatic osteolytic lesion. A multimodular diagnostic assessment involving CBCT imaging, ultrasound, and histopathologic examination led to a definite diagnosis of a TBC overlapping with apical periodontitis (AP). Subsequently, a multidisciplinary treatment approach was performed, including surgical excision and biopsy of the lesion, endodontic retreatment of the right mandibular first molar, and postsurgical root canal treatment of the second molar. During the follow-up period of five years, the patient was reassessed periodically once a year and showed, in the absence of signs and symptoms, progressive healing of the affected area. The present article reports a case following the CARE guidelines of a TBC combined with AP where a multimodular diagnostic assessment was performed and discusses the possible pathogenetic mechanisms involved in its generation.
    The dynamic obstruction of the left ventricular outflow tract (LVOT) is a well-known complication in mitral annuloplasty but rarely seen in nonmitral cardiovascular surgery. The dynamic LVOT obstruction can lead to hemodynamic instability, even shock and the treatment is significantly different from the standard approach.

We reported a case of low cardiac output syndrome (LCOS) with severe mitral regurgitation (MR), dramatically reduced left ventricular ejection fraction (LVEF) after coronary artery bypass grafting in a 72-year-old female requiring an escalation of inotropic support, volume restriction, and mechanical support. The detailed echocardiography combined with lung ultrasound revealed a dynamic systolic anterior movement of the anterior mitral leaflet (SAM), apical ballooning, and no significant lung congestion. Intravenous fluids were given, diuretics withdrawn, inotrope discontinued, and vasopressors uptitrated. The dynamic SAM was rapidly relieved, the hemodynamics was stabilized, and the LVEF was improving. The patient was discharged in good condition without residual LVOT obstruction and trace MR.

    We strongly suggest that a detailed echocardiography should be performed in any patient who presents in shock to rule out a dynamic LVOT obstruction. Lung ultrasound should be a routine examination in addition to echocardiography. Once SAM is detected, treatment should be based on volume expansion, inotrope discontinuation, and a careful afterload increasing.

We describe two patients with Candida spp. prosthetic valve endocarditis (PVE) and 18fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG-PET/CT) as a part of diagnostic workup. We also refer to 5 other cases we found from the published literature. These cases highlight that 18F-FDG-PET/CT can improve diagnostic accuracy in prosthetic valve Candida endocarditis.

A 27-year-old Caucasian male with a noncontributory medical history was referred to our department for the endodontic evaluation of the mandibular right first and second molars, which were connected to an extensive asymptomatic osteolytic lesion. A multimodular diagnostic assessment involving CBCT imaging, ultrasound, and histopathologic examination led to a definite diagnosis of a TBC overlapping with apical periodontitis (AP). We reported a case of low cardiac output syndrome (LCOS) with severe mitral regurgitation (MR), dramatically reduced left ventricular ejection fraction (LVEF) after coronary artery bypass grafting in a 72-year-old female requiring an escalation of inotropic support, volume restriction, and mechanical support. The detailed echocardiography combined with lung ultrasound revealed a dynamic systolic anterior movement of the anterior mitral leaflet (SAM), apical ballooning, and no significant lung congestion. Intravenous fluids were given, diuretics withdrawn, inotrope discontinued, and vasopressors uptitrated. The dynamic SAM was rapidly relieved, the hemodynamics was stabilized, and the LVEF was improving. The patient was discharged in good condition without residual LVOT obstruction and trace MR. We strongly suggest that a detailed echocardiography should be performed in any patient who presents in shock to rule out a dynamic LVOT obstruction. We defined an action primitive as a triplet comprised of a verb, target object, and hand object. A recurrent neural network was trained to recognize a verb and target object, and was tested on three different activities. For a representative activity (making a powdered drink), the average recognition accuracy was 77% for the verb and 83% for the target object. Using a non-specific approach to classifying and indexing objects in the workspace, we observed a modest level of generalizability of the action primitive classifier across activities, including those for which the classifier was not trained. The novel input features of gaze object angle and its rate of change were especially useful for accurately recognizing action primitives and reducing the observational latency of the classifier.In this paper, an improved obstacle-avoidance-scheme-based kinematic control problem in acceleration level for a redundant robot manipulator is investigated. Specifically, the manipulator and obstacle are abstracted as mathematical geometries, based on the vector relationship between geometric elements, and the Cartesian coordinate of the nearest point to an obstacle on a manipulator can be found. The distance between the manipulator and an obstacle is described as the point-to-point distance, and the collision avoidance strategy is formulated as an inequality. To avoid the joint drift phenomenon of the manipulator, bi-criteria performance indices integrating joint-acceleration-norm minimization and repetitive motion planning is adopted by assigning a weighing factor. From the perspective of optimization, therefore, an acceleration level quadratic programming (QP) problem is eventually formulated. Considering the physical structure of robot manipulators, inherent joint angle, speed, and acceleration limits are also incorporated. To solve the resultant QP minimization problem, a recurrent neural network based neural dynamic solver is proposed. Then, simulation experiments performing on a four-link planar manipulator validate the feasibility and effectiveness of the proposed scheme.Drowsiness is a leading cause of traffic and industrial accidents, costing lives and productivity. Electroencephalography (EEG) signals can reflect awareness and attentiveness, and low-cost consumer EEG headsets are available on the market. The use of these devices as drowsiness detectors could increase the accessibility of safety and productivity-enhancing devices for small businesses and developing countries. We conducted a systemic review of currently available, low-cost, consumer EEG-based drowsiness detection systems. We sought to determine whether consumer EEG headsets could be reliably utilized as rudimentary drowsiness detection systems. We included documented cases describing successful drowsiness detection using consumer EEG-based devices, including the Neurosky MindWave, InteraXon Muse, Emotiv Epoc, Emotiv Insight, and OpenBCI. Of 46 relevant studies, ~27 reported an accuracy score. The lowest of these was the Neurosky Mindwave, with a minimum of 31%. The second lowest accuracy reported was 79.4% with an OpenBCI study.

  • Assessment of the vascular status following limb fracture in children is important to evaluate the risk of compartment syndrome, which is an emergency condition.

    To establish a simple and efficient grading scale of limb perfusion in children undergoing surgery for limb fracture.

    This retrospective study included pediatric patients with a limb fracture and postoperative plaster fixation who were admitted at The Department of Pediatric Orthopedics of Xinhua Hospital between February 2017 and August 2017. The outcome was poor limb perfusion, which is defined as the postoperative use of mannitol. The children were divided into two groups The normal perfusion group and the poor perfusion group. Key risk factors have been selected by univariable analyses to establish the Grading Scale for Vascular Status.

    A total of 161 patients were included in the study 85 in the normal perfusion group and 76 in the poor perfusion group. There were no significant differences in age, sex, body mass index, ethnicity, cause urrence of poor limb perfusion. A prospective study is required for validation.

    Focal intrahepatic strictures (FIHS) refer to local strictures of the small and medium intrahepatic bile ducts. FIHS are easily misdiagnosed due to their rare incidence, and few studies have focused on the diagnosis and treatment approaches.

    To propose a new classification for FIHS in order to guide its diagnosis and treatment.

    The symptoms, biochemistry results, imaging results, endoscopic examination results and initial and final diagnoses of 6 patients with FIHS admitted between January 2010 and December 2019 were retrospectively analyzed.

    The 6 patients were diagnosed with intratubular growth-type intrahepatic cholangiocarcinoma (IG-ICC), recurrent multiple hepatocellular carcinoma (rmHCC) with bile duct tumor thrombus (BDTT), adenosquamous carcinoma (ASC), hepatolithiasis, small duct primary sclerosing cholangitis (SD-PSC) and autoimmune hepatitis (AIH). The initial and final diagnoses were not consistent in 4 patients. Hepatectomy was performed in patients with IG-ICC, ASC and hepatolithiasis according to the locations of their FIHS. Patients with rmHCC with BDTT received lenvatinib/sintilimab, while patients with SD-PSC and AIH received UDCA. We proposed the following classification system for FIHS type I FIHS located within one segment of the liver; type II FIHS located at the confluence of the bile ducts of one segment or two adjacent segments; type III FIHS connected to the left or right hepatic duct; and type IV Multiple FIHS located in both lobes of the liver.

    Our proposed classification system might help to guide the diagnosis and treatment of FIHS. Hepatectomy should be performed not only for malignant FIHS but also for benign strictures with severe secondary damage that cannot be improved by nonsurgical methods.

FIHS are easily misdiagnosed due to their rare incidence, and few studies have focused on the diagnosis and treatment approaches. To propose a new classification for FIHS in order to guide its diagnosis and treatment. The symptoms, biochemistry results, imaging results, endoscopic examination results and initial and final diagnoses of 6 patients with FIHS admitted between January 2010 and December 2019 were retrospectively analyzed. The 6 patients were diagnosed with intratubular growth-type intrahepatic cholangiocarcinoma (IG-ICC), recurrent multiple hepatocellular carcinoma (rmHCC) with bile duct tumor thrombus (BDTT), adenosquamous carcinoma (ASC), hepatolithiasis, small duct primary sclerosing cholangitis (SD-PSC) and autoimmune hepatitis (AIH). The initial and final diagnoses were not consistent in 4 patients. Hepatectomy was performed in patients with IG-ICC, ASC and hepatolithiasis according to the locations of their FIHS. Patients with rmHCC with BDTT received lenvatinib/sintilimab, while patients with SD-PSC and AIH received UDCA. We proposed the following classification system for FIHS type I FIHS located within one segment of the liver; type II FIHS located at the confluence of the bile ducts of one segment or two adjacent segments; type III FIHS connected to the left or right hepatic duct; and type IV Multiple FIHS located in both lobes of the liver. Our proposed classification system might help to guide the diagnosis and treatment of FIHS. Hepatectomy should be performed not only for malignant FIHS but also for benign strictures with severe secondary damage that cannot be improved by nonsurgical methods. Intracranial infection is a common clinical disease. Computed tomography (CT) and magnetic resonance imaging (MRI) have certain sensitivity and have good diagnostic efficacy. To study the application value of MRI and CT in the diagnosis of intracranial infection after craniocerebral surgery. We selected 82 patients who underwent craniocerebral surgery (including 40 patients with intracranial infection and 42 patients without infection) during the period from April 2016 to June 2019 in our hospital. All 82 patients received CT and MRI examinations, and their clinical data were reviewed. A retrospective analysis was performed, and the coincidence rate of positive diagnosis and the overall diagnosis coincidence rate of different pathogenic infection types were measured with the two examination methods. The diagnostic sensitivity and specificity as well as the positive and negative predictive values of the two examination methods were compared.     Neuropathic pain (NP) is one of the most intractable complications of spinal cord injury (SCI). This study aims to explore the role of long non-coding RNA (lncRNA) SNHG1 in influencing SCI-induced NP.

    After establishment of the spinal nerve ligation (SNL) model in rats, spinal tissues were extracted. SNHG1 level in rat spinal tissues was determined by quantitative real-time polymerase chain reaction (qRT-PCR). The role of SNHG1 in the development of NP was explored by assessing paw withdrawal threshold (PWT) and paw withdrawal latency (PWL) in model rats. The interaction between SNHG1 and CDK4 was explored by Luciferase assay and RIP (RNA-Binding Protein Immunoprecipitation). Enzyme-linked immunosorbent assay (ELISA) and qRT-PCR were conducted to determine inflammatory factor levels in rat spinal tissues.

Student's t-tests were performed for statistical analysis.

  • Psoriasis is a long-lasting, noncontagious chronic inflammatory disease of skin and joints. Previous epidemiological studies have demonstrated that psoriatic patients have a shorter life expectancy, mainly due to cardiovascular (CV) events with a higher prevalence of cardiovascular risk factors like dyslipidemia, diabetes mellitus, insulin resistance, obesity, and hypertension. Besides these risk factors, psoriasis likely plays an independent role in increasing CV events probably due to the chronic inflammatory state. This literature review aims to summarize the mechanism of atherosclerosis formation, CV risk factors, tools to diagnose subclinical atherosclerosis, and the effects of various therapies in psoriatic patients to prevent cardiovascular-related deaths in psoriasis. This review was performed by searching the relevant articles in PubMed and Google Scholar databases without including any exclusion criteria and time limitations. Our review documented that psoriatic patients are at increased risk of CV events due to chronic inflammatory profile and the associated CV risk factors. Also, anti-inflammatory therapies may prevent early subclinical atherosclerotic vascular changes reducing cardiovascular events. However, the available studies lack to establish the exact targets for CV risk factors, to assess the clinical importance of screening for subclinical vascular changes and the impact of anti-inflammatory therapies on CV risk profile in psoriatic patients. This heightened awareness about the CV involvement in psoriasis should encourage conducting large, well planned comprehensive studies to address these issues that can reduce cardiovascular morbidity and mortality.Introduction Solid papillary carcinoma (SPC) is a distinct rare subtype of breast tumour that often exhibits a neuroendocrine differentiation. Due to the rarity of these tumours, few studies have assessed the clinicopathological features of these tumours. Therefore, in this study, we evaluated the clinical and pathological profiles of SPC and compared the pathologic features with conventional invasive ductal carcinoma (IDC) in our population. Methods It was a retrospective cross-sectional study conducted at Liaquat National Hospital and Medical College from January 2013 until December 2019 over seven years. Cases with histological diagnosis of SPC and IDC were included in the study, and clinicopathological characteristics were compared. Results We included 39 cases of SPC in our study diagnosed during the study period. During the same timeline, 634 cases of IDC were reported and therefore included in the study for comparison. The mean age of the patients with SPC was 53.97 ± 12.15 years, and the mean tumour size was 3.42 ± 1.87 cm. Axillary metastasis was noted in 15.4% of cases. 94.9% of cases of SPC were invasive. https://www.selleckchem.com/products/pds-0330.html Estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor-2 (HER2/neu) and synaptophysin positivity was seen in 84.6%, 87.2%, 10.3%, and 59% respectively. Ventricular arrhythmias (VAs) contribute significantly to an increase in the rates of death in patients with TTC, especially during the acute phase, in which patients with TTC are more susceptible to develop life-threatening arrhythmias (LTA) such as ventricular tachycardia (VT), ventricular fibrillation (VF), torsades de pointes (TdP), and sudden cardiac death (SCD). However, the pathophysiology of TTC and how VA occurs are still a mystery. We aim to review previous literature and discuss the possible mechanisms of VA in TTC patients. VA usually complicates the acute phase of the disease and worsens the long-term prognosis. Alterations of repolarization (negative T wave, prolonged QTc) indicate a high risk of arrhythmic events (TdP, VT, VF, and SCD). Catecholamine effect on myocardial cells and myocardial edema can create a substrate for the development of VA. Some of the most commonly proposed mechanisms for the development of VA in patients with TTC are coronary vasospasm, myocardial stunning due to catecholamines, re-entry, and triggered activity. Further prospective studies, including a more significant number of patients, are required to understand the disease's pathophysiology better and improve LTA management in patients with TTC.

    = 234) were analyzed to calculate second R01 success rates. Only R01 awards to ophthalmology or optometry clinician-scientists were included. Demographic data were obtained from clinicians with first-time NEI R01 funding spanning from 1962 to 2019 (

We obtained information regarding time span of the first R01, year of second R01, institution, and project title on the National Institutes of Health (NIH) Research Portfolio Online Reporting Tool, Expenditures and Results (RePORTER) database, and additional measures of gender, clinical specialty, and degree by performing Internet searches.

    Overall, from 1985 to 2014, 62.8% of ophthalmology or optometry clinician-scientists were awarded a second R01; at 5 years after receipt of the first R01 (the typical length of an R01), only 3.9% received their second R01. None of the factors examined (temporal cohort, gender, clinical specialty, degree, institution, or bench vs. non-bench research) was significantly associated with successful attainment of a second R01.

    We found an overall success rate of 62.8% for receiving a second R01, but 5 years after the first R01 an attainment rate for a second R01 of only ∼4%.

Ventricular arrhythmias (VAs) contribute significantly to an increase in the rates of death in patients with TTC, especially during the acute phase, in which patients with TTC are more susceptible to develop life-threatening arrhythmias (LTA) such as ventricular tachycardia (VT), ventricular fibrillation (VF), torsades de pointes (TdP), and sudden cardiac death (SCD). However, the pathophysiology of TTC and how VA occurs are still a mystery. We aim to review previous literature and discuss the possible mechanisms of VA in TTC patients. VA usually complicates the acute phase of the disease and worsens the long-term prognosis. Alterations of repolarization (negative T wave, prolonged QTc) indicate a high risk of arrhythmic events (TdP, VT, VF, and SCD). Catecholamine effect on myocardial cells and myocardial edema can create a substrate for the development of VA. Some of the most commonly proposed mechanisms for the development of VA in patients with TTC are coronary vasospasm, myocardial stunning due to catecholamines, re-entry, and triggered activity. Further prospective studies, including a more significant number of patients, are required to understand the disease's pathophysiology better and improve LTA management in patients with TTC. To evaluate the success of ophthalmology and optometry clinician-scientists in obtaining a second R01 (renewal or new) and factors associated with this success, including gender, clinical specialty, degree, institution, and bench versus non-bench research. First-time National Eye Institute (NEI) R01 awardee data from 1985 to 2014 ( = 234) were analyzed to calculate second R01 success rates. Only R01 awards to ophthalmology or optometry clinician-scientists were included. Demographic data were obtained from clinicians with first-time NEI R01 funding spanning from 1962 to 2019 ( = 386). We obtained information regarding time span of the first R01, year of second R01, institution, and project title on the National Institutes of Health (NIH) Research Portfolio Online Reporting Tool, Expenditures and Results (RePORTER) database, and additional measures of gender, clinical specialty, and degree by performing Internet searches. Overall, from 1985 to 2014, 62.8% of ophthalmology or optometry clinician-scientists were awarded a second R01; at 5 years after receipt of the first R01 (the typical length of an R01), only 3.9% received their second R01. None of the factors examined (temporal cohort, gender, clinical specialty, degree, institution, or bench vs. non-bench research) was significantly associated with successful attainment of a second R01. We found an overall success rate of 62.8% for receiving a second R01, but 5 years after the first R01 an attainment rate for a second R01 of only ∼4%. Our study provides insight on significant leaks in the clinician-scientist pipeline and raises questions of how stakeholders should support this important group of individuals at the intersection of clinical medicine and biomedical research.
