A constructivist grounded theory approach was adopted to explore men's transition to fatherhood. This paper reports on one of the sub-categories derived from the data. First-time fathers with a child under two were recruited predominantly via social media. Audio-recorded semi-structured interviews were undertaken with an opening question asking men to tell their story of becoming a father. Interviews were transcribed and analysed using constructivist grounded theory methods. This paper reports one core aspect of the research findings which has particular relevance for healthcare professionals. The men in this study were highly appreciative of the care their partner and baby received but consistently reported a lack of father-specific support throughout their journey to fatherhood. This ranged from generally poor communication with healthcare professionals to being ignored and side-lined in maternity settings where they continued to be treated as visitors before, during and after the birth of their baby. Despite similar findings being reported over the last 30 to 40 years and policy directives emphasising the importance of working with fathers, change within healthcare services remains slow. Currently, fathers' needs are not being adequately met by perinatal services.This study reports the data on the contamination caused by polycyclic aromatic hydrocarbons (PAHs) drained from the Volturno River. The seasonal and spatial distribution of PAHs in water and sediment samples was assessed. The 16 PAHs were determined in the water dissolved phase (DP), suspended particulate matter (SPM), and sediments. A multidimensional statistical approach was used to identify three pollution composite indicators. Contaminant discharges of PAHs into the sea were calculated in about 3158.2 kg/year. Total concentrations of PAHs varied in ranges 434.8 to 872.1 ng g-1 and 256.7 to 1686.3 ng L-1 in sediment samples and in water (DP + SPM), respectively. The statistical results indicated that the PAHs mainly had a pyrolytic source. Considering the sediment quality guidelines (SQGs), the water environmental quality standards (USEPA EQS), and risk quotient (RQ), the Volturno River would be considered as an area in which the environmental integrity is possibly at risk.Diabetes is a chronic metabolic disorder characterized by inappropriately elevated glucose levels as a result of impaired pancreatic β cell function and insulin resistance. Extensive studies have been conducted to elucidate the mechanism involved in the development of β cell failure and death under diabetic conditions such as hyperglycemia, hyperlipidemia, and inflammation. Of the plethora of proposed mechanisms, endoplasmic reticulum (ER) stress, mitochondrial dysfunction, and oxidative stress have been shown to play a central role in promoting β cell dysfunction. It has become more evident in recent years that these 3 factors are closely interrelated and importantly aggravate each other. Oxidative stress in particular is of great interest to β cell health and survival as it has been shown that β cells exhibit lower antioxidative capacity. Therefore, this review will focus on discussing factors that contribute to the development of oxidative stress in pancreatic β cells and explore the downstream effects of oxidative stress on β cell function and health. Furthermore, antioxidative capacity of β cells to counteract these effects will be discussed along with new approaches focused on preserving β cells under oxidative conditions.It is generally accepted that empathy should be the basis of patient care. However, this ideal may be unrealistic if healthcare professionals suffer adverse effects when engaging in empathy. The aim of this study is to explore the effect of inferring mental states and different components of empathy (perspective-taking; empathic concern; personal distress) in burnout dimensions (emotional exhaustion; depersonalization; personal accomplishment). A total of 184 healthcare professionals participated in the study (23% male, Mage = 44.60; SD = 10.46). We measured participants' empathy, the inference of mental states of patients, and burnout. Correlation analyses showed that inferring mental states was positively associated with perspective-taking and with empathic concern, but uncorrelated with personal distress. Furthermore, emotional exhaustion was related to greater levels of personal distress and greater levels of inferences of mental states. Depersonalization was associated with greater levels of personal distress and lower levels of empathic concern. Personal accomplishment was associated with the inference of mental states in patients, lower levels of personal distress, and perspective-taking. These results provide a better understanding of how different components of empathy and mental state inferences may preserve or promote healthcare professionals' burnout.

Based on the proposed robust trajectory optimization formation, the motion trajectory optimization of an overhead crane system under two different uncertainty types of is solved. All simulation results are compared with traditional sampling-based Monte Carlo simulations to demonstrate the feasibility and effectiveness of the proposed method.This paper addresses the issue of sliding mode control(SMC) for a class of interval type-2(IT2) fuzzy system subject to the fading channel. For relieving the transmission burden, a dynamic event-triggered mechanism is introduced to determine the transmission of the measurement signals via the sensor-to-controller(S/C) channel. In the communication channel, the transmitted signal may suffer from fading in wireless communication networks. By means of known bounds of the membership functions (MFs) in IT2 fuzzy model, both the fuzzy state observer and the fuzzy SMC law are designed to ensure the reachability of the sliding mode dynamics. Sarcopenia worsens the prognosis of patients with hepatocellular carcinoma (HCC). The aim of this study was to elucidate the plasma free amino acids (PFAAs) associated with sarcopenia or myosteatosis in the course of HCC recurrence.
In this cross-sectional study, 187 patients were enrolled retrospectively. All patients experienced more than one hospitalization (mean times, 2.65) owing to HCC recurrence. The skeletal muscle index (SMI) and muscle attenuation (MA) were measured by a transverse computed tomography (CT) scan image at the third lumbar vertebra (L3). The changes in the concentration of 24 PFAAs, SMI, and MA in the same patient between recurrences were defined as Δ. The associations between sarcopenia, myosteatosis, and PFAAs were evaluated by a logistic regression model. The ΔSMI and ΔMA were compared between the patients who received branched-chain amino acids (BCAAs) formulation and those who did not.
Patients with sarcopenia showed lower survival rate; the 1-, 3-, and 5-y survival rates were 85%, 42%, and 9%, respectively. Multivariate analysis revealed that the level of total BCAAs was significantly associated with sarcopenia. The correlation coefficient value between the change of leucine (ΔLeu) and ΔSMI was highest (R=0.256; P < 0.001) among the PFAAs. In the Child-Pugh grade B or C, the decrease of SMI was significantly more suppressed in the patients with the BCAAs formulation than in those without BCAAs formulation (ΔSMI mean change -0.98 versus -3.45 cm²/m²; P=0.038).

To analyze differences in mandibular cortical width (MCW) among children diagnosed with Obstructive sleep apnea (OSA) or at high- or low-risk for OSA.
161 children were assessed 60 children with polysomnographically (nPSG) diagnosed OSA, 56 children presenting symptoms suggestive of high-risk for OSA, and 45 children at low-risk for OSA. Children at high- and low-risk for OSA were evaluated through the Pediatric Sleep Questionnaire (PSQ). MCW was calculated from panoramic radiograph images available from all subjects using ImageJ software. Differences between MCW measurements in the three groups were evaluated using ANCOVA and Bonferroni post-hoc tests, with age as a covariate. The association between MCW and specific cephalometric variables was assessed through regression analysis.
The participants' mean age was 9.6±3.1 years (59% male and 41% female). The mean BMI z-score was 0.62±1.3. The nPSG-OSA group presented smaller MCW than at low-risk for OSA group (mean difference= -0.385 mm, p=0.001), but no difference with the at high-risk for OSA group (nPSG-OSA vs. high-risk OSA p=0.085). In addition, the MCW in the at high-risk for the OSA group was significantly smaller than the at low-risk for the OSA group (mean difference= -0.301 mm, p=0.014). To evaluate short-term efficacy and subjective outcomes of TSD therapy as compared to those of mandibular advancement device (MAD) therapy in an adult population diagnosed with OSA.
This study is a parallel, non-randomized clinical trial of the TSD and MAD therapies. The efficacy of both interventions was evaluated objectively by level 3 home sleep testing and subjectively, with the Epworth Sleepiness Scale (ESS), Functional Outcomes Sleep Questionnaire (FOSQ-10), Chalder Fatigue Scale (CFQ), and the sleep-related quality of life questionnaire (SF-36). Compliance and adverse effects were self-reported.
Of the 39 patients who received the TSD therapy, 27 managed to adapt and complete the trial and were matched with 26 patients who received the MAD therapy. At the two-month follow-up, the acceptance rate of the TSD therapy was 53.8%. We found three emergent themes (1) the cost of quality health care was difficult to value for both GPs and patients; there was belief in universal coverage and the importance of quality, but trade-offs in quality of care were a common perception; (2) the doctor-patient relationship patient-centred care was a common goal and perceived as a good measure of quality care and a way for patients to place a value on the service/care; and (3) the business of general practice participants wanted to see sustainable business models for primary care that incentivised quality of care. A Patient-Chosen Gap Payment (PCGP) funding model could incentivise doctors to provide better care without limiting access to health care.

This article uses a normal mode approach to predict atmospheric sound propagation over a locally reacting impedance plane. The semi-analytic finite element method is used to compute the normal modes, which enables the exact governing wave equation for a moving fluid to be solved in two dimensions. A locally reacting surface is added using the general Ingard-Myers boundary condition, and the transmission loss is obtained for cylindrical and spherical spreading for range independent problems. The approach developed in this article will, in principle, converge toward the exact solution and so has the potential to provide benchmark predictions for complex, range independent, outdoor sound propagation problems. Predictions are shown to provide good agreement with benchmark solutions available in the literature, including those with a logarithmic wind velocity profile. Results are also reported for a combination of a logarithmic wind velocity profile and a temperature inversion for ranges of up to 5 km. These SBBs can only be perceived when different signals are presented dichotically (to both ears). Event-related potentials were recorded in 22 normal-hearing subjects. Dichotic stimulus presentation reliably evoked auditory late responses (ALRs) in all subjects using such manipulated signals. As control measurements, diotic stimulation modalities were presented to confirm that the ALRs were not evoked by the speech signal itself or that the signal manipulation scheme created audible artifacts. Since diotic measurements evoked no ALRs, responses from dichotic stimulation are a pure correlate of binaural interaction. These findings suggest that the interaction of applied currents with fibers' specific intrinsic properties may introduce fundamental limits on presentation of fine temporal structure in electrical stimulation.
This article uses a normal mode approach to predict atmospheric sound propagation over a locally reacting impedance plane. The semi-analytic finite element method is used to compute the normal modes, which enables the exact governing wave equation for a moving fluid to be solved in two dimensions. A locally reacting surface is added using the general Ingard-Myers boundary condition, and the transmission loss is obtained for cylindrical and spherical spreading for range independent problems. The approach developed in this article will, in principle, converge toward the exact solution and so has the potential to provide benchmark predictions for complex, range independent, outdoor sound propagation problems. Predictions are shown to provide good agreement with benchmark solutions available in the literature, including those with a logarithmic wind velocity profile. Results are also reported for a combination of a logarithmic wind velocity profile and a temperature inversion for ranges of up to 5 km. As control measurements, diotic stimulation modalities were presented to confirm that the ALRs were not evoked by the speech signal itself or that the signal manipulation scheme created audible artifacts. The reliability of the information on the Internet, which people use as an easy and practical solution about diseases, is essential for public health. Our aim in this study is to evaluate the quality and readability of websites related to myofascial pain syndrome.
On April 4, 2020, websites were searched on the Google search engine using the term "myofascial pain syndrome". The typologies, quality, readability, and content parameters of the sites were analyzed. Websites were divided into eight categories according to typology. To evaluate the quality, we evaluated the websites according to the JAMA scoring system or a HONcode certificate existence. Flesch-Kincaid grade and the Simple Measure of Gobbledygook was used to evaluate readability. Content analysis was performed for etiology, symptoms, diagnosis, and treatment.
56 of the 151 websites evaluated were classified as high-quality websites. It was determined that the quality scores of scientific publications and professional websites were high. Comparing the readability parameters of the websites in terms of quality, there was no significant difference between high-quality websites and low-quality websites (p=0.391 and 0.746 respectively). The content was focused on etiology on scientific websites, while on commercial and professional websites, the content was focused on treatment.

The thoracolumbar junction syndrome or Maigne's syndrome is one of the causes of low **** pain, for which no comprehensive treatment has yet been proposed. This case report aims to assess the effectiveness of a global physiotherapy approach, such as the Mézières method, to reduce pain and improve posture and mobility in a patient with thoracolumbar junction syndrome (TLJS).
A 42-year-old woman patient presented a chronic nonspecific low **** pain associated with severe pain in the right inguinal fold which has disabled her for two years. She described her pain as deep and intermittent. She complained of difficulties performing tasks that involved on long walks and bending forward lumbar flexion.
After ten sessions the pain completely disappeared (numeric rating scale-NRS improved from 6 to 0). A slight improvement in certain morphostatic parameters in the frontal and sagittal planes was reported, analyzed by ADiBAS software. Six months after the final session, the patient was asymptomatic, with improved bodily awareness and muscular extensibility.

The study included 10 chronic stroke patients; 5 with left brain injury (LI), mean age 48.8 years (±4.76), and 5 with right brain injury (RI), mean age 52 years (±10.93). Patients were evaluated for electroencephalographic activity (alpha and beta frequencies) and performance (absolute error) in a darts game on XBOX Kinect (Microsoft®). Then they underwent a virtual darts game training task, 12 sessions for 4 weeks (acquisition stage). After training, they were revaluated (long-term retention).

Bidirectional integrin αIIbβ3 signaling is essential for platelet activation. The platelet adaptor protein Disabled-2 (Dab2) is a key regulator of integrin signaling and is phosphorylated at serine 24 in eukaryotic cells. However, the mechanistic insight and function of Dab2-serine 24 phosphorylation (Dab2-pSer24) in platelet biology are barely understood. This study aimed to define whether and how Dab2 is phosphorylated at Ser24 during platelet activation and to investigate the effect of Dab2-pSer24 on platelet function.
An antibody with confirmed specificity for Dab2-pSer24 was generated. By using this antibody as a tool, we showed that protein kinase C (PKC)-mediated Dab2-pSer24 was a conservative signaling event when human platelets were activated by the platelet agonists such as thrombin, collagen, ADP, 12-O-tetradecanoylphorbol-13-acetate, and the thromboxane A2 activator U46619. The agonists-stimulated Dab2-pSer24 was attenuated by pretreatment of platelets with the RGDS peptide which inhibits inteably, Ser24 and the previously reported Ser723 phosphorylation (Dab2-pSer723) occurred exclusively in a single Dab2 molecule and resulted in distinctive subcellular distribution and function of Dab2. Dab2-pSer723 was mainly distributed in the cytosol of activated platelets and associated with integrin inside-out signaling, while Dab2-pSer24 was mainly distributed in the membrane fraction of activated platelets and associated with integrin outside-in signaling.

Small interfering RNAs were used to determine the function of target genes. Co-immunoprecipitation (Co-IP) was performed to detect the interaction between DDB2 and CDT2. Immunofluorescence assays and fluorescence activating cell sorting (FACS) were used to measure the change of DNA content. In vivo ubiquitination assay was carried out to clarify the ubiquitination of CDT2 mediated by DDB2. Cell synchronization was performed to arrest cells at G1/S and S phase. The mechanism involved in DDB2-mediated CDT2 degradation was investigated by constructing plasmids with mutant variants and measured by Western blot. Immunohistochemistry was performed to determine the relationship between DDB2 and CDT2. Pairedex. Our results broaden the horizon for understanding the opposite function of CDT2 and DDB2 in tumorigenesis, and may provide clues for drug discovery in cancer therapy.

To investigate the potential beneficial effect of fecal microbiota transplantation (FMT) on gastrointestinal symptoms, gut dysbiosis and immune status in discharged COVID-19 patients.
A total of 11 COVID-19 patients were recruited in April, 2020, about one month on average after they were discharged from the hospital. All subjects received FMT for 4 consecutive days by oral capsule administrations with 10 capsules for each day. In total, 5 out of 11 patients reported to be suffered from gastrointestinal symptoms, which were improved after FMT. After FMT, alterations of B cells were observed, which was characterized as decreased naive B cell (P = 0.012) and increased memory B cells (P = 0.001) and non-switched B cells (P = 0.012).The microbial community richness indicated by operational taxonomic units number, observed species and Chao1 estimator was marginally increased after FMT. Gut microbiome composition of discharged COVID-19 patients differed from that of the general population at both phylum and genera level, which was characterized with a lower proportion of Firmicutes (41.0%) and Actinobacteria (4.0%), higher proportion of Bacteroidetes (42.9%) and Proteobacteria (9.2%). FMT can partially restore the gut dysbiosis by increasing the relative abundance of Actinobacteria (15.0%) and reducing Proteobacteria (2.8%) at the phylum level. At the genera level, Bifidobacterium and Faecalibacterium had significantly increased after FMT.

To investigate the potential beneficial effect of fecal microbiota transplantation (FMT) on gastrointestinal symptoms, gut dysbiosis and immune status in discharged COVID-19 patients. A total of 11 COVID-19 patients were recruited in April, 2020, about one month on average after they were discharged from the hospital. All subjects received FMT for 4 consecutive days by oral capsule administrations with 10 capsules for each day. In total, 5 out of 11 patients reported to be suffered from gastrointestinal symptoms, which were improved after FMT. After FMT, alterations of B cells were observed, which was characterized as decreased naive B cell (P = 0.012) and increased memory B cells (P = 0.001) and non-switched B cells (P = 0.012).The microbial community richness indicated by operational taxonomic units number, observed species and Chao1 estimator was marginally increased after FMT. Gut microbiome composition of discharged COVID-19 patients differed from that of the general population at both phylum and genera level, which was characterized with a lower proportion of Firmicutes (41.0%) and Actinobacteria (4.0%), higher proportion of Bacteroidetes (42.9%) and Proteobacteria (9.2%). However, monolinguals outperformed the monoliterate bilinguals in SR accuracy, as well as in the visuospatial working memory and updating tasks. The results also demonstrate that updating and visuospatial working memory significantly predicted monolingual children's SR accuracy scores, whereas Greek vocabulary predicted the performance of our monoliterate bilingual children in the same task. However, from the perspective of user needs and the content adaptive system, there are few suitable intelligent navigation and user demand anticipatory systems for intangible cultural heritage content. Purple clay culture is one of the first batches of national intangible cultural heritage protection projects in China. Therefore, taking purple clay culture exhibition as an example, this paper attempts to analyze the personalized information demand of tourism consumption experience in intangible cultural heritage communication activities with affective computing and meaning-driven innovative design method, by taking the content design in the navigation system as the research object. This paper uses the theory of planned behavior to calculate the relationship between tourists' attitude, experience behavior, and display information demand. The findings indicate two issues. First, tourists' demand for the entertainment and leisure attributes of intangible cultural heritage is greater than the demand for educational function attributes. Second, the meaning elements of information can change tourists' beliefs in intangible cultural heritage and affect their attitude and behavior toward such heritage. According to the research results, strengthening the meaning elements of specific group information can improve people's cultural identity and tourism satisfaction. This is a retrospective cohort study conducted in paediatric emergency department (ED) of a tertiary hospital in Rome. Children (0-18 years) evaluated for abdominal pain in the paediatric ED between January 2018 and December 2018 were enrolled. Patients were divided in 2 groups organic and functional groups. The main outcome was the prevalence of different subtypes of functional disorders (according to Rome IV criteria) and the role of possible biopsychosocial disorders related to patients with functional symptoms.
In this study, 1130 patients were included. In the functional group, 37.6% of patients were classified as affected by functional dyspepsia, 26.7% by functional abdominal pain-non otherwise specified (FAP-nos), 20.8% by inflammatory bowel syndrome (IBS), and 15.9% by abdominal migraine. Children of our functional sample reported at least one of the items present in the inventory CSSI-24 (24-item Children's Somatic Symptoms Inventory), but more than 50% reported at least 4 of the items investigated.
At 6 and 12 months follow up and, new prospective studies on these disorders could improve the inclusion criteria for FGIDs, thinning the dubious rate of patients expected from the literature on the Rome IV criteria.

To elucidate the role of TUG1 in the onset of type 2 diabetes mellitus (T2DM) and the potential mechanism.
Relative levels of TUG1 and SP1 in high-fat diet animal model and high-glucose cell model were detected by quantitative Real Time-Polymerase Chain Reaction (qRT-PCR), and their correlation was analyzed. Potential binding sites in the promoter sequences of TUG1 and SP1 were predicted using the JASPAR. Their interaction was further confirmed by chromatin immunoprecipitation (ChIP) and Dual-Luciferase reporter assay. The influences of TUG1 on proliferative and apoptotic potentials in Min6 cells were examined by Cell Counting Kit-8 (CCK-8), 5-Ethynyl-2'-deoxyuridine (EdU) assay and flow cytometry, respectively. Subsequently, the interaction in the TUG1/miR-188-3p /FGF5 axis was similarly explored by Dual-Luciferase reporter assay.
SP1 and TUG1 were downregulated in high-fat and high-glucose models, and they displayed a positive correlation. TUG1 bound E2 region in SP1 promoters. Knockdown of TUG1 inhibited proliferative rate and induced apoptosis in high-glucose-treated Min6 cells. The aim of this study is to examine the maternal serum and cord blood irisin and preptin levels in gestational diabetes mellitus (GDM) and correlate their levels with demographic and biochemical parameters.
A total of 21 pregnant women with GDM and 21 BMI and age-matched pregnant women without GDM were included in the study. They underwent 50 g glucose challenge test (GCT) between 24-28th gestational weeks. Women with a GCT result higher than 140 mg/dl received 100 g oral glucose tolerance test (OGTT). Detection of one of the following criteria after OGTT was accepted as GDM fasting plasma glucose level 92 mg/dL; 1-h plasma glucose level 180 mg/dL; and 2-h plasma glucose 153 mg/dL. When we disrupted or delayed head movement-contingent updating of the visual display, the proportion of motion-in-depth misreports again increased, reflected in both a reduction in sensitivity and an increase in bias. Our findings identify a critical function of head jitter in visual motion perception, which has been obscured in most (head-fixed and non-head jitter contingent) laboratory experiments.
Inflammatory bowel disease affects all aspects of life, yet little is known about the impact of the condition on intimacy and sexuality, and if such concerns should be discussed with healthcare professionals. This hermeneutical phenomenological study aimed to explore the experiences of people living with inflammatory bowel disease discussing their sexuality concerns with healthcare professionals.

Participants described the lack conversations with their healthcare professionals on sexual wellbeing issues, in spite of the importance they gave to the topic, and identified barriers to having such conversations. They made suggestions for future clinical practice that would better meet their needs. The few who had discussed sexual wellbeing issues with healthcare professionals reported negative experiences.
Patients' needs and preferences about addressing concerns related to their sexual well-being during clinical appointments should be addressed routinely and competently by healthcare professionals. Understanding the implications of inflammatory bowel disease on intimate aspects of the lives of those living with the condition could improve the quality of the care provided.

Phylodynamic analyses using a birth-death model generated effective reproductive number estimates (the average number of secondary infections per each infectious case, Re ) larger than one for most of the epochs considered. However, the possible range of the initial Re included estimates lower than one despite the known rapid spread of RHDV in Australia. Furthermore, the analyses that accounted for geographical structuring failed to converge. We argue that the difficulties that we encountered most likely stem from the fact that the samples available from 1996 to 2014 were too sparse with respect to both geographic and within outbreak coverage to adequately infer some of the model parameters. In general, while these phylodynamic analyses proved to be greatly informative in some regards, we caution that their interpretation may not be straightforward. We recommend further research to evaluate the robustness of these models to assumption violations and sensitivity to sampling regimes.
Hospital-at-home (HAH), a pioneering health care model, is an accepted alternative to hospital treatment for patients with a chronic obstructive pulmonary disease (COPD) exacerbations. The aim of the present study was to analyze the effectiveness of HAH for patients with COPD exacerbations.
Two hundred six patients with COPD exacerbations who were admitted to our emergency room (ER) received the HAH model between January 2008 and March 2010. The number of patient's hospitalization, admission to emergency room, unscheduled outpatient attendance, and the length of stay in hospital (day) were recorded before and after a one-year period of HAH.
After a one-year follow-up period of the HAH program, the number of patient who had hospitalization, admission to ER, unscheduled outpatient attendance rates was decreased 41.3%, 54.4%, 49.5% respectively. The decreases for all parameters were found to be statistically significant (P < 0.001). Additionally the total number of length of stay in hospital (day) after a one-year period after HAH was decreased (46.5%).
Integrated care services, including home care units where HAH models are performed, are necessary to improve the health of patients with COPD, as well as to better manage their condition in terms of disease burden. Physicians should consider this form of management, especially because there is increasing pressure on inpatient bed requirement in Turkey.
