  • Acute aortic occlusion is a rare life-threatening event. We present a case of a heavy smoking, 54-year-old man who was admitted in the emergency room with sudden paraplegia, associated to severe lower **** and lower limbs pain. A neurologic examination showed paralysis of the lower limbs and cold lower extremities. The pedal and femoral pulses were absent. A computed tomography revealed occlusion of the mesenteric superior artery, abdominal aorta, and both iliac arteries. Despite medical treatment, the patient died before evaluation of vascular surgery. This case demonstrates the presentation of C. koseri meningitis with pneumocephalus and cerebral edema in a neonate presenting with fever.Renal ectopia is a rare congenital anomaly that mostly occurs in the pelvic area. An ectopic kidney is usually associated with other anomalies such as a malrotation. We report the case of a 15-year-old male who consulted after a blunt abdominal trauma. A left iliac renal ectopia was incidentally discovered. This ectopic kidney was associated with a malrotation, and a pyelo-ureteral duplication. Iliac renal ectopia should be dissociated from other abdominal renal ectopias, and its association with other renal malformations should be further investigated.Secondary aortoenteric fistula or erosion (SAEFE), an abnormal connection between the aorta and gastrointestinal tract, is a rare but critical complication after abdominal aortic aneurysm repair. Most SAEFEs occur between the aorta or proximal graft anastomosis and the duodenum, and occurrence between the iliac graft and small intestine is rare. Standard SAEFE management involves graft removal and extra-anatomical bypass. However, this is extremely invasive and has a high mortality rate. We encountered a rare case of SAEFE with no sign of infection, which was successfully treated by ligating the iliac graft to reduce mechanical pulsatile stress and bleeding following the retroperitoneal approach.Leiomyosarcoma of the inferior vena cava (IVC) is a rare malignancy, but has been found more frequently with recent advances in diagnostic imaging. Local recurrence and metastases are frequent with this pathology, and prognosis is poor. We report a 70-year-old male who developed a rapidly progressing distal arch aneurysm caused by a distal stent graft-induced new entry (DSINE) 7 months after the procedure. The JOSG was originally implanted at the curved part of the distal arch. It created its initial DSINE on the greater curve and rapidly "sprang" **** in 2 months. Urgent thoracic endovascular aortic repair fixed this serious complication.     This was a (2018-2020) prospective cohort study of prenatally THC-exposed newborns. Maternal and Infant demographics, urine (UDS) and umbilical cord drug screening (UCDS) were recorded. A limited channel continuous aEEG was obtained within 48 h of birth. Statistical analysis included univariate, multivariate, and logistical regression.

    A total of 30 mother/infant dyads were enrolled. 60% (18/30) of neonates had abnormal aEEGs with sleep wake cycle (SWC) disturbances (p < 0.001). UCDS Carboxy-THC pg/g levels were similar in infants with abnormal [1758 (296,2838)] and normal aEEG [1589 (332,2794)], p = 0.82.

Population density explains the long-term trend in Dixie Alley. The step-increase induced due to the installation of the Doppler Radar systems explains the long-term trend in Tornado Alley. The genome consists of 3,607,821 bp distributed in 18 contigs with 3337 protein coding genes and 53% of the annotated CDS are having putative functions. Gene annotation disclosed the presence of genes involved in ABC transporter-dependent pathway of EPS biosynthesis. As the ABC transporter-dependent pathway is also implicated in the capsular polysaccharide (CPS) biosynthesis, we employed extraction protocols for both EPS (from the culture supernatants) and CPS (from the cells) and found that the secreted polysaccharide i.e., EPS was predominant. The EPS showed good emulsifying activities against the petroleum hydrocarbons and its production was dependent on the carbon source supplied.

This study conceptualizes, develops, and validates the first comprehensive CCBT-informed self-transcendence questionnaire. Using a CCBT-informed theory, we propose four self-transcendence facets distancing oneself from mental content, distinguishing an observer of mental experience that is separate from the content of experience, experiencing innate connectedness with other beings, and noticing the constantly changing nature of experience. We measured these facets with items from existing relevant questionnaires and novel, expert-informed items. This study conceptualizes, develops, and validates the first comprehensive CCBT-informed self-transcendence questionnaire.   • Our findings add to existing evidence and raise awareness on PNNMOSD. We enhance the importance for the clinicians of recognizing tumor symptoms and signs whenever a NMOSD is newly diagnosed.Emergency departments (EDs) have a key role in the public health system. They are facing a constant growth of their volume. Forecasting the daily volume is a major tool to adapt the allocation of resources. In this paper, we focus on pediatric EDs. They are specific by their strong seasonal variation, determined by the academic pace. The main contribution of this paper is to integrate the effects of this pace to the annual seasonality. We also tried out to improve the daily forecasting by forecasting the week means of the flow first. We trained and tested these models specifically on the pediatric EDs of Paris university hospital trust. For the eight pediatric EDs gathered, on average for the years 2016 to 2019, we forecasted the daily volume with a Mean Absolute Percentage Error (MAPE) of 6.6% for a 7-days forecasting, 7.1% for a 14-days forecasting and 7.6% for a 28-days forecasting. Account of rhythm allows a performance increase, with results respectively 7%, 10.1% and 8.4% better relatively to a baseline model based on a periodic regression on the weeks.High expression of PD-L1 predicts PD-1/PD-L1 inhibitor benefit, meanwhile a few PD-L1-negative patients still benefit from these drugs. In this study, we aimed to explore the underlying cellular and molecular characteristics via single-cell sequencing. Before and after treatment with Pembrolizumab, peripheral blood mononuclear cells (PBMCs) were isolated via Ficoll gradient. In this study, we have examined a custom-made QIAseq Microhaplotype panel (Qiagen), including 45 different microhaplotype loci. DNA libraries were prepared according to the GeneRead DNAseq Targeted Panels V2 library preparation workflow (Qiagen) and sequenced on a MiSeq FGx instrument (Verogen). We evaluated the performance of the panel based on 75 samples of Swedish origin and haplotype frequencies were established. We performed sensitivity studies and could detect haplotypes at input amounts down to 0.8 ng. We also studied mixture samples with two contributors for which haplotypes, for the minor contributor, were detectable down to the level of 1100. Furthermore, we executed kinship simulations to evaluate the usefulness of this panel in kinship analysis. The results showed that both paternity and full sibling cases can clearly be solved. When simulating a half sibling versus unrelated case scenario, there were, however, some overlap of the likelihood ratio distributions potentially resulting in inconclusiveness. To conclude, the results of this initial study are promising for further implementation of this microhaplotype assay into the forensic field, although we noticed some primer design issues that could be optimized, which possibly would increase the power of the assay.Radiation exposure is a crucial factor to consider in forensic age estimation. The intra- and interobserver reliability values were κ = 0.906 and 0.869, respectively. Based on the results, the respective minimum ages estimated for stages 4 and 5 were 13.4 and 16.1 years for females, and 15.1 and 17.3 years for males; the method could not estimate an age of 18 years in any case. FSE PD MRI analysis of the distal radius epiphysis provides supportive data and can be used when evaluating the distal radius for forensic age estimation.In recent years, recorded cases related to forensic botany and, in particular, of plant poisoning have become rare. We report on the medicolegal characteristics of an undetermined sudden death (USD) of a woman in which scene there were remnants of a vegetal peeling. After the autopsy, macroscopic findings reported multiorgan failure and requested the investigation of the cause of death. Postmortem blood was firstly investigated on cyanide toxicity presumptively coming from a yucca-like root; however, found cyanide levels were under normality. Because of the lack of morphological features of the encountered plant remains, a genetic nrDNA ITS2 sequence investigation was followed. The resulting DNA sequence could identify the evidence as the water dropwort (Oenanthe spp.) which contains oenanthotoxin, a potent toxin that may be fatal, similar to the more commonly found in hemlock Conium or cowbane Cicuta species. A liquid chromatography-tandem high resolution mass spectrometry (LC-QTOF MS) was later applied to analyse the vegetal extract and stomach content and successfully confirmed the toxin existence.

The morphing median fins have the ability of adjusting the hydrodynamic forces, thereby affecting the yaw mobility of tuna to a certain extent. In this paper, the hydrodynamic analysis of the median fins under different morphing states is carried out by the numerical method, so as to clarify the influence of the erected median fins on the yaw maneuvers. By comparing the two morphing states of erected and depressed, it can be concluded that the erected median fins can increase their own hydrodynamic forces during the yaw movement. However, the second dorsal and anal fins have limited influence on the yaw maneuverability, and they tend to maintain the stability of tuna. The first dorsal fin has more lift increment in the erection state, which can obviously affect the hydrodynamic performance of tuna. Moreover, as the median fins are erected, the hydrodynamic forces of the tuna's body increase synchronously due to the interaction between the body and the median fins, which is also very beneficial to the yaw motion. This study indicates that tuna can use the morphing median fins to adjust its mobility and stability, which provides a new idea for the design of robotic fish.

In this paper, a hydraulic-driven MRI-compatible breast interventional robot is proposed to perform breast interventional procedure.

    First is the analysis of the design requirements of the hydraulic-driven MRI-compatible breast interventional robot, and then the design scheme is determined. Second, the three-dimensional model and the link frames are established. In this paper, the hydrodynamic analysis of the median fins under different morphing states is carried out by the numerical method, so as to clarify the influence of the erected median fins on the yaw maneuvers. By comparing the two morphing states of erected and depressed, it can be concluded that the erected median fins can increase their own hydrodynamic forces during the yaw movement. However, the second dorsal and anal fins have limited influence on the yaw maneuverability, and they tend to maintain the stability of tuna. The first dorsal fin has more lift increment in the erection state, which can obviously affect the hydrodynamic performance of tuna. Moreover, as the median fins are erected, the hydrodynamic forces of the tuna's body increase synchronously due to the interaction between the body and the median fins, which is also very beneficial to the yaw motion. Evidence from mechanistic and human data suggests that occupational exposure to ergonomic (or physical) risk factors may cause osteoarthritis and other musculoskeletal diseases (excluding rheumatoid arthritis, gout, and **** and neck pain). In this nationwide cross-sectional study in England, we included 38,573 COVID-19 deaths up to June 30, 2020 at the Lower Layer Super Output Area level (n = 32,844 small areas). We retrieved averaged NO2 and PM2.5 concentration during 2014-2018 from the Pollution Climate Mapping. We used Bayesian hierarchical models to quantify the effect of air-pollution while adjusting for a series of confounding and spatial autocorrelation. We find a 0.5% (95% credible interval -0.2%, 1.2%) and 1.4% (95% CrI -2.1%, 5.1%) increase in COVID-19 mortality risk for every 1 μg/m3 increase in NO2 and PM2.5 respectively, after adjusting for confounding and spatial autocorrelation. This corresponds to a posterior probability of a positive effect equal to 0.93 and 0.78 respectively. The spatial relative risk at LSOA level revealed strong patterns, similar for the different pollutants. This potentially captures the spread of the disease during the first wave of the epidemic.     Rats are common intruders into human settlements. Apart from their role as disease vectors, they can also cause bite injuries. We describe the clinical course of a series of children with rat bites, and characterise the injures.

    A retrospective review of hospital records of children admitted for rat bites in a large regional hospital was undertaken. The demographics, wound characteristics, treatment given and clinical outcome of the patients are described.

    Fifty-nine children, with a mean age of 3.7years, were admitted for rat bites. Three distinct types of wounds were treated superficial scratches (Type I), deeper bites often with infection and ulceration (Type II) and full-thickness with loss of skin or underlying soft tissues (Type III). Few wounds displayed signs of inflammation.     Previous studies have indicated that cerebral arterial morphology is linked to aging and some cerebrovascular diseases. However, the mechanisms of morphological changes remain unclear. This study evaluated age-related positional changes in the basilar artery (BA) bifurcation based on longitudinal computed tomography angiography (CTA) data.

    This retrospective study evaluated clinical and imaging data from 72 subjects who underwent two CTA scans between July 2011 and August 2019. Three-dimensional (3D) models were reconstructed for each subject based on the two CTA scans with the longest separating interval. Skull landmarks were used to fuse the two models, and the fused model was used to evaluate positional changes in the BA bifurcation. Univariable and multivariable analyses were used to identify variables that were correlated to BA bifurcation shifting. Pearson's correlation test was used to analyze the correlation between the shifting distance and change in the BA bifurcation angle.

    Significant differences between aneurysm and non-aneurysm cases were observed in terms of sex (p = 0.004), CTA scan interval (p = 0.023), and BA bifurcation shifting distance (p = 0.007). Multivariable linear regression analysis revealed that the BA bifurcation shifting distance was significantly correlated with the CTA scan interval (p = 0.038) and the presence of aneurysms (p < 0.001). Furthermore, the shifting distance was positively correlated with widening of the BA bifurcation angle (p = 0.002).

In patients with VT, left ventricular ejection fraction (LVEF) was lower (62.6 ± 9.3% vs. 66.6 ± 8.9%, p = 0.003), and B-type natriuretic peptide (BNP) was higher compared with patients without VT (median, 52.5 pg/mL vs. 32.8 pg/mL, p = 0.02). This study evaluated age-related positional changes in the basilar artery (BA) bifurcation based on longitudinal computed tomography angiography (CTA) data. This retrospective study evaluated clinical and imaging data from 72 subjects who underwent two CTA scans between July 2011 and August 2019. Three-dimensional (3D) models were reconstructed for each subject based on the two CTA scans with the longest separating interval. Skull landmarks were used to fuse the two models, and the fused model was used to evaluate positional changes in the BA bifurcation. Univariable and multivariable analyses were used to identify variables that were correlated to BA bifurcation shifting. Pearson's correlation test was used to analyze the correlation between the shifting distance and change in the BA bifurcation angle. Significant differences between aneurysm and non-aneurysm cases were observed in terms of sex (p = 0.004), CTA scan interval (p = 0.023), and BA bifurcation shifting distance (p = 0.007). Multivariable linear regression analysis revealed that the BA bifurcation shifting distance was significantly correlated with the CTA scan interval (p = 0.038) and the presence of aneurysms (p < 0.001). Furthermore, the shifting distance was positively correlated with widening of the BA bifurcation angle (p = 0.002). Aging-related widening of the BA bifurcation angle may be related to distal shifting of the BA bifurcation's position, and larger distal shifting of the BA bifurcation may be associated with the risk of aneurysm formation. Multiple measures were proposed for this diversity. We here focus on the V-gene usage and the CDR3 sequences of the beta chain. We compared multiple T cell repertoires to follow T cell repertoire changes post-allo-BMT in HLA-matched related donor and recipient pairs. Our analyses of the differences between donor and recipient complementarity determining region 3 (CDR3) beta composition and V-gene profile show that the CDR3 sequence composition does not change during restoration, implying its dependence on the HLA typing. In contrast, V-gene usage followed a time-dependent pattern, initially following the donor profile and then shifting back to the recipients' profile. The final long-term repertoire was more similar to that of the recipient's original one than the donor's; some recipients converged within months, while others took multiple years. In 601 patients, VT was detected in 46 patients (7.7%). In patients with VT, left ventricular ejection fraction (LVEF) was lower (62.6 ± 9.3% vs. 66.6 ± 8.9%, p = 0.003), and B-type natriuretic peptide (BNP) was higher compared with patients without VT (median, 52.5 pg/mL vs. 32.8 pg/mL, p = 0.02). The maximum number of consecutive beats of VT was more frequent in the patients with LVEF  less then  50% than in the patients with LVEF ≥ 50% (median, 11.5 beats vs. 3.0 beats, p = 0.01).     Data from a multicenter database of 2802 patients with clinical T1-2N0-1M0 NSCLC who underwent anatomical segmentectomy or lobectomy were retrospectively analyzed. Unexpected N2 disease was defined as pathologic N2 disease with clinical N0 or N1. The predictive criteria of unexpected N2 disease were established on the basis of the multivariable analysis results of a derivation cohort of 2019 patients, and the criteria were further tested in a validation cohort of 783 patients.

The long-term effect of sapropterin on intellectual functioning was assessed using the Full-Scale Intelligence Quotient (FSIQ) in 62 children who began treatment before the age of 6 years. Over each 2-year interval, the estimate of mean change in FSIQ was -0.5768 with a lower limit of the 95% confidence interval (CI) of -1.60. At the end of the follow-up period (Year 7), the least squares mean estimate of the change in FSIQ from baseline was 1.14 with a lower limit of the 95% CI of -3.53. These lower limits were both within the clinically expected variation of 5 points. During the whole study period, mean blood Phe levels remained within the American College of Medical Genetics (ACMG) target range of 120-360 μmol/L. In addition, height, weight, and head circumference were maintained within normal ranges throughout follow-up, as defined by growth charts from the World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) for children below and above the age of 24 months, respectively.   • In April 2012, the Manitoba Home Cancer Drug Program (HCDP) was introduced to allow 100% coverage for eligible oral anticancer agents (OAA) and supportive medications for Manitobans with cancer requiring these therapies.

    What is the extent of use and cost of OAAs among outpatients in Manitoba from 2003/04 to 2015/16? Did the HCDP change OAA user and prescription patterns?

    This was a retrospective, population-based study using administrative data to measure the prevalence of drug utilization over time and the impact of HCDP on OAA use and prescriptions using generalized linear models. Manitobans with cancer who filled an OAA or supportive medication covered by HCDP from 2003/04 to 2015/16 were included.

    This study included 22,393 people with cancer who filled an OAA prescription. The prevalence of OAA use increased from 222 per 100,000 to 328 per 100,000 from 2003/04 to 2015/16. Hormone therapy for breast cancer was the most common class of OAA used (increased from 154 per 100,000 to 231 per 100,000). We observed a 2.6-fold decrease in the prevalenceof oral alkylating agents and a 10.7-fold increase in the prevalence of protein kinase inhibitors during the study period. The total cost of targeted OAAs per year for all Manitobans with cancer increased from $1.8 million to $19 million.

    We observed an increase in OAA prevalence and the cost of oral targeted chemotherapy is high. Our findings underline the need for addressing these high-cost medications in future developments of a national drug program.

    Other forces related to socio-economic and cultural factors, besides biomedical and behavioural fields, also influence health but receive little attention in health research. This study aims to illuminate social determinants of health and to identify challenges and opportunities in addressing social determinants of child health (SDCH) in rural Cambodia.

    This is a qualitative study based on interviews of frontlineprimary healthcare providers, health officials, local authorities and community volunteers in two health districts in Cambodia. The data were supplemented by secondary data on different aspects of the districts and Cambodia.

    Poverty, lack of basic commodities and adverse social conditions remained problems for population health. While access to health services wasconsideredadequate, households and communities had several major risk exposures. Challenges in addressing SDCH were the high prevalence of social and household adverse conditions, andthe lack of training of providers, of information abos, training primary health care providers and providing them with adequate information about local resources and available supports. Improving social care services and infrastructures requires strong coordination, planning and adequate resource allocation.We aimed to investigate contemporary management and outcomes of bile leakage in patients who underwent hepatectomy with hepaticojejunostomy for liver malignancy. The NSQIP database was used to study clinical data of patients who underwent a hepatectomy with hepaticojejunostomy for a primary hepatobiliary cancer and developed bile leakage between 1/2014 and 12/2017. Multivariate regression analysis was performed to investigate outcomes. Five hundred patients underwent a hepatectomy with hepaticojejunostomy for a malignant primary hepatobiliary cancer (41% intrahepatic cholangiocarcinoma, 38.2% hilar cholangiocarcinoma, 9.8% hepatocellular carcinoma, 6% gallbladder cancer, and 5% others). The rate of bile leakage was 33.4%. Most patients (90.4%) did not require re-exploration. In 77 of 157 patients (49.1%), bile leakages were contained with intraoperatively placed drain(s) and no additional surgical intervention was required. The protocols described here also include computer scripts for users to improve their experimental design, run genome-wide association studies (GWAS), perform multi-testing corrections, and obtain the Manhattan plots, enabling the identification of loci associated with salinity tolerance. Notably, the computer scripts provided here can be used for any stress or GWAS experiment and independently of HTP.Plant genomes can withstand small- and large-scale duplications, at a far greater success than any other kingdom in the tree of life, resulting in the existence and evolution of gene families, often with over a hundred members! Due to the large number of such cases in the plant kingdom, it has become a routine task for plant biologists to investigate their specific gene family of interest. In this chapter, we provide a simple and standard pipeline for this effort, taking the example of steroidogenic acute regulatory protein (StAR) related lipid transfer (START) domains in rice, as reference. We describe the extraction, processing, and downstream analysis of Oryza sativa var. japonica proteome towards identification and comparative exploration of START domains. This was done by training profile Hidden Markov Models (HMM) of 35 reported START domains in Arabidopsis, which were then used to search potential homologs in rice.

Data analyses were conducted between December 2019 and May 2019.

Consistent with the functional importance of PA binding, mutation of this region abrogates recruitment of Chm7 to membranes and abolishes Chm7 function in the context of NE herniations that form during defective nuclear pore complex (NPC) biogenesis. As the laser wavelength decreases from 290 to 250 nm, the relative yield of H2O oxidation increases significantly. Likewise, photoinduced H2O desorption was also observed in the range of 320-250 nm, and the relative yield of H2O desorption also increases with a decrease in wavelength. The strong wavelength-dependent H2O photooxidation and photodesorption suggest that the energy of charge carriers is important in these two processes. More importantly, the result raises doubt about the widely accepted photocatalysis model of TiO2 in which the excess energy of charge carriers is useless for photocatalysis. The computed electric transport properties also show that the SiAs monolayer exhibits strong anisotropy in the biased voltage in the range of -1 to 1 V. In particular, the current reflects the ON state in the a direction but the OFF state in the b direction. In addition, we find that the uniaxial strain can significantly improve the electric transport performance and even lead to the negative differential conductance at 10% strain. Herein, we report on a series of dyads consisting of a naphthalenediimide (NDI) electron acceptor with one or two phenyl-based donors for which photoinduced intramolecular electron transfer is fast enough to be competitive with pure electronic dephasing. As a consequence, the absorption band of the π-π* transition of these dyads is broader than that of the NDI alone to an extent that scales with the electron transfer rate. Additionally, this reaction is so fast that it leads to the impulsive excitation of a low-frequency vibrational mode of the charge-separated product. Quantum-chemical calculations suggest that this vibration involves the C-N donor-acceptor bond, which shortens considerably upon electron transfer.Herein, an organic fluorophore termed NLAC is introduced into 2D hybrid perovskites with wide band gap (>3.54 eV) to give a green emission with quantum yield up to 81%. The highly efficient luminescence is ascribed to avoiding the aggregation of NLAC and formation of an inorganic free exciton which is easy to thermally quench. On this basis, a new strategy to generate efficient white emission with afterglow has been proposed by codoping a short-wavelength fluorophore and long-wavelength phosphor into 2D organic-inorganic hybrid perovskites (OIHPs). As a result, a single-component white-light-emitting material PEPC-3N based on NLAC with CIE of (0.33, 0.36) and quantum yield up to 43% can be obtained. Interestingly, PEPC-3N shows a dual-color organic afterglow and excitation-wavelength-dependent emission, consequently forming a switch between green fluorescence and yellow afterglow. Thermal and electrical properties were measured utilizing a specialized sample cell assembly designed for the Paris-Edinburgh large-volume press to a maximum pressure of 3.5 GPa. High-pressure structural measurements performed up to 50 GPa in a diamond-anvil cell indicated the emergence of a new high-pressure phase around 20 GPa. A first-principles structure search performed using an ab initio random structure search approach identified the high-pressure phase as an orthorhombic type, in good agreement with the experimental results.
