The effects of pH, adsorbent dosage, co-existing ions, initial Cr(VI) concentration and temperature were investigated systematically by batch experiments. Benefiting from the incorporation of MoS2, the obtained MoS2@PPy composites showed a dramatic increase of specific surface area (149.82 m2·g-1) and adsorption capacity (230.97 mg·g-1) when compared with the pure PPy nanoparticles. Based on the thermodynamics study and X-ray photoelectron spectroscopy analyses, the removal process of Cr(VI) was proved to be exothermic and spontaneous, and accessible under-coordinated Mo(IV) and pyrrolic N groups coupled with redox reactions were conducive to the efficient removal of Cr(VI). The analytical sample included 83 children (60.2% girls) who provided valid accelerometer-measured 24-h MB data during their final year of primary school (T1) and first year of secondary school (T2). Self-reported participation in domain-specific SB and PA, socio-demographic characteristics and weight status were also assessed. Change in 24-h MB composition from T1 to T2 was analysed using a compositional multivariate linear model for repeated measures. The difference in the proportion of meeting the 24-hour integrated movement guidelines was assessed using a McNemar-Bowker test. An unfavourable change was observed in the 24-h MB composition (p  less then  .0001), with increased time spent in SB (+58 min/day) and decreased time in sleentegrated movement guidelines during the primary to secondary school transition period.There was an unfavourable change in the accelerometer-measured 24-hour movement behaviour composition, with increased time spent in sedentary behaviour and decreased time in sleep, light-intensity physical activity and moderate- to vigorous-intensity physical activity. The results indicated that media attention to child sexual abuse increased dramatically after 2013, and a trend of shaping child sexual abuse as a social problem began in 2015. This study also found an interesting cultural difference in the application of framing theory. The Chinese news stories of child sexual abuse were more likely to present individual cause frames but societal solution frames. This apparent inconsistency was not seen in previous research with U.S. media and may be attributable to a cultural preference for victim blame and collective solutions. The findings provide insights into news portrayals of child sexual abuse in a developing society.

  • Glyco-metabolic deteriorations are the most limiting adverse reactions to antipsychotics in the long term. They have been incompletely investigated and the properties of antipsychotics that determine their magnitude are not clarified.To rank antipsychotics by the magnitude of glyco-metabolic alterations and to associate it to their pharmacological and chemical properties, we conducted a network meta-analysis.

Of 2094 articles, 46 were included in network meta-analysis. Study quality was assessed by the RoB 2 and ROBINS-I tools. Mean differences (MD) were obtained by random-effects network meta-analysis; relations between MD and antipsychotic properties were analyzed by linear regressions.     We performed a retrospective chart review of included patients. Univariate and multivariable logistic regression methods were used to evaluate the association between Ct values and CDI severity, 8-week recurrence, and 30-day mortality.

    Among 318 included patients, 51% were male and the mean age was 62 years; ~32% of the patients developed severe CDI and 11% developed severe-complicated CDI. The 30-day all-cause mortality rate was 11% and the 8-week recurrence rate was 9.5%. The overall mean Ct value was 32.9 (range, 23-40). Multivariable analyses showed that lower values of PCR Ct were associated with increased odds of 30-day morality (odds ratio [OR] 0.83; 95% confidence interval [CI], 0.72-0.96) but were not independently associated with CDI severity (OR, 0.99; 95% CI, 0.90-1.09) or recurrence (OR, 0.88; 95% CI, 0.77-1.00).

    Our findings suggest that PCR Ct values at the time of diagnosis may have a limited predictive value and utility in clinical decision making for inpatients with CDI. Larger, prospective studies across different patient populations are needed to confirm our findings.

    This study aims to assess stakeholder perceptions on the challenges and value of real-world evidence (RWE) post approval, the differences in regulatory and health technology assessment (HTA) real-world data (RWD) collection requirements under the German regulation for more safety in drug supply (GSAV), and future alignment opportunities to create a complementary framework for postapproval RWE requirements.

    Eleven semistructured interviews were conducted purposively with pharmaceutical industry experts, regulatory authorities, health technology assessment bodies (HTAbs), and academia. The interview questions focused on the role of RWE post approval, the added value and challenges of RWE, the most important requirements for RWD collection, experience with registries as a source of RWD, perceptions on the GSAV law, RWE requirements in other countries, and the differences between regulatory and HTA requirements and alignment opportunities. The interviews were recorded, transcribed, and translated for coding immon postlicensing evidence generation (PLEG) plans and joint scientific advice to address uncertainties regarding evidence needs and to optimize drug development.

  • There are few data on the prevalence and clinical consequences of coronary artery aneurysms (CAA) in adult patients with Marfan syndrome (MFS).

    We performed a retrospective cohort study of 109 patients with pathogenic variants in the FBN1 gene. Diameters of the left main coronary artery (LMCA) and right coronary artery (RCA) were measured by computed tomography angiography.

    The overall prevalence of CAA was 46%. The prevalence rates of CAA were 18% and 68% in patients with a native aortic root (group 1) and patients with previous aortic root replacement (group 2), respectively. Previous aortic dissection or aortic intervention, longer time from aortic root replacement, higher systemic score, significant mitral valve involvement, and diffuse aortic disease were correlated with CAA. During a mean follow-up of 8.5 ± 7.6 years, four patients developed pseudoaneurysms of the coronary anastomoses requiring surgery.

    CAA are common in adult Marfan patients and are associated with a more severe aortic phenotype and a longer follow-up after aortic root replacement. Our study demonstrates that coronary artery size should be regularly followed, mostly after aortic root replacement and in patients with a severe aortic phenotype. Large multicentre studies are warranted to elucidate the most appropriate surveillance plan.

    There are few data on the prevalence and clinical consequences of coronary artery aneurysms (CAA) in adult patients with Marfan syndrome (MFS). We performed a retrospective cohort study of 109 patients with pathogenic variants in the FBN1 gene. Diameters of the left main coronary artery (LMCA) and right coronary artery (RCA) were measured by computed tomography angiography. The overall prevalence of CAA was 46%. The prevalence rates of CAA were 18% and 68% in patients with a native aortic root (group 1) and patients with previous aortic root replacement (group 2), respectively. Previous aortic dissection or aortic intervention, longer time from aortic root replacement, higher systemic score, significant mitral valve involvement, and diffuse aortic disease were correlated with CAA. During a mean follow-up of 8.5 ± 7.6 years, four patients developed pseudoaneurysms of the coronary anastomoses requiring surgery. CAA are common in adult Marfan patients and are associated with a more severe aortic phenotype and a longer follow-up after aortic root replacement. Our study demonstrates that coronary artery size should be regularly followed, mostly after aortic root replacement and in patients with a severe aortic phenotype. Cultivability (measured at 1.38 MPa and at the harvest osmotic pressure) and glucose consumption compared with the corresponding sudden stress showed that yeasts were able to grow until about 10.5 MPa (0.926 aw ) and to survive until about 58.5 MPa, whereas glucose consumption occurred until 13.5 MPa (about 0.915 aw ). The impact of extended charge-discharge cycling operation on carbon electrodes is investigated using carbon paper as a model electrode. Electrode degradation along with 70 % degradation of charge-discharge capacity was observed after 100 charge-discharge cycles of a single cell vanadium redox flow battery operating at a current density of 80 mA cm-2 at room temperature (23 °C). Raman mapping of the electrodes shows a decrease in structural defects in the negative electrode, and an increase in defects in the positive electrode, indicating differences in the degradation mechanism at each electrode. Electrochemical investigation reveals an increase in the activation overpotential at both the positive and negative electrodes. However, the negative electrode showed a higher activation overpotential indicating a higher impact of electrode degradation on the negative side. X-ray photoelectron spectroscopy shows around an eightfold increase in surface oxygen functional groups after degradation in both positive and negative electrodes. The composition of oxygen functional groups was also observed to change significantly after degradation from dominantly carbonyl-based to a combination of carbonyl- and carboxyl-based groups.   • Seawater desalination plants that use reverse osmosis (RO) membranes have become a core part of social infrastructure, and should be designed to meet the needs of product water quality and production capacity, while considering various environmental factors such as the seawater quality, temperature and geographical features. Furthermore, stable operation while overcoming various problems should be achieved alongside the increasing demands for energy saving and cost reduction. As no universal plant apparatus and operation technology meets these various requirements, the plants need to be customized for individual solutions. This paper reviews and summarizes the proven technologies, including their advantages/disadvantages, and points to cutting-edge technologies related to the design and operation maintenance of seawater intake, pre-treatment and the RO desalination process.Brazilian red propolis (BRP) is a natural product widely known for its phenolic composition and strong antioxidant properties. In this study, we used the Box-Behnken Design (BBD) with Surface Response Methodology to optimize the extraction conditions for total phenolic content (TPC) and Trolox equivalent antioxidant capacity(TEAC) of bioactive phenolics from BRP. The extraction time, ethanol/water concentration and temperature, were tested. All variables had significant effects (p  ≤ 0.05), with a desirability coefficient of 0.88. Under optimized conditions (90% ethanol at 80 °C for 30 min), the BRP extract showed a TPC of 129.00 ± 2.16 mg GAE/g and a TEAC of 3471.76 ± 53.86 µmol TE/g. Moreover, FRAP and ORAC assays revealed that the optimized BRP extract had 1472.86 ± 72.37 µmol Fe2+/g and 4339.61 ± 114.65 µmol TE/gof dry weight, respectively. Thirty-two phenolic compounds were tentatively identified by LC-QTOF-ESI-MS/MS, of which thirteen were found for the first time in BRP, including four flavones, one flavanol, two flavanones, two chalcones, and four isoflavonoids. We show the differentially expressed genes and enriched pathways in DEP-exposed WI-38 cells using RNA sequencing data. We observed a significant increase in single-strand DNA breaks and the frequency of MN in DEP-exposed cells in a dose-dependent manner.     Seawater desalination plants that use reverse osmosis (RO) membranes have become a core part of social infrastructure, and should be designed to meet the needs of product water quality and production capacity, while considering various environmental factors such as the seawater quality, temperature and geographical features. Furthermore, stable operation while overcoming various problems should be achieved alongside the increasing demands for energy saving and cost reduction. As no universal plant apparatus and operation technology meets these various requirements, the plants need to be customized for individual solutions. This paper reviews and summarizes the proven technologies, including their advantages/disadvantages, and points to cutting-edge technologies related to the design and operation maintenance of seawater intake, pre-treatment and the RO desalination process.Brazilian red propolis (BRP) is a natural product widely known for its phenolic composition and strong antioxidant properties. In this study, we used the Box-Behnken Design (BBD) with Surface Response Methodology to optimize the extraction conditions for total phenolic content (TPC) and Trolox equivalent antioxidant capacity(TEAC) of bioactive phenolics from BRP. The extraction time, ethanol/water concentration and temperature, were tested. All variables had significant effects (p  ≤ 0.05), with a desirability coefficient of 0.88. Under optimized conditions (90% ethanol at 80 °C for 30 min), the BRP extract showed a TPC of 129.00 ± 2.16 mg GAE/g and a TEAC of 3471.76 ± 53.86 µmol TE/g. Moreover, FRAP and ORAC assays revealed that the optimized BRP extract had 1472.86 ± 72.37 µmol Fe2+/g and 4339.61 ± 114.65 µmol TE/gof dry weight, respectively. Thirty-two phenolic compounds were tentatively identified by LC-QTOF-ESI-MS/MS, of which thirteen were found for the first time in BRP, including four flavones, one flavanol, two flavanones, two chalcones, and four isoflavonoids.     Recently, the relationships between uncoupling protein-2 (UCP2) -866G/A (rs659366) and Ala55Val (rs660339) polymorphisms and the risk of type 2 diabetes mellitus (T2DM) have been explored considerably, but the results are greatly inconsistent. This meta-analysis was performed to further identify the association of UCP2 rs659366 and rs660339 with the risk of T2DM.

    Eligible studies were searched from PubMed, Embase, Cochrane Library, VIP database, Chinese National Knowledge Infrastructure, and Chinese WanFang database until March 8, 2020. The odds ratios with corresponding 95% confidence intervals (CIs), and P-values were used to assess the strength of the association.

    A total of 26 studies were included in this study. UCP2 rs659366 was associated with the risk of T2DM in allele model (OR 1.112, 95%CI 1.009-1.224, P = 0.032), dominant model (OR 1.189, 95%CI 1.035-1.366, P = 0.014), and heterozygous model (OR 1.177, 95%CI 1.032-1.342, P = .015). A significantly increased risk of T2DM was detected in Asians by UCP2 rs659366 allele (OR 1.132, 95%CI 1.016-1.262, P = .025), dominant (OR 1.218, 95%CI 1.046-1.418, P = .011), homozygous (OR 1.254, 95%CI 1.022-1.540, P = .031) or heterozygous (OR 1.198, 95%CI 1.047-1.371, P = .009) models.     Correction of calcium (Ca), phosphorus (P), and parathyroid hormone (PTH) disorders is the standard of treatment in non-dialysis patients with chronic kidney disease-mineral and bone disorder (CKD-MBD), but the side effects and adverse reactions brought by western medicine (WM) are still the main problems. More importantly, the lack of protection of kidney function in the treatment greatly affects the health of patients. Although traditional Chinese medicine (TCM), specifically tonifying kidney and strengthen bone (TKSB) therapy is wildly applied for patients with CKD-MBD in China, the evidence of TKSB therapy in the treatment of CKD-MBD is limited. Thus, we pretent to conduct this protocol to evaluate the efficacy and safety of TKSB therapy combined with WM for non-dialysis patients with CKD-MBD.

    A system research of randomized controlled trials (RCTs) of TKSB therapy for CKD-MBD will be conducted by 2 investigators from 7 electronic databases. Methodological quality evaluations will be performed by using the Cochrane collaboration tool and data analysis will be conducted by RevMan V5.3 software and STATA v15.0.

    The results of this paper will be submitted to a peer-reviewed journal for publication.

  • This is a commentary on the manuscript by Mpody C, Arends J, Aldrink J, et al., titled "Prognostic Profiling of Children with Serious Post-Operative Complications A Novel Probability Model for Failure to Rescue".
    The aims of this study were to evaluate the long-term skeletodental effects, the volume of maxillary tuberosity, and airway space changes after maxillary molar distalization using modified C-palatal plate (MCPP) in adolescents with Class II malocclusion.

    The sample consisted of 20 adolescent patients (MCPP group; mean age, 12.9±1.0year) who underwent bilateral distalization of their maxillary dentition and 20 subjects as a control group. In the MCPP group, cone-beam computed tomography images were taken before distalization, at the end of the treatment, and during retention with a minimum of a 3-year posttreatment follow up period. Repeated measures ANOVA followed by post-hoc analysis with the Bonferroni test were used to identify significant differences between time points.

    After the long-term observation period, sagittal skeletal and dental relationships were maintained (there were no significant changes in ANB, occlusal plane angle, and overjet postretention). The vertical skeletal dimension did not egative long-term effect on airway space associated with the maxillary arch distalization.     A retrospective review was undertaken of 200 brain magnetic resonance imaging (MRI) examinations in patients aged between 5 months to 16 years. Exclusion criteria included possible causes for lymphadenopathy. Studies were reported previously as normal. Eight regions were assessed for the presence of nodes, short and long axis of the largest node measured, and the ratio was calculated.

    Most commonly identifiable nodes were the deep cervical, submandibular, and posterior cervical in 100%, 99.5%, and 92.5% of studies. In the long axis, the three largest were the submandibular, deep, and posterior cervical with mean values of 19.7, 18.1, and 15.4 mm, respectively.     Hemorrhage is an important cause of preventable injury-related death. Many low- and middle-income country (LMIC) patients do not have timely access to safe blood. We sought to determine the degree of appropriateness of blood transfusion among patients with injuries requiring surgical intervention at presentation to a tertiary hospital in Ghana.

    We performed a retrospective review of such patients presenting to Komfo Anokye Teaching Hospital (KATH), from January 2015 to December 2016. Patients' hemoglobin levels at presentation were determined as the first record of hemoglobin after presentation and their receipt of blood transfusion was determined by explicit documentation in the chart. We defined appropriate blood transfusion practice as patients receiving transfusion when hemoglobin was equal or below a threshold, or patients not being transfused when hemoglobin was above the threshold. We considered both restrictive (hemoglobin ≤7 g/dL) and liberal (hemoglobin ≤10 g/dL) transfusion thresholds.

Currently, stroke care is provided by 13 Comprehensive and 32 Primary Stroke Centres. The paper explains factors that supported the reform implementation, reviews implications, and discusses future challenges. Mandatory reporting of quality indicators, the introduction of a benchmarking system, integration with pre-hospital emergency care, and the introduction of countrywide patient triage have supported more timely treatment for stroke patients and better quality of care. Data from the Stroke Care Quality Indicators of the Czech Stroke Society show positive trends in many areas the number of patients treated with intravenous thrombolysis quadrupled in eight years, with 26.4 % of all acute stroke patients receiving thrombolysis in 2018. Czech Republic now ranks third in Europe in the number of thrombolysis per population and second in the number of mechanical thrombectomies per population.     This is a commentary on the manuscript by Mpody C, Arends J, Aldrink J, et al., titled "Prognostic Profiling of Children with Serious Post-Operative Complications A Novel Probability Model for Failure to Rescue". The aims of this study were to evaluate the long-term skeletodental effects, the volume of maxillary tuberosity, and airway space changes after maxillary molar distalization using modified C-palatal plate (MCPP) in adolescents with Class II malocclusion. The sample consisted of 20 adolescent patients (MCPP group; mean age, 12.9±1.0year) who underwent bilateral distalization of their maxillary dentition and 20 subjects as a control group. In the MCPP group, cone-beam computed tomography images were taken before distalization, at the end of the treatment, and during retention with a minimum of a 3-year posttreatment follow up period. Repeated measures ANOVA followed by post-hoc analysis with the Bonferroni test were used to identify significant differences between time points. After the long-term observation period, sagittal skeletal and dental relationships were maintained (there were no significant changes in ANB, occlusal plane angle, and overjet postretention). We sought to determine the degree of appropriateness of blood transfusion among patients with injuries requiring surgical intervention at presentation to a tertiary hospital in Ghana. We performed a retrospective review of such patients presenting to Komfo Anokye Teaching Hospital (KATH), from January 2015 to December 2016. Patients' hemoglobin levels at presentation were determined as the first record of hemoglobin after presentation and their receipt of blood transfusion was determined by explicit documentation in the chart. We defined appropriate blood transfusion practice as patients receiving transfusion when hemoglobin was equal or below a threshold, or patients not being transfused when hemoglobin was above the threshold. We considered both restrictive (hemoglobin ≤7 g/dL) and liberal (hemoglobin ≤10 g/dL) transfusion thresholds. Currently, stroke care is provided by 13 Comprehensive and 32 Primary Stroke Centres. The paper explains factors that supported the reform implementation, reviews implications, and discusses future challenges. Mandatory reporting of quality indicators, the introduction of a benchmarking system, integration with pre-hospital emergency care, and the introduction of countrywide patient triage have supported more timely treatment for stroke patients and better quality of care. Data from the Stroke Care Quality Indicators of the Czech Stroke Society show positive trends in many areas the number of patients treated with intravenous thrombolysis quadrupled in eight years, with 26.4 % of all acute stroke patients receiving thrombolysis in 2018. Czech Republic now ranks third in Europe in the number of thrombolysis per population and second in the number of mechanical thrombectomies per population. The Czech experience provides an example of positive outcomes of concentrated stroke care, while highlighting the importance of proper implementation processes.   • The hypothalamic-pituitary-adrenal axis is the primary neuroendocrine system activated to re-establish homeostasis during periods of stress, including critical illness and major surgery. During critical illness, evidence suggests that locally induced inflammation of the adrenal gland could facilitate immune-adrenal cross-talk and, in turn, modulate cortisol secretion. It has been hypothesized that immune cells are necessary to mediate the effect of inflammatory stimuli on the steroidogenic pathway that has been observed in vivo. To test this hypothesis, we developed and characterized a trans-well co-culture model of THP1 (human monocytic cell)-derived macrophages and ATC7 murine zona fasciculata adrenocortical cells. We found that co-culture of ATC7 and THP1 cells results in a significant increase in the basal levels of IL-6 mRNA in ATC7 cells, and this effect was potentiated by treatment with LPS. Addition of LPS to co-cultures of ATC7 and THP1 significantly decreased the expression of key adrenal steroidogenic enzymes (including StAR and DAX-1), and this was also found in ATC7 cells treated with pro-inflammatory cytokines. Moreover, 24-h treatment with the synthetic glucocorticoid dexamethasone prevented the effects of LPS stimulation on IL-6, StAR and DAX-1 mRNA in ATC7 cells co-cultured with THP1 cells. Our data suggest that the expression of IL-6 and steroidogenic genes in response to LPS depends on the activation of intra-adrenal immune cells. Additionally, expression of GR-α, HDAC2, BNDF, TrkB, and C-Cbl were significantly increased by induction with NMDA, and this effect was reversed by ACTH and/or MT treatment. Hence, our data suggest that combined ACTH and MT treatment is effective for reducing the number of spasms and increasing the latency period in NMDA rats, by restoring dysregulation of the HPA axis. Spatial learning and memory performance was mostly unimpaired or only slightly impaired in our brain-lesioned animals compared to sham-lesioned control ****. However, contralaterally lesioned **** were significantly impaired during the early phase of reversal learning, whereas the other lesion groups performed similar to controls. Here, in n = 297 individuals, we demonstrate how item response theory (IRT), which provides information on psychometric functioning at both the item and test level, can be used to select negative and positive pictures for eliciting the late positive potential (LPP), a measure of emotional attention. We present results for 50 negative and 50 positive pictures, and show how pictures with higher discrimination values improve differentiation between individuals with different levels of emotional attention. Moreover, "strong" modulators of the LPP - i.e., erotic and mutilation pictures - provided the most information about individuals with low levels of emotional attention, whereas, "weak" modulators of the LPP - i.e., affiliative and exciting pictures - provided the most information about individuals with high levels of emotional attention. Results demonstrate how IRT can inform emotional picture selection and improve the psychometrics of psychophysiological tasks, which can ultimately increase the replicability of findings based on standardized pictures.

METHODS In this open-label study, 34 patients with allergic rhinoconjunctivitis, with or without asthma, were treated with SCIT using an allergoid for grass pollen or birch or mix trees with an increase in accelerated induction dose comprising only 3 injections, one per week, compared to a conventional induction pattern in five injections (once a week). Safety determination was assessed by evaluating local and systemic adverse events. Tolerability was evaluated by patients and physicians who performed the treatment. RESULTS No treatment-related adverse events were observed in any of the patients undergoing rush SCIT. No local reactions, no systemic reactions of any degree (WAO Grade) have been observed. Tolerability has always been rated as very good by both patients and physician. CONCLUSIONS The induction phase, needed to achieve the monthly maintenance dose for a pollen extract, can be greatly accelerated, ensuring a tolerability comparable to that of the conventional schedule.

    Retrospective chart review performed on 56 consecutive adult patients who received intravenous insulin infusion for persistent hyperglycemia in the ICU at Saint Vincent Hospital, a tertiary care community hospital an urban setting in Northeast region of USA. The frequency of fingerstick blood glucose (FSBG) readings was reviewed for compliance with hourly FSBG monitoring per protocol and the impact of FSBG testing at different time intervals on the glycemic status. Depending on time interval of FSBG monitoring, the data was divided into three groups Group A (<90 min), Group B (91-179 min) and GroG monitoring. The compliance to hourly blood glucose monitoring and ICU was variable, and hypoglycemic episodes were similar across the groups despite the variation in monitoring.From February 2017 to December 2018, 20 patients had undergone the proposed modified Wilson-SERI osteotomy technique, for moderate hallux valgus. The mean age of patients was 58,25 years (range 19 to 78). The hallux valgus angle (HVA), the intermetatarsal angle between first and second metatarsal bone (IMA) and the distal metatarsal articular angle (D.M.A.A) were measured. The feet were assessed based on the scoring system used by Broughton and Winson and by the American Orthopedic Foot and Ankle Society (AOFAS) hallux-metatarsophalangeal-interphalangeal scale. All twenty one patients were followed up postoperatively for a minimum of 12 months. The mean HVA angle decreased significantly from 31,1° before surgery (range 22.9°-40°SD 5.0) at 11,2° (range 2.5° to 22.0°SD 5.3) at twelve months follow up. The mean IMA angle decreased significantly from 12,5° (range 8.0°-18.6°SD 3.8) before surgery at 7,4° (range 3.4°-14.0°SD 2.5) at twelve months follow up. The mean DMMA angle decreased significantly from 15.1° (range 5.3° to 20.0°SD 4.4) before surgery at 7,4 °(1.5°- 10.7°SD 2.5) at twelve months follow up. We evaluated the compliance of hourly BG monitoring and relation of less frequent BG monitoring to glycemic status. Retrospective chart review performed on 56 consecutive adult patients who received intravenous insulin infusion for persistent hyperglycemia in the ICU at Saint Vincent Hospital, a tertiary care community hospital an urban setting in Northeast region of USA. The frequency of fingerstick blood glucose (FSBG) readings was reviewed for compliance with hourly FSBG monitoring per protocol and the impact of FSBG testing at different time intervals on the glycemic status. Depending on time interval of FSBG monitoring, the data was divided into three groups Group A (<90 min), Group B (91-179 min) and GroG monitoring. The compliance to hourly blood glucose monitoring and ICU was variable, and hypoglycemic episodes were similar across the groups despite the variation in monitoring.From February 2017 to December 2018, 20 patients had undergone the proposed modified Wilson-SERI osteotomy technique, for moderate hallux valgus. The mean age of patients was 58,25 years (range 19 to 78). The hallux valgus angle (HVA), the intermetatarsal angle between first and second metatarsal bone (IMA) and the distal metatarsal articular angle (D.M.A.A) were measured. The feet were assessed based on the scoring system used by Broughton and Winson and by the American Orthopedic Foot and Ankle Society (AOFAS) hallux-metatarsophalangeal-interphalangeal scale. All twenty one patients were followed up postoperatively for a minimum of 12 months. The mean HVA angle decreased significantly from 31,1° before surgery (range 22.9°-40°SD 5.0) at 11,2° (range 2.5° to 22.0°SD 5.3) at twelve months follow up. The mean IMA angle decreased significantly from 12,5° (range 8.0°-18.6°SD 3.8) before surgery at 7,4° (range 3.4°-14.0°SD 2.5) at twelve months follow up. The mean DMMA angle decreased significantly from 15.1° (range 5.3° to 20.0°SD 4.4) before surgery at 7,4 °(1.5°- 10.7°SD 2.5) at twelve months follow up. The mean score according to the AOFAS forefoot was increased from 22,1 (range 13-30 SD 5.0) to 88,2 (Range 77-96 SD 5.2) (p less then 0.0001). No complications, like dislocations, avascular necrosis of the first metatarsal and deep venous thrombosis, were observed in the post-operative period. Short term results at twelve months after surgery are quite satisfactory but further studies are necessary, to better comprehend an overall outcome of such approach in the long run.
