Furthermore, we introduce a simple but very effective center prior in designing the learning cost function of the DNN by attaching high importance to the errors around the image center. We also present extensive experimental results on four commonly used public databases to demonstrate the superiority of the proposed method over classical and state-of-the-art methods on various evaluation metrics.Recent progress in vision-based fire detection is driven by convolutional neural networks. However, the existing methods fail to achieve a good tradeoff among accuracy, model size, and speed. In this paper, we propose an accurate fire detection method that achieves a better balance in the abovementioned aspects. Specifically, a multiscale feature extraction mechanism is employed to capture richer spatial details, which can enhance the discriminative ability of fire-like objects. Then, the implicit deep supervision mechanism is utilized to enhance the interaction among information flows through dense skip connections. Finally, a channel attention mechanism is employed to selectively emphasize the contribution between different feature maps. Experimental results demonstrate that our method achieves 95.3% accuracy, which outperforms the suboptimal method by 2.5%. Moreover, the speed and model size of our method are 3.76% faster on the GPU and 63.64% smaller than the suboptimal method, respectively.The goal of our work is to discover dominant objects in a very general setting where only a single unlabeled image is given. This is far more challenge than typical colocalization or weakly-supervised localization tasks. To tackle this problem, we propose a simple but effective pattern mining-based method, called Object Location Mining (OLM), which exploits the advantages of data mining and feature representation of pretrained convolutional neural networks (CNNs). Specifically, we first convert the feature maps from a pre-trained CNN model into a set of transactions, and then discovers frequent patterns from transaction database through pattern mining techniques. We observe that those discovered patterns, i.e., co-occurrence highlighted regions, typically hold appearance and spatial consistency. Motivated by this observation, we can easily discover and localize possible objects by merging relevant meaningful patterns. Extensive experiments on a variety of benchmarks demonstrate that OLM achieves competitive localization performance compared with the state-of-the-art methods. We also evaluate our approach compared with unsupervised saliency detection methods and achieves competitive results on seven benchmark datasets. In this paper, the ULA-OP 256 implementation of a novel ultrasound modality, called virtual real-time (VRT), is described. This work started from the examination of effective piezoelectricity of commercial structural adhesives/sealants, and five of them were found to possess effective piezoelectric property, with effective piezoelectric coefficient d33 from -0.11 to -1.77 pm/V depending on frequency under substrate clamping condition. With stable piezoelectric response at least up to MHz, an epoxy adhesive with inorganic filler was selected for SHM feasibility demonstration via generating or sensing guided ultrasonic Lamb waves. The presence of disbond in the adhesive joint is detectable by comparing the Lamb waves signal with a reference baseline signal associated with an intact structure. The results show that the selected adhesive with piezoelectric response can perform the dual roles of structural bonding and ultrasonic joint integrity monitoring.Ultrasonography and photoacoustic tomography provide complementary contrasts in preclinical studies, disease diagnoses, and imaging-guided interventional procedures. Here, we present a video-rate (20 Hz) dual-modality ultrasound and photoacoustic tomographic platform that has a high resolution, rich contrasts, deep penetration, and wide field of view. A three-quarter ring-array ultrasonic transducer is used for both ultrasound and photoacoustic imaging. Plane-wave transmission/receiving approach is used for ultrasound imaging, which improves the imaging speed by nearly two folds and reduces the RF data size compared with the sequential single-channel scanning approach. GPU-based image reconstruction is developed to advance computational speed. We demonstrate fast dual-modality imaging in phantom, mouse, and human finger joint experiments. The results show respiration motion, heart beating, and detailed features in the mouse internal organs. To our knowledge, this is the first report on fast plane-wave ultrasound imaging and single-shot photoacoustic computed tomography in a ring-array system.

Cases with predominant single-cell pattern, clinically suspicious of carcinoma lung were segregated. These were de-stained and immunocytochemistry (ICC) for TTF-1 was performed.
Of 103 cases reported as either suspicious or positive for malignancy on PFC, 29 had a predominant single-cell pattern. Of these, 13 (44.8%) were primary lung carcinoma. The rest were metastasis from ovary (5; 17.2%), breast (2; 6.9%), stomach (2; 6.9%), lymphoma (1; 3.5%), and Ewing's sarcoma (1; 3.5%). Five (17.2%) were those with unknown primary. All cases of lung carcinoma were positive for TTF-1 ICC.

There is no definitive explanation for this stability due to the limited understanding of the structure of LiPON. Herein, we provide a structural model of RF-sputtered LiPON. Information about the short-range structure results from 1D and 2D solid-state NMR experiments. These results are compared with first principles chemical shielding calculations of Li-P-O/N crystals and ab initio molecular dynamics-generated amorphous LiPON models to unequivocally identify the glassy structure as primarily isolated phosphate monomers with N incorporated in both apical and as bridging sites in phosphate dimers. Structural results suggest LiPON's stability is a result of its glassy character. Free-standing LiPON films are produced that exhibit a high degree of flexibility, highlighting the unique mechanical properties of glassy materials.

Systematic review.
A search was conducted on PubMed, EMBASE, the Cochrane Library, and Web of Science to identify measurement methods for dysarthria severity in adults after stroke.
Two reviewers independently reviewed articles and came to a consensus about which ones to include. The authors excluded all duplicates, articles involving individuals with aphasia or other speech problems other than dysarthria, and articles unrelated to stroke. Articles were included if diagnostic measures were used to examine the effectiveness of speech rehabilitation in stroke patients.
The search identified 1154 articles with the keywords "stroke" OR "ictus" OR "cerebral vascular accident" AND "dysarthria" OR "Speech and Language Disorders" AND "diagnosis" OR "assessment." The reviewers analyzed 86 full texts. There were 37 publications that met the criteria and were included in the systematic review. These articles were used to describe the main methods used for measuring the severity of stroke-related dysarthria before and after speech rehabilitation.

Open thoracotomy allows palpation of nodules not identified on imaging but the impact on survival is unknown. The objective of this study was to compare overall survival (OS) and pulmonary disease-free survival (DFS) in children who underwent thoracotomy vs thoracoscopic surgery for pulmonary metastasectomy. A multi-institutional collaborative group retrospectively reviewed 202 pediatric patients with osteosarcoma who underwent pulmonary metastasectomy by thoracotomy (n = 154) or thoracoscopy (n = 48). Results were analyzed by Kaplan-Meier survival estimates and multivariate Cox proportional hazard regression models. With median follow-up of 45 months, 135 (67.5%) patients had a pulmonary relapse and 95 (47%) patients were deceased. Kaplan-Meier analysis showed no significant difference in 5-year pulmonary DFS (25% vs 38%; P = .18) or OS (49% vs 42%, P = .37) between the surgical approaches of thoracotomy and thoracoscopy. In Cox regression analysis controlling for other factors impacting outcome, there was a significantly increased risk of mortality (HR 2.11; P = .027; 95% CI 1.09-4.09) but not pulmonary recurrence (HR 0.96; P = .90; 95% CI 0.52-1.79) with a thoracoscopic approach. However, in the subset analysis limited to patients with oligometastatic disease, thoracoscopy had no increased risk of mortality (HR 1.16; P = .62; 0.64-2.11). In conclusion, patients with metastatic osteosarcoma and limited pulmonary disease burden demonstrate comparable outcomes after thoracotomy and thoracoscopy for metastasectomy. While significant selection bias in these surgical cohorts limits the generalizability of the conclusions, clinical equipoise for a randomized clinical trial in patients with oligometastatic disease is supported.

Out of a total of 43 patients, 14 were initially provided with remote rehabilitation and 29 with direct rehabilitation. Four patients were switched from direct to remote rehabilitation during the study, thus at the end of the study there were 18 in the remote rehabilitation group and 25 in the direct rehabilitation group. Patients in remote rehabilitation were significantly younger than those in direct rehabilitation. Of 12 patients who required intubation, 3 were given remote rehabilitation. One extracorporeal membrane oxygenation survivor underwent direct rehabilitation. All patients on remote rehabilitation were discharged home or to a hotel. Twelve out of 29 patients were transferred to a rehabilitation hospital due to delayed recovery of activities of daily living. No serious adverse events occurred.

Resistin is a cysteine-rich adipokine that promotes the release of inflammatory cytokines, particularly interleukin 1 beta (IL-1β) and tumor necrosis factor alpha (TNF-α), which are critical pro-inflammatory mediators in osteoarthritis (OA) pathogenesis. We describe evidence of significantly higher levels of resistin, IL-1β, and TNF-α expression in OA knee synovial tissue compared with that from non-OA knees. Resistin-induced enhancement of IL-1β and TNF-α expression in human OA synovial fibroblasts (OASFs) were attenuated by MEK and ERK inhibitors, as well as their respective siRNAs. Our data reveal that resistin enhances the expression of TNF-α and IL-1β in OASFs by inhibiting miR-149 expression via MEK and ERK signaling. Our findings elucidate the inter-relationships between resistin and pro-inflammatory mediators during OA pathogenesis and could help to facilitate the development of synovium-targeted therapy in OA.Fractures in the pediatric population are less common than in adults. This paper reports a rare case of a traumatic fracture of the mandible in a 4-year-old child with osteogenesis imperfecta type I. This is an unusual complication in children, even in this group of patients. Open reduction and rigid fixation are not often described as the best treatment. As there are controversies about the subject, a literature review and discussion are presented.Many opioid control policies target the prescribing behavior of health care providers. In this paper, we study the first comprehensive state-level policy requiring providers to access patients' opioid history before making prescribing decisions. We compare prescribers in Kentucky, which implemented this policy in 2012, to those in a control state, Indiana. Our main difference-in-differences analysis uses the universe of prescriptions filled in the two states to assess how the information provided affected prescribing behavior. We find that a significant share of low-volume providers stopped prescribing opioids altogether after the policy was implemented, though this change accounted for a small share of the reduction in total volume. The most important margin of response was to prescribe opioids to fewer patients. Although providers disproportionately discontinued treating patients whose opioid histories showed the use of multiple providers, there were also economically meaningful reductions for patients without multiple providers and single-use acute patients.

We compare UK response and outcomes with European countries of similar income and healthcare resources. We calibrate estimates of the economic costs as different % loss in Gross Domestic Product (GDP) against possible benefits of avoiding life years lost, for different scenarios where current COVID-19 mortality and comorbidity rates were used to calculate the loss in life expectancy and adjusted for their levels of poor health and quality of life. We then apply a quality-adjusted life years (QALY) value of £30,000 (maximum under national guidelines).

There was less variation in the trajectory of mortality rates and excess deaths, which have fallen across all countries durinid easing in restrictions is now warranted.

An electronic search was undertaken in the subsequent databases PubMed, Web of Science, Scopus, EMBASE, and LILACS. All papers included were case reports or case series. Absolute and relative frequencies were obtained by descriptive analysis. Overall survival was defined by the Kaplan-Meier method.
This systematic review identified 217 studies from 33 countries, comprising 348 cases. Male individuals were more affected (n=199/57.68%) in the 5th to 7th decades of life. The most usual primary site was the lung in men (n=49/14.08%) and the breast in women (n=40/11.49%). Jawbones were affected in 183 cases (53.19%), with the posterior mandible (n=96/66.66%) representing the preferred site. In soft tissues, the gingiva (n=80/23.25%) was more affected. Clinical presentation was a nodular mass with or without ulceration (n=280/85.89%). Imaginological examinations revealed radiolucent/hypodense lesions (n=115/59.28%). The mean survival time was eight months.

In December 2019, a novel coronavirus emerged in Wuhan City, and a retrospective analysis is necessary to provide clinicians with the characteristics of traumatic dental injuries (TDIs) during the epidemic. The aim of this study was to evaluate the changes in the characteristics of TDIs under the transmission control measures in Wuhan City utilizing an epidemiologic investigation.
In this retrospective study, epidemiologic information, including the number of patients, gender, age, and TDI parameters such as time since injury to the clinic visit, etiology, tooth location, and the type of injury was extracted from the records of patients in the hospital from two periods period 1 (between January 23, 2020, and April 7, 2020) and period 2 (between January 23, 2019, and April 7, 2019). The data from the two periods were compared and analyzed.

The ototoxic effects of aminoglycosides are well known. Gentamicin carries a substantial risk of hearing loss. Gentamicin is widely used to combat life-threatening infections, despite its ototoxic effects. Calcium dobesilate is a pharmacologically active agent used to treat many disorders due to its vasoprotective and antioxidant effects. We investigated the therapeutic role of calcium dobesilate against gentamicin-induced cochlear nerve ototoxicity in an animal model.
Thirty-two Sprague Dawley rats were divided into four groups Gentamicin, Gentamicin + Calcium Dobesilate, Calcium Dobesilate, and Control. Preoperative and postoperative hearing thresholds were determined using auditory brainstem response thresholds with click and 16-kHz tone-burst stimuli. Histological analysis of the tympanic bulla specimens was performed under light and transmission electron microscopy. The histological findings were subjected to semiquantitative grading, of which the results were compared between the groups.
Gentamicin + Calcium Dobesilate group had, on average, 27 dB better click-evoked hearing than Gentamicin group (p < 0.01), whereas the difference was not significant with 16-kHz tone-burst stimuli (p > 0.01). Histologically examining the Control and Calcium Dobesilate groups revealed normal ultrastructural appearances. The Gentamicin group showed the most severe histological alterations including myelin destruction, total axonal degeneration, and edema. The histological evidence of damage was significantly reduced in the Gentamicin + Calcium Dobesilate group compared with the Gentamicin group.
Adding oral calcium dobesilate to systemic gentamicin was demonstrated to exert beneficial effects on click-evoked hearing thresholds, as supported by the histological findings.

Complex bony orbital defects are reconstructively challenging due to loss of intraoperative anatomical landmarks and adjacent support. Presized and precontoured porous polyethylene-titanium implants (Medpor Titan 3D Orbital Floor Implant) are designed to reestablish normal orbital floor and medial wall anatomy and are modeled after anatomically averaged orbits. This is the first study to report clinical outcomes with this implant.
This retrospective case series reviewed clinical data and outcomes for patients undergoing orbital reconstruction with a presized and precontoured porous polyethylene-titanium orbital implant from January 2016 to June 2018.
A total of 34 orbits of 33 patients were identified (mean age 43 ± 16 years, 70% men). Most bony defects were a result of trauma and included large orbital floor deformities (100%), medial wall defects (74%), disrupted inferomedial struts (68%), and broken posterior ledges (82%). Symptomatic diplopia (73%) and enophthalmos (89%, mean 3.7 ± 2.1 mm) were common preoperatively. Many cases were revisions (44%). Mean follow up was 7.8 ± 6.7 months. All patients had improved globe positioning, enophthalmos, and hypoglobus. Seven patients had persistent postoperative diplopia 6 responded to prism therapy and 1 required strabismus surgery. One patient required retrobulbar hematoma drainage and 1 patient required implant explantation due to chronic infection.

Preoperative and postoperative hearing thresholds were determined using auditory brainstem response thresholds with click and 16-kHz tone-burst stimuli. Histological analysis of the tympanic bulla specimens was performed under light and transmission electron microscopy. The histological findings were subjected to semiquantitative grading, of which the results were compared between the groups. Gentamicin + Calcium Dobesilate group had, on average, 27 dB better click-evoked hearing than Gentamicin group (p < 0.01), whereas the difference was not significant with 16-kHz tone-burst stimuli (p > 0.01). Histologically examining the Control and Calcium Dobesilate groups revealed normal ultrastructural appearances. The Gentamicin group showed the most severe histological alterations including myelin destruction, total axonal degeneration, and edema. The histological evidence of damage was significantly reduced in the Gentamicin + Calcium Dobesilate group compared with the Gentamicin group. Adding oral calcium dobesilate to systemic gentamicin was demonstrated to exert beneficial effects on click-evoked hearing thresholds, as supported by the histological findings. Complex bony orbital defects are reconstructively challenging due to loss of intraoperative anatomical landmarks and adjacent support. Presized and precontoured porous polyethylene-titanium implants (Medpor Titan 3D Orbital Floor Implant) are designed to reestablish normal orbital floor and medial wall anatomy and are modeled after anatomically averaged orbits. This is the first study to report clinical outcomes with this implant. This retrospective case series reviewed clinical data and outcomes for patients undergoing orbital reconstruction with a presized and precontoured porous polyethylene-titanium orbital implant from January 2016 to June 2018. A total of 34 orbits of 33 patients were identified (mean age 43 ± 16 years, 70% men). Most bony defects were a result of trauma and included large orbital floor deformities (100%), medial wall defects (74%), disrupted inferomedial struts (68%), and broken posterior ledges (82%). Symptomatic diplopia (73%) and enophthalmos (89%, mean 3.7 ± 2.1 mm) were common preoperatively. Many cases were revisions (44%). Mean follow up was 7.8 ± 6.7 months. All patients had improved globe positioning, enophthalmos, and hypoglobus. Seven patients had persistent postoperative diplopia 6 responded to prism therapy and 1 required strabismus surgery. One patient required retrobulbar hematoma drainage and 1 patient required implant explantation due to chronic infection. Commercially available presized and precon-toured porous polyethylene-titanium implants are useful for complex orbital bony defects and can achieve functional improvements in diplopia, enophthalmos, and extraocular motility with a low incidence of postoperative complications or revisional surgery. The authors present a case of a benign hidradenoma arising from the eyelid, presenting with rapid growth, ulceration, and bleeding, suggestive of a malignant lesion.Isolated orbital periphlebitis in the context of nonspecific orbital inflammation is rare. This case series describes 3 patients with periphlebitis of the superior ophthalmic vein, 2 of which were confirmed with pathology. All 3 cases had a history of Graves hyperthyroidism. Superior ophthalmic vein periphlebitis is a rare form of orbital inflammation presenting with proptosis and motility restriction yet few inflammatory signs. It may be associated with autoimmune hyperthyroidism and represent a rare feature of thyroid eye disease, or perhaps could be an overlap syndrome between thyroid eye disease and nonspecific orbital inflammation. However, discovery of new products suffers from a lack of high-throughput analytical techniques. Post-data process techniques have become popular tools for natural product confirmations and mycotoxin family analysis. In this work, a visible post-data process procedure with MZmine, GNPS, and Xcalibur was used for efficient analysis of high-resolution mass spectrometry. The constructed biocatalytic system converted 300 mM (83.9 g L-1) and 154 mM (43.5 g L-1) oleic acid in the olive oil hydrolysate obtained after resin extraction, which were derived from 110 and 54 g L-1 olive oil, respectively, into 202 mM (35.2 g L-1) ω-hydroxynonanoic acid and 103 mM (19.4 g L-1) α,ω-nonanedioic acid, which are 21- and 24-fold higher values than the previously reported results, respectively. To supply more chorismate, we introduced the shikimate gene module construct and selected the best construct for increased yields.

Results A total of 47142 eyes of 23571 persons of all age groups (range 5 years to 70 years) with approximate equal gender distribution attending the mega eye camp were examined. The most common ocular morbidity noticed after ocular examination was refractive error with underlying cause majorly as presbyopia, followed by cataract, after cataract, convergence insufficiency, lids, and adnexal disease, etc. Conclusions This study highlights the changing pattern of common ocular morbidities presently among middle and elderly populations in developing countries like India. Results The sociodemographic characteristics presented no significant differences between bruxers and non-bruxers (P > 0.005). There was a significant difference in QoL between the case and control group ( less then 0.001). Bruxers had worse QoL than controls. Conclusion Sleep bruxism may be associated with a negative impact on QoL. Eugenol was taken as marker compound. Methods Nitric oxide, Hydroxyl radical and Reducing power assay methods were carried out for assessment of antioxidant activity of Piper betel. Results The antioxidant activity for Nitric oxide, Hydroxyl radical and Reducing power assay at 1000 to 62.5μg/ml was performed. The antioxidant activity of Piper betel leaf extract exhibited the IC50 value for Nitric oxide and Hydroxyl radical >1000 whereas Eugenol exhibited the IC50 value 114.34± 0.46 and 306.44 ± 5.28 respectively, for reducing power assay (RPA) Piper betel leaf extract and Eugenol revealed the RPA value ranging from 0.44-0.08 and 0.53-0.12. Conclusion The benefits of Piper betel have been mentioned in our ancient texts. Keeping in view the emergence of various diseases and the benefits of Piper betei, there is need that every effort should be made to revive this treasure of nature into our daily supplement. Data collection was conducted through self-administered questionnaires. Chi-square test was performed to determine the significant differences between variables. A P value of less then 0.05 was considered statistically significant. Result Of 480 participants, 247 (51.5%) of them were male. The educational level of most was bachelor's degree (253; 52.7%). Most of the participants (326; 67.9%) are not aware about BMI. However, 80 (16.7%) participants knew the true answer to obese BMI. Of the total participants, 283 (59.0%) did not knew about sleeve gastrectomy indications; however, 311 (64.8%) of the participants had heard about the complications of sleeve gastrectomy. All these results are correlated with the educational level of the participants. Conclusion Our study shows a lack of awareness of sleeve gastrectomy indications and complications among study population. We need to increase public awareness about sleeve gastrectomy indications and complication by proper scientific health education in the community. In the United Arab Emirates, around 15.7% adult males and 2.4% adult females are current tobacco smokers. Primary healthcare clinics (PHCs) have an important role in battling the smoking epidemic by raising awareness among patients attending them daily. The aim of our study was to assess the knowledge, attitude, and practice regarding smoking among patients attending PHCs. Methods A cross-sectional descriptive study was conducted on patients attending primary healthcare clinics in Dubai. Data was collected using a pretested self-administered questionnaire developed by the researchers and was adopted from the Global Adult Tobacco Survey. Results Among a total of 500 patients, around 66.4% were never smokers, 23.6% were current smokers, and 10.0% were ex-smokers. https://www.selleckchem.com/products/frax597.html The age of first time smoking among current smokers was the highest between 18 and 19 years, with friends being the source of first attempt of smoking (53.4%). Around 53.4% of the current smokers had poor knowledge, while most of never and ex-smokers had a good knowledge level (47.9%, 70.0%), respectively (P value for both less then 0.05). Majority (71.2%) of current smokers had negative attitude toward anti-smoking statements, while never and ex-smokers had a positive attitude (54.5% and 58.0%), respectively (P value less then 0.05). Conclusion The majority of never and ex-smokers had good knowledge level and positive attitude toward anti-smoking statements; however, poor knowledge level and negative attitude was found more among current smokers. Handgrip measurement is simple but also a valid measure of overall muscle strength and also provides an objective index of functional integrity of upper extremity. Aims To provide population-based HGS reference values for Nepalese adults from 19 to 70 years of age. Settings and Design A cross-sectional study was conducted in Dhulikhel community among 526 participants. Methods and Materials Jamar Dynamometer was used for measuring HGS based on the recommendation provided by the American Society of Hand Therapists. Statistical Analysis Used Data were analyzed using STATA version 14. Results Men exhibited higher HGS compared to women with maximum grip strength observed in age group of 19-29 which were 47.24 kg and 32.51 kg for men and women, respectively. HGS decreases with increasing age in both dominant and nondominant hands.

Of the total participants, 283 (59.0%) did not knew about sleeve gastrectomy indications; however, 311 (64.8%) of the participants had heard about the complications of sleeve gastrectomy. All these results are correlated with the educational level of the participants. Conclusion Our study shows a lack of awareness of sleeve gastrectomy indications and complications among study population. We need to increase public awareness about sleeve gastrectomy indications and complication by proper scientific health education in the community. The aim of our study was to assess the knowledge, attitude, and practice regarding smoking among patients attending PHCs. Methods A cross-sectional descriptive study was conducted on patients attending primary healthcare clinics in Dubai. Data was collected using a pretested self-administered questionnaire developed by the researchers and was adopted from the Global Adult Tobacco Survey. Methods and Materials Jamar Dynamometer was used for measuring HGS based on the recommendation provided by the American Society of Hand Therapists. Statistical Analysis Used Data were analyzed using STATA version 14. DC-LL also had the highest metmyoglobin reducing activity and OC among all the samples, thus, the greatest color stability, although very dark throughout storage, with lowest values for lightness (L*) and yellowness (b*). Np-LL also exhibited relatively high color stability, as a result of its lower % MMb and OC and higher MRA than psoas muscle samples. The 0.2 unit difference of the pH between DC and Np psoas muscle, resulted in the difference of the color intensity, not the color stability. Interestingly, high pH psoas muscle (DC-PM) did not have better color stability than Np-PM, and in fact had lower color stability than even Np-LL. To address this, we generated H3K27me3 profiles of Norway spruce (Picea abies) embryonic callus and non-embryogenic callus using CUT&RUN, which is a powerful method for chromatin profiling. While calcineurin signaling plays important roles in pancreatic β-cell survival, proliferation, and function, its effects on human β-cells remain understudied. In particular, we do not understand why some calcineurin inhibitors have more profound effects on the incidence of NODAT. We compared the effects of TAC and VCS on the dynamics of insulin secretory function, programmed cell death rate, and the transcriptomic profile of human islets. We studied 2 clinically relevant doses of TAC (10 ng/mL, 30 ng/mL) and VCS (20 ng/mL, 60 ng/mL), meant to approximate the clinical trough and peak concentrations. TAC, but not VCS, caused a significant impairment of 15 mM glucose-stimulated and 30 mM KCl-stimulated insulin secretion. This points to molecular defects in the distal stages of exocytosis after voltage-gated Ca2+ entry. No significant effects on islet cell survival or total insulin content were identified. RNA sequencing showed that TAC significantly decreased the expression of 17 genes, including direct and indirect regulators of exocytosis (SYT16, TBC1D30, PCK1, SMOC1, SYT5, PDK4, and CREM), whereas VCS has less broad, and milder, effects on gene expression. Clinically relevant doses of TAC, but not VCS, directly inhibit insulin secretion from human islets, likely via transcriptional control of exocytosis machinery.

We tested whether greater variation in red blood cell size, measured by red cell distribution width (RDW), may predict aging-related degenerative conditions and therefore, serve as a marker of biological aging.
3,635 community-dwelling elderly men were enrolled in the prospective Osteoporotic Fractures in Men study (MrOS). RDW was categorized into 4 groups (≤13.0%, 13.1%-14.0%, 14.1%-15.0%, ≥15.1%). Functional limitations, frailty, strength, physical performance and cognitive function were measured at baseline and 7.4 years later. Falls were recorded in the year after baseline; hospitalizations were obtained for two years after baseline. Mortality were assessed during a mean of 8.3 years of follow-up.

The BODE Index is the most recognised mortality risk score in COPD but includes a 6-minute walk test (6MWT) that is seldom available in practise; the BODE Index may be better adopted if the 6MWT was replaced. we investigated whether a modified BODE Index in which 6MWT was replaced by an alternative measure of physical capacity, specifically the short physical performance battery (SPPB) or components, retained its predictive ability for mortality in individuals with COPD. we analysed 630 COPD patients from the ERICA cohort study for whom UK Office for National Statistics verified mortality data were available. Variables tested at baseline included spirometry, 6MWT, SPPB and its components (4-m gait speed test [4MGS], chair stand and balance). Predictive models were developed using stratified multivariable Cox regression, and assessed by C-indices and calibration plots with 10-fold cross-validation and replication. during median 2years of follow-up, 60 (10%) individuals died. We tested whether greater variation in red blood cell size, measured by red cell distribution width (RDW), may predict aging-related degenerative conditions and therefore, serve as a marker of biological aging. 3,635 community-dwelling elderly men were enrolled in the prospective Osteoporotic Fractures in Men study (MrOS). RDW was categorized into 4 groups (≤13.0%, 13.1%-14.0%, 14.1%-15.0%, ≥15.1%). Functional limitations, frailty, strength, physical performance and cognitive function were measured at baseline and 7.4 years later. Falls were recorded in the year after baseline; hospitalizations were obtained for two years after baseline. Mortality were assessed during a mean of 8.3 years of follow-up. Participants with greater variability in red cell size were weaker, walked more slowly, and had worse cognitive function. They were more likely to have functional limitations (35.2% in the highest RDW category vs. 16.0% in the lowest, p & 0.001) and frailty (30.3% vs. 11.3%, p & 0.001).
