It is therefore urgent to review and revise the COVID-19 communication messages currently used in sub-Saharan Africa.The COVID-19 pandemic has resulted in significant morbidity, mortality, and strained healthcare systems worldwide. Thus, a search for modalities that can expedite and improve the diagnosis and management of this entity is underway. Recent data suggested the utility of lung ultrasound (LUS) in the diagnosis of COVID-19 by detecting an interstitial pattern (B-pattern). Hence, we aimed to pool the proportion of various reported lung abnormalities detected by LUS in symptomatic COVID-19 patients. We conducted a systematic review (PubMed, MEDLINE, and EMBASE until April 25, 2020) and a proportion meta-analysis. We included seven studies examining the role of LUS in 122 COVID-19 patients. The pooled proportion (PP) of B-pattern detected by lung ultrasound (US) was 0.97 (95% CI 0.94-1.00 I 2 0%, Q 4.6). The PP of finding pleural line abnormalities was 0.70 (95% CI 0.13-1.00 I 2 96%, Q 103.9), of pleural thickening was 0.54 (95% 0.11-0.95 I 2 93%, Q 61.1), of subpleural or pulmonary consolidation was 0.39 (95% CI 0.21-0.58 I 2 72%, Q 17.8), and of pleural effusion was 0.14 (95% CI 0.00-0.37 I 2 93%, Q 27.3). Our meta-analysis revealed that almost all SARS-CoV-2-infected patients have abnormal lung US. The most common abnormality is interstitial involvement depicted as B-pattern. The finding from our review highlights the potential role of this modality in the triage, diagnosis, and follow-up of COVID-19 patients. A sizable diagnostic accuracy study comparing LUS, computed tomography scan, and COVID-19-specific tests is warranted to further test this finding and to delineate the diagnostic and prognostic yield of each of these modalities.During the early postnatal period, dietary manipulations can alter the developmental trajectory of the growing offspring, causing beneficial or adverse health outcomes later in adult life. We investigated the potential preventive effects of neonatal zingerone intake on the development of fructose-induced metabolic derangements in rats.Four-day old male and female Sprague-Dawley rat pups (n = 79) were randomly grouped and administered 10 ml/kg body weight (bwt) of distilled water (W), 10 ml/kg bwt 20% fructose solution (FS), 10 ml/kg bwt fructose solution + 40 mg/kg bwt of zingerone in distilled water (ZF) or 40 mg/kg bwt of zingerone in distilled water (ZW) pre-weaning. After weaning, W and ZW continued on unlimited tap water, while FS and ZF continued on unlimited fructose solution for 10 weeks. Body mass and food and fluid intake were evaluated, plasma was collected for metabolic assays and visceral fat was quantified.Food intake was decreased, fructose and overall caloric intake were increased due to fructose feeding in both sexes (P 0.05). Inspiratory and expiratory muscle strength were inversely associated with frailty and pre-frailty. Cut-off points ≥-50cmH2O and ≤60cmH2O for maximum inspiratory and expiratory pressures, respectively, were established as optimal discriminators of frailty. The cut-off point ≤65cmH2O for maximum expiratory pressure was established as a discriminant for the presence of pre-frailty. Conclusions Inspiratory and expiratory muscle strength were lower in frail than in pre-frail older adults, and lower in pre-frail than in non-frail peers. Frailty and pre-frailty were inversely associated with inspiratory and expiratory muscle strength. Cut-off points for inspiratory and expiratory muscle strength may be useful in clinical practice for discriminating frailty and pre-frailty in older adults.Objectives The aim of this study was to examine changes in the prevalence of loneliness over time from 2011 to 2017 in long-term care facilities; and its related factors. Residents in temporary respite care or with severe cognitive impairment, and those unable or refusing to respond to the loneliness item were excluded. The total number of participants in this analysis was 1563 in 2011, and 1367 in 2017. In both samples, we used the same loneliness measurement by asking "Do you suffer from loneliness?" (never/sometimes/often or always). When comparing the samples in order to reduce the effect of confounding between them, we used propensity score matching. A multivariable logistic regression model explored the relationship between various characteristics and loneliness. Results Loneliness showed no change in prevalence over time propensity score-adjusted loneliness was 36 % in 2011 and 2017. Inspiratory and expiratory muscle strength were inversely associated with frailty and pre-frailty. Cut-off points ≥-50cmH2O and ≤60cmH2O for maximum inspiratory and expiratory pressures, respectively, were established as optimal discriminators of frailty. The cut-off point ≤65cmH2O for maximum expiratory pressure was established as a discriminant for the presence of pre-frailty. Conclusions Inspiratory and expiratory muscle strength were lower in frail than in pre-frail older adults, and lower in pre-frail than in non-frail peers. We used a combination of morphological data (genitalia structure) and a molecular marker (a 658bp fragment of the COI gene) to demonstrate that carpenter moth populations from central and southern Morocco, previously identified as Cossus cossus (Linnaeus, 1758) based on external morphology, represent a new species, described herein as C. romantsovi Yakovlev Shapoval, sp. n. The genetic divergence of the new species with respect to other members of genus Cossus is significant and includes at least 23 fixed nucleotide substitutions in the 658 bp of the COI barcode.Three new Brazilian species of the sharpshooter genus Amblyscarta Stål, 1869 are described and illustrated A. elianeae sp. nov. Cystic echinococcosis (CE) is common in Africa especially in northern and eastern countries where it is highly prevalent and mainly caused by Echinococcus granulosus sensu stricto and Echinococcus granulosus sensu lato (G6/7). In a recent epidemiological survey in Nigeria, the G6/7 genotype was reportedly responsible for the majority of CE infection. In this study, considering the taxonomic challenges of the G6/7 genotype and the limitation of the cox1/nad1 genes in resolving the differences, we sequenced the complete mitochondrial (mt) genome of seven larval isolates of E. granulosus s.l. G6/7 haplotypes recently reported in Nigeria to correctly assign them to either (G6/7) genotype and to understand the mt genome variation with isolates from other geographical regions. On analysis, a total of 13,731 bp in a covalently closed circular molecule were realized. The total mt length was ≥10 bp than previously reported G6 and G7 mt genome sequences. Also, the median-joining network and phylogeny based on the 12 protein-coding sequences correctly identified them as G6 genotype. Since longer mt genome sequences have shown some advantage over individual genes in resolving taxonomic challenges, we confirm that the genotype responsible for the majority of CE infection in livestock in Nigeria is the G6 genotype and the availability of the complete mt genome from different Nigerian intermediate hosts will prove useful in future genetic population studies across the country and the West African sub-region.Background High-power short-duration ablation for pulmonary vein isolation (PVI) in treating atrial fibrillation (AF) has been shown to facilitate the procedure and improve the effectiveness; however, esophageal injury remains one safety concern. Objective To investigate the role of luminal-esophageal-temperature (LET) monitoring during high-power ablation for PVI in terms of endoscopic esophageal lesion. Methods Patients with symptomatic AF underwent ablation-index guided high-power (AI-HP) PVI (50W, AI anterior-wall/posterior-wall 550/400). Objective To investigate 1-year incidence and predictors of GI cancers after GI bleeding among atrial fibrillation (AF) patients treated with warfarin or NOACs. The risks of mortalities after GI cancers between patients receiving warfarin or NOACs were compared. Methods A total of 10,845 anticoagulated AF patients experiencing hospitalizations due to GI bleeding without prior history of GI cancers were identified from the Taiwan National Health Insurance Research Database. Conclusions Incident GI cancers were diagnosed in 1 in 37 AF patients at 1 year after OAC-related GI bleeding, which were more common among patients treated with NOACs (1 in 26) compared to warfarin (1 in 41). Chronic bidirectional block was achieved in 13 of 14 (93%) CTI and 10 of 14 (71%) MI ablations after 1 month follow-up and was consistent with lesion continuity and transmurality upon pathology. The lowest tissue temperature correlated well with the closest distance to the linear cryocatheter (r=0.688; p less then 0.001). Conclusions This linear cryocatheter has created continuous and transmural linear lesions with a "single shot" cryoenergy application and thus has the potential for clinical use in the setting of various arrhythmias.
Cystic echinococcosis (CE) is common in Africa especially in northern and eastern countries where it is highly prevalent and mainly caused by Echinococcus granulosus sensu stricto and Echinococcus granulosus sensu lato (G6/7). In a recent epidemiological survey in Nigeria, the G6/7 genotype was reportedly responsible for the majority of CE infection. In this study, considering the taxonomic challenges of the G6/7 genotype and the limitation of the cox1/nad1 genes in resolving the differences, we sequenced the complete mitochondrial (mt) genome of seven larval isolates of E. granulosus s.l. G6/7 haplotypes recently reported in Nigeria to correctly assign them to either (G6/7) genotype and to understand the mt genome variation with isolates from other geographical regions. On analysis, a total of 13,731 bp in a covalently closed circular molecule were realized. The total mt length was ≥10 bp than previously reported G6 and G7 mt genome sequences. Also, the median-joining network and phylogeny based on the 12 protein-coding sequences correctly identified them as G6 genotype. Since longer mt genome sequences have shown some advantage over individual genes in resolving taxonomic challenges, we confirm that the genotype responsible for the majority of CE infection in livestock in Nigeria is the G6 genotype and the availability of the complete mt genome from different Nigerian intermediate hosts will prove useful in future genetic population studies across the country and the West African sub-region.Background High-power short-duration ablation for pulmonary vein isolation (PVI) in treating atrial fibrillation (AF) has been shown to facilitate the procedure and improve the effectiveness; however, esophageal injury remains one safety concern. The purpose of this study was to examine the effects of internal and external foot rotation on knee, hip, and ankle joint reaction forces during running. Motion capture and force data were recorded of nineteen healthy adults running at 3.5 m/s during three conditions 1) normal, and maximal comfortable 2) internal and 3) external foot rotation. Musculoskeletal simulations were performed using OpenSim and the Rajagopal 2015 model, modified to a two degree of freedom knee joint. Muscle excitations were derived using static optimization, including muscle physiology parameters. Joint reaction forces were obtained by combining inverse kinematics, force data, and muscle excitations. Differences between joint reaction force waveforms were determined using one-way ANOVAs via statistical parametric mapping. Internal and external foot rotation reduced resultant hip and ankle forces throughout the stance phase. Three-dimensional and resultant Knee joint reaction forces only differed at very early and very late stance phase. The results revealed a monotonic increase in activation in the cortical regions of the cognitive control network (CCN) as a function of uncertainty in the visual and auditory modalities. The intrinsic connectivity between the CCN and sensory regions was similar for the visual and auditory modalities. Furthermore, multivariate patterns of activation in the CCN predicted the level of uncertainty within and across stimulus modalities. These findings suggest that the CCN implements cognitive control by processing uncertainty as abstract information independent of stimulus modality.
Left ventricular (LV) mechanical dispersion (MD) may result from heterogeneous electrical conduction and is associated with adverse events. The present study investigated (i) the association between LV MD and the extent of LV scar as assessed with contrast-enhanced cardiac magnetic resonance (CMR) and (ii) the prognostic implications of LV MD in patients after ST-segment elevation myocardial infarction.
LV MD was calculated by echocardiography and myocardial scar was analysed on CMR data retrospectively. Infarct core and border zone were defined as ≥50% and 35-50% of maximal signal intensity, respectively. Patients were followed for the occurrence of the combined endpoint (all-cause mortality and appropriate implantable cardioverter-defibrillator therapy). In total, 96 patients (87% male, 57 ± 10 years) were included. Median LV MD was 53.5 ms [interquartile range (IQR) 43.4-62.8]. On CMR, total scar burden was 11.4% (IQR 3.8-17.1%), infarct core tissue 6.2% (IQR 2.0-12.7%), and border zone was 3.5% (IQR 1.5-5.7%). The purpose of this study was to examine the effects of internal and external foot rotation on knee, hip, and ankle joint reaction forces during running. Motion capture and force data were recorded of nineteen healthy adults running at 3.5 m/s during three conditions 1) normal, and maximal comfortable 2) internal and 3) external foot rotation. Musculoskeletal simulations were performed using OpenSim and the Rajagopal 2015 model, modified to a two degree of freedom knee joint. Muscle excitations were derived using static optimization, including muscle physiology parameters. Joint reaction forces were obtained by combining inverse kinematics, force data, and muscle excitations. Differences between joint reaction force waveforms were determined using one-way ANOVAs via statistical parametric mapping. Internal and external foot rotation reduced resultant hip and ankle forces throughout the stance phase. Three-dimensional and resultant Knee joint reaction forces only differed at very early and very late stance phase. This study used functional magnetic imaging together with visual and auditory perceptual decision-making tasks to examine brain activation as a function of uncertainty in the two stimulus modalities. The results revealed a monotonic increase in activation in the cortical regions of the cognitive control network (CCN) as a function of uncertainty in the visual and auditory modalities. The intrinsic connectivity between the CCN and sensory regions was similar for the visual and auditory modalities. Furthermore, multivariate patterns of activation in the CCN predicted the level of uncertainty within and across stimulus modalities. The present study investigated (i) the association between LV MD and the extent of LV scar as assessed with contrast-enhanced cardiac magnetic resonance (CMR) and (ii) the prognostic implications of LV MD in patients after ST-segment elevation myocardial infarction. LV MD was calculated by echocardiography and myocardial scar was analysed on CMR data retrospectively. Infarct core and border zone were defined as ≥50% and 35-50% of maximal signal intensity, respectively. Electronic correlation energies from the random-phase approximation converge slowly with respect to the plane wave basis set size. We study the conditions under which a short-range local density functional can be used to account for the basis set incompleteness error. Furthermore, we propose a one-shot extrapolation scheme based on the Lindhard response function of the homogeneous electron gas. The different basis set correction methods are used to calculate equilibrium lattice constants for prototypical solids of different bonding types.In this work, the oxygen reduction reaction (ORR) on tellurium-modified Pt(111) surfaces has been studied. Adsorption of Te adatoms on Pt(111) progressively shifts toward less positive values of both the ORR reaction onset and the half-wave potential in 0.1M HClO4 for 0 0.25, the ORR activity increases relative to the one at θTe less then 0.25, but remains lower than that on clean Pt(111). Results were analyzed in light of simulations of kinetic currents as a function of θTe, calculated by employing a simple mean field model including both site blocking and electronic effects. Inside this framework, experimental data are best explained by considering that oxygenated Te species inhibit the ORR by either negatively modifying adsorption energies of reaction intermediates or combined site-blocking and electronic effects. A redox ORR catalysis due to redox properties of Te adatoms is discarded. Contrarily, in 0.05M H2SO4, a positive catalytic effect has been found, interpreted in terms of a competitive adsorption-desorption mechanism involving the replacement of adsorbed sulfate by Te adatoms. Shielding sensitivity to the choice of method approximation and atomic basis sets used has been evaluated. The number of required grid points is reduced by roughly a factor of two in comparison to alternative systematic sampling methods. Instead of performing the direct evaluation of spherical harmonic series expansion, we use Green's second identity to transform the series expansion into a spherical integral, which can be accurately represented by discrete charges on the sphere. Therefore, the fast multipole method can be readily employed to sum over all charges within and on the sphere, achieving truly linear O(N) complexity. Objective Recent development is reviewed in biomarkers of inflammation and immune activation in risk stratification of bladder cancer (**). Methods PubMed, Wiley Online Library, and Science Direct databases were reviewed in November 2019 for relevant studies limited to those published in English from 2008 to 2019. Articles were included if they contained references to **, urological cancers, inflammation, immune activation, disease risk, disease progression, genomics, proteomics, and biomarkers. Results Inflammatory biomarkers show promise in prognostication in **, including neutrophil-to-lymphocyte ratio, C-reactive protein, selected cytokines and stress proteins. Most of the current evidence, however, stems from retrospective studies. None of these biomarkers are sufficient by themselves to be used for prognostication. Using a panel of different biomarkers, alongside clinical and pathological data, seems to improve risk stratification. More robust data is necessary, however, before these biomarkers will be suitable for use in routine practice. Conclusion Biomarkers of inflammation and immune system activation can assist in risk stratification of **. Currently most of these biomarkers lack robust external validity. In this report, we review the current literature and further investigated the landscape of radiation sensitivity in nodal metastases of PeCa utilizing our well-established genome-based radiosensitivity index (RSI) platform. We hypothesized that optimal therapeutic gain could be achieved in PeCa stratified by the combination of clinicopathological parameters, genomic heterogeneity, and RSI-based radiation dose prescription (RxRSI). Similar to primary PeCa lesions, we found that the majority of PeCa nodal metastases are genomically radioresistant with significant heterogeneity. RxRSI should be considered to inform and optimize the radiation therapy dose prescription to the individual tumor biology.Poly(L-lactic acid) (PLLA) is currently the bioresorbable polymer of choice for vascular stents with its superior biocompatibility and mechanical properties. However, it is still difficult to enhance the radial supporting capacity of PLLA stents without increasing the strut thickness. In this study, the performance of laser-cut thin-strut stents from two groups of PLLA tubes are investigated. We considered two groups of PLLA tubes. Group 1 indicates the longitudinally stretched from original 150-μm-thick tubes, and Group 2 indicates the directly thinned from original 150-μm-thick tubes. Three stages of mechanical tests were conducted in this study, which are defined as tensile tests of dog-bone specimens, radial loading tests of tubes and radial loading tests of stents. The results suggest that Group 2 has higher radial supporting capacity than Group 1 with the same wall thickness. This work serves as a basis for manufacturing thin-strut stents with sufficient radial supporting capacity.Background Medication reviews conducted in community pharmacies are a measure to reduce drug-related problems and to increase medication safety. This project therefore aims at developing suitable indicators to assess the quality of medication reviews type 2a in community pharmacies. Methods Based on a literature review, potential quality indicators were generated. Using a two-step Delphi method applying the RUMBA criteria, a set of structure, process, and outcome indicators was developed. Results The literature review identified 23 potential indicators. Nine further indicators derived from the guideline for medication reviews and expert opinion were amended. After discussion in a focus group, the 32 indicators were reduced to a preliminary set of twelve indicators used for the Delphi survey. Following two Delphi rounds, a final indicator set consisting of three structure indicators, one process indicator and two outcome indicators, was generated. Discussion The set of quality indicators is potentially suitable for measuring the quality of medication reviews in German community pharmacies.
