  • To evaluate the risk of severe infection and infection-related mortality among patients with newly diagnosed systemic lupus erythematosus (SLE).

    We conducted an age- and gender- matched cohort study of all patients with incident SLE between January 1, 1997 and March 31, 2015 using administrative health data from British Columbia, Canada. Primary outcome was the first severe infection after SLE onset necessitating hospitalization or occurring during hospitalization. Secondary outcomes were total number of severe infections and infection-related mortality.

    We identified 5,169 SLE patients and matched them with 25,845 non-SLE individuals from the general population, yielding 955 and 1,986 first severe infections during 48,367 and 260,712 person-years follow-up, respectively. The crude incidence rate ratios for first severe infection and infection-related mortality were 2.59 (95% CI, 2.39-2.80) and 2.20 (95% CI, 1.76-2.73), respectively. The corresponding adjusted hazard ratios were 1.82 (95% CI 1.66-1.99) and 1.61 (95% CI, 1.24-2.08). SLE patients had an increased risk of a greater total number of severe infections with crude rate ratio of 3.24 (95% CI, 3.06-3.43) and adjusted rate ratio of 2.07 (95% CI, 1.82-2.36).

    Recent evidence suggests that "vulnerable plaques," which have received intense attention as underlying mechanism of acute coronary syndromes over the decades, actually rarely rupture and cause clinical events. Superficial plaque erosion has emerged as a growing cause of residual thrombotic complications of atherosclerosis in an era of increased preventive measures including lipid lowering, anti-hypertensive therapy, and smoking cessation. The mechanisms of plaque erosion remain poorly understood, and we currently lack validated effective diagnostics or therapeutics for superficial erosion. Eroded plaques have a rich extracellular matrix, an intact fibrous cap, sparse lipid, and few mononuclear cells, but do harbor neutrophil extracellular traps (NETs). We recently reported that NETs amplify and propagate the endothelial damage at the site of arterial lesions that recapitulate superficial erosion in ****. We showed that genetic loss of protein arginine deiminase (PAD)-4 function inhibited NETosis and preser erosion.
    NPs directed to Col IV show selective uptake and delivery of their payload to experimentally eroded regions, illustrating their translational potential. Our results further support the role of PAD4 and NETs in superficial erosion.

    We performed a case-control study of patients with IgG4-RD, compared in a 15 ratio with age-, race- and sex-matched controls. We included cases evaluated at Massachusetts General Hospital, a hospital within the Mass General Brigham (MGB) System. Controls were identified from the MGB Biobank. Smoking status at date of IgG4-RD diagnosis or corresponding index date was determined. Conditional logistic regression was used to estimate the association between cigarette smoking and odds of having IgG4-RD.

    There were 234 IgG4-RD cases and 1,170 controls. Mean age (59 years), sex (62% male), and race (75% white) were well-balanced. IgG4-RD cases were more likely to be current smokers compared with controls (25 [11%] vs 70 [6%], OR 1.79 [95% CI 1.08-2.95]). This association was strongest among female cases (13 [14%] vs 19 [4%], OR 3.79 [95% CI 1.71-8.39]) and those with retroperitoneal fibrosis (RPF, 13 [28%] vs 13 [6%], OR 6.93 [95% CI 2.78-17.26]) or normal IgG4 concentrations (21 [21%] vs 21 [4%], OR 6.22 [95% CI 3.09-12.49]). When RPF cases were excluded, there was no longer an association between current smoking and the odds of having IgG4-RD (12 [6%] vs 57 [6%], OR 0.95 [95% CI 0.49-1.86]).

In this study, we conducted a systematic review and meta-analyzed data from RCTs (n=18) and observational (n=7) studies separately, to assess the consistency of the magnitude of association between SGLT-2 inhibitors and DKA. We also illustrate the strengths and weakness of the two designs. Results from RCTs and observational studies consistently show almost a doubling in the risk of DKA in patients using an SGLT-2 inhibitor compared to placebo or active comparator. Using a random-effects model, the pooled relative risk [RR], (95% confidence interval [CI]) was 2.08, (95%CI 1.28, 3.40) from placebo-controlled RCTs and was 0.82 (95%CI 0.25, 2.68) from active-comparator RCTs.     Hepatitis C virus (HCV) infections in Switzerland are mainly related to intravenous drug use. Since 2017, all patients with chronic hepatitis C can be treated with direct-acting antivirals (DAAs) irrespective of fibrosis stage. Here, we aim to describe differences in diagnostic and infection prevention and control (IPC) measures in the fight against MDROs.

    In 2019, we systematically assessed diagnostic algorithms and IPC measures implemented for detection and control of MDROs at three tertiary academic care centres (Freiburg; Strasbourg; Basel). Data were collected using a standardised data collection sheet to be filled in by every centre. Uncertainties were clarified by direct contact via telephone or email with the data supplier.   • Ambient ozone (O3) concentrations have shown an upward trend in China and its health hazards have also been recognized in recent years. High-resolution exposure data based on statistical models are needed. Our study aimed to build high-performance random forest (RF) models based on training data from 2013 to 2017 in the Beijing-Tianjin-Hebei (BTH) region in China at a 0.01 ° × 0.01 ° resolution, and estimated daily maximum 8h average O3 (O3-8hmax) concentration, daily average O3 (O3-mean) concentration, and daily maximum 1h O3 (O3-1hmax) concentration from 2010 to 2017. Model features included meteorological variables, chemical transport model output variables, geographic variables, and population data. The test-R2 of sample-based O3-8hmax, O3-mean and O3-1hmax models were all greater than 0.80, while the R2 of site-based and date-based model were 0.68-0.87. From 2010 to 2017, O3-8hmax, O3-mean, and O3-1hmax concentrations in the BTH region increased by 4.18 μg/m3, 0.11 μg/m3, and 4.71 μg/m3, especially in more developed regions. Due to the influence of weather conditions, which showed high contribution to the model, the long-term spatial distribution of O3 concentrations indicated a similar pattern as altitude, where high concentration levels were distributed in regions with higher altitude.Freshwater ecosystems are facing increasing contamination by major ions. Results were compared to 24-h and 96-h LC50 data from the literature, mainly from fathead minnow (Pimephales promelas). All salts caused concentration-dependent increases in TEP to less negative/more positive values, in patterns well-described by the Michaelis-Menten equation, or a modified version incorporating substrate inhibition. The ΔTEP above baseline became close to a maximum at the 96-h LC50, except for the HCO3- salts. Furthermore, the range of ΔTEP values at the LC50 within one species was **** more consistent (1.6- to 2.1-fold variation) than the molar concentrations of the different salts at the LC50 (19- to 25-fold variation).     We used data from 2548 older adults from the Seniors-ENRICA II cohort in Madrid, Spain. Whole blood Cd levels were measured at baseline using inductively coupled plasma-mass spectrometry. At baseline (2017) and follow-up (2019), overall physical function was evaluated using the physical component summary (PCS) of the SF 12-Item Health questionnaire, lower-extremity performance with the Short Physical Performance Battery (SPPB), muscle weakness with a hand dynamometer, and frailty with a Deficit Accumulation index. Mobility limitations and disability in instrumental activities of daily living (IADL) were ascertained with standardized questionnaires. Analyses were adjusted for relevant confounders, including tobacco smoke, number of cigarettes smoked per day and time since cessation in former smokers.

    In cross-sectional analyses, odds ratios (95% confidence interval) per two-fold increase in blood Cd were 1.16 (1.03; 1.31) for low PCS scores, 1.08 (0.97; 1.20) for impaired lower-extremity performance, 1.10 (0.98; 1.23) for low grip strength, 1.11 (1.02; 1.20) for mobility limitations, 1.16 (1.02; 1.31) for frailty, and 1.26 (1.08; 1.47) for IADL disability.   • Two-dimensional materials such as graphene and transition metal dichalcogenides (TMDCs) have received extensive research interest and investigations in the past decade. In this research, we used a refined opto-thermal Raman technique to explore the thermal transport properties of one popular TMDC material WSe2, in the single-layer (1L), bilayer (2L), and trilayer (3L) forms. This measurement technique is direct without additional processing to the material, and the absorption coefficient of WSe2 is discovered during the measurement process to further increase this technique's precision. By comparing the sample's Raman spectroscopy spectra through two different laser spot sizes, we are able to obtain two parameters-lateral thermal conductivities of 1L-3L WSe2 and the interfacial thermal conductance between 1L-3L WSe2 and the substrate. We also implemented full-atom nonequilibrium molecular dynamics simulations (NEMD) to computationally investigate the thermal conductivities of 1L-3L WSe2 to provide comprehensive evidence and confirm the experimental results. The trend of the layer-dependent lateral thermal conductivities and interfacial thermal conductance of 1L-3L WSe2 is discovered.     Two-dimensional materials such as graphene and transition metal dichalcogenides (TMDCs) have received extensive research interest and investigations in the past decade. In this research, we used a refined opto-thermal Raman technique to explore the thermal transport properties of one popular TMDC material WSe2, in the single-layer (1L), bilayer (2L), and trilayer (3L) forms. This measurement technique is direct without additional processing to the material, and the absorption coefficient of WSe2 is discovered during the measurement process to further increase this technique's precision. By comparing the sample's Raman spectroscopy spectra through two different laser spot sizes, we are able to obtain two parameters-lateral thermal conductivities of 1L-3L WSe2 and the interfacial thermal conductance between 1L-3L WSe2 and the substrate. We also implemented full-atom nonequilibrium molecular dynamics simulations (NEMD) to computationally investigate the thermal conductivities of 1L-3L WSe2 to provide comprehensive evidence and confirm the experimental results. The trend of the layer-dependent lateral thermal conductivities and interfacial thermal conductance of 1L-3L WSe2 is discovered. The room-temperature thermal conductivities for 1L-3L WSe2 are 37 ± 12, 24 ± 12, and 20 ± 6 W/(m·K), respectively. The suspended 1L WSe2 possesses a thermal conductivity of 49 ± 14 W/(m·K).   • As most recently demonstrated by the SARS-CoV-2 pandemic, congenital and perinatal infections are of significant concern to the pregnant population as compared to the general population. These outcomes can range from no apparent impact all the way to spontaneous abortion or fetal infection with long term developmental consequences. While some pathogens have developed mechanisms to cross the placenta and directly infect the fetus, other pathogens lead to an upregulation in maternal or placental inflammation that can indirectly cause harm. The placenta is a temporary, yet critical organ that serves multiple important functions during gestation including facilitation of fetal nutrition, oxygenation, and prevention of fetal infection in utero. Here, we review trophoblast cell immunology and the molecular mechanisms utilized to protect the fetus from infection. Lastly, we discuss consequences in the placenta when these protections fail and the histopathologic result following infection.The ability of dairy farmers to keep their cows for longer could positively enhance the economic performance of the farms, reduce the environmental footprint of the milk industry, and overall help in justifying a sustainable use of animals for food production. Increase deposition and crosslinking of matrices by stromal cells lead to a heterogeneous microenvironment that aids in PDAC development. In the past decade, various hydrogel-based, in vitro tumor models have been developed to mimic and recapitulate aspects of the tumor microenvironment in PDAC. Advances in hydrogel chemistry and engineering should provide a venue for discovering new insights regarding how matrix properties govern PDAC cell growth, migration, invasion, and drug resistance. These engineered hydrogels are ideal for understanding how variation in matrix properties contributes to the progressiveness of cancer cells, including durotaxis, the directional migration of cells in response to a stiffness gradient. This review surveys the various hydrogel-based, in vitro tumor models and the methods to generate gradient stiffness for studying migration and other cancer cell fate processes in PDAC.Patients with severe facial deformities present serious dysfunctionalities along with an unsatisfactory aesthetic facial appearance. Several methods have been proposed to specifically plan the interventions on the patient's needs, but none of these seem to achieve a sufficient level of accuracy in predicting the resulting facial appearance. In this context, a deep knowledge of what occurs in the face after bony movements in specific surgeries would give the possibility to develop more reliable systems. This study aims to propose a novel 3D approach for the evaluation of soft tissue zygomatic modifications after zygomatic osteotomy; geometrical descriptors usually involved in face analysis tasks, i.e., face recognition and facial expression recognition, are here applied to soft tissue malar region to detect changes in surface shape. As ground truth for zygomatic changes, a zygomatic openness angular measure is adopted. The results show a high sensibility of geometrical descriptors in detecting shape modification of the facial surface, outperforming the results obtained from the angular evaluation.An optical and dielectric biosensor based on a liquid crystal (LC)-photopolymer composite was established in this study for the detection and quantitation of bovine serum albumin (BSA). When the nematic LC E7 was doped with 4-wt.% NOA65, a photo-curable prepolymer, and photopolymerized by UV irradiation at 20 mW/cm2 for 300 s, the limit of detection determined by image analysis of the LC optical texture and dielectric spectroscopic measurements was 3400 and 88 pg/mL for BSA, respectively, which were lower than those detected with E7 alone (10 μg/mL BSA). The photopolymerized NOA65, but not the prepolymer prior to UV exposure, contributed to the enhanced optical signal, and UV irradiation of pristine E7 in the absence of NOA65 had no effect on the optical texture. The effective tilt angle θ, calculated from the real-part dielectric constant ε', decreased with increasing BSA concentration, providing strong evidence for the correlation of photopolymerized NOA65 to the intensified disruption in the vertically oriented LC molecules to enhance the optical and dielectric signals of BSA.

  • Vancomycin-resistant Enterococcus faecium (VREfm) carrying vanA was first isolated from patient at Siriraj Hospital, Thailand in 2004. Since then, VREfm isolates have been detected increasingly in this 2500-bed university hospital. To understand the epidemiology of vanA VREfm in this setting, the isolates collected during 2004-2013 were characterized.

    A total of 49 vanA VREfm isolates previously confirmed by multiplex PCR were characterized by determining resistance phenotypes to vancomycin, teicoplanin, ampicillin and ciprofloxacin by broth microdilution method. Multilocus sequence typing (MLST) and virulence genes of those isolates were investigated. The Tn1546 structure diversity was studied by long-range overlapping PCR and primer walking sequencing.

    Of all isolates studied, 9 sequence types (ST17, ST80, ST78, ST730, ST203, ST18, ST280, ST64, ST323) in clonal complex 17 and a novel ST1051 were revealed. The esp-positive isolates were 73.5%. Of all vanA operons characterized, at least 9 types of Tn1546-like structures were detected. All of vanA determinants contained 5'-end different from the Tn1546 prototype. Approximately 47% of them also carried the insertion sequence IS1251 at the intergenic region between vanS and vanH. Interestingly, another IS (ISEfa4) was found to be inside the sequence of IS1251 in ST17 isolate.

    Exvivo experiments were performed on a 1.5-Tesla (T) MR scanner using an MR conditional cryoablation system on porcine tissue phantoms. Cryoneedles were placed inside the tissue phantom either with or without an angiocatheter. Typical cryoneedle geometric configurations (including gas supply line) encountered in clinical procedures with low to high expected heating risks were investigated. Up to 4 fiber optic temperature sensors were attached to the cryoneedle/angiocatheter to measure the MR-induced cryoneedle heating at different locations during MR with different estimated specific absorption rates (SARs). The impact of cryoneedle heating on cryoablation treatment was studied by comparing temperature changes during 10-min freeze-thaw cycles with and without MR.

    Rapid temperature increases of >100 °C in < 2 minutes were observed during MR with a SAR of 2.1 W/kg. The temperature changes during a typical freeze-thaw cycle were also affected by cryoneedle heating when MR was used to monitor the ice-ball evolution. The observed cryoneedle heating was affected by multiple factors; including cryoneedle geometric configurations, sequence SAR, whether an angiocatheter was used, and whether the cryoneedle was connected to the rest of the cryoablation system.

    To determine whether socioeconomic status (SES) is associated with hepatic encephalopathy (HE) risk after transjugular intrahepatic portosystemic shunt (TIPS) creation.

Cryoneedles were placed inside the tissue phantom either with or without an angiocatheter. Typical cryoneedle geometric configurations (including gas supply line) encountered in clinical procedures with low to high expected heating risks were investigated. Up to 4 fiber optic temperature sensors were attached to the cryoneedle/angiocatheter to measure the MR-induced cryoneedle heating at different locations during MR with different estimated specific absorption rates (SARs). The impact of cryoneedle heating on cryoablation treatment was studied by comparing temperature changes during 10-min freeze-thaw cycles with and without MR. Rapid temperature increases of >100 °C in < 2 minutes were observed during MR with a SAR of 2.1 W/kg. The temperature changes during a typical freeze-thaw cycle were also affected by cryoneedle heating when MR was used to monitor the ice-ball evolution. The animals were divided into a healthy control (no treatment), CLP, IDE control (200 mg/kg), and CLP + IDE subgroups (50 mg/kg, 100 mg/kg, and 200 mg/kg), with nine rats in each group.IDE was administered 1 h after CLP induction.To evaluate the protective effects of IDE, lung tissues were collected 16 h after sepsis for biochemical, immunohistochemical staining, and histopathological examination.

Imatinib discontinuation after achieving a sustained deep molecular response was attempted in 14 patients; 12 (86%) are still in treatment-free remission (TFR) at the last follow-up.   • How cancer cells utilize nutrients to support their growth and proliferation in complex nutritional systems is still an open question. However, it is certainly determined by both genetics and an environmental-specific context. The interactions between them lead to profound metabolic specialization, such as consuming glucose and glutamine and producing lactate at prodigious rates. To investigate whether and how glucose and glutamine availability impact metabolic specialization, we integrated computational modeling on the genome-scale metabolic reconstruction with an experimental study on cell lines. We used the most comprehensive human metabolic network model to date, Recon3D, to build cell line-specific models. RNA-Seq data was used to specify the activity of genes in each cell line and the uptake rates were quantitatively constrained according to nutrient availability. To integrated both constraints we applied a novel method, named Gene Expression and Nutrients Simultaneous Integration (GENSI), that translates the relative importance of gene expression and nutrient availability data into the metabolic fluxes based on an observed experimental feature(s). We applied GENSI to study hepatocellular carcinoma addiction to glucose/glutamine. We were able to identify that proliferation, and lactate production is associated with the presence of glucose but does not necessarily increase with its concentration when the latter exceeds the physiological concentration. There was no such association with glutamine. However, there are still significant gaps in the evidence identifying common competencies for training and education programmes, and the clarification of strategies for workforce retention, motivation, deployment, and coordination. Initiated in June 2020, this project includes literature reviews, case studies, and an expert consensus (modified Delphi) study. Literature reviews in English, Japanese, and Chinese aim to identify research gaps and explore core competencies for Health EDRM workforce training. Thirteen Health EDRM related case studies from six WHO regions will illustrate best practices (and pitfalls) and inform the consensus study. Consensus will be sought from global experts in emergency and disaster medicine, nursing, public health and related disciplines. This paper will describe a solution for dynamic radio map creation, which is aimed to reduce the time required to build a radio map. The proposed solution is using measurements from IMUs (Inertial Measurement Units), which are processed with a particle filter dead reckoning algorithm. Reference points (RPs) generated by the implemented dead reckoning algorithm are then processed by the proposed reference point merging algorithm, in order to optimize the radio map size and merge similar RPs. The proposed solution was tested in a real-world environment and evaluated by the implementation of deterministic fingerprinting positioning algorithms, and the achieved results were compared with results achieved with a static radio map. Of these, the model 1 and 2 were chosen as they not only fulfil the recommended values for a good number of validation parameters (e.g., R2 = 0.77-0.79, Q2LMO = 0.75-0.76, CCCex = 0.86-0.87, Q2-F3 = 0.74-0.76, etc.) but also provide useful insights into the structural features that sway the Hs-NMT inhibitory activity of Nitrogen heterocycles. The achieved results presented in the paper show that positioning algorithms achieved similar accuracy even with a dynamic map with a low density of reference points.N-myristoyltransferase (NMT) is an important eukaryotic monomeric enzyme which has emerged as an attractive target for developing a drug for cancer, leishmaniasis, ischemia-reperfusion injury, malaria, inflammation, etc. In the present work, statistically robust machine leaning models (QSAR (Quantitative Structure-Activity Relationship) approach) for Human NMT (Hs-NMT) inhibitory has been performed for a dataset of 309 Nitrogen heterocycles screened for NMT inhibitory activity. Hundreds of QSAR models were derived. Of these, the model 1 and 2 were chosen as they not only fulfil the recommended values for a good number of validation parameters (e.g., R2 = 0.77-0.79, Q2LMO = 0.75-0.76, CCCex = 0.86-0.87, Q2-F3 = 0.74-0.76, etc.) but also provide useful insights into the structural features that sway the Hs-NMT inhibitory activity of Nitrogen heterocycles. That is, they have an acceptable equipoise of descriptive and predictive qualities as per Organisation for Economic Co-operation and Development (OECD) guidelines.
