Methods We determined FcγR polymorphisms in 303 patients with GBS and 302 ethnically matched healthy individuals from Bangladesh by allele-specific polymerase chain reaction. Pairwise linkage disequilibrium and haplotype patterns were analyzed based on D ́statistics and the genotype package of R statistics, respectively. Logistic regression analysis and Fisher's exact test with corrected P (Pc) values were employed for statistical comparisons. Results FcγRIIIa-V158F was associated with the severe form of GBS compared to the mild form (P = 0.005, OR = 2.24, 95% CI = 1.28-3.91; Pc = 0.015); however, FcγR genotypes and haplotype patterns did not show any association with GBS susceptibility compared to healthy controls. FcγRIIIa-V/V158 and FcγRIIIb-NA2/2 were associated with recent Campylobacter jejuni infection (P ≤ 0.001, OR = 0.36, 95% CI = 0.23-0.56; Pc ≤ 0.003 and P = 0.004, OR = 1.70, 95% CI = 1.18-2.44; Pc ≤ 0.012, respectively). Haplotype 1 (FcγRIIa-H131R- FcγRIIIa-V158F- FcγRIIIb-NA1/2) and the FcγRIIIb-NA2/2 genotype were more prevalent among anti-GM1 antibody-positive patients (P = 0.031, OR = 9.61, 95% CI = 1.24-74.77, Pc = 0.279; P = 0.027, OR = 1.62, 95% CI = 1.06-2.5, Pc = 0.081, respectively). Interpretation FcγR polymorphisms and haplotypes are not associated with susceptibility to GBS, though the FcγRIIIa-V158F genotype is associated with the severity of GBS.Nuclear accessibility of transcription factors controls gene expression, co-regulated by Ran-dependent nuclear localization and a competitive regulatory network. Kap114p, one of the karyopherin-βs (Kap-βs) that mediates nuclear import of yeast TATA-binding protein (yTBP), exhibits a yTBP-binding affinity four orders of magnitude greater than its counterparts and suppresses binding of yTBP with DNA. Our crystal structure of Kap114p reveals an extensively negatively charged concave surface, accounting for high-affinity basic-protein binding. KAP114 knockout in yeast leads to a high-salt growth defect, with transcriptomic analyses revealing that Kap114p modulates expression of genes associated with ribosomal biogenesis by suppressing yTBP binding to target promoters, a trans-repression mechanism we attribute to reduced nuclear Ran levels under salinity stress. This review will provide the current state of the art and recent advances in the field of injectable biomaterials for minimally invasive surgery.Herein, oil-soluble CdS quantum dots (QDs) are first prepared through a solvent-thermal process. Then, oil-soluble CdS QDs are changed into water-soluble QDs via ligand exchange using mercaptopropionic acid as capping agent at pH 13. The photocatalytic performance is investigated under the visible light irradiation using glycerol as sacrificial agent and Sn2+ as cocatalyst. No H2 -production activity is observed for oil-soluble CdS QDs. Water-soluble CdS QDs exhibit significantly enhanced hydrogen evolution rate. When the concentration of cocatalyst Sn2+ increases to 0.2 × 10-3 m, the rate of hydrogen evolution reaches 1.61 mmol g-1 h-1 , which is 24 times higher than that of the pristine water-soluble CdS QDs. The enhanced H2 -production efficiency is attributed to the adsorption of Sn2+ ions on the surface of CdS QDs that are further reduced to Sn atoms by photogenerated electrons. The in situ generated Sn atoms serve as photocatalytic cocatalyst for efficient hydrogen generation.

  • The value of minimally invasive pancreatic surgery (MIPS) is still uncertain, despite the growing number of publications, including reviews and meta-analyses, and the quick diffusion of these procedures worldwide. The Italian Group of Minimally Invasive Pancreas Surgery (IGoMIPS) was created under the auspices of three Scientific Societies Associazione Italiana Studio Pancreas (AISP), Associazione Italiana Chirurgia Epato-Bilio-Pancreatica (AICEP, former IT-IHPBA), and Società Italiana di Chirurgia Endoscopica (SICE). The main aim of IGoMIPS is to develop and implement a national registry for MIPS. IGoMIPS was founded on February 22, 2019 in Pisa. The IGoMIPS registry became operational in September 2019, following approval by the Ethic Committees of founding Institutions, inscription into the Registry of Patient Registries (RoPR), and a wrap-up meeting held in Bologna during the Annual Congress of the Italian Surgical Society. During this meeting IGoMIPS members approved that the Italian Registry will provide data to the European Registry, while retaining the right to analyze and publish Italian data. An audience survey was also conducted to obtain information on perceived value and current implementation of MIPS in founding Institutions. MIPS is performed in 94.7% of IGoMIPS centers, including pancreaticoduodenectomy in 42.1%. Robotic assistance was employed in 52.6% of Institutions. The annual volume of MIPS was 6-10 cases in 38.9% of the centers, 11-20 cases in 16.7%, 21-30 cases in 22.2%, and > 30 cases in 22.2%. The registry was felt to be extremely important for both safety improvement and educational purposes by 94.5% of the centers.Purpose Breast cancer is the leading cause of cancer death in females. Histone modifications have been shown to have an influence on the gene expression. This study focusses on the histone modifications H3K9ac and H3K4me3 in breast cancer and their impact on survival METHODS H3K4me3 and H3K9ac expression was immunohistochemically examined in 235 tissue samples. Results Positive estrogen receptor status was correlated with a higher IRS of the nuclear (p = 0.033), and of the cytoplasmic H3K4me3 staining (p = 0.009). H3K9ac intensity was associated to the Her2 status (p = 0.045) and to poor prognosis in cells with positive Ki67 status (p = 0.013). A high intensity of nuclear H3K4me3 staining was found to be correlated with a lower 10-year-survival (p = 0.026) and with lower breast cancer-specific survival (p = 0.004). High percentage score (> 190) of H3K9ac expression was correlated to worse breast cancer-specific survival (p = 0.005). Shorter progression-free survival was found in patients with nuclear (p = 0.013) and cytoplasmic H3K4me3expression (p = 0.024) and H3K9ac expression (p = 0.023). Conclusion This analysis provides new evidence of histone modifications in breast cancer. High H3K4me3 and H3K9ac expression was correlated with survival rates. Further investigation of histone modifications in breast cancer could lead to a more profound understanding of the molecular mechanisms of cancer development and could result in new therapeutic strategies.Monovalent thallium (Tl+) is a cation that can exert complex neurotoxic patterns in the brain by mechanisms that have yet to be completely characterized. Caenorhabditis elegans (C. elegans) is a nematode constituting a simple in vivo biological model with a well-characterized nervous system, and high genetic homology to mammalian systems. In this study, both wild-type (N2) and skn-1 knockout (KO) mutant C. elegans strains subjected to acute and chronic exposures to Tl+ [2.5-35 μM] were evaluated for physiological stress (survival, longevity, and worm size), motor alterations (body bends), and biochemical changes (glutathione S-transferase regulation in a gst-4 fluorescence strain). While survival was affected by Tl+ in N2 and skn-1 KO (worms lacking the orthologue of mammalian Nrf2) strains in ae nematode represents an optimal model to reproduce mammalian Tl+ toxicity, where toxic mechanisms and novel therapeutic approaches of clinical value may be successfully pursued.Purpose of review Cellular therapies, also known as "stem cell" interventions (SCI), have undergone a rapid popularization in the USA and worldwide. The current review aimed at outlining (1) the ethical challenges facing the implementation of SCI; (2) the applicability of the currently available SCI; and (3) recommendations to achieve ethical, well-regulated incorporation of SCI in the clinical setting. Recent findings Concerns regarding the inadequate characterization, poor adverse effects disclosure, and unorthodox, often inappropriate, market practices have engendered a genuine concern regarding the SCI compliance with ethical standards. Six instances of litigation on the basis of misrepresentation or inappropriate informed consent were recorded between 2012 and 2018. Such concerns have been furthered by the loopholes in the regulatory aspect governing the use of SCI coupled with the unclear literature-reported efficacy and diverse spectrum of profess indications. Similarly, the application of SCI in the clinical field is yet to prove its value. The uncertain efficacy, coupled with obscure true-costs of utilization, impedes a value-based assessment. A multidisciplinary approach involving legislative and medical professional societies should continue to advance regulations that govern SCI. A well-regulated system that allows for the ethical integration of SCI with appositely evidenced-based described benefits and risks should be sought.Recent breakthrough results from immune checkpoint inhibitors (ICI) have paved the way to a new era of cancer immunotherapy. In particular, inhibition of programmed death-1 (PD-1)/programmed death-ligand 1 (PD-L1) axis with ICI including nivolumab and pembrolizumab has been emerging as a novel treatment strategy for advanced gastric cancers (GC). In a meta-analysis for anti-PD-1/PD-L1 therapy in GC, the objective response rate was 12.0% and the disease control ratio was 34.7%. https://www.selleckchem.com/products/pentetic-acid.html The ICI treatment in GC provided modest survival benefit and especially, anti-PD-1 treatment could improve the 12-month and 18-month overall survival rate and prolonged the duration of the response. The next steps for developing ICI in GC are mainly two challenges as follows. First is the identification of accurate biomarkers that can predict the response to ICI.

  • BACKGROUND Human papillomavirus (HPV)-related disease remains a significant source of morbidity and mortality, and this underscores the need to increase HPV vaccination to reduce the burden of the disease. The objective of this study was to examine the association between the number of HPV vaccine doses and the risk of histologically confirmed preinvasive cervical disease and high-grade cytology. METHODS This retrospective matched cohort study used administrative data from Optum's Clinformatics DataMart Database to identify females aged 9 to 26 years who received 1 or more quadrivalent HPV vaccine doses between January 2006 and June 2015. Cases and controls were matched on region, age, sexually transmitted disease history, and pregnancy. All had a Papanicolaou test ≥1 year after the date of the matched case's final dose. Cox proportional hazards models were used to examine the association between the number of HPV vaccine doses and the incidence of preinvasive cervical disease and high-grade cytology. The Kaplan-Meier method was used to estimate the cumulative incidence rate at the 5-year follow-up. RESULTS The study included 133,082 females (66,541 vaccinated and 66,541 unvaccinated) stratified by the number of HPV vaccine doses and the vaccine initiation age. Mean (SD) intra-operative blood loss was 1038 (778) ml across all procedures. Salvaged blood was re-infused in 505 of 664 (76%) patients. Mean (SD) re-infusion volume was 253 (169) ml. In total, 246 of 664 (37%) patients received an allogeneic red blood cell transfusion within 72 h of surgery. Patients undergoing femoral component revision only (OR (95%CI) 0.41 (0.23-0.73)) or acetabular component revision only (0.53 (0.32-0.87)) were less likely to generate sufficient blood salvage volume for re-infusion compared with revision of both components. Compared with aseptic indications, patients undergoing revision surgery for infection (1.87 (1.04-3.36)) or fracture (4.43 (2.30-8.55)) were more likely to generate sufficient blood salvage volume for re-infusion. Our data suggest that cell salvage is efficacious in this population.     BACKGROUND Human papillomavirus (HPV)-related disease remains a significant source of morbidity and mortality, and this underscores the need to increase HPV vaccination to reduce the burden of the disease. The objective of this study was to examine the association between the number of HPV vaccine doses and the risk of histologically confirmed preinvasive cervical disease and high-grade cytology. Here, the H2/SF6 permeance ratios were between 150 and 450. The methanol conversion rate was 70% with 38% ethylene selectivity and 28% propylene selectivity as determined using a cross-flow membrane contactor. In order to improve the olefin selectivity of the membrane, the MFI zeolite layer (Si/Al = ∞) was coated on an MFI-type zeolite membrane (Si/Al = 25). We also examined the influence of resilience on three key elements of healthy aging level of disability, participation and health-related quality of life (HRQoL). Methods Data were extracted from the Canadian Survey of Health, Lifestyle and Aging with MS (n = 743). Lifestyle, psychological health, and quality of life variables were compared between people with high and low resilience scores controlling for confounding variables. We used hierarchical regression to determine the unique contribution of resilience and related variables to healthy aging. Results Roughly, 1 in 5 respondents reported high resilience (18.8%), while 1 in 3 reported low resilience (33.9%). The group having higher resilience scores lived with less disability (~ 10%) and fatigue, reported greater participation, exercised more, consumed a healthier diet and lived with greater social support and financial security, compared to the lower scoring group. Resilience added only 1-2% of predictive value explaining disability, participation and HRQoL when confounding variables were accounted for. Years since diagnosis, type of MS, depression, fatigue and resilience significantly predicted healthy aging. Conclusion Resilience contributed minimally (but significantly) to healthy aging. Older participants scoring higher on resilience reported healthier lifestyle behaviors (more exercise, better diet) and social/financial support compared to lower scoring respondents. Our findings suggest that self-management programs for older persons with MS should focus on three key factors to foster healthy aging depression, fatigue and resilience.Purpose The self-rated health (SRH) item is frequently used in health surveys but variations of its form (wording, response options) may hinder comparisons between versions over time or across surveys. The objectives were to determine (a) whether three SRH forms are equivalent, (b) the form with the best construct validity and (c) the best coding scheme to maximize equivalence across forms. Methods We used data from 58,023 respondents of the Swiss Health Survey. Three SRH forms were used. Response options varied across forms and we explored four coding schemes (two considering SRH as continuous, two as dichotomous). Construct validity of the SRH was assessed using 34 health predictors to estimate the explained variance. Results Distributions of response options were similar across SRH forms, except for the "good" and "very good" options ("good" in form 1 58.6%, form 2 65.0% and form 3 44.1%). Explained variances differed across SRH forms, with form 3 providing the best overall explained variance, regardless of coding schemes. The linear coding scheme maximised the equivalence across SRH forms. Conclusion The three SRH forms were not equivalent in terms of construct validity. Studies examining the evolution of SRH over time with surveys using different forms should use the linear coding scheme to maximise equivalence between SRH forms.Background Previous studies identified alarming use of potentially inappropriate medications (PIMs) in Pakistani population but its effect on health-related quality of life (HRQoL) is still largely unknown. Objective This study aimed to determine the association between PIMs use and HRQoL among elderly cardiac outpatients. Method A descriptive, non-experimental, cross-sectional study was carried out from June 2018 to September 2018 in two outpatient departments of tertiary-care hospitals in the Punjab Province of Pakistan. The population under study were patients aged ≥ 65 years with at least one cardiovascular condition taking at ≥ 1 prescribed medication. Patients with PIMs were identified by using Beers criteria. HRQoL was assessed using EuroQoL-5 dimension (EQ-5D) and EuroQoL-visual analogue scale (EQ-VAS). The association of PIMs with HRQoL was analyzed using χ2 tests, independent sample t-test, and one-way ANOVA tests. Multiple linear regression analysis was used to determine how HRQoL varied by PIMs use after adjusting for patient-level covariates. Results Of 386 elderly cardiac patients, 260 (67.4%) patients were receiving at least one PIM. Mean EQ-5D scores were significantly lower among patients with PIMs (0.51) compared to patients without PIMs (0.65) (P less then 0.001). It has been stated that a history of arterial thrombosis and triple positivity was associated with a higher risk of thrombosis in APS patients treated with DOACs. However, their efficacy in non-high-risk APS patients with isolated venous manifestations is unsolved. Therefore, we performed a sub-group analysis of a previously published meta-analysis after the exclusion of patients with triple positivity and those with history of arterial or small vessel thrombosis. Recent findings We identified 290 APS patients with previous isolated venous event treated with DOACs; among them, 25 (8.6%) patients experienced a recurrent thrombosis in comparison to 16% in the original cohort. We found that the rate of recurrent thrombosis is lower in APS patients with isolated venous manifestations than in overall APS patients including high-risk patients.

We also examined the influence of resilience on three key elements of healthy aging level of disability, participation and health-related quality of life (HRQoL). Methods Data were extracted from the Canadian Survey of Health, Lifestyle and Aging with MS (n = 743). Lifestyle, psychological health, and quality of life variables were compared between people with high and low resilience scores controlling for confounding variables. We used hierarchical regression to determine the unique contribution of resilience and related variables to healthy aging. Results Roughly, 1 in 5 respondents reported high resilience (18.8%), while 1 in 3 reported low resilience (33.9%). The group having higher resilience scores lived with less disability (~ 10%) and fatigue, reported greater participation, exercised more, consumed a healthier diet and lived with greater social support and financial security, compared to the lower scoring group. Resilience added only 1-2% of predictive value explaining disability, participation and HRQoL when confounding variables were accounted for. Years since diagnosis, type of MS, depression, fatigue and resilience significantly predicted healthy aging. Conclusion Resilience contributed minimally (but significantly) to healthy aging. Older participants scoring higher on resilience reported healthier lifestyle behaviors (more exercise, better diet) and social/financial support compared to lower scoring respondents. Our findings suggest that self-management programs for older persons with MS should focus on three key factors to foster healthy aging depression, fatigue and resilience.Purpose The self-rated health (SRH) item is frequently used in health surveys but variations of its form (wording, response options) may hinder comparisons between versions over time or across surveys. The objectives were to determine (a) whether three SRH forms are equivalent, (b) the form with the best construct validity and (c) the best coding scheme to maximize equivalence across forms. Methods We used data from 58,023 respondents of the Swiss Health Survey. Three SRH forms were used. Response options varied across forms and we explored four coding schemes (two considering SRH as continuous, two as dichotomous). Construct validity of the SRH was assessed using 34 health predictors to estimate the explained variance. Results Distributions of response options were similar across SRH forms, except for the "good" and "very good" options ("good" in form 1 58.6%, form 2 65.0% and form 3 44.1%). Explained variances differed across SRH forms, with form 3 providing the best overall explained variance, regardless of coding schemes. The linear coding scheme maximised the equivalence across SRH forms. Conclusion The three SRH forms were not equivalent in terms of construct validity. Alzheimer-type pathology was assessed to assign the likelihood of clinical presentations of dementia with Lewy body (DLB) using the consensus criteria for DLB. We reviewed medical records to characterize clinicopathologic features of MSA+LBD. Genetic risk factors for LBD, including APOE ε4 allele and mutations in GBA, SNCA, LRRK2, and VPS35, were analyzed. RESULTS LBD was observed in 11 MSA patients (5%); seven were brainstem type, three were transitional type, and one was diffuse type. The latter four had an intermediate or high likelihood of DLB. Three of the four had an antemortem diagnosis of Parkinson's disease with dementia (PDD) or clinically probable DLB. Two patients had neuronal loss in the substantia nigra, but not in striatal or olivocerebellar systems with widespread glial cytoplasmic inclusions, consistent with minimal change MSA. https://www.selleckchem.com/products/lithocholic-acid.html In these cases, LBD was considered the primary pathology, and MSA was considered coincidental. Nevertheless, the effects and mechanisms of APA-induced 3'UTR shortening in nasopharyngeal carcinoma (NPC) remain largely unclear. Fibronectin type III domain containing 3B (FNDC3B) tended to use proximal polyadenylation site and produce shorter 3'UTR according to our previous sequencing study. Herein, we found that FNDC3B with shorter 3'UTR could escape from miRNA-mediated gene repression, and caused its increased expression in NPC. Knocking down of FNDC3B inhibited NPC cell proliferation, migration, invasion, and metastasis in vitro and in vivo. Overexpression of FNDC3B, especially those with shorter 3'UTR, promoted NPC progression. Furthermore, the mechanism study revealed that FNDC3B could bind to and stabilize myosin heavy chain 9 (MYH9) to activate the Wnt/β-catenin signaling pathway. In addition, MYH9 could reverse the inhibitory effects of FNDC3B knock-down in NPC. Altogether, our results suggested that the 3'UTR shortening of FNDC3B mRNA mediated its overexpression in NPC and promoted NPC progression by targeting MYH9. This newly identified FNDC3B-MYH9 -Wnt/β-catenin axis could represent potential targets for individualized treatment in NPC. This article is protected by copyright. The donor site should be designed to be hidden without previous or present lesions; this limits donor sites. Aim of the study was to clinically and histopathologically evaluate donor areas 3 months and 1 year after Thiersch grafting (TG) and suction blister epithelial grafting (SBEG). Forty patients with stable vitiligo were equally divided into two groups before TG and SBEG. In each patient, the donor site was clinically and histopathologically evaluated after 3 months and 1 year. After 3 months in Group I, hyperpigmentation, mixed pigmentation with atrophic scarring, hyperpigmentation with hypertrophic scarring, erythema, and tough skin were detected in 10%, 50%, 10%, 10%, and 20% of cases, respectively. The condition improved with time leaving, leaving tough skin in all cases after 1 year. In Group II, hyperpigmentation was observed in all patients after 3 months and disappeared completely after 1 year. Histologically, biopsies taken 3 months after TG, showed increased basal pigmentation with dermal changes mimicking scar tissue in 92.5% of cases. Collagen homogenization of and fragmented elastic tissue were reported in all cases. Biopsies showed the same features after 1 year. In the SBEG group, biopsies revealed only basal hyperpigmentation that disappeared after 1 year in all patients. We concluded that SBEG is a scarless operation and regrafting from the same area can be performed. METHOD 200 psoriasis vulgaris patients with moderate to severe plaque psoriasis from January 2013 to June 2014 were evenly divided into two groups the consolidated therapy group (CTG) and unconsolidated therapy group (UTG), the remission period of the two groups was compared. RESULTS There was no significant difference in PASI score between the two groups at the beginning and the end of the treatment. However, the average remission time in CTG was 10.99 months, which was significantly longer than that of 7.94 months in UTG (p = 0.001). After a correction of age, course of disease, skin type as well as PASI baseline value using a COX model, we found that the risk of recurrence of psoriasis vulgaris in UTG was higher than that in the CTG (p less then  0.001). No adverse reactions were discovered when combing the two treatments together.

  • The purpose of this study was to evaluate the prevalence of Neisseria gonorrhoeae (NG) and Trichomonas vaginalis (TV) in Portuguese women of childbearing age. Cervicovaginal self-collected samples of 680 childbearing-age women (15-44 years) were tested for NG and TV by polymerase chain reaction. https://www.selleckchem.com/products/ml351.html Sociodemographic, clinical and behavioural data were assessed through an anonymous self-administered questionnaire. NG and TV prevalence was 1.3% (95% confidence interval (CI) 0.7-2.5%) and 1.0% (95% CI 0.5-2.1%), respectively. The prevalence of TV was significantly higher in women aged >22 years (p = .003), with >6 years after sexual intercourse (p = .003), and who reported previous pregnancy (p = .004). Our study suggests that NG and TV are rare in Portuguese women of childbearing age. However, larger epidemiological studies with a nationally representative sample of female subjects are warranted, to clarify the need for screening of these microorganisms in Portuguese women, since its prevalence is probably underestse microorganisms. In clinical practice, it should be highlighted the appropriate management of NG and TV infection in specific situations, such as pregnancy. Also, sexual partners must be treated to prevent the recurrences in the index cases and reduce transmission to other partners.Background Although rivaroxaban has demonstrated consistent drug levels in normal weight and obese patients, sufficient confirmation of equal clinical effectiveness and safety is currently lacking.Purpose To evaluate the effectiveness and safety of rivaroxaban versus warfarin for prevention of stroke and systemic embolism (SSE) in obese nonvalvular atrial fibrillation (NVAF) patients.Methods Using Optum de-identified Electronic Health Record (EHR) data from November 2011 to September 2018,we evaluated NVAF patients with a body mass index (BMI)≥30 kg/m2 newly initiated on rivaroxaban or warfarin (index date), with ≥12-months of EHR activity and ≥1 encounter before the index date. We excluded patients with valvular disease or evidence of oral anticoagulant (OAC) use at baseline. Patients who were prescribed rivaroxaban were 11 propensity-score matched to patients who were prescribed warfarin (standard differences less then 0.10 achieved for all covariates). Outcomes included SSE and major bleeding using an intent-to-treat approach. Subanalyses stratified by BMI (30.0-34.9, 35.0-39.9 and ≥40 kg/m2) were performed. Cox regression was performed and reported as hazard ratios (HRs) and 95% confidence intervals (CIs).Results We included 35,613 rivaroxaban and 35,613 warfarin users with NVAF. Patients were followed for a median of 2.6 years (25%-75% range = 1.2-4.1). Rivaroxaban was associated with a reduced risk of SSE (HR = 0.83, 95%CI = 0.73-0.94) and major bleeding (HR = 0.82, 95%CI = 0.75-0.89) compared to warfarin. Subanalysis did not show a statistically significant interaction across BMI categories for SSE (p-interaction = .58) or major bleeding (p-interaction = .44) outcomes.Conclusions Among obese NVAF patients, prescription of rivaroxaban was associated with a reduced risk of SSE and major bleeding compared to warfarin, which remained consistent across BMI classes.Aims To describe the real-world economic burden of patients with anaplastic lymphoma kinase-positive (ALK+) non-small cell lung cancer (NSCLC) treated with post-crizotinib, second-line ALK inhibitor therapy.Materials and methods Retrospective analysis using data from US Optum Clinformatics Data Mart administrative claims database. Adult patients with ALK + NSCLC treated with ceritinib or alectinib as second-line ALK inhibitors between 1/1/2011-9/30/2017 were included. Healthcare costs and resource utilization for up to 1 year of therapy were calculated on a per-patient-per-month (PPPM) basis and stratified by presence or absence of brain metastases (BM). Multivariate regression analysis was performed to identify factors associated with costs. Top 10 cost drivers of non-inpatient procedure costs were recorded.Results 112 patients received second-line ALK inhibitors. Total mean PPPM healthcare costs were $23,984 for all patients receiving up to 1 year of post-crizotinib, second-line ALK inhibitor therapy. Total mean PPPM costs for patients with BM on or prior to post-crizotinib, second-line ALK inhibitor therapy were 1.37 times as high as those for patients without BM (P = 0.0406). Mean PPPM outpatient visits and inpatient hospitalization stays were higher for patients with BM versus no BM. The main cost drivers for non-inpatient procedures were radiation therapy, medications, and diagnostic radiology.Limitations Analyses did not include newer ALK-directed therapies. BM development after the index date (defined as the date of the first claim for a second-line ALK inhibitor) may have been misclassified as non-BM. Findings may not be generalizable to patients with no health insurance coverage.Conclusions Treatment of patients with ALK + NSCLC with ceritinib or alectinib as post-crizotinib, second-line ALK inhibitor therapy represents a high economic burden. Healthcare costs and resource utilization were significantly higher for patients with ALK + NSCLC with BM versus no BM.Peer relationships among youth have been examined as predictors of mental health outcomes for at least fifty years, revealing dozens of discrete peer constructs that each are associated with adjustment in childhood, adolescence, and later in adulthood. Future research may benefit by examining a range of new outcomes and psychological processes that have been discussed recently in related literatures. This paper reviews recent research on interpersonal determinants of physical health outcomes, and opportunities for greater examination of 1) peer influence processes toward health risk behaviors; 2) neural correlates of peer adversity; 3) adverse peer experiences that may affect physiological markers of stress response; and 4) immune system markers of peer adversity. Additional future directions include the study of differences in the forms and functions of peer interactions within the digital age.

Also, sexual partners must be treated to prevent the recurrences in the index cases and reduce transmission to other partners.Background Although rivaroxaban has demonstrated consistent drug levels in normal weight and obese patients, sufficient confirmation of equal clinical effectiveness and safety is currently lacking.Purpose To evaluate the effectiveness and safety of rivaroxaban versus warfarin for prevention of stroke and systemic embolism (SSE) in obese nonvalvular atrial fibrillation (NVAF) patients.Methods Using Optum de-identified Electronic Health Record (EHR) data from November 2011 to September 2018,we evaluated NVAF patients with a body mass index (BMI)≥30 kg/m2 newly initiated on rivaroxaban or warfarin (index date), with ≥12-months of EHR activity and ≥1 encounter before the index date. We excluded patients with valvular disease or evidence of oral anticoagulant (OAC) use at baseline. Patients who were prescribed rivaroxaban were 11 propensity-score matched to patients who were prescribed warfarin (standard differences less then 0.10 achieved for all covariates). Outcomes included SSE and major bleeding using an intent-to-treat approach. Subanalyses stratified by BMI (30.0-34.9, 35.0-39.9 and ≥40 kg/m2) were performed. Cox regression was performed and reported as hazard ratios (HRs) and 95% confidence intervals (CIs).Results We included 35,613 rivaroxaban and 35,613 warfarin users with NVAF. Patients were followed for a median of 2.6 years (25%-75% range = 1.2-4.1). Rivaroxaban was associated with a reduced risk of SSE (HR = 0.83, 95%CI = 0.73-0.94) and major bleeding (HR = 0.82, 95%CI = 0.75-0.89) compared to warfarin. Adult patients with ALK + NSCLC treated with ceritinib or alectinib as second-line ALK inhibitors between 1/1/2011-9/30/2017 were included. Healthcare costs and resource utilization for up to 1 year of therapy were calculated on a per-patient-per-month (PPPM) basis and stratified by presence or absence of brain metastases (BM). Multivariate regression analysis was performed to identify factors associated with costs. Top 10 cost drivers of non-inpatient procedure costs were recorded.Results 112 patients received second-line ALK inhibitors. Total mean PPPM healthcare costs were $23,984 for all patients receiving up to 1 year of post-crizotinib, second-line ALK inhibitor therapy. Total mean PPPM costs for patients with BM on or prior to post-crizotinib, second-line ALK inhibitor therapy were 1.37 times as high as those for patients without BM (P = 0.0406). Mean PPPM outpatient visits and inpatient hospitalization stays were higher for patients with BM versus no BM. The main cost drivers for non-inpatient procedures were radiation therapy, medications, and diagnostic radiology.Limitations Analyses did not include newer ALK-directed therapies. BM development after the index date (defined as the date of the first claim for a second-line ALK inhibitor) may have been misclassified as non-BM. Findings may not be generalizable to patients with no health insurance coverage.Conclusions Treatment of patients with ALK + NSCLC with ceritinib or alectinib as post-crizotinib, second-line ALK inhibitor therapy represents a high economic burden. Healthcare costs and resource utilization were significantly higher for patients with ALK + NSCLC with BM versus no BM.Peer relationships among youth have been examined as predictors of mental health outcomes for at least fifty years, revealing dozens of discrete peer constructs that each are associated with adjustment in childhood, adolescence, and later in adulthood. This paper reviews recent research on interpersonal determinants of physical health outcomes, and opportunities for greater examination of 1) peer influence processes toward health risk behaviors; 2) neural correlates of peer adversity; 3) adverse peer experiences that may affect physiological markers of stress response; and 4) immune system markers of peer adversity. Additional future directions include the study of differences in the forms and functions of peer interactions within the digital age.

  • By integration of benzene-constructed porous organic polymer (KBF) and multiwalled carbon nanotube (MWCNT), a MWCNT-KBF hybrid material was constructed through in situ knitting benzene with formaldehyde dimethyl acetal in the presence of MWCNTs to form a network. MWCNT-KBF was then adopted as a novel solid-phase microextraction (SPME) fiber coating. Coupled to gas chromatography-flame ionization detection, the MWCNT-KBF-assisted SPME method showed large enhancement factors (483-2066), low limits of detection (0.04-0.12 μg L-1), good linearity (0.13-50 μg L-1), and acceptable reproducibility (4.2-10.2%) for the determination of polycyclic aromatic hydrocarbons (PAHs). The method recoveries of seven PAHs were in the range 80.1-116.3%, with relative standard deviations (RSDs) ranging from 3.5 to 11.9%. The SPME method was successfully applied to the determination of PAHs in river, pond, rain, and waste water, providing a good alternative for monitoring trace level of PAHs in environmental water. A total of 1326 samples were collected from 2003 to 2006 at four sites in southwest Georgia and north Florida from nine-banded armadillos (Dasypus novemcinctus), bobcats (Lynx rufus), feral cats (Felis catus), coyotes (Canis latrans), grey foxes (Urocyon cinereoargenteus), opossums (Didelphis virginiana), and raccoons (Procyon lotor). Data from armadillos, bobcats, opossums, and raccoons were published previously to examine age, sex, spatial (between sites), and temporal (between years) variation. In this paper, we present similar comparisons for the remaining three species as well as cocies for every metal but only a single main effect of year for Cu, and no main effects of site. In sum, our results provide an extensive survey of metal concentrations in a diverse assemblage of mammals and suggest that metal accumulation may be heavily influenced by species identity, which in turn may reflect ecological lifestyle.Animals travelling through the world receive input from multiple sensory modalities that could be important for the guidance of their journeys. Given the availability of a rich array of cues, from idiothetic information to input from sky compasses and visual information through to olfactory and other cues (e.g. gustatory, magnetic, anemotactic or thermal) it is no surprise to see multimodality in most aspects of navigation. In this review, we present the current knowledge of multimodal cue use during orientation and navigation in insects. Multimodal cue use is adapted to a species' sensory ecology and shapes navigation behaviour both during the learning of environmental cues and when performing complex foraging journeys. The simultaneous use of multiple cues is beneficial because it provides redundant navigational information, and in general, multimodality increases robustness, accuracy and overall foraging success. We use examples from sensorimotor behaviours in mosquitoes and flies as well as from large scale navigation in ants, bees and insects that migrate seasonally over large distances, asking at each stage how multiple cues are combined behaviourally and what insects gain from using different modalities.BACKGROUND The transcribed ultraconserved regions (T-UCRs) are a novel class of long non-coding RNAs and are involved in the development of several types of cancer. A majority of what we know about spatial skills today is a result of more than 100 years of research focused on understanding and identifying the kinds of skills that make up this skill set. Over the last two decades, the field has recognized that, unlike the spatial skills measured by psychometric tests developed by psychology researchers, the spatial problems faced by STEM experts vary widely and are multifaceted. Thus, many psychological researchers have embraced an interdisciplinary approach to studying spatial thinking with the aim of understanding the nature of this skill set as it occurs within STEM disciplines. In a parallel effort, discipline-based education researchers specializing in STEM domains have focused **** of their research on understanding how to bolster students' skills in completing domain-specific spatial tasks. In this paper, we discuss four lessons learned from these two programs of research to enhance the field's understanding of spatial thinking in STEM domains. We demonstrate each contribution by aligning findings from research on three distinct STEM disciplines structural geology, surgery, and organic chemistry. Lastly, we discuss the potential implications of these contributions to STEM education.

  • OBJECTIVE The goal of this study was to retrospectively analyze clinical and surgical data of a consecutive series of twenty-six patients with unilateral cystic vestibular schwannomas. METHODS Tumors were classified as type A (central cyst) and type B (peripheral cyst), and as small (tumor diameter 95%) was achieved in 16 cases (61.5%), subtotal removal (90-95%) in 9 cases (34.6%), and partial removal ( less then 90%) in 1 case (3.9%). Position pattern of FN was anterior-inferior in 10 cases (38.4%), anterior-superior in 10 cases (38.4%), anterior in 23.2% of cases. At hospital discharge, 9 (36%), 10 (38%), 3 (12%), 3 (12%) and 1 (4%) patients had a FN function of HB I-V, respectively; at final follow-up HB I, II, III and IV accounted respectively for 18 (72%), 6 (24%), 1 and 1 cases. At a follow-up ranging from 6 months to 10 years, a reoperation for growing of residue was never necessary. CONCLUSIONS According to the literature and to the results of our series, microneurosurgery of cystic vestibular schwannomas is associated with good outcomes in terms of extent of resection and FN function. These patients were evaluated for PJK/PJF, and patient reported outcomes were recorded. RESULTS In total, 40 patients were included. Mean follow-up duration was 2.2 years (SD 0.3). Patients in this cohort had poor preoperative sagittal alignment (PI-LL 26.7°, TPA 29.0°, SVA 93.4mm) and achieved substantial sagittal correction (ΔSVA -62.2mm, ΔPI-LL -19.8°, ΔTPA -11.1°) following caudal extension surgery. At final follow-up, there was a 0% rate of PJF among patients undergoing caudal extension of previous fusion without creation of a new UIV, but 27.5% of patients experienced PJK. Patients experienced significant improvement in both ODI and SRS-22r total score at 2-years post-operatively (p less then 0.05). In total, 7.5% (n=3) of patients underwent further revision, at an average of 1.1 years (SD 0.54) after the surgery with unaltered UIV. All three of these patients underwent revision for rod fracture with no revisions for PJK/PJF. CONCLUSIONS Patients undergoing caudal extension of previous fusions for sagittal alignment correction have high rates of clinical success, low revision surgery rates, and very low rates of PJF. Minimizing repetitive tissue trauma at the UIV may result in decreased PJF risk, as the PJF rate in this cohort of unaltered UIV patients is below historical PJF rates of patients undergoing sagittal balance correction. OBJECTIVE We aimed to determine the characteristic alignment change in patients with myelopathy recurrence after multilevel anterior cervical corpectomy and fusion (m-ACCF). METHODS Fifty-two patients who underwent m-ACCF, including 20 who underwent revision surgeries for myelopathy recurrence (R-group) and 32 postoperative asymptomatic patients (A-group), were analyzed. Classic alignment parameters (cervical lordosis angle [CL], cervical sagittal vertical axis, and fusion area angle and length) and original alignment parameters (α-β, β-BG, BG-γ, and γ-δ angles) were measured preoperatively, postoperatively, and at follow-up or before revision surgery. The difference in the amount of change in parameters between the groups was analyzed. The relationship between the distribution of restenotic lesions and characteristic alignment change in the R-group was evaluated. RESULTS The CL, fusion area angle, and fusion area length in the R-group significantly decreased postoperatively compared with those in the A-group (p less then 0.01, p less then 0.01, p=0.04, respectively). Compared with those in the A-group, α-β and β-BG angles in the R-group significantly decreased (p less then 0.01), indicating kyphotic change on the cranial side. BG-γ and γ-δ angles in the R-group significantly increased (p less then 0.01), indicating lordotic change in the caudal fused area. Restenotic lesions significantly increased on the cranial side in the R-group (cranial side, 19 levels; caudal side, 5 levels; p less then 0.01). CONCLUSIONS In patients with myelopathy recurrence after m-ACCF, the cranial side has significant kyphosis and the caudal side has lordosis. Moreover, 79.2% of the restenotic lesions were significantly maldistributed on the cranial side. Surgeons should pay close attention to cranial kyphosis inducing myelopathy recurrence after m-ACCF. OBJECTIVE We evaluated whether a previously developed educational material by health professionals about hydrocephalus and its treatment corresponded with the daily life of caregivers of children with. METHODS We conducted a qualitative study at a university hospital in Brazil, interviewing 32 informal caregivers of children with hydrocephalus. The methodological framework of Bardin's content analysis was used to analyze the data. RESULTS The educational material adequately represented caregivers' experience regarding daily life, surgery experiences, and care needed by children with hydrocephalus. Additionally, the educational material may help to identify the signs and symptoms of ventriculoperioteneal shunt. However, the material did not address the limitations of children disabled with hydrocephalus in daily life. CONCLUSIONS From the perspective of informal caregivers of children with hydrocephalus, the educational material corresponded with families' daily life and surgical experience and may be used by health professionals to reinforce important points for the care for children with hydrocephalus, facilitating the health education process. BACKGROUND Subdural drain (SDD) and Subdural Evacuating Port System (SEPS) are bedside options for management of non-acute subdural hematomas (SDH). These interventions have not been compared with each other. Our objective is to compare the need for second bedside procedure, need for craniotomy, complication rate and other outcomes related to bedside drainage of SDH with SDD or SEPS. We hypothesized that SDD would be associated with superior outcomes to SEPS. METHODS Database queries and direct patient chart reviews were used to gather patient data. T-tests, Fisher's exact tests, and proportional odds models were performed. RESULTS Of 41 SDD and 25 SEPS, baseline characteristics were similar except more isodense SDHs were present in SDD (p = .0312). SEPS was associated with significant risk of requiring a second bedside procedure (OR 3.2381, 99% CI 1.0345 - 10.1355) relative to SDD. Minimizing repetitive tissue trauma at the UIV may result in decreased PJF risk, as the PJF rate in this cohort of unaltered UIV patients is below historical PJF rates of patients undergoing sagittal balance correction. OBJECTIVE We aimed to determine the characteristic alignment change in patients with myelopathy recurrence after multilevel anterior cervical corpectomy and fusion (m-ACCF). METHODS Fifty-two patients who underwent m-ACCF, including 20 who underwent revision surgeries for myelopathy recurrence (R-group) and 32 postoperative asymptomatic patients (A-group), were analyzed. Classic alignment parameters (cervical lordosis angle [CL], cervical sagittal vertical axis, and fusion area angle and length) and original alignment parameters (α-β, β-BG, BG-γ, and γ-δ angles) were measured preoperatively, postoperatively, and at follow-up or before revision surgery. The difference in the amount of change in parameters between the groups was analyzed. The relationship between the distribution of restenotic lesions and characteristic alignment change in the R-group was evaluated. RESULTS The CL, fusion area angle, and fusion area length in the R-group significantly decreased postoperatively compared with those in the A-group (p less then 0.01, p less then 0.01, p=0.04, respectively). Compared with those in the A-group, α-β and β-BG angles in the R-group significantly decreased (p less then 0.01), indicating kyphotic change on the cranial side. BG-γ and γ-δ angles in the R-group significantly increased (p less then 0.01), indicating lordotic change in the caudal fused area. Restenotic lesions significantly increased on the cranial side in the R-group (cranial side, 19 levels; caudal side, 5 levels; p less then 0.01). CONCLUSIONS In patients with myelopathy recurrence after m-ACCF, the cranial side has significant kyphosis and the caudal side has lordosis. Moreover, 79.2% of the restenotic lesions were significantly maldistributed on the cranial side. Surgeons should pay close attention to cranial kyphosis inducing myelopathy recurrence after m-ACCF. OBJECTIVE We evaluated whether a previously developed educational material by health professionals about hydrocephalus and its treatment corresponded with the daily life of caregivers of children with. METHODS We conducted a qualitative study at a university hospital in Brazil, interviewing 32 informal caregivers of children with hydrocephalus. The methodological framework of Bardin's content analysis was used to analyze the data. RESULTS The educational material adequately represented caregivers' experience regarding daily life, surgery experiences, and care needed by children with hydrocephalus. Additionally, the educational material may help to identify the signs and symptoms of ventriculoperioteneal shunt. However, the material did not address the limitations of children disabled with hydrocephalus in daily life. CONCLUSIONS From the perspective of informal caregivers of children with hydrocephalus, the educational material corresponded with families' daily life and surgical experience and may be used by health professionals to reinforce important points for the care for children with hydrocephalus, facilitating the health education process. BACKGROUND Subdural drain (SDD) and Subdural Evacuating Port System (SEPS) are bedside options for management of non-acute subdural hematomas (SDH). These interventions have not been compared with each other. Our objective is to compare the need for second bedside procedure, need for craniotomy, complication rate and other outcomes related to bedside drainage of SDH with SDD or SEPS. We hypothesized that SDD would be associated with superior outcomes to SEPS. METHODS Database queries and direct patient chart reviews were used to gather patient data. T-tests, Fisher's exact tests, and proportional odds models were performed. RESULTS Of 41 SDD and 25 SEPS, baseline characteristics were similar except more isodense SDHs were present in SDD (p = .0312). SEPS was associated with significant risk of requiring a second bedside procedure (OR 3.2381, 99% CI 1.0345 - 10.1355) relative to SDD.
