The results from both tasks indicate that people anticipate their future motor performance so as to make decisions that will improve their expected future reward.The relationship between plant traits and species relative abundance along environmental gradients can provide important insights on the determinants of community structure. Here we bring extensive data on six key traits (specific leaf area (SLA), seed mass, seed germination rate, height, leaf proline content and photosynthesis rate) to test trait-abundance relationships in a successional chronosequence of subalpine meadow plant communities. Our results show that in late-successional meadows, abundant species had higher values for seed mass, seed germination rate, and SLA, but had lower values for height, photosynthesis rate, and leaf proline content than rarer species. The opposite patterns of trait-abundance relationships were observed for early-successional meadows. Observations of strong trait convergence and divergence in these successional communities lend greater support for niche processes compared to neutral community assembly. We conclude that species' niches that determine plant growth (plant height and photosynthesis rate), carbon balance (SLA, photosynthesis rate), regeneration (seed mass and seed germination rate), and abiotic stress resistance (leaf proline content) under different environmental conditions have strong influence on species relative abundance in these sub-alpine meadow communities during succession.Periodontal disease, the most prevalent infectious disease in the world, is caused by biofilms formed in periodontal pockets. Interest and consideration of nongenetic risk factors (e.g., lead exposure and schizophrenia) have, in contrast, lagged behind heritable frameworks of explanation. Thus, the association of neuropsychiatric illness to environmental chemical exposure, and their potential interactions with genetic susceptibility, are largely unexplored. In this review, we describe emerging approaches for considering the impact of chemical risk factors acting alone and in concert with genetic risk, and point to the potential role of epigenetics in mediating exposure effects on transcription of genes implicated in mental disorders. We highlight recent examples of research in nongenetic risk factors in psychiatric disorders that point to potential shared biological mechanisms-synaptic dysfunction, immune alterations, and gut-brain interactions. We outline new tools and resources that can be harnessed for the study of environmental factors in psychiatric disorders. These tools, combined with emerging experimental evidence, suggest that there is a need to broadly incorporate environmental exposures in psychiatric research, with the ultimate goal of identifying modifiable risk factors and informing new treatment strategies for neuropsychiatric disease.Major depressive disorder with psychotic features (psychotic depression) is a severe disorder. By 2017, there were 19 states without laws or regulations mandating services. The most common mandated service was facial surgery followed by habilitation/rehabilitation/speech therapy, orthodontics, dental care, and oral surgery. Nutrition, audiology, genetic testing and psychological counseling were rarely included in mandated services. CONCLUSIONS States vary widely in their requirements for coverage of services needed by children with CL/P in private health insurance plans. There has been an increase in mandates over the past two decades to cover services, although significant variation continues to exist across states.A mass casualty incident (MCI) is an event that generates more patients at one time than locally available resources can manage using routine procedures. By their nature, many of these incidents have no prior notice but result in large numbers of casualties with injuries that range in severity. They can happen anywhere and at any time and regional hospitals and health-care providers have to mount a response quickly and effectively to save as many lives as possible. Radiologists must go from passenger to pilot when it comes to MCI planning. When involved at the hospital-wide planning stage, they can offer valuable expertise on how radiology can improve triage accuracy and at what cost in terms of time and resources and thereby contribute a pragmatic understanding of radiology's role and value during MCIs. By taking ownership of MCI planning in their own departments, radiologists can ensure that the radiology department can respond quickly and effectively to unforeseen emergencies. Well-designed radiology protocols will save lives in an MCI setting.OBJECTIVE To identify concepts and constructs important to parents of children with Pierre Robin Sequence (PRS). DESIGN Qualitative study. SETTING All children received some care at a tertiary hospital with additional care at outside facilities. Interviews were conducted in nonclinical locations, including remote locations. PARTICIPANTS Parents of children less then 5 years old with a diagnosis of PRS. Prior treatments included observation, positioning, nasal trumpet, mandibular distraction osteogenesis, tracheostomy, and gastrostomy. INTERVENTION Semi-structured interviews with individuals (4) and with groups (focus groups, 4) were conducted using open-ended questions and non-leading prompts. Transcripts were analyzed with iterative open and axial coding. Concepts and constructs were identified and refined into codes and central themes. Interviews were conducted until thematic saturation was achieved. RESULTS Sixteen parents were interviewed. Their experiences were coded into 5 main themes, which can be summarized as (1) child's symptoms/well-being, (2) parents' grief/isolation, (3) family stress, (4) relationships with providers, and (5) psychological and technical growth. Difficulty with feeding, weight gain, and breathing problems were core physical issues described by participants with associated intense fear. Participants described frustration from not only lack of care coordination, slow diagnoses, and poor communication but also gratitude for providers who served as advocates. Participants described gradual development of knowledge/competencies. CONCLUSIONS Families of children with PRS have experiences that profoundly affect their lives. Child's physical symptoms/well-being and parents' psychosocial well-being provide content for a future PRS-specific quality-of-life instrument. Concepts that emerged also provide a framework to improve parents' experience and enhance their children's quality of care.We present an extreme rare case of extramedullary nasal plasmacitoma that arise from nasal septum. The mass surgically removed was analyzed by a pathologist who diagnosed an extramedullary nasal plasmacytoma. The patient did not present systemic involvement. A short cycle of radiotherapy was performed after the surgery. At 9-month follow-up, the patient is recurrence free.PURPOSE The purpose of this study is to compare the diagnostic performance of an autonomous artificial intelligence (AI) system for the diagnosis of referable diabetic retinopathy (RDR) to manual grading by Spanish ophthalmologists. METHODS Subjects with type 1 and 2 diabetes participated in a diabetic retinopathy (DR) screening program in 2011 to 2012 in Valencia (Spain), and two images per eye were collected according to their standard protocol. Mydriatic drops were used in all patients. Retinal images-one disc and one fovea centered-were obtained under the Medical Research Ethics Committee approval and de-identified. Exams were graded by the autonomous AI system (IDx-DR, Coralville, Iowa, United States), and manually by masked ophthalmologists using adjudication. The outputs of the AI system and manual adjudicated grading were compared using sensitivity and specificity for diagnosis of both RDR and vision-threatening diabetic retinopathy (VTDR). RESULTS A total of 2680 subjects were included in the study. According to manual grading, prevalence of RDR was 111/2680 (4.14%) and of VTDR was 69/2680 (2.57%). Against manual grading, the AI system had a 100% (95% confidence interval [CI] 97%-100%) sensitivity and 81.82% (95% CI 80%-83%) specificity for RDR, and a 100% (95% CI 95%-100%) sensitivity and 94.64% (95% CI 94%-95%) specificity for VTDR. CONCLUSION Compared to manual grading by ophthalmologists, the autonomous diagnostic AI system had high sensitivity (100%) and specificity (82%) for diagnosing RDR and macular edema in people with diabetes in a screening program. Because of its immediate, point of care diagnosis, autonomous diagnostic AI has the potential to increase the accessibility of RDR screening in primary care settings.Background. Through extensive spectroscopic analysis and single-crystal X-ray diffraction, their structures including absolute configurations were unambiguously determined. A plausible biosynthetic pathway for the two compounds was proposed. Although a few studies have investigated selective bromide removal from produced water, the low removal efficiencies and complex system setups are not desirable. In this study, we examined a simple cost-effective approach for selective bromide removal from produced water relying on the oxidation by un-activated peroxymonosulfate (PMS). More than 95% of bromide was removed as Br2(g) in less than 10 min under weakly acidic conditions without significant formation of Cl2(g) even when the chloride concentration was more than two orders of magnitude higher. A kinetic model considering the involved reactions was then developed to describe the process well under various reaction conditions. The organic compounds in produced water neither noticeably lowered bromide removal efficiency nor reacted with the halogen species to form halogenated byproducts. The tests in batch and continuously-stirred tank reactor systems suggested that it was feasible to achieve both high bromide removal and neutral effluent pH such that further pH adjustment was not necessary before discharge. After the treatment, the effect of the produced water on DBP formation was largely eliminated.Herein, we reported a simple solvothermal and chemical oxidation method to synthesize a magnetic core-shell composite (Fe3O4@UiO-66@PANI) for Cr(VI) removal from wastewater. Due to the porosity and stability of UiO-66 and stability, high acid resistance, and multiple active (reducing and chelating) groups of polyaniline (PANI), Fe3O4@UiO-66@PANI exhibited excellent efficiency, regeneration, and reusability performance for Cr(VI) removal. Its maximum adsorption capacity and removal rate were 474.42 mg·g-1 and 99.90%, respectively. The effects of initial pH values, contact time, and initial Cr(VI) concentration on Cr(VI) removal were investigated. The fitted data showed that the adsorption process was consistent with the pseudo-second-order kinetic model and Langmuir isothermal model. Although a few studies have investigated selective bromide removal from produced water, the low removal efficiencies and complex system setups are not desirable. In this study, we examined a simple cost-effective approach for selective bromide removal from produced water relying on the oxidation by un-activated peroxymonosulfate (PMS). More than 95% of bromide was removed as Br2(g) in less than 10 min under weakly acidic conditions without significant formation of Cl2(g) even when the chloride concentration was more than two orders of magnitude higher. A kinetic model considering the involved reactions was then developed to describe the process well under various reaction conditions. The organic compounds in produced water neither noticeably lowered bromide removal efficiency nor reacted with the halogen species to form halogenated byproducts. The tests in batch and continuously-stirred tank reactor systems suggested that it was feasible to achieve both high bromide removal and neutral effluent pH such that further pH adjustment was not necessary before discharge. After the treatment, the effect of the produced water on DBP formation was largely eliminated.Herein, we reported a simple solvothermal and chemical oxidation method to synthesize a magnetic core-shell composite (Fe3O4@UiO-66@PANI) for Cr(VI) removal from wastewater. Due to the porosity and stability of UiO-66 and stability, high acid resistance, and multiple active (reducing and chelating) groups of polyaniline (PANI), Fe3O4@UiO-66@PANI exhibited excellent efficiency, regeneration, and reusability performance for Cr(VI) removal. The effects of initial pH values, contact time, and initial Cr(VI) concentration on Cr(VI) removal were investigated. The fitted data showed that the adsorption process was consistent with the pseudo-second-order kinetic model and Langmuir isothermal model. The study of the mechanism shows that the excellent efficiency of Fe3O4@UiO-66@PANI is due to the electrostatic adsorption and reduction of Cr(VI) and the chelation of Cr3+. A set of six specific primers targeting a total 240 nt sequence regions in the DNA A of CuLCrV were designed and synthesized for detection of CuLCrV from infected leaf tissues using real-time LAMP amplification with the Genie® III system, which was further confirmed by gel electrophoresis and SYBR™ Green I DNA staining for visual observation. The optimum reaction temperature and time were determined, and no cross-reactivity was seen with other begomoviruses. The LAMP assay could amplify CuLCrV from a mixed virus assay. The sensitivity assay demonstrated that the LAMP reaction was more sensitive than conventional PCR, but less sensitive than qPCR. However, it was simpler and faster than the other assays evaluated. Filtering for larger variants (>10 kbp) and combined with genotyping of SVs using short-read data from the Great Ape Genome Project, we identified 425 deletions and 59 inversions, of which 88 and 36, respectively, were novel. Compared with gene expression in humans, we found a significant enrichment of chimpanzee genes with differential expression in lymphoblastoid cell lines and induced pluripotent stem cells, both within deletions and near inversion breakpoints. We examined chromatin-conformation maps from human and chimpanzee using these same cell types and observed alterations in genomic interactions at SV breakpoints. Finally, we focused on 56 genes impacted by SVs in >90% of chimpanzees and absent in humans and gorillas, which may contribute to chimpanzee-specific features. Design A prospective case-control study. Setting A pediatric ward of a general hospital in a low-resource setting. Participants Totally, 445 consecutive admissions to the pediatric ward of a general hospital over a period of 1 year were included in this prospective case-control study; hemodynamically unstable subjects (11) were excluded. Interventions Age, PEM, fever, and ALC were assessed in 59 patients with diarrhea and compared with 375 control patients without diarrhea. Odds ratios with confidence intervals were determined; the chi-square test and binary logistic regression analysis were also performed. Main Outcome Measures Associations of diarrhea with age, PEM, fever and ALC singly and various combinations of predispositions and SIRS parameters. Results Infancy and ALC were significantly associated with diarrhea. PEM or fever alone was not significantly associated with diarrhea; however, the probability of developing diarrhea was significantly higher when a combination of ALC and PEM was observed. The combination of infancy, PEM, and ALC carried a sensitivity of 81·36%; for other combinations, sensitivity varied between 70% and 80%. The combination of infancy and ALC had the lowest sensitivity (59·32%) but the best specificity (61·07%). Conclusion The association/presence of a combination of SIRS parameters (fever and ALC) and predispositions (infancy and PEM) in children with diarrhea may help in deciding whether antibiotic therapy should be initiated. © 2020 Daga et al.Introduction According to rapid growth of innovation in use of technologies in education and learning, blended learning has become an effective method in the student's education. The purpose of this study was to extract the factors related to effective blended learning and compare the viewpoints of students and instructors in this regard. Methods Mixed method model was used in this study. A content analysis approach was employed in the qualitative phase of the study. Seven faculty members and eight master's students of medical sciences selected through purposive sampling participated in the qualitative phase. Individual semi-structured interviews were conducted for data collection. In the quantitative phase, appropriate and relevant items were designed based on the main concepts of theoretical and practical definitions that were extracted in the qualitative phase. Results The results of the qualitative phase were 5 categories and 40 subcategories, including student's capabilities, teacher merits, technological aspects, pedagogic topics and supportive environment. Discussion The results of the questionnaire analysis showed no significant difference in student capabilities and supportive environment between the viewpoints of students and faculty members while there were significant differences in other categories. Although new technologies have offered promising opportunities, they do not result in effective education. The results of the present study suggest that a major revision in student and instructor admission and provision of required technical facilities can promote the status of this type of education. A total of 35 subjects were interviewed one on one. Interviews were transcribed and analyzed for common themes. Medical staff outlined novel limitations and views of future directions for the field of medical clowning. Most importantly, many previously unpublished benefits of medical clowns were described. These reported benefits included cost-saving measures for the hospital, increases in staff efficiency, better patient outcomes, and lower stress in medical staff. Given that most of the limitations on medical clowning are financial in nature, these findings have substantial implications for the future of the field. As medical clowning continues to grow, this study outlines the potential for various future research projects within this field of study. © The Author(s) 2020.

Design A prospective case-control study. Setting A pediatric ward of a general hospital in a low-resource setting. Participants Totally, 445 consecutive admissions to the pediatric ward of a general hospital over a period of 1 year were included in this prospective case-control study; hemodynamically unstable subjects (11) were excluded. Interventions Age, PEM, fever, and ALC were assessed in 59 patients with diarrhea and compared with 375 control patients without diarrhea. Odds ratios with confidence intervals were determined; the chi-square test and binary logistic regression analysis were also performed. Main Outcome Measures Associations of diarrhea with age, PEM, fever and ALC singly and various combinations of predispositions and SIRS parameters. Results Infancy and ALC were significantly associated with diarrhea. PEM or fever alone was not significantly associated with diarrhea; however, the probability of developing diarrhea was significantly higher when a combination of ALC and PEM was observed. The combination of infancy, PEM, and ALC carried a sensitivity of 81·36%; for other combinations, sensitivity varied between 70% and 80%. The combination of infancy and ALC had the lowest sensitivity (59·32%) but the best specificity (61·07%). Conclusion The association/presence of a combination of SIRS parameters (fever and ALC) and predispositions (infancy and PEM) in children with diarrhea may help in deciding whether antibiotic therapy should be initiated. © 2020 Daga et al.Introduction According to rapid growth of innovation in use of technologies in education and learning, blended learning has become an effective method in the student's education. The purpose of this study was to extract the factors related to effective blended learning and compare the viewpoints of students and instructors in this regard. Methods Mixed method model was used in this study. A content analysis approach was employed in the qualitative phase of the study. Seven faculty members and eight master's students of medical sciences selected through purposive sampling participated in the qualitative phase. Individual semi-structured interviews were conducted for data collection. In the quantitative phase, appropriate and relevant items were designed based on the main concepts of theoretical and practical definitions that were extracted in the qualitative phase. Results The results of the qualitative phase were 5 categories and 40 subcategories, including student's capabilities, teacher merits, technological aspects, pedagogic topics and supportive environment. Discussion The results of the questionnaire analysis showed no significant difference in student capabilities and supportive environment between the viewpoints of students and faculty members while there were significant differences in other categories. Although new technologies have offered promising opportunities, they do not result in effective education. The results of the present study suggest that a major revision in student and instructor admission and provision of required technical facilities can promote the status of this type of education. A total of 35 subjects were interviewed one on one. Interviews were transcribed and analyzed for common themes. Medical staff outlined novel limitations and views of future directions for the field of medical clowning. Most importantly, many previously unpublished benefits of medical clowns were described. These reported benefits included cost-saving measures for the hospital, increases in staff efficiency, better patient outcomes, and lower stress in medical staff. Given that most of the limitations on medical clowning are financial in nature, these findings have substantial implications for the future of the field. There was some evidence that the BMI z score reduction was greater in settings with limited well-child health care programmes (interaction P value = .03). Improvements were also detected in television viewing time, feeding practices, and breastfeeding duration. BACKGROUND Transcarotid artery revascularization (TCAR) with cerebral flow reversal is an emerging treatment option for carotid artery stenosis in patients with high risk for traditional endarterectomy. The purpose of this study was to compare real-world, procedure-related outcomes in similarly comorbid patients undergoing TCAR or carotid endarterectomy (CEA). METHODS A retrospective review of all patients receiving either TCAR or CEA outside of clinical trial regulations at our institution was performed. Participants were propensity-matched by age, gender, body mass index, smoking status, presence of restenosis, history of neck radiation, presence of contralateral carotid occlusion, history of previous neck dissection, and symptom status. Bivariate analysis was followed by a penalized Firth logistic regression to compare treatments. RESULTS Between January 2011 and July 2018, 342 CEAs and 109 TCARs were captured for analysis. After matching, 87 distinct treatment pairs were created without evidence of variathorter operative time and less blood loss. BACKGROUND We hypothesized that administration of cilostazol may clarify the occult atrial fibrillation (AF) during hospitalization in mild stroke patients, who has no history of AF. METHODS From our prospective non-cardioembolic stroke study, randomized to dual antiplatelet therapy using cilostazol and aspirin or aspirin alone trial (ADS), data on the presence or absence of AF were retrospectively analyzed. In the ADS, during hospitalization, as a routine examination, presence of AF was investigated using electrocardiogram (ECG), ECG monitoring and Holter ECG. Multivariate regression analysis was conducted to evaluate the independent parameters related to the AF. Clinical outcome at 3 months was evaluated using modified Rankin Scale (mRS) score. RESULTS Data on 1194 patients (793 [66%] men; median age [interquartile range] of 69 [61-77] years, National Institutes of Health Stroke Scale score 2 [1-4], onset-to-admission 10.8 [4.7-20.5] hours) were retrospectively analyzed. AF was newly detected in 41 (3%) patients (3 by ECG, 21 by the ECG monitoring and 17 by the Holter ECG) during hospitalization. Patients treated with combined cilostazol and aspirin therapy frequently had the AF than those took aspirin alone (5% vs. 2%, p = .007). V.BACKGROUND Children with congenital heart disease (CHD) have an increased risk of neurocognitive impairment. No prior studies have evaluated the role of obstructive sleep apnea (OSA), which is associated with neurocognitive impairment in children without CHD. RESEARCH QUESTION Is OSA is associated with neurocognitive impairment in children with CHD? STUDY DESIGN AND METHODS Children age 6-17 years with corrected moderate-complex CHD without syndromes that may affect neurocognition were recruited from the pediatric cardiology clinic. Participants underwent home sleep testing and neurocognitive testing including a validated IQ test as well as validated tests of memory (Paired Associates Learning test), executive function (Intra-Extra Dimensional set shift test) and attention (Simple Reaction Test) from the CANTAB neurocognitive testing battery. RESULTS Complete results were available for 30 children. Seventeen children (57%) were found to have OSA. Total IQ was markedly lower in children with CHD and comorbid OSA compared to children with CHD without comorbid OSA (mean 86 ± 12, vs 98 ± 11, p=0.01). Children with CHD and OSA did significantly worse on the Paired Associates Learning test, with a median of 8 total errors (interquartile range [IQR] 2.25-15) compared to children with CHD without OSA (median total errors 2, IQR 1-8), p=0.02. INTERPRETATION Children with CHD and comorbid OSA have impaired neurocognition compared to children with CHD without comorbid OSA. OSA may be a reversible cause of neurocognitive impairment in children with CHD. Further research is needed to evaluate the effects of OSA treatment on neurocognitive impairment in children with CHD. Methamphetamine (METH) is a major public health and safety problem worldwide. METH is psychostimulant that activates microglia via the toll-like receptor (TLR) 4/MD2 complex, modulating the abundant production of pro-inflammatory cytokines in the central nervous system (CNS). The TLR4/MD2 complex on the surface of microglia recognizes pathogen-associated molecular patterns such as lipopolysaccharide (LPS) resulting in brain tissue inflammation and neuronal damage. Since METH has been associated with microglia-induced neurotoxicity, we hypothesized that METH impairs the expression of TLR4 and activation of NF-κB in NR-9460 microglia-like cells after LPS challenge. We demonstrated that METH decreases the distribution and expression of TLR4 receptors on the surface of microglia-like cells after incubation with endotoxin. Moreover, METH impairs the TLR4/MD2 complex signaling pathways, compromises the activation of NF-κB, and reduces the production of pro-inflammatory mediators in microglia-like cells upon LPS stimulation. Interestingly, microglia-like cells treated with METH and challenged with LPS showed considerable cellular morphological changes including enlarged nuclei and ruffled surface. Our results suggest that METH may have a significant impact on microglial-induced neuroinflammation, neurotoxicity, and the CNS defense against infection. It also highlights the importance of studying the effects of METH on the molecular and cellular components of users' CNS immunity. Finally, animal studies exploring the role of METH on the effectors functions of microglia after antigenic exposure are necessary to understand drug-related inflammation and neural damage in users. Pioneer studies on FcγR N-glycans have unveiled an additional complexity in that the N-glycan linked on the Asn-162 of FcγRIIIa was shown to be directly involved in the strong affinity for afucosylated IgG1. The last few years have thus seen the emergence of many studies investigating the complex influence of FcγRIIIa N-glycans on the interaction with IgG1 through their glycosylation sites or their glycoprofiles. METHODS From our prospective non-cardioembolic stroke study, randomized to dual antiplatelet therapy using cilostazol and aspirin or aspirin alone trial (ADS), data on the presence or absence of AF were retrospectively analyzed. In the ADS, during hospitalization, as a routine examination, presence of AF was investigated using electrocardiogram (ECG), ECG monitoring and Holter ECG. Multivariate regression analysis was conducted to evaluate the independent parameters related to the AF. Clinical outcome at 3 months was evaluated using modified Rankin Scale (mRS) score. RESULTS Data on 1194 patients (793 [66%] men; median age [interquartile range] of 69 [61-77] years, National Institutes of Health Stroke Scale score 2 [1-4], onset-to-admission 10.8 [4.7-20.5] hours) were retrospectively analyzed. AF was newly detected in 41 (3%) patients (3 by ECG, 21 by the ECG monitoring and 17 by the Holter ECG) during hospitalization. Patients treated with combined cilostazol and aspirin therapy frequently had the AF than those took aspirin alone (5% vs. 2%, p = .007). V.BACKGROUND Children with congenital heart disease (CHD) have an increased risk of neurocognitive impairment. No prior studies have evaluated the role of obstructive sleep apnea (OSA), which is associated with neurocognitive impairment in children without CHD. RESEARCH QUESTION Is OSA is associated with neurocognitive impairment in children with CHD? STUDY DESIGN AND METHODS Children age 6-17 years with corrected moderate-complex CHD without syndromes that may affect neurocognition were recruited from the pediatric cardiology clinic. Participants underwent home sleep testing and neurocognitive testing including a validated IQ test as well as validated tests of memory (Paired Associates Learning test), executive function (Intra-Extra Dimensional set shift test) and attention (Simple Reaction Test) from the CANTAB neurocognitive testing battery. RESULTS Complete results were available for 30 children. Seventeen children (57%) were found to have OSA. Total IQ was markedly lower in children with CHD and comorbid OSA compared to children with CHD without comorbid OSA (mean 86 ± 12, vs 98 ± 11, p=0.01). Children with CHD and OSA did significantly worse on the Paired Associates Learning test, with a median of 8 total errors (interquartile range [IQR] 2.25-15) compared to children with CHD without OSA (median total errors 2, IQR 1-8), p=0.02. INTERPRETATION Children with CHD and comorbid OSA have impaired neurocognition compared to children with CHD without comorbid OSA. OSA may be a reversible cause of neurocognitive impairment in children with CHD. Further research is needed to evaluate the effects of OSA treatment on neurocognitive impairment in children with CHD. Methamphetamine (METH) is a major public health and safety problem worldwide. METH is psychostimulant that activates microglia via the toll-like receptor (TLR) 4/MD2 complex, modulating the abundant production of pro-inflammatory cytokines in the central nervous system (CNS). The TLR4/MD2 complex on the surface of microglia recognizes pathogen-associated molecular patterns such as lipopolysaccharide (LPS) resulting in brain tissue inflammation and neuronal damage. Since METH has been associated with microglia-induced neurotoxicity, we hypothesized that METH impairs the expression of TLR4 and activation of NF-κB in NR-9460 microglia-like cells after LPS challenge. We demonstrated that METH decreases the distribution and expression of TLR4 receptors on the surface of microglia-like cells after incubation with endotoxin. Moreover, METH impairs the TLR4/MD2 complex signaling pathways, compromises the activation of NF-κB, and reduces the production of pro-inflammatory mediators in microglia-like cells upon LPS stimulation. Interestingly, microglia-like cells treated with METH and challenged with LPS showed considerable cellular morphological changes including enlarged nuclei and ruffled surface. Our results suggest that METH may have a significant impact on microglial-induced neuroinflammation, neurotoxicity, and the CNS defense against infection. It also highlights the importance of studying the effects of METH on the molecular and cellular components of users' CNS immunity. While the conventional biomarkers for diabetes have been glucose and insulin concentrations in the blood, the direct determination of the pancreatic β cell mass would provide critical information for the disease status and progression. By combining fluorination and diversity-oriented fluorescence library strategy, we have developed a multimodal pancreatic β cell probe PiF for both fluorescence and for PET (positron emission tomography). By simple tail vein injection, PiF stains pancreatic β cells specifically and allows intraoperative fluorescent imaging of pancreatic islets. PiF-injected pancreatic tissue even facilitated an antibody-free islet analysis within 2 h, dramatically accelerating the day-long histological procedure without any fixing and dehydration step. Not only islets in the pancreas but also the low background of PiF in the liver allowed us to monitor the intraportal transplanted islets, which is the first in vivo visualization of transplanted human islets without a prelabeling of the islets. All of the compounds show delayed fluorescence under ambient conditions and persistent phosphorescence at low temperature. Solvent-dependent studies and temperature-dependent luminescence measurements established that quadrupolar (2 and 5) and octupolar (3 and 6) compounds underwent symmetry breaking in the excited state. Curiously, delayed fluorescence and phosphorescence spectra are found to be blue-shifted and follow the same trend as the fluorescence upon an increase in the branches. Using numerical and experimental studies, we demonstrate the existence of a critical heat transfer coefficient that dramatically affects the design and performance of TEGs. Results provide a set of concrete design criteria for developing efficient TEGs that meet the metrics for field deployments. High-performance TEGs demonstrated in this study generated up to 28% higher power and 162% higher power per unit mass of thermoelectric materials as compared to the commercial module deployed for low-grade waste heat recovery. Most importantly, a correlative EBSD/electron beam-induced current (EBIC) experiment reveals that LAGBs are carrier recombination centers and that the magnitude of recombination is dependent on the degree of misorientation. The presented results directly link increased LAGB misorientation to degraded device performance, and therefore, strategies to reduce LAGB misorientations and densities would improve highly oriented semiconductor devices.An inexpensive, solution phase modification of flat carbon electrodes by electrochemical reactions of a 1,8-diaminonaphthalene derivative results in a 120- to 700-fold increase in capacity by formation of a 15-22 nm thick organic film. Modification of high surface area carbon electrodes with the same protocol resulted in a 12- to 82-fold increase in capacity. The modification layer contains 9-15% nitrogen present as -NH- redox centers that result in a large Faradaic component involving one H+ ion for each electron. The electrodes showed no capacity loss after prolonged cycling in 0.1 M H2SO4 and exhibited significantly higher charge density than similar reported electrodes based on graphene and polyaniline. Investigation of the deposition conditions revealed that N-doped oligomeric ribbons are formed both by diazonium ion reduction and diaminonaphthalene oxidation, and the 1,8 isomer is essential for the large capacity increases. The capacity increase has at least three contributions increased microscopic surface area from ribbon formation, Faradaic reactions of nitrogen-containing redox centers, and changes in ribbon conductivity resulting from polaron formation.

The aim of the present study was to examine the changes found on magnetic resonance imaging (MRI) studies of the TMJ in patients with skeletal Class II malocclusion who had undergone combined orthodontic and bilateral sagittal split ramus osteotomy (BSSRO) advancement. Our objective was to measure the changes in the disc position, condylar translation, secondary bony changes, and joint effusion on MRI before and after treatment. MATERIALS AND METHODS An analytical, single-surgeon, single-institution, retrospective radiological (MRI) study was designed. We included patients who had undergone combined orthodontic and BSSRO advancement from 2011 to 2018. All 36 patients were examined using a 1.5-Tesla MRI unit (Siemens Symphony, Erlangen, Germany) with a 6 × 8-cm diameter surface coil, which allowed for simultaneous imaging of both TMJs. RESULTS Analysis using the Wilcoxon signed rank test revealed statistically significant differences in the pre- and post-treatment groups in the changes in the position of the disc from anterior disc displacement with reduction (ADDWR) to the normal position (P = .008), condylar translation from excessive to normal (P = .046), and an increase in secondary bony changes (P = .005). CONCLUSIONS Combined orthodontic and orthognathic movement in the treatment of skeletal Class II malocclusion can increase secondary bony changes, improve the disc position in ADDWR cases, and control excessive translation of the TMJ. No improvement was noted in the position of the disc in those with anterior disc displacement without reduction, hypomobility and joint effusion. PURPOSE Zygomaticomaxillary complex (ZMC) fractures occur often. However, no clinical consensus has been reached regarding the number of fixation points required when performing open reduction and internal fixation (ORIF). The objective of the present study was to explore the utility of single-point fixation in the management of noncomminuted ZMC fractures. PATIENTS AND METHODS We analyzed the data from a retrospective case series of 211 patients treated during a 20-year period. RESULTS The mean length of follow-up was 3.4 months. Of the 211 patients, 162 with noncomminuted ZMC fractures had been treated with single-point fixation of the zygomaticomaxillary buttress. During the follow-up period, 1 patient experienced tooth loss because of a root present in the fracture line, 7 experienced intraoral plate exposure, with 2 subsequently undergoing plate exchange, and 8 developed a wound infection. No patients required orthognathic surgery or cheek implants for malar asymmetry. No patient developed hypoglobus or enophthalmos, and none required revision ORIF of their ZMC fracture. CONCLUSIONS To the best of our knowledge, the present study represents the largest series in the literature reporting the surgical results and outcomes of patients with noncomminuted ZMC fractures treated with single-point fixation. In experienced hands, we believe this is a viable surgical option if appropriate surgical considerations are made. PURPOSE Cone-beam computed tomography (CBCT) is commonly requested before dental implant treatment for the anatomic assessment of the inferior alveolar canal (IAC) to prevent its neurovascular content from being traumatized. CBCT images can be saved in different types of resolutions and bit depths; these parameters may significantly affect the diagnostic accuracy of images. This study aimed to assess the effect of resolution and bit depth on IAC visualization on exported mandibular CBCT images. MATERIALS AND METHODS Forty-one mandibular CBCT images of differing image resolutions and voxel sizes (0.16, 0.32, and 0.48 mm) and differing bit depths (12 and 15) were exported from a software program as a single file. Two observers evaluated the cross-sectional images in terms of IAC visibility using a 3-point scale (good, moderate, and poor). Disagreements were resolved by including a third observer, and the highest agreement was recorded. RESULTS Study interobserver agreement was acceptable (84.2%) for IAC observation. The percentage of IAC observation was from 84.1 to 100% with the 12- and 15-bit depths, with a constant image resolution of 0.16 and 0.32 mm, respectively. A significant difference (from 19.5 to 48.8%) was noted between the 2 bit depths in the percentage of good IAC visualization with a constant resolution of 0.48 mm. Reduction in the image resolution to 0.48 mm showed a significant difference (19.5 to 100%) between the 12- and 15-bit depths in good IAC visualization. CONCLUSIONS Exporting the mandibular CBCT images with 0.32 mm of resolution and a 12-bit depth will produce good and moderate radiographic IAC observation with the benefit of a smaller file size. PURPOSE The application of bio-resorbable plates in craniomaxillofacial surgery is increasing because of the advantage of avoiding secondary surgery. This study aimed to evaluate the effects of osteosynthesis with prebent bio-resorbable plates for treating zygomaticomaxillary complex (ZMC) fractures. MATERIALS AND METHODS We implemented a prospective case series composed of patients with ZMC fractures who underwent treatment at the School of Stomatology at China Medical University. Bio-resorbable plates were used for fracture fixation. The fractures were stabilized with bio-resorbable plates prebent on a 3-dimensionally printed skull model with the fractures reduced using virtual simulation. The primary outcome variable was the stability rate of reduced bone segments. Other study variables were mouth opening, occlusion, paresthesia or anesthesia in the infraorbital nerve region (PAIN), and diplopia. Outcome variables were determined by calculating stability rates of reduced bone segments, resolution rates of postoperative restricted mouth opening, malocclusion, PAIN, and diplopia.

The objective of the present study was to explore the utility of single-point fixation in the management of noncomminuted ZMC fractures. PATIENTS AND METHODS We analyzed the data from a retrospective case series of 211 patients treated during a 20-year period. RESULTS The mean length of follow-up was 3.4 months. Of the 211 patients, 162 with noncomminuted ZMC fractures had been treated with single-point fixation of the zygomaticomaxillary buttress. During the follow-up period, 1 patient experienced tooth loss because of a root present in the fracture line, 7 experienced intraoral plate exposure, with 2 subsequently undergoing plate exchange, and 8 developed a wound infection. No patients required orthognathic surgery or cheek implants for malar asymmetry. No patient developed hypoglobus or enophthalmos, and none required revision ORIF of their ZMC fracture. CONCLUSIONS To the best of our knowledge, the present study represents the largest series in the literature reporting the surgical results and outcomes of patients with noncomminuted ZMC fractures treated with single-point fixation. In experienced hands, we believe this is a viable surgical option if appropriate surgical considerations are made. PURPOSE Cone-beam computed tomography (CBCT) is commonly requested before dental implant treatment for the anatomic assessment of the inferior alveolar canal (IAC) to prevent its neurovascular content from being traumatized. CBCT images can be saved in different types of resolutions and bit depths; these parameters may significantly affect the diagnostic accuracy of images. This study aimed to assess the effect of resolution and bit depth on IAC visualization on exported mandibular CBCT images. MATERIALS AND METHODS Forty-one mandibular CBCT images of differing image resolutions and voxel sizes (0.16, 0.32, and 0.48 mm) and differing bit depths (12 and 15) were exported from a software program as a single file. Two observers evaluated the cross-sectional images in terms of IAC visibility using a 3-point scale (good, moderate, and poor). Disagreements were resolved by including a third observer, and the highest agreement was recorded. RESULTS Study interobserver agreement was acceptable (84.2%) for IAC observation. The percentage of IAC observation was from 84.1 to 100% with the 12- and 15-bit depths, with a constant image resolution of 0.16 and 0.32 mm, respectively. A significant difference (from 19.5 to 48.8%) was noted between the 2 bit depths in the percentage of good IAC visualization with a constant resolution of 0.48 mm. Reduction in the image resolution to 0.48 mm showed a significant difference (19.5 to 100%) between the 12- and 15-bit depths in good IAC visualization. CONCLUSIONS Exporting the mandibular CBCT images with 0.32 mm of resolution and a 12-bit depth will produce good and moderate radiographic IAC observation with the benefit of a smaller file size. PURPOSE The application of bio-resorbable plates in craniomaxillofacial surgery is increasing because of the advantage of avoiding secondary surgery. This study aimed to evaluate the effects of osteosynthesis with prebent bio-resorbable plates for treating zygomaticomaxillary complex (ZMC) fractures. MATERIALS AND METHODS We implemented a prospective case series composed of patients with ZMC fractures who underwent treatment at the School of Stomatology at China Medical University. Bio-resorbable plates were used for fracture fixation. The fractures were stabilized with bio-resorbable plates prebent on a 3-dimensionally printed skull model with the fractures reduced using virtual simulation. The primary outcome variable was the stability rate of reduced bone segments. Other study variables were mouth opening, occlusion, paresthesia or anesthesia in the infraorbital nerve region (PAIN), and diplopia.
