  • Whereas the COVID-19 pandemic has changed our lives worldwide, we hope that vaccination can combat the disease. We propose how to evaluate suspected severe allergic reactions to the vaccines so that as many as possible may be safely vaccinated.

    The aim of this observational study was to determine whether bilateral bispectral index (BIS) monitoring can detect seizures in epileptic patients.

    Four-channel frontal BIS monitoring and standard 40-channel electroencephalography monitoring were conducted in epileptic patients undergoing evaluation for epilepsy surgery. The BIS numerical value, signal quality index, electromyography, suppression ratio, and color density spectral array were continuously recorded. In patients with electroencephalography-confirmed seizures, the mean value and trend (slope of linear regression) of bilateral BIS monitor parameters were analyzed from 1 minute before to 1 minute after seizure onset.

    Of 48 patients included in the study, 21 (43.8%) had at least 1 seizure. BIS numerical value was not able to detect focal or focal to bilateral tonic-clonic seizures. Considering all seizures, the only significant differences between recordings 1 minute before and 1 minute after seizure onset were a decrease in the signal quality index slope from 1 hemisphere (0.039±0.297 vs. -0.085±0.321, respectively; P=0.029) and in the mean signal quality index recorded from both hemispheres (left hemisphere 65.775±30.599 vs. 61.032±26.285; P=0.016 and right hemisphere 63.244±31.985 vs. 59.837±27.360; 0.029); these differences were not maintained after Hochberg adjustment for multiple comparisons. In seizures occurring during sleep, there was a change in the electromyography slope of 1 hemisphere before and after seizure onset (-0.141±0.176 vs. 0.162±0.140, respectively; P=0.038). There were variable responses in BIS parameters in the 3 patients who developed focal nonconvulsive seizure clusters.

    The use of thiopurine therapy in Epstein-Barr virus (EBV)-naïve inflammatory bowel disease (IBD) patients remains controversial due to a risk of EBV-associated complications. We evaluated EBV status and outcomes within our paediatric IBD population over an 8-year period; finding that 217/409(53%) screened patients were seropositive for EBV at IBD diagnosis; that thiopurines were used in 189/217(87%) seropositive and 159/192(83%) seronegative patients (p = 0.22); and that 7/192(4%) previously seronegative patients subsequently tested positive for EBV with 6/7(86%) patients having concurrently recorded thiopurine use. All 6 patients continued thiopurine with/without a period of cessation; no EBV-associated lymphoproliferative disorders/serious complications were recorded within our cohort. A significant proportion of our patients would not receive thiopurine therapy should their use be avoided in EBV-negative patients (47%) or seronegative males (30%).     Tofacitinib, a selective Janus kinase inhibitor, effectively induces and maintains remission in adults with inflammatory bowel disease (IBD), but data are limited in children. This study aimed to evaluate the efficacy and safety of tofacitinib for medically-refractory pediatric-onset IBD.

    This single-center retrospective study included subjects aged 21 years and younger who started tofacitinib for medically-refractory IBD. Clinical activity indices, clinical response, steroid-free remission, biochemical response, and adverse events (AEs) were evaluated over 52 weeks.

    Twenty-one subjects, 18 with ulcerative colitis or indeterminate IBD, received tofacitinib. At the end of the 12 week induction period, 9/21 (42.9%) subjects showed clinical response and 7/21 (33.3%) were in steroid-free remission. Of evaluable subjects at 52 weeks, 7/17 (41.2%) showed clinical response and were in steroid-free remission. Of those remaining on tofacitinib at one year, none required concomitant systemic corticosteroids. Tofaung individuals with moderate-to-severe IBD. Larger, well-powered, prospective studies are warranted.
    The objective of this study was to assess the tolerance and efficacy of endoscopic intrapyloric botulinum toxin injection compared with pyloric dilatation in children with gastroparesis.

    This was a retrospective descriptive multicentre study that included pediatric patients treated between 2010 and 2018 at four tertiary hospitals.

    Data were collected for 24 patients. The median age at diagnosis was 2.5 years (range 0.5-4.7). A total of 46 endoscopic procedures were performed. GSEA analysis showed that some tumor-related pathways were enriched in the high-risk group, while metabolism-related pathways were enriched in the low-risk group. This examination unveiled NAA40 as the NAT family member with the most prominent upregulation and significant disease prognosis for this cancer. Focusing on this enzyme, which selectively targets histone proteins, we show that its upregulation occurs from early stages of LIHC and is not specifically correlated with any established risk factors such as viral infection, obesity or alcoholic disease. Notably, in silico analysis of TCGA and other LIHC datasets found that expression of this epigenetic enzyme is associated with high proliferating, poorly differentiating and more aggressive LIHC subtypes. In particular, NAA40 upregulation was preferentially linked to mutational or non-mutational P53 functional inactivation. Accordingly, we observed that high NAA40 expression was associated with worse survival specifically in liver cancer patients with inactivated P53. Based on correlation analysis of integrated scRNA-seq and bulk RNA-seq datasets from patients, we confirmed that myeloid-derived S100A9 was involved in TNFSF13B-dependent myeloma cell proliferation and survival. In the animal experiments, S100A9 was found to be critical for MM cell proliferation and survival via TNFSF13B production by myeloid cells, neutrophils, and macrophages. In-vitro analysis of patient primary myeloma cells further demonstrated that enhanced TNFSF13B signaling triggered the canonical NF-κB pathway to boost tumor cell proliferation. All these results suggest that myeloid-derived S100A9 is required for TNFSF13B/TNFRSF13B-dependent cell-fate specification, which provides fresh insights into MM progression.

  • Most patients presented with myalgias/arthralgias (first dose, 7.7%; second dose, 14.6%), fatigue (first dose, 15.4%; second dose, 29.2%), and injection site pain (first dose, 40.4%; second dose, 43.8%). The aim of this study was to determine the safety and efficacy of reduced-intensity peripheral blood stem cell transplantation (PBSCT) from a human leukocyte antigen (HLA)-haploidentical donor using PTCy as graft-versus-host disease (GVHD) prophylaxis. This was a prospective, multicenter phase I/II study (UMIN000021783) conducted at 16 hospitals in Japan. The primary endpoint was the probability of survival with engraftment and without grade III/IV acute GVHD at day 60 after PBSCT. The expected probability of the primary endpoint was estimated to be 60%, and the threshold probability was set at 30% on the basis of previous studies.     We conducted a systematic review to identify RCTs (up to October 21, 2020) evaluating drugs or blood products to treat or prevent Covid-19. We extracted trial characteristics (number of centers, funding sources, and sample size) and assessed risk of bias (RoB) using the Cochrane RoB 2.0 tool. We performed logistic regressions to evaluate the association between RoB due to randomization, single vs. multicentre, funding source, and sample size, and finding a statistically significant effect.

    We included 91 RCTs (n=46,802); 40 (44%) were single-center, 23 (25.3%) enrolled <50 patients, 28 (30.8%) received industry funding, and 75 (82.4%) had high or probably high RoB. Thirty-eight trials (41.8%) reported a statistically significant effect. RoB due to randomization and being a single-center trial were associated with increased odds of finding a statistically significant effect.

    There is high variability in RoB among Covid-19 trials. Researchers, funders, and knowledge-users should be cognizant of the impact of RoB due to randomization and single-center trial status in designing, evaluating, and interpreting the results of RCTs.

    Most patients presented with myalgias/arthralgias (first dose, 7.7%; second dose, 14.6%), fatigue (first dose, 15.4%; second dose, 29.2%), and injection site pain (first dose, 40.4%; second dose, 43.8%). Hypertension was defined as taking medication to treat hypertension, having a systolic blood pressure ≥ 140, or a diastolic blood pressure ≥ 90. Modified Poisson regression was performed to estimate the prevalence ratios (PR) and corresponding 95% confidence intervals (CI) for race/ethnicity, as it relates to hypertension controlling for potential confounders. RESULTS Of the 524 breast cancer survivors included in our study, 107 (20.4%) were African American and 417 (80.0%) were White. After adjusting for age, marital status, education, annual household income, health insurance, smoking and drinking status, physical inactivity, obesity, and diabetes, African American breast cancer survivors had a 30% higher prevalence of hypertension (PR = 1.30 [95% CI, 1.11-1.52]) than White breast cancer survivors. After extensive trials, we have been successful in determining a series of high-resolution structures of Nav channels, including NavPaS from American cockroach, Nav1.4 from electric eel, and human Nav1.1, Nav1.2, Nav1.4, Nav1.5, and Nav1.7, with distinct strategies. These structures established the framework for understanding the electromechanical coupling and disease mechanism of Nav channels, and for facilitating drug discovery.   • The early actions enacted under the state-of-emergency carve new directions in negotiating the delicate balance between freedom of expression and online censorship, in particular by imposing limitations on access to information and inducing self-restraint in reporting. Based on comparative legal analysis, this article provides a timely discussion of intended and unintended consequences of such legal responses to the "infodemic," reflecting on a basic set of safeguards needed to preserve trust in online information.At the time of writing (mid-May 2020), mental health charities around the world have experienced an unprecedented surge in demand. At the same time, record-high numbers of people are turning to social media to maintain personal connections due to restrictions on physical movement. But organizations like the mental health charity Mind and even the UK Government have expressed concerns about the possible strain on mental health that may come from spending more time online during COVID-19. These concerns are unsurprising, as debates about the link between heavy social media use and mental illness raged long before the pandemic. But our newly heightened reliance on platforms to replace face-to-face communication has created even more pressure for social media companies to heighten their safety measures and protect their most vulnerable users. To develop and enact these changes, social media companies are reliant on their content moderation workforces, but the COVID-19 pandemic has presented them with two related conundrums (1) recent changes to content moderation workforces means platforms are likely to be less safe than they were before the pandemic and (2) some of the policies designed to make social media platforms safer for people's mental health are no longer possible to enforce. This Social Media + Society 2K essay will address these two challenges in depth.One of the most striking features of the COVID-19 pandemic in the United Kingdom has been the disproportionate way in which it has affected Black, Asian, ethnic minority, and working class people. In this article, I argue that digital technologies and data practices in the response to COVID-19 amplify social inequalities, which are already accentuated by the pandemic, thus leading to a "second-order disaster"-a human-made disaster which further traps disadvantaged people into precarity. Inequalities are reproduced both in the everyday uses of technology for distance learning and remote work as well as in the public health response. Applications such as contact tracing apps raise concerns about "function creep"-the reuse of data for different purposes than the one for which they were originally collected-while they normalize surveillance which has been traditionally used on marginalized communities. The outsourcing of the digital public health response consolidates the arrival of the privatized digital welfare state, which increases risks of potential discrimination.Since the coronavirus outbreak, YouTube has altered its content moderation policies to surface more "authoritative information" while removing videos that contain "medically unsubstantiated claims." This was made urgent by incidents like a live-stream interview of renowned British conspiracy theorist David Icke-in which he falsely linked the spread of the coronavirus to 5G technology-that gained substantial traction online. Behind these events, however, lies a tension between the need for authoritative medical information and the socio-technical mediation that enables multiple, competing voices to lay claim to such authority on YouTube; a tension exacerbated by the current pandemic. Following an investigation into the sources and types of video content average users were likely to see when searching for information about the coronavirus on the site, we suggest that through its incentive structure and participatory affordances, YouTube may have subordinated expertise to a logic of likability-leaving institutional experts trailing behind.All of the authors who have contributed to this issue on COVID19 and the Media have stepped out of the rush of change, if only for a 2000-word moment, to look into a corner of the mediated pandemic and grapple with the significance of what's happening in our field, our societies, our lives. Although reporting and widely shared data visualizations about COVID-19 have made many people newly aware that their movements are being tracked and surveilled, governments are already implementing new measures such as geofencing and artificial intelligence (AI)-based facial recognition to facilitate the enforcement of social distancing. The tracking of COVID-19 spread and social distancing behaviors of the public has made more visible the practices of biopolitics but also generated new opportunities for even greater surveillance and control. The current moment offers an opportunity to shift public perceptions about data surveillance, technological control, and the racial disparities of biopower, **** in the same way that public perceptions around social media shifted during and after the Arab Spring. How we collectively respond to these biopolitical processes will, in part, determine how such power relations are articulated in the future.As COVID-19 is rapidly spreading around the world, some countries have launched or plan to implement contact-tracing apps to detect exposure risks. In China, the government relies on Health Code, developed by Alipay and WeChat, for identifying people potentially exposed to COVID-19.   • The PD classification and corresponding treatment strategies may serve as a better option for the clinical treatment of condylar fractures in children.Oral squamous cell carcinoma (OSCC) remains the most common cancer among males in Sri Lanka. Metastasis to neck is a crucial prognostic factor. A modified radical/radical neck dissection including levels I -V, was performed in patients with OSCC who had a clinically positive neck (cN+). Currently, evidence suggests that sparing level V in a cN+ may be justified due to less chance of metastasis in early stages of the disease. To the best of our knowledge, the incidence of metastasis to level V in patients with cN+s has not been previously investigated in a Sri Lankan context. We aimed to determine level V lymph node metastasis and related clinicopathological indicators in cN+s in patients with OSCC. A multicentre retrospective study investigated postoperative biopsy reports of 187 patients for five years. OSCC patients with cN+s who underwent neck dissections of levels I-V were included. Only 6.4% of patients had histopathologically positive level V lymph nodes. A total of 127 lymph nodes were harvested from level V of those who showed level V positivity and out of them 68 were positive with a third of cases showing extranodal extension (ENE). The buccal mucosa (n=4) and lateral aspect of the anterior two thirds of the tongue (n=4) were the common primary sites for level V metastasis. In patients who showed positivity in levels III and IV, a considerably higher probability of level V nodes being positive was seen, which was statistically significant (p = 0.0001). We have concluded that the routine performance of a modified radical/radical neck dissection for cN+s should be stopped, as the incidence of Level V positivity is significantly low. Assessing the cN+ for N stage, status of levels III and IV, pattern of invasion, differentiation, and the site may be used instead as predictors for level V positivity.Genotype MTBDRsl [SL-LPA] was endorsed as a tool for early diagnosis of fluoroquinolones (FQ) and injectable second-line TB drugs (SLID) resistance in DR-TB. No significant differences were observed for rs1800682 in FAS between the study groups. In contrast, rs763110 CC genotype of FASL was found in 35.7% of group I females, and in 50.5% of group II (p = 0.0027; OR = 1.83 (95% CI = 1.21-2.79)). When only females infected with high-risk HPV were analysed, these differences were even higher (p = 0.0024; OR = 2.21 (95% CI = 1.30-3.75)). We argue that the relatively low level of medical authority is a significant contributor to the problem.     Pancreaticopleural fistula (PPF) is a rare complication of chronic pancreatitis (CP) that requires a high index of clinical suspicion in the patient who presents with a pleural effusion. Visualizing the fistula tract from the pancreatic duct to the pleural space by radiological imaging provides confirmation of this complication.

    A 9-year-old boy who presented with massive right pleural effusion secondary to PPF, a complication of CP from a genetic mutation involving two mutations of SPINK1. We successfully managed the case with by endoscopic pancreatic duct stent placement after failure of conservative treatment approaches.
