These methodologies were further utilized for appending porphyrin moieties onto the BCP core.1,3-Dipolar cycloaddition of 2- and 3-nitrobenzaldehydes with 2-aminomethylpyridine and ethyl (2E)-2-cyano-3-(4-nitrophenyl)prop-2-enoate yielded endo-cycloadducts as the sole products under various reaction conditions. Fortuitously, 4-nitrobenzaldehyde behaved differently in three- and four-component cascades to produce a mixture of endo- and exo'-cycloadducts. This direction of research is inspired by biological systems where multiple pores with different functionalities embedded in a cell membrane transmit external signals and underlie all physiological processes. In this manuscript, we describe the modeling of ion transport through small arrays of nanopores consisting of 3, 6, and 9 nanopores and an integrated gate electrode placed on the membrane surface next to one pore opening. We show that by tuning the gate voltage and strategically placing nanopores with nonlinear current-voltage characteristics, the local signal at the gate affects ionic transport through all nanopores in the array. Conditions were identified when the same gate voltage induced opposite rectification properties of neighboring nanopores. We also demonstrate that an ionic diode embedded in a nanopore array can modulate transport properties of neighboring pores even without a gate voltage. The results are explained by the role of concentration polarization and overlapping depletion zones on one side of the membrane. The modeling presented here is intended to become an inspiration to future experiments to create nanopore arrays that can transduce signals in space and time.Omecamtiv mecarbil (OM), currently investigated for the treatment of heart failure, is the first example of a new class of drugs (cardiac myotropes) that can modify muscle contractility by directly targeting sarcomeric proteins. Using atomistic molecular dynamics simulations, we show that the binding of OM to the pre-power stroke state of cardiac myosin inhibits the functional motions of the protein and potentially affects Pi release from the nucleotide binding site. We also show that the changes in myosin ATPase activity induced by a set of OM analogues can be predicted from their relative affinity to the pre-power stroke state compared to the near rigor one, indicating that conformational selectivity plays an important role in determining the activity of these compounds.

  • In on-farm hatching systems, eggs are transported at d 18 of incubation to the broiler farm, where chickens have immediate access to feed and water after hatching. In hatchery-fed systems, newly hatched chickens have immediate access to feed and water in the hatchery and are transported to the farm thereafter. Conventionally hatched chickens can remain without access to feed and water up to 72 h after hatching until placement on the farm. The current study compared day-old chicken quality, performance, and slaughter yield of broiler chickens that were on-farm hatched (OH), hatchery-fed (HF), or conventionally hatchery-hatched (HH). The experiment was performed in 6 rooms in 1 house. Each room contained 2 duplicate pens with approximately 1,155 chickens per pen; 2 rooms with each 2 duplicate pens were assigned to 1 treatment. Results showed that HF and OH chickens were heavier and longer than HH chickens at day (D) 1. Relative weight of stomach and intestines were highest for OH chickens. The trial lasted for 8 wk. Compared with average egg-laying rate broiler breeders, LELR broiler breeders had lower laying rate and qualified egg rate (P less then 0.05), higher egg weight and feed conversion ratio (P less then 0.05), higher malondialdehyde (MDA) levels in the jejunum (P less then 0.05), higher IL-6 (P less then 0.05) and tumor necrosis factor α (TNF-α) (P = 0.07) mRNA expressions in the jejunal mucosa, and lower microflora diversity in cecal digesta. Dietary POS supplementation increased egg weight of broiler breeders (P less then eders with different egg-laying rates.This study was conducted to evaluate the effects of the fiber source (wheat bran [WB] or sugar beet pulp [SBP]) and xylanase supplementation on production, egg quality, ileal digestibility, intestinal morphology, and gastrointestinal pH in aged laying hens. A total of 540 laying hens (Lohman LSL Lite; 70 wk of lay) were randomized into 10 treatments (6 replicate cages of 9 birds) consisting of a corn soy control supplemented with 0, 3, or 6% WB or SBP with or without xylanase (100 mg of xylanase preparation per kg) for a period of 9 wk in a 5 × 2 factorial arrangement. Hens fed with the diets containing either of the levels of SBP or 6% WB had lower hen-day production, and addition of the enzyme improved hen-day production (P less then 0.05), but it could not compensate for the lost production due to the higher levels of either of the fiber sources. Supplementation of 6% SBP to the control diet decreased egg mass (P less then 0.05). All fiber-supplemented diets significantly decreased ADFI, which was restility.
    Over the past two decades, the average age of hip fractured patients has increased, patients are increasingly fragile and their management is more complex. Most of the literature suggest that care improvement lowered short-term mortality but there is no clear evidence whether mid- and long-term mortality rates are improving. The aim of this study was to evaluate the variations in comorbidities in hip fractured patients over 15 years, the changes in mortality and identify the predictive factors for mortality for identifying the patients at higher risk.

    Hip fractured patients admitted in hospital in 2000-2001 (192 patients) and 2015-2016 (323 patients) were retrospectively reviewed. Demographic, clinical and management data from the two cohorts were compared. Thirty-day and 1-year mortality were calculated and compare between the two cohorts. A multivariate logistic regression model were performed to identify the most significant predictors of mortality.

    After fifteen years, mean age of hip fracture patierbidities were hypertension, COPD, diabetes, arrhythmia, renal impairment and dementia. In the 2015-2016 cohort, the age-adjusted mortality at 30 days significantly declined compared to the 2000-2001 cohort (respectively 6.9% vs. 12.5%) but the age-adjusted mortality at 1-year was equivalent. Older age, reduced mobility, higher comorbidity, lateral fractures and male sex were significant risk factors for reduced survival time CONCLUSIONS After 15 years, there was a significant improvement in 30-days mortality in hip fractured patients despite their increase in comorbidities but this advantage was not observed in 1-year mortality. This suggests the need to implement targeted and longer-term care support for males, older patients and those with greater comorbidities which are at higher risk.
    Hip fracture is a common and serious injury in the elderly. Hip arthroplasty is the most frequently performed procedure for patients with an intracapsular hip fracture. The majority of national guidelines recommend total hip arthroplasty (THA) for more active patients. Literature indicates significant stability advantages for dual mobility (DM) acetabular components in non-emergent scenarios. Evidence supporting the use of DM in hip fracture patients is limited.

    We set out to ascertain if DM implants offer stability and/or functional advantages over standard THA in patients with hip fracture.

    We utilised our local National Hip Fracture Database to identify all patients undergoing either a standard or DM THA for hip fracture (n=477) We matched cohorts based on age, AMTS, mobility status pre-operatively, gender, ASA and source of admission. Our primary outcome of interest was functional status using the oxford hip score (OHS). Secondary outcome measures included dislocation, fracture and deep infection requiring further surgery.

    Left atrial appendage closure (LAAC) using the LAmbre device has been associated with prevention of stroke in patients with nonvalvular atrial fibrillation (AF). Here, we interrogated the long-term safety and efficacy of using the LAmbre device in percutaneous LAAC.

    We analyzed 56 records of patients with nonvalvular AF undergoing LAAC procedures with the LAmbre device. We collected and analyzed the data to define the safety and efficacy of the LAmbre device implantation.

    The LAAC was successfully occluded in the 56 patients. Our data showed no serious residual leak or pericardial effusion occurred during the perioperative period. At a mean follow-up of 37.8 ± 23.5 months, there were 7.1%, 3.6%, and 3.6% rates of death, stroke, and device-related thrombus, respectively. There were no cases of severe residual leak or systemic embolism.

    Taken together, we demonstrate that execution of LAAC with the LAmbre device has high procedural success and prevents AF-related stroke. However, further large-scale trials might be required to confirm our findings.

The World Health Organization's QOL Assessment-BREF (WHOQOL-BREF) questionnaire was administered among study participants. The questionnaire addresses four domains of QOL which are physical, psychological, social relationships, and environment.

    There were 313 dental professionals in the study with a mean age of 35.72 (8.58) years. The mean score of QOL in the sample was 63 (13.9) on a 0-100 scale. 75% of the participants rated their QOL as good or very good. Of four domains, the social relationship domain had the highest mean score of QOL (67.04 SD 23.52) and the physical domain had the lowest score (59.66 SD 14.69). There were significant differences in the QOL of consultants (66.46 SD 12.55), specialists (65.42 SD 12.91), and general dentists (61.06 SD 14.18) (
     0.010). The participants with medical illness had significantly lower QOL (56.91 SD 12.83) than those without medical illness (63.67 SD 13.92) (
     0.01). There were significant differences in the QOL of participants with 0-10 years since graduation (61.97 SD 14.71), 11-20 years (61.92 SD 13.56), and more than 20 years (68.53 SD 10.71) (
     0.009).

    The qualifications, medical illness, and years since graduation were important determinants of QOL among dental professionals. Measures should be taken to improve QOL of dental professionals which can enhance the quality of patient care.

We retrospectively reviewed the left ventricular opening of the aneurysm (a), diameter of the midsegment of the aneurysm (b), and diameter of the right ventricular opening of the aneurysm (c). If there were multiple defects within the aneurysm, the largest defect was denoted as and so forth. We developed a novel VSD classification method in which pmVSD with aneurysm was classified into three types (A, B, and C). When  >  ≥  , it was classified as type A, when  >   ≥  , it was type B, and when  >   ≥  , it was type C; / described the relationship among defects. We collected and analyzed the data to define the safety and efficacy of the LAmbre device implantation. The LAAC was successfully occluded in the 56 patients. Our data showed no serious residual leak or pericardial effusion occurred during the perioperative period. At a mean follow-up of 37.8 ± 23.5 months, there were 7.1%, 3.6%, and 3.6% rates of death, stroke, and device-related thrombus, respectively. There were no cases of severe residual leak or systemic embolism. Taken together, we demonstrate that execution of LAAC with the LAmbre device has high procedural success and prevents AF-related stroke. Recent research suggests that JDM may not just be the classic antibody driven complements mediated microangiopathy as was thought to be in the past. The etiopathogenesis of JDM also involves inappropriate stimulation of innate immune system followed by dysregulation of the adaptive immune response through dendritic cells. Many variable immune factors such as genetics, major histocompatibility complex expressions, immunohistochemical variabilities, and diversity in specific and associated autoantibodies may make individual IIM and JDM cases unique. The diversity in IIM and JDM also explains individual variability in response to specific therapies. Classifying and matching the right patients to the right treatment is crucial to the successful treatment of these patients with better outcomes. Sub-type specific biologic therapy may be the best current treatment that can match the patient to the best treatment options. A PubMed search was performed to find all the available cases of refractory myositis patients treated with biologics up to July 2020. Using this search this article reviews all the current biologic treatment options and experiences for both adults and children in the context of recent basic science to assist pediatric rheumatologists in choosing the optimal biologic therapy for a child with recalcitrant JDM.
    There is a tremendous growing need to address the burden of geriatric psychiatric disorders. Recent developments relevant to geriatric psychiatry have focused on Alzheimer's disease (AD), severe/refractory depression, and cancer/end of life care.

    This is a non-systematic, narrative review (databases and websites for search PubMed, Google Scholar, Medscape, ClinicalTrials.gov; focusing on the last 6years), and covers developments in disease-modifying therapies for AD, diagnostic radiotracers for AD, medications for neuropsychiatric symptoms of dementia, ketamine/esketamine, psychedelics, and cannabinoids.

    The focus of on-going trials of anti-amyloid agents has been on individuals with very early stage AD; several agents are under phase 3 investigation, and aducanumab is under FDA review. Amyloid and tau PET scans have been approved by the FDA to assist in the diagnoses of AD. Promising pharmaceuticals for neuropsychiatric symptoms of dementia include pimavanserin, brexpiprazole, escitalopram, dextromethoc-assisted psychotherapy in end-of-life and cancer-related depression/anxiety. Evidence for the use of cannabinoids is currently lacking.Introduction An effective vaccine against malaria forms a global health priority. Both naturally acquired immunity and sterile protection induced by irradiated sporozoite immunization were described decades ago. Still no vaccine exists that sufficiently protects children in endemic areas. Identifying immunological correlates of vaccine efficacy can inform rational vaccine design and potentially accelerate clinical development.Areas covered We discuss recent research on immunological correlates of malaria vaccine efficacy, including insights from state-of-the-art omics platforms and systems vaccinology analyses; functional anti-parasitic assays; pre-immunization predictors of vaccine efficacy; and comparison of correlates of vaccine efficacy against controlled human malaria infections (CHMI) and against naturally acquired infections.Expert Opinion Effective vaccination may be achievable without necessarily understanding immunological correlates, but the relatively disappointing efficacy of malaria vaccine candidates in target populations is concerning. Despite studies that attempt to distinguish between factors associated with antibiotic use and misuse (e.g., knowledge and beliefs and provider-patient communication), few studies have tested comparative hypotheses related to antibiotic use behavior. This study 1) compares two theoretical models (health belief and patient-centered communication) to learn which best represents the pathways associated with antibiotic use; and 2) describes urban consumers' knowledge, beliefs, and behaviors regarding antibiotic use. Interviewers completed 505 intercept surveys across six clinic- and community-based sites in Southeast Michigan. Structural equation modeling was utilized to compare two competing theoretical models predicting antibiotic behavior. Findings support the assertion that a patient-provider communication model fits the data better than the null model. Descriptive statistical analysis explicated participant knowledge was mixed.   • A 73-year-old Japanese man with a medical history of sarcoidosis was diagnosed with meningitis caused by an undetermined fungus. For further identification, the cerebrospinal fluid sample was analyzed for the rDNA internally transcribed spacer regions, and the fungus was identified as Irpex lacteus. I. lacteus is classified under phylum Basidiomycota and is a wood-rotting bracket mushroom. Although there is no standard treatment regimen for I. lacteus infections, amphotericin B was effective in this patient. Herein, we present, to our knowledge, the first reported case of fungal meningitis caused by I. lacteus, its treatment course, and review relevant published literature.The bone cannot be evaluated using magnetic resonance attenuation correction (MRAC) with the Dixon sequence. To solve this issue, the present study aimed to evaluate model-based AC for whole-body 2-[fluorine-18]-fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography (PET)/magnetic resonance imaging (MRI) by creating bone segmentation. We analyzed and evaluated the data of 31 consecutive patients. The Biograph mMR (Siemens Healthcare) was used for clinical whole-body 18F-FDG PET/MRI with the conventional MRAC method, and OSIRIX MD software was used to analyze the images. After the examination, the new model-based post-processing MRAC was applied to create μ-maps with bone segmentation, and retrospective PET reconstruction was performed using this μ-map. The bone structures of all patients created using model-based MRAC were visually evaluated. Standard uptake values (SUVs) at 11 anatomical positions in PET images, corrected using the μ-map with and without bone segmentation, were measured and compared. The model-based post-processing MRAC was run for all patients, without errors. Visual evaluation revealed that the model-based post-processing MRAC exhibited poor results for six patients. Furthermore, it exhibited an increasing trend of SUV in the brain compared to the conventional method. Locations other than the brain indicated a similar or decreasing trend. The two methods showed a good linear correlation for all patients. However, patients aged less then  20 years exhibited a different trend from those aged ≥ 20 years. We should exercise caution when applying this model-based MRAC for younger patients.Fast kilovoltage (kVp)-switching technology cannot obtain conventional 120 kVp images; thus, 70 keV virtual monochromatic spectral computed tomography (CT) images (VMSI) are generally used. The contrast-to-noise ratio (CNR) is used to evaluate the image quality of VMSI; however, CNR does not include frequency characteristics. The present study aimed to investigate the evaluation methods of VMSI considering frequency characteristics by comparing the image quality of 70 keV VMSI with that of conventional 120 kVp images. The evaluated object contrasts were 70 and 300 Hounsfield units (HU). Scans used two radiation dose levels low (LD) and standard (SD). The volume CT dose index of LD and SD was 4.8- and 12 mGy, respectively. Images were reconstructed by filtered **** projection, evaluating CNR, noise power spectrum (NPS), task transfer function (TTF), and system performance (SP) function calculated as TTF2/ NPS. The total NPS values (spatial frequency range 0.2 ~ 0.4 mm-1) of 70 keV VMSI were higher than those of 120 kVp images. The spatial frequency TTF values that reached 10% (f10%) of the 70 keV VMSI changed based on object contrast. For the low-contrast condition, a lower f10% was observed with 70 keV VMSI. The CNR of 70 keV VMSI was comparable to that of 120 kVp images in low- and high-contrast conditions. However, for 70 keV VMSI, SP of low-contrast was low, and SP of high-contrast was high, compared with those of 120 kVp images.     A 73-year-old Japanese man with a medical history of sarcoidosis was diagnosed with meningitis caused by an undetermined fungus. For further identification, the cerebrospinal fluid sample was analyzed for the rDNA internally transcribed spacer regions, and the fungus was identified as Irpex lacteus. I. lacteus is classified under phylum Basidiomycota and is a wood-rotting bracket mushroom. Although there is no standard treatment regimen for I. lacteus infections, amphotericin B was effective in this patient. Herein, we present, to our knowledge, the first reported case of fungal meningitis caused by I. lacteus, its treatment course, and review relevant published literature.The bone cannot be evaluated using magnetic resonance attenuation correction (MRAC) with the Dixon sequence. To solve this issue, the present study aimed to evaluate model-based AC for whole-body 2-[fluorine-18]-fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography (PET)/magnetic resonance imaging (MRI) by creating bone segmentation. We analyzed and evaluated the data of 31 consecutive patients. The Biograph mMR (Siemens Healthcare) was used for clinical whole-body 18F-FDG PET/MRI with the conventional MRAC method, and OSIRIX MD software was used to analyze the images. After the examination, the new model-based post-processing MRAC was applied to create μ-maps with bone segmentation, and retrospective PET reconstruction was performed using this μ-map. The bone structures of all patients created using model-based MRAC were visually evaluated. Standard uptake values (SUVs) at 11 anatomical positions in PET images, corrected using the μ-map with and without bone segmentation, were measured and compared. The model-based post-processing MRAC was run for all patients, without errors. Visual evaluation revealed that the model-based post-processing MRAC exhibited poor results for six patients. Furthermore, it exhibited an increasing trend of SUV in the brain compared to the conventional method. Locations other than the brain indicated a similar or decreasing trend. The two methods showed a good linear correlation for all patients. However, patients aged less then  20 years exhibited a different trend from those aged ≥ 20 years. The present study aimed to investigate the evaluation methods of VMSI considering frequency characteristics by comparing the image quality of 70 keV VMSI with that of conventional 120 kVp images. The evaluated object contrasts were 70 and 300 Hounsfield units (HU). Scans used two radiation dose levels low (LD) and standard (SD). The volume CT dose index of LD and SD was 4.8- and 12 mGy, respectively. Images were reconstructed by filtered back projection, evaluating CNR, noise power spectrum (NPS), task transfer function (TTF), and system performance (SP) function calculated as TTF2/ NPS. The total NPS values (spatial frequency range 0.2 ~ 0.4 mm-1) of 70 keV VMSI were higher than those of 120 kVp images. The spatial frequency TTF values that reached 10% (f10%) of the 70 keV VMSI changed based on object contrast. For the low-contrast condition, a lower f10% was observed with 70 keV VMSI. The CNR of 70 keV VMSI was comparable to that of 120 kVp images in low- and high-contrast conditions. However, for 70 keV VMSI, SP of low-contrast was low, and SP of high-contrast was high, compared with those of 120 kVp images. At an S/L ratio of 12.5%, multi-objective optimization indicates that pH 1 and a leaching time of 135 min are the parameters that allow both maximum phosphorus yield and minimum ash content.Phytoplankton is capable of responding to aquatic conditions and can therefore be used to monitor freshwater reservoir water quality. Numerous classification techniques, including morpho-functional approaches, have been developed. This study examined changes in phytoplankton assemblages and water quality, which were sampled quarterly from July 2018 to April 2019. The purpose was to contrast the applicability of three classification approaches (functional, morpho-functional and morphological-based functional groupings) for understanding the spatial and seasonal distribution of the biomass variance in phytoplankton functional groups and their driving environmental factors in the ecological zones of the Shanxi Reservoir through multivariate analysis. The results showed that the phytoplankton biomass was highest in the watercourse zone and lowest in the transition zone. Furthermore, the Shanxi Reservoir was characterized by several cyanobacteria (Microcystis spp.) and numerous bacillariophytes (Asterionella sp., Navicula spp. and Aulacoseira granulata). After evaluating the advantages and disadvantages of morpho-functional classifications, we determined that water temperature appeared to be an essential factor, and the morphology-based functional group approach provided the best results for demonstrating phytoplankton succession, despite having lower sensitivity than the others. Nevertheless, these approaches are all appropriate for identifying and monitoring phytoplankton community structure in aquatic systems of reservoirs with complex terrains.This work aims to investigate the electrochemical treatment of petrochemical industry effluents (from the northwest region of Brazil) mediated by active chlorine species electrogenerated at ruthenium-titanium oxide supported in titanium (Ti/Ru0.3Ti0.7O2) and boron doped diamond (BDD) anodes by applying 15 and 45 mA cm-2. Chemical oxygen demand (COD) determinations and toxicity analyses were carried out in order to evaluate the process extension as well as the possible reuse of the wastewater after treatment. Toxicity was evaluated by assessing the inhibition of lettuce (Lactuca sativa) stem growth, seed germination, and the production of nitrite (NO-2) and nitrate (NO-3) species. Results clearly showed that the best COD reduction performances were reached at the BDD anode, achieving almost 100% of removal in a short time. Degradation of nitrogen-organic compounds generated NO-2 and NO-3 which act as nutrients for lettuce. Toxicity results also indicated that the electrogenerated active chlorine species are persistent in the effluent after the treatment, avoiding the stem growth, and consequently affecting the germination.In this study, the removal effect of free and immobilized bacteria on crude oil was determined. Sodium alginate and polyvinyl alcohol were used as embedding agent, and ramie was modified as an adsorbent to immobilize free bacteria. The conditions for preparing immobilized pellets were optimized using the response surface method, and the best combination was simulated and obtained by Design-Expert 8.0. The best degradation rate of immobilized bacteria was 75.52%. As a result, experiments were completed on treated fruit processing wastewater to identify the causes of fouling that originated from the MBR effluent and develop best management practices (BMPs) to minimize fouling of the RO membrane. Physical and chemical analyses along with visual inspection of the membrane surface using scanning electron microscopy (SEM), energy diffusive X-ray (EDX) and Fourier transform infrared (FTIR) spectroscopy were completed. The issue of RO membrane fouling and subsequent flux decline was directly related to the presence of soluble microbial products, specifically dissolved organic matter (DOM) in the MBR effluent. The developed BMPs show that the previously completed enhanced coagulation-GAC sorption process, when combined with an online non-chemical flushing regimen and proper membrane preservation, keeps the flux readings high, resolving frequent fouling and cleaning problems of the RO membrane.Field work was performed to investigate the release of hydrogen sulphide (H2S) and its transport in the sewer trunk with drops in the Bonnie Doon area in Edmonton, Alberta, Canada, in order to develop a proper odor control strategy. The liquid sulfide concentration in the upstream trunk was low (less than 1.0 mg/L), and no H2S gas was detected in the head space under this low concentration. However, high H2S gas concentration was detected in the middle reach of the trunk due to the stripping effect of the three drops (2.7 m, 5.2 m and 2.0 m) along the trunk. The released H2S at drops was then transported in the sewer system and emitted at various locations and caused odor concerns. These drops played an important role in H2S release, and the overall H2S mass transfer coefficient at drops was **** higher than that in normal gravity sewers. The overall oxygen and H2S mass transfer coefficient (KLa) was estimated to be around 200 h-1 and 300 h-1 at the first two drops, respectively. A good alternative is an MBR system to treat the processing wastewater, followed by tertiary treatment using a reverse osmosis (RO) unit to enable water reuse. However, fouling of the RO membrane causes operational challenges. As a result, experiments were completed on treated fruit processing wastewater to identify the causes of fouling that originated from the MBR effluent and develop best management practices (BMPs) to minimize fouling of the RO membrane. Physical and chemical analyses along with visual inspection of the membrane surface using scanning electron microscopy (SEM), energy diffusive X-ray (EDX) and Fourier transform infrared (FTIR) spectroscopy were completed. The issue of RO membrane fouling and subsequent flux decline was directly related to the presence of soluble microbial products, specifically dissolved organic matter (DOM) in the MBR effluent. The developed BMPs show that the previously completed enhanced coagulation-GAC sorption process, when combined with an online non-chemical flushing regimen and proper membrane preservation, keeps the flux readings high, resolving frequent fouling and cleaning problems of the RO membrane.Field work was performed to investigate the release of hydrogen sulphide (H2S) and its transport in the sewer trunk with drops in the Bonnie Doon area in Edmonton, Alberta, Canada, in order to develop a proper odor control strategy. The liquid sulfide concentration in the upstream trunk was low (less than 1.0 mg/L), and no H2S gas was detected in the head space under this low concentration. However, high H2S gas concentration was detected in the middle reach of the trunk due to the stripping effect of the three drops (2.7 m, 5.2 m and 2.0 m) along the trunk. The released H2S at drops was then transported in the sewer system and emitted at various locations and caused odor concerns. These drops played an important role in H2S release, and the overall H2S mass transfer coefficient at drops was much higher than that in normal gravity sewers. The overall oxygen and H2S mass transfer coefficient (KLa) was estimated to be around 200 h-1 and 300 h-1 at the first two drops, respectively. Routine investigations including serum Creatine phosphokinase of both babies were normal. Muscle biopsy showed features of macrophagic myofasciitis in both infants.     The aim of this study was to compare the efficacy and safety of intravenous levetiracetam and fosphenytoin in the management of pediatric status epilepticus.

    This is an open-labeled randomized controlled trial, conducted at tertiary care pediatric intensive care unit. Subjects between 1 month and 18 years who presented with status epilepticus were enrolled. If seizures persisted even after two doses of lorazepam, participants were randomized to receive either fosphenytoin 30 mg/kg or levetiracetam 30 mg/kg intravenously and followed up till 48h, for seizure recurrence and adverse drug effects. Outcome measures were cessation of seizures within 10-20 min following the end of the infusion of drugs fosphenytoin and levetiracetam, respectively, and no recurrence of seizures was noted over next 48h.

    Type I gastric neuroendocrine tumors (GNETs) originate from hyperplasia of enterochromaffin-like (ECL) cells and are commonly detected in patients with chronic atrophic gastritis, including autoimmune gastritis. Typical treatment for type I GNETs comprises simple surveillance and/or endoscopic resection. For alleviation of hypergastrinemia resulting in ECL cell hypertrophy, antrectomy is a treatment option. Type I GNETs mostly have excellent prognosis, and if a surgical approach is chosen, the procedure must be minimally invasive. One such technique for multiple type I GNETs, minimally invasive single-incision laparoscopic antrectomy (SILA), is reported here for the first time.

Lymphocytes' and monocytes' counts were obtained from a differential complete blood count profile. Participants' DNA was extracted and genotyped for rs1800629 and rs1799724 in TNFα, and rs909253 in LTA. Controls had a significantly higher mean lymphocyte count (P = 0.018), while the mean monocyte count and serum TNFα levels did not differ between the two groups (P > 0.05). With respect to gene polymorphisms, the rs1800629 and rs1799724 variants showed significant association with migraine in all subjects, and in males and females when analyzed separately (P 0.05). In addition, a novel frame-shift mutation in the MLH1 gene (c.1621dupG, p.A541Gfs*16) was found in the genetic panel sequence. It may render declining of MLH1 protein and also associate with the patient's progressive clinical manifestations of multiple systems.     The aim of this study was to examine the phenomenon of existential anxiety when living with esophageal cancer in the context of receiving general palliative care in a hospital setting.

    This qualitative study is inspired by phenomenological and hermeneutical aspects of the philosophies of Ricoeur and Heidegger. Their existence was reduced to the present, with a break in temporal continuity. An anxious mood permeated their entire being-in-the-world in an unhomelike way. Despite this, patients initiated a restoration of home and meaning expressed as a soothing sense of nostalgia that served as an atmospheric, safe space allowing them to inhabit the borderline between past, present, and future.

Patients underwent PSMA PET/CT as well as portal venous phase abdominal SDCT for clinical cancer follow-up. Only scan pairs with a stable nodal status indicated by constant size as well as comparable prostate-specific antigen (PSA) levels were included. One hundred benign and 96 suspected metastatic lymph nodes were annotated and correlated between SDCT and PSMA PET/CT.     This institutional review board-approved retrospective study examined biopsy-proven schwannomas that underwent FDG PET/CT and/or MRI at our institution between January 1, 2002, and April 1, 2018. PET/CT features analyzed included SUVmax, metabolic ratios, volumetric metabolic measures, presence of calcification, and pattern of FDG activity. MRI features included T1/T2 signal, enhancement pattern, margins, perilesional edema, presence of muscular denervation, and size.

    Ninety-five biopsy-proven schwannomas were identified (40 with both PET and MRI, 35 with PET only, and 20 with MRI only), 46 females and 49 males, average age of 57.7 ± 15.3 years. The average largest dimension was 4.6 ± 2.7 cm, the average SUVmax was 5.4 ± 2.7, and lesion SUVmax/liver SUVmean was 2.2 ± 1.2. Eleven (15%) of 75 lesions had SUVmax greater than 8.1, 26/75 (35%) had SUVmax greater than 6.1, and 14/75 (19%) had lesion SUVmax/liver SUVmean greater than 3.

  • Ultraviolet radiation (UVR) is an established cause of non-melanoma skin cancer (NMSC)-basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The aim of this study was to estimate the current burden of ****and SCC associated with UVR and modifiable UVR behaviours (sunburn, sunbathing, and indoor tanning) in Canada in 2015.

    The current burden of ****and SCC associated with UVR was estimated by comparing 2015 incidence rates with rates of less exposed body sites (trunk and lower limbs) after adjusting for estimated surface areas. The burden associated with modifiable UVR behaviours was estimated by using prevalence estimates among Caucasians from the Second National Sun Survey, and relative risks that are generalizable to Canadians from conducting meta-analyses of relevant studies.

    We estimated that 80.5% of **** and 83.0% of SCCs were attributable to UVR. Adult sunburn was associated with relative risks of 1.85 (95% CI 1.15-3.00) for ****and 1.41 (95% CI 0.91-2.18) for SCC, while adult sunbathing was associated with relative risks of 1.82 (95% CI 1.52-2.17) for ****and 1.14 (95% CI 0.53-2.46) for SCC. We estimated that 18.6% of **** and 9.9% of SCCs were attributable to adult sunburn, while 28.1% of **** were attributable to adult sunbathing. We estimated that 46.2% of **** and 17.3% of SCCs were attributable to modifiable UVR behaviours combined.

    To investigate men's experiences of receiving external-beam radiotherapy (EBRT) with neoadjuvant Androgen Deprivation Therapy (ADT) for localized prostate cancer (LPCa) in the ProtecT trial.

    A longitudinal qualitative interview study was embedded in the ProtecT RCT. Sixteen men with clinically LPCa who underwent EBRT in ProtecT were purposively sampled to include a range of socio-demographic and clinical characteristics. They participated in serial in-depth qualitative interviews for up to 8 years post-treatment, exploring experiences of treatment and its side effects over time.

    Men experienced bowel, sexual, and urinary side effects, mostly in the short term but some persisted and were bothersome. Most men downplayed the impacts, voicing expectations of age-related decline, and normalizing these changes. There was some reticence to seek help, with men prioritizing their relationships and overall health and well-being over returning to pretreatment levels of function. Some unmet needs with regard to information about treatment schedules and side effects were reported, particularly among men with continuing functional symptoms.

    These findings reinforce the importance of providing universal clear, concise, and timely information and supportive resources in the short term, and more targeted and detailed information and care in the longer term to maintain and improve treatment experiences for men undergoing EBRT.

These findings reinforce the importance of providing universal clear, concise, and timely information and supportive resources in the short term, and more targeted and detailed information and care in the longer term to maintain and improve treatment experiences for men undergoing EBRT.Four types of mycelial extracts were derived from the airlift liquid fermentation (ALF) of Pleurotus flabellatus, namely exopolysaccharide (EX), endopolysaccharide (EN), hot water (WE), and hot alkali (AE) extracts. Such extracts were screened for their active components and biological potential. EN proved to be most effective in inhibition of lipid peroxidation (EC50 = 1.71 ± 0.02 mg/mL) and in Cupric ion reducing antioxidant capacity (CUPRAC) assay (EC50 = 2.91 ± 0.01 mg TE/g). AE exhibited most pronounced ability to chelate ferrous ions (EC50 = 4.96 ± 0.08 mg/mL) and to scavenge ABTS radicals (EC50 = 3.36 ± 0.03 mg TE/g). β-glucans and total phenols contributed most to the chelating ability and quenching of ABTS radicals. Inhibition of lipid peroxidation correlated best with total glucans, total proteins, and β-glucans. Total proteins contributed most to CUPRAC antioxidant capacity. Antifungal effect was determined against Candida albicans ATCC 10231 (MIC 0.019-0.625 mg/mL; MFC 0.039-2.5 mg/mL), and towards C.
